자가 골수 줄기세포 치료 - 중증 하지 허혈 - 골수 채취 후 원심분리하여 허혈부위에 주사하는 경우

Abstract

평가 배경 자가 골수 줄기세포 치료 – 중증하지허혈 – 골수 채취 후 원심분리하여 허혈부위에 주사하는 경우는 기존 치료법에 실패하거나 불가능한 Fontaine’s Stage III 혹은 IV인 폐색성 말초동맥질환, 중증 하지 허혈 환자를 대상으로 족부절단을 지연시키거나 치료 목적으로 사용되는 기술이다. 동 기술은 2012년 신의료기술평가 시 안전하고 유효한 기술로 고시된 후 2015년 비급여로 등재된 기술이다(보건복지부 고시 제2014-204호, 2015.11.27.). 동 기술은 2020년 의료기술재평가사업의 내부모니터링을 통해 재평가 주제로 발굴되었으며, 2020년 제2차 의료기술재평가위원회(2020.2.14.)에서 안전성 및 유효성에 대해 총 8인(외과(혈관외과)1인, 흉부외과 1인, 순환기내과 2인, 영상의학과 2인, 정형외과 1인, 근거기반의학 1인)으로 구성된 소위원회에서 체계적 문헌고찰을 통해 평가하도록 심의되었고, 2021년 제1차 의료기술재평가위원회(2021.1.15.)에서 최종 심의하였다.

평가 방법 동 기술의 안전성 및 유효성 평가를 위해 관련 임상진료지침 검토와 업데이트 체계적 문헌고찰 방법을 수행하였다. 모든 평가방법은 연구목적을 고려하여 “자가 골수 줄기세포 치료 – 중증하지허혈 – 골수 채취 후 원심분리하여 허혈부위에 주사하는 경우 평가 소위원회(이하 ‘소위원회’라 함)”의 심의를 거쳐 확정하였다. 관련 임상진료지침은 국내외 관련 학회 및 기관에서 발표한 임상진료지침을 검색해 검토하였다. 업데이트 체계적 문헌고찰을 위한 문헌검색은 국외 3개, 국내 5개 데이터베이스에서 검색하였다. 문헌선정 및 배제기준 및 자료추출, 비뚤림 위험평가는 모두 두 명의 검토자가 독립적으로 수행하였다. 자료분석은 합성이 가능한 자료의 경우 메타분석하여 제시하였다. 문헌의 비뚤림위험 평가는 Cochrane의 Risk of Bias를 사용하였고, 문헌의 근거수준을 평가하기 위해 Grading of Recommendations Assessment, Development and Evaluation (GRADE)를 사용하였다. 소위원회의 검토의견을 고려하여 의료기술재평가위원회에서 최종심의 후 권고등급을 제시하였다.

