단분절 경추전방유합술 시 케이지와 플레이트 병용사용의 임상적 안전성과 유효성 분석

Abstract

연구목적 본 연구에서는 경추 퇴행성 추간판 질환 환자와 경추 골절 환자를 대상으로 전방 경추 추간판제거술 및 유합술(이하 경추전방유합술) 시 현존하는 치료법(자가골과 플레이트 병용사용, 케이지 단독사용, 일체형 케이지 사용)과 비교한 케이지와 플레이트 병용사용의 임상적 안전성과 유효성을 분석하기 위하여 신속 체계적 문헌고찰을 수행하였다.

연구방법 경추 퇴행성 추간판 질환 환자와 경추 골절 환자를 대상으로 경추전방유합술 시 [KQ1] 자가골과 플레이트 병용사용, [KQ2] 케이지 단독사용, [KQ3] 일체형 케이지 사용과 비교한 케이지와 플레이트 병용사용의 임상적 안전성과 유효성을 보고한 문헌을 평가하였다. 문헌 검색은 국외 3개 데이터베이스(Ovid-MEDLINE, Ovid-EMBASE, Cochrane Library) 및 국내 2개 데이터베이스(KoreaMed, KMbase)에 대한 검색과 수기 검색을 병행하였으며, 출판연도 및 언어에 제한을 두지 않았다. 동료 심사된 학술지에 게재된 한국어 또는 영어 원저 문헌으로, 무작위배정 비교임상연구(randomized controlled trial, RCT) 또는 비무작위 비교연구(non-randomized controlled study, NRS)를 선택하였다. 문헌 선택은 2명 이상의 연구자가 독립적으로 검토한 뒤, 의견 일치를 통하여 최종 문헌을 선정하였다. 최종 선택된 문헌들의 비뚤림 위험 평가는 RCT의 경우 Cochrane의 Risk of Bias (RoB)를 이용하였고, NRS의 경우 Risk of Bias for Nonrandomized Studies (RoBANS) 2.0 국문판을 사용하였다. 가능할 경우 임상 결과변수에 대하여 메타분석을 수행하였고, 양적 합성을 수행할 수 없을 경우에는 질적으로 기술하였다. 연구 수행 과정에서 임상 자문을 구하기 위하여 자문단을 구성하였으며, 자문단의 명단 및 활동 현황은 [부록 1]에 제시하였다.

연구결과 연구에 포함된 문헌은 31편(경추 퇴행성 추간판 질환 30편, 경추 골절 1편)이었다. 경추 퇴행성 추간판 질환에 대한 문헌을 핵심질문별로 구분시 [KQ1] 8편, [KQ2] 10편, [KQ3] 15편이었다([KQ1]과 [KQ2] 동시 보고 2편, [KQ2]와 [KQ3] 동시 보고 1편). 경추 골절에 대한 문헌 1편은 [KQ1]에 대한 문헌이었다. 연구결과 요약은 정량적으로 분석 가능한 주요 결과를 위주로 정리하였다. 유효성 주요 결과지표로는 NDI, 목 통증(VAS), 팔 통증(VAS), 삶의 질(SF-36), 유합률을 검토하였다. 안전성 주요 결과지표로는 연하 곤란, 공여부위 통증, 침하를 검토하였다.

