뇌혈관 정량적 자기공명혈관조영술

Abstract

평가배경 한국보건의료연구원은 예비급여 도입 항목의 2020년 의사결정 예정 건 중 기존 신의료기술평가 완료 항목에 대한 재평가 사업을 수행하고 있다. 이중 2011년에 신의료기술평가를 통해 검토되었던 뇌혈관 정량적 자기공명혈관조영술(quantitative magnetic resonance angiography)의 안전성 및 유효성 평가의 업데이트를 수행하고자 한다.

위원회 운영 뇌혈관 정량적 자기공명혈관조영술 소위원회는 2011년 해당안건을 평가하였던 소위원회 위원과 신의료기술평가 전문평가위원회 명단에서 무작위로 선정한 전문가로 구성하였으며, 최종 5인(신경외과 2인, 영상의학과 2인, 신경과 1인)으로 구성되었다. 2019년 의료기술재평가위원회(2019.10.11.)에서는 뇌혈관 정량적 자기공명혈관조영술의 평가결과를 최종 심의하였다.

평가 방법 본 연구에서는 뇌혈관질환 환자에서 뇌혈관 정량적 자기공명혈관조영술의 안전성 및 유효성을 재평가하기 위하여 체계적 문헌고찰을 수행하였다. 모든 평가방법은 연구목적을 고려하여 “뇌혈관 정량적 자기공명혈관조영술에 대한 안전성 및 유효성 평가 소위원회의 심의를 거쳐 확정하였다

평가 결과

안전성 뇌혈관 정량적 자기공명혈관조영술은 MRI 영상을 이용하여 체외에서 비침습적 방식으로 뇌혈관에서의 혈류량과 흐름 방향을 정량화하는 기술로 체외에서 이루어지기 때문에 환자에게 직접적인 위해를 가하지 않아 검사 수행에 따른 안전성에는 문제가 없는 것으로 평가하였다.

