로봇 보조 수술 - 순환기

Abstract

평가 배경 로봇 보조 수술은 컴퓨터가 제공하는 3차원 영상을 바탕으로 집도의가 로봇 팔을 원격조정하여 수술을 시행하는 기술이다. 2005년 식품의약품안전처에서 허가된 후 2006년 의료행위전문평가위원회에서 관혈적 수술 대비 재원기간을 단축시키고 수술부위 상처를 적게 하는 등의 장점은 있으나 비용-효과성 등 진료상의 경제성이 불분명한 점을 들어 비급여로 결정된 바 있다. 이후 지속적으로 로봇 보조 수술의 급여화와 관련한 논의가 있었으며 2022년 건강보험심사평가원(이하 ‘심평원’) 예비 급여부에서는 로봇 보조 수술에 대한 급여 적용 타당성 판단 등 비급여의 급여 추진과 관련하여 의사결정에 필요한 근거자료를 도출하기 위하여 해당 행위의 재평가를 본원에 의뢰하였다(예비급여부-6, 2022.01.05.). 심평원에서는 관련 학회의견을 참조하여 기존수술(복강경 포함) 대비 임상적 유용성이 유사하거나 높은 로봇 보조 수술 및 관련 적응증 54건(11개 영역)에 대하여 평가를 의뢰하였으며 2022년 제2차 의료기술재평가위원회(2022.2.18.)에서 재평가계획서 및 소위원회 구성안에 대한 심의 후 재평가를 수행하였다. 본 평가의 목적은 로봇 보조 수술의 임상적 안전성 및 효과성에 대한 과학적 근거를 제공함으로써 관련 정책적 의사결정을 지원하는 것이며, 본 보고서는 ’관상동맥협착’, ‘심방중격결손증’, ‘삼첨판역류증’, ‘승모판역류증’ 환자에서 로봇 보조 수술이 임상적으로 안전하고 효과적인지 평가하였다.

평가 방법 로봇 보조 수술의 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “로봇 보조 수술 평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회 구성은 비뇨의학과 3인, 산부인과 3인, 흉부외과 2인, 이비인후과 2인, 외과 1인, 내분비외과 1인, 근거기반의학 2인의 전문가 14인으로 구성하였다. 소위원회에서는 심평원에서 의뢰한 54건의 로봇 보조 수술에 대하여 질환에 따라 33개 항목으로 재분류하고 재평가를 수행하기로 하였다. 본 평가의 핵심질문은 “관상동맥협착 환자에서 로봇 보조 수술은 기존 수술 대비 임상적으로 안전하고 효과적인가?”, “심방중격결손증 환자에서 로봇 보조 수술은 기존 수술 대비 임상적으로 안전하고 효과적인가?”, “삼첨판역류증 환자에서 로봇 보조 수술은 기존 수술 대비 임상적으로 안전하고 효과적인가?”, “승모판역류증 환자에서 로봇 보조 수술은 기존 수술 대비 임상적으로 안전하고 효과적인가?”이었다. 안전성 지표는 합병증, 개흉술로의 전환이었고, 효과성 지표는 수술 관련 지표(수술시간, 출혈량, 재원기간), 병리학적 또는 종양학적 지표(재발률, 생존율), 기능 또는 삶의 질 지표 등이었다. 또한 본 평가의 목적을 고려하여 연구유형은 무작위배정 비교임상시험(Randomized controlled trial, RCT)으로 제한하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌선택 및 배제기준에 따라 6명의 평가자가 짝을 이루어 독립적으로 수행하고, 의견의 불일치가 있는 경우에는 평가자간 합의를 통해 최종 논문을 결정하였다. 선택문헌이 있을 경우, 문헌의 비뚤림위험 평가, 자료분석, 근거수준 평가를 수행하였다. 평가결과 및 소위원회의 검토의견을 고려하여 의료기술재평가위원회에서 최종심의 후 권고등급을 제시하였다.

평가 결과 1. 관상동맥협착 관상동맥협착 환자에서 로봇 보조 동맥간우회로조성술(대동맥-관동맥간)에 대한 안전성 및 효과성을 평가하는 체계적 문헌고찰을 수행하였으나 최종 선정된 RCT 문헌은 없었다. 2. 심방중격결손증 심방중격결손증 환자에서 로봇 보조 심방중격결손증수술(최소 침습적 방법)에 대한 안전성 및 효과성을 평가하는 체계적 문헌고찰을 수행하였으나 최종 선정된 RCT 문헌은 없었다. 3. 삼첨판역류증 삼첨판역류증 환자에서 로봇 보조 판막성형술(삼첨판)에 대한 안전성 및 효과성을 평가하는 체계적 문헌고찰을 수행하였으나 최종 선정된 RCT 문헌은 없었다. 4. 승모판역류증 승모판역류증 환자에서 로봇 보조 판막성형술(승모판)에 대한 안전성 및 효과성을 평가하는 체계적 문헌고찰을 수행하였으나 최종 선정된 RCT 문헌은 없었다.

