체외충격파치료[근골격계질환] 어깨 및 상지

Abstract

평가 배경 체외충격파치료 [근골격계질환]은 상완골 내상과염 및 외상과염, 족저 근막염, 견관절 석회화 건염, 골절 지연 유합 등에 체외에서 충격파를 병변에 가해 혈관 재형성을 돕고, 건 및 그 주위조직과 뼈의 치유 과정을 자극하거나 재활성화시켜, 통증의 감소와 기능의 개선을 위한 치료법이다. 체외충격파치료 [근골격계질환]은 현재 비급여 행위(조-84)로 사용되고 있으며, 의학적 비급여의 급여화 추진과 관련하여 건강보험심사평가원과의 협의를 통해 재평가 항목으로 발굴되었다. 이에 2020년 제5차 의료기술재평가위원회(2020. 5. 11.~13.)에서는 체계적 문헌고찰을 통해 해당 기술의 안전성 및 효과성을 평가하며 이 때 적용 부위를 크게 ‘어깨 및 상지’, ‘고관절 및 요추부’, ‘하지 및 족부’로 나누어 평가하도록 심의하였다. 이후 의료기술재평가 소위원회와 평가위원회에서의 추가적인 논의 의료기술재평가 소위원회 및 평가위원회의 안건 관련 논의사항과 세부적인 경과과정은 [부록 1], [부록 2] 참고 를 통해 체외충격파치료의 대표적 적응 질환으로, 상지 부위에서는 어깨 건병증, 내외측 상과염을, 하지 부위에서는 대전자동통증후군, 족저근막염, 아킬레스건병증, 무릎건병증, 불유합/지연유합, 근막동통증후군을 선정하고 이에 대해 평가하기로 하였다. 체외충격파치료 [근골격계질환]에 대한 재평가 수행 중, 2021년 3월 건강보험심사평가원으로부터 그 외 질환에 대해 재평가를 추가 의뢰받았다(예비급여부-265, 2021. 3. 23.). 2021년 제5차 의료기술재평가 위원회(2021. 5. 14.)에서는 심평원에서 추가 의뢰된 16개 질환(골관절염, 피로골절, 무혈성괴사, 박리성 골연골염, 내전근 건병증, 거위발 건병증, 비골근 건병증, 발․발목 건병증, 골수 부종, 오스굿-슐라터 병, 경골 스트레스 증후군, 근육 염좌, 뒤퓌트랑, 발바닥 섬유종증, 드퀘르벵 병, 방아쇠 수지)에 대해 평가계획서 및 소위원회 구성에 대해 심의하였다. 이에 체외충격파치료 [근골격계질환]에 대해 안전성 및 효과성에 대한 과학적 근거를 제공하고, 의료기술의 적정 사용 등 정책적 의사결정을 지원하고자 체외충격파치료의 재평가를 수행하였다. 본 보고서에서는 체외충격파치료 [근골격계질환]의 적용 질환 중 ‘어깨 및 상지’에 대해 안전성 및 효과성에 대한 과학적 근거를 평가하고, 이를 제시하였다.

평가 방법 체외충격파치료 [근골격계질환] 어깨 및 상지에 대한 안전성 및 효과성 평가는 체계적 문헌고찰을 통해 수행하였다. 모든 평가방법은 평가목적을 고려하여 “체외충격파치료 [근골격계질환] 어깨 및 상지 재평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회는 정형외과 2인, 재활의학과 2인, 마취통증의학과 1인, 영상의학과 1인, 근거기반의학 1인 총 7인의 위원으로 구성하였다. 평가의 핵심질문은 ‘내‧외측 상과염 환자 또는 어깨 건병증 환자를 대상으로 체외충격파로 치료하는 것이 안전하고 효과적인가?’이었고, 안전성은 시술 관련 부작용 및 이상반응 지표로, 효과성은 통증완화, 기능개선, 삶의 질 지표로 평가하였다. 문헌검색은 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림위험 평가는 무작위배정 비교임상시험 연구(Randomized Controlled Trial, RCT)는 Cochrane의 Risk of Bias (RoB)를, 비무작위 비교연구(Non-randomized Study, NRS)는 Risk of Bias Assessment tool for Non-randomized Study (RoBANS)(Ver.2)를 각각 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우에는 제3자와 함께 논의하여 합의하였다. 자료분석은 정량적 분석이 불가능하여 정성적 분석을 적용하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 방법을 이용하여 근거 수준을 평가하고, 평가 결과를 토대로 권고등급을 결정하였다.

