Does Medical lemniscus terminate convey chronic pain due to CRPS to dorsal lateral thalamus?

Abstract

Introduction : CRPS 환자의 통증 조절을 위한 dorsal lateral thalamotomy가 효과적이었다. 그러나 문헌상에 위 시상핵 부위의 역할에 대한 기술이 없는 상태이다. 저자들은 diffusion tensor imaging(DTI) 을 이용한 in vivo fiber tracking(FT)을 이용하 여 과연 somatosensory pathway와 dorsal lateral nucleus가 만성 통증 전달에 어떤 관 계가 있는 지에 대하여 연구하였다. Method and material : 2003년 및 2004년에 좌안에 CRPS로 인한 불인통을 호소하여 dorsal lateral thalamotomy을 시행 한 환자 2명을 대상으로 하였다. 1.5T로 Philips MR과 SENSE head coil을 이용하여 600mm2/s의 b value로 15방향의 diffusion gradient를 주어 diffusion tensor영상을 획득하였다. PRIDE software(Philips)을 이용하여 FT를 시행하 였고, termination criteria는 FA<0.2, angle<0.70로 하였다. FT를 위한 ROI 위치는 ventral pons의 superior level, cerebral peduncle level에서 medical lemniscus 와 spinal lemniscus로 하여 2 ROI를 동시에 통과하는 tract을 3차원으로 구하였다. Result : 위의 방법으로 somatosensory pathway를 tracking한 결과 시술한 우측 dorsal lateral thalamotomy에서 tract이 끊긴 영상을 얻게 되었고 좌측의 시술안한 쪽의 tract 은 sensory cortex까지 tract이 연결된 영상을 얻게 되었다. Conclusion : 이상의 결과로 CRPS와 같은 만성 신경인성 통증들은 일반적인 nociception에 관여하는 tract를 통하여 VPL을 통하여 상부로 전달되는 것보다는 medial lemniscus를 통하여 dorsal lateral thalamus를 통하여 상부로 전달된다고 생각할 수 있겠다. 이와같은 설명을 뒷받침하기 위한 신경해부생기 연구가 필요할 것이라 생각된다.
