암항원 CA-50

Abstract

평가배경 '암항원 CA 50'은 사람의 결장직장선암주를 면역원으로서 얻은 단클론 항체 C-50(IgM)에 의해 검출되는 당쇄항원으로, 동 기술은 환자의 혈액 내 암항원 CA 50을 측정하여 위장관 악성 종양 등의 다양한 암 진단 및 치료반응을 관찰하는데 사용하는 혈청학적 종양표지자 검사이다. 신의료기술평가 이전에 등재된 비급여 항목의 예비급여 도입 검토를 위하여, 건강보험심사평가원에서 관련 학(협)회 의견 수렴 절차를 진행한 바 있으며, 암항원 CA-50이 ‘안전성 및 유효성 검증 필요’ 하다는 의견이 제시되어 본원으로 평가를 의뢰하였다. 이에, 소위원회를 구성하여 “암항원 CA-50"에 대한 안전성 및 유효성 평가를 구행하고자 한다.

위원회 운영 총 9인(진단검사의학과 2인, 소화기내과 2인, 혈액종양내과 2인, 호흡기내과 1인, 산부인과 1인, 근거기반의학 1인)으로 구성된 소위원회는 2019년 5월 30일부터 2019년 9월 26일까지 약 4개월간 총 4회의 소위원회의 운영을 통해 동 기술의 안전성 및 유효성을 평가하였다.

평가방법 암항원 CA-50에 대한 안전성 및 유효성 평가는 체계적 문헌고찰을 통해 수행하였다. 본 평가에서는 건강보험요양급여비용목록에 등재(노-282)되어 있는 대상질환(악성종양질환) 및 문헌에서 보고된 여러 대상 질환 중 임상에서 유효성 검증 필요가 있는 질환 위주로 대상환자를 정의하였으며, 대상질환별로 구분하여 참조검사 및 비교검사를 설정하였다. 동 기술의 안전성은 검사수행으로 인한 부작용, 검사결과로 인한 위해, 유효성은 진단정확성, 예후와의 관련성, 의료결과에의 영향 지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌선택기준으로 동 검사의 임상적 안전성 및 유효성 평가를 위해 비급여로 등재된 시점인 2001년 이후의 연구로 연도제한을 하였다. 문헌의 비뚤림 위험 평가는 QUADAS-2, RoBANS를 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 자료분석은 정성적(qualitative review) 분석을 수행하였다.