평가 결과 체외충격파치료 [근골격계질환] 어깨 및 상지의 안전성과 효과성은 총 10편(내‧외측 상과염 5편, 어깨 건병증 5편)에 근거하여 평가하였다.

내‧외측 상과염 내·외측 상과염 환자에서 체외충격파치료의 안전성은 시술 관련 부작용 및 이상반응 지표로, 총 5편의 문헌을 토대로 평가하였다. 이 중 3편에서는 시술 관련 부작용 및 이상반응을 보고하지 않았으며, 나머지 2편(무작위배정 비교임상시험연구) 중 1편은 스테로이드 주사요법과, 다른 1편은 보존적 요법과 비교하여 제시하였다. 스테로이드 주사요법과 비교한 1편에서 중재군은 일시적 통증(100%), 메스꺼움(21%), 팔꿈치 홍반(21%), 팔꿈치 부종(15.7%), 팔 떨림(5.2%)을, 대조군은 일시적 통증(100%), 평균 5일 이상 지속된 통증(25%), 주사부위 변색(5%)을 보고하였다. 보존적 요법과 비교한 1편에서 중재군은 일시적 통증(100%), 홍조(32.5%)를 보고하였으나, 대조군은 시술 관련 부작용 및 이상반응이 발생하지 않았다고 보고하였다. 내·외측 상과염 환자에서 체외충격파치료의 효과성은 통증완화, 기능개선, 삶의 질을 기준으로 평가하였다. 총 5편의 문헌에서 통증완화는 Visual Analogue Scale (VAS), 기능개선은 악력, 삶의 질은 36-item short-form survey (SF-36) 지표로 설정하여 평가하였다. 스테로이드 주사요법과 체외충격파치료를 비교한 무작위배정 임상시험연구 2편에서 통증완화와 기능개선 결과를 보고하였다. 통증완화의 경우 1편에서 중재군은 대조군과 유사한 수준으로 통증이 감소하였으며, 다른 1편에서는 대조군보다 중재군의 VAS 점수가 더 크게 감소하였으나, 두 편 모두 군간 통계적 유의성을 보고하지 않았다. 기능개선의 경우는 2편 모두 중재군의 악력 개선 정도가 대조군보다 컸으나, 군간 통계적 유의성은 보고하지 않았다. 보존적 요법과 체외충격파치료를 비교한 4편(무작위배정 비교임상시험연구 3편, 비무작위 비교연구 1편)에서 통증완화, 기능개선, 삶의 질 결과를 보고하였다. 통증완화의 경우 무작위배정 비교임상시험연구 3편 중 1편은 대조군보다 중재군에서 VAS 점수가 더 유의하게 개선되었고, 그 이외 2편의 연구에서는 중재군과 대조군의 VAS 개선 정도가 유사하였으나 1편은 군간 유의한 차이가 없었고, 다른 1편은 군간 통계적 유의성을 보고하지 않았다. 비무작위 비교연구 1편에서 중재군은 대조군보다 더 유의하게 VAS 점수가 개선되었다. 기능개선의 경우, 무작위배정 비교임상시험연구 2편 중 1편에서 중재군은 대조군보다 악력이 더 개선되었으나 군간 통계적 유의성을 보고하지 않았다. 다른 1편에서는 중재군과 대조군의 악력 개선 정도가 통계적으로 유의한 차이는 없었다. 삶의 질의 경우 1편(무작위배정 비교임상시험연구)에서 중재군 및 대조군 모두 시술 이후 SF-36 점수가 개선되었으나 군간 유의한 차이는 없었다.

