전립선암 진단을 위한 전립선 특이항원 검사의 성과분석 연구

Abstract

연구 배경

최근 국내 전립선암의 유병률과 사망률의 지속적인 증가가 있었다. 암등록통계에 따르면 2018년 전립선암은 남성에서 세 번째로 높은 다빈도 암에 속하며, 유병률은 2007년 10만 명당 63.5명에서 2017년 190.3명으로 약 200% 증가하였다. 전립선암으로 인한 사망률도 증가세에 있어, 2009년 기준 10만 명당 5.0명에서 2019년 8.0명으로 10년간 약 60% 증가하였다. 건강보험청구자료에서 전립선암 환자는 2015년 61,973명에서 2019년 96,814명으로, 이들의 요양급여 총비용 또한 2015년 1,300억 원에서 2019년 2,600억 원으로 증가하였다. 전립선암에 대한 관심이 증가하면서 전립선암 선별검사인 전립선특이항원(Prostate Specific Antigen, PSA) 검사를 받는 환자도 크게 증가하였다. PSA는 전립선의 상피세포에서 생성되는 단백분해 효소물질로, 전립선 이외의 조직에서는 거의 발현되지 않는다. 이에 전립선암의 진단 또는 선별 및 감시의 표지자로 사용하며, 전립선암의 치료효과 및 수술 후 재발 판정에도 유용하게 이용할 수 있다. 건강보험청구자료에서 2019년 PSA 검사를 받은 환자는 2010년 대비 132% 증가하였다. 전립선암을 진단하는데 있어서 PSA 검사의 민감도는 높으나 특이도가 낮아 불필요한 생검을 유도하여 이에 따른 부작용이나 과잉 진단의 우려가 있다는 지적이 있어왔다. 이러한 이유로 미국과 유럽의 가이드라인에서는 일반인 남성에게 일괄적인 전립선암 선별검사를 수행하는 것을 권고하지 않고 있다. 미국 암 협회(American Cancer Society)에서는 PSA 2.5ng/ml 이상인 경우만 매년 선별검사를 시행할 것을 권고하며, 반복되는 PSA 검사에서 2.5~4.0ng/ml인 경우에는 인종, 가족력, 나이, 비정상적인 직장수지검사(Digital Rectal Exam, DRE) 결과 등의 개인별 위험인자들을 종합적으로 평가하여 전립선 조직검사를 고려하라고 권고하고 있다. 미국 종합 암 네트워크(National Comprehensive Cancer Network, NCCN)에서는 PSA 3ng/ml을 초과하거나 DRE 검사 결과에서 매우 의심스러운 소견을 가진 경우에는 전립선 조직검사를 고려하기 위한 단계로 들어가게 된다. 영국 NHS 가이드라인에서는, PSA 검사 결과가 증가했더라도 기대수명을 고려하여 결정하라고 권고한다. 이에 일반적으로 이상 여부 판정을 위한 절단치로 사용되고 있는 기준을 다양한 수준으로 차별화하여 주요 결과지표에 미치는 효과를 비교하는 연구가 이루어졌으며, 환자의 특성에 따른 절단치의 차등 적용에 관한 연구도 다수 수행된 바 있다. 유럽에서 진행한 대규모 무작위 비교 연구(European Randomized Study of Prostate Cancer, ERSPC)에서는 PSA 절단치를 3ng/ml로 하여 전립선암 조기검진의 효과를 추적 관찰하였고, 미국의 대규모 무작위 비교 임상시험에서는(Prostate, Lung, Colon and Ovarian Trial, PLCO)에서는 PSA 절단치를 4ng/ml로 하여 연구를 수행하였다. PSA 검사는 비침습적이며 검사방법이 편리하여 임상현장에서 전립선암 진단이 필요한 환자들에게 사전적으로 수행하는 선별검사로서의 기능을 무시할 수 없다. 따라서 절단치를 일괄 적용하는 것이 아니라 연령 등의 환자특성을 고려하여 보다 신중하게 조직검사 여부를 결정하고 여러 선행연구에서 우려하는 생검 부작용, 과잉진단 및 과잉치료를 줄이는 노력이 필요하다. 국내에서 전립선암 진단이 필요한 환자들에 대한 PSA 절단치에 대한 가이드라인이 미비하며 전립선암 진단을 위한 PSA 검사의 정확도에 대한 대규모 연구가 실시된 바 없다.

