체외충격파치료[근골격계질환] 하지 및 족부

Abstract

평가 배경 체외충격파치료 [근골격계질환]은 상완골 내상과염 및 외상과염, 족저 근막염, 견괄절 석회화 건염, 골절 지연 유합 등에 체외에서 충격파를 병변에 가해 혈관 재형성을 돕고, 건 및 그 주위조직과 뼈의 치유 과정을 자극하거나 재활성화 시켜, 통증의 감소와 기능의 개선을 위한 치료법이다. 체외충격파치료 [근골격계질환]은 현재 비급여 행위(조-84)로 사용되고 있으며, 의학적 비급여의 급여화 추진과 관련하여 건강보험심사평가원과의 협의를 통해 재평가 항목으로 발굴되었다. 이에 2020년 제5차 의료기술재평가위원회(2020.05.11-13.)에서는 체계적 문헌고찰을 통해 해당 기술의 안전성 및 효과성을 평가하며 이 때 적용 부위를 크게 ‘어깨 및 상지’, ‘고관절 및 요추부’, ‘하지 및 족부’로 나누어 평가하도록 심의하였다. 이후 소위원회와 의료기술재평가위원회에서의 추가적인 논의를 통해 체외충격파치료의 대표적 적응 질환으로, 상지 부위에서는 어깨 건병증, 내외측 상과염을, 하지 부위에서는 대전자동통증후군, 족저근막염, 아킬레스건병증, 무릎건병증, 불유합/지연유합, 근막동통증후군을 선정하고 이에 대해 평가하기로 하였다. 체외충격파치료 [근골격계질환]에 대한 재평가 수행 중, 2021년 3월 건강보험심사평가원으로부터 그 외 질환에 대해 재평가를 추가 의뢰받았다(예비급여부-265, ‘21.03.23.). 2021년 제5차 의료기술재평가 위원회(‘21.05.14.)에서는 심평원에서 추가 의뢰된 16개 질환(골관절염, 피로골절, 무혈성괴사, 박리성 골연골염, 내전근 건병증, 거위발 건병증, 비골근 건병증, 발․발목 건병증, 골수 부종, 오스굿-슐라터 병, 경골 스트레스 증후군, 근육 염좌, 뒤퓌트랑, 발바닥 섬유종증, 드퀘르벵 병, 방아쇠 수지)에 대해 평가계획서 및 소위원회 구성에 대해 심의하였다. 이에 체외충격파치료 [근골격계질환]에 대해 안전성 및 효과성에 대한 과학적 근거를 제공하고, 의료기술의 적정 사용 등 정책적 의사결정을 지원하고자 체외충격파치료(extracorporeal shock wave therapy, ESWT)의 재평가를 수행하였다.

평가 방법 ‘체외충격파치료 [근골격계질환] 하지 및 족부’의 안전성 및 효과성을 재평가하기 위하여 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “체외충격파치료 [근골격계질환] 하지 및 족부 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌 선택 및 배제기준에 따라 두 명의 검토자가 독립적으로 수행하고, 의견의 불일치가 있는 경우, 소위원회 논의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림 위험 평가는 Cochrane의 Risk of Bias 및 비무작위 연구 문헌의 비뚤림 위험 평가 도구인 Risk of Bias Assessment tool for Non-randomized Studies (RoBANS)를 사용하여 두 명의 검토자가 독립적으로 수행하여 의견 합의를 이루었다. 자료추출은 미리 정해 놓은 자료추출 서식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 자료분석은 정량적 분석과 정성적 분석을 적용하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 방법을 이용하여 근거수준을 평가하고, 평가결과를 토대로 권고등급을 결정하였다.

평가 결과 소위원회 논의를 바탕으로 ‘체외충격파치료 [근골격계질환] 하지 및 족부’의 재평가 대상질환으로 족저근막염, 아킬레스건병증, 무릎건병증을 선정하였다. 3개 질환을 대상으로 체외충격파치료를 적용하여 평가한 선택된 문헌은 71편(국외 70편, 국내 1편)이었다. 선택된 문헌을 질환별로 분류하면, 족저근막염 54편(국외 53편, 국내 1편), 아킬레스건병증 11편(국외 11편, 국내 0편), 무릎건병증 7편(국외 7편, 국내 0편)이었다.

1) 족저근막염 족저근막염 환자 대상 체외충격파치료의 평가에 포함된 문헌은 총 54편(무작위 배정 임상시험 연구[randomized controlled trial, RCT] 48편, 비무작위 연구[non-randomized study, NRS] 6편)으로, 대상자 수는 중재군 2,448명, 비교군 2,381명이었다. 비교중재의 유형에 따라 (1) 스테로이드 주사와 비교한 연구가 14편, (2) 보존적 요법과 비교한 연구가 19편, (3) sham ESWT와 비교한 연구가 21편이었다. 평가에 포함된 대상자는 대부분 증상 지속기간이 6개월 이상 지속된 만성 족저근막염 환자를 대상으로 하였다.

