말라리아 신속 진단 검사의 급여 전환 필요성

Abstract

연구 배경 우리나라에서는 삼일열말라리아(Plasmodium vivax) 가 토착 질병으로 존재하였다가 1959-1969년에 말라리아 근절사업(WHO Project Korea-13)을 실시하면서, 1979년에 말라리아 완전 퇴치(malaria free)지역이 되었다. 그러나 1993년 파주지역 DMZ에 복무 중이던 군인 1명에서 삼일열말라리아가 재발생한 이후 현재까지 정부의 강력한 퇴치 노력에도 불구하고 경기･인천･강원의 휴전선 인접지역을 중심으로 환자 발생이 지속되고 있다. 우리나라는 경제협력개발기구(OECD) 국가 중 말라리아 발생률이 1위이며, 세계보건기구로부터 2020년까지 말라리아 퇴치가 가능한 나라로 선정되어 퇴치를 위한 국가적 노력을 기울이도록 권고 받았으나 이를 달성하지 못했다. 세계보건기구에서는 2023년 퇴치로 수정 권고하였고 이에 질병관리본부는 2019년 6월 17일 말라리아 없는 자유롭고 건강한 국가 만들기를 추진한다는 슬로건 하에 말라리아 재퇴치 5개년 실행계획(2019-2023년)을 발표하였다. 5개년 실행계획의 주요 내용에는 말라리아 신속진단검사법의 도입 및 보험 급여 추진이 포함되어 있다. 말라리아 신속진단검사법의 보험 급여 도입에 의한 말라리아 조기 진단에 따른 감염 예방 효과가 실제로 어느 정도 있는지, 국가의료재정에 끼치는 영향은 어떠한지에 대해서는 연구가 필요하나 현재까지 해당 내용에 대한 연구는 진행된 바 없다.

연구 목적 말라리아 신속진단검사를 사용하여 신속, 간편하게 진단을 하게 되면 혈액도말검사 대비 진단 비용의 부담은 늘어나지만, 혈액도말검사 결과 보고 지연에 따른 추가 의료이용 비용은 줄어들게 된다. 또한 말라리아 환자를 흡혈한 모기로 열원충이 이동하게 되어 궁극적으로 또 다른 사람이 해당 모기에 물려 삼일열원충에 감염되는 전파 주기를 보다 빠르게 차단할 수 있게 하므로 장기적으로 보았을 때 우리나라 삼일열말라리아의 발생을 줄일 수 있을것으로 판단된다. 본 연구에서는 우리나라 삼일열말라리아 동적 모델(dynamic model)을 개발하여 향후 10년의 삼일열말라리아 발생을 예측하고 현시점의 비용-편익 분석뿐만 아니라 2차 감염의 위험이 줄어들면서 생기는 미래의 비용까지 고려하여 비용-편익 분석을 진행하고자 한다.

