제1형 당뇨병에서 연속혈당측정기 및 인슐린펌프 사용의 안전성, 유효성, 비용효과성 체계적 문헌고찰

Abstract

1. 연속혈당측정기

평가목적 연속혈당측정기는 메드트로닉사(Medtronic, Minneapolis, MN, USA)에서 처음 개발 되어 1999년 6월 미국 FDA 승인을 받았으며, 혈당 변동 폭이 크고, 저혈당이 빈번한 당뇨병 환자들의 치료에 도움을 주고 있다. 연속혈당측정기는 센서를 활용하여 체내 혈당 변화량을 실시간으로 측정하는 것으로 효율적인 당뇨관리를 목적으로 한다. 해당 기기는 연속적인 혈당 검사에 활용한다. 제1형 당뇨병환자가 의사(내과·소아청소년과·가정의학과 전문의)가 발급한 제1형 당뇨병 소모성 재료 처방전(처방기간 90일 이내)에 의하여 자가혈당측정에 사용되는 소모성 재료를 국민건강보험공단에 등록한 의료기기판매업소에서 구입 및 사용한 경우에 건강보험에서 지원을 받을 수 있다. 따라서 연속혈당측정기 사용의 안전성, 유효성. 비용효과성에 대한 연구결과를 토대로 개선된 임상적 유효성 및 경제적 가치를 확인할 수 있는 객관적 근거자료 마련을 위해 국민건강보험공단은 한국보건의료연구원에 체계적 문헌고찰 수행을 의뢰하였다. 이에 한국보건의료연구원은 본 연구를 통해 “제1형 당뇨병 환자가 연속혈당측정기를 활용하여 혈당관리를 하는 것은 자가혈당측정방법과 비교하여 안전성 및 유효성, 비용효과성이 있는가?’를 핵심질문으로 선정하고 연구를 수행하였다.

평가방법 분석 평가에 최종 선택된 문헌 연속혈당측정기의 안전성, 유효성, 비용효과성에 대한 검토는 질적 체계적 문헌고찰(systematic review, SR)을 통해 평가하였다. 안전성, 유효성(안·유) 분석은 SR로 국한하여 검토한 결과 총 11편의 문헌이 선택되었다. 비용효과성 분성용은 문헌의 연구 유형에 제한을 두지 않았고, 선택된 문헌은 4편이었다.

평가결과 안전성 안전성 분석 결과, 선택된 문헌에서 보고된 연속혈당측정기로 인한 합병증과 부작용 결과는 선행연구(주예일 등., 2018)와 동일하게, 연속혈당측정기로 인한 부작용으로는 ‘중등도의 질’로 평가된 2편의 연구에서 센서 부착부위의 피부 농양, 피부자극 등을 보고 하고 있었다. 이에 본 연구에서는 연속혈당측정기로 인한 부작용은 보고된 사례가 드물고 관련된 이상반응이 미미한 것으로 보고 피부관련 부작용이 발생할 수 있으나 사용에는 큰 문제가 없는 것으로 평가하였다.

유효성 유효성 결과는 연속혈당측정기의 유효성은 선행연구(주예일 등, 2018)에서 사용한 지표에서 당화혈색소(HbA1c) 변화량, 고혈당 발생정도, 저혈당 발생정도, 삶의 질 등의 지표를 활용하여 평가하였다. 제1형 당뇨의 소아청소년 대상으로 진행된 주예일 등. (2018)의 연구에서는 ‘높은 질’로 평가된 6편의 문헌 모두에서 연속혈당측정으로 인 한 당화혈색소(HbA1c) 변화량은 자가혈당측정에 비교하여 통계적으로 유의한 감소효과 를 보고하고 있었고, ‘중등도의 질’로 평가된 5편의 연구에서 대부분 통계적으로 유의하지는 않으나 감소효과가 있는 것으로 보고한 바, 연속혈당측정법이 소아 제1형 당뇨에서 자가혈당측정법에 비교하여 동등이상의 당화혈색소(HbA1c) 감소효과를 보고하고 있으므로 유용한 것으로 평가하였다. 이에 본 연구에서는 제1형 당뇨 소아청소년과 성인에서 연속혈당측정법이 자가혈당측정법에 비교하여 동등이상의 당화혈색소(HbA1c) 감소효과 보고하고 있음에 따라 유용한 것으로 평가하였다. 저혈당 발생정도는 발생빈도와 발생시간, 발생위험비 등으로 비교되었다. 우리 연구에 서도 선행연구(주예일 등., 2018)와 같이 소아청소년 및 성인에서 연속혈당측정으로 인한 저혈당 발생정도를 감소시킬 수 있다는 결론을 도출하기에는 근거가 부족한 것으로 평가하였다. 연속혈당측정기 사용에 따른 삶의 질을 비교하기 위해 정상 ‘높은 질’의 연구 2편에 포함된 5개 일차문헌의 연구 결과를 정리하였다. ‘높은 질’로 평가된 Langendam et al. (2012)의 연구에서는 유의한 차이를 보이는 두 연구가 모두 연구 시작 전에 인슐린 펌프 를 사용하지 않아 이후 인슐린 펌프와 CGM을 함께 사용하였기 때문에 환자 만족도를 상승시키는 요인으로 해석하였다. 이에, 우리 연구도 선행연구(주예일 등., 2018)와 같이 소아청소년과 성인에서 연속혈당측정이 자가혈당측정을 수행하는 것보다 삶의 질을 향상 시킬 수 있는 결론을 도출하기에는 근거가 부족한 것으로 평가하였다. 단, 최근에 평가한 제1형 당뇨병 성인에서의 삶의 질을 평가한 DIAMOND 연구(2017년)에서는 당뇨병에 대한 저혈당 관련 신뢰도가 유의하게 증가하고 스트레스 감소와 웰빙, 건강상태, 저혈당 공포 등의 개선. 특히, 수면이나 운전 중 심각한 저혈당 문제로부터 안전에 도움을 준다 고 한 바, 추가 연구를 통해 재평가될 필요가 있겠다.

