경피적 경막외강 신경성형술

Abstract

평가배경 경피적 경막외강 신경성형술은 척추성 통증 환자를 대상으로 통증 감소를 위해 수행되는 기술로 경피적으로 신경성형술용 카테터를 경막외강으로 진입시켜 경막외강 병변부위의 유착을 박리하고 약물을 투여하는 시술이다. 건강보험 보장성 강화대책이 발표된 후(2017년 8월), 2020년에는 ‘척추·근골격계·통증’분야에 대해 의학적 비급여의 급여화를 추진하게 되면서 신경성형술(내시경적 경막외강 신경근 성형술, 경피적 경막외강 신경성형술, 경피적 풍선확장 경막외강 신경성형술)이 비급여 규모가 크고 급여 전환시 다양한 쟁점이 제기될 것으로 예측됨에 따라 급여 전환 타당성 검토의 필요성이 제시된 바 있다. 이후 보건복지부는 척추 관련 기존 행위 4개 항목(추간판내 고주파 열치료술, 내시경적 경막외강 신경근성형술, 경피적 경막외강 신경성형술, 경피적 풍선확장 경막외강 신경성형술)에 대한 재평가를 한국보건의료연구원에 의뢰하였다(2020년 1월). 이에 따라 경피적 경막외강 신경성형술에 대해 2020년 제2차 의료기술재평가위원회(2020.2.14.)에서 평가계획서 및 소위원회 구성안을 심의한 후 임상적 안전성 및 유효성을 평가하였다.

평가 방법 본 평가는 척추성 통증환자를 대상으로 경피적 경막외강 신경성형술이 임상적으로 안전하고 유효한지 평가하기 위해 체계적 문헌고찰을 수행하였다. 체계적 문헌고찰은 위의 핵심질문을 토대로 국내 데이터베이스 5개(KoreaMed, 의학논문데이터베이스, 학술데이터베이스, 한국교육학술정보원, 과학기술정보통합서비스) 및 국외 데이터베이스 3개(Ovid MEDLINE, Ovid EMBASE, Cochrane Central Register of Controlled Trials)을 이용하여 문헌검색하였으며, 문헌 선정기준 및 배제기준 적용 및 자료추출, 비뚤림 위험평가는 모두 2명의 평가자가 독립적으로 수행하였다. 비뚤림위험 평가는 연구유형에 따라 무작위연구는 Cochrane의 Risk of Bias, 그 외에 비무작위연구는 Risk of bias for nonrandomized studies (RoBANS) 2.0 국문판을 사용하였고, 문헌의 근거수준을 평가하기 위해 Grading of Recommendations Assessment, Development and Evaluation (GRADE)를 사용하였다. 소위원회의 검토의견을 고려하여 의료기술재평가위원회에서 최종 심의 후 권고등급을 제시하였다. 본 평가에서는 비교치료법의 경우 중재시술의 효과를 평가하기 위해 기존 급여권에서 활용되고 있는 치료법을 적절한 치료법으로 보고 해당 비교치료법(위약대조군 포함)과 비교된 연구만 선택문헌에 포함하여 평가하였다. 본 평가를 위해 총 10인(마취통증의학과 2인, 신경외과 2인, 정형외과 2인, 영상의학과 1인, 재활의학과 1인, 근거기반의학 2인)으로 구성된 소위원회에서 체계적 문헌고찰을 통해 동 기술의 안전성 및 유효성에 대한 문헌적 근거를 평가하였고, 2020년 제12차 의료기술재평가위원회(2020.12.11.)에서 경피적 경막외강 신경성형술의 임상적 안전성 및 유효성 평가 결과에 대해 최종심의하였다.

