수핵성형술

Abstract

평가 배경 수핵성형술은 고주파가 장착된 탐침을 추간판 안쪽으로 삽입하여 전진과 후퇴를 통해 소작술(ablation)과 응고술(coagulation)을 반복하면서 추간판 물질들을 기화시켜 제거함으로서 추간판 내 압력을 감소시키는 기술이다. 국내에서는 2005년부터 ‘추간판내 고주파 열응고술’의 행위명으로 비급여 항목인 “추간판내 열치료술(조-83)”의 항목에 포함되어 사용되고 있다. 동 기술은 정부의 2020년도 「등재비급여의 보장성 강화 추진 계획 -‘척추・근골격계・통증’」 분야에 포함되고, 해당 항목의 비급여 규모가 크며, 급여 전환 시 다양한 쟁점이 제기될 것으로 예측됨에 따라 급여전환 타당성을 검토하기 위해 보건복지부로터 재평가를 의뢰받아 안전성 및 유효성 검토를 수행하였다. 2020년 제2차 의료기술재평가위원회에서 평가계획서를 심의하였고, 소위원회 구성은 근거기반의학 2인, 마취통증의학과 2인, 신경외과 2인, 정형외과 2인, 재활의학과 1인, 영상의학과 1인의 전문가 10인으로 포함하는 것으로 하였다.

평가 방법 추간판성 통증 환자에서 수핵성형술의 임상적 안전성 및 유효성을 확인하기 위하여 체계적 문헌고찰 방법을 이용하여 평가하였고, 안전성 평가 시에는 비교연구만으로 결론을 내리기에 제한적이어서 단일군 연구까지 포함하여 검토하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림위험 평가는 RoB, RoBANS를 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 자료분석은 질적 검토(qualitative review)를 수행하였다. 체계적 문헌고찰의 결과는 Grading of Recommendations Assessment Development and Evaluation (GRADE) 방법에 따라 근거수준을 평가하였다.

평가 결과 수핵성형술 평가에 선택된 문헌은 총 75편이었으며, 적용되는 부위별로 구분하면 요추 49편, 경추 26편이었다. 요추 부위는 연구유형별로 비교연구 13편(무작위배정 비교연구 6편, 비무작위 비교연구 7편)(연구대상자수 총 1,539명), 단일군 연구 36편(연구대상자수 총 3,589명)이었다. 경추 부위는 연구유형별로 비교연구 7편(무작위배정 비교연구 4편, 비무작위 비교연구 3편)(연구대상자수 총 650명), 단일군 연구 19편(연구대상자수 총 1,330명)이었다. 수핵성형술의 평가결과는 요추와 경추 부위별로, 각각의 안전성과 유효성에 대해 비교치료군(수술적 치료, 최소침습적 치료, 보존적 치료)에 따라 구분하여 제시하였다.

요추 수핵성형술의 안전성은 총 45편(비교연구 9편, 단일군 연구 36편)의 문헌을 토대로 평가되었다. 비교연구의 경우, 수술적 치료와 비교한 연구(3편)에서는 1편의 연구에서 비교군에서만 1.3-3.8%의 합병증 사례가 보고되었으며, 최소침습적 치료와 비교된 연구(5편)에서는 중재군 0-11.5%, 비교군 0-17.5%, 보존적 치료와 비교된 연구(1편)에서는 중재군에서 임상적 부작용 및 합병증은 없는 것으로 보고되었다. 단일군 연구는 22편(64.7%)의 연구에서 수핵성형술 시술과 관련된 합병증은 발생되지 않았다고 보고되었으며, 이외 14편의 연구에서 부작용 및 합병증 사례가 보고되었다. 소위원회에서는 합병증 중에서도 추간판염, 신경학적 손상, 뇌척수액 누출, 혈종을 임상적으로 의미 있게 볼 필요가 있는 주요합병증으로 살펴보았다. 추간판염(5편)은 0.25% (1/396) - 10% (5/50), 신경학적 이상반응(6편)은 0.5 (2/396) - 20.4% (10/49), 뇌척수액 누출(1편)은 9.6% (5/52), 혈종(1편)은 5.8% (3/52)으로 보고되었다. 그 밖에 경미한 합병증으로는 바늘천자 부위 통증(5편)이 6.4-75.5%로 다수 보고되었고 이들의 사례는 모두 일시적인 것으로 시술 후 2주, 4주 내 해소된 것으로 확인하였다. 또한, 바늘천자 부위 관련 출혈이 2편, 바늘천자 부위 관련 감염이 1편에서 발생되었다고 보고되었다. 경막외 섬유증도 증례보고 1편(1례)에서 보고되었다. 수핵성형술의 유효성은 총 13편의 문헌을 토대로 평가되었다. 수술적 치료와 비교한 연구(3편)에서 통증 및 기능개선정도는 두 치료군 모두 시술 전후 개선됨이 확인되었고, 두 치료 군 간의 비교 시 통계적인 유의미한 효과 차이는 1편에서만 비교군이 더 좋은 것으로 확인되었다. 최소침습적 치료와 비교된 연구(8편)에서 통증 및 기능개선정도는 두 치료군 모두 시술 전후 개선됨이 확인되었고, 두 치료군 간의 비교 시 5편의 연구에서는 중재군에서 통증 및 기능개선정도가 좋은 것으로 나타났으며, 3편은 비교군에서 개선효과가 좋은 것으로 확인되었다. 삶의 질, 환자만족도는 중재군에서 유의하게 좋은 것으로 나타났다. 보존적 치료와 비교된 연구(2편)에서 통증 및 기능개선은 중재군에서 좋은 것으로 확인되었다.