어깨 건병증 어깨 건병증 환자에서 체외충격파치료의 안전성은 시술 관련 부작용 및 이상반응 지표로, 총 5편의 문헌을 토대로 평가하였다. 초음파 유도하 천공 및 세척술과 비교한 4편(무작위배정 비교임상시험연구) 중 3편에서 중재군은 통증 35.7 ~ 41% (2편), 불편감 100% (1편), 오십견 2.4% (1편), 견봉하 윤활낭염 악화 2.4% (1편), 대조군은 통증 9.1 ~ 22% (2편), 오십견 4.9% (1편), 견봉하 윤활낭염 악화 12.2% (1편), 미주신경반응 5% (1편)를 각각 보고하였다. 보존적 요법과 비교한 1편(비무작위 비교연구)에서는 중재군 및 대조군 모두 시술 관련 부작용 및 이상반응이 발생하지 않았다. 어깨 건병증 환자의 체외충격파치료의 효과성 결과변수는 통증완화, 기능개선, 삶의 질이었으며, 총 5편의 문헌을 토대로 통증완화는 VAS와 Numeric Rating Scale (NRS)를, 기능개선은 Constant-Murley Score (CMS), Oxford Shoulder Score (OSS), American Shoulder and Elbow Surgeons evaluation (ASES) 등을 지표로 설정하여 평가하였다. 초음파 유도하 천공 및 세척술과 비교한 4편(무작위배정 비교임상시험연구)에서 통증완화, 기능개선 결과를 보고하였다. 먼저 통증완화를 보고한 4편 중 2편에서는 대조군이 중재군보다 VAS 점수가 더 유의하게 감소하였고, 다른 2편에서는 중재군과 비교군의 VAS 점수 감소량이 유사하였으나 통계적으로 유의하지 않았다. 기능개선의 경우, 3편 모두 중재군보다 비교군에서 기능 개선 정도가 더 컸으나, 이 중 1편만 통계적으로 유의한 차이였고 나머지 2편은 유의하지 않았다. 보존적 요법과 체외충격파치료를 비교한 1편(비무작위 비교연구)에서 통증완화, 기능개선을 보고하였다. 통증완화의 경우 중재군은 대조군보다 VAS 점수가 더 유의하게 감소하였으며, 기능개선의 경우 대조군보다 중재군에서 CMS 점수가 더 유의하게 증가하였다.

결론 및 제언 따라서 소위원회에서는 현재 문헌적 근거를 바탕으로 다음과 같이 결과를 제시하였다. 소위원회는 내·외측 상과염 환자를 대상으로 시행한 체외충격파치료로 심각한 부작용이 발생하지 않아 기존기술과 비교하여 유사한 수준의 안전성을 가진 의료기술로 평가하였다. 효과성은 보존적 요법과 비교하여 체외충격파치료 이후 통증완화 및 기능개선이 일관되게 우위에 있지 않고, 스테로이드 주사요법과 군간 비교 결과를 제시하지 않아 평가에 제한이 있다고 판단하였다. 어깨 석회성 건병증 환자에서 체외충격파치료 시술 이후 비교기술보다 더 심한 통증이 보고되었으나 다른 질병의 발생 등 중대한 부작용이 없었기 때문에 기존기술과 유사한 수준의 안전성을 가진 의료기술로 평가하였다. 초음파 유도하 천공 및 세척술과 체외충격파치료 두 군에서 모두 시술 이후 통증완화 및 기능개선 효과가 있었으나 체외충격파치료 이후 통증(VAS) 감소 정도가 더 적었다. 어깨 비석회성 건병증 환자를 대상으로 체외충격파치료를 시행한 문헌은 한 편도 선택되지 않아 소위원회는 해당 환자에서 체외충격파치료의 안전성 및 효과성을 평가하기 어려웠다. 소위원회의 소수 의견으로, 동 평가에서 보고된 문헌이 없으나, 현재 개원가에서 비석회성 및 석회성 건병증 환자 모두 체외충격파치료를 시행하고 있고 경험적으로 치료 효과가 있으므로, 수술적 치료가 불가능한 비석회성 건병증 환자의 경우에도 석회성 건병증 환자와 동일한 범주에 포함하여 치료할 수 있는 의료기술이라고 제시하였다. 2022년 제2차 의료기술재평가위원회(2022.2.18.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “체외충격파치료 [근골격계질환] 어깨 및 상지”에 대해 치료대상 환자의 적응증별로 다음과 같이 심의하였다. 내·외측 상과염 환자를 대상에서 체외충격파치료는 심각한 부작용이 발생하지 않아 기존기술과 비교하여 유사한 수준으로 안전하지만, 보존적 요법과 비교하여 결과가 일관되게 우위에 있지 않고, 스테로이드 주사요법과 군간 효과성 비교 결과가 확인되지 않아 근거 ‘불충분’으로 심의하였다(권고등급: 불충분). 석회성 어깨 건병증 환자에서 체외충격파치료는 중대한 부작용이 없고 기존기술과 유사한 수준으로 안전한 기술이며, 체외충격파치료와 초음파 유도하 천공 및 세척술 모두 시술 이후 통증완화 및 기능개선 효과를 나타내어 ‘조건부 권고함’으로 심의하였다(권고등급: 조건부 권고함). 비석회성 어깨 건병증 환자를 대상으로 하는 체외충격파치료의 임상적 유용성을 보고한 연구가 확인되지 않아 근거 ‘불충분’으로 심의하였다(권고등급: 불충분).