안전성 족저근막염 환자 대상 체외충격파치료의 안전성은 시술 관련 부작용 및 합병증으로 평가하였다. 시술 관련 부작용 및 합병증을 보고한 문헌은 총 28편으로, 이 중 16편(57.1%)의 문헌에서 체외충격파치료 시행 후 관련 부작용은 발생하지 않은 것으로 보고하였다. 나머지 12편의 문헌에서 보고된 체외충격파치료 관련 부작용은 대부분 시술 중 또는 시술 후 3-5일 내에 발생한 통증이었으며, 그 외 경증의 신경학적 증상(무감각 등)이 확인되었다. 보고된 부작용은 치료 후 1주일 이내 해소되었으며, 대부분 경증으로 ESWT 치료로 인해 예상 가능한 부작용으로 확인되었다.

효과성 족저근막염 환자 대상 체외충격파치료의 효과성은 통증, 기능, 삶의 질을 기준으로 평가하였으며, 효과성 결과는 비교중재의 유형에 따라 제시하였다. 첫째, 체외충격파치료와 스테로이드 주사의 효과를 비교한 연구는 총 14편이었다. 통증 결과를 보고한 연구는 총 13편으로, 메타분석 결과(RCT 10편 포함) ESWT군과 스테로이드 주사군 간 통계적으로 유의한 차이는 확인되지 않았다(MD –0.60; 95% CI –1.29, 0.09; I2=91%). 기능 결과를 보고한 연구는 총 8편으로, 메타분석 결과(RCT 7편 포함) 스테로이드 주사군에 비해 ESWT 치료군에서 기능 지표가 통계적으로 유의하게 개선된 것으로 확인되었다(SMD 0.45; 95% CI 0.27, 0.63; I2=0%). ESWT와 스테로이드 주사를 비교한 연구에서 삶의 질을 보고한 연구는 확인되지 않았다. 둘째, 체외충격파치료와 보존적 요법을 비교한 연구는 총 19편이었다. 통증 결과를 보고한 연구는 17편(비교 20개)으로, 메타분석 결과(RCT 15편, 비교 15개 포함) ESWT 치료군과 보존적 요법군 간 차이는 통계적으로 유의하지 않았다(SMD 0.07; 95% CI –0.59, 0.74; I2=95%). 기능 결과를 보고한 연구는 총 13편(비교 15개)으로, 메타분석 결과(RCT 11편, 비교 12개 포함) 보존적 요법군에 비해 ESWT 치료군에서 통계적으로 유의하게 기능이 개선되는 것으로 확인되었다(SMD –0.99; 95% CI –1.91, -0.07; I2=96%). 삶의 질은 총 4편의 연구에서 보고하였으며, 이 중 3편의 연구에서 보존적 요법군에 비해 ESWT 치료군에서 삶의 질이 통계적으로 유의하게 개선되었다. 셋째, 체외충격파치료와 sham ESWT를 비교한 연구는 총 21편이었다. 통증 결과는 총 21편의 연구에서 보고되었으며, 메타분석 결과(RCT 11편 포함) sham ESWT 치료군에 비해 ESWT 치료군에서 통증 점수가 통계적으로 유의하게 낮았다(MD –1.71; 95% CI –2.44, -0.98; I2=70%). 기능 결과를 보고한 연구는 총 12편으로 메타분석 결과(RCT 7편 포함) sham ESWT 치료군에 비해 ESWT 치료군에서의 기능 정도가 통계적으로 유의하게 개선된 것으로 확인되었다(SMD 0.84; 95% CI 0.23, 1.45; I2=91%). ESWT 치료군과 sham ESWT군을 비교한 연구 중 3편에서 삶의 질을 보고하였으며, 1편의 연구에서 sham ESWT군에 비해 중재군에서 신체건강 영역 및 정신건강 영역 점수 모두 통계적으로 유의하게 향상된 것으로 보고하였으며, 다른 2편에서는 특정 시점에서만 일부 하위영역 점수에서 유의한 차이를 보고하였다.

2) 아킬레스건병증 아킬레스건병증 환자 대상 체외충격파치료의 평가에 포함된 문헌은 총 11편(RCT 8편, NRS 3편)으로, 대상자 수는 중재군 332명, 비교군 281명이었다. 비교중재의 유형에 따라 (1) 보존적 요법과 비교한 연구가 7편, (2) sham ESWT와 비교한 연구가 4편이었다. 평가에 포함된 대상자는 대부분 증상 지속기간이 6개월 이상 지속된 환자를 대상으로 하였다.