비용효과성 비용효과성 분석에 최종 선택된 4편의 문헌 중 2개 문헌은 미국에서 수행한 논문이었고 1개 문헌은 캐나다, 1개 문헌은 스페인에서 수행한 문헌이었다. 분석관점은 3개는 사 회적 관점이었고 1개의 문헌만 지불자 관점이었다. 사회적 관점으로 수행된 연구 3개 문헌 (McQueen et al., 2011; Chaugule et al., 2017; Wan et al., 2018)은 연속혈당측정기는 비용효과적 대안인 것으로, 지불자 관점에서 수행된 1개 연구(Garcia-Lorenzo et al., 2018) 에서만 비용효과적 대안이 아닌 것으로 평가되었다. 비용효과적이라고 평가한 3개 문헌의 ICER $45,033(USD), $33,789(CAD), $33,459(USD)였고, 비용효과적이지 않은 1개 문헌의 ICER값은 €880,229/QALY 이었다. 비용효과적이지 않다고 보고한 스페인 연구 (Garcia-Lorenzo et al., 2018)는 사회적 관점으로 효용계산에서 당뇨 외 질환의 효용값을 distuility 값으로 사용하였고, Jesus (2019)에 의해 제안된 몇 가지 논점(최근 문헌 미포함, 저혈당 및 케톤산증 등에 의한 응급실 방문 또는 입원 비용 미고려, 최근 기기 사용을 포함하지 않음 등)등에 근거하여 ICER 값이 낮게 측정되었고 이를 고려한다면 비용효과적 대안으로 결정될 수 있을 것이라 제안한 바, 우리나라에서 비용효과성을 판단 할 때 한국적 상황을 고려한 정책적 의사결정이 필요할 것으로 사료된다.

결론 및 제언 본 연구에서 효과성에 포함된 총 11편의 연구들을 종합적으로 판단하여 볼 때, 선행연구(주예일 등. 2018)의 연구결과와 같이 1차 당뇨 성인에서의 연속혈당측정은 자가혈당 측정을 수행하는 것과 동등 이상의 임상적 안전성과 유효성을 가진 것으로 볼 수 있다. 다만, 지속적인 측정을 위해 연속혈당측정기의 센서를 피부에 부착함으로서 발생할 수 있는 피부 부작용에 대하여 고려해야하며, 유효성과 관련하여 당화혈색소(HbA1c) 감소 효과는 0.1%~0.5% 정도 있다고 할 수 있으나, 저혈당 및 고혈당에 대한 발생정도를 줄 여주는 효과, 정상혈당 유지 정도와 치료결과에의 영향, 삶의 질 등은 현존하는 근거를 기반으로 판단하기 어려운 실정인 것으로 결론 낼 수 있겠다. 추가적으로 몇 몇 논문에서는 실시간 연속혈당측정기의 추가적 이득(저혈당 에피소드 감소, 라이프 스타일 자가 모니터링 가능, 약리학적 개입과 라이프 스타일 개입 둘 다를 위한 지원 도구로 활용 가능성)을 설명하고 있다.(Wojciechowski et al., 2011; Poolsup et al, 2013) 이는 센서 사용량과 혈당관리의 연관성, 소아청소년에서 부모 협력이 있을 경우 혈당관리의 유용성 등이 선행문헌들에서 제시되고 있는 바, 센서 사용을 위한 교육과 부모와 함께 자녀가 주의 깊게 교육을 받고 수행할 경우 제1형 소아청소년 당뇨에 보다 더 혈당 조절에 유용한 도구가 될 수 있을 것이라고 할 수 있겠다. 최근 논문들에서 저혈당의 효과와 삶의 질 등의 개선, 센서 사용량에 따른 효과 등과 함께 환자 생활습관 중재에 있어서 편익이 있음을 보고하고 있는 바, 정책적으로 환자 중심의 편익을 고려한 의사결정을 수행할 필요가 있을 것이다. 현재 연속혈당측정기 기술이 발전하고 있음으로 간헐적 판독 방법이 가능한 기계를 사용함으로서 센서 비용 감소와 장치 착용 불편함 감소가 가능한 형태를 고려하는 환자의 편익을 극대화하는 방향 으로 정책적 의사결정이 필요할 것으로 사료된다. 연구의 제한점으로는 정책결정을 위해 짧은 연구기한에 맞추어 수행한 review of systematic reviews이기 때문에 다음과 같은 한계점이 존재하여 해석에 주의를 필요로 한다. 첫 번째는 선행연구(주예일 등., 2018)의 연구와 같이, 연구유형을 체계적 문헌고찰로 제한했기 때문에 1) 각 체계적 문헌고찰에서 선택된 연구들이 서로 1회 이상 중첩 되어 있다는 점, 2) 중첩된 연구들이 가지는 통계적 효과가 과대 해석될 수 있으므로 해석에 유의해야 한다는 점, 3) 포함된 일차 연구들에 대한 결과 결합과정을 별도로 수행 하여야 한다는 점 등을 주요 한계점으로 볼 수 있다. 그러나, 포함된 일차 연구들의 결과 합성은 논문 간 큰 이질적 특성으로 현재 연구에 포함된 일차문헌으로만 결론을 내리기 어려웠다. 그러므로 향후 고혈당, 저혈당(야간 저혈당 포함)을 비롯한 삶의 질, 비용효과성 분석 등을 향후 수행된 일차문헌 등을 토대로 근거 마련 연구가 수행될 필요가 있겠다.