경추 수핵성형술의 안전성은 총 26편(비교연구 7편, 단일군 연구 19편)의 문헌을 토대로 평가되었다. 비교연구의 경우, 수술적 치료와 비교한 연구(1편)에서는 두 치료군 모두 합병증은 발생되지 않은 것으로 보고되었고, 최소침습적 치료와 비교된 연구(3편)에서는 중재군 0-17.6%, 비교군 1.0-17.6%, 보존적 치료와 비교된 연구(3편)에서는 중재군에서 합병증 발생은 없는 것으로 보고되었다. 단일군 연구는 4편(33.3%)의 연구에서 수핵성형술 시술과 관련된 합병증은 발생되지 않았다고 보고되었으며, 이외 11편 연구들에서는 통증, 추간판염, 기기 관련 문제, 혈종 등의 사례가 보고되었다. 소위원회에서는 합병증 중에서도 추간판염, 기기 관련 문제(예; 기기 부서짐), 혈종을 임상적으로 의미 있게 볼 필요가 있는 합병증으로 살펴보았다. 추간판염은 4편에서 각 1례씩 보고되었다. 기기 관련 문제로는 2편에서 전극 팁/spinewand가 깨져 추간판 공간에 잔존하는 사례가 각각 1례씩(0.79, 2.2%) 보고되었으며 환자 상태는 합병증 발생 없이 임상결과가 좋은 상태로 유지되었다고 보고하였다. 소위원회에서는 환자 상태가 추가적인 합병증 발생 없이 임상결과가 좋은 상태로 유지하였다고 보고되어 안전성의 큰 문제로 이어지지는 않았으나, 동 시술 자체가 시술 중 tip이 잘 부러지는 사건이 발생할 수 있는 점과 tip 부러짐으로 인해 자칫 심각한 2차 합병증 발생의 우려는 배제할 수 없어 시술 시 안전성에 대한 주의가 필요하다는 의견을 제시하였다. 전경부 혈종이 2편에서 각각 1례 보고되었다. 이외 경미한 합병증으로 일시적이거나 수일 내 해소되는 통증(3편) 0.74-46.4%, 전경부 통증 및 애성 등 호너증후군(2편) 0.39-10.4%, 바늘삽입부위 반상출혈(1편) 10.7% 등의 사례가 보고되었다. 수핵성형술의 유효성은 총 13편의 문헌을 토대로 평가되었다. 수술적 치료와 비교한 연구(1편)에서 통증 및 기능개선정도는 두 치료군 모두 시술 전후 유의하게 개선됨이 확인되었으나 두 군 간의 효과 차이는 없었다. 최소침습적 치료와 비교된 연구(3편)에서 통증 및 기능개선정도는 두 치료군 모두 치료 후 지표점수가 좋아졌으나 두 치료군 간의 비교 시 유의한 차이가 없었다. 환자만족도(1편)에서도 유의한 차이가 없었다. 보존적치료와 비교된 연구(3편)에서 통증 및 기능개선정도는 중재군에서 유의한 개선효과가 있는 것으로 보고되었다. 신체적 삶의 질(1편)은 중재군이 비교군에 비해 통계적으로 유의하게 개선된 것으로 나타났다.

