경동공 온열치료의 안전성 및 유효성 평가

Abstract

연구배경 및 목적 경동공 온열치료는 안내종양과 혈관증식, 망막하 장액을 유발하는 안질환에서 병변 제거 목적으로 저에너지 다이오드 레이저를 종양, 혈관, 장액 등에 조사하는 시술이다. 본 연구는 경동공 온열치료의 안전성 및 임상적 유효성을 평가하여 합리적인 급여기준 설정에 필요한 의학적 근거자료를 마련하고자 한다.

연구 방법 경동공 온열치료에 대한 안전성 및 유효성은 행위정의를 마련하고 체계적 문헌고찰을 이용하여 문헌적 근거를 검토하였다. 체계적 문헌고찰에 사용된 PICO-TS는 아래와 같다.

체계적 문헌고찰은 핵심질문을 작성하여 국외 3개 DB와 국내 4개 DB를 검색하였다. 문헌선정은 미리 작성된 선정 및 배제기준에 따라 검색된 모든 문헌들에 대해 두 명 이상의 검토자가 독립적으로 시행하였고, 모두 2차례의 문헌선택과정을 거쳤다. 문헌의 비뚤림 위험 평가는 두 명 이상의 검토자가 독립적으로 시행하여 의견합의를 했으며, 무작위 임상시험(RCT)과 비무작위 임상시험(NRS)은 Cochrane의 Risk of Bias (Higgins 등, 2011), 코호트연구, 환자-대조군 연구는 ROBANS ver 2.0로 평가하였다. 자료 추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명 이상의 검토자가 각각 독립적으로 수행하였으며, 의견 불일치가 있을 경우 제 3자와 함께 논의하여 합의하였다. 자료분석은 양적 분석(quantitative analysis)이 가능할 경우 양적 분석(메타분석)을 수행하며 불가능할 경우 질적 검토(qualitative review) 방법을 적용하였다.

연구 결과 경동공 온열치료의 임상적 안전성 및 유효성을 평가하기 위하여 최종 선정된 문헌은 총 17편이었고, 평가결과는 사용대상에 따라 안내종양과 혈관증식, 망막하 장액을 유발하는 안질환으로 구분하였다.

Ⅰ. 안내종양

안내종양 문헌은 총 6편으로 맥락막 흑색종(코호트연구 5편), 포도막 흑색종(무작위 임상시험 1편)을 대상으로 하였다. 경동공 온열치료는 근접방사선치료(3편)와 방사선치료(2편)를 병행하거나 단독시술(1편)로 수행하였고, 근접방사선치료, 방사선치료와 임상적 안전성 및 유효성을 비교하였다. 안내종양을 대상으로 한 문헌의 비뚤림 위험은 코호트연구(5편)는 낮고, 무작위 임상시험(1편)은 높은 것으로 판단하였다. 안전성 결과(3편)는 치료관련 합병증 발생률이 두 군간 유의한 차이가 없었고, 신생혈관 녹내장, 망막제거 등이 발생하였다. 유효성 결과는 종양제거, 종양재발, 생존율, 종양전이, 추가치료, 사망률, 동반증상 해결을 보고하였다. 안내종양의 유효성 결과는 문헌마다 지표정의와 제시한 결과값이 상이하여 추가치료를 제외한 나머지 의료결과에서 양적분석을 시행하지 못하였다. 전반적으로 종양제거(코호트연구 2편)와 생존율(코호트연구 2편)에서 두 군간 차이는 없지만 경동공 온열치료를 시행한 군에서 대조군보다 높게 보고되었다. 치료실패로 정의할 수 있는 종양재발(코호트연구 3편), 종양전이(코호트연구 5편), 추가치료(코호트연구 3편, 무작위 임상시험 1편), 사망률(코호트연구 2편)의 경우 두 군간 유의한 차이를 보이지 않았다. 다만 1편(Marinkovic 등 2016)의 경우, 경동공 온열치료를 시행한 군에서 종양전이, 추가치료 발생률, 사망률이 높게 보고되고 있으나, 대상환자의 종양크기, 병기에서 유의한 차이를 보이므로 해석에 주의가 필요하다. 동반증상 해결은 총 4편(코호트연구)에서 시력변화 정도가 두 군간 유의하지 않았다.