2. 인슐린 펌프

평가목적 인슐린펌프(continuous subcutaneous insulin infusion, CSII)는 인슐린을 24시간 지속적으로 투여하기 위해 개발된 의료기기로 인체에서 분비되는 인슐린과 거의 흡사한 혈중농도를 유지하여 혈당을 보다 더 정상에 가깝게 조절할 수 있게 도와준다. 국민건강보험공단에서는 소아당뇨병 어린이 보호대책 마련과 관련하여 사용이 편리한 혈당관리 의료기기에 대한 건강보험 지원 확대를 검토 중에 있으며, 이에 보장성 강화 요구도 및 시급성 등을 고려하여, 연속혈당측정기에 필요한 소모성재료(센서)에 대해 우선적으로 2019년 1월1일부터 건강보험을 적용하였다(국무조정실, 2017). 하지만, 고가 이며 건강보험에서 지원되지 않는 연속혈당측정기, 인슐린펌프에 대해서는 관련 검토를 통해 급여화 방안 마련에 있다. 따라서 인슐린펌프 사용의 안전성, 유효성 및 비용효과성에 대한 연구결과를 토대로 개선된 임상적 유효성 및 경제적 가치를 확인할 수 있는 객관적 근거자료 마련을 위해 국민건강보험공단은 한국보건의료연구원에 체계적 문헌고찰 수행을 의뢰하였다. 이에 한국보건의료연구원은 본 연구를 통해 제1형 당뇨병 환자에게 있어 기존 인슐린 다회주사법(Multiple daily insulin injection, MDI) 대비 인슐린펌프(CSII) 사용 비교 를 통해 인슐린펌프 사용의 안전성, 유효성 및 비용효과성에 대해 평가하고자 한다.

평가방법 분석 평가에 최종 선택된 문헌 인슐린펌프의 안전성, 유효성, 비용효과성에 대한 검토는 체계적 문헌고찰(systematic review, SR)을 통해 평가하였다. 안전성, 유효성 분석용은 문헌의 연구유형을 체계적 문 헌고찰(SR)로 국한하여 검토한 결과 총 17편의 문헌이 선택되었다. 비용효과성 분석용은 문헌의 연구 유형에 제한을 두지 않았고, 선택된 문헌은 총 17편으로, SR(4편), 무작위 배정 임상연구(4편), 파일럿 연구(1편), 시뮬레이션 또는 가설연구와 같은 기타연구(8편) 로 확인되었다. 총 17편의 연구 중 HTA 보고서는 5편으로 확인되었다.

연구결과 지표 인슐린펌프 사용에 대한 안전성 결과는 심한 저혈당, 야간 저혈당, 당뇨병케톤산증을 주요 결과지표로 평가하였다. 또한 2차적으로 심하지 않은(non-severe), 경증/중등도의 (mild/moderate) 저혈당, 고혈당, 사망과 같은 중대한 이상반응을 포함하여 인슐린펌프 관련 이상반응(인슐린 주사부위 문제, 인슐린펌프 기기 관련 문제 등)도 문헌에 보고한 내용이 있으면 포함하여 제시하였다. 유효성 결과는 당화혈색소(HbA1c) 수치 변화를 주요 결과지표로 보았으며, 이 외에 혈당수치(blood glucose level), 식전 또는 식후 혈당수치 결과도 문헌에서 보고한 내용은 자료추출 결과에 제시하였다. 또한 연구대상자별로 세 그룹(소아/청소년, 성인, 임신부)으로 나누어 결과를 제시하였다. 비용효과성 결과는 경제성지표인 ICER, QALY, DALY 등을 주요 결과지표로 분석하였다. 비용효과성 분석에 최종 선택된 17편의 문헌 중 1편(Colquitt et al., 2004)은 비용효과성 연구결과를 제시하지 않아 결과분석에는 포함하지 못했다.

문헌선택 개요 및 분석 본 체계적 문헌고찰은 국외 데이터베이스 3개(Ovid-MEDLINE, Ovid-EMBASE, Cochrane Library), 국내데이터베이스 5개(코리아메드, RSISS, KISS, NDSL, KMBASE)를 이용하여 수행하였다. 안전성, 유효성, 비용효과성 분석은 담당 및 부담당 연구원 2명이 독립적으로 수행하였으며, 이견이 있을 경우 논의 및 원문 확인, 소위원회 위원의 검토를 통해 결정하였다. 국내외 데이터베이스를 통해 검색된 안전성, 유효성 분석용 관련문헌은 총 456편(국내 143편, 국외 313편)이었으며, 이 중 중복 검색문헌(국내 65편, 국외 54편)을 제외한(국내 78편, 국외 259편)을 대상으로 제목, 초록 및 원문을 검토하였다. 선택/배제 기준에 따라 최종적으로 배제된 문헌은 총 320편(국내 78편, 국외242편)이었다. 따라서 안전성· 유효성 분석에 최종 선택된 문헌은 총 17편(국내 0편, 국외 17편)이었다. 선택된 문헌의 목록은 부록에 별도로 제시하였다. 국내외 데이터베이스를 통해 검색된 비용효과성 분석용 관련문헌은 총 1,865편(국내 0편, 국외 1,865편)이었으며, 이 중 중복 검색문헌(314편)을 제외한 1,551편을 대상으로 제목, 초록 및 원문을 검토하였다. 선택/배제 기준에 따라 최종적으로 배제된 문헌은 총 1,534편이었고, 비용효과성 분석에 최종 선택된 문헌은 총 17편 이었다.

