수술 중위험 대상 경피적 대동맥판삽입

Abstract

평가 배경 및 목적 경피적 대동맥판삽입(Transcatheter Aortic Valve Replacement, 이하 TAVR)은 중증 대동맥판협착증의 치료를 위해 스텐트 형태의 인공 판막을 경피적으로 삽입하는 시술로 2015년 6월부터 실시 의료기관 제한 및 시술 자료를 제출하는 조건으로 조건부 선별급여(본인부담률 80%)로 도입되었다. 미국 FDA에서는 수술 저위험군까지 허가되었고(2019년), 기존 국내 연구에서도 수술 중ㆍ저위험군의 경우 향후 근거 자료축적 후 재평가가 필요하다고 보고하였다(박동아 등, 2019). 이러한 변화 및 요구를 반영하기 위해 건강보험심사평가원에서는 「경피적 대동맥판삽입(TAVR)」의 수술 중·저위험군 환자에서의 안전성·유효성 등에 대한 평가를 한국보건의료연구원으로 의뢰하였다(예비급여평가부-262 2020.04.08.). 따라서 본 평가는 중증 대동맥판협착증 환자 중 수술 중위험군에서의 경피적 대동맥판삽입에 대한 임상적 안전성 및 유효성에 대한 의과학적 근거를 평가하였다.

위원회 운영 총 7인으로 구성된 소위원회는 2020년 6월 26일부터 9월 18일까지 4개월간 총 3차례 소위원회 운영을 통해 문헌적 근거에 따라 동 기술의 안전성 및 유효성을 평가하였다. 2020년 제10차 의료기술재평가위원회(2020.10.16.)에서 소위원회의 안전성 및 유효성 평가 결과에 대해 최종 심의하였다.

평가방법 경피적 대동맥판삽입의 수술 중위험군에서 안전성 및 유효성에 대해 체계적 문헌고찰을 수행하고, 비용 등에 대해 소위원회 의견을 정리하였다. 모든 평가방법은 “경피적 대동맥판삽입 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 체계적 문헌고찰의 핵심질문은 “증상이 있는 중증 대동맥판협착증 환자 중 수술 중위험(intermediate risk) 대상에서의 경피적 대동맥판삽입은 수술적 대동맥판 치환술에 비해 안전하고, 효과적인가?”이다. 비교시술은 중증 대동맥판협착증의 표준적 치료인 대동맥판막 치환술(Surgical Aortic Valve Replacement, 이하 SAVR)을 선정하였다. 대동맥판막 치환술은 외과적 방법으로 대동맥판막을 인공판막으로 교체하는 수술로 고령 및 동반 질환 등으로 수술 위험이 증가하는 고위험군의 일부에서는 SAVR를 시행하기 어려운 점이 있다. 경피적 대동맥판삽입은 대동맥판막 치환술을 받기 어려운 고령 환자나 수술 위험이 높은 고위험군의 대체 시술로 사용되기 시작하였다. 체계적 문헌고찰의 문헌검색은 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌선택 및 배제기준에 따라 2명의 평가자가 독립적으로 수행하고, 의견의 불일치가 있는 경우에는 평가자간 합의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림 위험 평가는 Cochrane의 Risk of Bias를 사용하여 평가하였으며, 최종 선택된 문헌을 대상으로 2명의 평가자가 독립적으로 평가를 실시하였으며, 의견이 불일치한 경우 평가자간 합의를 통해 일치된 결과를 도출하였다. 모든 자료는 연구단위로 추출하고, 동일 연구의 결과지표가 중복 보고된 경우 최신 문헌을 기준으로 분석에 활용하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 GRADE 방법을 이용하여 근거 수준을 평가하고, 평가 결과를 토대로 권고등급을 결정하였다. 또한, 경제성 결과지표를 보고한 문헌을 정리하고, 국내 TAVR 비용과 관련 보고서(박동아 등, 2019) 내용을 바탕으로 소위원회 의견을 정리하였다.

평가 결과 본 평가에 최종 선택된 문헌은 임상적 안전성ㆍ유효성 결과보고 연구 2개(6편)와 경제성 결과 보고 문헌 7편으로 총 13편이었다. 임상적 안전성ㆍ유효성 결과보고 문헌은 모두 무작위배정 비교임상연구(RCT)로 총 환자 수는 3,692명이었으며, 대상자의 흉부외과학회 수술위험 점수(the Society of Thoracic Surgeons operative risk score, 이하 STS) 평균은 4.4~5.8%이었다. 안전성 결과 임상적 안전성 결과지표는 30일 전체 사망, 뇌졸중 등 신경학적 사건, 심근경색, 심방세동, 심내막염 등의 발생으로 정하였다. 안전성 결과지표에 대한 메타분석 결과, 1개월 시점 심각한 뇌졸중, 심방세동 발생은 경피적 대동맥판삽입이 대동맥판 치환술에 비해 통계적으로 유의하게 더 안전하였다. 하지만, 1년 이상 결과에서는 두 군간 차이가 없었다.