결론 및 제언 소위원회는 현재 문헌에 근거하여 요추 및 경추 추간판성 통증 환자를 대상으로 시행되는 수핵성형술의 안전성 및 유효성 결과를 다음과 같이 제시하였다. 수핵성형술의 안전성 관련 요추 및 경추 비교연구에서는 합병증이 발생하지 않거나 일부 보고된 사례는 낮거나 유사한 수준으로 확인되었다. 단일군 연구에서는 요추 12편(35.2%), 경추 8편(66.7%)의 연구들에서 다양한 합병증을 보고하고 있었다. 문헌상에서 모든 사례는 일시적이거나 해소된 것으로 보고되었으나, 몇몇의 요추 연구에서 보고된 추간판염, 신경학적 이상반응, 뇌척수액 누출, 혈종 사례와 경추 연구에서 보고된 추간판염, 기기관련 문제, 혈종의 사례는 임상적으로 주의 이상의 의미가 있어 고려해야 할 필요가 있다는 의견이었다. 유효성 관련 요추에서의 수핵성형술은 수술적 치료와 비교하여 통증감소 및 기능개선의 효과가 적었으나, 보존적 치료 대비 유의한 효과가 확인되고, 다른 최소침습적 치료와 유사하거나 유의한 효과가 확인되었다. 경추에서의 수핵성형술은 보존적 치료와 비교 시 통증감소 및 기능개선에 유의한 효과가 확인되었으나, 수술적 치료 및 최소침습적 치료와는 유의한 효과 차이는 없었다. 따라서 소위원회에서는 수핵성형술의 안전성 및 유효성에 대하여 요추 및 경추 추간판성 통증 환자를 치료하기 위한 시술로, 단일군 연구에서 심각한 합병증이 보고된 적이 있었으나 후유증없이 해소되었고, 타 치료와 비교 시 합병증이 발생하지 않거나 유사한 수준으로 안전성이 있으며, 보존적 치료보다는 효과가 있고 다른 최소침습적 치료와 유사한 수준으로 유효성이 있는 기술이라는 의견이었다. 의료기술재평가위원회는 “수핵성형술”에 대해 소위원회 검토결과에 근거하여 다음과 같이 심의하였다(2020.12.11.). 수핵성형술은 추간판성 통증 환자를 치료하기 위한 의료기술로 다른 치료와 비교 시 합병증이 발생하지 않거나 유사한 수준의 안전성이 있는 것으로 나타났으나, 단일군 연구에서 주요 합병증이 일부 보고되고 있어 주의가 필요하다. 또한 통증 및 기능개선정도에 있어 보존적 치료보다는 효과가 있고, 다른 최소침습적 치료와 유사한 수준이거나 유의한 효과가 확인되어 유효성이 있는 기술로 평가하였다. 이에, 의료기술재평가위원회는 추간판성 통증 환자의 통증을 치료하는 의료기술로 수핵성형술을 권고함(권고등급 I-b)으로 심의하였다.

Background of Assessment Nucleoplasty is a technology that reduces intra-discal pressure by inserting a radiofrequency probe into the disc and repeating ablation and coagulation through advancing and retracting to vaporize and remove disc materials. In Korea, since 2005, it has been used including in the category of “Intradiscal Electrothermal Therapy (Article-83)”, which is a non-benefit item, as the name of the service in ‘intradiscal radiofrequency thermal coagulation’. This technology is included in the government's 「Plan for Benefit Expansion in Non-benefit Items 'Spine, Musculoskeletal System, and Pain' in 2020. Since the related item has a large non-reimbursement scale and it is expected that various issues will arise when changing benefits, the Ministry of Health and Welfare requested a re-assessment, and safety and effectiveness were evaluated to review the feasibility of benefit conversion. The assessment protocol was deliberated by the 2nd Health Technology Reassessment Committee in 2020, and the subcommittee consisted of a total of 10 experts including 2 from the Department of Evidence-Based Medicine, 2 from the Department of Anesthesiology and Pain Medicine, 2 from the Department of Neurosurgery, 2 from the Department of Orthopedic Surgery, 1 from the Department of Rehabilitation Medicine, and 1 from Department of Radiology.

