무탐침 정위기법의 안전성 및 유효성 평가(비골, 상악골 등 골절수술)

Abstract

평가배경 무탐침 정위기법은 해부학적 주요지표를 확인하기 어려운 환자에게 시술부위로의 정확한 유도와 병소부위의 정밀한 위치 측정을 가능하게 하는 행위이다. 국내에서는 뇌수술과 이비인후과 등의 영역에서 선택급여로 본인부담률 각각 50%, 80%가 적용되어 사용되고 있다. 동 항목에 대한 적합성 평가(3년 주기, 2018년 7월)를 위해 청구 현황 모니터링을 수행한 결과, ‘안전성·유효성이 있는 의료기술(고시)’ 범위 외에서도 무탐침 정위기법의 사용이 확인됨에 따라 급여 적용 범위 외에서의 무탐침 정위기법 사용에 대한 전문적이고 심층적인 안전성 및 유효성을 확인이 필요하게 되었다. 이에 따라 건강보험심사평가원은 한국보건의료연구원에 해당 평가를 의뢰하였으며 본 연구는 비골, 상악골 등 골절 부위 수술에서 사용되는 무탐침 정위기법의 안전성 및 유효성을 평가, 합리적인 급여기준 설정에 필요한 의학적 근거자료를 마련하고자 하였다.

위원회 운영 총 7인으로 구성된 소위원회는 2019년 5월 23일까지 약 5개월간 총 4회의 소위원회 운영을 통해 동 기술의 안전성 및 유효성을 평가하였다. 2019년 연구기획관리위원회(2019.6.14.)에서는 무탐침 정위기법의 안전성 및 유효성 평가 결과를 최종 심의하였다.

평가 목적 및 방법 본 연구에서는 비골, 상악골 골절 영역에서 무탐침 정위기법의 안전성 및 유효성을 평가하여 합리적인 급여기준 설정에 필요한 근거자료를 생성하고자 한다. 구체적인 연구목적 및 핵심질문은 다음과 같다. 1) 비골, 상악골, 관골, 하악골, 비-안와-사골 골절 및 기타 다발 골절에서 무탐침 정위기법의 사용은 (무탐침 정위기법 미사용보다) 임상적으로 안전한가? 2) 비골, 상악골, 관골, 하악골, 비-안와-사골 골절 및 기타 다발 골절에서 무탐침 정위기법의 사용은 (무탐침 정위기법 미사용보다) 임상적으로 효과적인가?

본 연구에서는 비골, 상악골 등의 골절 수술과 관련한 무탐침 정위기법의 안전성 및 유효성을 평가하기 위하여 체계적 문헌고찰을 수행하였으며 PICO-TS는 아래와 같다.

문헌검색은 5개의 국내 데이터베이스(KoreaMed, KMBASE, KISS, RISS, KISTI)와 3개의 국외 데이터베이스(Ovid-Medline, Ovid-EMBASE, Cochrane CENTRAL)를 이용하였으며 검색어는 Ovid-Medline에서 사용되는 검색어를 기본으로 각 자료원의 특성에 맞게 수정하여 적용하였다. 기기의 도입 시기 등을 고려하여 검색기간은 2000년 이후로 제한하였으며 골절의 특성상 영상유도는 모두 CT(computed tomography)로 이루어진다는 점을 고려하여 MRI(magnetic resonance imaging)는 검색어에 포함하지 않았다. 비골, 상악골, 관골, 하악골, 비-안와-사골 골절 및 기타 다발골절 환자를 대상으로 무탐침 정위기법을 사용한 연구로 사전에 선정한 결과변수가 하나 이상 보고된 연구를 선택하였으며 원저가 아니거나 동료심사를 거쳐 학술지에 게재된 연구가 아니거나 한국어 및 영어로 출판되지 않은 문헌, 중복 출판 문헌은 제외하였다. 문헌선택은 두 명의 검토자가 독립적으로 수행하였으며 의견 불일치가 있는 경우 검토자 간의 논의를 통해 합의를 이루도록 하였다. 문헌의 질평가는 무작위배정임상 연구의 경우 Cochrane의 Risk of Bias(RoB)를, 비무작위배정임상 연구(non-randomized studies)는 Risk of Bias for Nonrandomized Sutdies(RoBANS) version 2.0을 사용하였다. 자료추출은 사전에 정의한 지표를 토대로 진행하였으며 안전성은 수술 중·후 합병증으로, 유효성은 수술 관련 요인(위치확인 정확성, 수술 정확성, 수술성공률, 대칭성), 평균 수술 시간, 재원일수, 환자 관련 요인(증상호전, 환자 만족도)을 살펴보았다. 결과에 대한 양적 분석(quantitative analysis)이 가능한 경우 메타분석을 수행할 계획이었으나 도출된 결과가 양적합성이 가능하지 않아 질적 검토(qualitative review) 방법을 적용하여 결과를 정리하였다.

