폐경기 호르몬 요법의 성과 연구

Abstract

연구 배경 폐경기 호르몬 요법은 폐경으로 인한 갱년기 증상 치료를 위한 방법으로 사용되어 왔으나 치료에 따른 유익성과 더불어 위험으로 인한 논란이 역사적으로 계속되어 온 치료법이다. 한 때 위험을 강조했던 초기 논란으로 인해 사용이 급격히 감소한 바 있으나 이후 치료법 적용 연령과 기간 등에 대한 표준화된 가이드라인이 마련됨으로써 사용이 증가하고 있다. 그러나 여전히 유익성과 위험에 관한 연구가 심혈관 질환 등 만성질환 예방을 위한 사용이나 일반적인 위험인 유방암 위험 증가 여부에 대하여 서로 다른 방향의 결과를 제시하고 있어 여전히 논의가 지속되고 있는 치료법이기도 하다. 따라서 전문가나 폐경 여성의 치료법에 대한 인식과 사용 양상에 있어서 차이가 발생할 소지가 높다. 따라서 폐경기 호르몬 요법에 대한 논의 및 근거에 대하여 검토하고, 특히 우리 나라에서의 사용 양상과 성과를 파악하며, 아울러 전문가인 의료진과 폐경 여성의 인식에 대해 종합적으로 분석해보는 것은 의미있는 일이다.

연구 목적 본 연구에서는 우리나라에서 이루어지고 있는 폐경기 호르몬 요법 처방 현황을 파악하고 성과를 분석하고자 한다. 아울러 폐경기 호르몬 요법을 처방하는 임상의와 이를 수용하고 폐경의 증상에 대처하는 폐경 여성의 관련 치료에 대한 인식을 함께 조사하고자 한다. 이를 통해 우리나라에서의 폐경기 호르몬 요법의 현황, 성과, 그리고 이에 대한 인식에 대한 타당한 근거를 제시하고자 한다. 이를 위해 본 연구에서는 폐경기 호르몬요법의 성과에 대한 근거와 주요 쟁점을 파악하고, 건강보험 청구자료를 이용하여 폐경기 호르몬 요법의 이용 현황과 성과를 분석하였다. 그리고 의료인의 본 요법 사용과 성과에 대한 인식과 함께 폐경기 여성의 인식을 함께 조사하여 분석하였다.

