New uritest set를 이용한 뇨검사 및 혈액검사의 안전성 및 유효성 평가

Abstract

연구배경 및 목적 본 연구는 선천성 대사이상증 환자를 조기발견하기 위해 시행하는 New-Uritest Set을 이용한 뇨검사 및 혈액검사에 대한 안전성 및 임상적 유효성을 평가하기 위하여 시행하였다.

연구 방법 본 연구는 New-Uritest Set을 이용한 뇨검사 및 혈액검사에 대한 안전성 및 임상적 유효성을 평가하기 위해 체계적 문헌고찰을 수행하였으나, 관련 문헌이 확인되지 않아 현황 조사를 추가로 시행하였다.

체계적 문헌고찰은 핵심질문을 작성하여 국외 3개 DB와 국내 5개 DB를 검색하였다. 앞서 기술한 바와 같이 국내외 총 8개 DB를 검색하였으나, 관련 문헌을 확인할 수 없었다. 이에 따라 관련 자료 수집을 위하여, 건강보험심사평가원, 식품의약품안전처, 대한진단검사의학회, 대한병원협회 등에 관련 자료 요청 공문을 발송하여, 동 기술의 현황을 파악하고자 하였다. 또한, 대한진단검사의학회, 대한소아과학회에 추천을 받은 위원들을 대상으로 동 기술의 임상적 자문과 함께 관련 기술의 현황을 파악하고자 하였다.

연구 결과 동 기술에 사용되는 의료기기의 식약처 허가사항을 파악하고, 관련 자료를 구득하고자 하였으나, 건강보험심사평가원, 식품의약품안전처, 대한진단검사의학회, 대한병원협회 등에서 관련 의료기기의 식약처 허가사항을 파악할 수 없다는 회신을 받았으며, 또한 문헌 및 검색 엔진을 통한 검색에서도 확인할 수 없었다. 또한 2016년 건강보험심사평가원에서 동 기술의 사용현황은 파악하였을 당시, 1개 의료기관에서 사용하고 있는 것으로 확인되었으나, 현재 재차 확인한 결과, 당시 사용하던 의료기관에서도 현재는 사용하지 않아 비급여 코드를 삭제한 것으로 파악되었다. 전문가 자문결과, 동 기술에 사용하는 의료기기가 9종의 시약이 키트화된 형태라는 정보에 비추어보았을 때, 선천성 대사 이상 질환의 선별검사 목적으로 사용되는 것으로 추정되며, 이미 40여종 이상의 질환을 조기에 선별할 수 있는 최신의 기술들이 도입되어 사용되고 있는 만큼 임상적 유용성이 떨어진다는 의견이었다.

결론 및 제언 현재 ‘New Uritest Kit를 이용한 뇨검사 및 혈액검사‘ 기술에 대한 안전성・유효성 등을 평가할 만한 문헌이 검색되지 않으며, 국내에서 사용되지 않는 기술로 확인하였다. 이에 대한진단검사의학회, 대한소아과학회에서 추천 받은 전문가 자문위원들 자문결과, 현재 해당 기술은 국내에서 사용되지 않는 것으로 확인되었으며, 국내 대부분의 기관에서 보다 광범위한 질병을 정확한 방법으로 선별할 수 있는 질량분석법 등으로 선천성 대사이상증 선별검사를 수행하고 있으므로, 해당 비급여 항목의 고시 삭제도 가능할 것으로 판단된다는 의견이었다.

Safety and effectiveness assessments of urinalysis using the New-Uritest Set and Blood Test

Background and Objective This study aimed to assess the safety and clinical effectiveness of Urinalysis using the New-Uritest Set and Blood Test for the early detection of inborn errors of metabolism.

Methods A systematic review was performed to assess the safety and clinical effectiveness of Urinalysis using the New-Uritest Set and Blood Test based on the following criteria:

● Subjects - Neonates for IEM screening

● Index test - Urinalysis using the New-Uritest Set and Blood Test

● Comparators - Immunoassay - Mass spectrometry (e.g., tandem mass spectrometry)

● Outcomes ▪ Safety - Test-related complications ▪ Effectiveness - Diagnostic accuracy - Correlation with comparator, etc.

● Time - No limit

● Study type - Randomized controlled trials - Diagnostic evaluation studies, etc.

Once the key question was established, a literature search was conducted in three international databases and five Korean databases. However, no relevant articles were found. Consequently, an official request for data was sent to the Health and Insurance Review and Assessment (HIRA), Ministry of Food and Drug Safety (MFDS), Korean Society for Laboratory Medicine, and Korean Hospital Association to understand the current status of this technology. Additionally, clinical consultations were sought from committee members recommended by the Korean Society for Laboratory Medicine and Korean Pediatric Society.

Results While efforts were made to obtain relevant data and check the approval status for the medical device used in this technology, responses from HIRA, MFDS, the Korean Society for Laboratory Medicine, and Korean Hospital Association indicated that they could not find the MFDS approval status for the device. Additionally, the literature review and further searches using search engines did not yield any relevant information. In 2016, HIRA assessed the use of this technology and found that it was being used by one medical facility at the time. However, upon rechecking, the facility in question no longer used the technology, and the non-reimbursable code has been deleted. Considering that the medical device used in this technology is a kit comprising nine types of reagents, the consulted experts presumed that the kit is used for IEM screening. Given that more than 40 advanced technologies capable of early screening for a wide range of conditions are already in use, the clinical utility of this technology was deemed to be low.

Conclusion and Recommendations Currently, no evidence evaluating the safety and effectiveness of urinalysis using the New-Uritest Set and Blood Test has been reported. Consultations with expert advisors recommended by the Korean Society for Laboratory Medicine and the Korean Pediatric Society confirmed that this technology is not currently in use in Korea. Most institutions in Korea are performing IEM screening using more advanced and accurate methods, such as mass spectrometry, which can screen for a broader range of diseases. Therefore, the committee determined that this item could be removed from the non-reimbursable category.
