추간판내 열치료술

Abstract

평가 배경 추간판내 열치료술(Intradiscal Electrothermal Therapy, IDET)은 국내에서 2005년부터 비급여로 등재되어 현재까지 사용되고 있는 의료기술로, 손상된 섬유륜(annulus) 내로 자라서 들어온 신경조직에 고온의 열 손상을 주어 통증을 완화하는 행위이다. 해당 항목은 비급여 규모가 크고 급여 전환 시 다양한 쟁점이 제기될 것으로 예측됨에 따라 보건복지부의 요청에 의해 의료기술재평가를 수행하게 되었다. 2020년 제2차 의료기술재평가위원회(2020.02.14.)에서 평가계획서가 심의되었고, 2020년 제12차 의료기술재평가위원회(2020.12.11.)에서 안전성 및 유효성 평가결과를 최종 심의하였다.

평가 방법 추간판내 열치료술에 대한 안전성 및 유효성 평가는 체계적 문헌고찰을 통해 수행하였다. 모든 평가 방법은 연구목적을 고려하여 “추간판내 열치료술 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 평가의 핵심질문은 “추간판성 통증 환자에서 추간판내 열치료술의 임상적 안전성 및 유효성은 어떠한가”였으며, 안전성은 비교연구 및 단일군 연구를 대상으로 부작용과 합병증을 확인하고 유효성은 비교연구를 대상으로 통증 점수, 기능 평가, 삶의 질 등의 지표로 평가하였다. 문헌검색은 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림 위험 평가는 연구유형에 따라 Cochrane의 Risk of Bias 및 ROBANS ver. 2.0을 사용하여 두 명의 검토자가 독립적으로 수행하여 의견 합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 자료 분석은 기본적으로 대조법의 유형에 따라 나누어 수행하였으며, 정량적 분석이 가능할 경우 메타분석을 수행하고 불가능한 경우 질적 검토를 수행하였다. 체계적 문헌고찰 결과는 GRADE 방법론을 이용하여 근거수준을 평가하였다.

평가 결과 체계적 문헌고찰을 수행한 결과, 추간판내 열치료술과 관련된 총 56개의 연구(62편 문헌)가 선정되었다. 모든 연구는 요통(Low back pain, back pain)을 대상으로 수행되었고, 2000년 초반에 출판된 연구가 53.2%로 가장 많았다. 비교연구가 10개(13편 문헌), 단일군 연구가 46개(49편 문헌) 포함되었다.

안전성 54개의 연구(비교연구 8개, 단일군 연구 46개)에서 추간판내 열치료술의 임상적 안전성 결과를 확인할 수 있었다. 추간판내 열치료술과 수술적 치료를 비교한 1개의 비무작위 비교연구에서는 두 군 모두 감염 또는 합병증이 발생하지 않은 것으로 나타났으며, 최소침습적 치료를 비교한 1개의 비무작위 비교연구에서는 추간판내 열치료술에서의 합병증에 대해 언급하고 있지 않았다. 추간판내 열치료술과 보존적 치료를 비교한 6개의 연구 중, 3편의 연구에서는 두 군 모두 부작용 또는 합병증이 발생하지 않았다고 보고되었으나 2편의 연구에서는 추간판내 열치료술 후 11~12%에서 부작용 및 일시적 신경근병증이 발생한 것으로 나타났다. 추간판내 열치료술을 받은 환자를 대상으로 수행된 46개의 단일군 연구 중, 22개(47.8%)의 연구에서는 추간판내 열치료술 후 합병증이 발생하지 않았다고 보고되었으나, 24개(52.2%)의 연구에서는 1.3~29.4% 범위 내에서 다양한 부작용 및 합병증이 나타났다고 보고되었다. 경미한 증상부터 추간판염, 골괴사증, 마미증후군, 복합부위 통증증후군과 같은 심각한 합병증도 확인할 수 있었다.

유효성 10개의 비교연구에서 추간판내 열치료술의 임상적 유효성 결과를 확인할 수 있었다. 추간판내 열치료술과 수술적 치료를 비교한 2개의 비무작위 비교연구에서는 통증개선 정도, 기능개선 정도 및 삶의 질에 있어 두 군간 유의미한 차이가 없었으나 최소침습적 치료와 비교한 1개의 비무작위 비교연구에서는 통증 점수 감소는 다른 최소침습적 치료(intradiscal injection)에서 더 좋았고, 통증 재발 기간은 추간판내 열치료술에서 유의하게 더 좋은 것으로 나타났다. 추간판내 열치료술과 보존적 치료를 비교한 7개의 연구에서는 통증개선 정도, 기능개선 정도 및 삶의 질에 있어 추간판내 열치료술이 보존적 치료보다 유의하게 더 좋은 것으로 나타났으나 그 외 우울 점수 및 치료 성공여부, 신체 지각 점수에서는 두 군간 유의한 차이가 없는 것으로 나타났다.

결론 및 제언 추간판내 열치료술 소위원회는 현재 평가결과에 근거하여 다음과 같이 제언하였다. 추간판내 열치료술은 다른 치료와 비교하여 합병증이 발생하지 않거나 유사한 수준의 안전성을 가진 것으로 확인되었으나, 골괴사증, 추간판염, 마미증후군, 복합부위 통증증후군과 같은 심각한 합병증 사례가 일부 보고되고 있어 주의가 필요하다. 유효성 측면에 있어, 추간판내 열치료술은 수술적 치료와 유의한 차이가 없었으나 다른 최소침습적 치료와 비교한 일부 결과지표에서 유의하게 더 좋은 것으로 나타났으며 보존적 치료보다는 효과가 더 좋은 것으로 나타났다. 이에 의료기술재평가위원회는 소위원회 검토 결과에 근거하여 다음과 같이 심의하였다(2020.12.11.). 추간판성 통증 환자의 통증을 치료하는 의료기술로 “추간판내 열치료술”을 권고함(권고등급 Ⅰ-b)으로 심의하였고, 권고사유는 다음과 같다. 추간판내 열치료술은 다른 치료와 비교 시 합병증이 발생하지 않거나 유사한 수준의 안전성이 있는 것으로 나타났으나, 단일군 연구에서 주요 합병증이 일부 보고되고 있어 주의가 필요하다. 현재 임상에서 사용되는 수술적 치료 및 다른 최소침습적 치료와 유효성을 비교한 근거는 충분하지 않았으나 보존적 치료보다는 효과가 있는 의료기술로 판단하였다.