연구 방법 본 연구는 체계적 문헌 고찰을 통한 신속진단검사의 효용성 확인, 국내 환경에 맞는 삼일열말라리아 수학모델 구축을 통한 모델 모의실험, 그리고 수학모델을 활용하여 삼일열말라리아 환자 1인당 평균 의료비용을 이용한 비용-편익 분석으로 구성되었다. 먼저 체계적 문헌 고찰을 위해 국내 선행 체계적 문헌 고찰 내용을 정리하였으며 해외에서 기존에 수행된 메타분석 결과를 참고하였다. 한편, 기존에 보고된 결과 이후에 새로이 발표된 연구 논문들을 체계적 문헌 고찰 과정을 통해 분석하였다. 수학모델은 우리나라 삼일열말라리아의 특징인 단기, 장기 잠복기 환자 발생을 잘 설명할 수 있는 지연형 미분 방정식을 이용하여 구축하였다. 삼일열말라리아 발생의 계절성은 모기의 역학과 관련된 모수를 기온에 대한 함수로 설정하여 구현하였다. 모수 값은 기존 통계 자료 등으로 잘 알려진 값의 경우 그 값을 활용하거나 선행연구에서 구한 값을 활용하였다. 그 값을 알 수 없거나 불확실한 모수들은 수학적 방법을 이용해 과거 삼일열말라리아 발생자료로부터 역으로 값을 추정하는 최소자승법(least squares method)을 이용하였다. 구축된 모델과 변수의 타당도를 검증하기 위해서 삼일열말라리아 위험지역의 2014-2018년 주간 민간인 발생자료를 활용하였다. 이후 검증된 수학모델을 활용하여 모델 모의실험 (model simulation) 과정을 진행하여 향후 10년간의 삼일열말라리아 발생을 예측하였다. 비용-편익 분석은 신속진단검사의 도입이 기존의 혈액도말검사보다 비용 대비 편익이 있는지, 그리고 국가의료재정에 장기적으로 미치는 영향을 확인하기 위해 진행되었으며 국민건강보험 일산병원(이하 일산병원)의 삼일열말라리아 환자 의료비용 자료와 건강보험심사평가원(이하 심평원)의 의료비용 자료 두 가지 자료원을 활용하여 진행되었다. 일산병원은 혈액도말검사와 신속진단검사+혈액도말검사를 통해 삼일열말라리아로 진단된 환자 1인당 평균 의료비용을 구하였으며, 심평원 자료의 경우 신속진단검사가 청구자료로는 확인이 불가하여 우리나라의 신속진단검사 도입 시점을 기준으로 도입 이전은 혈액도말검사만 사용된 것으로 정의하고, 도입 이후에는 신속진단검사+혈액도말검사 가 사용된 것으로 정의하였다. 비용 산출 시점은 2019년 기준으로 하였고, 신속진단검사를 도입했을 때 발생하는 편익과 비용의 증가량을 계산하였으며 누적 편익 증가량에서 누적 비용 증가량을 나누어 비용-편익 분석의 지표 중 하나인 점증적 편익-비용 비(incremental benefit cost ratio, IBCR) 값을 구하였다. 이후 단변량, 이변량 민감도 분석을 진행하여 모수의 불확실성이 만드는 정량적이고 정성적인 결과값의 변화를 확인하였다.

연구 결과

⦁ 체계적 문헌 고찰 결과, 말라리아 신속진단검사는 말라리아 유행지역이나 유행지역에 노출된 경험이 있으며 말라리아 의심 증상이 있는 환자에서 신속하게 말라리아 감염을 확인하고 치료하고자 할 때 민감도 및 특이도 측면에서 유용한 검사이다. ⦁ 구축된 국내 삼일열말라리아 수학모델을 통한 모의실험 결과, 말라리아 신속진단검사는 삼일열말라리아 신규 발생을 줄이는 효과가 있다. ⦁ 신속진단검사 도입에 따른 신규환자 발생 감소 효과와 환자 1인당 평균 의료비용을 고려하면, 신속진단검사 도입에 따른 국가의료재정 지출 감소 효과가 확인되었다. ⦁ 비용-편익 분석을 통해 신속진단검사의 도입은, 비용 대비 편익이 우월하며 특히 해가 거듭될수록 편익이 증가하였다.

Ⅰ. 체계적 문헌 고찰 1. 신속진단검사의 진단 정확성에 관한 국내 문헌 고찰 2019년 12월 31일까지 국내 비열대열말라리아 환자에 대하여 신속진단검사의 진단 정확성에 대한 연구 7건을 확인하였으며, 7건 중 6건에서 민감도가 0.90 이상으로 높았으며 특이도 역시 높아 진단유효성이 높음을 확인하였다.

2. 신속진단검사의 효용성에 관한 2014년도 이후의 해외 문헌 고찰 2014년 이후 2019년 12월 31일까지 해외에서 출판된 비열대열말라리아 의사 환자에서 신속진단검사의 효용성 연구 현황을 확인하였으며, 4건의 연구를 확인한 결과 신속진단검사의 특이도는 높았으며(0.92-1.00) 민감도 또한 (0.94-1.00)으로 이전보다 개선된 것을 확인하였다.

