각막내 링 삽입술 [원추각막]의 안전성 및 유효성 평가

Abstract

평가배경 각막내 링 삽입술 [원추각막]은 원추각막 환자의 각막변형을 줄이고 시력을 향상시켜 원추각막의 궁극적 치료방법인 각막이식을 지연시키는 시술이다. 해당 기술은 2009년 제1차 신의료기술평가위원회(2009.1.30.)에서 신의료기술평가 대상으로 판단되었으며, 신의료기술평가 결과 안전성 및 유효성의 근거가 있다는 평가를 받았다(권고등급 D). 평가결과는 2009년 8월 7일 보건복지가족부장관에게 보고되었으며, 신의료기술로 인정되어 2010년에 비급여 항목으로 등재되었다.

위원회 운영 총 5인으로 구성된 각막내 링 삽입술 [원추각막] 소위원회는 2019년 4월부터 2019년 9월까지 약 6개월에 걸쳐 총 3회의 소위원회 운영과 1회의 서면자문을 통해 문헌적 근거에 따라 동 시술을 평가하고 검토결과를 제출하였다.

평가 목적 및 방법 각막내 링 삽입술 [원추각막]의 안전성 및 유효성의 근거를 평가하기 위하여 체계적 문헌고찰을 실시하였다. 자세한 연구방법은 아래와 같으며, 모든 평가방법은 연구목적을 고려하여 “각막내 링 삽입술 [원추각막]에 대한 안전성 및 유효성 평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 체계적 문헌고찰은 위의 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림 위험 평가는 RoBANS ver 2.0를 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 자료분석은 정량적 분석(quantitative analysis)이 가능할 경우 양적 분석(메타분석)을 수행하였고 불가능한 경우 정성적(qualitative review) 분석을 적용하였다.

........................................중 략............................................ 평가 결과 안전성 각막내 링 삽입술 [원추각막]의 안전성 결과는 각막염 등 감염과 천공, 각막용해 및 침윤, 수포각막, 링 돌출 및 제거 등의 주합병증과 표면성 각막신생혈관, 터널 침전 및 혼탁, 안구건조, 결막하출혈, 링의 이동 및 양 끝의 겹침 등의 부합병증을 주요 지표로 설정하고 3편의 비교연구, 22편의 전후연구를 근거로 평가하였다. 각막내 링 삽입술과 각막이식술을 비교한 3편의 문헌에서 링 삽입군의 경우 침전, 신생혈관 발생, 상피결손, 링 돌출 등의 합병증을 보고하였고, 각막이식군의 경우 이식거부반응, 신생혈관 발생, 내피세포 감소, 데스메막 박리 등이 발생하였다고 보고하였으나, 두 군간 합병증 발생률 차이의 통계적 유의성을 보고한 연구는 없었다. 17편의 전후연구에서 주합병증 발생을 보고하였고, 합병증의 발생률은 연구마다 상이하며 각막염 등 감염 0.8-5.6%, 천공 0.7-1.4%, 각막용해 0.2-3.3%, 링 돌출 0.6-19.6% 등으로 나타났다. 17편의 전후연구에서 부합병증 발생을 보고하였고, 합병증의 발생률은 표면성 각막신생혈관 1.4-3.0%, 터널 침전 및 혼탁 0.6-31.4%, 링의 이동 1.3-6.0% 등으로 나타났다.

