dc.description.abstract | ObjectivesNeurocysticercosis (NC), an infection of the central nervous system with Taenia solium metacestodes (TsM), invokes a formidable neurological disease. A bundle of antigens is applicable for serodiagnosis of active cases, while they demonstrate fairly low reactivity against sera of chronic NC. Identification of sensitive biomarkers for chronic NC is critical for appropriate management of patients. MethodsProteome analysis revealed several isoforms of 65- and 83-kDa TsM fasciclin-like proteins (TsMFas) to be highly reactive with sera of chronic NC. A cDNA encoding one of the 83-kDa TsMFas (TsMFas1) was isolated from a cDNA library. We expressed a recombinant protein (rTsMFas1) and evaluated its diagnostic potential employing sera from chronic NC (n=80), tissue-invasive cestodiases (n=169) and trematodiases (n=80) and those of normal controls (n=50). ResultsSecretory TsMFas1 was composed of 766 amino acid polypeptide and harboured fasciclin and fasciclin-superfamily domains. The protein was constitutively expressed in metacestode and adult stages, with preferential locality in the scolex. Bacterially expressed rTsMFas1 exhibited 78.8% sensitivity (63/80 cases) and 93% specificity (278/299 samples) in diagnosing chronic NC. Some cross-reactivity was observed with sera of cystic echinococcosis (10/56, 17.8%) and sparganosis (4/50, 8%). Positive and negative predictive values were 75% and 95.5%, respectively. ConclusionTsM fasciclin-like protein may be useful for differential diagnosis of chronic NC in clinical settings, especially where both NC and other infectious cerebral granulomatoses are prevalent. ObjectifsLa neurocysticercose (NC), une infection du systeme nerveux central par les metacestodes du Taenia solium (TsM), invoque une maladie neurologique redoutable. Une panoplie d'antigenes sont applicables pour le diagnostique serologique des cas actifs, ils demontrent cependant une reactivite assez faible avec les serums de NC chronique. L'identification de biomarqueurs sensibles pour la NC inactive est essentielle pour une prise en charge appropriee des patients. MethodesUne analyse proteomique a revele plusieurs isoformes de 65 et 83 kDa du TsM, semblables a la proteine fascicline (TsMFas) tres reactives avec des serums de NC inactive. Un ADNc codant pour l'une des 83 kDa de TsMFas (TsMFas1) a ete isole a partir d'une banque d'ADNc. Nous avons exprime une proteine recombinante (rTsMFas1) et evalue son potentiel diagnostic en utilisant des serums de NC inactive (n = 80), de cestodiases invasives de tissus (n = 169) et de trematodiases (n = 80), et ceux de controles sains (n = 50). ResultatsLe TsMFas1 secretoire etait compose d'un polypeptide de 766 acides amines et comportait des domaines de fascicline et de la super famille de fascicline. La proteine a ete exprimee de facon constitutive dans les stades metacestode et adulte, avec une localisation preferentielle dans le scolex. L'expression bacterienne de rTsMFas1 presentait une sensibilite de 78,8% (63/80 cas) et une specificite de 93% (278/299 echantillons) pour le diagnostic de NC chronique. Une certaine reactivite croisee a ete observee avec des serums d'echinococcose kystique (10/56; 17,8%) et de sparganose (4/50; 8%). Les valeurs predictives positives et negatives etaient de 75% et 95,5%, respectivement. ConclusionTsMFas pourrait etre utile pour le diagnostic differentiel de la NC inactive dans les milieux cliniques, en particulier lorsque a la fois la NC et d'autres granulomatoses cerebrales infectieuses sont prevalentes. | - |