평가 결과 평가에 선택된 문헌은 총 11편으로 보존적 치료(2편), 위약(1편), 경막외강 신경차단술(8편)과 비교되었으며, 대상환자 특성으로는 경추 추간판 탈출증으로 인한 경추 통증 환자(1편), 추간판 돌출 또는 척추수술후 증후군으로 인한 만성 요추 신경근통증 환자 혹은 척추수술후 증후군로 인한 만성 하지통 환자(4편), 척추 협착증으로 인한 난치성 통증 환자 혹은 척추 협착증으로 인한 만성 신경근 통증 환자(2편), 만성 요통 및 좌골신경통 혹은 하지통 환자(3편), 추간판 탈출증으로 인한 방사통 환자(1편)가 포함되었다. 만성 요통 및 하지통 환자를 대상으로 수행된 경피적 경막외강 신경성형술은 총 10편의 문헌을 근거로 평가되었으며, 연구결과는 다음과 같다. 안전성은 경피적 경막외강 신경성형술을 수행한 그룹(이하 ‘중재군’)에서 카테터 파열, 경막내 위치, 지주막하 진입 등의 합병증이 2 ~ 11.67%로 확인되었으며, 그 외에 종창, 발적, 가려움증, 일시적 감각결손 등은 6.5 ~ 91.3%까지 보고되었다. 이에 소위원회에서는 종창, 발적, 가려움증, 일시적 감각결손 등의 합병증은 일시적이거나 해결가능한 합병증으로 수용가능하다는 의견이었다. 다만, 카테터 파열, 카테터 경막내 위치, 지주막하 진입 등의 합병증과 관련해서 카테터가 척추강내로 진입된 상태에서 약물이 투여되면 심각한 합병증을 야기시킬 수 있다는 의견으로 현재 선택문헌에서는 조영제를 투여하면서 경막외강 조영술을 통해 카테터의 위치를 확인한 후에 시술하였고, 카테터가 경막외강 외에 다른 부위로 진입되었을 경우 바로 카테터를 제거한 것으로 보여져 위험한 상황을 야기하지 않았기 때문에 관련 합병증에 대해 문헌에서는 간단하게 언급한 것으로 판단되며, 동 시술은 위험도가 높은 기술이므로 상당한 주의와 감독하에 수행하고 위험 상황에서 적절히 대처하면 안전성은 수용가능하다는 의견이었다. 유효성은 중재군이 보존적 치료, 위약군, 경막외강 신경차단술과 비교시 통증 및 기능에 있어 유의하게 개선된 효과를 보고하였다. 다만 선택문헌 대부분 탈락률이 높거나 맹검해제된 환자비율이 높았고, 중재군과 비교군간 동반치료에 대한 명확한 설명이 제시되어 있지 않거나 해당 치료효과에 대한 보정을 어떻게 했는지에 대한 설명이 제시되어 있지 않았으며, 일부 연구에서는 중재군에서 주사를 1회 이상 투여했거나 혹은 주사횟수에 대해 명확히 설명이 제시되어있지 않아 비뚤림위험이 높게 평가되어 근거수준은 ‘낮음’으로 평가되었다. 이에 소위원회에서는 만성 요통 및 하지통 환자에서 일관적으로 중재시술이 통증 및 기능개선에 있어 더 효과적인 연구결과를 확인할 수 있으나 근거수준이 낮게 평가되어 근거에 대한 확신이 제한적일 수 있다는 의견이었다. 경추통증 환자 대상에서 경피적 경막외강 신경성형술은 경막외강 신경차단술과 비교한 1편의 문헌을 근거로 평가하였다. 안전성 결과 두 그룹 모두 합병증은 없는 것으로 보고되었으며, 유효성 결과 통증개선정도는 12개월 추적관찰시 중재군이 유의하게 더 높은 것으로 보고되었고, 기능개선정도는 6개월 시점에서는 중재군이 더 유의하게 높았으나 12개월 시점에서는 군간 유의한 차이가 없는 것으로 보고되었다. 이에 소위원회에서는 경추통증환자에서는 선택문헌이 1편으로 동 기술의 안전성 및 유효성을 판단할 수 있는 근거가 부족하여 평가할 수 없다고 판단하였다.

결론 경피적 경막외강 신경성형술 소위원회는 현재 평가 결과에 근거하여 다음과 같이 제언하였다. 경피적 경막외강 신경성형술은 만성 요통 및 하지통 환자를 대상으로 수행시 동 기술이 시술 수행에 따른 위험도가 높은 기술임을 고려하여 상당한 주의와 감독하에 수행할 필요가 있으며, 유효성은 기존의 보존적 치료 및 경막외강 신경차단술과 비교시 통증 개선 및 기능 개선에 있어 일관되게 유의한 효과를 확인할 수 있어 해당 의료기술을 임상에서 사용하는데는 문제가 없다는 의견이었다. 다만 그 근거수준이 제한적임을 고려하여 권고 강도를 높이기 위해 잘 설계된 연구를 통한 근거축적은 지속적으로 더 필요하다고 판단하였다. 경추통증 환자를 대상으로 수행시에는 선택문헌이 1편으로 안전성 및 유효성을 평가하기 위해서는 근거가 부족해 평가하기 어려우며, 추후 동 기술의 평가를 위해 경막외강 신경차단술과 같은 급여권내에 있는 기술과 비교한 잘 설계된 연구 결과를 통한 근거 축적이 더 필요하다고 판단하였다. 의료기술재평가위원회는 ‘경피적 경막외강 신경성형술’에 대해 소위원회의 검토결과에 근거하여 만성통증 및 하지통과 경추통증 환자를 포괄하여 척추성 통증 환자를 대상으로 다음과 같이 심의하였다(2020.12.11.). 경피적 경막외강 신경성형술은 척추성 통증 환자를 대상으로 통증 감소를 위해 수행시 안전성은 수용가능하며, 기존의 보존적 치료 및 경막외강 신경차단술과 비교시 통증 개선 및 기능개선에 있어 일관되게 유의한 효과를 확인할 수 있어 유효성이 있는 기술로 평가하였으나, 근거수준이 낮은 제한점을 고려하여 추후 현재 급여권에서 활용되고 있는 적절한 비교시술과 비교한 양질의 근거축적은 지속적으로 필요하다고 판단하였다. 다만, 선택문헌 대부분에서 카테터 위치를 확인하고 시술을 진행하였고, 실제 카테터가 척추강내로 진입된 상태에서 약물이 투여되면 심각한 합병증을 야기할 수 때문에 상당한 주의와 감독하에 수행할 필요가 있고, 경추통증 환자의 경우 해부학적 위치 및 시술 난이도를 고려하였을 때 더 주의깊은 접근이 필요하다는 의견이 있었다. 이에 의료기술재평가위원회는 척추성 통증 환자를 대상으로 통증감소를 위한 경피적 경막외강 신경성형술을 권고함(권고등급 Ⅰ-b)으로 심의하였다.