Ⅱ. 혈관증식 유발 안질환

혈관증식을 유발하는 안질환을 대상으로 한 문헌은 연령관련 황반변성(무작위 임상시험 5편, 비무작위 임상시험 1편, 코호트연구 2편)과 결절성 맥락막 혈관병증(코호트연구 1편)을 대상으로 하였다. 경동공 온열치료는 항체치료와 병행하거나(1편) 단독시술(8편)로 수행하였고, 항체치료(2편), 광역동치료(6편), 무치료(2편)와 임상적 안전성 및 유효성을 비교하였다. 비뚤림 위험은 무작위 임상시험(5편), 비무작위 임상시험(1편)은 높고, 코호트연구(3편)는 낮다고 판단하였다. 안전성 결과(4편)는 치료관련 합병증 발생률이 두 군간 유사하거나 모든 군에서 발생하기 않았고, 사망 또는 혈전과 같은 중증 합병증은 발생하지 않았다. 유효성 결과는 시술성공으로 혈관폐쇄(3편)와 시력변화(8편)를 보고하였고, 혈관폐쇄와 시력변화 모두 두 군간 유의한 차이를 보이지 않았다.

Ⅲ. 망막하 장액 유발 안질환

망막하 장액을 유발하는 안질환을 대상으로 한 문헌은 만성중심장액맥락망막병증(코호트연구 2편)을 대상으로 경동공 온열치료 단독시술과 광역동치료(1편), 무치료(1편)의 임상적 안전성 및 유효성을 비교하였다. 코호트연구(2편)의 비뚤림 위험은 낮은 것으로 판단하였다. 안전성 결과(1편)는 치료관련 합병증이 두 군 모두 발생하지 않았다. 유효성 결과는 시술성공으로 장액제거(2편)와 시력변화(2편)를 보고하였고, 두 지표에서 모두 두 군간 유의한 차이를 보이지 않았다.

결론 및 제언 경동공 온열치료의 임상적 안전성과 유효성은 안구종양에서 방사선치료(근접방사선치료 포함)와, 혈관증식을 유발하는 안질환에서 항체치료, 광역동치료, 무치료와, 망막하 질환에서 광역동치료, 무치료와 비교하였다. 경동공 온열치료의 안전성은 비교시술과 유사한 수준으로 혈전과 같은 중증 합병증이 발생하지 않아 안전성에는 문제가 없고, 유효성 측면에서도 열등하지 않다고 판단하였다. 동 기술은 최근에 널리 사용되는 기술은 아니고 특정 의료기관에서 주로 안내종양을 대상으로 사용되고 있으나 안구적출술을 통해 비가역적으로 안구를 상실하기 전에 시도하는 보조치료 중 하나로서 의미가 있다는 전문가 의견이었다. 따라서, 본 연구의 안전성 및 유효성에 대한 문헌적 근거와 임상적 의미 등을 종합하여 적절한 정책적 결정이 필요할 것으로 판단된다.

Safety and Effectiveness of Transpupillary Thermotherapy

Background and Objective Transpupillary thermotherapy (TTT) is used to remove lesions with the use of low-energy diode lasers on intraocular tumors or the vessels and serous fluid in patients with intraocular tumors, diseases that cause vasoproliferation, and diseases that cause the accumulation of subretinal fluids. The objective of this study was to assess the safety and clinical effectiveness of TTT to provide the medical evidence necessary for establishing reasonable reimbursement criteria.

Method First, TTT was defined, and the safety and effectiveness of TTT were assessed through a systematic review. The following PICO-TS framework was used.

Literature searches were performed using three international and four Korean databases based on the key questions. Literature selection was conducted independently by two or more reviewers using the predefined inclusion and exclusion criteria. Two rounds of literature selection were performed. Risk of bias was assessed independently by two or more reviewers using the Cochrane Risk of Bias tool for RCTs and NRS (Higgins et al., 2011) and ROBANS ver 2.0 for cohort and case-control studies. Data extraction was performed independently using a predefined data extraction form and disagreements were resolved through discussion with a third reviewer. Data were analyzed through quantitative analysis (meta-analysis) where possible and qualitative review otherwise.

