고주파 전류를 이용한 내측 접근 섬유주절개술의 안전성 및 유효성 평가

Abstract

평가배경 고주파 전류를 이용한 내측 접근 섬유주절개술은 녹내장 환자를 대상으로 섬유주와 쉴렘관의 내측벽만 선택적으로 제거하고 이외의 쉴렘관의 외측벽, 집결관 및 방수정맥까지 정상적인 유출로를 최대한 보존하는 수술이다. 해당 기술은 2011년과 2013년에 신의료기술평가를 받았으며 2015년 4월 20일에 등재비급여로 분류되었다(보건복지부 고시 제2015-59호). 예비급여 도입 항목에 대한 재평가 사업(NR19-001 의료기술재평가사업, 연구책임자: 최인순 연구위원)의 일환으로 과거에 신의료기술평가를 수행했던 안건 중 현재 등재비급여로 분류된 고주파 전류를 이용한 내측 접근 섬유주절개술에 대해 최신 근거로 업데이트를 수행하였다.

위원회 운영 총 5인으로 구성된 소위원회는 2019년 4월 26일부터 2019년 7월 24일까지 약 3개월간 총 3회의 소위원회 운영을 통해 체계적 문헌고찰의 PICO-TS, 문헌선정, 자료합성 및 근거수준 평가 결과를 논의 후 결론을 도출하였다. 2019년 연구기획관리위원회(2019.9.20.)에서는 고주파 전류를 이용한 내측 접근 섬유주절개술의 임상적 안전성 및 유효성의 평가 결과를 최종 심의하였다.

평가 목적 및 방법 본 연구는 합리적인 급여기준 설정에 필요한 의학적 근거자료를 마련하기 위해 현재 등재비급여로 분류된 고주파 전류를 이용한 내측 접근 섬유주절개술에 대해 체계적 문헌고찰을 수행하였다. 동 평가는 기 평가 이후 추가된 문헌들의 연구결과를 검토하기로 하였으므로, 체계적 문헌고찰의 방법론에 따라 기평가 당시 검색일로부터 1년 전인 2013년부터 2019년 4월까지의 문헌을 검색한 후 2011년과 2013년 기평가 시 포함된 문헌들과 함께 검토하였다. 녹내장 환자를 대상으로 한 고주파 전류를 이용한 내측 접근 섬유주절개술의 안전성 및 유효성은 각각 시술관련 합병증 및 부작용(앞방출혈, 안압 상승, 내안구염, 재수술, 시력 감소, 맥락막 손상, 지속적인 저안압증, 여과포 누출, 악성녹내장, 얕은 전방)과 안압의 감소, 항녹내장 약물 사용량의 변화, 시술성공률 지표로 평가하였다. 시술성공률은 TVT 연구그룹(Tube versus Trabeculectomy Study Group)의 정의에 따라 수술 결과 평균안압이 21 mmHg 이하이면서, 수술 3개월 이후 연속 2회의 추적 관찰시 기저값보다 20% 이상 감소된 경우, 그리고 이차 수술이 불필요한 경우로 정의하였다. 시술이 정의에 따라 실패하지 않고 추가적 약물치료가 불필요한 경우는 ‘완전 성공(complete success)’으로, 추가적 약물치료가 필요한 경우는 ‘부분 성공(qualified success)’으로 구분하였다. 동 평가의 문헌 비뚤림 위험 평가는 무작위배정 임상시험 연구(randomized controlled trial, RCT)일 경우 Cochrane의 Risk of Bias(RoB)를 이용하여 평가하였고 비무작위배정 연구(non-randomized controlled trial, NRCT)일 경우 Risk of Bias for Nonrandomized Studies(ROBANS) Ver 2.0을 이용하여 평가하였다. 최종 선정된 문헌들의 주요결과는 GRADE(Grading of Recommendations Assessment Development and Evaluation) 방법에 따라 GRADEpro를 이용하여 근거수준을 평가하였다.

