Introduction The aetiology and natural course of Dermatomyositis/polymyositis (DM/PM) patients with acute respiratory failure has not been well studied. Methods A total of 36 patients who were admitted to medical intensive care unit (ICU) between January, 2002 to August, 2018 in Hanyang Medical centre due to acute respiratory failure were retrospectively evaluated. Results Of the 36 patients, 19 had acute exacerbation of DM/PM-associated interstitial lung disease and 17 had pneumonia. The overall in-hospital mortality was 44.4% (16/36) including 9 (52.6%) in patients with AE of DM/PM-associated ILD and 7 (41.2%) in patients with pneumonia. There was no significant difference in the survival rate between the two groups (P = 0.492). Conclusion The treatment outcomes in DM/PM patients who developed respiratory failure was relatively poor. There was no significant difference in-hospital mortality according to aetiologies-