평가결과 국내외 데이터베이스를 통해 검색한 결과 본 연구 설계에 부합하여 CSII와 MDI의 안 전성, 유효성 및 비용효과성을 비교한 국내문헌은 검색되지 않았다.

안전성 결과 심한 저혈당을 주요 결과지표로 제시한 문헌은 분석에 포함된 총 17편 중 16편 이었으며, 야간 저혈당에 대해 보고한 문헌은 2편이였다. 심한 저혈당 및 야간 저혈당을 포함한 저혈당 발생과 관련해서는 소아/청소년, 성인 모두에서 CSII와 MDI 군 간 유의한 차이가 없음으로 평가되었다. 임신부에 대한 심한 저혈당 및 야간 저혈당을 포함한 저혈당 발생관련 결과는 단 2편에서 제시한 결과만을 가지고는 두 군간 유의성을 판단하기 어려웠지만 유의한 차이는 없는 것으로 보였다.

당뇨병케톤산증(Diabetic ketoacidosis, DKA)에 대해 보고한 문헌은 임신부만을 대상으로 한 연구 2편을 포함하여 총 6편 이였다. 6편에 대한 분석 결과 DKA 발생은 소아/청소년, 성인, 임신부 모두에서 CSII와 MDI 두 군간 유의한 차이는 없음으로 평가되었다.

이 외에 심하지 않은(non-severe) 저혈당, 경증/중등도(mild/moderate)의 저혈당에 대해 보고한 문헌은 총 4편이였고, MDI 대비 CSII 군에서 유의한 차이가 없다고 보고하였다.

고혈당(hyperglycemia)에 대한 결과를 제시한 문헌은 단 1편(Yeh et al., 2012)으로 소아/청소년 및 성인을 대상으로 중재법과 비교법을 세 군(CSII vs. MDI; CGM vs. SMBG; sensor-augmented pump, SAP vs. MDI+SMBG)으로 나누어 수행한 SR 연 구이다. 이 문헌에서 포함한 두 개의 연구(Bergenstal et al., 2010; Hermanides et al., 2011) 결과에서 고혈당 지속시간이 MDI 대비 SAP에서 유의하게 짧았다고 제시하였다(p < 0.001).

인슐린 주사부위 문제 또는 기타 이상반응 등 인슐린펌프 사용관련 이상반응을 보고한 문헌은 1편(Jeitler et al., 2008)이었으며, 이 연구에 포함한 23편의 RCT 문헌 중 CSII 군 환자 사망을 보고한 문헌은 1편(Nosadini et al., 1988)이었으며 이 사망환자는 사망 2일 전부터 고혈당, 발열 및 케톤뇨가 있었으며 인슐린 주입량을 증가했음에도 불구 하고 당수치의 변화가 없었다고 보고하였다. 인슐린 주입부위 관련문제(infusion site problem)를 보고한 문헌은 2편(Hoogma et al., 2006; Schottenfeld-Naor et al., 1985)이였다. Schottenfeld-Naor et al. (1985)의 연구에서 제시한 주입부위 관련 문제의 세부사항을 보면 인슐린주입 카테터 문제(catheter problems), 펌프기기 성능관련 문제(pump arrest or dosage errors), 펌프 배터리 문제(battery problems), 인슐린 주사기 위치 어긋남(syringe dislocation) 등이었다. 이외에 기타 이상반응을 보고한 문 헌은 2편(Hoogma et al., 2006; Ziegler et al., 1990)이었고 이상반응에 대한 세부사 항은 제시되지 않았다.

유효성 결과 당화혈색소(HbA1c) 변화량은 17편 모두에서 주요 결과지표로 보고하였으며, 총 13편 의 문헌에서 CSII(중재법) 치료는 MDI(비교법) 대비 당화혈색소(HbA1c) 수치의 유의한 감소를 보였다 보고하였다. 13편의 결과 분석을 토대로 소아/청소년, 성인에서 CSII(중 재법) 치료는 MDI(비교법) 대비 당화혈색소(HbA1c) 수치의 유의한 감소를 보이는 것으로 평가되었다. 반면 임신부 대상의 경우 연구 문헌이 2편으로 적었고, 2편의 문헌에서 임신주수에 따른 기간(first, second, third trimester)별로 차이가 있다고 제시하였다. 따라서 두 군간 유의성 여부는 평가하기 어려웠다.

비용효과성 결과 인슐린펌프의 사용이 단기적 측면으로 보면 비용 효과적이지 않게 분석될 수 있으나, 3편(Roze et al., 2015; Cohen et al., 2007; Roze et al., 2005)의 연구에서 제시 하였듯이 정상에 가까운 혈당유지 및 혈당조절을 통해 당뇨병성 합병증 발생률을 줄이는 것은 장기적 관점으로 보았을 때 합병증 관련 치료비용 절감 효과를 기대할 수 있어 비용 효과적 이라고 평가 할 수 있을 것이다.