Ⅱ. 삼일열말라리아 수학모델 구축 1. 삼일열말라리아 수학모델 삼일열말라리아 수학모델의 구조를 세우고 2014-2018년 군 환자를 제외한 민간인 주간 삼일열말라리아 발생 현황 자료를 이용하여 모수를 추정하여 모형의 비교 타당도를 검증하였다.

2. 수학모델 모의실험 개발한 수학모델을 활용하여 신속진단검사 도입과 혈액도말검사 유지 시 향후 10년간 삼일열 말리리아 환자 발생에 대해 모의실험을 진행하였다. 모의실험은, 혈액도말검사만을 유지했을 때는 현재의 발생 수가 향후 10년간 비교적 일정하게 유지되도록 하였고 신속진단검사를 활용할 경우 조기 치료에 따른 전염기에 머무르는 시간이 단축되는 것에 따른 환자 발생수의 변화를 평가하였다. 그 결과 2차 감염자가 줄어들게 되고, 시간이 경과할 수록 연쇄작용에 의한 영향이 누적되어 5년 후 56.5%, 10년 후 83.1%의 발생이 예방되었고, 10년간 전체 누적으로 살펴보면, 혈액도말검사 대비 신속진단검사 사용 시 총 54.2%의 환자 발생이 예방됨을 확인하였다.

III, 비용-편익 분석 1. 의료이용비 환자 1인당 평균 의료이용비를 계산하기 위해 일산병원 자료와 심평원 청구자료를 활용하였다. 각 자료를 활용하여 2019년을 기준으로 삼일열말라리아 환자 1명당 발생하는 평균 의료이용비를 산출하였다. 일산병원 자료에는 혈액도말검사 시행으로 진단 및 치료를 받은 환자의 1인당 평균 의료비용과 신속진단검사+혈액도말검사로 진단 및 치료를 받은 환자의 1인당 평균 의료비용을 구해 비교하였다. 심평원 자료에서는 신속진단검사 시행 유무를 확인할 수 없어 우리나라에 신속진단검사가 도입된 2012년을 기준으로 삼일열말라리아 발생 위험지역의 2011년의 의료이용비(혈액도말검사만 시행된 환자의 의료비용)와 2013년의 의료이용비 자료(혈액도말검사와 신속진단검사 모두 시행된 환자의 의료비용)를 활용하여 의료이용비를 구했다. 2019년 기준 일산병원 자료를 통해 확인된 혈액도말검사 의료이용비는 931,377원, 신속진단검사+혈액도말검사 의료이용비는 928,508원이었으며, 심평원 자료를 통해 확인된 혈액도말검사 의료이용비는 678,829원, 신속진단검사+혈액도말검사 의료이용비는 691,576원이었다.

2. 총 의료이용비 및 절감 비용 계산 일산병원 자료를 통해 총 의료이용비 절감에 대한 모의실험 결과는 다음과 같다. 혈액도말검사를 시행할 때의 의료이용비가 신속진단검사+혈액도말검사 대비 2019년 기준 2,869원 더 많으며, 발생 수가 곱해지면서 첫해부터 우리나라 총 삼일열말라리아 환자의 의료이용비가 약 12,590,000원이 절감되고, 매년 절감효과가 커지면서 10년째에는 약 138,570,000원이, 10년 누적으로 총 928,640,000원이 절감되는 효과가 확인되었다. 심평원 자료 분석에서는 반대로 신속진단검사를 사용한 경우의 의료이용비가 12,747원 더 높으나 역시 발생 수가 곱해지면서 첫해부터 신속진단검사+혈액도말검사 대비 약 6,680,000원의 비용이 절감되고 10년 누적으로 약 664,350,000원이 절감되는 것이 확인되었다.