연구 방법 폐경기 호르몬 요법의 근거 분석은 체계적 문헌고찰로 수행하였다. 주요 요법인 에스트로겐과 프로게스테론 병용요법과 에스트로겐 단독요법 외에도 국내 사용이 상당한 티볼론을 포함하였다. 국내 폐경기 호르몬 요법의 성과 분석은 건강보험 청구자료, 자격자료, 일반건강 검진자료, 국가 암 건강검진자료 및 통계청 사망원인통계자료를 연계하여 수행하였다. 일차적으로 건강보험 이용을 통해 확인 가능한 폐경진단 여성을 파악하고 이들의 폐경기 호르몬 요법 현황을 분석하였다. 성과 분석은 국내 요양기관을 통해 폐경을 진단받은 50세 이상 60세 미만 암검진 수검 여성을 대상으로 코호트를 구축하여 최대 2019년 12월 31일까지 추적하여 사망, 유방암, 대장암 및 심혈관 질환 등을 중심으로 하는 건강결과를 분석하였다. 코호트는 호르몬제 처방받은 여성과 처방받지 않은 미복용군으로 분류하여 위험비를 분석하였다. 호르몬제 복용 여성을 호르몬제 미복용군(이하 미복용군), 에스트로겐 단독군, 에스크로겐 및 프로게스테론 병용군, 티볼론 단독군으로 구분하였다. 폐경기 호르몬 요법의 효과 및 부작용을 확인하기 위한 1차 결과지표(primary outcome)는 유방암, 대장/직장암 및 심근경색증을 발생이며, 2차 결과 지표(second outcome)로 사망, 뇌졸중, 심혈관질환, 심부정맥혈전증 및 폐색전증, 허혈성뇌졸중의 발생을 확인하였다. 사망의 경우 건강보험공단의 사망자료를 이용하여 기록된 사망일자를 발생일로 정의하였다. 1차 결과 지표 및 2차 결과 지표의 경우 통계청의 사망원인 통계자료를 통해 사망원인이 해당 결과 발생의 상병 코드를 가진 경우 추가적으로 정의하여 사망일을 발생일로 정의하였다. 교란요인으로 연령은 자연폐경 연령 45~55세를 기준으로 55세 미만과 이상의 여성으로 범주화 하였고, 소득은 건강보험료 20분위를 4개 범주로 구분하여 정의하였다. 동반질환으로 당뇨, 고혈압, 이상지질혈증 등 만성적인 성인병에 해당하는 질환을 고려하였고 연구대상자의 체질량지수, 흡연, 음주력을 고려하여 분석을 수행하였다. 폐경기 호르몬 요법의 건강 결과에 미치는 영향은 범주형 요인에 대해서는 빈도와 백분율을 산출하여 교차검정(카이제곱검정, chi-sqare test) 또는 비모수 검정인 피셔의 정확검정(Fisher’s exact test) 결과를 제시하였고, 연속형 요인에 대해서는 평균과 표준편차를 산출하여 t-검정(t-test) 또는 비모수 검정인 맨휘트니U검정(Mann-whitney U test) 결과를 제시하였다. 미복용군 및 약물 군(에스트로겐 단독군, 에스트로겐/프로게스테론 병용군, 티볼론 단독군)별 결과지표 발생률은 100인년당 발생률(100 person year rate)로 산출하였다. 1차 결과지표에 대해서 시간에 따른 누적 발생률은 카플란마이어곡선(Kaplan-Meier curves)으로 추정하였다. 미복용군 및 약물 군별 시간 요인을 고려한 발생 위험비는 콕스비례위험모형(Cox proportional hazard model)을 사용하여 미복용군 기준 약물군별 1차, 2차 결과지표의 위험비를 산출하였다. 호르몬제의 경우 증상에 따라 복용하는 경우가 많고 호르몬제 복용 지속 효과는 호르몬제의 복용을 중지하는 시점에서 멀지 않음에 따라 시간 의존성 공변량을 고려한 콕스비례위험모형을 사용하였다. 시간 단위는 주(weekly)로 정의 하였으며, 조작적 정의에 따른 분석 결과의 안정성을 확보하기 위해 기저 시간 단위를 일(daily) 또는 월(monthly), 주 단위 이지만 1일 이상 처방한 경우 처방주간으로 정의를 변경하여 민감도 분석을 수행하였다. 의료진 인식은 국내외 설문조사 결과를 중심으로 검토, 분석하여 구성한 설문문항을 토대로 폐경기 호르몬 요법을 주로 사용하는 산부인과 전문의 약 1000명과 내분비내과, 가정의학과, 노인의학 관련 진료를 수행하는 전문의 약 500명을 대상으로 조사하였다. 폐경 여성 인식 또한 기존 설문조사 연구 논문을 검토, 분석하여 구성한 설문서를 토대로 만 40세 ~ 69세의 폐경여성 약 1,000명을 대상으로 모바일을 이용한 온라인 조사로 수행하였다.

연구 결과 1. 폐경기 호르몬 요법 국내 현황 국민건강보험 청구 데이터베이스의 의료이용내역을 통해 2009년 1월 1일에서 2020년 12월 31일까지 국내 40세 이상 폐경 10년 내외 여성은 누적 16,504,878명으로 이 중 호르몬제를 처방받은 여성은 누적 2,779,488명으로 전체의 16.8%로 확인되었다. 연도별로 폐경으로 진단되는 여성은 7.1~8.9%이며 이중 호르몬제를 처방받는 경우는 54%~66% 정도이다. 최초로 호르몬제 처방을 받는 곳은 의원이 약 66%로 대부분을 차지하였고 병원 15%, 종합병원 12%, 상급종합병원 6%이며, 호르몬제 처방건을 기준으로 하면 의원 약 75%, 병원 12%, 종합병원 9%, 상급종합병원 4%를 차지하였다. 호르몬제 처방 연령은 40대 비중이 줄고 60대의 처방비중이 증가하는 양상을 보여 2020년 기준 40대 18%, 50대 52%, 60대 25%, 70대 5%였다. 처방약물은 티볼론이 2009년 30.5%로부터 2020년 45.8%로 증가하였고 에스트로겐/프로게스테론 병용은 39.0%로부터 32.5%로, 에스토로겐 단독은 23.4%에서 15.3%, 프로게스테론 단독은 6.8%에서 6.1%로 감소하였다.