교육관련 결과 교육(인슐린펌프 사용법 포함)관련 결과를 제시한 문헌은 안·유 분석용 문헌 4편, 비용 효과성 분석용 문헌 4편에서 확인되었으나, 8편 모두 당뇨병 관련 실습 또는 교육제공 여부만을 제시하고 중재법과 비교법을 비교하여 유의성을 평가한 연구는 없었다. 비용효과성 평가 문헌 중 Heller et al. (2017)의 연구는 CSII 및 MDI 두 그룹 모두 체계화된 훈련을 받은 후에 당뇨병 조절이 개선되었다고 제시하였고, 체계화된 훈련 이전에 인슐린펌프의 광범위한 사용은 당뇨병 관리를 개선하거나 비용 효과적일 것 같지 않다고 제시하였다. Pollard et al. (2018)의 연구는 임상적으로 지체 없이 인슐린펌프 사용이 필요한 경우가 아닌 한, NHS 의료진은 제1형 당뇨병 환자에게 인슐린펌프 사용을 고려하기 전에 이점(benefit)이 입증된 체계적 교육과정을 제공해야 한다고 제시하였다. 따라서 CSII 사용에 있어 선행적이고 지속적인 실습 또는 교육은 혈당개선과 함께 장기적 측면의 비용효과성도 기대할 수 있을 것이라고 판단 할 수 있을 것이다.

평가 범주 및 제한점 국내외 데이터베이스를 통해 검색한 결과 본 연구 설계에 부합하여 CSII와 MDI의 안 전성, 유효성 및 비용효과성을 비교한 국내문헌은 검색되지 않았다. 인슐린펌프는 현재 국내에 수입되어 시판 허가된 제품에 국한하였으며 연구 설계에 제시한 PICO를 기반으로 관련문헌 검색을 수행하였다. 따라서 인슐린을 자동으로 주입해 주는 기능이 내제된 인공췌장기(artificial pancreas)(제품명 예시, MiniMed 670G)는 본 체계적 문헌고찰에서 배제하였다. 인슐린펌프는 고가의 의료기기로 사용자의 기기 사용관련 인지도 및 숙련도 등이 중요한 변수로 작용하는 만큼 인슐린펌프 사용을 통한 당뇨관리의 효과를 얻기 위해서는 인슐린펌프 사용법을 포함한 당뇨병 관리에서 교육의 중요성이 소위원회의 논의에서 여러 번 강조되었다. 따라서 본 체계적 문헌고찰 연구결과에는 CSII 대비 MDI 치료와 관련하여 당뇨병 교육에 대해 선택된 문헌에서 제시한 연구결과도 함께 제시하였다. 문헌에 따라 인슐린펌프 기기 명을 제시한 문헌이 있는 경우에는 그 내용도 자료추출 결과에 함께 작성하였으나, 본 연구의 주요 목적이 인슐린펌프 기기의 성능 및 사양에 따른 결과 비교가 아닌 제1형 당뇨병 환자에게 있어 기존 인슐린 다회 주사법(MDI) 대비 인슐린펌프(CSII) 사용의 안전성, 유효성, 비용효과성을 검토하기 위함이므로 인슐린 펌프 기기명은 고려하지 않고 CSII 또는 SAP 두 군으로만 나누어 결과를 분석하였다.

결론 및 제언 기존 인슐린 다회 주사법(MDI) 대비 인슐린펌프(CSII) 치료는 인슐린 펌프 가격 및 관련 소모품 가격으로 인해 단기적으로는 비용 효과적이라고 제시하기는 어렵다고 판단된다. 하지만 본 연구에서 선택된 SR 문헌들의 연구결과들을 토대로 볼 때 인슐린펌프는 사용법 연습 또는 관련 교육이 반드시 필요하며, CSII 치료에서 MDI 치료 대비 혈당관 리의 중요 지표인 당화혈색소(HbA1c)(%) 변화에서 유의한 감소를 보인다는 점은 매우 큰 의미를 갖는 것으로 보인다. 지속적이고 안정적인 혈당유지 및 관리를 통해 당뇨병 환자는 당뇨병 관련 만성 합병증인 망막병증(retinopathy), 신장병증(nephropathy), 신경병증(neuropathy), 심혈관계 질환 등의 발생을 예방하거나 늦출 수 있을 것이며, 이러한 결과는 합병증 발현관련 문제 감소로 의료기관의 이용 빈도나 횟수를 줄이는 효과를 가져올 수 있을 것이다. 아울러 이로 인해 환자는 병원 입원 또는 외래 방문 횟수 감소를 통해 장기적으로는 의료비 지출 감소효과를 기대할 수 있을 것으로 판단된다.

1.Continuous glucose monitoring system Purposes The continuous glucose monitoring system (CGMS) was first developed by Medtronic (Minneapolis, MN, USA) and approved by the US FDA in June 1999. CGMS is useful for treating diabetes patients with fluctuating glucose levels over a large range and frequent hypoglycemia. CGMS is aimed at efficient diabetes management and uses a sensor to measure changes in blood glucose levels in real-time. The device is used to continuously test the blood glucose level. When type 1 diabetes patients purchase disposable products to self-monitor blood glucose using a prescription issued by a physician (an internist, pediatrician, or family medicine specialist) within 90 days at one of the medical equipment and supply stores registered with the Korea National Health Insurance Service, they receive insurance benefits. Accordingly, to establish objective evidence to determine the clinical utility and financial value of CGMS based on its safety, effectiveness, and cost-effectiveness, the Korea National Health Insurance Service requested the Korea National Evidence-based Healthcare Collaborating Agency (NECA) to perform a systematic review (SR). The NECA conducted this study with the following core question: “in comparison to self-monitoring of blood glucose (SMBG), is blood glucose management by CGMS safe, effective, and cost-effective in type 1 diabetes?"