평가 결과 최종적으로 RCT 1개, NRCT 10개로 총 11개의 문헌이 선정되었다. 대조군이 다양한 관계로 소위원회의 논의를 통하여 대조군을 섬유주절제술(RCT 연구 1개, NRCT 연구 4개), iStent(NRCT 연구 4개), 방수유출장치삽입술(NRCT 연구 2개), 기타(두 군 모두 Ahmed 녹내장 밸브를 삽입하고 중재군에만 고주파 전류를 이용한 내측 접근 섬유주절개술을 추가, NRCT 연구 1개)로 분류하여 결과를 제시하였다.

가. 안전성 결과 고주파 전류를 이용한 내측 접근 섬유주절개술의 안전성은 앞방출혈, 안압 상승, 안구내염, 재수술, 시력 감소, 맥락막 손상, 지속적인 저안압증, 여과포 누출, 악성녹내장, 얕은 전방의 발생을 대조군별로 보았다. 수술 후 앞방출혈의 발생은 대조군이 섬유주절제술인 경우 RCT 연구 1개, NRCT 연구 1개에서 보고하였다. RCT 연구 1개에서는 중재군에서 40%, 대조군에서 0% 발생했으나 두 군의 차이는 유의하지 않았다(p-value=0.60). NRCT 연구 1개에서는 중재군에서 100%, 비교군에서 2.9% 발생했으나 이는 국소적이었고 특별한 치료 없이 1일~7일 이내로 증상이 사라졌다고 보고하고 있었다. 대조군이 iStent인 경우 수술 후 앞방출혈의 발생은 RCT 연구 2개에서 보고하였으며 두 연구 모두 중재군에서의 발생이 유의하게 높았고 증상은 1주 혹은 1개월 후 사라졌다고 보고하고 있었다. 앞방출혈의 발생은 시술 방법상의 특성에 기인하는 것으로 고주파 전류를 이용한 내측 접근 섬유주절개술은 섬유주와 쉴렘관의 내벽에 틈새를 만들어 혈액 역류가 발생하기 쉽기 때문에 앞방출혈은 흔히 발생하는 증상이라고 각 연구에서는 설명하고 있었다. 수술 후 10 mmHg를 초과한 안압 상승의 발생은 대조군이 섬유주절제술인 경우 NRCT 연구 1개에서 보고하였고, 중재군에서 3.5%, 대조군에서 2.9%로 두 군 모두 발생했다. 대조군이 iStent인 경우 수술 후 안압 상승의 발생은 NRCT 연구 2개에서 보고했으며 이 중 1개의 연구에서는 중재군에서 33%, 대조군에서 16% 발생했고 두 군의 차이는 유의하지 않았다(p-value=0.07). 다른 1개 연구에서는 중재군에서 2.8%, 대조군에서 0% 발생했고 두 군간 유의한 차이를 보였으나 수술 후 1개월 후에는 대부분의 증상이 사라졌다고 보고하고 있다. NRCT 연구 2개를 메타분석 한 결과 두 군간 유의한 차이가 있었다(OR 2.52, 95% CI 1.01~6.30, I2=0%). 수술 후 안구내염의 발생은 대조군이 iStent인 NRCT 연구 1개에서만 보고했으며 중재군과 대조군 모두 수술 후 안구내염은 발생하지 않았다. 녹내장 재수술의 발생은 대조군이 섬유주절제술인 경우 RCT 연구 1개, NRCT 연구 2개에서 보고하였다. RCT 연구 1개에서는 녹내장 재수술이 중재군에서 10%, 대조군에서 0% 발생했으나 두 군의 차이는 유의하지 않았다(p-value=0.36). NRCT 연구 2개에서는 메타분석을 한 결과 두 군간 유의한 차이는 없었다. 대조군이 iStent인 경우 재수술의 발생은 NRCT 연구 3개에서 보고했으며 두 군간 유의한 차이는 없었다. 대조군이 방수유출장치삽입술인 경우 재수술의 발생은 NRCT 연구 2개에서 보고했고 메타분석을 한 결과 두 군간 유의한 차이는 없었다. 수술 후 ≥2 Snellen lines의 시력 감소 발생은 대조군이 섬유주절제술인 경우 NRCT 연구 1개에서 보고하였고 두 군 모두 발생하지 않았다. 대조군이 방수유출장치삽입술인 경우 시력 감소의 발생은 NRCT 연구 1개에서 보고했으며 중재군에서 0%, 대조군에서 3.1% 발생했다. 수술 후 맥락막 손상(맥락막 박리, 삼출)의 발생은 대조군이 섬유주절제술인 경우 RCT 연구 1개, NRCT 연구 2개에서 보고하였다. RCT 연구 1개에서는 중재군에서는 맥락막 손상이 발생하지 않았고 대조군에서 22.2% 발생했다. NRCT 연구 2개에서는 모두 중재군에서는 발생하지 않았고 대조군에서만 발생했으며(각각 9%, 3.9%) 메타분석 결과 두 군간 차이는 유의하였다. 대조군이 iStent인 경우 맥락막 손상의 발생은 NRCT 연구 2개에서 보고했으며 두 군 모두 발생하지 않았다. 대조군이 방수유출장치삽입술인 경우 맥락막 손상의 발생은 NRCT 연구 1개에서 보고했으며 중재군에서 0%, 대조군에서 9.3% 발생했다. 수술 후 지속적인 저안압증의 발생은 대조군이 섬유주절제술인 경우 NRCT 연구 3개에서 보고하였다. 3개 연구 모두 중재군에서는 발생하지 않았고 대조군에서만 발생했으며(각각 17%, 8.3%, 4.9%) 메타분석 결과 두 군간 차이는 유의하였다(OR 0.06, 95% CI 0.01~0.35, I2=0%). 대조군이 iStent인 경우 지속적인 저안압증의 발생은 NRCT 연구 1개에서 보고했고 두 군 모두 발생하지 않았다. 대조군이 방수유출장치삽입술인 경우 지속적인 저안압증의 발생은 NRCT 연구 1개에서 보고했으며 중재군에서는 발생하지 않았고 대조군에서만 6.9% 발생했다. 수술 후 여과포 누출의 발생은 대조군이 섬유주절제술인 RCT 연구 1개, NRCT 연구 2개에서만 보고하였다. RCT 연구 1개에서는 중재군에서는 여과포 누출이 발생하지 않았고 대조군에서 22.2% 발생했다. NRCT 연구에서는 1개 연구에서 두 군 모두 여과포 누출이 발생하지 않았고 나머지 1개 연구에서는 중재군에서 0%, 대조군에서 11.8% 발생했다. 수술 후 악성녹내장의 발생은 대조군이 섬유주절제술인 경우 NRCT 연구 1개에서 보고하였으며 중재군에서는 발생하지 않았고 대조군에서만 4.3% 발생했다. 대조군이 방수유출장치삽입술인 경우 악성녹내장의 발생은 NRCT 연구 1개에서 보고했으며 중재군에서 0%, 대조군에서 3.4% 발생했다. 수술 후 얕은 전방의 발생은 대조군이 섬유주절제술인 NRCT 연구 1개에서만 보고하였으며 중재군에서는 발생하지 않았고 대조군에서만 7.8% 발생했다.