Background Extracorporeal shock wave therapy (ESWT) [musculoskeletal disorders] is a treatment method that involves applying extracorporeal shock wave to the lesion to promote vascular regeneration and reactivate or stimulate healing in tendons and surrounding tissues and bones to achieve pain relief and functional improvement in cases with humeral medial epicondylitis, humeral lateral epicondylitis, plantar fasciitis, calcific tendinitis of the shoulder, or delayed union of fracture. ESWT [musculoskeletal disorders] is currently used as a non-coverage service (SZ083) and it was identified as an item for reassessment through discussion with the Health Insurance Review and Assessment Service (HIRA) in relation to implementation of coverage for uninsured medical benefits. Accordingly, during the fifth session of the Committee of Health Technology Reassessment in 2020 (May 11–13, 2020), the decision was made to assess the safety and effectiveness of this technology by a systematic review with the applied areas divided largely into “shoulders and upper extremities,” “pelvic and lumbar regions,” and “lower extremities and feet.” Subsequently, based on additional discussions by the health technology reassessment subcommittee and assessment committee, shoulder tendinopathy and medial and lateral epicondylitis in the upper extremities and greater trochanteric pain syndrome, plantar fasciitis, Achilles tendinopathy, knee tendinopathy, nonunion/delayed union, and myofascial pain syndrome in the lower extremities were selected as typical indications for ESWT for inclusion in the assessment. During the reassessment of ESWT [musculoskeletal disorders], additional request for reassessment of other disorders from HIRA was received in March 2021 (Preliminary Coverage Division-265; March 23, 2021). At the fifth session of the Committee of Health Technology Reassessment in 2021 (May 14, 2021), the committee deliberated on the assessment plan and subcommittee composition for the 16 disorders additionally requested by HIRA (osteoarthritis, fatigue fracture, avascular necrosis, osteochondritis dissecans, adductor tendinopathy, pes anserine tendinopathy, peroneal tendinopathy, foot/ankle tendinopathy, bone marrow edema, Osgood-Schlatter disease, medial tibial stress syndrome, muscle sprain and strain, Dupuytren's contracture, plantar fibromatosis, de Quervain's disease, and trigger finger). Accordingly, reassessment of ESWT was conducted to provide scientific evidence for the safety and effectiveness of ESWT [musculoskeletal disorders] and support policy decision-making, including appropriate use of health technology. This report presents the findings on the assessment of scientific evidence for the safety and effectiveness of ESWT [musculoskeletal disorders] for “shoulders and upper extremities” among the disorders for which ESWT [musculoskeletal disorders] is applied.