1.1. 경추 퇴행성 추간판 질환 경추 퇴행성 추간판 질환 환자를 대상으로 단분절 경추전방유합술 시 케이지와 플레이트 병용사용과 자가골과 플레이트 병용사용의 안전성과 유효성을 비교한 문헌[KQ1]은 RCT는 없었고, NRS만 8편이 확인되었다. 삶의 질에 대해서는 NRS에서도 관련 연구결과가 없었다. 메타분석이 가능했던 유합률(NRS 6편), 목 통증(NRS 2편), 침하(NRS 2편)에서 모두 케이지와 플레이트 병용사용과 자가골과 플레이트 병용사용 간에 통계적으로 유의한 차이가 없는 것으로 나타났다. 나머지 결과지표의 경우 근거가 없거나 적은 수의 대상자에 대한 연구결과가 보고되었다. 경추 퇴행성 추간판 질환 환자를 대상으로 단분절 경추전방유합술 시 케이지와 플레이트 병용사용과 표준적인 케이지 단독사용의 안전성과 유효성을 비교한 문헌[KQ2]은 총 10편(RCT 2편, NRS 8편)이 확인되었다. NDI, 삶의 질, 연하 곤란, 공여부위 통증, 침하에 대해서는 RCT의 연구결과가 없었다. 삶의 질에 대해서는 NRS에서도 관련 연구결과가 없었다. 메타분석이 가능했던 결과의 경우, 플레이트를 병용사용 시 케이지 단독사용보다 침하(NRS 8편)는 감소하고 유합률(NRS 7편)은 높게 나타났다. 반면, NDI (NRS 2편), 목 통증(NRS 3편), 팔 통증(NRS 2편)은 플레이트 병용사용과 케이지 단독사용 간에 통계적으로 유의한 차이가 없었다. 나머지 결과지표의 경우 근거가 없거나 적은 수의 대상자에 대한 연구결과가 보고되었다. 경추 퇴행성 추간판 질환 환자를 대상으로 단분절 경추전방유합술 시 케이지와 플레이트 병용사용과 일체형 케이지 사용의 안전성과 유효성을 비교한 문헌[KQ3]은 총 15편(RCT 2편, NRS 13편)이 확인되었다. NDI, 삶의 질, 공여부위 통증에 대해서는 RCT의 연구결과가 없었다. 공여부위 통증에 대해서는 NRS에서도 관련 연구결과가 없었다. 메타분석이 가능했던 연하 곤란(NRS 8편)은 케이지와 플레이트 병용사용보다 일체형 케이지 사용에서 더 낮게 나타났다. 반면, 유합률(NRS 9편), NDI (NRS 4편), 목 통증(NRS 3편), 침하(NRS 2편)는 케이지와 플레이트 병용사용과 일체형 케이지 사용 간에 통계적으로 유의한 차이가 없었다. 나머지 결과지표의 경우 근거가 없거나 적은 수의 대상자에 대한 연구결과가 보고되었다.

1.2. 경추 골절 경추 골절 환자를 대상으로 단분절 경추전방유합술 시 케이지와 플레이트 병용사용과 자가골과 플레이트 병용사용의 안전성과 유효성을 비교한 문헌[KQ1]은 RCT는 없었고, NRS만 1편(Park 등, 2009)이 확인되었다. 대부분의 주요 결과지표(NDI, 목 통증, 팔 통증, 삶의 질, 연하 곤란, 공여부위 통증)에 대해서는 NRS에서도 관련 연구결과가 없었다. 선택된 NRS 1편에서 유합률은 수술 후 평균 12.6개월 시점에 두 군 모두 골유합이 확인되었고, 침하는 두 군 모두 발생하지 않았다고 보고하였다. 경추 골절 환자를 대상으로 단분절 경추전방유합술 시 케이지와 플레이트 병용사용과 표준적인 케이지 단독사용[KQ2]의 안전성과 유효성을 비교한 연구결과는 없었다. 경추 골절 환자를 대상으로 단분절 경추전방유합술 시 케이지와 플레이트 병용사용과 일체형 케이지 사용[KQ3]의 안전성과 유효성을 비교한 연구결과는 없었다.