평가 결과 동 평가에는 임상진료지침 2편과 업데이트 체계적 문헌고찰로 총 7편의 문헌이 선택되었다. 2017년 유럽심장학회(ESC), 유럽혈관외과학회(ESVS)이 공동으로 발표한 말초동맥질환의 치료 임상진료지침에서는 만성(중증) 하지 허혈(Chronic(critical) limb-threatening ischemia, CLTI)환자에서 혈관형성 유전자 치료 및 줄기세포 치료법(Angiogenic gene and stem cell therapy)은 근거가 부족해 추가적인 연구가 필요한 기술로, 권고등급 III(권고하지 않음), 근거수준 B(데이터를 단일 RCT 혹은 대규모 nRCT에서 획득)로 언급하였다. 또한 2019년 미국혈관외과학회(SVS), 유럽혈관외과학회(ESVS), 국제혈관학회(WFVS)이 공동으로 발표한 만성(중증) 하지 허혈(CLTI) 치료 임상진료지침에서는 유전자 치료 및 세포치료법에 대해 현재 유효성을 입증하기에는 임상 3상연구가 더 필요하며, 근거가 더 축적될 때 까지 ‘연구 목적의(investigational)’ 기술로 간주되어야 한다고 언급하였다. 따라서 이 치료법을 임상시험에 등록된 만성하지허혈 환자를 대상으로 혈관형성(angiogenesis) 치료목적으로 제한적으로 사용하는 것에 대해 권고등급 1(strong), 근거수준 B(moderate)로 권고한 바 있다. 안전성 평가는 업데이트 체계적 문헌고찰로 이상 반응, 사망을 지표로 평가하였다. 이상반응을 보고한 7편의 문헌을 메타분석 한 결과, 중재군 241명 중 73명(30.3%), 대조군 203명 중 51명(25.1%)이 발생한 것으로 보고하였고, 군 간 이상반응 발생에 통계적으로 유의한 차이가 없었다(RR 0.98, 95% CI 0.86, 1.11). 보고된 이상반응으로는 백혈병, 서혜부 혈종, 발열, 뇌졸중, 부종, 하지통증, 피부궤양, 국소감염 등이 발생했다고 보고하였으나, 이들 이상반응과 중재시술과의 관련을 보고한 문헌은 2편(Teraa 등, 2015b; Powell 등, 2011)으로 제한적이었다. 사망을 보고한 6편의 문헌을 메타분석 한 결과, 중재군 213명 중 13명(6.1%), 대조군 177명 중 14명(7.9%)에서 사망을 보고하였고, 군 간 사망 발생에 통계적으로 유의한 차이가 없었다(RR 0.85, 95% CI 0.42, 1.73). 사망과 중재시술간의 연관성을 보고한 문헌은 2편(Li 등, 2013; Powell 등, 2011)으로 사망이 시술과 직접적인 연관이 없었다고 보고하였다. 유효성 평가는 업데이트 체계적 문헌고찰로 족부절단(대절단(Major Amputation), 소절단(Minor Amputation)), 무절단생존율(Amputation-free survival), 궤양호전, 발목위팔(혈압)지수(Ankle-Brachial pressure Index, ABI)증가, 경피산소압(TcPO2) 증가, 통증변화, 보행 관련 지표, 삶의 질의 지표로 평가하였다. 대절단을 보고한 7편의 문헌을 기준으로 메타분석 결과, 중재군과 대조군(placebo) 간 대절단 발생비율은 통계적으로 유의한 차이가 없는 것으로 나타났고(RR 0.81, 95% CI 0.54, 1.23), 소절단에 대해 보고한 2편을 메타분석 한 결과에서도 두 군간 통계적으로 유의한 차이가 없었다(RR 1.02, 95% CI 0.46, 2.24). 무절단생존율을 보고한 1편의 연구에서는 중재군과 대조군(placebo) 간 무절단생존율이 통계적으로 유의한 차이가 있었다고 보고하였다(p=0.0376). 궤양 크기 변화를 연속형 지표로 보고한 2편과 궤양이 호전 된 환자의 비율을 이분형으로 보고한 2편 문헌에서, 두 군 간의 유의한 차이가 없는 것으로 나타났다. 발목위팔(혈압)지수(ABI) 증가에 대해 보고한 7편의 문헌 중 ABI 지수의 변화량을 연속형으로 보고한 5편의 문헌 중 합성이 가능한 3편의 메타분석 결과 두 군간 통계적 유의한 차이가 없었다(MD 0.03, 95% CI -0.03, 0.08). ABI 지수가 개선된 환자의 비율을 이분형으로 보고한 3편의 문헌에 대해 메타분석 결과 또한 중재군과 대조군 간의 유의한 차이가 없는 것으로 나타났다(RR 4.37, 95% CI 0.80, 23.97). 통증이 개선된 환자의 비율을 보고한 3편의 문헌 중 합성이 가능한 2편의 문헌의 메타분석 결과 중재군과 대조군 간의 통계적으로 유의한 차이가 없는 것으로 나타났고(RR 0.97, 95% CI 0.54, 1.72), 통증변화를 연속형 지표로 보고한 4편, 경피산소압(TcPO2)을 보고한 4편, 보행관련지표를 보고한 1편, 삶의 질을 보고한 3편의 문헌에서 모두 군 간 통계적으로 유의한 차이를 보이지 않았다고 보고하였다.