3. 비용-편익 분석 일산병원 자료를 통한 비용-편익 분석에서는 신속진단검사 도입 첫해부터 IBCR 값이 1보다 크며 10년째 시행하면 그 값이 2.62까지 증가하면서 비용 대비 편익이 크기 때문에 신속진단검사 도입이 매우 유익함(beneficial)을 확인할 수 있다. 심평원 자료의 경우 신속진단검사의 1인당 편익이 작아지면서 IBCR 값도 작아졌으나 신속 진단 검사 도입 후 9년째부터는 유익한 결과가 확인되었다.

4. 민감도 분석 일산병원 자료와 심평원 자료 각각에 대하여 단변량, 다변량 민감도 분석을 시행하였으며 장기 잠복 기간의 변화에 가장 민감하게 반응하는 것을 확인하였고 신속진단검사 도입이 비용-편익 분석에서 유익하게 되는 편익과 비용(신속진단검사 가격)의 임계값이 일산병원 자료에서는 각각 11,738원, 46,880원이었으며, 심평원 자료에서는 각각 11,738원, 31,265원이었다.

결론 및 정책적 제언 이 연구에서는 체계적 문헌 고찰을 통하여 말라리아 신속진단검사가 말라리아 의심 증상이 있는 환자에서 신속하게 말라리아 감염을 확인하고 치료하고자 할 때 민감도 및 특이도 측면에서 유용한 검사임을 확인하였다. 국내 삼일열말라리아 수학모델을 통한 모의실험 결과, 말라리아 신속진단검사는 국내 삼일열말라리아의 신규 발생을 줄이는 효과가 있음을 확인하였다. 또한, 신속진단검사 도입에 따른 신규 환자발생 감소 효과와 1인당 평균 의료비용을 고려하면, 신속진단검사 도입에 따른 국가의료재정지출 감소 효과가 있음을 확인하였다. 비용-편익 분석에서 신속진단검사의 도입은, 비용 대비 편익이 우월하며 특히 해가 거듭될수록 편익이 증가함을 확인하였다. 2019년 9월 1일부터 말라리아 항원검사(젖산탈수효소)[간이검사] 가 보험급여 항목에 포함되었으나 이것이 실제 말라리아 발생 감소 효과와 비용절감 효과가 있는지에 대해서는 평가되지 않았다. 이 연구는 신속진단검사의 보험급여 도입의 기대 효과에 대하여 체계적인 분석을 통해 확인하였으며 향후 말라리아 퇴치 및 국가의료재정 지출 감소에 기여할 것으로 기대된다.

Background In Korea, Plasmodium vivax existed as an indigenous disease, but the WHO Project Korea-13 was implemented in 1959-1969 and became a malaria free area in 1979. However, since a soldier who was serving in the DMZ in Paju was re-emerged as a malaria patient in 1993, the number of patients has continued to grow in areas adjacent to the border in Gyeonggi, Incheon and Gangwon despite the government's strong eradication project. Korea has the highest incidence rate of malaria among OECD countries and is recommended by the WHO to make national efforts to fight against malaria by 2020. In response, the Korea Centers for Disease Control and Prevention announced a five-year action plan (2019-2023) to fight malaria under the slogan of creating a free and healthy country without malaria on June 17, 2019, and the main contents of the plan include the introduction of a rapid diagnostic test for malaria and the promotion of national health insurance coverage for malaria rapid diagnostic test. Although research is urgently needed on how much infection prevention effects are actually caused by the introduction of insurance coverage of the malaria rapid diagnostic test and how much impact it has on the national medical finances, no research has been conducted so far.

Objective Using malaria rapid diagnostic tests will increase the burden of diagnosis costs, but will reduce the cost of additional medical use due to delays in reporting the results. In addition, it is expected that the risk of secondary infection will be reduced as the period of infectivity that can infect others is shortened, reducing the incidence of malaria in Korea in the long run. In this study, we developed a dynamic model to predict malaria incidences in the next 10 years and conduct cost-benefit analysis not only including current cost, but also the including future cost of reducing the risk of secondary infection.