Methods Articles included in the assessment The safety, effectiveness, and cost-effectiveness of CGMS were assessed by performing a qualitative SR. To assess safety and effectiveness, a literature search limited to SRs was performed, and 11 articles were selected. To assess cost-effectiveness, there was no article type restriction, and a total of 4 articles were selected.

Results Safety outcomes As reported in a previous study (Joo, et al., 2018), 2 of the selected articles, evaluated as "moderate-quality evidence," reported that complications and adverse events of CGMS were skin infection and allergic skin reaction, among others, in the area the sensor was attached. Accordingly, it was determined that although adverse skin reactions could occur, safety would not be a serious issue with CGMS because adverse events have rarely been reported, and they are mild. Effectiveness outcomes To assess the effectiveness of CGMS, the evaluation of HbA1c change, hyperglycemia, hypoglycemia, and quality of life, out of the indices used in the previous study (Joo et al., 2018), was performed. Joo et al. (2018), in a study involving pediatric patients with type 1 diabetes, reported that CGMS, compared with SMBG, showed a statistically significant reduction in HbA1c in all 6 articles evaluated as "high-quality evidence;" in 5 articles evaluated as "moderate-quality evidence," HbA1c was reduced although the change was not statistically significant in most of the studies. Based on the finding that CGMS facilitated an equivalent or greater HbA1c reduction than SMBG, Joo et al. (2018) concluded that CGMS was effective. Accordingly, it is concluded in this study that CGMS is effective, given that it showed an equivalent or greater effect, compared with SMBG, in reducing HbA1c in both pediatric and adult type 1 diabetes patients. The frequency, timing, and risk ratio of hypoglycemia were also examined. Similar to the study by Joo et al. (2018), this study determined that evidence was insufficient to conclude that the use of CGMS reduces the incidence of hypoglycemia in pediatric or adult patients. To examine the quality of life following the use of CGMS, the findings of 5 original articles included in 2 "high-quality evidence" SR articles were summarized. Based on 2 studies reporting significant differences, Langendam et al. (2012) interpreted that the significant difference was attributable to the increased satisfaction due to the combined use of an insulin pump and CGM as the patients had not used an insulin pump before participating in the study. Accordingly, the current study, similar to Joo et al. (2018), determined that evidence was insufficient to conclude that the use of CGMS increases the quality of life in pediatric or adult patients in comparison with SMBG. However, in the DIaMonD (2017), a clinical trial recently conducted to evaluate the quality of life in type 1 adult patients, the confidence related to hypoglycemia increased, stress reduced, and wellbeing, the health status, and the fear for hypoglycemia improved. In particular, it helped reduce severe hypoglycemic events during sleep or driving. Thus, the quality of life issue needs to be reevaluated in additional studies. Cost-effectiveness outcomes Of the 4 articles selected for cost-effectiveness analysis, 2 were conducted in the US, 1 in Canada, and 1 in Spain. Regarding the analytic point of view, 3 of the articles used the social perspective, and 1 used the payer perspective. The articles that used the social perspective (McQueen et al., 2011; Chaugule et al., 2017; Wan et al., 2018) assessed CGMS as a cost-effective alternative, and the one article that used the payer perspective (Garcia-Lorenzo et al., 2018) assessed it as not cost-effective. The ICER thresholds were $45,033 (USD), $33,789 (CAD), and $33,459 (USD) for the 3 articles that concluded that CGMS was cost-effective, respectively, and €880,229/QALY for the article that concluded that it was not cost-effective. The Spanish study (Garcia-Lorenzo et al., 2018), which concluded that CGMS was not cost-effective from a social perspective, used the utility values of diseases instead of diabetes as a disutility in the utility estimation. Jesus (2019) raised several issues with the Spanish study (that is, recent articles were not included, the costs of ER visits and hospital admissions due to hypoglycemia and ketoacidosis were not factored in, and the most recent devices were not considered) and pointed out that the estimated ICER threshold was too low, suggesting that it could be a cost-effective alternative. Hence, in Korea, to determine cost-effectiveness, policy decisions should be made with considerations of the domestic situation.

Conclusion and suggestions Considering the 11 articles included in the effectiveness assessment, it seems that the clinical safety and effectiveness of CGMS are equivalent or superior to those of SMBG for adult type 1 diabetes patients, as reported by Joo et al. (2018). However, allergic skin reactions are likely to occur, and the continuous attachment of the CGMS sensor to the skin should be considered. Concerning effectiveness, although the magnitude of the HbA1c reduction was 0.1–0.5%, it is difficult to conclude on the decrease in the incidence of hypoglycemia and hyperglycemia, the maintenance of glucose level, impact on treatment outcome, and quality of life with the use of CGMS based on existing evidence. In addition, a few other articles (Wojciechowski et al., 2011; Poolsup et al, 2013) described the additional benefits of real-time CGMS, such as the decreased incidence of hypoglycemic episodes, self-monitoring as a lifestyle, and the feasibility to use it as a device supporting both pharmacological and lifestyle interventions. It is found in the literature that the extent of usage of the sensor was related to glucose management, and the sensor may be effective for managing glucose levels in pediatric patients with the help of parents. Thus, CGMS could be more effective in controlling blood glucose in pediatric type 1 diabetes patients if the patients and their parents are carefully trained on the use of the sensor. Recent articles have reported the convenience of CGMS as a lifestyle intervention, as well as its effect on hypoglycemia, its improvement of the quality of life, and the usage level-dependent effect. Accordingly, policymakers need to consider patient convenience. Currently, CGMS is continually advancing. Hence, a policy direction would be to encourage maximal patient convenience by recommending a device design that will facilitate intermittent monitoring of glucose levels to minimize the discomfort in wearing while reducing the cost of sensors. This review of SRs was conducted over a short period to aid in policy decisions. Thus, the study has the following limitations, and caution should be taken in interpreting the results. The main limitations are as follows. Because the article type was limited to a systematic review, as in the previous study (Joo et al., 2018), (1) some of the articles were used in two or more SRs, (2) the results should be interpreted with care as the statistical effect of the overlapping articles could be over-interpreted, and (3) the synthesis of the results reported in the original articles should be performed separately. However, regarding the synthesis of the study findings, it was difficult to make conclusions only based on the original articles included in the selected SRs because they were highly heterogeneous. Accordingly, future research to establish evidence should be conducted based on original articles that will be conducted in the future to investigate quality of life issues, such as hyperglycemia and hypoglycemia (including nocturnal hypoglycemia), and cost-effectiveness.