나. 유효성 결과 고주파 전류를 이용한 내측 접근 섬유주절개술의 유효성은 안압의 감소, 항녹내장 약물 사용량의 변화, 시술성공률을 대조군별로 보았다. 수술 후 평균 안압은 대조군이 섬유주절제술인 경우 RCT 연구 1개, NRCT 연구 4개에서 보고하였다. RCT 연구 1개에서는 최종적으로 중재군의 평균 안압이 대조군 대비 낮았으나 이 차이는 유의하지 않았고, 두 군 모두 비슷한 안압 감소를 보였다고 보고했다. 4개의 NRCT 연구들을 메타분석 한 결과 중재군의 수술 후 최종 평균 안압이 대조군에 비해 유의하게 높았다(WMD 5.26 mmHg, 95% CI 3.83~6.68, I2=0.0%). 대조군이 iStent인 경우 수술 후 최종 평균 안압은 4개의 NRCT 연구들을 메타분석한 결과 중재군이 대조군에 비해 유의하게 높았다(WMD 2.12 mmHg, 95% CI 1.15~3.09, I2=7.6%). 대조군이 방수유출장치삽입술인 경우 수술 후 최종 평균 안압은 2개의 NRCT 연구들을 메타분석했을 때 중재군과 대조군 간 유의한 차이가 없었다. 대조군이 기타(두 군 모두 Ahmed 녹내장 밸브를 삽입하고 중재군에만 고주파 전류를 이용한 내측 접근 섬유주절개술을 추가)인 1개의 NRCT 연구에서는 수술 후 최종 평균 안압은 중재군이 대조군 대비 유의하게 낮았다고 보고했다. 수술 후 평균 항녹내장 약물 사용량은 대조군이 섬유주절제술인 경우 RCT 연구 1개, NRCT 연구 4개에서 보고하였다. RCT 연구 1개에서는 평균 항녹내장 약물 사용이 최종적으로 중재군이 대조군 대비 더 감소했고 두 군의 차이는 유의하지 않았다고 보고했다. NRCT 연구 4개 중 메타분석이 가능한 3개 연구들을 분석한 결과 중재군의 수술 후 평균 항녹내장 약물 사용량이 대조군 대비 유의하게 높았다(WMD 1.59개, 95% CI 1.18~2.01, I2=0.0%). 대조군이 iStent인 경우 수술 후 평균 항녹내장 약물 사용량은 3개의 NRCT 연구들을 메타분석한 결과 중재군과 대조군간 유의한 차이는 없었다. 대조군이 방수유출장치삽입술인 경우 2개의 NRCT 연구들을 메타분석한 결과 중재군의 수술 후 평균 항녹내장 약물 사용량이 대조군 대비 유의하게 낮았다(WMD -1.04개, 95% CI –1.55~-0.53, I2=0.0%). 대조군이 기타(두 군 모두 Ahmed 녹내장 밸브를 삽입하고 중재군에만 고주파 전류를 이용한 내측 접근 섬유주절개술을 추가)인 1개의 NRCT 연구에서 또한 수술 후 평균 항녹내장 약물 사용량이 대조군 대비 유의하게 낮았다(p-value=0.001). 시술성공률은 대조군이 섬유주절제술인 경우 RCT 연구 1개, NRCT 연구 3개에서 보고하였다. RCT 연구 1개에서 완전 성공률은 중재군이 20%, 대조군이 50%였고 두 군간 차이는 유의하지 않았다. 부분 성공률은 중재군이 20%, 대조군이 37.5%였고 두 군간 차이는 유의하지 않았다. NRCT 연구 3개에서 완전 성공을 보고한 연구는 없었고 부분 성공은 메타분석을 수행한 결과 대조군이 중재군 대비 유의하게 높았으나 이질성이 높았다(OR 0.55, 95% CI 0.39~0.78, I2=82.5%). 대조군이 iStent인 경우 완전 성공과 부분 성공을 보고한 NRCT 연구는 각각 1개였다. 완전 성공률의 경우 중재군은 18.2%, 대조군은 9.7%였으나 두 군간 차이는 유의하지 않았다. 부분 성공률의 경우 중재군은 34.6%, 대조군은 59.2%였으며 두 군간 유의한 차이를 보였다(p-value=0.01). 대조군이 방수유출장치삽입술인 경우 완전 성공을 보고한 연구는 없었고 부분 성공을 보고한 2개의 NRCT 연구를 메타분석한 결과 중재군과 대조군간 유의한 차이는 없었다. 대조군이 기타(두 군 모두 Ahmed 녹내장 밸브를 삽입하고 중재군에만 고주파 전류를 이용한 내측 접근 섬유주절개술을 추가)인 1개의 NRCT 연구에서는 완전 성공률은 중재군이 56.2%, 대조군이 13.5%였고 부분 성공률은 중재군이 70%, 대조군이 65%였으며 두 군간 유의한 차이는 없었다. 다. 근거수준 평가 결과 근거수준의 평가 결과는 RCT 1개 연구의 경우 모든 지표가 ‘중등도’였고 NRCT 10개 연구의 경우 대조군별로 근거수준을 평가한 결과 모든 지표가 ‘낮음’ 혹은 ‘매우 낮음’이였다.