평가 결과 1. 안전성 무탐침 정위기법을 이용한 비골, 상악골 등 골절수술의 안전성은 수술 중·후 합병증의 발생률을 주요 지표로 설정하고 무탐침 정위기법의 이용과 관련하여 발생 할 수 있는 안전성 결과를 평가하였다. 최종 선정문헌 26편 중 동 기술의 안전성에 대해 보고된 13편의 연구를 통해 평가하였으며 결과는 다음과 같았다. 무탐침 정위기법을 이용한 비골 골절 수술에서는 안전성 결과를 보고하지 않았다. 상악골 골절 환자를 대상으로 한 무탐침 정위기법의 안전성은 1개의 문헌에서 보고되었으며 무탐침 정위기법과 직접적인 관련성이 있는 안전성은 보고되지 않았으나 뼈의 부정유합, 판의 노출, 두개강내 농양, 소구증 발생을 보고하였다. 무탐침 정위기법을 이용한 관골 골절 수술의 안전성을 보고한 문헌은 9편이었다. 무작위배정임상시험 2편에서는 무탐침 정위기법과 직접적으로 관련이 있는 합병증이 없었다고 보고하였다. 무작위배정임상시험 1편과 코호트 연구 2편에서 무탐침 정위기법과 직접적으로는 관련이 없는 합병증을 보고하였으나 두 군 간 차이는 유의하지 않았다. 하악골 골절 환자를 대상으로 한 무작위배정임상연구 1편에서는 무탐침 정위기법과 직접적인 관련이 있는 합병증 및 수술 중/후 합병증에 있어서 무탐침 정위기법의 사용유무에 따른 차이가 없었다. 비-안와-사골 골절에서의 안전성은 2편의 증례연구 중 1편에서 보고되었으나 무탐침 정위기법과 직접적인 관련성이 있는 안전성은 아니었다. 기타 다발골절에서 관골안와상악골 골절 환자를 대상으로 무탐침 정위기법을 사용하였을 때 모두 심각한 이상반응이 발생하지 않았다고 보고하였다.

