초음파 유도하 치핵동맥결찰술

Abstract

평가 배경 초음파 유도하 치핵동맥결찰술은 치핵내의 동맥을 도플러 유도에 의해 결찰하여 혈류를 차단함으로써 치핵조직을 축소시켜서 치핵을 치료하는 기술이다. 동 기술은 2008년 5월부터 행위비급여로 건강보험에 등재(보건복지가족부 고시 제2008-30호, 2008.4.29.)되어 지금까지 사용 중인 의료기술로 2022년도 제3차 의료기술재평가위원회(2022.3.11.)에서 체계적 문헌고찰을 통해 해당 기술의 임상적 안전성 및 효과성에 대한 최신 근거를 확인하여 의료기술의 적정 사용을 지원하고자 의료기술재평가를 수행하기로 하였다.

평가 방법 초음파 유도하 치핵동맥결찰술에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였고 국민 참여단으로부터 사회적 가치를 평가하였다. 모든 평가방법은 평가목적을 고려하여 “초음파 유도하 치핵동맥결찰술 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 평가의 핵심질문은 “치핵 및 항문주위정맥혈전증 환자에서 초음파 유도하 치핵동맥결찰술은 임상적으로 안전하고 효과적인가?”이며, 안전성은 수술 중 이상반응 및 수술 후 이상반응 지표로, 효과성은 9개로 범주화하여 출혈, 치핵증상완화, 재시술, 첫 배변까지의 기간, 수술 관련 지표(수술시간, 재원기간, 응급실 또는 병원 내원, 진통제 사용, 수술 후 통증), 재발, 환자만족도, 삶의 질, 일상 또는 직장으로의 회복기간 지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하여 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림위험은 연구유형 별로 Cochrane의 Risk of Bias (RoB)를 사용하여 두 명의 검토자가 독립적으로 평가하여 의견합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하였으며 자료분석은 양적 분석(quantitative analysis)이 가능할 경우 메타분석을 수행하였으며, 불가능할 경우 질적 검토(qualitative review) 방법을 적용하였다. 체계적 문헌고찰 결과의 근거 수준은 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 접근 방법으로 평가하였다. 또한, 국민참여단을 대상으로 사회적 가치평가는 온라인 설문조사를 통해 4개의 평가영역을 평가하였다.

평가 결과 동 평가에 포함된 문헌은 총 30편의 무작위배정 임상시험 연구였으며, 안전성 및 효과성의 결과는 다음과 같다.

안전성 동 기술의 안전성은 수술 중, 후 이상반응에 대해 비교시술별로 구분하여 살펴보았다. 보조술식과 비교한 총 9편의 연구 중 수술 중 이상반응을 보고한 문헌은 없었다. 수술 후 이상반응은 8편에서 시술과 관련된 출혈, 배변기능 장애, 요폐, 협착, 감염 등 다양한 지표가 보고되었다. 메타분석 결과, 전체 이상반응(overall adverse events) 발생은 두 군간 유의한 차이가 없었고, 시술 관련 출혈 및 요폐는 대조군에 비해 중재군에서 유의하게 더 많이 발생하였다. 이외 다른 안전성 지표(예: 협착, 배변기능 장애)들은 질적분석 결과, 두 군간 발생률이 유사하였다. 치핵수술과 비교한 연구는 총 21편이었다. 그중 7편에서 수술 중 이상반응으로 수술 중 출혈 및 기기 오작동이 보고하였으며, 메타분석 결과 수술 중 출혈은 두 군간 유의한 차이가 없었다. 기기 오작동은 중재군에서는 2~6%, 대조군에서는 1~4% 발생한 것으로 보고되었다. 수술 후 이상반응을 보고한 20편에서 배변기능 장애, 요폐, 시술과 관련된 출혈, 항문 주위 혈전, 협착 등 다양한 지표가 보고되었다. 메타분석 결과, 전체 이상반응(overall adverse events)은 두 군간 유의한 차이가 없었으며, 요폐발생은 대조군에 비해 중재군에서 유의하게 더 낮았다. 이외 다른 지표들은 두 군간 유의한 차이가 없었으나 심각한 합병증에 해당하는 협착이 대조군에서만 발생(3명)이 보고되었다.