2. 성과 분석 유방암: 미복용군 기준으로 에스트로겐 단독군에서 발생 위험비가 0.83(95% CI:(0.77, 0.90)), 티볼론 단독군에서 발생 위험비가 0.90(95% CI: (0.84, 0.96))으로 통계적으로 유의하게 낮았다. 에스트로겐/프로게스테론 병용군 발생 위험비의 경우 0.97(95% CI: (0.91, 1.03))이었다. 대장/직장암: 미복용군 기준 에스트로겐 단독군 1.01(95% CI: (0.9, 1.15)), 에스트로겐/프로게스테론 병용군 1.07(95% CI: (0.97, 1.19)), 티볼론 단독군 1.05(95% CI: (0.95, 1.17))이나 통계적 유의성이 없었다. 심근경색증: 미복용군 기준 에스트로겐 단독군에서 1.08(95% CI: (1.03, 1.14)), 티볼론 단독군에서 1.11(95% CI: (1.06, 1.17))로 통계적으로 유의하게 위험비가 높았고 에스트로겐/프로게스테론 병용군은 통계적 유의성이 없었다(HR: 1.02, 95% CI: (0.97, 1.07))(표 32). 사망: 호르몬제 복용군에서 사망의 위험비가 통계적으로 유의하게 감소하하였다(에스트로겐 단독군: HR=0.93, 에스트로겐/프로게스테론 병용군: HR=0.89, 티볼론 단독군: HR=0.91). 뇌졸중 및 허혈성 뇌졸중, 혈전/색전증, 심혈관 질환: 에스트로겐/프로게스테론 병용군의 건강 결과 발생의 위험비는 통계적 유의성이 없고 에스트로겐 단독군의 경우 뇌졸중의 발생 위험비 1.09(95% CI: (1.04, 1.15)), 허혈성 뇌졸중의 발생 위험비 1.11(95% CI: (1.04, 1.17))로 통계적으로 유의하게 높았고, 티볼론 단독군에서도 유의하게 높았다(뇌졸중: HR=1.08, 허혈성 뇌졸중: HR=1.10). 혈전/색전증: 에스트로겐 단독군에서는 에스트로겐/프로게스테론 병용군과 유사하게 통계적 유의성이 나타나지 않았지만, 티볼론 단독군에서는 발생 위험비 1.07(95% CI: (1.03, 1.11))로 통계적으로 유의하게 위험비가 높은 것으로 확인되었다. 심혈관질환: 미복용군 기준 에스트로겐 단독군, 티볼론 단독군에서 통계적으로 유의하게 발생 위험비가 높은 것으로 확인되었다(에스트로겐 단독군: HR=1.07, 티볼론 단독군: HR=1.09). 1년 이내 심근경색증, 혈전/색전증, 혀혈성 뇌졸중 발생 위험비: 미복용군 기준 에스트로겐 단독군의 발생 위험비가 2.82(95% CI: (2.34, 3.4)), 에스트로겐/프로게스테론 병용군의 발생 위험비 2.89(95% CI: (2.47, 3.37)), 티볼론 단독군의 발생 위험비 3.44(95% CI: (2.96, 3.99))로 통계적으로 유의하게 위험비가 높게 확인 되었다. 이러한 경향은 혈전/색전증(에스트로겐 3.09, 에스트로겐/프로게스테론 3.24, 티볼론 3.87), 허혈성 뇌졸중(에스트로겐 3.03, 에스트로겐/프로게스테론 2.59, 티볼론 3.17)에서도 유사하게 확인되었다.