평가 결과 암항원 CA-50검사의 안전성 및 유효성을 평가하기 위해 수행된 체계적 문헌고찰 결과, 총 20편(국외) 문헌이 선택되었다. 선택된 연구들은 연구유형별로 진단정확성을 보고한 단면연구 12편, 코호트 연구 8편이 포함되었으며, 대상질환에 따라 구분시 췌장암 9편, 위암 7편, 대장직장암 4편이었으며, 이외 대상 질환군(담도암, 간암, 난소암, 폐암)에서는 관련 연구결과가 확인되지 않았다. 동 검사의 안전성에 대해서는 부작용 및 검사결과에 따른 위해와 관련된 의료결과를 보고한 연구는 확인되지 않았다. 다만, 소위원회에서는 동 기술이 검체 채취 후 체외에서 이루어지는 검사로, 검체 채취 등의 과정으로 인체에 직접적인 위해를 가하지 않으므로 검사수행에 따른 안전성의 문제는 낮을 것으로 보았다. 동 검사의 유효성에 대해서는 진단정확성(진단/예후예측), 예후와의 관련성 지표를 통해 평가하였다. 질환 감별에 대한 진단정확성은 총 13편에서 보고되었다. 질환별로 구분하여 살펴보면, ① 췌장암과 비췌장암(양성췌장질환 등) 환자를 대상으로 한 연구에서 췌장암을 진단하는데 중재검사(6편)는 민감도 0.13-0.78, 특이도 0.68-1.00, AUC값(2편) 0.73-0.74이었으며, 비교검사는 CA 19-9 (6편)의 경우 민감도 0.19-0.84, 특이도 0.60-1.00, AUC값(2편) 0.81-0.82, CEA (4편)는 민감도 0.2-0.64, 특이도 0.60-0.94, AUC값(2편) 0.64-0.71, CA 125 (1편)은 민감도 0.51, 특이도 0.89이었다. 임상진료현장에서 가장 많이 활용되고 있는 CA 19-9 검사와 비교시 중재검사는 비교적 낮은 민감도를 보였다. 또한, 연구결과가 1편으로 제한적이긴 하지만 루이스 항원 음성 환자 대상으로 수행된 연구에서, 중재검사의 민감도는 0.13, CA 19-9는 0.19로 나타났는데 이는 기존의 CA 19-9가 루이스(lewis) 항원 음성인 환자에서는 발현이 되지 않아 진단 검사로서 제한점이 있는 임상 상황에서 이를 보완할 검사로써 중재검사의 임상적 의미를 확인하기는 어려웠다. ② 위암 환자와 건강인 대상으로 한 연구(2편)에서는 위암 진단에 대해 중재검사는 민감도 0.37-0.5, 특이도 0.97-0.99, AUC값은 0.81-0.83, 비교검사로 CA 19-9는 민감도 0.26-0.67, 특이도 0.67, AUC값 0.63-0.64, CEA는 민감도 0.13-0.17, 특이도 0.98, AUC값 0.66-0.68, CA 72-4는 민감도 0.10-0.28, 특이도 0.98-0.99, AUC값 0.67이었다. 또한, 위암 환자군에서의 민감도는 중재검사(4편) 0.14-0.43, CA 19-9 (4편)는 0.17-0.35, CEA (4편)는 0.095-0.20, CA 72-4 (1편)은 0.22이었다. ③ 대장직장암 환자에서의 민감도는 중재검사가 0.81, CEA는 0.39 으로, 중재검사가 높게 보고되었으나 일반화하기에는 연구결과가 매우 부족하였다. 예후예측에 대한 진단정확성은 총 3편에서 보고되었다. 췌장암 환자에서 수술 후 전이(stage IV) 예측에 대한 정확성을 보고한 1편의 연구에서는 중재검사 AUC값은 0.554, 비교검사인 CA 19-9, CEA, CA 125는 각 AUC값 0.722, 0.716, 0.892 으로 보고되었다. 다른 2편은 대장직장암 환자를 대상으로 수행되었으며, 수술 후 전이 예측에 대한 정확성을 보고한 1편에서는 AUC값이 중재검사는 0.75, 비교검사 CEA, CA 19-9는 0.77이었고, 수술 후 5년 내 재발 발생에 대한 정확성을 보고한 1편에서는 중재검사 민감도 0.46-0.57, 특이도 0.77-0.82, CEA는 민감도 0.63-0.79, 특이도 0.64-0.82이었다. 평가에 포함된 문헌의 숫자가 적어 예후를 예측하는데 동 검사가 임상적으로 유효한지 결론을 내리기는 어려웠다. 예후와의 관련성은 총 10편(췌장암 3편, 위암 5편, 대장직장암 2편)에서 보고되었다. 췌장암과 위암에서는 전반적으로 단변량 분석시에는 수술 전 중재검사가 임계치보다 상승하거나 수술 후 수치 감소가 없는 것은 전체 생존, 무질병 생존에 대해 유의한 위험인자로 보고되었으나, 다른 위험요인을 보정한 다변량 분석시 대부분 유의하지 않은 것으로 나타났다. 대장직장암 연구에서는 1편은 유의하였고 다른 1편은 유의하지 않아 이질적인 결과를 보고하였다.

결론 본 연구에서 체계적 문헌고찰을 수행한 결과, 암항원 CA 50 검사는 췌장암 등 악성 종양 감별진단, 암환자에서의 예후를 확인하는 목적으로 사용시 검사 수행에 따른 안전성에는 문제가 없으나, 국내에서 거의 검사가 이루어지지 않는 기술이며, 국내외의 임상진료지침 및 가이드라인에서 종양표지자로서 권고되고 있지 않고, 동 검사가 악성종양의 진단 및 암환자의 예후를 추적 관찰하는데 유효성이 있다고 결론을 내리기에는 문헌적 근거가 불충분하다고 평가하였다. 의료기술재평가위원회는 “암항원 CA-50”에 대해 소위원회 검토결과에 근거하여 다음과 같이 심의하였다(2019.11.8.). 의료기술재평가위원회는 췌장암, 위암, 대장직장암 등 악성종양 (의심)환자에서 진단, 예후예측 및 치료반응을 관찰하는데 암항원 CA50을 권고하지 않는다(권고등급 II).