효과성 초음파 유도하 치핵동맥결찰술의 효과성은 출혈, 치핵증상, 재시술, 첫 배변까지의 기간, 수술 관련 지표(수술시간, 재원기간, 응급실 또는 병원 내원, 진통제 사용, 수술 후 통증), 재발, 환자만족도, 삶의 질, 일상 또는 직장으로의 회복기간으로 범주화(9개)하여 비교기술별로 구분하여 평가하였다. 보조술식과 비교한 연구에서 출혈을 보고한 2편 중 1편에서는 두 군간 출혈 발생이 유의한 차이가 없었으나 다른 1편에서는 대조군에 비해 중재군에서 경미한 출혈 발생이 더 많았지만, 통계적 유의성에 대해서는 보고하지 않았다. 메타분석 결과(3편) 대조군에 비해 중재군에서 수술시간이 유의하게 더 길었다. 재원기간을 보고한 2편 중 1편에서는 두 군간 유의한 차이가 없었으며, 다른 1편에서는 중재군에서 재원기간이 유의하게 길었다. 진통제 사용은 3편에서 보고되었는데, 2편에서는 두 군간 진통제 사용기간이 유의한 차이가 없었지만, 나머지 1편에서는 중재 직후 시점 대조군에 비해 중재군에서 진통제 사용기간 및 사용량이 유의하게 더 많았다. 시술 후 재발은 메타분석(5편) 결과 대조군에 비해 중재군에서 유의하게 더 적었다. 환자만족도를 보고한 2편 중 1편에서는 대조군에 비해 중재군에서 환자만족도 점수가 유의하게 낮았으나, 다른 1편에서는 시술에 만족한 환자 비율에 두 군간 유의한 차이가 없었다. 삶의 질은 2편에서 보고되었는데, 1편에서는 추적관찰 1-7일 시점에서는 두 군간 삶의 질에 유의한 차이가 있었으나, 이후(21일-1년 시점) 차이가 없었다. 다른 1편에서는 추적관찰 1개월 및 1년 시점에서 삶의 질 영역 중 대부분에서 대조군에 비해 중재군에서 유의하게 삶의 질 점수가 더 낮았다. 일상 또는 직장으로의 회복기간을 보고한 3편 중 2편에서는 대조군에 비해 중재군에서 유의하게 길었으며, 나머지 1편에서는 중재군에서의 회복기간이 더 짧았으나 그 차이가 통계적으로 유의하지는 않았다.치핵증상개선, 재시술, 수술 후 통증(환자 수 및 점수)은 메타분석 결과, 두 군간 유의한 차이가 없었다. 시술 후 첫 배변까지의 기간, 응급실 또는 병원 내원 환자의 경우 보고된 문헌이 없었다. 치핵수술과 비교한 연구에서 메타분석 결과, 대조군에 비해 중재군에서 출혈 발생(11편)이 유의하게 적었다. 치핵증상은 중재군에서 치핵증상 완화비율이 더 적었으며(4편), 치핵증상완화 점수(3편)는 두 군간 유의한 차이가 없었다. 대조군에 비해 중재군에서 재시술을 받은 환자 수가 유의하게 많았다(7편). 수술 관련 지표 중 수술시간 및 수술 후 통증 점수를 제외한 다른 세부지표들에서 두 군간 차이가 없었다. 중재군에서의 수술시간(14편)은 대조군에 비해 유의하게 짧았고, 수술 후 통증 점수(10편)도 대조군에 비해 중재군에서 더 낮았다. 대조군에 비해 중재군에서 재발(10편)이 유의하게 높았다. 첫 배변까지의 기간, 환자만족도, 삶의 질, 일상 또는 직장으로의 회복기간은 메타분석 결과, 두 군간 유의한 차이가 없었다.

결론 및 제언 해당 소위원회는 현재 평가결과에 근거하여 다음과 같이 제언하였다. 초음파 유도하 치핵동맥결찰술의 합병증 발생률은 다른 보조술식 및 치핵수술과 전체적으로 유사한 수준이며, 치핵수술과 비교한 연구에서 심각한 합병증에 해당하는 협착은 치핵수술에서만 보고되어 소위원회는 치핵수술에 비해 초음파 유도하 치핵동맥결찰술이 상대적으로 안전한 술식으로 판단하였다. 동 기술은 다른 보조술식보다 재발이 유의하게 적었고, 치핵수술보다 출혈 및 수술 후 통증 점수는 유의하게 개선되었으나, 재발 및 재시술은 유의하게 많았다. 또한 치핵수술에서보다 치핵동맥결찰술에서 치핵증상완화는 유의하게 낮았다. 소위원회는 이를 치핵수술은 치핵을 제거하는 근본적인 수술인데 반해 초음파 유도하 치핵동맥결찰술은 치핵을 결찰하는 시술로 치핵수술과 비교하여 우월한 효과를 보이기는 어렵다고 판단하였다. 다른 효과지표에서는 치핵동맥결찰술과 보조술식 및 치핵수술간의 차이가 없었다. 이에 소위원회는 해당 시술을 다른 보조술식 및 치핵수술과 효과성이 유사한 의료기술로 판단하였으며, 특히 치핵수술보다 안전한 기술로써 치핵 치료대안 중 하나가 될 수 있다고 판단하였다. 2022년 제12차 의료기술재평가위원회(2022.12.9.)에서는 소위원회 검토 결과에 대하여 논의한 결과, 의료기술재평가사업 관리지침 제4조제10항에 의거 “초음파 유도하 치핵동맥결찰술”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 치핵 및 항문주위정맥혈전증 환자에서 치핵 치료 목적으로 초음파 유도하 치핵동맥결찰술을 사용하는 데 있어 치핵수술보다 안전한 기술로써 재발과 같은 지표에서는 효과가 떨어지지만 수술 후 통증을 원하지 않는 환자 및 짧은 회복기간을 원하는 환자에게 치핵 치료 대안 중 하나가 될 수 있다고 판단하였다. 이에 의료기술재평가위원회는 ‘치핵 및 항문주위정맥혈전증 환자에서 치핵 치료 목적으로 사용하는데 있어 초음파 유도하 치핵 동맥결찰술’을 ‘조건부 권고함’으로 심의하였다.