결론 고주파 전류를 이용한 내측 접근 섬유주절개술 소위원회에서는 안전성의 경우 중재시술의 합병증으로 보고된 앞방출혈은 특정한 좁은 부위를 뚫어 방수유출 경로를 확보하는 동 시술 방법상의 특성에 기인하는 것이며 앞방출혈과 안압 상승은 대부분 수일 내 자연 치유되는 초기 합병증으로 유의할만한 합병증 및 부작용으로 볼 수 없다는 의견이었다. 또한 이를 제외한 안구내염, 재수술, 시력 감소, 맥락막 손상, 지속적인 저안압증, 여과포 누출, 악성녹내장, 얕은 전방의 발생은 대조군(섬유주절제술, iStent, 방수유출장치삽입술, 기타)과 비교했을 때 유의한 차이가 없거나 대조군에서의 발생빈도가 더 높아 동 시술은 안전한 것으로 판단하였다. 소위원회에서는 유효성의 경우 모든 연구에서 중재시술이 수술 전에 비해 수술 후 평균 안압의 감소(평균 감소율 29.9%)와 항녹내장 약물 사용량의 감소(평균 감소율 37.2%)가 확인 가능하므로 동 수술법의 유효성을 인정 가능하다는 의견이었다. 또한 녹내장은 환자상태가 다양하고 장기적인 관리가 필요한 질병이므로 침습도가 높은 타 시술의 시행 전에 고려해 볼 수 있는 기술이라고 보았다. 단, 동 수술법은 섬유주와 쉴렘관의 내측벽만 선택적으로 제거하는 것으로 장비의 접근이 어려운 폐쇄각녹내장, 신생혈관성녹내장, 종양으로 인한 이차성녹내장의 경우에 적용하는 것은 적절치 않다고 보고, 이를 제외한 녹내장 환자를 대상으로 하는 것이 적절하다고 판단하였다. 따라서 고주파 전류를 이용한 내측 접근 섬유주절개술 소위원회에서는 현재 평가 결과에 근거하여 다음과 같이 제언하였다. 고주파 섬유주절개술은 장비의 접근이 어려운 폐쇄각녹내장, 신생혈관성녹내장, 종양으로 인한 이차성녹내장을 제외한 녹내장 환자를 대상으로 기존 여과수술에 비해 덜 침습적인 방법으로 합병증을 줄이고, 안압을 효과적으로 조절할 수 있는 안전성과 유효성에 대한 근거가 있는 기술이라고 평가하였다. 의료기술재평가위원회는 “고주파 전류를 이용한 내측 접근 섬유주절개술”에 대해 소위원회 검토결과가 타당하다고 심의하였다(2019.09.20.).