유효성 각막내 링 삽입술 [원추각막]의 유효성 결과는 각막곡률측정치, 원주굴절력, 구면렌즈대응치, 나안시력, 최대교정시력, 환자만족도 및 삶의 질, 링 삽입술 후 콘택트렌즈 착용을 주요 지표로 설정하고 3편의 비교연구, 21편의 전후연구를 근거로 평가하였다. 각막곡률측정치는 3편의 비교연구와 13편의 전후연구에서 보고하였는데, 후향적 코호트연구 2편에 대하여 메타분석을 수행한 결과, Kmax와 Kmin의 수술 전후 평균변화량이 각막이식군에서 링 삽입군보다 유의하게 큰 것으로 나타났다. 전후연구 13편에서는 모두 수술 후의 평균 각막곡률측정치가 수술 전보다 감소하였으며, 이 중 12편에 대하여 메타분석을 수행한 결과 수술 후 2.948D가 유의하게 감소한 것으로 나타났다. 원주굴절력은 2편의 비교연구와 13편의 전후연구에서 보고하였는데, 후향적 코호트연구 2편에 대하여 메타분석을 수행한 결과, 원주굴절력의 수술 전후 평균변화량이 각막이식군에서 링 삽입군보다 유의하게 큰 것으로 나타났다. 전후연구 13편에서는 모두 수술 후의 원주굴절력이 수술 전보다 감소하였으며, 이 중 9편에 대하여 메타분석을 수행한 결과 수술 후 0.988D가 유의하게 감소한 것으로 나타났다. 구면렌즈대응치는 2편의 비교연구와 17편의 전후연구에서 보고하였는데, 후향적 코호트연구 2편에 대하여 메타분석을 수행한 결과, 구면렌즈대응치의 수술 전후 평균변화량이 각막이식군에서 링 삽입군보다 유의하게 큰 것으로 나타났다. 전후연구 17편에서는 모두 수술 후의 구면렌즈대응치가 수술 전보다 감소하였으며, 이 중 13편에 대하여 메타분석을 수행한 결과 수술 후 1.821D가 유의하게 감소한 것으로 나타났다. 나안시력은 2편의 비교연구와 19편의 전후연구에서 보고하였는데, 1편의 후향적 코호트연구에서는 나안시력이 각막이식군에서 링 삽입군보다 유의하게 향상되었다고 보고하였으며, 1편의 비무작위배정 비교임상시험에서는 나안시력의 수술 전후 평균변화량이 각막이식군에서 링 삽입군보다 큰 것으로 나타났으나 통계적 유의성을 보고하지 않았다. 수술 전후의 평균 나안시력의 차이를 보고한 14편의 연구 모두 수술 후에 나안시력이 향상되었으며, 단위를 logMAR로 보고한 7편에 대하여 메타분석 수행 결과 나안시력은 수술 후 0.213logMAR가 유의하게 감소하였고. 소수시력으로 보고한 6편에 대하여 메타분석 수행 결과 수술 후 0.103이 유의하게 증가한 것으로 나타났다. 수술 전후의 나안시력 변화 정도를 비율로 보고한 5편에서는 향상의 비율이 61.8-81.0%로 나타났다. 최대교정시력은 3편의 비교연구와 20편의 전후연구에서 보고하였는데, 후향적 코호트연구 2편에 대하여 메타분석을 수행한 결과, 최대교정시력이 각막이식군에서 링 삽입군보다 유의하게 향상된 것으로 나타났다. 수술 전후의 평균 최대교정시력의 차이를 보고한 15편의 연구 모두 수술 후에 시력이 향상되었으며, 단위를 logMAR로 보고한 7편에 대하여 메타분석 수행 결과 수술 후 최대교정시력은 0.057logMAR가 유의하게 감소하였고. 소수시력으로 보고한 6편에 대하여 메타분석 수행 결과 수술 후 0.104가 유의하게 증가한 것으로 나타났다. 수술 전후의 최대교정시력 변화 정도를 비율로 보고한 5편에서는 향상의 비율이 62-87.1%로 나타났다. 환자만족도 및 삶의 질은 3편의 전후연구에서 보고하였는데 2편에서는 삶의 질 점수 또는 자신의 시력에 만족하는 환자의 비율이 수술 후에 수술 전보다 증가하였다고 보고하였고, 1편에서는 12%의 환자가 시력 불만족으로 링을 제거하였다고 보고하였다. 링 삽입술 후 콘택트렌즈 착용은 2편의 전후연구에서 보고하였는데, 1편에서는 착용을 원치 않는 3명을 제외하고 모두 콘택트렌즈 착용이 가능하게 되었다고 보고하였으며, 1편에서는 45.1%에서 2년 동안 콘택트렌즈를 착용할 수 있었다고 보고하였다.