Background of Assessment Percutaneous epidural neuroplasty is a technique performed to reduce pain in patients with spinal pain. It is a procedure that percutaneously inserts a catheter for neuroplasty into the epidural space to remove adhesions in lesions of the epidural space and inject therapeutic drugs. After the announcement of measures to strengthen health insurance coverage (August 2017), as the conversion to the reimbursement benefits from health non-insured benefits in the areas of 'spine, musculoskeletal system, and pain' was promoted in 2020, the need to review the feasibility of benefit conversion has been suggested since the neuroplasty surgery (endoscopic epidural neuroplasty, percutaneous epidural neuroplasty, percutaneous epidural neuroplasty with balloon catheter) has a large non-reimbursement scale and it is expected that various issues will arise when changing benefits. Afterward, the Ministry of Health and Welfare requested the National Evidence-based Healthcare Collaborating Agency to re-evaluate four existing spinal-related practices/procedures (intradiscal electrothermal therapy, endoscopic epidural neuroplasty, percutaneous epidural neuroplasty, percutaneous epidural neuroplasty with balloon catheter) (January 2020). Accordingly, the clinical safety and effectiveness of percutaneous epidural neuroplasty were evaluated at the 2nd Health Technology Reassessment Committee (2020.2.14.) in 2020 after reviewing the evaluation protocol and subcommittee composition plan.

Assessment Method In this assessment, a systematic literature review was performed to evaluate whether percutaneous epidural neuroplasty is clinically safe and effective for patients with spinal pain. For the systematic literature review, the literature search was conducted in 5 domestic databases (KoreaMed, Korean Medical database, Academic Database, Korea Education and Research Information Service (KERIS), National Digital Science Links (NDSL)) and 3 foreign databases (Ovid MEDLINE, Ovid EMBASE, Cochrane Central Register of Controlled Trials) based on the above key questions. The application of the literature inclusion and exclusion criteria, data extraction, and risk of bias evaluation were all independently performed by two evaluators. For the risk of bias assessment, Cochrane's Risk of Bias was used for randomized studies, and the Risk of bias for nonrandomized studies (RoBANS) 2.0 Korean version was used for other non-randomized studies, depending on the study type. To evaluate the level of evidence in the literature, Grading of Recommendations Assessment, Development and Evaluation (GRADE) was used. In consideration of the subcommittee's review opinion, the Health Technology Reassessment Committee presented the recommendation grade after final deliberation. In this evaluation, in the case of comparative treatment, to evaluate the effectiveness of interventional treatment, the treatment used in the existing reimbursement coverage was regarded as appropriate treatment and only studies compared with the corresponding comparative treatment (including placebo control group) were selected as literature for evaluation. For this assessment, a subcommittee consisting of a total of 10 members (2 from the Department of Anesthesiology and Pain Medicine, 2 from Department of Neurosurgery, 2 Department of from Orthopedic Surgery, 1 from the Department of Radiology, 1 from the Department of Rehabilitation Medicine, 2 from the Evidence-Based Medicine)evaluated the literature basis for the safety and effectiveness of the technology through systematic literature review. At the 12th Health Technology Reassessment Committee (2020.12.11.) in 2020, the clinical safety and effectiveness assessment results of percutaneous epidural neuroplasty were finally reviewed.

