F-18 FLT 양전자단층촬영의 안전성 및 유효성 평가

Abstract

평가배경 F-18 FLT 양전자단층촬영은 영상을 통해 비침습적으로 세포증식도를 평가하는 핵의학영상진단법으로 종양의 감별진단 및 치료 효과의 평가 등에 사용된다. 해당 의료기술은 국내에서 2012년 신의료기술로 인정된 후, 2015년 보건복지부 고시에 따라 비급여(도-225)로 등재되어 현재까지 사용되고 있다. 본 연구에서는 신의료기술평가 이후 현재 시점에서 해당 의료기술의 안전성 및 유효성을 확인하기 위한 의료기술재평가를 수행하고자 한다.

위원회 운영 해당 의료기술은 「C-11 아세트산 양전자단층촬영」 의료기술과 함께 소위원회를 공동 운영하였다. 두 의료기술의 기존 신의료기술평가에 참여한 위원을 우선적으로 섭외하였고, 참여가 어려우신 경우 신의료기술평가사업본부 분야별 전문평가위원회 pool을 대상으로 무작위 추출하여 총 7인의 소위원회를 구성하였다. 소위원회는 총 3회에 걸쳐 운영되었다.

평가 목적 및 방법 본 연구는 폐암 환자에서 사용되는 F-18 FLT 양전자 단층촬영 검사의 임상적 안전성 및 유효성을 평가하고자 체계적 문헌고찰을 수행하였다. 해당 의료기술평가는 건강보험심사평가원에서 명시하고 있는 고시항목(도-225)에 준하여 행위를 정의하고, 기존 신의료기술평가시 사용된 검색전략을 준용하였다.

체계적 문헌고찰은 위의 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림 위험 평가는 QUADAS-2를 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하고 의견 불일치가 있을 경우 제 3자와 함께 논의하여 합의하였다. 자료분석은 정량적 분석(quantitative analysis)이 가능할 경우 메타분석을 수행하고 불가능할 경우 정성적 검토(qualitative review)를 적용하였다.

평가 결과 체계적 문헌고찰을 수행한 결과, F-18 FLT PET 또는 PET/CT와 관련된 총 21편의 문헌을 확인하였고, 해당 문헌을 바탕으로 안전성과 유효성을 평가하였다. 1편의 문헌에서 F-18 FLT PET/CT 검사와 관련된 부작용은 발생하지 않은 것으로 보고하고 있어 안전성에는 문제가 없는 것으로 확인되었다. 해당 검사의 유효성은 전이 및 병기판정 진단정확도, 세포증식도 평가(비교검사와의 상관성), 치료반응 평가, 의료결과에의 영향으로 평가하였다. 전이 및 병기판정 진단정확도는 총 3편의 연구에서 F-18 FLT PET와 F-18 FDG PET을 비교하고 있었으며, 모든 연구에서 F-18 FLT PET의 전이 및 병기판정 진단정확도가 더 좋은 것으로 보고하고 있었다. 그러나 Yamamoto 등 (2008)의 연구에서 통계적 유의성은 없었다. 세포증식도 평가에서는 해당 검사와 Ki-67(6편) 및 cyclin D1(1편)의 세포증식도 평가에 대한 상관성을 비교한 문헌이 총 7편 확인되었다. F-18 FLT PET(또는 PET/CT)은 세포증식도 평가에 있어 비교검사와 중등도(moderate)의 유의한 상관관계를 갖고 있는 것으로 나타났다(r=0.555, 95% CI 0.048, 0647). 치료반응 평가는 무진행생존율, 전체 생존율 및 표준화섭취계수 결과를 중심으로 확인하였다. 무진행생존율에 대하여 총 7편의 문헌에서 보고되었다. 항암제 치료 후, F-18 FLT PET(또는 PET/CT) 반응군과 비반응군의 무진행생존율을 비교한 6편의 연구 중 5편의 연구에서 반응군과 비반응군간 유의한 차이를 나타내고 있었다. 특히, 초기(1주) 시점의 결과가 무진행생존율을 유의하게 예측하는 것으로 보였다. 그러나 1편의 연구(Bhoil 등, 2014)에서는 유의한 차이가 없는 것으로 나타나 무진행생존율 예측에 대한 엇갈린 결과가 제시되었다. 화학방사선요법 치료 후, F-18 FLT PET(또는 PET/CT) 반응군과 비반응군의 무진행생존율을 비교한 1편의 연구(Everitt 등, 2017)에서는 비반응군의 무진행생존율이 더 좋은 것으로 나타났으나 통계적으로 유의한 결과는 아니었다. 전체 생존율에 대하여 총 8편의 문헌에서 보고되었다. 항암제 치료 후, F-18 FLT PET(또는 PET/CT) 반응군과 비반응군의 전체 생존율을 비교한 5편의 연구 모두 두 군간 유의한 차이가 없어 F-18 FLT PET(또는 PET/CT)이 전체 생존율을 예측하기는 어려운 것으로 나타났다. 화학방사선요법 치료 후, F-18 FLT PET(또는 PET/CT) 반응군과 비반응군의 무진행생존율을 비교한 2편의 연구에서도 두 군간 유의한 차이가 없는 것으로 나타났다. 1편의 연구(Scheffler 등, 2013)에서는 치료 전, F-18 FLT PET(또는 PET/CT) 반응군과비반응군의 전체 생존율을 확인하였고 F-18 FLT PET 및 F-18 FDG PET의 비반응군이 반응군보다 통계적으로 유의하게 전체 생존율이 좋은 것으로 나타났다. 8편의 연구에서 F-18 FLT PET(또는 PET/CT)의 표준화섭취계수를 보고하고 있었으며, 치료 전 후의 F-18 FLT PET/CT의 표준화섭취계수를 보고하고 있는 3편의 연구에서 치료 후에 F-18 FLT PET/CT의 표준화섭취계수가 감소하고 있음을 확인할 수 있었다.해당 검사의 수행에 따라 환자 관리 변화와 같은 치료결과의 영향을 보고한 연구는 없었다.