2. 유효성 무탐침 정위기법을 이용한 비골, 상악골 등 골절수술의 유효성은 수술 정확성, 위치확인의 정확성, 수술성공률, 대칭성, 재수술률, 침습정도를 포함한 수술 관련 요인, 평균 수술 시간, 재원일수, 그리고 증상호전, 삶의 질, 환자 만족도를 포함하는 환자 관련 요인을 주요 지표로 설정하여 무탐침 정위기법의 유효성을 평가하였다. 문헌 검토 결과 수술 관련 요인 중 재수술률 및 침습정도를 보고한 연구는 없어 이들 지표는 제외되었다. 최종 선정문헌 26편 모두 한 개 이상의 유효성 관련 지표를 보고하였으며 그 결과는 다음과 같았다. 비골 골절에서 무탐침 정위기법을 사용한 연구는 환자 5명을 대상으로 한 증례연구 1편이 있었다. 이 연구에서는 비골 골절 환자에게 무탐침 정위기법을 사용하였을 때 수술은 성공적이었으며 평균 등록시간은 4분이었다고 보고하였다. 상악골 골절 환자를 대상으로 무탐침 정위기법을 수행한 문헌은 총 5편(코호트 연구 1편, 증례연구 2편, 증례보고 1편)이 있었다. 수술 관련 요인 중 무탐침 정위기법의 등록과 관련한 위치확인의 정확성의 경우, 1편의 증례연구에서 등록 불일치 정도가 1 mm 이하임을 보고하였다. 수술 정확성을 보고한 대규모 후향적 코호트 연구에서는 무탐침 정위기법을 사용하는 것이 그렇지 않은 경우보다 수술 정확성이 유의하게 향상됨을 보고하였으며 3편의 비비교연구에서도 무탐침 정위기법을 사용하였을 때 수술 전후의 편차가 1 mm 내였다고 보고하였다. 환자 관련 요인 중 증상호전은 코호트 연구 1편과 증례연구 2편에서 확인할 수 있었다. 코호트 연구에서는 대조군 대비 무탐침군에서 교합회복률(good recovery)이 더 높았다고 보고하였으며 증례연구에서는 영양공급 튜브를 제거하거나 미용적, 기능적 측면에서 유의한 개선을 보이는 등의 증상호전이 있었다고 밝혔다. 관골 골절 환자를 대상으로 무탐침 정위기법의 유효성을 평가한 연구는 무작위배정임상 연구 2편, 코호트 연구 2편, 증례연구 8편, 증례보고 1편으로 총 13편이 있었다. 수술 관련 요인 중 위치확인의 정확성은 증례연구 2편에서 보고하였으며 모두 2 mm 미만이었다. 수술 정확성은 비교연구 3편(무작위배정임상 연구 2편, 코호트 연구 1편), 비비교연구 3편(증례연구 2편, 증례보고 1편)에서 보고하였다. 비교연구에서는 대체적으로 무탐침 정위기법을 사용하는 것이 그렇지 않은 경우보다 수술 정확성이 유의하게 향상됨을 보고하였으며 비비교연구에서도 수술 전후의 오차가 2 mm 미만으로 수술 정확성이 좋았음을 보고하였다. 수술성공률은 2편의 증례연구에서 모두 정확한 정복을 확인하였다. 대칭성은 비교연구 2편(무작위배정임상 연구 1편, 코호트 연구 1편)과 증례연구 4편에서 보고하였다. 비교연구의 경우, 각 연구에서 사전에 설정한 여러 개의 지표를 토대로 대칭성을 확인하였는데 대부분의 지표에 대하여 무탐침 정위기법을 사용하였을 때가 그렇지 않을 때보다 수술 후의 대칭성이 우수하였다. 증례 연구에서도 이와 유사하게 무탐침 정위기법을 사용하였을 때 수술 후 대칭성이 확인되었으며 이러한 결과는 문헌 출판 시기에 영향이 없었다. 평균 수술 시간은 무작위임상 연구 및 코호트 연구 각 1편에서 확인하였는데, 모두 무탐침 정위기법 사용 유무에 따른 차이가 없었다고 보고하였다. 코호트 연구 1편에서 보고한 재원일수 역시 무탐침 정위기법 사용에 영향을 받지 않았다. 환자 관련 요인 중 증상호전은 4편의 증례연구에서 보고되었으며 ZMC, 관골궁, 관골궁의 시상봉합 등 골절 부위에는 조금씩 차이가 있었으나 모두 증상이 호전되었음을 보고하였고 출판 시기는 증상호전에 영향을 주지 않았다. 환자 만족도의 경우, 이를 보고한 2편의 비교연구(무작위배정임상 연구 및 코호트 연구 각 1편)에서 모두 무탐침 정위기법 사용에 따른 차이는 확인할 수 없었다. 하악골 골절에서 무탐침 정위기법의 유효성은 1편의 무작위배정임상 연구를 근거로 평가하였다. 수술 관련 요인으로 수술 정확성 및 수술성공률을 보고하였으며 수술 정확성의 경우, 무탐침 정위기법을 사용하는 것이 그렇지 않은 경우보다 유의한 향상을 나타내었다. 수술성공률은 해부학적 정복률로 확인하였으며 통계적 유의성은 보고하지 않아 정확한 비교는 어려웠으나 무탐침 정위기법을 사용하였을 때가 그렇지 않은 경우보다 높은 경향성이 있었다. 평균 수술 시간은 무탐침 정위기법 사용 유무에 따른 차이가 없었다. 비-안와-사골 골절에서 무탐침 정위기법의 유효성은 증례연구 3편에서 보고되었다. 1편의 증례연구에서 수술 관련 요인 중 수술 정확성 및 위치확인 정확성을 보고하였으며 두 지표 모두 1 mm 이하이었다. 평균 수술 시간을 보고한 1편의 증례연구에서는 무탐침 정위기법의 사용이 수술 시간을 감소시키지는 않을 것으로 판단하였다. 환자 관련 요인 중 증상호전을 보고한 연구는 2편의 증례연구이었으며 모두 증상이 개선되었다고 밝혔다. 기타 다발골절에서는 안와관골 골절 및 관골안와상악골 골절을 포함하였다. 안와관골 골절은 1편의 코호트 연구와 3편의 증례연구를 근거로 해당 골절 부위에서의 무탐침 정위기법의 유효성을 평가하였다. 수술 관련 요인 중 위치확인 정확성 및 수술 정확성은 각각 증례연구 1편과 증례연구 2편에서 보고하였는데 모두 2 mm 미만으로 무탐침 정위기법의 정확성을 확인할 수 있었다. 수술성공률은 코호트 연구 및 증례연구 각 1편에서 확인하였다. 코호트 연구에서는 완벽한 관골 정복을 수술성공률 지표로 설정하였으며 무탐침 정위기법을 사용한 경우가 그렇지 않은 경우보다 수술성공률이 더 높은 경향이 있었으나 통계적 유의성은 보고하지 않았다. 증례연구의 경우, 무탐침 정위기법을 사용한 수술의 교정결과가 좋다고 보고하였다. 환자 관련 요인 중 증상호전은 1편의 증례연구에서 보고하였으며 안면형태의 개선이 있었다고 밝혔다. 관골안와상악골 골절에서의 무탐침 정위기법 유효성은 비비교연구 2편(증례연구 및 증례보고 각 1편)에서 확인하였다. 관골안와상악골 골절에서 무탐침 정위기법을 사용하였을 때 위치확인 정확성 및 수술정확성은 모두 2 mm 미만이었다.