유효성 결과 임상적 유효성 결과지표 메타분석 결과, 1개월 시점의 생명위협 출혈*, 급성 신장손상, 삶의 질, 뉴욕심장협회 분류(New York Heart Association class, 이하 NYHA class) 뉴욕심장협회에서 개발한 심장질환으로 인한 환자의 운동 및 호흡관련 증상을 4단계로 구분한 분류도구 의 개선 등이 경피적 대동맥판 삽입이 대동맥판막 치환술에 비해 더 통계적으로 유의하게 효과적이었고, 반면 대동맥판막 역류, 대동맥판막 관련 재시술, 영구 심박동기 삽입* 등은 대동맥판막 치환술이 경피적 대동맥판삽입에 비해 더 효과적이었다. 1년 이상 장기결과에서도 NYHA class 3등급 이상 발생*은 경피적 대동맥판삽입이 대동맥판막 치환술에 비해 통계적으로 유의하게 효과적이었고, 반면 2년 시점 대동맥판막 역류, 대동맥판막 관련 재시술*, 1년 시점 삶의 질 변화량 등은 SAVR군이 TAVR군보다 통계적으로 유의하게 더 효과적이었다(*이질성이 50% 초과이면서 통계적으로 유의한 결과).

경제성 관련 결과 경제성 결과지표를 보고한 문헌은 총 7편이었고, 연구국가별로 캐나다 3편, 일본, 프랑스, 호주, 미국이 각각 1편씩이었다. 각 나라의 비용-효과성 판단기준(threshold)을 고려했을 때 경피적 대동맥판삽입이 대동맥판 치환술에 비해 비용도 저렴하고, 효과도 높은 우월한(dominant) 대안으로 보고한 연구는 3편이었고, 비용-효과성 판단기준을 고려할 때 비용효과적일 가능성이 있다고 보고한 연구는 3편이었다(불확실성 중등도~높음). 일본에서 수행된 1편의 연구에서는 경피적 대동맥판삽입이 비용효과적이지 않다는 결과를 제시하였다(Kodera, 2018). 비용-효과성에는 효과차이보다는 비용차이가 더 많은 영향을 미쳤으며 비용에 영향을 많이 주는 요인은 경피적 대동맥판삽의 밸브가격이었다. 또한, 국내 경피적 대동맥판삽입 비용과 관련하여 박동아 등(2019) 보고서에 따르면 경피적 대동맥판삽입의 본인부담률이 80%인 2018년 기준 환자당 경피적 대동맥판삽입의 시술비용 40,981,000원 중 환자본인부담은 25,914,000원이며, 한편, 단독 대동맥판막 치환술의 시술비용은 경피적 대동맥판삽입보다 적은 22,874,000원이었으며, 그 중 환자본인부담은 1,344,000원으로 경피적 대동맥판삽입의 환자본인부담 비율에 비해 낮은 수준이었다.

