메티실린내성 황색포도알균 유전자검사 [실시간 중합효소연쇄반응]

Abstract

평가 배경 고위험군 환자의 능동감시를 위한 ‘메티실린내성 황색포도알균 유전자검사[실시간 중합효소연쇄반응]’은 MRSA(Methicillin-resistant Staphylococcus aureus) 전파 예방을 목적으로 하는 능동감시를 위한 검사로 MRSA 보균자로 의심되는 환자를 대상으로 비강 도말 검체를 사용하여 MRSA 감염을 조기에 확인하고자 수행하는 검사이다. 동 검사는 국내에서 2011년 신의료기술평가로 인정되었으며(보건복지부 고시 제2011-142호, 2011.11.17.), 2012년에 비급여 행위로 고시되었다(보건복지부 고시 제2012-116호, 2012.9.14.). 이에 한국보건의료연구원에서는 내부 모니터링을 통해 해당 검사를 의료기술의 적정사용유도 및 보건의료 자원의 효율적 사용을 위한 근거검토가 필요한 안건으로 판단하여 재평가를 시행하였다. 2020년 제11차 의료기술재평가위원회(2020.11.13.)에서는 동 기술의 임상적 안전성 및 유효성 평가에 대한 최종결과를 심의하였다.

평가 목적 및 방법 고위험군 환자(메티실린내성 황색포도알균 감염 또는 보균에 대한 감시가 필요한 사람) 대상으로 ‘메티실린내성 황색포도알균 유전자검사[실시간 중합효소연쇄반응]’를 이용한 능동감시의 임상적 안전성 및 유효성에 대한 의과학적 근거에 대해 체계적 문헌고찰을 수행하였다. 총 7인(감염내과 3인, 진단검사의학과 2인, 중환자의학과 1인, 근거기반의학 1인)으로 구성된 소위원회를 통해 평가방법, 비뚤림위험평가, 최종 권고 등의 내용을 포함하여 평가를 진행하였으며, 메티실린내성 황색포도알균 유전자검사[실시간 중합효소연쇄반응]의 진단정확성은 신의료기술평가를 통해 이미 확인된 기술로 이번 재평가에서는 해당 검사의 임상적 유용성을 중점으로 평가하였다.