결론 본 연구에서는 현재까지의 문헌을 토대로 체계적 문헌고찰을 수행하여 비골, 상악골 등의 골절 수술에서 무탐침 정위기법 이용의 안전성과 유효성 결과를 평가하였으며 이를 근거로 소위원회에서는 다음과 같이 제언하였다. 무탐침 정위기법은 비골, 상악골, 관골, 비-안와-사골 골절 및 기타 다발골절(관골안와상악골 골절, 안와관골 골절)에서 사용되고 있었다. 안전성을 보고한 13편의 문헌 중 무탐침 정위기법과 직접적으로 관련된 합병증을 보고한 문헌은 1편이었으며 무탐침군과 대조군 간의 유의한 차이는 없었다. 무탐침 정위기법과 직접적인 관련성이 없는 합병증은 발생하지 않았거나 두 군 간 차이가 없었다. 이와 같이 무탐침 정위기법과 직접적인 관련성이 있는 합병증 및 수술 전반의 합병증은 무탐침 정위기법 사용 여부에 따른 차이가 없었기 때문에 소위원회에서는 동 기술에 대하여 안전성이 있음을 확인하였다. 무탐침 정위기법의 유효성은 골절 부위에 따라 근거의 질 및 양에 차이가 있었다. 관골 골절에서 무탐침 정위기법은 위치확인 정확성 및 수술 정확성을 향상시킬 수 있는 유효한 의료기술로 판단되었으며 상악골 골절에서의 무탐침 정위기법의 이용은 관골 골절보다는 그 근거의 수는 적었으나 대규모 후향적 연구에서 상악골 골절의 주요 유효성 지표인 교합회복이 확인되었기 때문에 수술 관련 지표를 개선시킬 가능성이 있는 유효성이 있는 것으로 판단하였다. 그 외 비골, 하악골, 비-안와-사골 골절, 기타 다발골절은 문헌의 수가 적고 문헌의 질이 낮아 유효성을 확인하기 어렵다고 소위원회는 판단하였다.