유효성 뇌혈관 정량적 자기공명혈관조영술의 유효성은 16편의 문헌을 근거로 진단 정확성, 참고기준와의 일치율, 참고기준와의 상관관계, 임상증상과의 연관성, 의료결과에의 영향에 대한 의료결과로 평가하였다. 뇌혈관 정량적 자기공명혈관조영술의 진단 정확성은 혈관조영술과 비교한 3편의 문헌을 근거로 평가되었다. 두개 내 스텐트 삽입 시술환자 14명을 대상으로 참고기준인 혈관조영술과 중재검사인 정량적 자기공명 혈관조영술의 결과를 비교하였다. 참고기준는 혈관조영술 결과 50% 이상 협착을 보인 경우로 뇌혈관 정량적 자기공명 혈관조영술 결과 혈류량이 15% 이상 감소한 경우와 비교시 민감도 1.00(95% CI 0.21-1.00), 특이도 0.75(95% CI 0.62-0.75), 양성예측도 0.40(95% CI 0.09-0.40), 음성예측도 1.00(95% CI 0.83-1.00), 양성우도비 4.00(95% CI 0.56-4.00), 음성우도비 0.00(95% CI 0.00-1.27), 검사정확도 0.78(95% CI 0.56-0.79)를 보고하였고, 뇌혈관 정량적 자기공명 혈관조영술 결과 혈류량이 20% 이상 감소한 경우와 비교시 민감도 1.00(95% CI 0.22-1.00), 특이도 0.92(95% CI 0.79-0.92), 양성예측도 0.67(95% CI 0.15-0.67), 음성예측도 1.00(95% CI 0.86-1.00), 양성우도비 12.00(95% CI 1.03-12.00), 음성우도비 0.00(95% CI 0.00-0.99), 검사정확도 0.93(95% CI 0.71-0.93)를 보고하였다. 우회로 조성술 시술환자의 뇌졸중 발생 예측에 대한 정확성을 보고하였다. 술전 측정한 정량적 자기공명 혈관조영술 혈류 중 contralateral hemispheric flow 임계값 199 기준으로 AUC는 0.814 (95% CI 0.634~0.994) 이었다(ROC P-value=0.04). 내막절제술 시술환자 뇌과관류증후군(cerebral hyperperfusion syndrome)발생예측에 대한 정확성을 보고하였다. 내경동맥(internal carotid artery, ICA)의 정량적 자기공명 혈관조영술은 임계값 3.49 기준으로 민감도 75%, 특이도 84%, 우도비 4.75 이었으며, 경두개초음파도플러는 임계값 0.26 기준으로 민감도 100%, 특이도 80%, 우도비 5.0이었다. 중대뇌동맥의 정량적 자기공명 혈관조영술은 임계값 1.53기준으로 민감도 100%, 특이도 90%, 우도비 10 이었으며, 경두개초음파도플러는 임계값 1.59 기준으로 민감도 75%, 특이도 80%, 우도비 3.75이었다. 뇌혈관 정량적 자기공명혈관조영술과 혈관조영술의 일치율은 1편의 문헌을 근거로 평가하였다. 뇌혈관 정량적 자기공명혈관조영술 결과 혈류량이 15% 이상 감소를 보인 경우에서는 78.6%의 일치율을, 20% 이상 감소를 보인 경우에서는 92.9%의 일치율을 보고하였다. 뇌혈관 정량적 자기공명혈관조영술과 혈관조영술의 상관계수를 보고한 4편의 문헌을 근거로 평가되었다. 상관정도를 나타내는 피어슨상관계수는 뇌혈관질환환자에서는 –0.42 ~ 0.46이었으며, 2편의 문헌(Brunozzi (2018); Shakur (2018))에서는 유의한 상관관계임을 보고하였고, 우회로 조성술을 시행한 문헌 1편에서 보고한 피어슨상관계수는 0.07로 유의한 상관관계를 보임을 보고하였다(p=0.02). 뇌혈관 정량적 자기공명혈관조영술과 혈관조영술의 결정계수를 보고한 3편의 문헌을 근거로 평가되었다. 우회로 조성술 시행환자에서는 결정계수는 0.51로 유의한 관련성이 있는 것으로 보고하였고(p=0.0001), 색전술 시행환자에서는 Pedicles embolized의 결정계수는 0.64 Intranidal fistula occluded의 결정계수는 0.40으로 두 변수에서만 색전 술 후 뇌동정맥 기형 혈류량 감소와의 유의성을 보고하였으나, 다변량 분석 결과 Pedicles embolized만 유의성이 있는 것으로 분석되었다(p<0.001). 스텐트시술환자 최소혈관직경만 유의성이 있는 것으로 분석되었다(p=0.02). 뇌혈관 정량적 자기공명혈관조영술의 재협착 발생여부 측정 정도는 2편의 문헌(Amin-Hanjani 등, 2010; Prabhakaran 등, 2011)을 근거로 평가하였다. 뇌혈관 정량적 자기공명혈관조영술 결과 25% 이상 혈류량 감소가 확인된 6명에서는 혈관조영술 상 재협착 발생이 확인되었고, 혈류량 감소를 보이지 않은 경우에서는 재협착이 나타나지 않은 것으로 보고하였다. 또한 뇌혈관 정량적 자기공명혈관조영술 결과(Volumetric Flow Rates, VFR(분당혈류량))와 재협착 발생의 상관관계를 연구한 결과 상관계수(r)는 -0.670로 유의한 것으로 보고되었다(p=0.002). 시술 전ㆍ후의 협착정도는 71.0±13.3%에서 15.6±13.3%로 감소함을 보였고, 시술 전ㆍ후의 혈류량은 81.2±59.6mL/min에서 133.3±21.5mL/min로 유의하게 증가함을 보고하였다(p=0.008). 뇌혈관 정량적 자기공명혈관조영술과 혈관조영술의 개방성 평가는 2편의 문헌을 근거로 평가되었다. 2편 모두 혈관의 폐색과 개방의 경우에서 혈류량이 유의한 차이를 보임을 보고하였다(p＜0.05). 뇌혈관 정량적 자기공명혈관조영술의 뇌졸중 발생위험 예측정도는 3편의 문헌을 근거로 평가하였다. 뇌혈관질환환자 대상 2편의 문헌에서는 정상 원위부 혈류 환자의 뇌졸중 발생율은 0~9%이었고, 비정상원위부 혈류 환자의 뇌졸중 발생율은 28~29%이었다. 스텐트 삽입 시술환자 대상 1편의 문헌에서는 50% 이상의 혈류량 감소를 보인 경우 67%에서 뇌졸중이 발생하였고, 혈류변화가 없는 환자에서는 뇌졸중 발생이 0%로 유의한 결과 차이를 보임을 보고하였다(p=0.05).