연구 목적

PSA 절단치에 따른 전립선암 진단정확도를 파악하기 위한 체계적 문헌고찰을 실시하고자 한다. 더불어 국내 임상현장에서 수행한 PSA 검사에 대한 연령별 성과분석을 통해 국내 임상 현장에서의 PSA 절단치별 검사 정확도를 파악하고 진단 정확도를 높이기 위한 전략을 제시하고자 한다.

연구 방법

첫째, 체계적 문헌고찰을 통해 병원 환경에서 전립선암 진단이 필요한 환자를 대상으로 전립선암 진단에 대한 PSA 검사의 정확성을 파악하는 것이다. 유럽을 시작으로 PSA 절단치를 4ng/ml 미만으로 하여 전립선암 진단을 실시는 것에 대한 관심이 증가하고 있으나 이에 대한 연구가 미흡한 실정이다. 따라서 우리 연구에서는 PSA 절단치 4ng/ml 미만에 대한 진단정확도로 연구주제를 제한하였다. 문헌 검색은 국내 2개 데이터베이스(KoreaMed, KMbase) 및 국외 3개 데이터베이스(Ovid-Medline, Ovid-Embase, Cochrane library)를 이용하였으며 문헌선정은 2명의 연구자가 독립적으로 검토한 뒤 의견일치를 통하여 최종문헌을 선택하였다. 최종 선택문헌들의 비뚤림 위험평가는 QUADAS-2(Quality Assessment of Diagnostic Accuracy-2)를 사용하였다. 선정 문헌의 특성과 결과에 대해 질적 분석을 통해 서술하였으며, 동시에 민감도와 특이도에 대한 양적분석(메타분석)도 수행하였다. 결과지표는 PSA 절단치 별 진단정확성 지표로 민감도, 특이도, 양성우도비, 음성우도비, 양성예측도, 음성예측도, 검사정확도, 진단오즈비, 곡선 아래 면적(area under curve, AUC)이 포함되었다. 둘째, PSA 검사의 전립선암 진단정확성에 대한 국내 근거를 생성하기 위하여 하부요로증상으로 비뇨의학과에 내원하여 전립선 조직검사 시행 받은 한국인 남성 대상 후향적 코호트를 구축하였다. 2011년 1월 1일에서 2019년 12월 31일까지 대상 환자의 인구학적 정보, 검사정보, 생검합병증, 암 진단정보를 수집하였다. 분석대상자의 특성을 전체대상자, 전립선암 발생군, 전립선암 비발생군, 전립선암 발생군 중 Gleason score 7 이상군 각각에 대하여 일반적 특성을 분석하였다. PSA 검사의 유용성 및 절단치 판단을 위해 Receiver Operating Characteristic (ROC) curve를 분석하였고, 민감도 수준(95%, 90%, 85%, 80%)별, 연령별 PSA 절단치를 산출하였다. 또한 전립선암 예측인자를 확인하기 위하여 단변량 및 다변수 로지스틱 회귀 분석을 실시하였다. 오즈비(Odds Ratio, OR)는 95% 신뢰구간(Confidence Interval, CI)과 유의확률(p-value)을 함께 제시하였으며, 공변량으로 보정하지 않은 오즈비(unadjusted OR)와 공변량을 보정한 오즈비(adjusted OR)를 함께 제시하였다.