Methods This study consisted of verifying the effectiveness of rapid diagnostic test through systematic literature review, model simulation through the establishment of malaria model suitable for domestic environment, and cost-benefit analysis utilizing the average medical cost per person. First of all, the contents of the domestic prior systematic literature review were compiled and the results of the meta-analysis conducted overseas were referred to. The newly published research papers after the previous reported results were analyzed through the systematic literature review. The malaria model was built using a delayed differential equations that can better explain weekly malaria incidence by short-term and long-term incubation period. The seasonality of malaria incidence was realized by setting parameters related to the dynamics of mosquitoes as a function of temperature. The parameter values are obtained from existing statistics and from advance researches. The least squares method was used to estimate unknown or uncertain parameters in reverse from given malaria-producing data using mathematical methods. The established models and variables were validated using weekly civilian generation data from 2014 to 2018 in areas at risk of malaria. Subsequently, a model simulation process was conducted using a proven model to predict malaria incidence over the next 10 years. The cost-benefit analysis was conducted to determine whether the introduction of the rapid diagnostic test would be more cost-beneficial than the existing conventional smear examination and how the long-term impact on the national health budget was made using two sources: hospital medical expense data from Ilsan Hospital and medical expense data from the Health Insurance Review and Assessment Service (HIRA). The cost of Ilsan Hospital was obtained from the average medical expenses per patient diagnosed through the conventional smear examination and the rapid diagnostic test. In case of the data from the HIRA, because rapid diagnostic test was not available as requested, timing of the introduction of rapid diagnostic test in risky areas was used as reference point. Consequently, prior to introduction of rapid diagnostic test was defined as the use of conventional smear examination, and after introduction of rapid diagnostic test was defined as the use of rapid diagnostic test. In all cases, costs were calculated and compared on a 2019 basis, the increase in benefits and costs arising from the introduction of rapid diagnostic testing were calculated, and the cumulative increase in costs was divided from the cumulative increase in benefits to obtain the incremental benefit cost ratio (IBCR) value, one of the indicators of cost-benefit analysis. The univariate, bivariate sensitivity analysis was then conducted to confirm the quantitative and qualitative changes in the result values produced by parameter uncertainty.

Results ⦁ According to systematic literature review, malaria rapid diagnostic test is a useful test to quickly identify and treat malaria in patients who have been exposed to malaria epidemic areas and suspected malaria infections. ⦁ As a result of the simulation through the established malaria model, the rapid diagnostic test of malaria has the effect of reducing the malaria incidence. ⦁ Given the effect of reducing the incidence of new patients following the introduction of rapid diagnostic tests and the average cost per person, the effect of reducing the national medical expenditure due to the introduction of rapid diagnostic tests was verified. ⦁ The introduction of rapid diagnostic tests in cost-benefit analysis has shown superior cost-benefit compared to conventional smear examination, especially with increased benefits over the years.

I. Systematic literature review 1. A review of domestic literature on the effectiveness of rapid diagnostic testing By December 31, 2019, seven studies on the diagnosis accuracy of rapid diagnostic test were confirmed for patients with malaria in Korea, and six out of seven studies had a high sensitivity of 0.90 or higher, and high specificity of diagnosis was also confirmed.

2. A review of overseas literature after 2014 on the effectiveness of rapid diagnostic testing From 2014 to December 31, 2019, we checked the current status of efficacy studies of rapid diagnostic testing in patients with malaria published overseas, and after checking four studies, we found that the specificity of rapid diagnosis was high (0.92-1.00) and the sensitivity was also improved (0.94-1.00).

Ⅱ. Malaria model 1. Malaria model We developed the malaria model structure and validated the model using weekly malaria incidence data in 2014-2018 by estimating the parameters.