안전성 아킬레스건병증 환자 대상 체외충격파치료의 안전성은 시술 관련 부작용 및 합병증으로 평가하였다. 시술 관련 부작용 및 합병증을 보고한 문헌은 총 8편으로, 중대한 합병증이 발생하지 않았으며, 3편의 문헌에서 보고된 경미한 합병증 발생률은 8.8-12.5% 수준이었다. 보고된 시술 관련 부작용은 대부분 시술 중/후 통증, 발적 및 일시적 무감각이었으며, 이러한 부작용은 추가 중재 없이 해소된 것으로 확인되었다.

효과성 아킬레스건병증 환자 대상 체외충격파치료의 효과성은 통증, 기능, 삶의 질을 기준으로 평가하였으며, 효과성 결과는 비교중재의 유형에 따라 제시하였다. 첫째, 체외충격파치료와 보존적 요법을 비교한 연구는 총 7편이었다. 7편 모두에서 통증 결과를 보고하였으며, 메타분석 결과(RCT 4편 포함) 보존적 요법군에 비해 ESWT 치료군에서 통증 정도가 통계적으로 유의하게 개선되었다(SMD –0.56; 95% CI –1.04, -0.09; I2=68%). 기능 결과를 보고한 연구는 총 7편으로, 메타분석 결과(RCT 4편 포함) ESWT 치료군과 보존적 요법군 간 차이가 통계적으로 유의하지 않았다(SMD –0.36; 95% CI –0.44, 1.17; I2=89%). 삶의 질을 보고한 1편의 연구에서 보존적 요법군에 비해 ESWT 치료군에서의 삶의 질이 통계적으로 유의하게 개선되었다. 둘째, 체외충격파치료와 sham ESWT를 비교한 연구는 총 4편이었다. 통증 결과는 총 3편의 연구에서 보고되었으며, 메타분석 결과(RCT 3편 포함) ESWT 치료군이 sham ESWT에 비해 통증 개선 정도가 더 컸지만, 그 차이가 통계적으로 유의하지 않았다(MD –0.84; 95% CI –2.13, 0.45; I2=42%). 기능 결과를 보고한 연구는 총 4편으로, 메타분석 결과(RCT 4편 포함) ESWT 치료군이 sham ESWT에 비해 기능 향상 정도가 더 컸으나, 그 차이는 통계적으로 유의하지 않았다(SMD 0.72; 95% CI –0.09, 1.53; I2=87%). 삶의 질을 보고한 1편의 연구에서 추적관찰 1년 시점 기준 ESWT 치료군과 sham ESWT군 간 삶의 질에서 유의한 차이가 확인되지 않았다.

3) 무릎건병증 무릎건병증 환자 대상 체외충격파치료의 평가에 포함된 문헌은 총 7편(RCT 6편, NRS 1편)으로, 대상자 수는 중재군 172명, 비교군 173명이었다. 비교중재의 유형에 따라 (1) 보존적 요법과 비교한 연구가 3편, (2) sham ESWT와 비교한 연구가 4편이었다. 평가에 포함된 대상자는 증상 지속기간이 대부분 3개월 이상 지속된 환자를 대상으로 하였다.

안전성 무릎건병증 환자 대상 체외충격파치료의 안전성은 시술 관련 부작용 및 합병증으로 평가하였다. 시술 관련 부작용 및 합병증을 보고한 문헌은 총 4편으로, 2편의 문헌에서 어떠한 시술 관련 부작용 및 합병증도 발생하지 않은 것으로 보고하였으며, 1편의 연구에서 경미한 합병증이 6.1%, 다른 1편의 연구에서 1명(3.7%)의 환자에서 일시적 감각이상을 호소하였으나, 냉찜질 후 해소된 것으로 확인되었다.

효과성 무릎건병증 환자 대상 체외충격파치료의 효과성은 통증, 기능, 삶의 질을 기준으로 평가하였으며, 효과성 결과는 비교중재의 유형에 따라 제시하였다. 첫째, 체외충격파치료와 보존적 요법을 비교한 연구는 총 3편이었다. 3편에서 모두 통증 결과를 보고하였으며, 메타분석 결과(RCT 2편 포함) 보존적 요법군에 비해 ESWT 치료군에서 통증 정도가 통계적으로 유의하게 낮았다(MD –3.89; 95% CI –4.32, -3.46, I2=0%). 기능 결과를 보고한 2편 모두에서 보존적 요법군에 비해 ESWT 치료군에서 기능 정도가 통계적으로 유의하게 더 좋은 것으로 보고하였다. ESWT와 보존적 요법을 비교한 연구에서 삶의 질을 보고한 연구는 확인되지 않았다. 둘째, 체외충격파치료와 sham ESWT를 비교한 연구는 총 4편이었다. 4편에서 모두 통증 결과를 보고하였으며, 메타분석 결과(RCT 4편 포함) ESWT 치료군과 sham ESWT군 간 차이는 통계적으로 유의하지 않았다(MD –0.05; 95% CI –0.71, 0.61; I2=0%). 기능 결과는 4편에서 보고하였으며, 메타분석 결과(RCT 3편 포함) ESWT 치료군과 sham ESWT군 간 차이는 통계적으로 유의하지 않았다(MD –4.62; 95% CI –10.03, 0.79; I2=0%). ESWT와 sham ESWT군을 비교한 연구에서 삶의 질을 보고한 연구는 확인되지 않았다.

