dc.description.abstract | 배경: 직접 빌리루빈과 간접 빌리루빈이 함께 증가해 있는 많은 환자들의 황달의 원인을 구별하기 위하여 d/t ratio를 사용할 수 있다. 그러나 현재까지 d/t ratio의 참고범위가 일정한 값으로 확립되어 있지 않으며 임상적 유용성 자체를 의심받고 있다. 저자들은 d/t ratio의 임상적 유용성 유무 파악에 관련된 연구들을 진행하였다. 방법: 담즙정체질환, 용혈성 빈혈, 그리고 신생아 황달 환자들의 총 빌리루빈과 직접 빌리루빈 검사값 4,357쌍을 대상으로 연구를 진행하였다. 각 질환들을 대상으로 총 빌리루빈과 직접 빌리루빈, 그리고 총 빌리루빈과 d/t ratio 간에 각각 회귀분석을 시행하여 서로 비교하였다. 그리고 본 저자들이 세운 회귀 분석 이론 모델들과 실제 결과들을 비교분석하였다. 결과: 담즙정체질환에서 총 빌리루빈과 직접 빌리루빈 사이의 회귀방정식은 이론 모델과 같은 직선 상관관계를 나타냈다. 마찬가지로 총 빌리루빈과 d/t ratio 사이의 회귀방정식도 이론 모델과 같은 역 모형의 회귀곡선을 보였으며, 총 빌리루빈 값이 약 10 mg/dL보다 높아질수록 d/t ratio의 상승률은 점차 완만해져 0.7과 0.9 사이의 일정한 값으로 수렴하는 결과를 보였다. 결론: 총 빌리루빈 값이 10 mg/dL보다 높은 경우에는 d/t ratio가 0.7 이상이면 담즙정체질환으로 진단할 수 있을 것이나 총 빌리루빈 값이 10 mg/dL보다 낮은 경우에는 d/t ratio가 0.7보다 낮더라도 담즙정체질환을 배제할 수 없다. 그러므로 d/t ratio를 임상적으로 유용하게 사용하기 위해서는 총 빌리루빈 값을 반드시 같이 고려해야 할 것이다. Background: The direct/total (d/t) bilirubin ratio can be used to distinguish the causes of jaundice in many patients who have increased levels of direct and indirect bilirubin. However, the reference range of the d/t ratio has not been established, hindering its clinical usefulness. This study assessed the clinical usefulness of the d/t ratio. Methods: Paired total bilirubin and direct bilirubin tests (N=4,357) of cholestasis, hemolytic anemia, and neonatal jaundice were evaluated. Regression analyses were performed between total bilirubin and direct bilirubin, and between total bilirubin and the d/t ratio for each disease. Theoretical correlation models were established and used to compare the regression analyses data. Results: The theoretical model and regression equation between total bilirubin and direct bilirubin displayed linear correlations for all three cholestatic diseases. The model and regression equation between total bilirubin and the d/t ratio showed reciprocal curve correlations for the cholestatic diseases. When the total bilirubin concentration exceeded approximately 10 mg/dL, the rate of change of the d/t ratio decreased and converged to a constant value between 0.7 and 0.9. Conclusions: If the total bilirubin concentration exceeds 10 mg/dL, cholestatic diseases can be diagnosed if the d/t ratio is more than 0.7. However, if the total bilirubin concentration is lower than 10 mg/dL, cholestatic diseases should be considered even if the d/t ratio is lower than 0.7. Therefore, use of the d/t ratio with total bilirubin could prove to be valuable in clinical settings. | - |