연구 결과

Ⅰ. 체계적 문헌고찰 최종 선정된 총 11편의 연구(전향적 연구 9편, 후향적 연구 2편)의 결과를 대상으로 병원 환경에서 tPSA 절단치 4ng/ml 미만에서 전립선암 진단의 진단정확도에 대한 출판된 근거를 질적분석과 메타분석을 수행하여 분석하였다. 문헌 별 분석대상의 선정제외기준에 다소 차이가 있었는데, PSA 범위, 연령 기준, 직장수지검사 의심소견, 하부요로증상 여부, 약물 복용 여부, 기타 질환 과거력 등의 기준이 이에 해당한다. 또한 각 문헌에서 PSA 절단치 4ng/ml 미만 혹은 그 이상에 대한 전립선암 진단정확도를 보고하고 있었는데 우리 연구에서는 4ng/ml 미만에 해당하는 전립선암 진단정확도 지표를 추출하여 분석하였다. 메타분석 결과에 따르면, PSA 절단치 2~3.99ng/ml 구간의 통합 민감도는 0.92(95% CI, 0.86-0.95)이며, 통합 특이도는 0.16(95% CI, 0.09-0.28)이었다. 또한 이질성 (Bivariate Correlation=-0.979, HSROC beta=0.17, p=0.250) 및 출판비뚤림 (Deeks’ Funnel Plot Asymmetry Test, p=0.17)은 모두 통계적으로 유의미하지 않았다. PSA 절단치 구간을 세분화 하여 2~2.99ng/ml과 3~3.99ng/ml로 나누어 각 구간에 해당하는 통합 민감도와 통합 특이도를 비교할 때, 차이를 확인할 수 없었다. 한편, 질적 분석을 통해 각 문헌의 결과를 검토했을 때는 대부분의 연구에서 PSA 절단치가 증가할수록 전립선암 진단의 민감도는 감소하고, 특이도는 증가하며, 검사정확도는 증가하는 경향을 보였다. 메타분석에서 PSA 절단치 2~2.99ng/ml과 3~3.99ng/ml로 각 구간별 검사정확도의 차이가 없었던 이유는, 분석에 사용된 문헌이 각 구간별 6편과 9편으로 소수였으며 절단치 구간도 1ng/ml로 매우 좁아 검정력이 낮았기 때문으로 판단하였다.