Safety and Effectiveness of the Navigation System for Fracture Surgeries, Including Nasal and Maxillary Bone Fractures

Background The Navigation System is employed to precisely guide surgeons in locating lesion sites during surgery, particularly in patients where identifying key anatomical landmarks is challenging. In Korea, this technology is selectively reimbursed, with a copayment rate of 50% for brain surgeries and 80% for ear, nose, and throat procedures. In July 2018, it was confirmed that the use of the Navigation System had extended beyond the scope of “safe and effective health technologies” during eligibility assessment monitoring, which occurs every 3 years. As a result, an in-depth safety and efficacy assessment of the Navigation System, when used outside the scope of insurance coverage, was deemed necessary. Consequently, the Health Insurance Review & Assessment (HIRA) Service requested that the National Evidence-based Healthcare Collaborating Agency (NECA) conduct this assessment. This study aimed to evaluate the safety and effectiveness of the Navigation System in surgeries for fractures of the nasal bone, maxilla, and other related areas, thereby providing the necessary medical evidence for establishing rational insurance coverage criteria.

Subcommittee The seven-member Subcommittee evaluated the safety and effectiveness of the technique during four meetings held over a 5-month period, concluding on May 23, 2019. The final determination regarding the safety and effectiveness of the Navigation System for fracture surgeries was made at the Research Planning and Management Committee meeting on June 14, 2019.

Objectives and Methods This study aimed to assess the safety and effectiveness of the Navigation System for nasal bone and maxillary fracture surgeries to generate the necessary evidence for establishing rational insurance coverage criteria. The specific research objectives and key questions were as follows: · Is the use of the Navigation System clinically safe for surgeries involving nasal bone, maxillary, zygomatic, mandibular, and naso-orbito-ethmoid (NOE) fractures, as well as other multiple fractures (compared with not using the Navigation System)? · Is the use of the Navigation System clinically effective for surgeries involving nasal bone, maxillary, zygomatic, mandibular, and NOE fractures, as well as other multiple fractures (compared with not using the Navigation System)?

A systematic review was conducted to evaluate the safety and effectiveness of the Navigation System for fracture surgeries, including nasal and maxillary bone fractures. The following PICO-TS criteria were used.

Literature searches were conducted in five Korean databases (KoreaMed, RISS, KISS, KMbase, and KISTI) and three international databases (Ovid-Medline, Ovid-EMBASE, and Cochrane CENTRAL). The search terms were revised for each database based on the search terms used in Ovid-Medline. Considering the year when the device was introduced, the search was restricted to studies published after 2000. Given that computed tomography (CT) is the only imaging study used due to the nature of fractures, magnetic resonance imaging (MRI) was not included in the search terms. Studies that used the Navigation System in surgeries for nasal bone, maxillary, zygomatic, mandibular, and NOE fractures, as well as other multiple fractures, and reporting at least one predetermined outcome measure were selected. Non-original studies and studies that were not peer-reviewed, not published in Korean or English, or duplicate publications were excluded. Two reviewers independently performed study selection, and any disagreements were resolved through discussion to reach consensus.