평가 결과 임상적 유용성을 평가하기 위한 결과지표로 적절한 항생제 사용, MRSA 감염률, 의료이용(재원 기간, 격리 기간, 의료비용)을 선정하였다. 국내외 데이터베이스를 통하여 총 1,390편이 검색되었고, 중복제거가 완료된 문헌 총 1,082편으로 문헌검토를 진행하였다. 문헌검토 결과, 최종 선택된 문헌은 7편이었다. 적절한 항생제 사용을 보고한 문헌은 3편이었으며, 3편 모두 의료결과에 대한 비교군이 없는 후향적 연구였다.(Giancola et al., 2016; Smith et al., 2017; Dangerfield et al., 2014). Giancola 등(2016)은 중환자실에 입원한 폐렴 환자 200명을 대상으로 중재검사를 실시하는 경우 전체 환자를 대상으로 경험적 항생제 치료 기간을 300일, 환자 당 치료 기간은 2일(중위수)로 예방 가능하다고 제시하였다. Smith 등(2017)은 중재검사 결과 음성으로 확인된 중환자실 내원 환자는 전체의 77.3%(309/400명) 해당했으며, 이 중 경험적 항생제 치료를 중단한 환자는 54.7%(169/309명)였다고 보고하였다. 이는 중재 방법으로 인해 조기에 불필요한 항생제 사용을 줄일 수 있다고 제시하였다. Dangerfield 등(2014)은 경험적 항생제를 투여한 폐렴 환자 243명 중 중재검사 결과 MRSA 양성이면서 배양검사 결과 진양성으로 확인된 환자 비율이 43.6%(17/39명)로, 이는 MRSA 유전자검사를 통한 능동감시로 인해 43.6%에서 조기에 적절한 항생제가 사용되었음을 보여주었다. 또한 Real-time PCR 검사 결과 음성이면서 배양검사 결과 진음성으로 확인된 환자 비율이 99.5% (203/204명)로, 이는 Real-time PCR 방법으로 인해 불필요한 항생제 사용을 감소시켜, 동 검사가 유용하게 사용되었음을 제시하였다. 3편 연구에 대해 GRADE 평가 결과, 근거수준이 ‘Low’ 등급으로 평가되었다. MRSA 감염률을 보고한 문헌은 2편이었으며, 2편 모두 과거 대조군 연구였다(Leung et al., 2013; Stano et al., 2013). Leung 등(2013)은 중환자실 입원 환자에서 검출한 MRSA 7,950개의 검체를 대상으로 능동감시를 시행하지 않은 그룹(대조군)과 Real-time PCR 방법을 이용하여 능동감시를 시행한 그룹(중재군)을 비교했다. 대조군에 비해 중재군에서 중환자실 평균 MRSA 감염률은 재원 일수 1,000일 기준으로 1.94건 감소했다(3.67/1,000 patient bed days → 1.73/1,000 patient bed days). Stano 등(2013)은 중환자실 입원 환자 1,008명을 대상으로 Real-time PCR 방법을 이용한 능동감시를 시행하였다. Real-time PCR 검사 결과 진양성이 나오면, 나머지 치료는 동일하게 유지하면서 추가적으로 코호트 격리 등의 능동감시 조치를 시행한 그룹과 능동감시 조치를 시행하지 않은 그룹을 비교했다. 중재군에서의 MRSA 감염률이 2.0%(9/431명)에서 0.3%(2/577명)로 감소했으며, MRSA 감염 위험도가 15% 감소했다(Relative risk: 0.85(95% CI 0.02-0.63; p=0.002)). 2편 연구에 대해 GRADE 평가 결과, 근거수준이 ‘Low’ 등급으로 평가되었다. 의료이용(재원 기간, 격리 기간, 의료비용)에는 재원 기간, 격리 기간, 의료비용을 포함하여 검토하였다. 의료이용을 보고한 문헌은 3편이었다(Smith et al., 2017; Wassenberg, et al., 2012; Wassenberg et al., 2010). Smith 등(2017) 연구에서는 Real-time PCR 검사 결과 음성으로 확인된 환자 중 항생제 치료를 조기에 중단할(De-escalation) 수 있었던 환자는 54.7%(169/309명)에 해당하였으며, 이에 따라 항생제(Vancomycin) 관련하여 $21,031의 의료비용이 전체 환자에서 절감될 수 있었으며, 환자 당 $108의 의료비용이 절감될 수 있음에 대해 제시하였다. Wassenberg 등(2012)은 전향적 코호트 연구로서, 중환자실 입원 환자 163명을 대상으로 Real-time PCR 기기 ‘BD GeneOhm’를 이용하여 89명, Real-time PCR 기기 ‘GeneXpert’를 이용하여 74명을 능동감시하였다. 배양검사와 중재검사를 비교했을 때, 전체 환자에게서의 격리 기간이 831일에서 463일로, 총 368일(44.3%) 감소했으며, ‘BD GeneOhm’ 기기에서는 96.0시간에서 27.6시간으로, ‘GeneXpert’ 기기에서는 21.4시간으로 MRSA 감염으로 인한 격리 기간이 감소하였다. 또한 격리 기간 감소로 인해 절감된 의료비용은 ‘BD GeneOhm’ 기기에서는 격리 기간 1일당 €136.04, ‘GeneXpert’ 기기에서는 €121.76으로 제시하였다. Wassenberg 등(2010)은 전향적 코호트 연구로서, 고위험군 입원 환자 1,764명을 대상으로 Real-time PCR 기기 ‘BD GeneOhm’를 이용하여 853명, Real-time PCR 기기 ‘GeneXpert’를 이용하여 911명을 능동감시하였다. 배양검사와 중재검사를 비교했을 때, 전체 환자에게서의 격리 기간이 6,112일에서 2,442일로, 총 3,670일(60.0%) 감소했으며, ‘BD GeneOhm’ 기기에서는 76.2시간에서 19.7시간으로, ‘GeneXpert’ 기기에서는 16.1시간으로 격리 기간이 감소하였다. 또한 격리 기간 감소로 인해 절감된 의료비용은 ‘BD GeneOhm’ 기기는 격리 1일당 €95.77, ‘GeneXpert’ 기기는 €25.43으로 제시하였다. 3편 연구에 대해 GRADE 평가 결과, 근거수준이 ‘Low’ 등급으로 평가되었다.