결론 각막내 링 삽입술 [원추각막] 소위원회는 현재 평가 결과에 근거하여 다음과 같이 제언하였다. 2009년 신의료기술평가 이후 현재까지 후향적 코호트연구 2편과 다수의 전후연구가 증가하였지만, 근거의 내용 면에서 특기할만한 차이가 없으며 근거수준에서의 차이 역시 미미하여, 신의료기술평가 당시의 결론과 크게 상이하지 않다고 판단하였다. 각막내 링 삽입술 [원추각막]은 원추각막 환자를 대상으로 시행한 경우 각막이식술에 비해 비교적 안전하며 유효성은 각막이식술에 비해 다소 떨어지나, 각막이식 대기자인 원추각막 1-3단계에서 각막의 변형을 교정해주고 시력을 향상시켜 원추각막의 궁극적 치료법인 각막이식을 지연시키는 데 있어 안전성 및 유효성의 근거가 있는 기술로 평가하였다. 의료기술재평가위원회는 “각막내 링 삽입술 [원추각막]”에 대해 소위원회 검토결과가 타당하다고 심의하였다(2019.11.08.).

Background Corneal ring implantation [for keratoconus] is used to decrease corneal deformation and improve visual acuity in patients with keratoconus to delay keratoplasty, the final treatment for the condition. During the 1st New Health Technology Assessment Committee meeting (January 30, 2009), the technology was determined as a candidate for a new health technology assessment. Based on the new health technology assessment, it was concluded that the technology was safe and effective (Grade of recommendation D) based on evidence. The assessment conclusion was reported to the Minister of Health and Welfare on August 7, 2009. In 2010, the technology was recognized as a new health technology and listed on the no-coverage list.

Committee Operation Within 6 months spanning April through September 2019, a Corneal Ring Implantation [Keratoconus] subcommittee composed of 5 members held 3 meetings, performed consultations to assess the said technology based on evidence in the literature, and presented the assessment results.

Purposes and Methods To assess the evidence on the safety and effectiveness of corneal ring implantation [keratoconus], a systematic review was performed. The details of the methods used are presented below. The final decisions on all specific methods of assessment were made in accordance with the study purposes and through a review by the Corneal Ring Implantation [Keratoconus] Subcommittee (hereafter, the subcommittee). The systematic review was performed based on the key question described above. A literature search was conducted using 5 domestic and 3 foreign databases. The articles were screened and selected independently by two reviewers according to the selection and exclusion criteria. Two reviewers independently assessed the risk of bias for the selected articles using RoBANS ver 2.0 and reached a consensus; they extracted data independently of each other, using a predetermined data extraction form. If there were disagreements, a consensus was reached by discussing with a 3rd researcher. The data were analyzed using a meta-analysis if it was possible to conduct quantitative analysis and by qualitative review if it was not.

Results Safety To assess the safety of corneal ring implantation [keratoconus], the indices were predetermined as primary and secondary. Primary complications were infection, including infective keratitis, perforation, corneal melting and infiltration, bullous keratopathy, and ring protrusion and removal. The secondary complications were superficial corneal neovascularization, deposits and opacity in the tunnel, dry eye, subconjunctival hemorrhage, and ring dislocation and overlap of distal ends. These indices were examined by reviewing 3 non-randomized studies and 22 pre-post studies. The 3 articles comparing corneal ring implantation and keratoplasty reported the occurrence of complications such as deposits, neovascularization, epithelial defect, and ring protrusion in the ring implant group and transplant rejection, neovascularization, epithelial cell reduction, and Descemet's membrane separation in the keratoplasty group. None of the articles reported on the statistical significance of the inter-group difference. A total of 17 pre-post studies reported the occurrence of primary complications, with the occurrence rates varying across the studies. The occurrence rates of infection (including infective keratitis), perforation, corneal melting, and ring protrusion were 0.8-5.6%, 0.7-1.4%, 0.2-3.3%, and 0.6-19.6%, respectively. Secondary complications were also reported in 17 pre-post studies, and the occurrence rates of superficial corneal neovascularization, deposits and opacity in the tunnel, and ring dislocation were 1.4-3.0%, 0.6-31.4%, and 1.3-6.0%, respectively.