Assessment background Ab-interno trabeculotomy (AIT) with high-frequency current is a procedure used to selectively remove on the trabecula and inner wall of Schlemm’s canal in patients with glaucoma to preserve as much as possible the normal outflow tract, from the outer wall of Schlemm’s canal, collector channel, to aqueous vein. This technology received new health technology assessment in 2011 and 2013 and was subsequently classified as registered non-coverage item on April 20, 2015 (Ministry of Health and Welfare notification 2015-59). As a part of the project to reassess preliminary coverage implementation items (NR19-001 health technology reassessment project, Principal investigator: In-Soon Choi), AIT with high-frequency current, which is currently classified as a registered non-coverage item among the items assessed in previous new health technology assessments, was updated with the latest evidence.

Committee operation A subcommittee consisting of five members held a total of three subcommittee sessions over a 3-month period between April 26 and July 24, 2019 to discuss and reach conclusions on PICO-TS, article selection, and data synthesis of the systematic literature review and assessment of level of evidence. In 2019 (September 20, 2019), the research planning and management committee conducted a final review of the findings in the assessment of clinical safety and effectiveness of AIT with high-frequency current.

Assessment objectives and methods A systematic literature review was performed on AIT with high-frequency current, which is currently classified as a registered non-coverage item, to establish the medical evidence needed for setting a reasonable coverage standard. This assessment was designed to review the study results reported in additional articles published since the previous assessment. In accordance with the methodology for a systematic literature review, articles published between 2013 (one year prior to the previous assessment) and April 2019 were searched and those articles were reviewed together with the articles that were included in the assessments in 2011 and 2013. The safety and effectiveness of AIT with high-frequency current in patients with glaucoma were assessed based on the following indicators: procedure-related complications and adverse events [anterior chamber bleeding, increased intraocular pressure (IOP), endophthalmitis, re-operation, visual decline, choroidal damage, persistent ocular hypotony, aqueous outflow, malignant glaucoma, and shallow anterior chamber), decrease in IOP, change in the amount of anti-glaucoma drug used, and procedure success rate. Procedure success rate followed the definition given by the Tube versus Trabeculectomy (TVT) Study Group – mean postoperative IOP of ≤ 21 mmHg and being ≥ 20% lower than the baseline value during at least two consecutive follow-ups after at least three months after the procedure, while also not requiring a secondary procedure. Cases that did not need additional drug therapy and the procedure did not fail according to the definition were classified as complete success, whereas cases that needed additional drug therapy were classified as qualified success. Risk of bias assessment was performed using Cochrane’s risk of bias for randomized clinical trials (RCTs) and risk of bias for nonrandomized studies (ROBANS) ver 2.0 for non-RCTs. The major findings in the articles that were ultimately selected were assessed for level of evidence using GRADEpro according to the grading of recommendations assessment development and evaluation (GRADE) method.

Assessment results A total of 11 articles were ultimately selected, including one RCT and 10 non-RCTs. Because the control groups varied, the results were presented with the control groups classified as the trabeculectomy (one RCT and four non-RCTs), iStent (four non-RCTs), glaucoma drainage device implantation (two non-RCTs), and other (both groups receiving Ahmed glaucoma valve implantation but only the intervention group receiving additional AIT with high-frequency current; one non-RCT) based on subcommittee discussions.

1. Safety The safety of AIT with high-frequency current was examined by the incidence of anterior chamber bleeding, increase in IOP, endophthalmitis, re-operation, visual decline, choroidal damage, persistent ocular hypotony, aqueous outflow, malignant glaucoma, and shallow anterior chamber by each control group. Postoperative anterior chamber bleeding in trabeculectomy control group was reported in one RCT and one non-RCT. One RCT reported incidence of 40% in the intervention group and 0% in the control group, but the difference between the two groups was not significant (p-value=0.60). One non-RCT reported incidence of 100% in the intervention group and 2.9% in the control group, but the symptoms were localized and disappeared within 1-7 days without any