결론 및 제언 자가 골수 줄기세포 치료 – 중증하지허혈 – 골수 채취 후 원심분리하여 허혈부위에 주사하는 경우의 소위원회는 안전성과 관련하여 위약군(placebo)과 비교한 결과 이상 반응과 사망 결과에서 문헌의 근거수준은 ‘보통(Moderate)'로 평가하였다. 하지만 모두 통계적 유의한 차이가 없고, 이상 반응 및 사망이 시술과 직접적 연관성이 있다고 보고한 문헌이 부족하며, 안전성을 판단하기에 근거가 부족한 기술로 평가하였다. 소위원회는 유효성과 관련하여 주요지표인 대절단/소절단, 무절단생존율, 궤양호전 결과에서 중재군의 효과가 일관되게 보고되지 않았으며, 기타지표(ABI, TcPO2, 통증변화, 보행관련지표, 삶의 질)에서 또한 유의한 개선효과를 확인할 수 없어 동 기술은 유효성이 없는 기술로 평가하였다. 문헌의 근거수준 또한 일부 문헌에서 ‘연구참여자, 연구자에 대한 눈가림’, ‘불완전한 결과자료’ 에 대해 주요 지표의 비뚤림 위험이 높게 평가된 것을 고려하였으며, ‘Moderate’와 ‘High’ 로 평가하였다. 이에 의료기술재평가위원회는 ‘자가 골수 줄기세포 치료 – 중증하지허혈 – 골수 채취 후 원심분리하여 허혈부위에 주사하는 경우’에 대해 소위원회의 검토결과에 근거하여 다음과 같이 심의하였다(2021.1.15.). 의료기술재평가위원회는 중증 하지 허혈 환자의 혈관신생을 유도하여 족부절단 지연, 피부궤양 치료, 기능적 향상을 위해 자가 골수 줄기세포 치료 – 중증 하지 허혈 – 골수 채취 후 원심분리하여 주사하는 것에 대해 권고하지 않음(권고등급 II)으로 심의하였다. 권고사유는 다음과 같다. 자가 골수 줄기세포 치료 – 중증하지허혈 – 골수 채취 후 원심분리하여 허혈부위에 주사하는 경우는 위약군(placebo)과 비교하여, 안전성과 관련하여 이상 반응 및 사망의 군간 유의한 차이를 확인할 수 없고, 중재시술과 직접적으로 연관성이 있다고 보고한 문헌이 부족하여, 안전성을 판단할 수 있는 근거가 충분하지 않으며, 유효성과 관련된 주요 지표에서 군간 통계적으로 유의한 차이를 확인할 수 없어 동 기술은 유효성이 없는 기술로 판단하였다.

Background The concerned technology, Autologous Bone Marrow Stem Cell Treatment - in patient with Critical Limb Ischemia - Injection of Centrifuged Bone Marrow at Ischemic Area, is used to delay foot amputation in patient with Fontaine’s Stage III or IV occlusive peripheral arterial disease or severe critical limb ischemia or treat the same diseases.

With its safety and effectiveness announced as a result of New Health Technology Assessment in 2012, the technology was publicly announced (by the Ministry of Health and Welfare as Public Announcement #2014-204, Nov. 27, 2015) as a new technology as a non-benefit item. Becoming subject to reassessment in the internal monitoring process as part of Health Technology Reassessment Project 2020, the technology was brought to the 2nd meeting of Health Technology Reassessment Committee in 2020 (Feb. 14, 2020), followed by the safety and effectiveness review in the competent sub-committee comprised of 8 different specialists (1 vascular surgeon, 1 thoracic surgeon, 2 cardiovascular physicians, 2 radiologists, 1 orthopedic surgeon, 1 evidence-based medicine specialist) by way of systematic literature review(SLR), and the final screening for conclusion in the 1st Health Technology Reassessment Committee in 2021 (Jan. 15, 2021).