Results In total, 17 studies were selected for the assessment of the clinical safety and effectiveness of TTT, categorized by the specific eye conditions treated (intraocular tumors, diseases that cause vasoproliferation, and diseases that cause the accumulation of subretinal fluids)

Ⅰ. Intraocular tumors Six studies reported data on intraocular tumors, namely choroidal melanoma (five cohort studies) and uveal melanoma (one RCT). TTT was performed alone (one study) or in combination with brachytherapy (three studies) and radiation therapy (two studies). The clinical safety and effectiveness of TTT were compared with those of brachytherapy and radiotherapy. The risk of bias was low for cohort studies and high for the RCT.

Regarding safety (three studies), there were no significant differences in the incidence of treatment-related complications, such as neovascular glaucoma and retinal detachment, between TTT and control groups.

Effectiveness was assessed based on tumor removal, tumor recurrence, survival rate, metastasis, additional treatment, mortality rate, and resolution of comorbid symptoms. Owing to inconsistencies in outcome definitions and reported values across the included studies, quantitative analysis was not performed on the medical outcomes except for additional treatments. Overall, there was no significant difference between groups in tumor removal (two cohort studies) and survival (two cohort studies), with slightly higher rates in the TTT group than in the control group. There were no significant differences in the outcomes that indicate treatment failure, such as recurrence (three cohort studies), metastasis (five cohort studies), additional treatments (three cohort studies, one RCT), and mortality (two cohort studies) between groups. One study (Marinkovic et al., 2016) reported higher rates of metastasis, additional treatments, and mortality in the TTT group than in the control group, but the tumor size and stage of the study population significantly differed, calling for the cautious interpretation of these results. There were no significant differences in symptom resolution (vision changes) in four studies (cohort studies).

Ⅱ. Eye diseases that cause vasoproliferation Nine studies reported data on eye diseases that cause vasoproliferation, namely age-related macular degeneration (five RCTs, one non-RCT, two cohort studies) and nodular choroidal vasculopathy (one cohort study). TTT was performed alone (eight studies) or in combination with antibody therapy (one study) and compared with antibody therapy (two studies), photodynamic therapy (six studies), and no treatment (two studies). Risk of bias was high for RCTs and NRSs and low for cohort studies.

Regarding safety (four studies), the incidence of treatment-related complications was either similar between the groups or zero. Serious complications, such as death or thrombosis, did not occur. Regarding effectiveness, vascular closure (three studies) and vision changes (eight studies) were reported as a result of a successful procedure. The rate of vascular closure and vision changes did not significantly differ between the groups.

Ⅲ. Eye diseases that cause subretinal fluid accumulation Two cohort studies reported data on diseases that cause subretinal fluid accumulation, namely chronic central serous chorioretinopathy (two cohort studies). The clinical safety and effectiveness of TTT performed alone were compared with those of photodynamic therapy (one study) and no treatment (one study). Risk of bias was low for cohort studies (two studies).

Regarding safety (one study), there were no treatment-related complications in both groups. Regarding effectiveness, fluid removal (two studies) and vision changes (two studies) were reported as a result of a successful procedure. The rate of these outcomes did not significantly differ between the groups.

Conclusion and Recommendation The clinical safety and effectiveness of TTT were compared with those of radiation therapy (including brachytherapy) for intraocular tumors, antibody therapy and photodynamic therapy for vasoproliferative eye diseases, and photodynamic therapy and no treatment for diseases that cause subretinal fluid accumulation.

The safety of TTT was comparable to that of the control treatments, with no severe complications such as thrombosis observed. In terms of effectiveness, TTT was not inferior to the controls.

Although TTT is not widely used and often only in specific healthcare facilities for intraocular tumors, experts noted its value as an adjunctive treatment to prevent irreversible eye loss through enucleation.

Therefore, appropriate policy decisions should be made in consideration of the safety and effectiveness of TTT based on the current literature and its clinical significance.