2. Insulin pump Purposes Continuous subcutaneous insulin infusion (CSII), also known as an insulin pump, is a medical device developed to administer insulin continuously for 24 hours. CSII helps to regulate and maintain the blood glucose level close to normal by maintaining blood insulin levels close to the natural levels for the human body. Currently, the Korea National Health Insurance Service is considering expanding the health insurance benefits to cover convenient medical devices for glucose monitoring in relation to the effort to establish protective measures for pediatric diabetes patients. Considering the demand for health insurance benefit expansion and timeliness, disposable products (sensors) necessary for the use of CGMS were given the priority and started being covered on January 1, 2019 (the Office for Government Policy Coordination, 2017). However, CGMS and CSII, which are expensive and currently not covered by the national health insurance, are still under review for insurance coverage. Accordingly, to establish objective evidence for the clinical utility and economic value of CSII based on its safety, effectiveness, and cost-effectiveness, the Korea National Health Insurance Service requested the Korea National Evidence-based Healthcare Collaborating Agency (NECA) to perform a systematic review (SR). In this study, the NECA aimed to assess the safety, effectiveness, and cost-effectiveness of CSII in type 1 diabetes patients by comparing it with multiple daily insulin injections (MDIs), which is the conventional treatment.

Methods Articles included in the assessment The safety, effectiveness, and cost-effectiveness of CSII were assessed by performing an SR. For the assessment of safety and effectiveness, the article type was limited to an SR, and 17 SR articles were selected. For the assessment of cost-effectiveness, the article type was not restricted. Seventeen articles were selected, of which 4 were SRs, 4 were randomized clinical trials, 1 was a pilot study, and 8 were categorized as others such as simulation and hypothesis-testing. Five of the 17 articles were health technology assessment (HTA) reports.

Outcome indices The safety of CSII was primarily assessed with such indices as severe hypoglycemia, nocturnal hypoglycemia, and diabetic ketoacidosis (DKA). Additionally, any results reported in the articles regarding non-severe mild/moderate hypoglycemia, hyperglycemia, and adverse effects associated with CSII (like insulin injection site problem and problems with the insulin pump device), including severe adverse events such as death, were also included. The primary outcome index used in the effectiveness assessment was a change in HbA1c. Any reports in the articles regarding blood glucose levels before or after a meal were also presented in the Data Extraction section. The results were presented separately for each of the 3 patient groups (pediatrics, adults, and pregnant women). Cost-effectiveness was examined using economic indicators such as ICER, QALY, and DALY as primary outcome measures. Of the 17 articles selected for cost-effectiveness assessment, one (Colquitt et al., 2004) was excluded from analysis because the cost-effectiveness assessment results were not reported.

Overview of the article selection process and analysis For this SR, a literature search was performed using 5 domestic databases (KoreaMed, RSISS, KISS, NDSL, and KMBASE) and 3 foreign databases (Ovid-MEDLINE, Ovid-EMBASE, and Cochrane Library). The assessment of safety, effectiveness, and cost-effectiveness was independently performed by 2 researchers. Disagreements were resolved by the two researchers through a discussion based on the original article or through a review by a subcommittee. A total of 456 articles on safety and effectiveness assessment were extracted from the domestic and foreign databases (143 domestic and 313 foreign articles). The duplicates (65 domestic and 54 foreign) were excluded, and the title, abstract, and full-text of the remaining articles (78 domestic and 259 foreign) were examined. Three hundred and twenty articles (78 domestic and 242 foreign) were excluded based on the selection/exclusion criteria. Finally, 17 articles (all foreign articles) were selected for the safety and effectiveness assessment. The list of the 17 finally selected articles is presented in the appendix. Regarding cost-effectiveness, 1,865 articles (all foreign articles) were obtained from the literature search using the domestic and foreign databases. Of those, 314 duplicates were excluded, and the title, abstract, and full-text of the remaining 1,551 articles were examined. A total of 1,534 articles were excluded based on the selection/exclusion criteria, and 17 articles were finally selected to assess cost-effectiveness.

Results A literature search using domestic and foreign databases showed no domestic articles comparing the safety, effectiveness, and cost-effectiveness of CSII and MDI.

Safety outcomes Sixteen out of the 17 articles presented severe hypoglycemia as the primary outcome measure, and 2 articles presented nocturnal hypoglycemia. Regarding the incidence of hypoglycemia (including severe hypoglycemia and nocturnal hypoglycemia), it was determined that there was no significant difference between CSII and MDI patient groups in pediatric or adult patients. Regarding the incidence of hypoglycemia in pregnant women (including severe hypoglycemia and nocturnal hypoglycemia), it was difficult to determine the significance of the inter-group difference based on only 2; however, the difference seemed not to be significant.