3. 전문가 인식조사 폐경기호르몬요법의 필요성: 폐경증상의 정도와 상관없이 사용(21.7%) 및 폐경기 여성의 일반적인 사용 필요(26.6%) 등 적극적인 사용의사 표명이 위험이 있으나 폐경 증상이 심한 사람을 대상으로 시행할 수 있다(48.0%)에 필적하는 수준으로 높았다. 적극적 사용 의사는 여성보다 남성에서 높고 연령이 증가하고 진료기간이 길수록, 산부인과 전문의일수록, 의원 근무자일수록 높아졌다. 폐경기호르몬요법 이익의 근거 수준: ‘안면 홍조 및 발한 완화’(5.98점), ‘질 위축 완화’(5.56점), ‘골다공증 치료 및 예방’(5.51점), ‘우울증 완화’ (5.33점), ‘수면장애 완화’(5.30점)에서 높았고 ‘재발 심근경색 위험을 낮춤’(3.97점)’, ‘주름예방’(4.01점), ‘알츠하이머 치매 예방’(4.03점)에서 낮게 조사되었다. 산부인과 외 전문의의 경우 근거수준을 상대적으로 낮게 인식하였다. 폐경기호르몬요법 위험의 근거 수준: ‘유방암 위험을 높임’(3.44점), ‘정맥혈전증 발생’(3.25점), ‘체중 증가’(3.24점), ‘양성 유방질환 증가’(3.22점)으로 이익에 비해 근거 수준을 낮게 평가하였다. 여성과 30대, 산부인과 외 전문의, 대학병원 근무자에서 상대적으로 근거 수준을 높게 평가하였다. 폐경기호르몬요법 처방 동의 정도: ‘안면 홍조 및 발한 완화’(6.12점), ‘질 위축 완화’(5.57점), ‘골다공증 치료 및 예방’(5.45점), ‘우울증 완화’(5.40점) 순으로 높았고, ‘재발심근경색의 위험을 낮춤’(3.83점), ‘주름 예방’(3.83점), ‘알츠하이머 치매 예방’(3.88점), ‘대장암의 위험을 낮춤’(3.89점)에서 상대적으로 낮게 조사되었다. 여성보다는 남성에서 높고, 50대 이상 연령에서 높았으며, 산부인과 전문의, 그리고 병원과 의원 근무자에서 처방 동의 정도가 높았다. 폐경기호르몬요법 시작 시점: ‘폐경 이행기’(51.4%), ‘폐경 후 5년 이하’(34.6%), ‘폐경 여부와 상관없이 환자 요구가 있을 때’(11.8%) 순으로 조사되었다. ‘환자 요구가 있을 때’의 응답비율은 여성보다 남성, 그리고 의원에서 높은 경향이었다. 폐경기호르몬요법 종료 필요성: 종료가 필요하다(55.5%)이며 ‘특정 연령이 되면 중단’(29.4%)보다 ‘치료 개시 후 일정 기간이 되면 중단’(70.6%)였다. 종료 필요성에 대한 동의는 여성, 30대, 산부인과 외 전문의, 대학병원 근무자에서 높게 나타났다. 반대로 ‘효과가 있다면 종료 필요없이 지속하는 것이 타당하다’ 응답은 남성, 60대 이상, 산부인과 전문의, 의원 근무자에서 높았다. 폐경기호르몬요법 처방 약제: 티볼론(40.3%), 에스트로겐/프로게스틴 복합요법(38.6%), 에스트로겐 단록(13.3%) 순으로 조사되었다.

4. 폐경기 여성 인식조사 응답자: 총 1,100명으로 40대 34.7%, 50대 36.6%, 60대 28.7%가 조사되었다. 폐경기증상 및 대처: 매우 심함 또는 심함으로 응답한 비율은 수면 문제(38.1%), 피로감(35.54%)에서 높았고 안명홍조는 31.0%였다. 폐경기 대처로는 건강기능식품 또는 영양제(38.9%), 일반식품(19.7%), 건강보조식품(6.9%), 한약(5.3%) 순이었다. 또한 폐경으로 인해 경험하는 신체적 변화에 적극적인 치표가 필요하다는 인식에는 81.2%가 동의하였다. 폐경관련정보: 충분하지 않다는 인식이 62.5%였으며 폐경 관련 정보 습득은 대체로 지인 등(25.7%). 인터넷 등(25.3%), TV/라디오(18.3%)였고 의료기관의 진료와 상담은 15.0%였다. 의료기관 방문 경험: 폐경기 증상으로 인한 의료기관 방문 경험은 42.6%였고, 방문기관 유형은 산부인과 전문병원이 전체의 절반 정도를 차지하였다. 호르몬요법인식: 인지도는 80.4%로 높았고 효과와 위험에 대한 인지도 62.7%였다. 호르몬요법 처방 경험은 331명으로 전체 조사대상의 30.0%였고 의료기관 방문경 험자의 70.6%였다. 처방을 받지 않은 이유는 암 발생 등 질병 부담(31.9%), 증상 경미(27.5%). 의사가 권유하지 않음(23.9%)였다. 호르몬요법 이용 기간은 6개월 미만이 45.0%, 6개월~1년미만이 26.3%, 1년~5년미만이 21.8%, 5년~10년 미만이 5.1%, 10년 이상이 1.8%로 조사되었다. 호르몬 요법 중단 사유는 증상 호전(36.6%), 효과 없어서(25.5%), 부작용(22.2%), 복용 불편(9.9%), 장기복용/부작용 우려(3.3%) 순이었다. 호르몬요법에 대한 만족은 60.6%이었으나 타인에 대한 추전 의향은 비추천이 52.3%를 차지하여 추천 15.1%에 비해 높았다.