Ⅱ. 후향적 코호트 연구 (국내 PSA 검사의 진단정확도 분석) 2011년부터 2019년까지 6개 상급종합병원에서 하부요로증상으로 전립선 조직검사를 받은 한국인 남성은 총 22,309명 중 선정 및 제외기준에 따라 최종 분석 대상자는 17,539명이었다. 이 중 비암환자는 9,808명이고 암으로 진단받은 환자는 7,731명이었으며, 암 환자 중 Gleason score가 7 이상으로 임상적으로 유의미한 전립선암(clinically significant prostate cancer)으로 진단받은 환자는 5,695명이었다. 분석대상자 평균 연령은 66.33±9.21세 이었으며, 평균 PSA는 39.00±244.22ng/ml로 전립선암 진단 유무에 따라 진단군 79.24±363.77g/ml, 비진단군 7.28±8.54g/ml이었다. 전립선암 진단을 위해 PSA검사 외 추가적으로 수행한 검사 중 가장 많이 시행된 검사는 전립선 초음파 검사로 전체대상자 중 79.63%(13,966명)에서 수행되었고, 다음으로 직장수지검사 48.50%(8,506명), MRI 검사 7.08%(1,242명)순 이었다. 전립선암으로 진단된 환자의 T stage는 ⅡB stage가 21.23%(1,641명)으로 가장 많았으며, ⅡA stage 19.64%(1,518명), Ⅲ stage는 18.89%(1,460 명)순 이었다. 임상적으로 유의미한 전립선암 진단 환자군에서도 유사한 경항을 보였다. PSA 검사의 전립선암 진단정확도 분석 결과, 검사의 민감도는 높으나 특이도는 낮은 것으로 확인되었다; PSA 절단치 3ng/ml에서 민감도 98.1% 특이도 8.39%, 4ng/ml에서 민감도 92.2% 특이도 22.6%. PSA 검사의 민감도 90%에 해당하는 PSA 절단치는 50세 미만에서 3.87ng/ml, 50대 3.86ng/ml, 60대 3.98ng/ml, 70대 4.63ng/ml, 80세 이상 5.73ng/ml으로 70세가 넘어가면서 크게 증가하는 양상을 보였다. 한편, 임상적으로 유의미한 전립선암(clinically significant prostate cancer) 진단을 위한 PSA 검사의 민감도 90%에 해당하는 PSA 절단치는 4.885ng/ml이었다. 연령별 PSA 검사의 전립선암 진단정확도 분석 결과, 전체 대상자에서 PSA의 AUC(area uner curve)는 73.91%이었으며, 연령에 따라서는 50세 미만(n=706) 66.36%, 50대(n=3,229) 69.37%, 60대(n=6,665) 70.18%, 70대(n=5,946) 74.50%, 80세 이상(n=993) 78.07% 로 연령이 증가할수록 높아지는 것으로 분석되었다. 민감도 85%를 기준으로 살펴보았을 때 절단치 역시 연령에 따라 증가하는 것으로 분석되었으며(50세 미만 4.140, 50대 4.207, 60대 4.512, 70대 5.230, 80세 이상 6.990), 특히 검사정확도의 경우 50세 미만 29.75, 80세 이상 75.23으로 증가 경향이 더 뚜렷하게 나타났다(50세 미만 29.75, 50대 45.28, 60대 54.58 70대 64.98, 80세 이상 75.23).전립선암 진단시 진단기준을 gleason score 7 이상의 임상적으로 유의미한 전립선암으로 변경하여 민감도 분석을 수행한 결과도 유사한 경향으로 나타났다. 전립선암 진단 예측인자 확인을 위한 회귀분석 결과, 연령이 높고, 과거보다 최근에 수행한 생검인 경우, 동반질환이 있는 경우, 전립선암 및 기타 암 가족력이 있는 경우, PSA 수치가 높을수록, 전립선 크기가 작을수록 전립선암 진단이 증가하는 경향을 보였다. 회귀 모델에서 제시된 예측 가능성을 바탕으로 nomogram을 분석하였다. AUC으로 예측 정확성을 평가하였을 때, 전립선암 예측은 PSA 검사를 단독으로 수행하는 것보다 연령, 전립선크기(prostate volume), %fPSA(percentage of free PSA out of total PSA)를 추가했을 때 AUC는 7.37%p 증가하였고 추가 검사로서 MRI, TRUS, DRE 등을 수행하였을 때 AUC는 17.65%p 증가함을 확인하였다.

결론 및 정책적 제언

본 연구를 통해, PSA 검사는 민감도가 높아 전립선암을 진단하는데 있어서 유용한 선별검사임이 확인되었다. 한편 이 선별 검사의 특이도를 높이기 위해서는 연령, free fPSA, 전립선 크기및 기타 진단검사(MRI, TRUS)의 결과를 함께 고려해야한다는 결과도 확인할 수 있었다. 그동안 전립선암 조기검진의 효과와 관련된 연구는 다양하게 이루어져왔으나 전립선암 진단이 필요한 환자들에게 수행하는 대규모 국내 연구는 아직까지 수행된 바 없었다. 본 연구의 결과는, 환자특성을 고려하여 신중하게 생검 여부를 결정함으로써 전립선암 과잉진단이나 생검으로 인한 부작용을 줄여, 의료자원을 효율적으로 사용하고 국민건강을 증진하는데 기여할 수 있다. 더불어 국내 전립선암 검진 가이드라인 개발의 기초자료로 기여할 수 있다. 향후 본 연구의 결과를 확장하여 전립선암 발생 추적, 진단과 치료의 장기간 효과, 생검에 따른 합병증 등 후속 연구가 추가되어야 할 것이다.