결론 F-18 FLT 양전자단층촬영 소위원회는 현재 평가 결과에 근거하여 다음과 같이 제언하였다. F-18 FLT 양전자단층촬영은 안전성에는 문제가 없는 것으로 확인되었으나, 유효성에 있어서는 일부 결과에 있어서만 유효한 것으로 확인되었다. 현재까지 근거는 진단정확도(전이, 병기판정) 및 세포증식도 평가에 있어 명확한 임상적 유효성이 확인되었으나, 치료반응 평가를 통한 예후/예측에 유효할 것이라는 근거는 확인되지 못한 것으로 볼 수 있다. 그러나 소위원회는 이러한 결과가 현재 보편적으로 사용되고 있는 F-18 FDG 양전자단층촬영보다 월등하게 낮은 유효성이라고 볼 수 없으며, 이와 유사한 수준의 치료반응 평가의 유효성을 가진 것으로 보았다. 이에 F-18 FLT 양전자단층촬영 검사는 폐암 환자에서 항암제 또는 방사선 치료 후 치료효과 판정 및 예후 예측을 목적으로 사용하기에 안전하고 유효한 검사법이라고 판단하였다. 의료기술재평가위원회는 “F-18 FLT 양전자단층촬영”에 대해 소위원회 검토결과가 타당하다고 심의하였다(2019.10.11.).

Assessment background F-18 FLT positron emission tomography (PET) is a nuclear medicine imaging diagnosis method for non-invasively assessing cell proliferation based on images and it is used to assess the treatment effect and differential diagnosis of tumor. This health technology was recognized as a new health technology in South Korea in 2012. Subsequently, it was registered as a non-coverage item (do-225) by the decree of the Ministry of Health and Welfare in 2015 and is still used today. The present study aimed to perform health technology reassessment to confirm the safety and effectiveness of this health technology at the present time effectiveness after the initial health technology assessment.

Subcommittee operation The subcommittee for this health technology was jointly operated with C-11 acetate PET health technology. The subcommittee consisted of a total of seven members. The members were preferentially invited among those who participated in the previous new health technology assessments for both health technologies, and if it was difficult for them to participate, members were randomly selected from a pool of professional assessment committee members for various fields within the new health assessment project headquarters. A total of three subcommittee sessions were held.