결론 본 연구는 폐경기 호르몬 요법과 관련하여 우리나라의 대상 인구 및 건강보험 처방 현황에 대해 종합적으로 분석 제시하였다. 의사 1,501명과 폐경기 여성 1,100명을 대상으로 인식조사를 수행하여 이를 함께 제시하고 달라진 사회적 인식을 확인하였다. 건강보험 청구자료를 이용한 성과 분석 결과 임상적으로 관심이 큰 1차 건강효과인 유방암 위험도가 에스트로젠 단독, 티볼론군에서 호르몬제 미처방군에 비해 낮은 결과가 나타났다. 2차 건강결과 효과인 사망률은 병용군과 티볼론 군에서 유의하게 낮았으나, 심혈관질환 관련 2차 건강결과는 대체로 위험비가 높게 나타났고 심혈관 질환 관련 1차 건강결과의 추적관찰 1년 이내 위험비는 상당히 높게 나타났다. 다만, 해당 결과는 대상자를 폐경 증상으로 진단받고 치료한 여성(50대 이상의 여성만 포함)에 한정하여 폐경여성의 결과로 일반화하기 어려운 제한점이 있다. 폐경기 호르몬 요법에 대한 전문가 인식은 대체로 긍정적인 것으로 조사되었고 이는 연령 증가에 따라 높아지는 경향을 보였다. 폐경기 호르몬 요법의 이익에 대한 근거 수준 평가에 비해 위험에 대한 근거 수준 평가가 상대적으로 낮은 경향과 유사하게 나타나고 있어서 근거에 대한 이해와 이에 따른 의사 결정 노력이 상대적으로 취약함을 보여주고 있어 이에 대한 전문가적 노력이 더 강화되어야 할 필요성이 확인되었다. 폐경기 호르몬 요법에 대한 폐경 여성의 인식도 긍정적으로 나타났다. 폐경 증상의 치료 필요성에 대한 동의가 높고 의료기관 방문 비율 및 호르몬 요법 인지도, 경험 비율이 상당한 것으로 판단되었다. 치료법 만족도도 높으며 위험이 있지만 치료를 위해 필요하다고 인식하였으나, 다만 위험에 대한 우려로 6개월 미만 단기 사용이 많았음 또한 확인되어 보다 세심한 정보 제공이 필요한 것으로 파악되었다.

Background Menopausal hormone therapy (MHT) has been used as a method for treating menopausal symptoms, but the benefits and risks of this treatment method have historically been controversial. At one time, the use of MHT decreased sharply due to early controversy that emphasized its risks, but its use has subsequently increased with the establishment of standardized guidelines on applicable age and period. However, the debate on its benefits and risks continues on with various studies reporting conflicting results on it use for prevention of chronic diseases, such as cardiovascular disease (CVD), and common risk of increasing the risk of breast cancer. Therefore, there is a high likelihood of differences in awareness and dosage of this treatment method among experts or menopausal women. Accordingly, it is meaningful to review the discussions and evidence regarding MHT; identify the usage pattern and outcomes in Korea; and comprehensively analyze awareness among healthcare professionals and menopausal women.

Objective This study aimed to analyze the prescriptions of MHT in Korea and outcomes associated with MHT. In addition, the study also aimed to investigate awareness about the treatment among clinicians who prescribe MHT and menopausal women who accept the prescription and cope with menopausal symptoms. Based on the findings, the study aimed to present valid evidence on the current status, outcomes, and awareness of MHT in Korea. For this, this study identified major issues and evidence regarding MHT outcomes and used health insurance claims data to analyze the usage status and outcome of MHT. In addition, the study also investigated awareness about the usage and outcomes of this therapy among healthcare professionals, along with awareness among menopausal women.