Background Doppler ultrasound-guided hemorrhoidal artery ligation (DG-HAL) is a health technique used to treat hemorrhoids. Here, the hemorrhoidal artery is ligated under Doppler ultrasound guidance to restrict blood flow to the hemorrhoidal tissue to reduce its volume. This technique has been used since it was listed in national health insurance as a non-benefit service in May 2008 (Ministry of Health and Welfare Notice No. 2008-30; April 29, 2008). The third session of the Committee of Health Technology Reassessment, on March 11, 2022, made a decision to conduct a health technology reassessment to support the appropriate use of this technology by systematically reviewing the latest evidence of its clinical safety and effectiveness.

Methods A systematic review was performed to assess the safety and effectiveness of DG-HAL and a questionnaire survey was conducted with a public participation group to assess its social value. The assessment methods were finalized after review by the DG-HAL subcommittee (hereinafter referred to as the “subcommittee”) after considering the purpose of the assessment. The key question of the assessment was, “Is DG-HAL clinically safe and effective in patients with hemorrhoids or perianal venous thrombosis?” Safety was assessed for intraoperative and postoperative adverse events (AEs), while effectiveness was assessed for nine categories of indicators: bleeding, alleviation of hemorrhoid symptoms, reoperation, time to first defecation, surgery-related indicators (operating time, length of hospital stay, emergency room or hospital visits, use of analgesics, and postoperative pain), recurrence, patient satisfaction, quality of life (QoL), and time to return to daily life or work. In the systematic review, three international and five domestic databases were searched based on the key question and two reviewers independently screened and selected the articles in accordance with the inclusion and exclusion criteria. Two reviewers independently performed a risk of bias (RoB) assessment, using the Cochrane RoB tool, by type of study to reach a consensus. Data extraction was performed using a predetermined format. Data were analyzed using meta-analysis to check if a quantitative analysis was possible, if not, a qualitative review would be adopted. The systematic review results were assessed for the level of evidence using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) method. Furthermore, an online questionnaire survey was conducted with a public participation group to assess four domains of social value.

Results The articles included in this assessment were from 30 randomized controlled trials (RCTs) and the safety and effectiveness results were as follows.

Safety The safety of the use of this health technology was assessed by comparing intraoperative and postoperative AEs with those of alternative procedures. Among the nine articles that compared this health technology with alternative procedures, no articles reported any intraoperative AEs. However, eight articles reported postoperative AEs, which included procedure-related bleeding, defecatory dysfunction, urinary retention, stricture, and infection. Meta-analysis results showed no significant difference in overall AEs between the two groups, but there was a significantly higher incidence of procedure-related bleeding and urinary retention in the intervention group than in the control group. With respect to other safety indicators, such as stricture or defecatory dysfunction, qualitative review results showed similar incidence rates between the two groups. Among 21 studies with comparisons to hemorrhoid surgery, seven studies reported intraoperative bleeding and equipment malfunction as intraoperative AEs. Meta-analysis results showed no significant differences in intraoperative bleeding between the two groups, while equipment malfunction occurred in 2%–6% of cases in the intervention group and 1%–4% of cases in the control group. In the 20 studies that reported on postoperative AEs, the safety indicators included defecatory dysfunction, urinary retention, procedure-related bleeding, perianal thrombi, and strictures. Meta-analysis results showed no significant difference in overall AEs between the two groups, but the incidence of urinary retention was significantly lower in the intervention group than in the control group. There were no significant differences in other safety indicators between the two groups, but strictures, that could be considered a serious complication, were only found in the control group (n=3).