결론 및 제언 본 연구에서는 현존하는 치료법(자가골과 플레이트 병용사용, 케이지 단독사용, 일체형 케이지 사용)과 비교한 단분절 경추전방유합술 시 케이지와 플레이트 병용사용의 임상적 안전성과 유효성을 분석하였다. 경추 퇴행성 추간판 질환 환자를 대상으로 케이지와 플레이트 병용사용을 자가골과 플레이트 병용사용과 비교[KQ1]한 RCT는 없었기 때문에 명확한 근거를 확인할 수 없었다. 다만, NRS의 결과를 메타분석 시 유합률, 목 통증, 침하에서 두 중재법간 유의한 차이가 없는 것으로 나타났다. 환자의 자가골 채취 회피 경향이나 자가골 채취에 따른 합병증으로 인하여 케이지의 사용이 요구된다는 의견이 있으므로 환자보고결과(patient- reported outcomes)의 분석이 중요하였으나, 선택 문헌에서는 삶의 질에 대하여 보고한 연구결과가 없어서 자가골 미채취에 따른 삶의 질 향상 여부를 판단할 수 없었다. 또한, 본 연구에서는 비교연구만을 선택하여 검토하였으나, 공여부 통증에 대한 정확한 평가를 위해서는 비(非)비교연구를 포함한 분석을 통하여 유병률을 보고하는 것이 필요할 것으로 사료된다. 경추 퇴행성 추간판 질환 환자를 대상으로 케이지 단독사용에 비해 케이지와 플레이트의 병용사용[KQ2]의 안전성과 유효성이 어떠한지를 분석하였다. RCT에서는 NDI, 삶의 질, 연하 곤란, 공여부위 통증, 침하에 대한 연구결과가 없었다. NRS의 결과를 메타분석 시 플레이트 병용사용이 케이지 단독사용보다 유합률의 증가와 침하의 감소가 나타났다. 그 외의 결과에서는 두 중재법 간에 유의한 차이가 없는 것으로 나타났다. 경추 퇴행성 추간판 질환 환자를 대상으로 케이지와 플레이트 병용사용과 일체형 케이지 사용의 비교[KQ3]에서는 NRS의 결과를 메타분석 시 연하 곤란은 일체형 케이지 사용이 더 적었고, 그 외 대다수의 지표(유합률, NDI, 목 통증, 침하)에 대한 결과는 두 중재법 간에 유의한 차이가 없었다. 추후 두 중재법이 안전성과 유효성 측면에서 대등하다는 것이 입증될 경우에는 비용효과성 측면에서의 고려도 필요할 것으로 여겨진다. 한편, 경추 골절 환자를 대상으로 단분절 경추전방유합술 시 케이지와 플레이트 병용사용을 현존하는 치료법(자가골과 플레이트 병용사용, 케이지 단독사용, 일체형 케이지 사용)과 비교한 안전성 및 유효성에 대하여 판단하기에는 아직 근거가 불충분하였다. 따라서, 경추 퇴행성 추간판 질환 환자를 대상으로 단분절 경추전방유합술 시 케이지와 플레이트 병용사용의 임상적 안전성과 유효성에 대한 명확한 결론을 내리기에는 질 높고 엄격하게 수행된 임상 연구가 부족하여 향후 동 시술의 임상적 안전성 및 유효성을 입증할만한 추가 연구가 더 필요하다. 또한, 경추 골절 환자를 대상으로 단분절 경추전방유합술 시 케이지와 플레이트 병용사용의 임상적 안전성과 유효성에 대해서도 아직 연구가 더 필요할 것으로 여겨진다. 본 연구는 시의적절성을 달성하기 위하여 신속 평가로 의뢰되어 수행되었기 때문에 전술한 여러 제한점이 있음을 감안하여 결과 해석에 주의를 요하며, 추후 완전한 체계적 문헌고찰에 의해 보완될 필요가 있다고 사료된다.

Clinical Safety and Effectiveness of the Combining Cages and Plates in Single-Level Anterior Cervical Discectomy and Fusion

Objective This rapid systematic review aimed to compare the clinical safety and effectiveness of using a cage and plate with existing treatment modalities (the combined use of autograft and plate, cage alone, or integrated cage) in anterior cervical discectomy and fusion (ACDF) for patients with cervical degenerative disc disease and cervical fractures.

Methods This systematic review included studies that reported data on the clinical safety and effectiveness of using a cage and plate in ACDF. The studies focused on patients with cervical degenerative disc disease and cervical fractures. The key question (KQ) was categorized based on the treatment modality used for comparison: [KQ1] autograft and plate, [KQ2] cage alone, and [KQ3] integrated cage. Effectiveness was assessed using the Neck Disability Index (NDI), neck pain (Visual Analog Scale [VAS]), arm pain (VAS), quality of life (SF-36), and fusion rate. Safety was assessed based on dysphagia, donor site pain, and subsidence.

Literature searches were conducted without restrictions on publication year or language. Three international databases (Ovid-MEDLINE, Ovid-EMBASE, and Cochrane Library) and two Korean databases (KoreaMed and KMbase) were used, along with manual searches. Original studies published in peer-reviewed journals in Korean or English, including randomized controlled trials (RCTs) or non-randomized controlled studies (NRSs), were selected. Two or more researchers independently conducted the study selection. The final selection was made through discussions to reach a consensus. The risk of bias was assessed using the Cochrane Risk of Bias (RoB) tool for RCTs and the Korean version of the Risk of Bias Assessment Tool for Nonrandomized Studies (RoBANS) 2.0 for NRSs. A meta-analysis was performed on clinical outcome variables where possible. If quantitative analysis was not feasible, the results were described qualitatively. An advisory panel was formed to obtain clinical recommendations for the study; the list of members and activities is presented in Appendix 1.