Methodology Review of the relevant clinical therapeutic guidelines and systematic literature review(SLR) was conducted for assessment of safety and effectiveness of the technology. Taking into account the study's purpose, all methodologies were screened by the sub-committee in charge of assessing Autologous Bone Marrow Stem Cell Treatment - in patient with Critical Limb Ischemia - Injection of Centrifuged Bone Marrow at Ischemic Area (the “sub-committee”) into confirmation.

In this study, relevant clinical therapeutic guidelines mean clinical therapeutic guidelines announced by the local and international academic society and institutions. These guidelines were searched out for review. Three different international literature databases and five different domestic literature databases were referred to in selection of literatures to be reviewed in systematic literature review(SLR). This review, as well as data extraction and risk of bias assessment, was conducted under literature inclusion and exclusion criteria and by two separate reviewers. The synthesizable types of data were meta-analyzed. Cochrane’s Risk of Bias Tool was used for risk of bias assessment. Grading of Recommendations Assessment, Development and Evaluation (GRADE) was used to present evidence for making clinical practice recommendations. The final screening for conclusion of result and grading of the technology taking into account the sub-committee's opinion was held in the Health Technology Reassessment Committee.

Result In this study, two clinical therapeutic guidelines and 7 literatures for SLR were selected for assessment. According to the clinical therapeutic guideline published jointly by European Society of Cardiology(ESC) and European Society of Vascular Surgery(ESVS) in 2017, angiogenic gene and stem cell therapy in chronic(critical) limb-threatening ischemia(CLTI) patient has less than enough evidence and needs more researches to be done and is graded Recommendation Grade III (not recommended) and Evidence Level B (data derived from single randomized clinical trial or multiple nonrandomized clinical trials). According to the clinical therapeutic guideline on CLTI published jointly by The Society for Vascular Surgery(SVS), European Society of Vascular Surgery(ESVS) and World Federation of Vascular Societies(WFVS) in 2019, the technology needs additional Phase 3 studies to verify effectiveness of gene therapy and cell therapy and is considered as an ‘investigational’ technology until backed by further evidences by grading the technology as, when used for the restrictive purpose of treating angiogenesis in the registered subjects being CLTI patients, Recommendation Grade 1 (Strong) and Evidence Level B (Moderate).

Safety was assessed by way of SLR, using ‘Adverse Event’ and ‘Death’ as viable indices.

According to meta analysis of a total of 7 literatures where at least 1 adverse event was reported, 73 out of 241 Study Group subjects (30.3%) and 51 out of 203 Control Group subjects (25.1%) were reported to manifest adverse event, with no statistically significant inter-Group difference (RR 0.98, 95% CI 0.86, 1.11). Reported adverse events were leukemia, inguinal hematoma, fever, cerebral stroke, edema, leg pain, skin ulcer and focal inflammation. Out of those, adverse events from only 2 literatures (Teraa et al, 2015b; Powell et al, 2011) were reported to be related to surgical interventions taken. According to meta analysis of a total of 6 literatures reporting death, 13 out of 213 Study Group subjects (6.1%) and 14 out of 177 Control Group subjects (7.9%) were reported dead, with no statistically significant inter-Group difference (RR 0.85, 95% CI 0.42, 1.73). Despite 2 literatures (Li et al, 2013; Powell et al, 2011) reporting possible relationship between death and surgical interventions taken, it was determined the dead cases were not directly related to surgical interventions taken.