The quality of the studies was assessed using the Cochrane Risk of Bias (RoB) tool for randomized controlled trials (RCTs) and the Risk of Bias for Nonrandomized Studies (RoBANS) version 2.0 for nonrandomized studies (NRSs). Data extraction was based on predefined indicators; safety was assessed based on intraoperative and postoperative complications, and effectiveness was assessed based on surgical factors (accuracy of localization, surgical precision, success rate, and symmetry), average surgical duration, length of stay (LOS), and patient-related factors (symptom improvement and patient satisfaction). Although the intention was to conduct meta-analyses of the data, quantitative analysis was not feasible; thus, the results were summarized using a qualitative review method.

Results

Safety The safety of using the Navigation System in nasal bone, maxillary, and other fracture surgeries was primarily assessed based on the incidence of intraoperative and postoperative complications. Safety outcomes were reported in 13 of 26 selected studies. The results were qualitatively evaluated based on the fracture site.

No safety outcomes were reported for nasal bone fracture surgeries using the Navigation System. For maxillary fractures, one study reported no safety issues directly related to the Navigation System, although complications such as malunion of the bone, plate exposure, intracranial abscess, and microstomia were observed. The safety of the Navigation System in zygomatic fracture surgeries was assessed in nine studies. Two RCTs reported no complications directly associated with the Navigation System, whereas another RCT and two cohort studies reported complications not directly related to the system, with no significant differences between the groups. For mandibular fractures, one RCT found no differences in the incidence of intraoperative and postoperative complications directly related to the Navigation System between the groups. Of the two case series that reported data on the safety of the Navigation System for NOE fracture surgeries, none of the complications were directly related to the system. For other multiple fractures, such as zygomatic-maxillary-orbital fractures, the use of the Navigation System did not result in any severe adverse events.

Effectiveness The effectiveness of the Navigation System in nasal bone, maxillary, and other fracture surgeries was assessed using various outcomes: surgical precision, accuracy of localization, surgical success rate, symmetry, reoperation rate, invasiveness, average surgical duration, LOS, and patient-related factors such as symptom improvement, quality of life, and patient satisfaction. Although none of the studies reported data on reoperation rates or invasiveness, all 26 studies included provided data on at least one of the other effectiveness outcomes. The results are summarized based on the fracture site.

For nasal bone fractures, one case series involving five patients reported successful surgeries using the Navigation System, with an average registration time of 4 minutes.

For maxillary fractures, five studies (including one cohort study, two case series, and one case report) were evaluated. In terms of localization accuracy, one case series reported registration discrepancies of <1 mm. A large retrospective cohort study that assessed surgical precision found that using the Navigation System significantly improved surgical precision compared with not using it. Additionally, three non-comparative studies reported surgical deviations within 1 mm both pre- and post-surgery. Symptom improvement was noted in one cohort study and two case series; the cohort study indicated better recovery rates in the Navigation System group than in the control group, whereas the case series reported significant improvements in feeding tube removal and both aesthetic and functional aspects.

