| dc.description.abstract | 방선균증은 주로 주타액선, 구강, 경부, 호흡기 및 소화관에서 그람양성의 혐기성세균인 Actinomycosis israelii, A bovis에 의해 발생하는 만성 감염성 질환이다. 구강과 소화관내에 정상균주로 존재하면서, 외상이나 염증으로 정상점막이 손상된 경우 주위조직으로 침범하며 농양이나 누공을 형성한다. 세균배양과 조직검사에서 sulfur granule 및 균이 증명되면 확진 할 수 있다. 상악동에서 발생한 경우는 매우 드물며, 이 경우 발치와 같은 치과적 처치 후 발생한 구강상악루가 원인일 수 있다. 치료는 항생제와 육아조직 및 농양에 대한 수술적 방법이 있으며 저자들은 발치 및 내시경수술로 치료하지 못했던 우측 상악동에 생긴 방선균증을 상악동 세척 및 경구 항생제로 후유증 없이 치유된 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. | - |