결론 및 제언 체계적 문헌고찰 결과, 중증 대동맥판협착증의 수술 중위험군에서 경피적 대동맥판삽입의 사망은 대동맥판막 치환술과 비교해 차이가 없었다. 심혈관계 관련 사망, 뇌졸중 등 신경학적 사건 발생, 주요 혈관 합병증, 주요 출혈(생명 위협 또는 장애초래) 등은 전체 기간을 고려했을 때 두 군간 통계적으로 유의한 차이가 없었고, 대동맥판막 관련 역류 발생, 대동맥판막 관련 재시술은 경피적 대동맥판삽입군에서 대동맥판막 치환술군에 비해 통계적으로 유의하게 발생률이 높았다. 결론적으로 중증 대동맥판협착증의 수술 중위험군에서 경피적 대동맥판삽입은 대동맥판막 치환술과 비교할 때, 안전성 및 유효성의 차이가 없어 안전하고 유효한 기술로 평가하였다(GRADE 신뢰도 Moderate-High). 경피적 대동맥판삽입 재평가 소위원회에서는 수술위험도에 따른 급여기준에 대한 논의 결과, 대동맥판막 치환술이 불가능하거나 수술 고위험군은 대동맥판막 치환술과 동일한 급여기준 적용(급여), 수술 중위험군은 본인부담률 50%, 수술 저위험군은 현행 급여기준 유지가 적절할 것이라는 의견이 있었고, 국내 경피적 대동맥판막삽입 비용과 대동맥판막 치환술 비용의 차이는 급여기준 및 치료재료비용의 차이로 발생한 것으로 판단되며, 경피적 대동맥판막삽입 및 대동맥판막 치환술 시술수가의 상향 및 수술위험 기준에 따른 급여확대가 필요하다는 것에 의견일치를 이뤘다. 의료기술재평가위원회는 의료기술재평가사업 관리지침 제4조 제10항에 의거 “중증 대동맥판협착증 수술 중위험 환자에서 경피적 대동맥판삽입(TAVR)을 권고함(권고등급 Ⅰ-b)”으로 심의하였다 (2020. 10. 16.). 경피적 대동맥판삽입(TAVR)에 대한 체계적 문헌고찰 결과, 대동맥판막 치환술과 차이가 없는 기술이면서 고가라는 점 등을 고려할 때, 현재 시점의 근거로는 강하게 권고하기 어려워, 권고강도는 ‘낮음(Ⅰ-b 등급)’으로 결정하였다.

Background and Purpose of Assessment TAVR (Transcatheter Aortic Valve Replacement, hereinafter TAVR) is a procedure to percutaneously insert a stent-type artificial valve for the treatment of severe aortic valve stenosis. It was introduced as a selective reimbursement with conditions (80% co-payment rate) from June 2015 as a condition of submitting the limitation of healthcare facilities and procedure data. The US FDA approved even the low-risk group for surgery (2019), and previous studies in Korea have also reported that a re-assessment is necessary after accumulating evidence in the future for the intermediate-low risk group (Dong-Ah Park et al., 2019). To reflect these changes and needs, the Health Insurance Review & Assessment Service commissioned the Korea Institute of Health and Medical Research to evaluate the safety and effectiveness of 「TAVR」 in patients in the intermediate-low risk group (Preliminary Benefit Evaluation Department-262 Apr. 08. 2020). Therefore, this assessment evaluated the medical and scientific evidence for the clinical safety and effectiveness of TAVR in the intermediate-risk group among patients with severe aortic valve stenosis.

Committee’s Operation The subcommittee consisted of a total of 7 members and held a total of three subcommittee meetings to evaluate the safety and effectiveness of the technology based on the literature for 4 months from June 26 to September 18, 2020. The final deliberation was conducted on the subcommittee's assessment results of safety and effectiveness at the 10th Health Technology Reassessment Committee (2020.10.16.). Assessment Method A systematic literature review was conducted on the safety and effectiveness of TAVR in the intermediate-risk group, and the opinions of the subcommittee were summarized on costs and others. All assessment methods were finalized after discussion of the “TAVR subcommittee (hereinafter referred to as the ‘subcommittee’). A key question in the systematic review is “Is TAVR safer and more effective than surgical aortic valve replacement in intermediate-risk patients with symptomatic severe aortic valve stenosis?” For the comparative procedure, the surgical aortic valve replacement (hereinafter referred to as SAVR), a standard treatment for severe aortic valve stenosis, was selected. SAVR is a surgical procedure to replace the aortic valve with an artificial valve, and it is difficult to perform SAVR in some high-risk groups, where the risk of surgery increases due to old age and comorbidities. TAVR has begun to be used as an alternative procedure for elderly patients who are difficult to get SAVR or for high-risk surgical groups. The literature search of a systematic literature review was conducted in three overseas and five domestic databases. The literature selection process was independently performed by two evaluators according to the literature inclusion and exclusion criteria. In case of disagreement, the final articles were decided through consensus among the evaluators. The risk of bias in literature was evaluated using Cochrane's Risk of Bias, and two evaluators independently evaluated the finally selected literature. In case of disagreement, concordant results were drawn through consensus among evaluators. All data were extracted by the research unit, and if the outcome indicators of the same study were reported repeatedly, the latest literature was used for analysis. Based on the results of the systematic literature review conducted in this assessment, the level of evidence was evaluated using the GRADE method, and the recommendation grade was determined based on the assessment results. In addition, the literature reporting the economic outcome index was compiled and domestic TAVR costs and related reports (Dong-ah Park et al., 2019) were summarized.