Methods A systematic review was performed to assess the safety and effectiveness of ESWT [musculoskeletal disorders] for shoulders and upper extremities. All assessment methods were finalized after the review by the “ESWT [musculoskeletal disorders] for shoulders and upper extremities reassessment subcommittee” (hereinafter referred to as “subcommittee”) with consideration for the purpose of the assessment. The subcommittee was composed of seven experts – two each from orthopedic surgery and rehabilitation medicine and one each from anesthesia and pain medicine, radiology, and evidence-based medicine. The key question in the assessment was “Is ESWT safe and effective for treating patients with medial/lateral epicondylitis or shoulder tendinitis?” Safety was assessed based on procedure-related adverse events (AEs) and adverse reactions as the indicators, while effectiveness was assessed based on pain relief and improvement in function and quality of life (QoL) as the indicators. Articles were searched in three international and five domestic databases. Two reviewers independently screened and selected the articles in accordance with the inclusion and exclusion criteria. Two reviewers independently performed risk of bias (RoB) assessment and reached a consensus on the findings – using Cochrane’s Risk of Bias Tool for randomized controlled trials (RCTs) and the Risk of Bias Assessment for Non-randomized Studies (RoBANS) ver. 2.0 for non-randomized studies (NRSs). Data extraction was performed independently by two reviewers, using a predetermined format. Any disagreements were resolved by discussing the matter with a third person to reach a consensus. Data were analyzed by qualitative review since quantitative analysis was not possible. The systematic review results were assessed for level of evidence using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) method, while the grade of recommendation was determined based on the assessment results.

Results The safety and effectiveness of ESWT [musculoskeletal disorders] for shoulders and upper extremities were assessed based on a total of 10 articles (five each on medial/lateral epicondylitis and shoulder tendinitis).

Medial/lateral epicondylitis Safety of ESWT for patients with medial/lateral epicondylitis was assessed based procedure-related AEs and adverse reactions reported in five articles. Of these, three articles reported no procedure-related AEs and adverse reactions, while among the remaining two articles (RCTs), one article reported on comparison with steroid injection and the other reported on comparison with conservative therapy. The article on comparison with steroid injection reported transient pain (100%), nausea (21%), elbow redness (21%), elbow swelling (15.7%), and arm tremor (5.2%) in the intervention group and transient pain (100%), persistent pain lasting an average of 5 days or more (25%), and discoloration around the injection site (5%) in the control group. The article on comparison with conservative therapy reported transient pain (100%) and redness (32.5%) in the intervention group, but no procedure-related AEs and adverse reactions in the control group. Effectiveness of ESWT for patients with medial/lateral epicondylitis was assessed based on pain relief, functional improvement, and QoL. A total of five articles reported on assessment of pain relief using Visual Analogue Scale (VAS), functional improvement using grip strength, and QoL using the 36-item short-form survey (SF-36). Two articles (RCTs) on comparison between ESWT and steroid injection reported on pain and functional outcomes. With respect to pain, one article reported similar level of pain relief in both the intervention and control groups, whereas the other article reported greater decrease in VAS scores in the intervention group than the control group. However, both articles did not report statistically significant differences between the two groups. With respect to functional improvement, both articles reported that grip strength improved more in the intervention group than the control group, but the articles did not report statistically significant differences between the two groups. Four articles (3 RCTs and 1 NRS) on comparison between ESWT and conservative therapy reported on pain, function, and QoL outcomes. With respect to pain, one of three RCTs reported more significant improvement in VAS scores in the intervention group than the control group. In the two remaining RCTs, improvement in VAS scores was similar between the intervention and control groups, with one study reporting no significant difference and the other study reporting no statistical significance between the two groups. In one NRS, the intervention group showed a significant improvement in VAS scores, as compared to the control group. With respect to function, one of two RCTs reported greater improvement in grip strength in the intervention group than the control group, but did not report on statistical significance between the two groups. In the other RCT, there was no statistically significant difference in improvement in grip strength between the intervention and control groups. QoL outcomes were reported in one article (RCT), reporting improvement in SF-36 scores after the procedure in both the intervention and control groups, but there was no significant difference between the two groups.