specific treatment. Postoperative anterior chamber bleeding in iStent control group was reported in two RCTs. Both RCTs reported significantly higher incidences in the intervention group and that the symptoms disappeared after one week or one month. Each study reported that anterior chamber bleeding could be attributed to the characteristics of the procedural method, explaining that anterior chamber bleeding is a common symptoms because blood backflow could occur easily due to AIT with high-frequency current creating a gap between the trabecula and the inner wall of Schlemm’s canal. Postoperative increase in IOP (> 10 mmHg) in trabeculectomy control group was reported in one non-RCT, occurring in both the intervention group (3.5%) and the control group (2.9%). Postoperative increase in IOP in iStent control group was reported in two non-RCTs. One study reported incidence of 33% in the intervention group and 16% in the control group, but the difference between the two groups was not significant (p-value=0.07). The other study reported incidence of 2.8% in the intervention group and 0% in the control group. Although a significant difference between the two groups was found, most of the symptoms disappeared by postoperative one-month. Meta-analysis of two non-RCTs showed a significant difference between the two groups (OR 2.52, 95% CI 1.01~6.30, I2=0%). Postoperative endophthalmitis was reported in only one non-RCT with iStent control group, with no incidence of postoperative endophthalmitis in both the intervention and control groups. Glaucoma re-operation in trabeculectomy control group was reported in one RCT and two non-RCTs. One RCT reported glaucoma re-operation rate of 10% in the intervention group and 0% in the control group, but the difference between the two groups was not significant (p-value=0.36). In two non-RCTs, meta-analysis results showed no significant difference between the two groups. Re-operation in iStent control group was reported in three non-RCTs, with no significant difference between the two groups. Re-operation in glaucoma drainage device implantation control group was reported in two non-RCTs and the meta-analysis results showed no significant difference between the two groups. Postoperative visual decline with ≥2 Snellen lines in trabeculectomy control group was reported in one non-RCT with no incidence in both groups. Postoperative visual decline in glaucoma drainage device implantation control group was reported in one non-RCT, with incidence of 0% in the intervention group and 3.1% in the control group. Postoperative choroidal damage (choroidal detachment, effusion) in trabeculectomy control group was reported in one RCT and two non-RCTs. One RCT reported incidence of 0% in the intervention group and 22.2% in the control group. Two non-RCTs reported incidences in only the control group (9% and 3.9% each), but none in the intervention group. The meta-analysis results confirmed a significant difference between the two groups. Postoperative choroidal damage in iStent control group was reported in two non-RCTs, with no incidence in both groups. Postoperative choroidal damage in glaucoma drainage device implantation control group was reported in one non-RCT, with incidence of 0% in the intervention group and 9.3% in the control group. Postoperative persistent ocular hypotony in trabeculectomy control group was reported with three non-RCTs. All three studies incidences in only the control group (17%, 8.3%, and 4.9% each), but none in the intervention group. The meta-analysis results confirmed a significant difference between the two groups (OR 0.06, 95% CI 0.01~0.35, I2=0%). Persistent ocular hypotony in iStent control group was reported in one non-RCT, with no incidence in both groups. Persistent ocular hypotony in glaucoma drainage device implantation control group was reported in one non-RCT, with incidence of 0% in the intervention group and 6.9% in the control group. Postoperative aqueous outflow in trabeculectomy control group was reported in one RCT and two non-RCTs. One RCT reported incidence of aqueous outflow of 0% in the intervention group and 22.2% in the control group. One non-RCT reported no incidence of aqueous outflow in both groups, whereas the other study reported incidence of 0% in the intervention group and 11.8% in the control group. Postoperative malignant glaucoma in trabeculectomy control group was reported in one non-RCT, with incidence of 0% in the intervention group and 4.3% in the control group. Postoperative malignant glaucoma in glaucoma drainage device implantation control group was reported in one non-RCT, with incidence of 0% in the intervention group and 3.4% in the control group. Postoperative shallow anterior chamber in trabeculectomy control group was reported in one non-RCT, with incidence of 0% in the intervention group and 7.8% in the control group.