Assessment Method To confirm the clinical safety and effectiveness of nucleoplasty in patients with discogenic pain, it was evaluated using a systematic literature review method. In the safety evaluation, it was limited to draw conclusions only from comparative studies, so single-arm studies were also included. For the systematic literature review, three overseas and five domestic databases were searched based on key questions, and two reviewers independently screened and selected them according to the literature inclusion and exclusion criteria. The risk of bias evaluation in the literature was conducted independently by two reviewers using RoB and RoBANS, and a consensus was reached. Data extraction was performed independently by two reviewers using a pre-determined data extraction format, and in case of disagreement, it was discussed with a third party and an agreement was reached. A qualitative review was performed for data analysis. As a result of the systematic literature review, the level of evidence was evaluated according to the Grading of Recommendations Assessment Development and Evaluation (GRADE) method.

Assessment Results A total of 75 pieces of literature were selected for the evaluation of nucleoplasty, and 49 articles for the lumbar vertebrae and 26 articles for the cervical vertebrae were classified by area to be applied. For the lumbar region, there were 13 comparative studies (6 randomized comparative studies, 7 non-randomized comparative studies) (1,539 subjects in total) and 36 single-arm studies (3,589 subjects in total) by study type. For the cervical spine, there were 7 comparative studies (4 randomized comparative studies, 3 non-randomized comparative studies) (650 subjects in total) and 19 single-arm studies (1,330 subjects in total) by study type. The evaluation results of nucleoplasty were presented separately for each lumbar and cervical spine region, according to the comparative treatment group (surgical treatment, minimally invasive treatment, and conservative treatment) for safety and effectiveness.

Lumbar spine The safety of nucleoplasty was evaluated based on a total of 45 pieces of literature (9 comparative studies, 36 single-arm studies). In the case of comparative studies, 1.3-3.8% of complications were reported only in the comparison group in one study compared to surgical treatment (3 articles). In a study compared with minimally invasive treatment (5 articles), complications were reported in 0-11.5% for the intervention group and 0-17.5% for the control group. In the study compared with conservative treatment (1 article), no clinical side effects and complications were reported in the intervention group. In single-arm studies, 22 studies (64.7%) reported no complications related to nucleoplasty, and 14 other studies reported side effects and complications. Among the complications, the subcommittee considered discitis, neurological damage, cerebrospinal fluid leakage, and hematoma as major complications that need to be considered clinically meaningfully. Discitis (5 articles) was reported 0.25% (1/396) - 10% (5/50), neurological adverse reactions (6 articles) was 0.5 (2/396) - 20.4% (10/49), cerebrospinal fluid leakage (1 article) was 9.6% (5/52), and hematoma (1 article) was reported in 5.8% (3/52). As for other minor complications, needle puncture site pain (5 articles) was reported in 6.4-75.5% of cases, and all of these cases were temporary and resolved within 2 weeks and 4 weeks after the procedure. It was also reported that needle puncture site-related bleeding occurred in 2 articles and needle puncture site-related infection occurred in 1 article. Epidural fibrosis was also reported in 1 article (1 case). The effectiveness of nucleoplasty was evaluated based on a total of 13 pieces of literature. In a study comparing surgical treatment (3 articles), it was confirmed that the degree of pain and functional improvement improved before and after the procedure in both treatment groups. When comparing the two treatment groups, it was confirmed that the comparison group was better in only one article for the statistically significant effect differences. In a study compared with minimally invasive treatment (8 articles), it was confirmed that the degree of pain and functional improvement improved before and after the procedure in both treatment groups. In 5 studies comparing the two treatment groups, the intervention group showed better pain and function improvement. In the 3 studies, the improvement effect was better in the control group. Quality of life and patient satisfaction were significantly better in the intervention group. In a study compared with conservative treatment (2 articles), It was confirmed that pain and functional improvement were better in the intervention group.