Shoulder tendinitis Safety of ESWT for patients with shoulder tendinitis was assessed based procedure-related AEs and adverse reactions reported in five articles. Among four articles (RCTs) on comparison between ESWT and ultrasound-guided needling and lavage, three articles reported pain (35.7%–41%; two articles), discomfort (100%; one article), frozen shoulder (2.4%; one article), and exacerbation of subacromial bursitis (2.4; one article) in the intervention group and pain (9.1%–22%; two articles), frozen shoulder (4.9%; one article), exacerbation of subacromial bursitis (12.2%; one article), and vagus nerve response (5%; one article). One article (NRS) on comparison with conservative therapy reported no procedure-related AEs and adverse reactions in the intervention and control groups. Effectiveness of ESWT for patients with shoulder tendinitis was assessed based on pain relief, functional improvement, and QoL. A total of five articles reported on assessment of pain relief using VAS and Numeric Rating Scale (NRS) and functional improvement using Constant-Murley Score (CMS), Oxford Shoulder Score (OSS), and American Shoulder and Elbow Surgeons evaluation (ASES). Four articles (RCTs) on comparison between ESWT and ultrasound-guided needling and lavage reported on pain and functional outcomes. In two out of four articles that reported on pain relief, the control group showed more significant decrease in VAS scores than the intervention group, while the other two articles reported similar decrease in VAS scores between the intervention and control groups, but the differences were not statistically significant. With respect to functional improvement, all three articles reported that the level of functional improvement was higher in the control group than the intervention group, and of these, only one article reported statistically significant difference, while two articles reported no significant difference. One article (RCT) on comparison between ESWT and conservative therapy reported on pain relief and functional improvement. With respect to pain relief, the intervention group showed more significant decrease in VAS scores than the control group. With respect to functional improvement, the intervention group showed more significant increase in CMS scores than the control group.

Conclusions and recommendations Based on the evidence currently available through existing literature, the subcommittee presented the following findings: The subcommittee determined that ESWT applied to patients with medial/lateral epicondylitis caused no serious AEs and has safety at a level similar to that of existing technologies. The subcommittee also determined that there are limitations in assessing the effectiveness of ESWT applied to patients with medial/lateral epicondylitis since its effectiveness was not consistently superior in relieving pain and improving function, as compared to conservative therapy, and articles reviewed did not present results on comparison between groups when compared to steroid injection. In patients with calcific tendonitis of the shoulder, patients reported more severe pain after ESWT, as compared to comparator technologies, but since there were no incidence of serious AEs, such as onset of disorders, the subcommittee determined that ESWT applied to patients with calcific tendonitis of the shoulder is a technology with safety at a level similar to that of existing technologies. In the comparison between ESWT and ultrasound-guided needling and lavage, both groups showed pain relief and functional improvement after the intervention, but decrease in VAS scores was less after ESWT. Since not a single article on ESWT applied to patients with non-calcific tendonitis of the shoulder was selected, the subcommittee determined that it would be difficult to assess the safety and effectiveness of ESWT for such patients. Although there were no articles included in this assessment, a minority opinion of the subcommittee suggested that patients with inoperable non-calcific tendonitis should be included in the same category as patients with calcific tendonitis since hospitals are currently using ESWT on both calcific and non-calcific tendonitis patients and there is empirical therapeutic effect. During the second session of the Committee of Health Technology Reassessment in 2022 (February 18, 2022), the following conclusions were made on “ESWT [musculoskeletal disorders] for shoulders and upper extremities” based on the subcommittee findings in accordance with Article 4-10 of the Guidelines for Management of Medical Technology Reassessment Project. ESWT applied to patients with medial/lateral epicondylitis caused no serious AEs and has safety at a level similar to that of existing technologies. However, its effectiveness was not consistently superior as compared to conservative therapy and comparison of effectiveness between groups when compared to steroid injection could not be confirmed. Accordingly, the level of evidence was determined to be “insufficient” (Grade of recommendation: Insufficient). ESWT applied to patients with calcific tendinitis of the shoulder caused no serious AEs and has safety at a level similar to that of existing technologies. Moreover, pain relief and functional improvement were found after both ESWT and ultrasound-guided needling and lavage. Accordingly, ESWT for patients with calcific tendinitis of the shoulder is “conditionally recommended” (Grade of recommendation: Conditionally recommended). Level of evidence for ESWT for patients with non-calcific tendonitis of the shoulder is determined to be “insufficient” since there were no studies that reported on the clinical usefulness of ESWT for this disorder (Grade of recommendation: Insufficient).