2. Effectiveness The effectiveness of AIT with high-frequency current was examined by decrease in IOP, change in the amount of anti-glaucoma drug used, and procedure success rate by each control group. Mean postoperative IOP in trabeculectomy control group was reported in one RCT and four non-RCTs. One RCT reported that the mean IOP was ultimately lower in the intervention group than in the control group, but the difference was not significant and both groups showed similar level of decrease in IOP. Meta-analysis of four non-RCTs showed that the final mean postoperative IOP was significantly higher in the intervention group than in the control group (WMD 5.26 mmHg, 95% CI 3.83~6.68, I2=0.0%). Meta-analysis of four non-RCTs with iStent control group showed significantly higher final mean postoperative IOP in the intervention group than in the control group (WMD 2.12 mmHg, 95% CI 1.15~3.09, I2=7.6%). Meta-analysis of two non-RCTs with glaucoma drainage device implantation control group showed no significant difference in final mean postoperative IOP between the two groups. One non-RCT with “other” control group ((both groups receiving Ahmed glaucoma valve implantation but only the intervention group receiving additional AIT with high-frequency current) reported that the final mean postoperative IOP was significantly lower in the intervention group than in the control group. Mean postoperative amount of anti-glaucoma drug used in trabeculectomy control group was reported in one RCT and four non-RCTs. One RCT reported that the mean amount of anti-glaucoma drug used ultimately decreased more in the intervention group than in the control group, but the difference between the two groups was not significant. Meta-analysis on three out of four non-RCTs that could be analyzed showed that the mean postoperative amount of anti-glaucoma drug used was significantly higher in the intervention group than in the control group (WMD 1.59개, 95% CI 1.18~2.01, I2=0.0%). Mean postoperative amount of anti-glaucoma drug used in iStent control group was reported in three non-RCTs and meta-analysis of these studies showed no significant difference between the intervention and control groups. Mean postoperative amount of anti-glaucoma drug used in glaucoma drainage device implantation control group was reported in two non-RCTs and meta-analysis of these studies showed that the mean postoperative amount of anti-glaucoma drug used was significantly lower in the intervention group than in the control group (WMD -1.04개, 95% CI –1.55~-0.53, I2=0.0%). One non-RCT with “other” control group (both groups receiving Ahmed glaucoma valve implantation but only the intervention group receiving additional AIT with high-frequency current) also showed that the mean postoperative amount of anti-glaucoma drug used was significantly lower in the intervention group than in the control group (p-value=0.001). Procedure success rate in trabeculectomy control group was reported in one RCT and three non-RCTs. One RCT reported complete success rate of 20% in the intervention group and 50% in the control group, with no significant difference between the two groups. Qualified success rate was 20% in the intervention group and 37.5% in the control group, with no significant difference between the two groups. Three non-RCTs did not report on complete success rate. Meta-analysis of qualified success showed that qualified success rate was significantly higher in the control group than in the intervention group, but heterogeneity was high (OR 0.55, 95% CI 0.39~0.78, I2=82.5%). Complete and qualified success rates in iStent control group was reported in one non-RCT each. Complete success rate was 18.2% in the intervention group and 9.7% in the control group, but the difference between the two groups was not significant. Qualified success rate was 34.6% in the intervention group and 59.2% in the control group, with a significant difference between the two groups (p-value=0.01). No study reported on completed success rate in glaucoma drainage device implantation control group, while qualified success rate was reported in two non-RCTs. Meta-analysis of these two non-RCTs showed no significant difference between the intervention and control groups. One non-RCT with “other” control group (both groups receiving Ahmed glaucoma valve implantation but only the intervention group receiving additional AIT with high-frequency current) reported complete success rate of 56.2% in the intervention group and 13.5% in the control group and qualified success rate of 70% in the intervention group and 65% in the control group, but the differences between the two groups were not significant.