Assessment objectives and methods A systematic review was performed to reassess the clinical safety and effectiveness of F-18 FLT PET used on patients with lung cancer. For the assessment, the technology was defined in accordance with the notification (do-225) stipulated by the Health Insurance Review and Assessment Service and search strategy used in the previous new health technology assessment was followed.

For systematic review, five Korean and three foreign databases were searched based on the key question. Two reviewers independently screened and selected the articles according to the selection and exclusion criteria. Risk of bias assessment was performed independently by two reviewers using QUADAS-2 until an agreement was reached. Data were extracted independently by two reviewers using pre-determined format. If there was a disagreement between the reviewers, such cases were discussed with a third party to reach an agreement. Meta-analysis was performed when quantitative analysis was possible and qualitative review was applied when otherwise.

Assessment results A total of 21 articles related to F-18 FLT PET or PET/CT were identified through the systematic review and these articles were used for the safety and effectiveness assessment. One article reported that there was no incidence of adverse events associated with F-18 FLT PET/CT. Accordingly, it was determined that there are no safety concerns. The effectiveness of this test was assessed by the diagnostic accuracy for metastasis, accuracy of disease staging, cell proliferation (correlation with comparator test), treatment response, and impact on health outcomes. With respect to diagnostic accuracy for metastasis and accuracy of disease staging, three articles compared F-18 FLT PET and F-18 FDG PET and all three articles reported that F-18 FLT PET showed higher accuracies. However, the study by Yamamoto et al. (2008) reported that there was no statistical significance. With respect to cell proliferation assessment, there was a total of seven articles that compared the correlation between the index test and comparator tests (Ki-67 in six articles and cyclin D1 in one article). The results showed that F-18 FLT PET (or PET/CT) has a moderately significant correlation with comparator tests in the cell proliferation assessment (r=0.555, 95% CI 0.048, 0647). Treatment response was assessed by progression-free survival (PFS), overall survival (OS), and standardized uptake value (SUV). A total of seven articles reported on PFS. Five out of six studies that compared PFS between F-18 FLT PET (or PET/CT) response and non-response groups after chemotherapy reported that there were significant differences between the two groups. In particular, early outcomes (first week) were found to be significant predictors of PFS. However, one study (Bhoil et al., 2014) reported no significant difference, presenting conflicting results on prediction of PFS. One study (Everitt et al., 2017) that compared PFS between F-18 FLT PET (or PET/CT) response and non-response groups after chemoradiotherapy reported that the non-response group showed better PFS, but the difference was not statistically significant. A total of eight articles reported on OS. All five studies that compared OS between F-18 FLT PET (or PET/CT) response and non-response groups after chemotherapy reported no significant differences between the two groups, and thus, predicting OS with F-18 FLT PET (or PET/CT) was found to be difficult. Two studies that compared OS between F-18 FLT PET (or PET/CT) response and non-response groups after chemoradiotherapy also reported no significant differences between the two groups. One study (Scheffler et al., 2013) compared OS between F-18 FLT PET (or PET/CT) response and non-response groups before treatment and reported that F-18 FLT PET and F-18 FDG PET non-response group had statistically significantly better OS than the response group. Eight studies reported on SUV of F-18 FLT PET (or PET/CT). Three studies that reported on SUV of F-18 FLT PET/CT at before and after treatment reported that SUV of F-18 FLT PET/CT decreased after the treatment. There were no studies that reported any impact on treatment outcomes, such as changes in patient care by performing this test.

Conclusions The F-18 FLT PET subcommittee proposed the following based on the currently available assessment results. While F-18 FLT PET was found to have no safety concerns, its effectiveness was confirmed only in some results. Evidence currently available has confirmed clear clinical effectiveness for diagnostic accuracy (metastasis and disease staging) and cell proliferation assessment, whereas, evidence of its effectiveness for prognosis/prediction through treatment response assessment has not been found. However, the subcommittee determined that such results did not demonstrate much inferior effectiveness than F-18 FDG PET, which is universally used today, and that the effectiveness of treatment response assessment was at a similar level. Accordingly, it was determined that F-18 FLT PET is a safe and effective test method for determining treatment effect and predicting prognosis after chemotherapy or radiotherapy in patients in lung cancer. The Health Technology Reassessment Committee reviewed and determined that the findings of the subcommittee on F-18 FLT PET are valid (October 11, 2019).