Background The prevalence of prostate cancer and the associated mortality have been steadily increasing in South Korea. According to the Cancer Registration Statistics Program, prostate cancer was the third most common cancer among males in 2018, and its prevalence had increased by 200%, from 63.5 cases per 100,000 persons in 2007 to 190.3 cases per 100,000 persons in 2017. The mortality from prostate cancer also had increased by 60%, from 5 cases per 100,000 persons in 2009 to 8 cases per 100,000 persons in 2019. Health insurance claims data show that the number of patients with prostate cancer had increased from 61,973 in 2015 to 96,814 in 2019, and the total medical care expenses had increased from 130 billion won in 2015 to 260 billion won in 2019. As the focus on prostate cancer increases, the number of patients undergoing the prostate-specific antigen (PSA) test, a screening test for prostate cancer, has also increased. PSAs are proteases produced from the epithelial cells and are expressed almost exclusively in the prostate. They are used for the detection, screening, or monitoring of prostate cancer and are useful for assessing the treatment outcomes and detecting postoperative recurrence. Health insurance claims data show a 132% increase in the number of patients who underwent the PSA test from 2010 to 2019. Since the PSA test has high sensitivity and low specificity for prostate cancer, using PSA test universally can lead to an unnecessary biopsy, resulting in overdiagnosis. Hence, United States and Europe guidelines do not recommend to conduct an early prostate cancer screening program for the general population at the national level. The American Cancer Society recommends screening tests annually to men based on PSA with a test value of 2.5 ng/ml or more. For repeated PSA tests value between 2.5 to 4.0 ng/ml, individual risks such as race, family history, age, and abnormal DRE results should be evaluated comprehensively for prostate biopsy. The National Comprehensive Cancer Network (NCCN) recommend to consider conducting prostate biopsy, if the PSA exceeds 3 ng/ml or the finding of Digital Rectal Exam (DRE) test is suspicious. The UK NHS Guidelines recommend that even if the PSA test results have increased, the decision should be made in consideration of life expectancy. Therefore, previous studies have compared the effect of the cut-off used for the detection of prostate cancer, on major outcome variables. Several studies have also examined the possibility of following different cut-off values based on the patient characteristics. The European Randomized Study of Prostate Cancer (ERSPC) monitored the effectiveness of early prostate cancer screening with PSA cut-off 3 ng/ml, and the Prostate, Lung, Colorectal and Ovarian (PLCO) study following a PSA cut-off value of 4 ng/ml, However, the benefits of PSA as a screening test for patients with suspected symptoms of prostate cancer cannot be overlooked, as the test is non-invasive and easy to perform. Thus Instead of universally following a specific cut-off value for all patients, it is necessary to judiciously decide the need for a histological examination based on the patient characteristics such as age and to reduce the rate of overdiagnosis and biopsy-related complications. There is a lack of guidelines about the appropriate PSA cut-offs for patients with suspected prostate cancer in South Korea, and no large-scale study has assessed the accuracy of the PSA test for prostate cancer.

Objective A systematic literature review was conducted to examine the diagnostic accuracy of the PSA test for patients of suspected prostate cancer. In addition, to confirm the usefulness of PSA test for prostate cancer diagnosis through an age-specific performance analysis of PSA test conducted at domestic clinical sites and to present a strategy to increase diagnostic accuracy.