Cervical spine The safety of nucleoplasty was evaluated based on a total of 26 pieces of literature (7 comparative studies, 19 single-arm studies). In the comparative study, no complications were reported in both treatment groups in the study comparing surgical treatment (1 article), and 0-17.6% of the intervention group and 1.0-17.6% of the control group in the study compared with minimally invasive treatment (3 articles). In a study comparing conservative treatment (3 articles), it was reported that there were no complications in the intervention group. In single-arm studies, it was reported that no complications related to nucleoplasty occurred in 4 studies (33.3%), and cases such as pain, disc herniation, device related problems, and hematomas were reported in 11 studies. Among the complications, the subcommittee considered discitis, device-related problems (eg, broken device), and hematomas as complications that need to be considered clinically meaningfully. Discitis was reported in 4 articles, 1 case each. As for device-related problems, 2 articles reported each one case (0.79, 2.2%) in which the electrode tip/spinewand was broken and remained in the intervertebral disc space, and the patient's condition was reported to have maintained good clinical results without complications. The subcommittee reported that the patient's condition maintained good clinical results without any additional complications, so it did not lead to a major safety problem. However, it is suggested that attention should be paid to safety during the procedure, as the procedure itself may cause the tip to break easily during the procedure, and the risk of serious secondary complications due to tip breakage cannot be ruled out. The anterior hematoma was reported in 2 articles in each of 1 case. Other minor complications, such as pain that are temporary or resolved within a few days, were reported in 0.74-46.4% (3 articles), Horner's syndrome such as anterior cervical pain and hoarseness in 0.39-10.4% (2 articles), and ecchymosis at the needle insertion site in 10.7% (1 article). The effectiveness of nucleoplasty was evaluated based on a total of 13 pieces of literature. In a study comparing surgical treatment (1 article), it was confirmed that the degree of pain and functional improvement was significantly improved before and after the treatment in both treatment groups, but there was no difference in effect between the two groups. In the study (3 articles) compared with minimally invasive treatment, the index score of both treatment groups improved after treatment, but there was no significant difference between the two treatment groups. There was no significant difference in patient satisfaction (1 article). In a study compared with conservative treatment (3 articles), it was reported that there was a significant improvement in pain and functional improvement in the intervention group. The quality of physical life (1 article) was found to be significantly improved in the intervention group compared to the control group.

Conclusion and Suggestions Based on the current literature, the subcommittee presented the safety and effectiveness results of nucleoplasty for patients with lumbar and cervical discogenic pain as follows. In a safety-related lumbar and cervical spine comparative study of nucleoplasty, no complications or some reported cases were confirmed at a low or similar level. In single-arm studies, various complications were reported on lumbar vertebrae in 12 articles (35.2%) and on cervical vertebrae in 8 articles (66.7%). In the literature, all cases have been reported as transient or resolved. However, it was the opinion that the case of discitis, neurological adverse events, cerebrospinal fluid leakage, and hematoma reported in some lumbar spine studies and cases of discitis, device-related problems, and hematomas reported in cervical spine studies, were clinically meaningful than caution and needed to be considered. Efficacy-related nucleoplasty in the lumbar spine showed less pain reduction and functional improvement compared to surgical treatment. However, a significant effect was confirmed compared to conservative treatment, and similar or significant effects were confirmed as other minimally invasive treatments. Nucleoplasty in the cervical spine showed significant effects on pain reduction and functional improvement compared with conservative treatment, but there was no significant difference in effect between surgical treatment and minimally invasive treatment. Therefore, the subcommittee had the following opinions on the safety and efficacy of nucleoplasty. As a procedure to treat patients with lumbar and cervical discogenic pain, serious complications were reported in a single-arm study, but they resolved without sequelae. It did not cause complications or is safe at a similar level compared to other treatments. It is a technology that is more effective than conservative treatment and is effective at a level similar to other minimally invasive. The Health Technology Reassessment Committee deliberated on “nucleoplasty” as follows based on the subcommittee’s review results (2020.12.11.). Nucleoplasty is a health technology for treating patients with discogenic pain, and it has been shown to have no complications or a similar level of safety compared to other treatments. However, some major complications have been reported in single-arm studies, so attention is needed. In addition, in terms of pain and functional improvement, it was evaluated as an effective technique because it was more effective than conservative treatment and it was found to have a similar level or significant effect to other minimally invasive treatments. Therefore, the Health Technology Reassessment Committee deliberated nucleoplasty as a 'recommended' (recommendation grade I-b) as a health technology for treating pain in patients with discogenic pain.