A total of 6 articles, including 2 articles focusing only on pregnant women, reported DKA. Based on a review of the 6 articles, it was determined that CSII and MDI patient groups did not show a significant difference in any of the pediatric, adult, and pregnant patient groups.

Additionally, 4 articles reported that the difference between the MDI and CSII patient groups was not significant based on the patterns of non-severe mild/moderate hypoglycemia.

Only 1 article (Yeh et al., 2012) reported the outcome of hyperglycemia. It was an SR performed by classifying the therapeutic interventions and comparators into 3 groups (CSII vs. MDI; CGM vs. SMBG; sensor-augmented pump, SAP vs. MDI+SMBG) in pediatric and adult patients. Two studies included in the article (Bergenstal et al., 2010; Hermanides et al., 2011) reported that the duration of hyperglycemia was significantly shorter in SAP than in MDI (p < 0.001).

The adverse events associated with the use of CSII (like insulin injection site problem and issues with insulin pump devices) were reported in 1 article (Jeitler et al., 2008). One (Nosadini et al., 1988) of the 23 RCT articles included in that article reported one case of death in the CSII patient group. The patient was reported to have had hypertension, fever, and ketonuria 2 days before death and there was no change in the glucose level although the insulin infusion dose was increased. Two articles reported infusion site problems (Hoogma et al., 2006; Schottenfeld-Naor et al., 1985). Specifically, the infusion site problems reported in Schottenfeld-Naor et al. (1985) were catheter problems, pump arrest or dosage errors, battery problems, and syringe dislocation. Additionally, 2 articles reported other adverse events without providing details (Hoogma et al., 2006; Ziegler et al., 1990).

Effectiveness outcomes All of the 17 articles reported a change in HbA1c as the primary outcome measure. In 13 of the articles, HbA1c significantly decreased in the patients treated with CSII (therapeutic intervention) compared with MDI (comparator). Based on the results of the 13 articles, CSII (therapeutic intervention) significantly reduced HbA1c compared with MDI (comparator) in pediatric as well as adult patients. On the other hand, only 2 articles reported results in pregnant women, and changes in HbA1c were reported although they varied depending on the trimester (first, second, and third) of the patients. Hence, it was difficult to determine the significance of the inter-group difference.

Cost-effectiveness outcomes From the short-term perspective, CSII may be considered as not cost-effective. However, it can be thought to be cost-effective from a long-term perspective, as shown in 3 articles (Roze et al., 2015; Cohen et al., 2007; Roze et al., 2005), because it is expected that the decrease in the incidence of diabetic compilations by controlling and maintaining the glucose levels close to normal has an effect of reducing treatment cost.

Education-related outcomes Education-related outcomes (including how to use insulin pumps) were reported in 8 articles; 4 were selected for the safety and effectiveness assessment whereas the other 4 were selected for the cost-effectiveness assessment. All of the articles indicated if there was a diabetes-related practice or education, but none performed significance testing on the difference between the therapeutic intervention and comparator. Of the articles selected for cost-effectiveness assessment, Heller et al. (2017) reported that there was an improvement in diabetes control in CSII and MDI groups alike and suggested that the extensive use of CSII before systematic training may not improve diabetes management or be cost-effective. Pollard et al. (2018) reported that NHS medical staff should provide patients with systematic training proven for benefits before considering prescribing CSII for type 1 diabetes, unless there are clinical reasons for them to use it right away. Accordingly, the use of CSII accompanied by proactive and ongoing practice or training is expected to improve the glucose level and be cost-effective from a long-term perspective.

Evaluated categories and limitations During the literature search using domestic and foreign databases, no domestic articles compared CSII and MDI based on safety, effectiveness, and cost-effectiveness. The CSII device was limited to those products currently imported and approved for sale in Korea, and the literature search was performed using the PICO strategy, as described in the Study Design section. Accordingly, studies on artificial pancreas systems equipped with the internal function of automatic insulin infusion (for example, MiniMed 670G) were excluded from the SR. CSII is an expensive medical device, and the user's knowledge about the device and skill level are important variables. Hence, the importance of training (including how to use the pump) in diabetes management was stressed several times during subcommittee meetings. Therefore, the current SR study too presented the study findings related to diabetes education in association with CSII and MDI that were reported in the selected articles. If an article revealed the CSII product name, it was also presented in the Data Extraction section. However, the main purpose of this study was to review the safety, effectiveness, and cost-effectiveness of CSII and MDI in type 1 diabetes patients, and not to compare various CSII devices based on function and specification. Thus, the results were for the CSII and SAP groups, and individual CSII products were not evaluated.

Conclusion and Suggestions It is difficult to say that treatment with CSII is cost-effective in the short-term because of the costs of the device and the related disposable products necessary for using the device. However, based on the results reported in the SR articles selected in this study, it seems of great significance that practice or training on how to use CSII is a must. The findings show that the decrease in HbA1c (%), an important outcome of glucose management, was significant in the treatment with CSII and MDI. Through continuous and stable glucose management, the occurrence of chronic complications of diabetes (such as retinopathy, nephropathy, neuropathy, and cardiovascular disease) are likely to be prevented or delayed, which will have an effect of reducing the frequency of healthcare use due to the decreased frequency of complication-related problems. Hospital admissions or outpatient visits of diabetes patients will also decrease. Accordingly, over the long-term, it is expected that healthcare spending will decrease