Effectiveness The effectiveness of DG-HAL was assessed by comparing DG-HAL to alternative procedures based on nine categories: bleeding, hemorrhoid symptoms, reoperation, time to first defecation, surgery-related indicators (operating time, length of hospital stay, emergency room or hospital visits, use of analgesics, and postoperative pain), recurrence, patient satisfaction, QoL, and time to return to daily life or work. Of the two studies that compared DG-HAL with alternative procedures, one study reported no significant difference in bleeding between the two groups, whereas the other study reported more frequent mild bleeding in the intervention group than in the control group. However, statistical significance was not reported. Meta-analysis (n=3) results showed that operating time was significantly longer in the intervention group than in the control group. Of the two studies that reported on the length of hospital stay, one reported no significant difference between the two groups, whereas the other reported a significantly longer length of hospital stay in the intervention group. Of the three studies that reported on the use of analgesics, two reported no significant differences in the duration of analgesic use between the two groups, whereas the other study reported that the duration and amount of analgesic use were significantly higher in the intervention group than in the control group immediately after the intervention. Meta-analyses (n=5) on postoperative recurrence showed that recurrence was significantly lower in the intervention group than in the control group. Of the two studies that reported on patient satisfaction, one reported that patient satisfaction scores were significantly lower in the intervention group than in the control group, whereas the other study reported no significant difference in the proportion of patients satisfied with the procedure between the two groups. QoL was reported in two studies, with one study reporting a significant difference in QoL between the two groups at 1–7 days of follow-up, but no difference at subsequent time points (21 days to 1 year of follow-up). In the other study, the intervention group showed significantly lower scores than the control group in most of the QoL domains at the 1-month and 1-year follow-ups. Of the three studies that reported on time to return to daily life or work, two studies reported significantly longer times in the intervention group than in the control group, whereas the other study reported a shorter time in the intervention group, but the difference was not statistically significant. Meta-analyses on improvements in hemorrhoid symptoms, reoperation, and postoperative pain (number of patients and severity score) found no significant differences between the two groups. There were no articles that reported on time to first defecation after the procedure and emergency room or hospital visits. In the meta-analysis of studies with comparisons to hemorrhoid surgery, 11 studies reported significantly less bleeding in the intervention group than in the control group. Four studies reported that the proportion of hemorrhoid symptom alleviation was lower in the intervention group, while three studies reported no significant differences in hemorrhoid symptom alleviation scores between the two groups. Moreover, the number of patients who received a reoperation was significantly higher in the intervention group than in the control group in seven studies, while there were no differences in other surgery-related indicators, other than operating time and postoperative pain scores, between the two groups. Operating time was significantly shorter in the intervention group in 14 studies and postoperative pain scores were lower in the intervention group than in the control group in 10 studies. Furthermore, recurrence was significantly higher in the intervention group than in the control group in 10 studies. There were no significant differences in time to first defecation, patient satisfaction, QoL, and time to return to daily life or work between the two groups.

Conclusions and recommendations Based on the current assessment results, the subcommittee recommended the following. DG-HAL showed an overall complication rate that was similar to alternative procedures and hemorrhoid surgery, and in studies that compared DG-HAL and hemorrhoid surgery, strictures that could be considered a serious complication were found only in the hemorrhoid surgery group. Accordingly, the subcommittee determined that DG-HAL was a relatively safe procedure compared to hemorrhoid surgery. DG-HAL showed significantly fewer recurrences than alternative procedures. DG-HAL showed significant improvements in bleeding and postoperative pain scores, but also significantly higher rates of recurrence and reoperation than hemorrhoid surgery. Moreover, the alleviation of hemorrhoid symptoms was significantly lower in the DG-HAL group than in the hemorrhoid surgery group. The subcommittee determined that it would be difficult for DG-HAL, which involves ligation of hemorrhoidal tissue, to show superior effectiveness compared with hemorrhoid surgery, which is a radical procedure to remove hemorrhoidal tissue. There were no differences in other effectiveness indicators between the DG-HAL group and the alternative procedure or hemorrhoid surgery groups. Accordingly, the subcommittee determined that DG-HAL had similar effectiveness to alternative procedures and hemorrhoid surgery, and that it could be a treatment option for hemorrhoids as a procedure that is safer than hemorrhoid surgery. During the twelfth session of the Committee of Health Technology Reassessment, on December 9, 2022, the committee discussed the subcommittee findings and reached the following conclusions in accordance with Article 4-10 of the Guidelines for the Management of Health Technology Reassessment Project. The Committee of Health Technology Reassessment determined that DG-HAL is a safer procedure than hemorrhoid surgery when used to treat patients with hemorrhoids or perianal venous thrombosis, and despite being less effective with respect to indicators such as recurrence, DG-HAL can be a treatment option for patients with hemorrhoids who do not want postoperative pain and want a shorter recovery period. Accordingly, the Committee of Health Technology Reassessment “conditionally recommended” the use of DG-HAL to treat patients with hemorrhoids or perianal venous thrombosis.