3. Level of evidence assessment Level of evidence assessment results showed moderate for all indicators in one RCT and low or very low for all indicators by control groups in 10 non-RCTs.

Conclusions The subcommittee on AIT with high-frequency current opined that, with respect to safety, anterior chamber bleeding, which was reported to be a complication associated with the intervention, could be attributed to the characteristics of the procedural method that creates an aqueous outflow pathway by perforating a specific, narrow area and that anterior chamber bleeding and increase in IOP could not be viewed as significant complications or adverse events since most are early complications that heal naturally within few days. Besides this, incidences of endophthalmitis, re-operation, visual decline, choroidal damage, persistent ocular hypotony, aqueous outflow, malignant glaucoma, and shallow anterior chamber showed no significant differences when compared by type of control group (trabeculectomy, iStent, glaucoma drainage device implantation, and other) or incidences were higher in the control group. Accordingly, AIT with high-frequency current was determined to be safe. The subcommittee also opined that the effectiveness of AIT with high-frequency current could be recognized since all studies confirmed decrease in IOP (mean decrease of 29.9%) and amount of anti-glaucoma drug used (mean decrease of 37.2%) after the intervention, as compared to before the intervention. Moreover, because glaucoma is a disease with diverse patient conditions that requires long-term care, this procedure may be considered before opting for other highly invasive procedures. However, because this procedure selectively removes only the trabecula and inner wall of Schlemm’s canal, it would be inappropriate for cases involving closed-angle glaucoma, neovascular glaucoma, and tumor-induced secondary glaucoma that are difficult to approach with the equipment. Except for such cases, this procedure was determined to be appropriate for patients with glaucoma. Accordingly, the subcommittee on AIT with high-frequency current proposed the following based on currently available assessment results. AIT with high-frequency current was assessed to be a technology with evidence of safety and effectiveness for use on patients with glaucoma, except in cases involving closed-angle glaucoma, neovascular glaucoma, and tumor-induced secondary glaucoma that are difficult to approach with the equipment, based on it being less invasive than conventional filtration surgery and having fewer complications, while enabling effective regulation of IOP. The Health Technology Reassessment Committee reviewed and determined that the findings of the subcommittee on AIT with high-frequency current are valid (September 20, 2019).