Meanwhile, effectiveness of the technology was assessed, by way of SLR, using such indices as major foot amputation, minor foot amputation, amputation-free survival, improvement of ulcer healing, increase in Ankle-Brachial pressure Index(ABI), increase in transcutaneous oxygen pressure(TcPO2), changes in pain, ambulatory indices and quality of life indices. According to meta analysis of a total of 7 literatures reporting major foot amputation, no statistically significant inter-Group difference between Study Group and Control (Placebo) Group was observed (RR 0.81, 95% CI 0.54, 1.23). According to meta analysis of a total of 2 literatures reporting minor foot amputation, no statistically significant inter-Group difference between Study Group and Control (Placebo) Group was observed (RR 1.02, 95% CI 0.46, 2.24). According to meta analysis of a literature reporting amputation-free survival, statistically significant inter-Group difference between Study Group and Control (Placebo) Group was observed (p=0.0376). According to meta analysis of a total of 2 literatures reporting continuous index of change in ulcer size and another 2 literatures reporting binary data of the percentage of subjects with improvement of ulcer healing, no statistically significant inter-Group difference was observed. According to meta analysis of 5 out of 7 literatures reporting increase in Ankle-Brachial pressure Index(ABI) - the meta-analyzed 5 literatures reported ABI as continuous variable, in 3 out of those 5 literatures with synthesizable data, no statistically significant inter-Group difference was observed (MD 0.03, 95% CI -0.03, 0.08). According to meta analysis of a total of 3 literatures reporting binary data of the percentage of subjects with increase in ABI, no statistically significant inter-Group difference between Study Group and Control (Placebo) Group was observed (RR 4.37, 95% CI 0.80, 23.97). According to meta analysis of 2 out of 3 literatures with synthesizable data, no statistically significant inter-Group difference was observed (RR 0.97, 95% CI 0.54, 1.72). According to meta analysis of 4 literatures reporting changes in pain as continuous index, another 4 literatures

reporting increase in transcutaneous oxygen pressure(TcPO2), 1 literature reporting ambulatory indices, and 3 literatures reporting quality of life indices, no statistically significant inter-Group difference was observed.

Conclusion and Suggestion As regards the concerned technology, Autologous Bone Marrow Stem Cell Treatment - in patient with Critical Limb Ischemia - Injection of Centrifuged Bone Marrow at Ischemic Area, the sub-committee assessed that, as a result of inter-Group comparison between Study Groups and Control (Placebo) Groups of safety, the Evidence Level of the relevant literatures is B (Moderate) in evidencing relationship between adverse events and death. With no statistically significant inter-Group difference observed, only few literatures reported direct relationship between adverse events and death. Therefore, it was determined that safety of the concerned technology is not sufficiently evidenced.

As regards effectiveness of the concerned technology, such major indices as major foot amputation, minor foot amputation, amputation-free survival and improvement of ulcer healing were not reported with consistency. With no statistically significant improvement in other indices (ABI, TcPO2, changes in pain, ambulatory indices, quality of life), it was determined that the concerned technology is not effective. As regards evidence level, some relevant literatures had high risk of bias when considering ‘blindedness of investigators and subjects’ and ‘incomplete result data.’ Therefore, the Evidence Level of the technology was assessed ‘Moderate’ or ‘High.’

With the above being said, based on the assessment result by the sub-committee, the Health Technology Reassessment Committee screened Autologous Bone Marrow Stem Cell Treatment - in patient with Critical Limb Ischemia - Injection of Centrifuged Bone Marrow at Ischemic Area as follows:

The Health Technology Reassessment Committee has screened that use of Autologous Bone Marrow Stem Cell Treatment - in patient with Critical Limb Ischemia - Injection of Centrifuged Bone Marrow at Ischemic Area in inducing angiogenesis in patient with Critical Limb Ischemia for delayed foot amputation, curing of skin ulcer and improvement of function is not recommended (Recommendation Grade II). The reason for such grading is as follows:

As for safety, no statistically significant inter-Group difference in relationship between adverse events and death between Control (Placebo) Group and Study (Autologous Bone Marrow Stem Cell Treatment - in patient with Critical Limb Ischemia - Injection of Centrifuged Bone Marrow at Ischemic Area) Group was observed. Only few literatures reported direct relationship between adverse events and death. No statistically significant inter-Group difference in effectiveness-related indices was observed. Therefore, it was determined that safety of the concerned technology is not effective.