결론 및 제언 소위원회에서는 현재 문헌적 근거를 바탕으로 다음과 같이 결과 및 의견을 제시하였다. 관상동맥협착, 심방중격결손증, 삼첨판역류증, 승모판역류증 환자에서 로봇 보조 수술에 대한 RCT 문헌은 확인되지 않아, 소위원회에서는 해당 질환에서 로봇 보조 수술이 기존 수술과 비교하여 안전하고 효과적인지 판단할 수 없다고 평가하였다. 또한 향후 잘 설계된 임상연구를 기반으로 관상동맥협착, 심방중격결손증, 삼첨판역류증, 승모판역류증에서의 로봇 보조 수술의 안전성과 효과성에 대한 근거가 필요하다고 제언하였다. 2022년 제12차 의료기술재평가위원회(2022.12.9.)에서는 의료기술재평가사업 관리지침 제4조 제10항에 의거 “로봇 보조 수술”에 대하여 로봇 보조 수술이 사용되는 전반적인 항목을 대상으로 문헌적 근거를 검토한 소위원회의 결론에 대하여 종합적으로 심의하였다. 1) 의료기술재평가위원회는 임상적 안전성과 효과성 등을 판단할 RCT 문헌을 확인하지 못하여 국내 임상상황에서 관상동맥협착 환자를 대상으로 수행하는 로봇 보조 수술의 권고등급을 결정할 수 없어 ‘불충분’으로 심의하였다(권고등급: 불충분). 2) 의료기술재평가위원회는 임상적 안전성과 효과성 등을 판단할 RCT 문헌을 확인하지 못하여 국내 임상상황에서 심방중격결손증 환자를 대상으로 수행하는 로봇 보조 수술의 권고등급을 결정할 수 없어 ‘불충분’으로 심의하였다(권고등급: 불충분). 3) 의료기술재평가위원회는 임상적 안전성과 효과성 등을 판단할 RCT 문헌을 확인하지 못하여 국내 임상상황에서 삼첨판역류증 환자를 대상으로 수행하는 로봇 보조 수술의 권고등급을 결정할 수 없어 ‘불충분’으로 심의하였다(권고등급: 불충분). 4) 의료기술재평가위원회는 임상적 안전성과 효과성 등을 판단할 RCT 문헌을 확인하지 못하여 국내 임상상황에서 승모판역류증 환자를 대상으로 수행하는 로봇 보조 수술의 권고등급을 결정할 수 없어 ‘불충분’으로 심의하였다(권고등급: 불충분).

Background Robotic surgical procedures use a technique in which a surgeon remotely controls a robotic arm to perform surgery based on three-dimensional images supplied by a computer. After approval by the Ministry of Food and Drug Safety in 2005, the Medical Practice Professional Assessment Committee determined that robotic surgical procedures would be non-benefit items in 2006 because of uncertainty regarding their economic feasibility, such as their cost-effectiveness, despite the advantages of shorter hospital stays and reduced scarring at the surgical site compared to open surgery. Subsequently, there have been ongoing discussions on the inclusion of robotic surgical procedures as benefit items, and in 2022, the Preliminary Coverage Division of the Health Insurance Review and Assessment Service (HIRA) contracted our institution to conduct a reassessment of this medical practice by identifying supporting data needed to make decisions regarding expansion of coverage, such as determination of the validity of including robotic surgical procedures as benefit items (Preliminary Coverage Division; May 1, 2022). HIRA referred to the opinions of relevant academic societies and requested the assessment of 54 types (11 domains) of robotic surgical procedures, and their applicable indications, with similar or high clinical usefulness compared to conventional surgery (including laparoscopic surgery). Accordingly, the reassessment was conducted after a review of the reassessment plan and proposed subcommittee composition during the second session of the Committee of Health Technology Reassessment on February 18, 2022. The purpose of the assessment was to provide scientific evidence of the clinical safety and effectiveness of robotic surgical procedures to support relevant policy decision-making. This assessment reports on whether robotic surgical procedures are clinically safe and effective in patients with coronary artery disease (CAD), heart (atrial) septal defect, tricuspid valve insufficiency, and mitral valve insufficiency.