Background Cancer Antigen (CA) 50 is a xenoantigen detected by C-50 (IgM), a monoclonal antibody obtained from human colorectal cancer cell lines. The technology is utilized as a serological tumor marker test by measuring the CA 50 level in the blood, and it is used to diagnose various cancers (including malignant tumors in the gastrointestinal tract) and for follow-up on treatment response. The Health Insurance Review & Assessment Service (HIRA) sought the opinions of relevant academic societies (and associations) on currently not-covered technologies that had been listed before the introduction of the Innovative Health Technology Assessment system to determine whether or not to change the coverage status of any of the items to preliminary coverage. An opinion was presented that CA-50 should be assessed on safety and effectiveness, and the Korea National Evidence-based Healthcare Collaborating Agency (NECA) was requested to assess the technology. Accordingly, NECA assembled a subcommittee and performed a safety and effectiveness assessment of Cancer Antigen CA-50. Committee operation To assess the safety and effectiveness of the said technology, the subcommittee of 9 members (2 clinical pathologists, 2 gastroenterologists, 2 hematologists, 1 pulmonologist, 1 obstetrician-gynecologist, and 1 evidence-based medicine specialist) held 4 meetings within approximately 4 months from May 30 to September 26, 2019.

Methods The safety and effectiveness assessment of Cancer Antigen CA-50 was performed by conducting a systematic review. In this assessment, target patient groups were defined with a focus on the target diseases (malignant tumors) listed in the health insurance benefit payment list (No-282) and diseases reported in the literature for which the technology's clinical effectiveness needs to be demonstrated. Reference and comparison tests were evaluated for each target disease. The safety of the said technology was evaluated by examining the adverse effects of the test and harm due to inaccurate test results. The effectiveness was evaluated by examining diagnostic accuracy, relationship with prognosis, and healthcare impact. For the systematic review, 5 domestic databases and 3 foreign databases were searched using the core questions. Two reviewers independently searched and selected articles based on the predetermined selection and exclusion criteria. Only studies published in 2001 and thereafter were selected, reflecting the period after the addition of the said technology to the no-coverage list. The two reviewers independently assessed bias risk during the article selection using QUADAS-2 and RoBANS and reached a consensus. The reviewers independently extracted data using a predetermined data extraction form. Regarding the items they disagreed on, a consensus was reached by discussing with a third reviewer. A qualitative review was performed to examine the extracted data.