Background of Assessment Intradiscal Electrothermal Therapy (IDET) is a health technology that has been registered as a non-benefit item since 2005 in Korea and has been used until now. It is the practice of relieving pain by giving high-temperature heat damage to the nerve tissue that has grown into the damaged annulus fibrosus. Health technology reassessment was performed at the request of the Ministry of Health and Welfare as this technology has a large non-reimbursement scale and it is expected that various issues will arise during the conversion of benefits. The evaluation plan was deliberated at the 2nd health technology reassessment committee (2020.02.14.) in 2020, and the safety and effectiveness evaluation results were finally reviewed at the 2020 12th health technology reassessment committee (2020.12.11.).

Assessment Method Safety and effectiveness evaluation of IDET was conducted through a systematic literature review. All evaluation methods were finalized after deliberation by the “Intradiscal Electrothermal Therapy Subcommittee (hereinafter referred to as the subcommittee)” in consideration of the research purpose. The key question of the assessment was “What is the clinical safety and effectiveness of Intradiscal Electrothermal Therapy (IDET) in patients with discogenic pain?”. Safety was evaluated for side effects and complications in comparative studies and single-arm studies, and effectiveness was evaluated by indicators such as pain score, functional evaluation, and quality of life in comparative studies. A literature search was conducted in 3 overseas databases and 5 domestic databases, and two reviewers independently screened and selected them according to the literature inclusion and exclusion criteria. According to the study type, the risk of bias assessment in the literature was conducted independently using Cochrane's Risk of Bias and ROBANS ver. 2.0 by two reviewers to reach a consensus of opinion. Data extraction was performed independently by two reviewers using a pre-determined data extraction format, and in case of disagreement, it was agreed upon by discussion with a third party. Data analysis was performed according to the type of control method. If the quantitative analysis was possible, meta-analysis was performed, and if a quantitative analysis was not possible, a qualitative review was performed. For the results of the systematic literature review, the level of evidence was evaluated using the GRADE method.

Assessment Results As a result of the systematic literature review, a total of 56 studies (62 articles) related to Intradiscal Electrothermal Therapy (IDET) were selected. All studies were conducted on low back pain (back pain), and the research published in early 2000 was the largest with 53.2%. 10 comparative studies (13 articles) and 46 single-arm studies (49 articles) were included.

Safety In 54 studies (8 comparative studies, 46 single-arm studies), the clinical safety results of Intradiscal Electrothermal Therapy (IDET) were confirmed. In one non-randomized comparative study comparing IDET and surgical treatment, neither group showed no infection or complications. In one non-randomized comparative study comparing IDET and minimally invasive treatment, it did not mention complications in IDET. Of the six studies comparing IDET with conservative treatment, three studies reported no side effects or complications in both groups, but in two studies, side effects and transient neuromyopathy occurred in 11 to 12% of patients after IDET. Of the 46 single-arm studies performed on patients who underwent IDET, 22 (47.8%) studies reported no complications after IDET. In 24 studies (52.2%), various side effects and complications were reported within the range of 1.3 to 29.4%. From mild symptoms, serious complications such as discitis, osteonecrosis, Cauda equina syndrome, and complex regional pain syndrome were also identified.

Effectiveness In 10 comparative studies, the clinical effectiveness of Intradiscal Electrothermal Therapy (IDET) was confirmed. In two non-randomized comparative studies comparing IDET and surgical treatment, there was no significant difference between the two groups in terms of pain improvement, functional improvement, and quality of life. However, in one non-randomized comparative study compared with minimally invasive treatment, pain score reduction was better with the other minimally invasive treatment (intradiscal injection), and the duration of pain recurrence was significantly better with IDET. Seven studies comparing IDET with conservative treatment showed that IDET was significantly better than conservative treatment in terms of pain improvement, functional improvement, and quality of life. Other than that, there was no significant difference between the two groups in the depression scale, treatment success, and body awareness rating.

Conclusion and Suggestions The Intradiscal Electrothermal Therapy (IDET) subcommittee made the following recommendations based on the current evaluation results. IDET was found to have no complications or a similar level of safety compared to other treatments. However, caution is needed since some serious complications such as osteonecrosis, discitis, cauda equina syndrome, and complex regional pain syndrome have been reported. In terms of effectiveness, IDET was not significantly different from surgical treatment, but it was found to be significantly better in some outcome indicators compared to other minimally invasive treatments and more effective than conservative treatment. Accordingly, the health technology reassessment committee deliberated as follows based on the subcommittee review results (2020.12.11.). “Intradiscal Electrothermal Therapy (IDET)” as a medical technology to treat pain in patients with discogenic pain was reviewed as 'recommended' (recommendation grade I-b), and the reasons for the recommendation are as follows. IDET has been shown to have no complications or a similar level of safety compared to other treatments, but caution is needed as some major complications have been reported in single-arm studies. Although there was not enough evidence to compare effectiveness with surgical treatment and other minimally invasive treatments currently used in clinical practice, it was judged as an effective health technology rather than conservative treatment.