2. Model simulation Using the established model, each simulation was conducted for the introduction of rapid diagnostic tests and maintenance of conventional smear examination for the next 10 years. In the simulation, we set the model to predict constant malaria incidences over 10 years in the smear test scenario and shortened the infectious period in the rapid diagnostic test scenario. As a result, the number of secondary infections decreases and this impact is accumulated every year so that the occurrence of 56.5% after 5 years and 83.1% after 10 years was prevented and the accumulation of 54.2% was prevented.

III, Cost-benefit Analysis 1. Medical expenses Ilsan Hospital data and HIRA data were used to calculate the average medical cost per person. Using each data, the average medical use cost per malaria patient was calculated as of 2019. The data from Ilsan Hospital were compared with the average cost of medical care per patient diagnosed and treated by the conventional smear examination and the average cost per person diagnosed and treated by rapid diagnostic testing. Based on the data from HIRA, it was difficult to confirm the existence of rapid diagnostic tests, so based on 2012, the medical use cost for dangerous areas in 2011 and 2013 was used to obtain medical use costs for conventional smear examination and rapid diagnostic tests. As of 2019, the medical cost of the conventional smear examination confirmed through data from Ilsan Hospital was 931,377 won, the medical cost for the rapid diagnostic test was 928,508 won. Meanwhile, by medical cost from HIRA, the medical cost for the conventional smear examination was 678,829 won, and the medical cost for the rapid diagnostic test was 691,576 won.

2. Calculating total medical costs and savings The results of the simulation of the total medical cost reduction through the Ilsan Hospital data are as follows. As of 2019, the cost of medical use in the conventional smear examination scenario was 2,869 won higher, and about 12.59 million won was saved from the first year as the number of cases multiplied, and as the savings grew every year, about 138.57 million won was saved in the 10th year and a total of 928.64 million won was found to have been saved in the 10th year. On the contrary, the analysis of the data by the HIRA found that the medical use cost was 12,747 won higher during the rapid diagnostic test, but when the number of cases multiplied, saving about 6.68 million won from the first year and saving about 664.35 million won from the 10-year accumulation.

3. Cost-benefit analysis Cost-benefit analysis through data from Ilsan Hospital confirms that the introduction of the rapid diagnostic test is very beneficial as the value of the IBCR is greater than 1 from the first year of the rapid diagnostic test and increases to 2.62 after 10 years of implementation since incremental benefits are larger than incremental costs. As for the data from the HIRA, the benefit of rapid diagnostic test for 1 person is decreased and this leads to the reduction of IBCR, and it was confirmed that the beneficial result can be acquired in the ninth year after the introduction of the rapid diagnostic test.

4. Sensitivity Analysis Univariate and bivariate sensitivity analysis was conducted on the Ilsan Hospital data and the HIRA data respectively, and the introduction of rapid diagnostic tests was confirmed to be most sensitive to changes in the long-term incubation period, and the threshold for benefits and costs that would benefit from the cost-benefit analysis was 11,738 won, 46,880 won by Ilsan Hospital data, and 11,738 won and 31,265 won, by HIRA data.

Conclusions In this study, a systematic literature review confirmed that malaria rapid diagnostic test was useful in order to quickly identify and treat malaria infection in patients with suspected malaria symptoms. The results of the simulation through the established malaria model confirmed that the rapid diagnostic test had the effect of reducing the occurrence of new malaria. In addition, considering the effect of reducing the incidence of new patients due to the introduction of rapid diagnostic tests and considering the average cost per person, it was confirmed that national medical expenditure was reduced due to the introduction of rapid diagnostic test. In the cost-benefit analysis, introduction of rapid diagnostic test was beneficial over conventional smear examination, especially to the increase in benefits over the years. As of September 1, 2019, the malaria antigen test (lactic acid dehydrogenase) [interim test] was included in the insurance coverage category, however the actual effects of reducing malaria incidence and reducing costs were not evaluated. Through this study, we have confirmed the expected effects of the introduction of insurance benefits in the rapid diagnostic test through systematic analysis, and hope that it will contribute significantly to the eradication of malaria and the reduction of national medical finance expenditure in the future.