Methods A systematic review was performed to assess the safety and effectiveness of robotic surgical procedures. The assessment methods were finalized after review by the “Robotic surgical procedure assessment subcommittee” (hereinafter referred to as the “subcommittee”) after considering the purpose of the assessment. The subcommittee comprised 14 experts—three each from urology and obstetrics/gynecology; two each from thoracic surgery, otolaryngology, and evidence-based medicine; and one each from surgery and endocrinology. The subcommittee conducted the reassessment by reclassifying the 54 types of robotic surgical procedures identified by HIRA into 33, according to disease type. The key questions in the assessment were, “Are robotic surgical procedures clinically safe and effective in patients with CAD, compared to conventional surgery?”; “Are robotic surgical procedures clinically safe and effective in patients with heart (atrial) septal defect, compared to conventional surgery?”; “Are robotic surgical procedures clinically safe and effective in patients with tricuspid valve insufficiency, compared to conventional surgery?”; and “Are robotic surgical procedures clinically safe and effective in patients with mitral valve insufficiency, compared to conventional surgery?” Safety indicators were complications and conversion to open thoracotomy, while effectiveness indicators were surgery-related indicators (operating time, amount of bleeding, and length of hospital stay), pathologic or oncologic indicators (recurrence and survival rates), and functional or quality of life indicators. Considering the purpose of this assessment, the study type was limited to randomized controlled trials (RCTs). In the systematic review, three international and five domestic databases were searched, based on the key questions, and six reviewers, working in pairs, independently screened and selected the articles in accordance with the inclusion and exclusion criteria. In the event of a disagreement, articles were selected through discussions among the reviewers to reach a consensus. For the selected articles, risk of bias (RoB) assessment, data analysis, and level of evidence assessment were performed. The Committee of Health Technology Reassessment issued the grade of recommendation after a final review considering the assessment results and the subcommittee review opinions.

Results 1. CAD A systematic review was performed to assess the safety and effectiveness of robot-assisted arterial bypass surgery (aorto-coronary) in patients with CAD. However, no RCT reports were ultimately selected. 2. Heart (atrial) septal defect A systematic review was performed to assess the safety and effectiveness of robotic surgical procedures for heart (atrial) septal defect repair (minimally invasive method) in patients with heart (atrial) septal defect. However, no RCT reports were ultimately selected. 3. Tricuspid valve insufficiency A systematic review was performed to assess the safety and effectiveness of robot-assisted valvuloplasty (tricuspid valve) in patients with tricuspid valve insufficiency. However, no RCT reports were ultimately selected. 4. Mitral valve insufficiency A systematic review was performed to assess the safety and effectiveness of robot-assisted valvuloplasty (mitral valve) in patients with mitral valve insufficiency. However, no RCT reports were ultimately selected.

Conclusions and recommendations Based on the lack of current evidence in the literature, the subcommittee presented the following findings and opinions. The subcommittee concluded that whether robotic surgical procedures in patients with coronary artery disease, heart (atrial) septal defect, tricuspid valve insufficiency, or mitral valve insufficiency are safe and effective, compared to conventional surgery, could not be determined due to the unavailability of articles on relevant RCTs. Furthermore, the subcommittee recommended that evidence of the safety and effectiveness of robotic surgical procedures for coronary artery disease, heart (atrial) septal defect, tricuspid valve insufficiency, and mitral valve insufficiency, based on well-designed clinical trials, is needed in the future. During the twelfth session of the Committee of Health Technology Reassessment on December 9, 2022, the committee comprehensively reviewed the subcommittee findings on the evidence in the literature for the general areas in which robotic surgical procedures are used in accordance with Article 4-10 of the Guidelines for the Management of Health Technology Reassessment Project. 1) The Committee of Health Technology Reassessment determined that the level of evidence for robotic surgical procedures performed in clinical practice in Korea for patients with CAD was “insufficient” due to the unavailability of RCTs reporting on their clinical safety and effectiveness (Grade of recommendation: insufficient). 2) The Committee of Health Technology Reassessment determined that the level of evidence for robotic surgical procedures performed in clinical practice in Korea for patients with heart (atrial) septal defect was “insufficient” due to the unavailability of RCTs reporting on their clinical safety and effectiveness (Grade of recommendation: insufficient). 3) The Committee of Health Technology Reassessment determined that the level of evidence for robotic surgical procedures performed in clinical practice in Korea for patients with tricuspid valve insufficiency was “insufficient” due to the unavailability of RCTs reporting on their clinical safety and effectiveness (Grade of recommendation: insufficient). 4) The Committee of Health Technology Reassessment determined that the level of evidence for robotic surgical procedures performed in clinical practice in Korea for patients with mitral valve insufficiency was “insufficient” due to the unavailability of RCTs reporting on their clinical safety and effectiveness (Grade of recommendation: insufficient).