Methods A systematic review was performed to analyze the evidence of MHT. In addition to estrogen-progesterone(E+P) combination therapy and estrogen-alone(E) therapy, tibolone(T) therapy, which is used extensively in Korea, were included in the study. The analysis of outcomes of MHT in Korea was performed by linking health insurance claims data, health insurance eligibility data, regular health checkup data, national health screening data, and cause of death statistics from Statistics Korea. Firstly, women diagnosed with menopause identifiable through health insurance data were identified and their MHT status was analysis. For the outcomes analysis, a cohort was established with women aged 50-59 years diagnosed with menopause through nursing institutions who were screened for cancer. The women were follow-up up to December 31, 2019 and their health outcomes were analyzed mostly based on death, breast cancer, colorectal cancer, and CVD. The cohort was divided into women who were prescribed hormone therapy (HT groups) and those who were not (non-HT group) to analyze their hazard ratio (HR). The women were divided into non-HT group (those who did not receive HT), E group (those receiving estrogen-alone therapy), E+P group (those receiving estrogen-progesterone combination therapy), and T group (those receiving tibolone therapy). To investigate the effect and adverse events (AEs) of MHT, incidence of breast cancer, colorectal cancer, and myocardial infarction was investigated as the primary outcome, while incidence of death, stroke, CVD, deep vein thrombosis (DVT), pulmonary embolism, and ischemic stroke was investigated as the secondary outcome. Incidence of death was defined as the date of death recorded in the mortality data from the National Health Insurance Service. For primary and secondary outcomes, if the cause of death statistics from Statistics Korea listed the applicable disease code as the cause of death, the date of incidence was defined as the date of death. As confounding factors, age was categorized into < 55 and ≥ 55 years based on 45-55 years as the natural menopausal age, while income was defined into four categories based on 20 health insurance premium quantiles. For comorbidities, chronic adult diseases, such as diabetes, hypertension, and dyslipidemia, were considered, while the analysis was performed with consideration for the body mass index, smoking status, and drinking status of the subjects. With respect to influence on health outcomes of MHT, frequency and percentage were derived for categorical factors and analyzed by chi-squared test or non-parametric Fisher’s exact test, while mean and standard deviation (SD) were derived for continuous variables and analyzed by t-test or non-parametric Mann-Whitney U test. The incidence of outcomes in the non-HT group and HT groups (E, E+P, and T groups) was calculated as 100 person-year rate. With respect to the primary outcome, cumulative incidence over time was estimated by Kaplan-Meier curves. For the HR of incidence consider time factor for non-HT and HT groups, HR of primary and secondary outcomes in HT groups, relative to non-HT group, was calculated using Cox proportional hazard model with consideration of time-dependent covariates since hormones are often taken according to symptoms and sustained effect of hormones, even if it has stopped to take, it would affect for a certain period of time. The time unit was defined as weekly. To assure stability of analysis results according to the operational definition, baseline time unit that was daily or monthly or weekly but prescribed for more than one day was changed to weekly to perform sensitivity analysis. For awareness among healthcare professionals, questionnaire items were constructed based on review and analysis of domestic and foreign questionnaire survey results. The questionnaire was used to survey approximately 1,000 obstetricians and gynecologists who primarily use MHT and approximately 500 specialists from endocrinology, family medicine, and geriatrics. For awareness among menopausal women, questionnaire items were also constructed based on review and analysis of existing survey study results. The questionnaire was used to conduct an online survey, approximately 1,000 menopausal women aged 40-69 years.

Results 1. Current status of MHT in Korea According to the healthcare utilization records in the national health insurance claims database, the total number of Korean women aged ≥ 40 years with at least 10 years since menopause between January 1, 2009 and December 31, 2020 was 16,504,878. Among them, 2,779,488 women (16.8%) had been prescribed HT. Each year, 7.1%–8.9% of women were diagnosed with menopause, of whom, 54%-66% were prescribed HT. With respect to the place that prescribed HT for the first time, clinic was the most common response with approximately 66%, followed in order by hospital (15%), general hospital (12%), and tertiary hospital (6%). With respect to number of prescriptions for HT, clinic, hospital, general hospital, and tertiary hospital accounted for approximately 75%, 12%, 9%, and 4%, respectively. With respect to the age of women who were prescribed HT, the percentage of those aged 40-49 years decreased, while percentage of those aged 60-69 years increased. As of 2020, the percentage of those aged 40-49, 50-59, 60-69, and 70-79 years was 18%, 52%, 25%, and 5%, respectively. With respect to hormones prescribed, tibolone increased from 30.5% in 2009 to 45.8% in 2020, whereas estrogen/progesterone combination decreased from 39.0% to 32.5%, estrogen alone decreased from 23.4% to 15.3%, and progesterone alone decreased from 6.8% to 6.1%.