Methods A systematic literature review was conducted to examine the diagnostic accuracy of the PSA test for patients of suspected prostate cancer in the hospital settings. European researchers showed a growing interest in carrying out prostate cancer diagnosis with PSA cut-off less than 4 ng/ml, however there is still lack of researches on this. Therefore, a systematic literature review was performed to examine the diagnostic accuracy of the PSA cut-off less than 4 ng/mL. Studies were searched in two domestic databases (KoreaMed, KMbase) and three international databases (Ovid-Medline, Ovid-Embase, Cochrane library). Two researchers independently reviewed the studies and mutually finalized the selection of studies. For a risk of bias assessment, the Quality Assessment of Diagnostic Accuracy-2 (QUADAS-2) was used. The characteristics and results of the selected studies were qualitatively analyzed and described. A quantitative analysis (meta-analysis) on the sensitivity and specificity was also conducted. The primary endpoints were the metrics of diagnostic accuracy for different PSA cut-off values, including sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, positive predictive value, negative predictive value, diagnostic accuracy, diagnostic odds ratio, and area under the ROC curve (AUC). In the next step, a retrospective cohort consisting of Korean men who underwent a histological examination of the prostate tissue at the department of urology due to lower urinary tract symptoms was created, to collect domestic data regarding the diagnostic accuracy of the PSA test for prostate cancer. The following data from January 1, 2011, to December 31, 2019, were collected: patient information, test information, biopsy complications, and cancer diagnosis. General characteristics were compared between all patients, patients with prostate cancer, patients without prostate cancer, and prostate cancer patients with a Gleason score ≥ 7. To determine the usefulness of the PSA test and the appropriate cut-off value, receiver operating characteristic (ROC) curves were analyzed, and the PSA cut-off values were calculated for different sensitivity levels (95%, 90%, 85%, 80%) and age groups. Additionally, univariate and multivariable logistic regression analyses were performed to identify the predictors of prostate cancer. Odds Ratios (ORs) were presented with their 95% confidence intervals (CI) and the significance of probabilities (p-value). Unadjusted ORs, which are not adjusted for covariates, and adjusted OR adjusted for covariates, were also analyzed.

Results I. Systematic literature review A qualitative analysis and meta-analysis were conducted, including data from the 11 selected studies (9 prospective and 2 retrospective studies) to review the existing findings on the diagnostic accuracy of the PSA cut-off of ≤ 4 ng/mL for prostate cancer in hospital settings. There were various inclusion and exclusion criteria of subject profiles between selected studies. They varied in their criteria for the PSA range and the patients'’ baseline characteristics like age, suspicious symptoms on digital rectal examination (DRE), urinary tract symptoms, history of medication, and past medical history. The studies reported about the diagnostic accuracy of the PSA test with cut-off values of < 4 ng/mL or ≥ 4 ng/mL. Our study extracted the metrics for the diagnostic accuracy of the PSA test for prostate cancer with a cut-off of < 4 ng/mL from the included studies and analyzed the data. In the meta-analysis, the pooled sensitivity of the PSA cut-off range of 2-3.99 ng/mL was 0.92 (95% CI, 0.86-0.95), and the pooled specificity was 0.16 (95% CI, 0.09-0.28). No significant heterogeneity (Bivariate Correlation=-0.979, HSROC beta=0.17, p=0.250) and publication bias (Deeks' Funnel Plot Asymmetry Test, p=0.17) were found. There was almost no difference in the pooled sensitivity and specificity between the PSA cut-off interval of 2-2.99 ng/mL and that of 3-3.99 ng/mL (Table 3-17). The qualitative analysis showed that the sensitivity and specificity for prostate cancer increased and decreased, respectively, and the diagnostic accuracy increased as the PSA cut-off value increased, in most studies. The lack of difference in the diagnostic accuracy between the different PSA intervals (2-2.99 ng/mL and 3-3.99 ng/mL) might be because only 6 and 9 studies used the respective intervals, and the intervals were narrow (1 ng/mL); hence, the outcomes were lacking in power.