Results This study included 31 articles—30 on cervical degenerative disc disease and 1 on cervical fracture. Regarding the treatment modality used as a comparator, 8 articles were identified for [KQ1], 10 for [KQ2], and 15 for [KQ3]; 2 articles reported both [KQ1] and [KQ2] and 1 included both [KQ2] and [KQ3]. The article on cervical fracture was compared with [KQ1]. The research findings were summarized based on key outcomes that could be quantitatively analyzed. Cervical degenerative disc disease [KQ1] autograft and plate No RCTs and eight NRSs were identified for comparing the safety and effectiveness of the concomitant use of a cage and plate with [KQ1] in ACDF for patients with cervical degenerative disc disease. None of the eight NRSs reported data on quality of life. A meta-analysis for fusion rates (6 NRSs), neck pain (2 NRSs), and subsidence (2 NRSs) showed no statistically significant differences between the concomitant use of cage and plate and the use of autograft with plate. For other outcome measures, there was either no evidence or only limited patient samples. [KQ2] cage alone Ten studies (2 RCTs, 8 NRSs) were identified to compare the safety and effectiveness of the concomitant use of a cage and plate with [KQ2] in ACDF for patients with cervical degenerative disc disease. No RCTs reported data on NDI, quality of life, dysphagia, donor site pain, and subsidence. None of the eight NRSs reported data on quality of life. A meta-analysis revealed that using a plate with a cage was associated with lower subsidence (8 NRSs) and increased fusion rates (7 NRSs) than using a cage alone. However, the treatment groups did not significantly differ in terms of NDI (2 NRSs), neck pain (3 NRSs), and arm pain (2 NRSs). For other outcome measures, there was either no evidence or only limited patient samples. [KQ3] integrated cage A total of 15 studies (2 RCTs, 13 NRSs) were identified to compare the safety and effectiveness of the concomitant use of a cage and plate with [KQ3] in ACDF for patients with cervical degenerative disc disease. No RCTs reported data on NDI, quality of life, or donor site pain. None of the NRSs reported data on donor site pain. A meta-analysis (8 NRSs) showed that dysphagia was less prevalent in the integrated cage group than in the cage and plate group. However, the treatment groups did not significantly differ in terms of fusion rate (9 NRSs), NDI (4 NRSs), neck pain (3 NRSs), or subsidence (2 NRSs). For other outcome measures, there was either no evidence or only small patient samples. Cervical fracture For patients with cervical fractures, only one NRS (Park et al., 2009) reported relevant data on the safety and effectiveness of the concomitant use of a cage and plate compared with [KQ1] in ACDF. However, there were no study findings for most of the key outcome measures, including NDI, neck pain, arm pain, quality of life, dysphagia, and donor site pain. In this NRS, fusion was achieved at an average of 12.6 months after surgery in both groups, with no cases of subsidence in either group.

Conclusions and Recommendations This study analyzed the clinical safety and effectiveness of the concomitant use of a cage and plate in ACDF compared with existing treatment modalities (combined use of autograft and plate, cage alone, or integrated cage). There was no clear evidence regarding the clinical safety and effectiveness of using a cage and plate versus the concomitant use of autograft and plate in ACDF for patients with cervical degenerative disc disease, as no RCTs reported relevant data. However, a meta-analysis of NRS results showed no significant differences between the two interventions in terms of fusion rates, neck pain, and subsidence. Considering the tendency for patients to avoid autografts and the complications associated with autograft harvesting, analyzing patient-reported outcomes was important. However, as none of the selected studies reported data on quality of life, improvements in this outcome could not be determined. Additionally, although only comparative studies were reviewed, a broader analysis including non-comparative studies is necessary to accurately assess the prevalence of donor site pain. For the comparison with standard cage alone [KQ2], no RCT results were found for NDI, quality of life, dysphagia, donor site pain, or subsidence in patients with cervical degenerative disc disease. A meta-analysis of NRS results showed that using a plate with a cage increased fusion rates and reduced subsidence compared with using a cage alone. However, no significant differences were found in other outcomes between the two interventions. When comparing with the use of an integrated cage [KQ3], a meta-analysis of NRS results indicated that dysphagia was less common with the use of the integrated cage than with the concomitant use of a cage and plate in patients with cervical degenerative disc disease. However, no significant differences were found in most other outcomes (fusion rate, NDI, neck pain, and subsidence) between the two interventions. If these two interventions are proven equivalent in safety and effectiveness, future considerations should include cost-effectiveness. When considering ACDF in patients with cervical fractures, the evidence on the safety and effectiveness of using a cage and plate was inadequate compared with existing treatment modalities. Therefore, further rigorously designed, high-quality studies are needed to determine the clinical safety and effectiveness of the concomitant use of a cage and plate in single-level ACDF for patients with cervical degenerative disc disease and cervical fractures. This study was conducted as a rapid review to provide timely data, and the interpretation of results should consider the previously mentioned limitations. These limitations should be addressed with full systematic reviews in the future.