Thirteen studies assessed the effectiveness of the Navigation System in zygomatic fractures, including two RCTs, two cohort studies, eight case series, and one case report. Accuracy of localization: Two case series reported accuracy within 2 mm when using the Navigation System. Surgical precision: This was assessed in three comparative studies (two RCTs and one cohort study) and three non-comparative studies (two case series and one case report). The comparative studies found significantly improved surgical precision with the Navigation System compared with not using it, whereas the non-comparative studies reported good surgical precision with deviations within 2 mm pre- and post-surgery. Surgical success rate: Two case series confirmed accurate reduction, indicating successful outcomes. Symmetry: This outcome was reported in two comparative studies (one RCT and one cohort study) and four case series. The comparative studies assessed symmetry using multiple pre-determined indicators, finding better postoperative symmetry when the Navigation System was used. Similarly, the case series reported improved postoperative symmetry with the Navigation System, regardless of the publication year. Average surgical duration: One RCT and one cohort study reported on this outcome, with both studies finding no significant differences in surgical duration associated with the use of the Navigation System. LOS: This was reported in one cohort study, which found no significant effects related to the use of the Navigation System. Patient-related factors: Symptom improvement was reported in four case studies. Despite variations in the fracture areas (e.g., zygomaticomaxillary complex, zygomatic arch, and zygomatic arch sutures), all studies reported symptom improvement, with no impact from the publication period. Patient satisfaction: Two comparative studies (one RCT and one cohort study) reported on patient satisfaction, finding no significant differences correlating with the use of the Navigation System.

The effectiveness of the Navigation System in mandibular fractures was evaluated based on one RCT. The study found that surgical precision was significantly improved when using the Navigation System compared with not using it. The surgical success rate was assessed by the anatomical reduction rate, and although a direct comparison was challenging due to the lack of reported statistical significance, there was a trend indicating higher surgical success when the Navigation System was used. Additionally, the average surgical duration did not differ between the groups.

For NOE fracture surgeries, the effectiveness of the Navigation System was reported in three case series. One case series assessed both surgical precision and accuracy of localization, finding both to be within ≤1 mm. Another case series determined that the use of the Navigation System had no impact on reducing surgical duration. Symptom improvement, as a patient-related factor, was reported in two case series, with both studies indicating that symptoms improved with the use of the Navigation System.

Other multiple fractures included orbital-zygomatic fractures and zygomatic-maxillary-orbital fractures. The effectiveness of the Navigation System in orbital zygomatic fractures was reported in one cohort study and three case series. Regarding surgical factors, accuracy of localization and surgical precision were reported in one and two case series, respectively. Both parameters were <2 mm, confirming the accuracy of the Navigation System. Surgical success rates were reported in one cohort study and one case series. The cohort study used complete zygomatic reduction as the indicator for surgical success and found a higher tendency for success with the Navigation System, though statistical significance was not reported. The case series also noted favorable correction outcomes with the Navigation System. Additionally, one case series reported symptom improvement, including aesthetic enhancements in facial form. For zygomatic-maxillary-orbital fractures, two non-comparative studies (one case series and one case report) confirmed that both accuracy of localization and surgical precision were <2 mm when using the Navigation System.

Conclusions This systematic review assessed the safety and effectiveness of the Navigation System in nasal bone, maxillary, zygomatic, NOE, and other multiple fracture surgeries. Based on the findings, the Subcommittee made the following recommendations:

The Navigation System has been used in surgeries involving nasal bone, maxillary, zygomatic, and NOE fractures, as well as other multiple fractures (zygomatic-maxillary-orbital and orbital-zygomatic fractures). Of the 13 studies reporting safety data, only one study reported complications directly related to the Navigation System, with no significant differences between the Navigation System and control groups. Complications not directly related to the Navigation System were either not observed or did not differ between the groups. Therefore, the Subcommittee confirmed that the Navigation System is safe, as there were no significant differences in complications directly related to the Navigation System or overall surgical complications associated with its use.

The quality and quantity of evidence for the effectiveness of the Navigation System varied by fracture site. For zygomatic fractures, the Navigation System was deemed effective in improving localization accuracy and surgical precision. Although the evidence for maxillary fractures was less extensive, a large-scale retrospective study confirmed that the Navigation System improved the restoration of occlusion, a key outcome for these fractures. For nasal bone, mandibular, and NOE fractures, as well as other multiple fractures, the Subcommittee concluded that the effectiveness of the Navigation System cannot be determined due to the limited number of studies and the poor quality of the evidence available at this time.