결론 뇌혈관 정량적 자기공명혈관조영술 소위원회는 현재 평가 결과에 근거하여 다음과 같이 제언하였다. 뇌혈관 정량적 자기공명혈관조영술의 안전성은 MRI 영상을 이용한 비침습적 방식으로 체외에서 이루어지기 때문에 직접적인 위해를 가하지 않아 안전성에는 문제가 없는 것으로 평가하였다. 뇌혈관 정량적 자기공명혈관조영술의 유효성은 참고기준인 혈관조영술과 비교하여 진단정확도 및 예후 예측 정확도가 높고, 통계적으로 유의하게 상관관계가 있는 것으로 판단하였다. 그러나 출판된 문헌 중 50%가 한 기관에서 출판되었고, 평가에 포함된 문헌의 질이 낮은 것으로 평가되었다. 의료기술재평가위원회는 “뇌혈관 정량적 자기공명혈관조영술”에 대해 소위원회 검토결과가 타당하다고 심의하였다(2019.10.11).

Safety and effectiveness assessment of Quantitative Magnetic Resonance Angiography

Background The National Evidence-based Healthcare Collaborating Agency (NECA) is conducting reassessments of items scheduled for decision in 2020 that have already been evaluated for preliminary benefits. Particularly, it is intended to update the safety and efficacy assessment of cerebrovascular quantitative magnetic resonance angiography, reviewed previously in 2011 through the New Health Technology Assessment.

Committee Operations The Subcommittee on Quantitative Magnetic Resonance Angiography comprised members of the subcommittee that evaluated this item in 2011 and experts randomly selected from the Expert Review Committee for the New Health Technology Assessment, with a total of five members (two from neurosurgery, two from radiology, and one from neurology). The 2019 Health Technology Reassessment Committee (October 11, 2019) deliberated the final results of the assessment of quantitative magnetic resonance angiography.

Method This study conducted a systematic review to reevaluate the safety and effectiveness of quantitative magnetic resonance angiography in patients with cerebrovascular disease. All methods were reviewed and finalized by the Subcommittee on the Safety and Efficacy Assessment for “Quantitative Magnetic Resonance Angiography” in light of the study objectives.

Results Safety Quantitative magnetic resonance angiography is a non-invasive imaging technique that utilizes magnetic resonance imaging (MRI) to quantify the volume and direction of blood flow in cerebral vessels. The procedure is conducted outside the body, eliminating the risk of direct harm to the patient and the need for further safety assessments.

Effectiveness The effectiveness of quantitative magnetic resonance angiography was assessed in terms of diagnostic accuracy, agreement with reference criteria, correlation with reference criteria, association with clinical symptoms, and impact on health outcomes based on 16 studies. The diagnostic accuracy of quantitative magnetic resonance angiography was evaluated based on three studies comparing it to angiography. The results of 14 patients undergoing intracranial stenting were compared between the comparator test, angiography, and the interventional test, quantitative magnetic resonance angiography. The reference criteria for the angiography results included a stenosis of 50% or greater, compared with a decrease in blood flow of 15% or greater on cerebrovascular quantitative magnetic resonance angiography, with a sensitivity of 1.00 (95% confidence interval [CI] 0.21–1.00), specificity of 0.75 (95% CI 0.62–0.75), positive predictive value of 0.40 (95% CI 0.09–0.40), negative predictive value of 1.00 (95% CI 0.83–1.00), positive likelihood ratio of 4.00 (95% CI 0.56–4.00), negative likelihood ratio of 0.00 (95% CI 0.00–1.27), and test accuracy of 0.78 (95% CI 0.56–0.79). Compared with quantitative magnetic resonance angiography with a decrease in blood flow of 20% or more, the reference criteria included a sensitivity of 1.00 (95% CI 0.22–1.00), specificity of 0.92 (95% CI 0.79–0.92), positive predictive value of 0.67 (95% CI 0.15–0.67), negative predictive value of 1.00 (95% CI 0.86–1.00), positive likelihood ratio of 12.00 (95% CI 1.03–12.00), negative likelihood ratio of 0.00 (95% CI 0.00–0.99), and test accuracy of 0.93 (95% CI 0.71–0.93). The accuracy of predicting stroke in patients undergoing bypass grafting was reported. The area under the curve was 0.814 (95% CI 0.634–0.994) (receiver operating characteristic curve P-value=0.04) according to a contralateral hemispheric flow threshold of 199, as measured preoperatively using quantitative magnetic resonance angiography. In patients undergoing endarterectomy, the accuracy of predicting the development of cerebral hyperperfusion syndrome was reported. Quantitative magnetic resonance angiography of the internal carotid artery had a sensitivity of 75%, sp