결론 소위원회는 현재 평가 결과에 근거하여 다음과 같이 제언하였다. 고위험군 환자(메티실린내성 황색포도알균 감염 또는 보균에 대한 감시가 필요한 사람) 대상으로 능동감시의 목적으로 사용되는 메티실린내성 황색포도알균 유전자검사[실시간 중합효소연쇄반응]의 임상적 유용성에 대해 총 7편의 문헌에 근거하여 평가하였다. 핵심 결과지표인 적절한 항생제 사용과 관련하여 근거수준이 ‘Low’ 등급이나, 중재검사를 활용한 능동감시는 기존 검사보다 신속하게 감염 또는 보균을 선별함으로써, 불필요한 항생제 사용을 감소시키는 등 적절한 항생제 사용을 가이드 할 수 있는 것으로 판단하였다. 또한 MRSA 감염률 감소, 항생제 사용 및 격리 기간 감소로 인한 의료비용 절감의 효과가 있어 임상적으로 유용하다고 판단하였다. 이에 의료기술재평가위원회는 고위험군 환자(메티실린내성 황색포도알균 감염 또는 보균에 대한 감시가 필요한 사람) 대상으로 능동감시의 목적으로 사용되는 메티실린내성 황색포도알균 유전자검사[실시간 중합효소연쇄반응]에 대하여 소위원회 검토 결과에 근거하여 다음과 같이 심의하였다(2020.11.13.). 실시간 중합효소연쇄반응은 고위험군 환자(메티실린내성 황색포도알균 감염 또는 보균에 대한 감시가 필요한 사람)를 대상으로 능동감시 목적으로 수행 시, MRSA 감염률 감소, 적정 항생제 사용 및 격리기간 감소로 인한 의료비용 절감의 효과가 있으며, 검사 소요시간이 짧아 임상적 유용성이 있을 것으로 판단하였다. 의료기술재평가위원회는 고위험군 환자(메티실린내성 황색포도알균 감염 또는 보균에 대한 감시가 필요한 사람) 대상으로 ‘메티실린내성 황색포도알균 유전자검사[실시간 중합효소연쇄반응]’를 이용한 능동감시를 권고함(권고등급 Ia)으로 심의하였다.

Background of Assessment The 'Methicillin-Resistant Staphylococcus Aureus(MRSA) genetic test [real-time polymerase chain reaction]' for active surveillance of high-risk patients is a test for active monitoring aimed at preventing MRSA transmission. This test is performed to identify MRSA infection at an early stage using a nasal swab in patients suspected of carrying MRSA. This test was approved by a new health technology assessment in Korea in 2011 (Ministry of Health and Welfare Notice No. 2011-142, 2011.11.17.), and was announced as a non-benefit service in 2012 (Ministry of Health and Welfare Notice No. 2012-116, September 2012. 14.). Accordingly, through internal monitoring, the National Evidence-based Healthcare Collaborating Agency determined that this test was an agenda that needed review of the evidence for inducing the proper use of health technology and efficient use of health and medical resources, and re-evaluation was carried out The 11th Health Technology Reassessment Committee (2020.11.13.) in 2020 deliberated on the final assessment results of the clinical safety and utility of the technology.

Purpose and Method of Assessment A systematic literature review was conducted on the medical and scientific evidence for the clinical safety and utility of active surveillance using 'MRSA genetic test [real-time PCR]' in high-risk patients (those who need monitoring for methicillin-resistant Staphylococcus aureus infection or carrier). The assessment, including the evaluation method, risk of bias evaluation, and final recommendations, was conducted through a subcommittee consisting of a total of 7 members (3 from the Department of Infectious Diseases, 2 from the Department of Laboratory Medicine, 1 from the Department of Critical Care Medicine, and 1 from the Department of Evidence-Based Medicine). The diagnostic accuracy of the MRSA genetic test [real-time PCR] has already been confirmed through the new health technology assessment. In this re-evaluation, the clinical utility of the test was evaluated with emphasis.

Assessment Results Appropriate use of antibiotics, MRSA infection rate, and medical service use (length of stay, quarantine period, medical expenses) were selected as outcome indicators to evaluate clinical utility. A total of 1,390 studies were searched through domestic and foreign databases, and a total of 1,082 studies for which duplicates were removed were reviewed. As a result of the literature review, 7 studies were finally selected.

There were three studies reporting the appropriate use of antibiotics, and all three studies were retrospective studies without a comparison group on medical outcomes (Giancola et al., 2016; Smith et al., 2017; Dangerfield et al., 2014). Giancola et al. (2016) suggested that when an interventional test was conducted for 200 pneumonia patients admitted to the intensive care unit, it could be prevented with 300 days of the empirical antibiotic treatment period for all patients and 2 days (median) of the treatment period per patient. Smith et al. (2017) found that 77.3% (309/400 patients) of the total number of patients admitted to the intensive care unit who were found to be negative as a result of the intervention test. Among them, it was reported that 54.7% (169/309 patients) had discontinued empirical antibiotic treatment. This suggests that the intervention method can reduce unnecessary antibiotic use at an early stage. Dangerfield et al. (2014) found that among 243 patients with pneumonia treated with empirical antibiotics, the proportion of patients with MRSA positive as a result of an interventional test and true positive as a culture test was 43.6% (17/39). This showed that appropriate antibiotics were used early in 43.6% of cases due to active surveillance through MRSA genetic testing. In addition, the proportion of patients who were negative in the real-time PCR test and confirmed as true negative in the culture test was 99.5% (203/204 patients), which reduces unnecessary antibiotic use due to the real-time PCR method, suggesting that the test was usefully used. As a result of the GRADE evaluation for the three studies, the level of evidence was evaluated as 'Low'.