Effectiveness To assess the effectiveness of corneal ring implantation [keratoconus], the following were predetermined as primary indices: keratometry findings, cylindrical refractive error, spherical equivalent, uncorrected visual acuity (UCVA), best-corrected visual acuity (BCVA), patient satisfaction and quality of life, and the use of contact lenses after ring implantation. Effectiveness was assessed by reviewing the data presented in 3 non-randomized studies and 21 pre-post studies. Keratometry was reported in 3 non-randomized studies and 13 pre-post studies. The results of a meta-analysis of 2 retrospective cohort studies showed that the mean pre- and post-surgery changes in Kmax and Kmin were significantly greater in the keratoplasty group than in the ring implant group. In all 13 pre-post studies, the mean keratometry was lower post-surgery compared to pre-surgery. The meta-analysis involved 12 of the articles that showed that corneal curvature was significantly decreased post-surgery (2.948D). The cylindrical refractive error was reported in 2 non-randomized studies and 13 pre-post studies. A meta-analysis of 2 retrospective cohort studies showed that the mean pre- and post-surgical changes in the cylindrical refractive error were significantly greater in the keratoplasty group than in the ring implant group. All 13 pre-post studies reported that the cylindrical refractive error decreased after surgery. The meta-analysis of 9 articles showed that the cylindrical refractive error decreased significantly (0.988D). The spherical equivalent was reported in 2 non-randomized studies and 17 pre-post studies. According to the meta-analysis of 2 retrospective cohort studies, the mean pre- and post-surgical changes in the spherical equivalent were significantly greater in the keratoplasty group than in the ring implant group. In all 17 pre-post studies, the spherical equivalent decreased post-surgically. A meta-analysis of 13 of the studies showed that the spherical equivalent was significantly lower postsurgically (1.821D). UCVA was reported in 2 non-randomized studies and 19 pre-post studies. In 1 retrospective cohort study, UCVA significantly improved in the keratoplasty group compared with the ring implant group. One non-randomized comparative clinical trial reported that the mean pre- and post-surgical changes in UCVA were greater in the keratoplasty group than in the ring implant group, but it did not report statistical significance. Visual acuity improved post-surgically in all of the 14 articles that reported mean pre-post changes in UCVA. A meta-analysis of 7 articles that reported UCVA in logMAR units showed that it was significantly lower post-surgically (0.213logMAR). A meta-analysis of 6 articles that used the Snellen fraction showed that UCVA was significantly higher after surgery (0.103). In 5 articles that reported a % change in UCVA before and after surgery, the improvement was 61.8–81.0%. BCVA was reported in 3 non-randomized studies and 20 pre-port studies. A meta-analysis of 2 retrospective cohort studies showed that the improvement in BCVA was significantly greater in the keratoplasty group than in the ring implant group. In all 15 articles that reported pre and post-surgical differences in the mean BCVA, visual acuity improved after surgery. A meta-analysis of 7 articles that reported BCVA in logMAR units showed that BCVA significantly reduced after surgery (0.057logMAR). A meta-analysis of 6 articles that reported BCVA in Snellen fraction showed that it was significantly higher after surgery (0.104). In 5 articles that reported a % change in BCVA, the range of improvement was 62–87.1%. Patient satisfaction and quality of life were reported in 3 pre-post studies. Two of the articles reported that the quality of life score and the proportion of patients satisfied with their visual acuity increased after surgery. A 3rd article reported that the ring was removed in 12% of patients because they were unsatisfied with visual acuity. The use of contact lenses after ring implantation was reported in 2 pre-post studies. One of the articles reported that all but 3 patients were eligible to wear contact lenses, and the other reported that 45.1% of patients were able to wear contact lenses over 2 years.

Conclusion Based on the results of the assessment, the "Corneal Ring Implantation [Keratoconus]" subcommittee made the following statements. Since the 2009 New Health Technology Assessment, 2 retrospective cohort studies and several pre-post studies have been added to the literature. However, there is no notable difference in the evidence, and the difference in the quality of evidence is negligible. Hence, it was determined that conclusions from the current assessment do not differ greatly from the previous conclusions of the New Health Technology Assessment in the past. Compared to keratoplasty, corneal ring implantation [keratoconus] is relatively safe. Although corneal ring implantation [keratoconus] is somewhat less effective than keratoplasty, the subcommittee assessed that it may be safe and effective to delay the final treatment for keratoconus (i.e., keratoplasty) in patients with keratoconus of stages 1-3 who await keratoplasty by correcting corneal deformation and improving visual acuity. The Health Technology Reassessment Committee determined that the conclusions by the "Corneal Ring Implantation [Keratoconus]" subcommittee are reasonable (November 8, 2019)