Results The systematic review conducted to assess the safety and effectiveness of the Cancer Agent CA-50 test involved 20 articles (all foreign studies). Grouped by study type, 12 of the 20 articles were cross-sectional studies reporting diagnostic accuracy, and 8 were cohort studies. Grouped by target disease, 9 articles were on pancreatic cancer, 7 were on stomach cancer, and 4 were on colorectal cancer. No findings were found regarding other target disease groups (gallbladder, liver, ovarian, and lung cancers). Regarding the safety of the said technology, no articles reported the adverse effects and harm due to inaccurate test results. However, the subcommittee thought that there was little concern about safety, because the test is conducted on biopsied tissues and it does not directly cause harm during a biopsy. The effectiveness of the said technology was assessed based on diagnostic accuracy (diagnosis/prognosis prediction) and prognostic indices. Diagnostic accuracy was reported in 13 articles. Based on the cancer type, the sensitivity, specificity, and AUC of the index diagnostic test (a total of 6 articles) were 0.13-0.78, 0.68-1.00, and 0.73-0.74 (2 articles), respectively, for the studies in which pancreatic cancer was diagnosed in patients with pancreatic or non-pancreatic cancers (including benign tumors of the pancreas). The sensitivity, specificity, and AUC were 0.19–0.84, 0.60–1.00, and 0.81–0.82 (2 articles) for the tests that used CA 19-9 as the reference standard (a total of 6 articles) and 0.2–0.64, 0.60–0.94, and 0.64–0.71 (2 articles), respectively, for those that used CEA as the reference standard (4 articles). A study in which CA 125 was used (1 article) reported a sensitivity of 0.51 and a specificity of 0.89. Compared to CA 19-9, the reference standard most commonly used in practice, the index test, showed relatively low sensitivity. Though it is difficult to generalize based on a single study, a study involving Lewis-negative patients reported sensitivities of 0.13 and 0.19 for the index test and the test using CA 19-9, respectively. In practice, the existing test using CA 19-9 has a limitation because the antigen is not expressed in Lewis-negative patients. Thus, it was difficult to determine the clinical significance of the index test. In studies involving stomach cancer patients and healthy controls (2 articles), the sensitivity, specificity, and AUC of the index test was 0.37-0.5, 0.97-0.99, and 0.81-0.83, respectively. The sensitivity, specificity, and AUC were as follows: 0.26–0.67, 0.67, and 0.63–0.64, respectively, for CA 19-9; 0.13–0.17, 0.98, and 0.66–0.68, respectively, for CEA; 0.10–0.28, 0.98–0.99, 0.67, respectively, for CA 72-4. For stomach cancer patients, the sensitivity was 0.14–0.43 for the index test (4 articles), 0.17–0.35 for CA 19-9 (4 articles), 0.095-0.20 for CEA (4 articles), and 0.22 for CA 72-4 (1 article). In colorectal cancer patients, the sensitivities were 0.81 and 0.39 for the index test and CEA, respectively. The index test showed higher sensitivity, but it was difficult to generalize given that only a few studies had been conducted. Diagnostic accuracy related to prognostic prediction was reported in 3 articles. In a study that examined the accuracy of prognostic prediction regarding post-surgical metastasis (stage IV) in pancreatic patients, the AUC of the index test was 0.554, whereas the AUCs of diagnostic testing based on a reference standard were 0.722, 0.716, and 0.892 for CA 19-9, CEA, and CA 125, respectively. The other 2 articles involved colorectal cancer patients. One of the articles, which examined prediction accuracy, reported that the AUC of the index test was 0.75, while both CEA and CA 19-9 showed an ACU of 0.77. In the other article, which reported the accuracy of the prognostic prediction of recurrence within 5 years after surgery, the sensitivity and specificity were 0.46–0.57 and 0.77–0.82 for the index test and 0.63–0.79 and 0.64–0.82 for CEA, respectively. Because only a few articles were included in the review, it was difficult to conclude on the clinical effectiveness of the test. The association with prognosis was reported in 10 articles (3 articles for pancreatic cancer, 4 for stomach cancer, and 2 for colorectal cancer). For pancreatic and stomach cancers, the elevation of the pre-surgery index test results above a threshold and the lack of a reduction in the post-surgery index test results were significant risk factors against overall survival and disease-free survival in the studies in which univariate analysis was conducted. However, they were not significant for most studies in which multivariate analysis was conducted with the other risk factors controlled. For colorectal cancer, they were significant in one of 2 articles and insignificant in the other; hence, the study findings were conflicting.

Conclusion Based on the systematic review, the subcommittee determined that the Cancer Antigen CA 50 test does not have a safety issue when used for the differential diagnosis of a malignant tumor (such as pancreatic cancer) or prognostic prediction in cancer patients. The subcommittee indicated that the test is rarely used in South Korea, and it is not recommended as a tumor marker in clinical practice guidelines in or outside South Korea. The committee added that evidence in the literature is insufficient for concluding on the effectiveness of the said test for diagnosing a malignant tumor and following-up on cancer patients. Based on the subcommittee's review, the Health Technology Reassessment Committee made the following assessment on "Cancer Agent CA-50” (November 8, 2019). The Health Technology Reassessment Committee does not recommend the use of the Cancer Agent CA50 for diagnosing patients with suspected malignant tumors (such as pancreatic, stomach, and colorectal cancers), predicting prognosis, or following-up on treatment response (Grade of recommendation grade - II)