There were two studies reporting the MRSA infection rate, and both studies were historical control studies. (Leung et al., 2013; Stano et al., 2013). Leung et al. (2013) compared a group without active surveillance (control group) and a group with active surveillance using the real-time PCR method (intervention group) on 7,950 MRSA samples detected in ICU patients. Compared with the control group, the average MRSA infection rate Of the ICU in the intervention group decreased by 1.94 cases per 1,000 days of hospitalization (3.67/1,000 patient bed days → 1.73/1,000 patient bed days). Stano et al. (2013) performed active surveillance using the real-time PCR method on 1,008 patients admitted to the intensive care unit. If the real-time PCR test result was true positive, the group with active surveillance measures, such as cohort isolation, and the group without active surveillance measures were compared while maintaining the remaining treatment the same. In the intervention group, the MRSA infection rate decreased from 2.0% (9/431 patients) to 0.3% (2/577 patients), and the risk of MRSA infection was reduced by 15% (Relative risk: 0.85(95% CI 0.02-0.63; p=0.002)). As a result of the GRADE assessment for the two studies, the level of evidence was evaluated as 'Low'.

Medical use (length of stay, quarantine period, and medical expenses) was reviewed including hospital stay, quarantine period, and medical expenses. There were three studies reporting medical use (Smith et al., 2017; Wassenberg, et al., 2012; Wassenberg et al., 2010). In a study by Smith et al. (2017), patients who were able to stop (de-escalation) antibiotic treatment early among those who were confirmed as negative as a result of real-time PCR were 54.7% (169/309 patients). Accordingly, it was suggested that $21,031 of medical expenses related to antibiotics (vancomycin) could be saved for all patients and that medical expenses of $108 per patient could be saved. In the study by Wassenberg et al. (2012), a prospective cohort study, active surveillance was conducted on 163 patients admitted to the intensive care unit, using the real-time PCR device 'BD GeneOhm' for 89 patients and using the real-time PCR device 'GeneXpert' for 74 patients. When the culture test and the intervention test were compared, the quarantine period for all patients decreased a total of 368 days (44.3%) from 831 days to 463 days. The quarantine period due to MRSA infection was reduced from 96.0 hours to 27.6 hours in the ‘BD GeneOhm’ device and 21.4 hours in the ‘GeneXpert’ device. Also, it was suggested that the medical cost savings due to the reduction of the quarantine period were €136.04 per day for the 'BD GeneOhm' device and €121.76 for the 'GeneXpert' device. In the prospective cohort study by Wassenberg et al.(2010), active surveillance was conducted for 1,764 high-risk hospitalized patients, using the real-time PCR device 'BD GeneOhm' for 853 patients and using the real-time PCR device 'GeneXpert' for 911 patients. When the culture test and the intervention test were compared, the quarantine period for all patients decreased a total of 3,670 days (60.0%) from 6,112 days to 2,442 days. The quarantine period was reduced from 76.2 hours to 19.7 hours on the 'BD GeneOhm' device and 16.1 hours on the 'GeneXpert' device. In addition, the medical costs saved due to the reduction of the quarantine period were suggested €95.77 per day for the 'BD GeneOhm' device and €25.43 for the 'GeneXpert' device. As a result of the GRADE assessment for the three studies, the level of evidence was evaluated as 'Low'.

Conclusion The subcommittee made the following recommendations based on the current evaluation results. The clinical utility of the MRSA genetic test [real-time PCR] used for active surveillance in high-risk patients (those who need monitoring for MRSA infection or carriers) was evaluated based on a total of 7 studies. Regarding the use of appropriate antibiotics, which is a critical outcome indicator, although the level of evidence is ‘Low’ grade, active surveillance using interventional tests was judged to be able to guide the use of appropriate antibiotics, such as reducing unnecessary use of antibiotics, by screening for infections or carriers more quickly than conventional tests. Also, it was judged to be clinically useful as it has the effect of reducing medical costs due to reduction of MRSA infection rate, use of antibiotics, and reduction of the quarantine period. Accordingly, the Health Technology Reassessment Committee deliberated as follows based on the subcommittee review results for the MRSA genetic test [real-time PCR] used for active surveillance in high-risk patients (those who need surveillance for MRSA infection or carriers). (2020.11.13.). Real-time polymerase chain reaction was judged to have clinical utility when it is performed for active surveillance purposes in high-risk patients (those who need surveillance for MRSA infection or carriers) because it has a short test time and the effect of reducing medical costs by decreasing the MRSA infection rate, using appropriate antibiotics, reducing the quarantine period. The Health Technology Reassessment Committee deliberated active surveillance using 'MRSA genetic test [real-time PCR]' as 'recommended' (recommendation grade Ia) for high-risk patients (those who need monitoring for MRSA infection or carrier).