2. Outcome analysis Breast cancer: Relative to non-HT group, HR of incidence was statistically significantly lower in E group with 0.83 (95% CI: (0.77, 0.90)) and T group with 0.90 (95% CI: (0.84, 0.96)). Meanwhile, HR of incidence in E+P group was 0.97 (95% CI: (0.91, 1.03)). Colorectal cancer: Relative to non-HT group, HR of incidence in E, E+P, and T groups was 1.01 (95% CI: (0.9, 1.15)), 1.07 (95% CI: (0.97, 1.19)), and 1.05 (95% CI: (0.95, 1.17)), respectively, showing no statistical significance. Myocardial infarction: Relative to non-HT group, HR of incidence was statistically significantly higher in E group with 1.08 (95% CI: (1.03, 1.14)) and T group with 1.11 (95% CI: (1.06, 1.17)), while there was no statistically significant difference in E+P group (HR: 1.02, 95% CI: (0.97, 1.07)) (Table 32). Death: There was a statistically significant decrease in HR of death in HT groups (E group: HR=0.93, E+P group: HR=0.89, T group: HR=0.91). Stroke/ischemic stroke, thrombosis/embolism, and CVD: E+P group showed no statistically significant HR of incidence of health outcome. Meanwhile, E group showed statistically significantly higher HR of stroke with 1.09 (95% CI: (1.04, 1.15)) and ischemic stroke of 1.11 (95% CI: (1.04, 1.17)), while T group showed significantly higher HR (stroke: HR=1.08, ischemic stroke: HR=1.10). Thrombus/embolism: E group did not show statistical significance, similar to E+P group, but T group showed statistically significantly higher HR of incidence with 1.07 (95% CI: (1.03, 1.11)). CVD: Relative to non-HT group, E and T groups showed statistically significantly higher HR of incidence (E group: HR=1.07, T group: HR=1.09). HR of incidence of myocardial infarction, thrombosis/embolism, and ischemic stroke within 1 year: Relative to non-HT group, E, E+P, and T groups showed statistically significantly higher HR of incidence with 2.82 (95% CI: (2.34, 3.4)), 2.89 (95% CI: (2.47, 3.37)), and 3.44 (95% CI: (2.96, 3.99)), respectively. Such trend appeared similarly for thrombosis/embolism (E group: 3.09, E+P group: 3.24, T group: 3.87) and ischemic stroke (E group: 3.03, E+P group: 2.59, T group: 3.17).

3. Expert awareness survey Necessity of MHT: Expression of intention for active use, such as use regardless of severity of menopausal symptoms (21.7%) and need for general use by menopausal women (26.6%) could be risky, but they were almost equivalent to use on people with severe menopausal symptoms. Intention for active use was higher among men than women, as well as among those who are older, have longer clinical experience, obstetricians and gynecologists, and those who work in clinics. Level of evidence for benefits of MHT: Scores were high for “alleviation of facial flushing and sweating” (5.98 points), “alleviation of vaginal atrophy” (5.56 points), “treatment and prevention of osteoporosis” (5.51 points), “alleviation of depression” (5.33 points), and “alleviation of sleep disturbance” (5.30 points), whereas the scores were low for “lowering the risk of recurrent myocardial infarction” (3.97 point), “prevention of wrinkles” (4.01 points), and “prevention of Alzheimer’s disease and dementia” (4.03 points). Obstetricians and gynecologists tended to perceive the level of evidence to be relatively low. Level of evidence for risks of MHT: Scores for “increased risk of breast cancer,” “incidence of venous thrombosis,” “weight gain,” and “increase d risk of benign breast disease” were 3.44, 3.25, 3.24, and 3.22 points, respectively. The level of evidence was assessed to be lower than that of benefits. Women, those aged 30-39 years, obstetricians and gynecologists, and those who work in university hospital tended to assess the level of evidence to be relatively high. Level of agreement with MHT prescription: Scores appeared in the highest to lowest order of “alleviation of facial flushing and sweating” (6.12 points), “alleviation of vaginal atrophy” (5.57 points), “treatment and prevention of osteoporosis” (5.45 points), and “alleviation of depression” (5.40 points). Meanwhile, scores for “lowering the risk of recurrent myocardial infarction” (3.83 point), “prevention of wrinkles” (3.83 points), “prevention of Alzheimer’s disease and dementia” (3.88 points), and “lowering the risk of colorectal cancer” (3.89 points) were relatively low. Level of agreement with MHT prescription was higher in males, those aged ≥ 50 years, obstetricians and gynecologists, and those who work in hospitals and clinics. Start of MHT: The results showed an order of “during menopause” (51.4%), “within 5 years after menopause” (34.6%), and “as requested by the patient regarding of menopause” (11.8%). Males and those who work in clinics tended to show higher response for “as requested by the patient.” Need for discontinuation of MHT: The results showed “discontinuation is necessary” (55%), with higher response for “discontinuation after a certain period after the start of therapy” (70.6%) than “discontinuation at a specific age” (29.4%). Females, those aged 30-39 years, specialists other than obstetricians and gynecologists, and those who work in university hospitals showed higher agreement with the need for discontinuation. In contrast, males, those aged ≥ 60 years, obstetricians and gynecologists, and those who work in clinics tended to show higher response for “valid to continue without discontinuation as long as it remains effective.” MHT prescribed: The results appeared in the order of tibolone therapy (40.3%), estrogen/progesterone combination therapy (38.6%), and estrogen-alone therapy (13.3%).