II. Retrospective cohort studies (Analysis of the accuracy of domestic PSA tests) Of 22,309 male patients who underwent a histological examination of the prostate due to lower urinary tract symptoms at six general tertiary hospitals between 2011 and 2019, 17,539 satisfying the inclusion and exclusion criteria were included. Of them, 9,808 did not have cancer, and 7,731 patients were diagnosed with cancer. Among those diagnosed with cancer, 5,695 patients had clinically significant prostate cancer (Gleason scores ≥7). The mean age of all patients was 66.33±9.21 years, and the mean PSA level was 39.00±244.22 ng/mL. The mean PSA of the prostate cancer group was 79.24±363.77 ng/mL and that of the non-prostate cancer group was 7.28±8.54 ng/mL. The most common test other than the PSA test for prostate cancer was a prostate ultrasound, which was performed in 79.63% of the patients (n=13,966), followed by the digital rectal examination (48.50%, n=8,506), and magnetic resonance imaging (MRI) (7.08%, n=1,242). Stage IIB was the most common T stage among the patients diagnosed with prostate cancer (21.23%, n=1,641), followed by IIA (19.64%, n=1,518), and III (18.89%, n=1,460). A similar trend was observed in patients diagnosed with clinically significant prostate cancer. In the analysis of the diagnostic accuracy of the PSA test, the test had high sensitivity and low specificity. At a cut-off of 3 ng/mL, the PSA test had a sensitivity of 98.1% and specificity of 8.39%. At a cut-off of 4 ng/mL, its sensitivity and specificity were 92.2% and 22.6%, respectively. The cut-off value at which the PSA test had 90% sensitivity was 3.87 ng/mL for patients aged <50 years, 3.86 ng/mL for those in their 50s, 3.98 ng/mL for those in their 60s, 4.63 ng/mL for those in their 70s, and 5.73 ng/mL for those aged ≥ 80 years. The PSA cut-off value significantly increased after the age of 70. The cut-off value at which the PSA test had 90% sensitivity in the patients with clinically significant prostate cancer was 4.885 ng/mL. On analyzing the diagnostic accuracy of the PSA test by age, the AUC of the PSA test was 73.91% for all patients, 66.36% for those aged <50 years (n=706), 69.37% for those in their 50s (n=3,229), 70.18% for those in their 60s (n=6,665), 74.50% for those in their 70s (n=5,946), and 78.07% for those aged ≥80 years. The diagnostic sensitivity increased as the age increased. The cut-off value at which the diagnostic sensitivity was 85% also increased with age (4.140 for patients aged <50 years, 4.207 for those in their 50s, 4.512 for those in their 60s, 5.230 for those in their 70s, and 6.990 for those aged ≥80 years). The Diagnostic accuracy showed a significant increase from 29.75 for patients aged <50 years to 75.23 for those aged ≥80 years (29.75 for patients aged <50 years, 45.28 for those in their 50s, 54.58 for those in their 60s, 64.98 for those in their 70s, and 75.23 for those aged ≥80 years). A similar trend in sensitivity was observed in patients with clinically significant prostate cancer with Gleason scores ≥7. In the regression analyses to identify the predictors of detection of prostate cancer, the likelihood of detection increased when the patient was older, had recently undergone a biopsy, had comorbidities or a family history of prostate or other cancers, had higher PSA levels, or a smaller prostate. A nomogram was analysed based on the predictors from the regression model. Prediction accuracy was assessed by the AUC analysis. The AUC increased by 7.37%p on adding age, prostate volume, and %fPSA (percentage of free PSA out of total PSA) compared to that when only the PSA test was used. Performing additional tests, including MRI, transrectal ultrasounds (TRUS), and DRE, increased the AUC by 17.65%p.

Conclusions This study demonstrated that the PSA is a useful screening test for prostate cancer due to its high sensitivity like previous studies. To increase the specificity of the PSA test, it is necessary to consider the patient's age, free fPSA, prostate volume, and other screening test results (MRI and TRUS). While several studies have examined the effect of prostate cancer screening for early detection, there has not been any large-scale, domestic study on patients with suspected prostate cancer. The results of this study show that carefully deciding whether to conduct biopsy by considering patient characteristics, can reduce side effects from excessive diagnosis or biopsy of prostate cancer, thereby contributing to efficient use of medical resources and improving national health. These data can also be a basis for the development of domestic prostate cancer screening guidelines. To expand on these findings, additional research on prostate cancer monitoring, diagnosis, and treatment effects is necessary.