Assessment Results The final selected literature for this assessment was 2 studies (6 articles) reporting clinical safety and effectiveness results and 7 publications reporting economic results, for a total of 13 articles. All studies reporting clinical safety and effectiveness results were randomized clinical trials (RCT), and the total number of patients was 3,692. The average of the Society of Thoracic Surgeons Operative Risk Score (hereinafter STS) of subjects was 4.4-5.8%.

Safety Results Clinical safety outcome indicators were defined as 30-day mortality rate, neurological events such as stroke, myocardial infarction, atrial fibrillation, and endocarditis. As a result of a meta-analysis of safety outcome indicators, TAVR was statistically significantly safer than aortic valve replacement in terms of severe stroke and atrial fibrillation at 1 month. However, there were no differences between the two groups in long-term outcomes of 1 year or longer.

Effectiveness Results As a result of meta-analysis on clinical effectiveness outcome indicators, TAVR was statistically significantly more effective than SAVR in terms of life-threatening bleeding* at 1 month, acute renal failure, aortic valve-related readmission, quality of life*, and improvements in New York Heart Association class (hereinafter NYHA class) . On the other hand, SAVR was more effective than TAVR in aortic regurgitation, aortic valve-related reoperation, and permanent pacemaker implantation*. TAVR was statistically significantly more effective than SAVR in terms of occurrence of NYHA class III or higher in the long-term outcome of 1 year or longer. On the other hand, the SAVR group was statistically significantly more effective than the TAVR group in terms of aortic regurgitation at 2 years, aortic valve-related reoperation*, and quality of life change at 1 year (*statistically significant result with >50% heterogeneity).

Economics-Related Results There were a total of 7 documents reporting economic outcome indicators, with 3 from Canada and 1 each from Japan, France, Australia, and the US by study country. Considering the cost-effectiveness threshold of each country, three studies reported that percutaneous aortic valve insertion was a dominant alternative, which is cheaper and more effective, than aortic valve replacement. Considering the cost-effectiveness threshold, Considering the cost-effectiveness criterion, there were three studies that reported the possibility of being cost-effective (moderate to high uncertainty). One study conducted in Japan suggested that TAVR was not cost-effective (Kodera, 2018). Cost-effectiveness was more affected by the cost difference than by the effect difference, and the factor that greatly affected the cost was the valve price of TAVR. In addition, according to a report by Dong-ah Park et al. (2019) regarding the cost of TAVR in Korea, the out-of-pocket cost of TAVR per patient was KRW 25,914,000 out of KRW 40,981,000 per patient as of 2018, when the co-payment rate for TAVR was 80%. On the other hand, the cost of the single SAVR procedure was 22,874,000 won, which was lower than that of TAVR, and the out-of-pocket cost was 1,344,000 won, which was lower than the patient co-payment rate of TAVR.

Conclusion and Suggestions As a result of a systematic review of the literature, there was no difference in mortality from TAVR compared to SAVR in the intermediate-risk group for severe aortic valve stenosis. There was no statistically significant difference between the two groups in terms of cardiovascular-related mortality, neurological events such as stroke, major vascular complications, major bleeding (life-threatening or disability). The incidence of aortic valve-related regurgitation and aortic valve-related reoperation was statistically significantly higher in the TAVR group than in the SAVR group. In conclusion, in the intermediate-risk group of severe aortic valve stenosis, TAVR was evaluated as a safe and effective technique because there was no difference in safety and effectiveness compared to SAVR (GRADE reliability Moderate-High). As a result of the discussion on the reimbursement coverages according to the risk of surgery in the TAVR reassessment subcommittee, there was an opinion that it would be appropriate to apply for the same reimbursement coverages as SAVR in the group where SAVR is impossible or high-risk, to apply 50% co-payment for the intermediate-risk group, and to maintain the current reimbursement coverages for the low-risk group. It is judged that the difference between TAVI cost and SAVR cost in Korea is caused by the difference in reimbursement coverages and treatment material cost. There was a consensus on the need for an increase in the cost of TAVI and SAVR procedures and the need for an expanded reimbursement coverage according to the surgical risk criteria. The Health Technology Reassessment Committee deliberated as “recommended TAVR for intermediate-risk patients with severe aortic valve stenosis surgery (recommendation grade I-b)” according to Article 4, Paragraph 10 of the Health Technology Reassessment Project Management Guideline (2020. 10. 16). As a result of a systematic literature review on TAVR, the recommendation grade was determined to be 'low (Grade I-b)' because it is difficult to make a strong recommendation based on the current evidence, considering that it is a technology that is not different from SAVR and expensive.