4. Menopausal women awareness survey Respondents: A total of 1,100 women were survey with 34.7%, 36.6%, and 28.7% of the respondents aged 40-49, 50-59, and 60-69 years. Menopausal symptoms and coping: Responses of “very severe” or “severe” tended to be high for sleep problems (38.1%), fatigue (35.54%), and facial flushing (31.0%). With respect to coping with menopause, the results appeared in the order of health functional food or nutritional supplement (38.9%), regular food (19.7%), health supplements (6.9%), and herbal medicine (5.3%). Moreover, 81.2% agreed that there is the need for active indicators for physical changes experienced due to menopause. Menopause-related information: Most of the respondents indicated that there is not enough information (62.5%), while they were acquiring menopause-related information through acquaintances (25.7%), the Internet (25.3%), and TV/radio (18.3%), with only 15.0% acquiring such information through consultation and treatment at a medical institution. Experience of visiting medical institutions: Experience of visiting medical institutions for menopausal symptoms was 42.6%, with hospitals specializing in obstetrics and gynecology accounting for approximately half. Awareness of HT: Overall awareness was high with 80.4%, while the awareness of benefits and risks was 62.7%. A total of 331 respondents (30.0%) had experience with HT prescription, while 70.6% had experience visiting a medical institution. The reasons for not being prescribed HT included burden of disease, such as cancer (31.9%), mild symptoms (27.5%), and not recommended by a physician (23.9%). The duration of HT consisted of < 6 months (45.0%), ≥ 6 months but < 1 year (26.3%), 1 to < 5 years (21.8%), 5 to < 10 years (5.1%), and ≥ 10 years (1.8%). The reasons for discontinuing HT including symptom improvement (36.6%), no efficacy (25.5%), AEs (22.2%), inconvenience (9.9%), and concern for long-term use/AEs (3.3%). Satisfaction with HT was 60.6%, but 52.3% would not recommend HT to others, as compared to 15.1% who would.

Conclusions This study comprehensively analyzed and presented the findings on the health insurance prescription status and target population in Korea in relation to MHT. Awareness surveys were conducted on 1,501 healthcare professionals and 1,100 menopausal women and the findings were presented to identify changes in social awareness. In the outcomes analysis using health insurance claims data, the risk of breast cancer, which was the primary outcome with significant clinical interest, was lower in E and T groups, as compared to non-HT group. Mortality rate, which was a secondary outcome, was significantly lower in E+P and T groups, but secondary health outcome related to CVD showed a mostly high HR, while the primary health outcome related to CVD showed significantly high HR within 1 year of follow-up. However, such findings were limited to women (only women aged ≥ 50 years) who were diagnosed with menopause and received treatment for menopause, and thus, there limitations in generalizing the findings for all menopausal women. Expert awareness about MHT was mostly positive, which tended to increase with age. Compared to the assessment of the level of evidence for the benefits of MHT, the assessment of the level of evidence for the risks of MHT appeared similar to the benefits. This indicated that understanding of the evidence and relevant decision-making efforts are relatively vulnerable, which confirmed the need to strengthen professional efforts for this. Menopausal women showed positive awareness of MHT. The level of agreement about the need for treating menopausal symptoms was high, while the percentage of visits to medical institutions, awareness of HT, and percentage of experience were also significant. Satisfaction with therapy was also high and women recognized the need for treatment despite some risks. However, concerns about the risks were high for short-term use of < 6 months, which indicated the need to provide more details information.