Assessment Results A total of 11 articles of literature were selected for assessment, compared with conservative treatment (2 articles), placebo (1 article), and epidural nerve block (8 articles). For the patient characteristics, patients with cervical pain due to cervical disc herniation (1 article), patients with chronic lumbar radicular pain due to disc protrusion or failed back surgery syndrome (FBSS) or patients with chronic lower extremity pain due to FBSS (4 articles), patients with intractable pain due to spinal stenosis, or patients with chronic radicular pain due to spinal stenosis (2 articles), patients with chronic low back pain and sciatica or lower extremity pain (3 articles), and patients with radiating pain due to herniated intervertebral disc (1 article) were included. Percutaneous epidural neuroplasty performed on patients with chronic low back pain and lower extremity pain was evaluated based on a total of ten articles of literature, and the study results are as follows. For safety, complications such as catheter rupture, placing the catheter in the epidural space, and subarachnoid entries were confirmed in 2 to 11.67% of the group that performed percutaneous epidural neuroplasty (hereinafter the 'intervention group'). In addition, swelling, redness, itching, and transient sensory deficit were reported in 6.5 to 91.3%. Accordingly, there was the subcommittee’s opinion that complications such as swelling, redness, itching, and transient sensory deficit were acceptable as temporary or resolvable complications. However, with respect to complications such as catheter rupture, placing the catheter in the epidural space, and subarachnoid entries, the currently selected literature was of the opinion that serious complications may occur if the drug is administered while the catheter is inserted into the spinal cavity. The procedure was performed after confirming the location of the catheter through epidurogram while administering a contrast agent. If the catheter entered a site other than the epidural space, the catheter was removed immediately and did not cause a dangerous situation, so it is judged that the related complications were simply mentioned in the literature. Since this procedure is a high-risk technique, there was the opinion that safety was acceptable if it was performed under considerable caution and supervision and appropriately dealt with in a dangerous situation. As for effectiveness, the intervention group reported significantly improved effects on pain and function compared with conservative treatment, placebo group, and epidural nerve block. However, most of the selected literature had a high dropout rate or a high proportion of patients who were unblinded, and there was no clear explanation of the concurrent treatment between the intervention group and the comparative group or how the treatment effect was adjusted. In some studies, the level of evidence was evaluated as 'low' due to the high risk of bias because injections were administered more than once in the intervention group or because the number of injections was not clearly explained. Accordingly, the subcommittee was of the opinion that although intervention procedures could consistently confirm more effective research results in reduction pain and improving function in patients with chronic low back pain and lower extremity pain, the level of evidence was low, so confidence in the evidence could be limited. Percutaneous epidural neuroplasty in patients with cervical pain was evaluated based on one literature review comparing it with an epidural nerve block. As a result of safety, no complications were reported in both groups. As a result of effectiveness, the degree of pain improvement was reported to be significantly higher in the intervention group at 12 months of follow-up, and the degree of functional improvement was significantly higher in the intervention group at 6 months. However, no significant difference was reported between groups at 12 months. Therefore, the subcommittee judged that it could not be evaluated because there was insufficient evidence to judge the safety and effectiveness of this technology since there was only one selected literature for cervical pain patients.

Conclusion The percutaneous epidural neuroplasty subcommittee made the following suggestions based on the current assessment results. When performing percutaneous epidural neuroplasty for patients with chronic low back pain and lower extremity pain, it is necessary to perform it under considerable caution and supervision, considering that this technique has a high risk of performing the procedure. Effectiveness was consistently significant in terms of pain reduction and functional improvement compared to conventional conservative treatment and epidural nerve block, so the opinion was that there was no problem in the clinical use of this health technology. However, considering that the level of evidence is limited, it was judged that it is necessary to continuously accumulate more evidence through well-designed studies to increase the strength of the recommendations. It is difficult to evaluate with only one selected literature because there is insufficient evidence to evaluate safety and effectiveness when it is performed on patients with cervical pain. For the assessment of this technique in the future, it was determined that it was necessary to accumulate more evidence through well-designed study results compared with techniques within the reimbursement coverage such as epidural nerve block. Based on the review results of the subcommittee, the Health Technology Reassessment Committee deliberated on ‘percutaneous epidural neuroplasty’ for spinal pain patients including chronic back pain, lower extremity pain, and cervical spine pain as follows (2020.12.11.). The safety of percutaneous epidural neuroplasty is acceptable when it is performed to reduce pain in patients with spinal pain. Compared with the existing conservative treatment and epidural nerve block, it was evaluated as an effective technique because it was able to confirm a consistently significant effect in reduction pain and improving function. However, considering the limitation of the low level of evidence, it was determined that it was necessary to continue to accumulate high-quality evidence compared to the appropriate comparative procedure used in the current reimbursement coverage. In addition, although the procedure was performed after confirming the catheter position in most of the selected literature, there was an opinion that it is necessary to be performed under considerable caution and supervision since serious complications may occur if the drug is administered while the catheter is inserted into the spinal cavity. In the case of patients with cervical pain, there was an opinion that a more careful approach is necessary considering the anatomical location and the difficulty of the procedure. Therefore, the Health Technology Reassessment Committee deliberated that percutaneous epidural neuroplasty for pain reduction in patients with spinal pain was recommended (Recommendation grade I-b).
