위암에서의 NK 세포 활성도 검사 [정밀면역검사]

Abstract

평가 배경 NK 세포 활성도 검사[정밀면역검사]는 위암 환자를 대상으로 환자의 혈액을 NK 세포 활성물질이 첨가된 튜브에서 배양한 후 효소면역분석법으로 인터페론 감마(IFN-γ)를 측정하여 환자의 상태 확인 및 치료경과 모니터링을 위해 수행되는 검사이다. 동 기술은 2014년 제4차 신의료기술평가위원회(2014.04.25.)에서 안전성 및 유효성이 있는 기술로 심의 및 신의료기술평가 결과 고시(제2014-89호)되었고, 선별급여로 등재(보건복지부 고시 제2016-104호)되었으며, 이후 건강보험심사평가원에서는 동 기술에 대한 급여적용 타당성 판단 등 의사결정에 필요한 근거자료를 도출하기 위해 재평가를 본원에 의뢰하였다. 이에 따라 동 기술에 대해 소위원회를 구성하여 안전성 및 유효성 평가를 수행하였다.

위원회 운영 총 11인으로 구성된 소위원회는 2020년 6월부터 8월까지 약 3개월간 총 3회의 소위원회 회의를 진행하였다. 2020년 9차 의료기술재평가위원회(2020.09.11.)에서는 동 기술에 대한 평가결과를 1차로 심의하여 다음 차수에 권고등급까지 심의하기로 결정하였고, 10차 의료기술재평가위원회(2020.10.16.)에서는 본 안건에 대한 평가결과 및 권고등급에 대한 최종 심의가 이루어졌다.

평가방법 본 연구에서는 위암 환자에 대하여 선별급여로 등재(2016.07.01.적용)된 ‘NK 세포 활성도 검사[정밀면역검사]’의 안전성 및 유효성에 대한 체계적 문헌고찰을 수행하였다. 평가 결과

평가대상 문헌 선정 국내외 데이터베이스를 통해 총 1,298편(국외 1,244편, 국내 54편)이 검색되었고, 최종 1편의 국외 문헌이 선정되었다. 해당 문헌에서 보고된 안전성 및 유효성 관련 내용은 아래와 같다.

안전성 검사관련 부작용에 대해서는 보고된 내용이 없었다.

유효성 위암 환자군의 인터페론 감마 수치가 건강인에 비해 낮게 보고되었다. 위암 환자에 대한 치료과정을 모니터링하는 지표를 해당 연구에서 보고하고 있지 않아 이에 대해서는 유효성이 있는지를 확인할 수 없었다.

결론 위암 환자에 대한 NK 세포 활성도 검사[정밀면역검사] 수행에 대한 체계적 문헌고찰 수행 결과, 평가대상 기술을 위암 환자에게 적용 시 안전성이 있는지는 확인할 수 없었고, 유효성 관련해서는 치료과정을 모니터링하는 지표를 평가대상 문헌에서 보고하지 않아 이에 대해서는 확인할 수 없었다. NK 세포 활성도 검사[정밀면역검사]는 위암 환자를 대상으로 환자의 상태확인 및 치료경과 모니터링을 위해 수행되는 검사로 안전성과 유효성을 입증하기에는 축적된 문헌적 근거가 부족하여 동 기술의 안전성 및 유효성을 확인할 수 없는 기술로 판단하였다. 이에 의료기술재평가위원회는 의료기술재평가사업 관리지침 제4조제10항에 의거 NK 세포 활성도 검사[정밀면역검사]를 위암 환자를 대상으로 환자의 상태확인 및 치료경과 모니터링의 목적으로 시행하는 것을 권고하지 않음(권고등급 II)으로 결정하였다(2020.10.16.).

Background of Assessment The NK cell activity test [High-quality immunoassay] is a test performed for gastric cancer patients to check their condition and monitor the progress of treatment by measuring the interferon-gamma (IFN-γ) using enzyme immunoassay after incubation of the patient's blood in a tube containing NK cell-active substances. At the 4th New Health Technology Assessment Committee (2014.04.25.) in 2014, this technology was reviewed as a technology with safety and effectiveness, the results of the new health technology assessment were noticed (No. 2014-89), and it was registered as a selective benefit (Ministry of Health and Welfare Notice No. 2016-104). Afterward, the Health Insurance Review and Assessment Service requested a re-assessment to derive the necessary data for decision-making, such as determining the feasibility of applying the benefit to the technology. Accordingly, a subcommittee was formed for this technology to evaluate safety and effectiveness.

Committee’s Operation The subcommittee consisted of a total of 11 members and held a total of three subcommittee meetings for about three months from June to August 2020. In 2020, the 9th Health Technology Reassessment Committee (2020.09.11.) decided to review the assessment results of this technology at the first and then to the grade of recommendation in the next round. The final deliberation was conducted on the assessment results and grades of recommendations for this issue at the 10th Health Technology Reassessment Committee (2020.10.16.).

Assessment Methods In this study, a systematic literature review was conducted on the safety and effectiveness of the 'NK cell activity test [High-quality immunoassay]' registered as a selective benefit for gastric cancer patients (applied on July 1, 2016).

Assessment Results Selection of literature subject to assessment A total of 1,298 pieces of literature (1,244 foreign articles, 54 domestic articles) were searched through domestic and foreign databases, and one foreign article was finally selected. The safety and effectiveness-related contents reported in the literature are as follows. There were no reports of side effects related to safety tests.

Safety There were no reports of test-related side effects.

Effectiveness The interferon-gamma level of the gastric cancer patient group was reported to be lower than that of the healthy volunteer group. Since the study did not report an indicator for monitoring the treatment process for patients with gastric cancer, it could not be confirmed whether it was effective.

Conclusion As a result of a systematic literature review on the NK cell activity test [High-quality immunoassay] for gastric cancer patients, it was not possible to confirm whether the technology to be evaluated was safe when applied to patients with gastric cancer. Regarding effectiveness, it could not be confirmed because indicators for monitoring the treatment process were not reported in the literature to be assessed. The NK cell activity test [high-quality immunoassay] is a test performed for patients with gastric cancer to check the patient's condition and monitor the progress of treatment. It was judged as a technology that could not confirm the safety and effectiveness of the technology due to insufficient literature evidence to prove its safety and effectiveness. Therefore, the Health Technology Reassessment Committee decided not to recommend that NK cell activity test [high-quality immunoassay] be performed for gastric cancer patients to check the patient's condition and monitor the progress of treatment in accordance with Article 4, Paragraph 10 of the Management Guidelines for the Health Technology Reassessment Project (Recommendation Grade II) (2020.10.16.).