결론 및 제언 소위원회에서는 현재 문헌적 근거를 바탕으로 다음과 같이 결과를 제시하였다.

1) 족저근막염 족저근막염 환자에서 체외충격파치료로 인한 심각한 부작용이 발생하지 않았으며, 보고된 부작용은 대부분 경증으로 체외충격파치료로 인해 예상 가능한 부작용으로 확인되었다. 이에 소위원회에서는 체외충격파치료를 임상 적용이 가능한 안전한 기술로 판단하였다. 체외충격파치료는 sham ESWT에 비해 통증 및 기능 지표에서 유의하게 개선되었으며, 스테로이드 주사와 비교했을 때에는 기능 지표에서만 유의한 개선을 확인하였다. 보존적 요법과 비교 시, 통증 및 기능 결과에서 유의한 차이는 확인되지 않았으나, 체외충격파치료군에서의 기능 향상 정도가 더 큰 것으로 나타났다. 이에 소위원회에서는 체외충격파치료는 스테로이드 주사 혹은 보존적 요법과 같은 통상적 치료와 치료 효과가 유사한 것으로 판단하였다. 또한, 평가에 포함된 대상자가 대부분 증상 지속기간이 6개월 이상 지속된 만성 족저근막염 환자였던 점을 고려하여 체외충격파치료를 만성 족저근막염 환자에서 장기적인 치료 효과를 기대할 수 있는 의미 있는 치료법으로 판단하였다.

2) 아킬레스건병증 아킬레스건병증 환자에서 체외충격파치료로 인한 심각한 부작용이 발생하지 않았으며, 보고된 부작용은 대부분 경증으로 체외충격파치료로 인해 예상 가능한 부작용으로 확인되었다. 이에 소위원회에서는 체외충격파치료를 임상 적용이 가능한 안전한 기술로 판단하였다. 체외충격파치료의 효과성은 보존적 요법에 비해 통증, 기능, 삶의 질에 있어 유의한 개선 효과가 있었으나, sham ESWT와 비교 시 유의한 차이가 확인되지 않았다. 이에 소위원회에서는 아킬레스건병증 환자에서 sham ESWT 대비 체외충격파치료의 유의한 개선 효과가 확인되지 않았으며, 평가에 포함된 연구가 대부분 소규모이고, 현재 문헌의 근거수준이 낮아 이를 바탕으로 아킬레스건병증 환자에서 체외충격파치료의 효과를 평가하기 어렵다는 의견이었다.

3) 무릎건병증 무릎건병증 환자에서 체외충격파치료로 인한 심각한 부작용이 발생하지 않았으며, 보고된 부작용은 대부분 경증으로 체외충격파치료로 인해 예상 가능한 부작용으로 확인되었다. 이에 소위원회에서는 체외충격파치료를 임상 적용이 가능한 안전한 기술로 판단하였다. 체외충격파치료는 보존적 요법에 비해 통증, 기능, 삶의 질에 있어 유의한 치료 효과가 확인되나, sham ESWT와 비교 시 유의한 차이가 확인되지 않았다. 이에 소위원회에서는 무릎건병증 환자에서 sham ESWT 대비 체외충격파치료의 유의한 개선 효과가 확인되지 않았으며, 평가에 포함된 연구가 대부분 소규모이고, 현재 문헌의 근거수준이 낮아 이를 바탕으로 무릎건병증 환자에서 체외충격파치료의 효과를 평가하기 어렵다는 의견이었다. 체외충격파치료 [근골격계질환]의 질환별 결론 검토 및 통합적 결론 도출을 위해 통합 소위원회를 진행하였으며, 통합 소위원회에서는 체외충격파치료의 효과를 평가하는데 있어 다른 비교중재 보다 보존적 요법 대비 체외충격파치료의 효과를 평가하는 것이 더 의미 있다는 의견으로, 보존적 요법 대비 유의한 효과가 확인된 족저근막염, 아킬레스건병증, 무릎건병증에 대해서는 체외충격파치료의 효과를 인정할 수 있다는 의견을 제시하였다. 2022년 제2차 의료기술재평가위원회(2022.02.18.)에서는 소위원회 및 통합 소위원회 검토결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “체외충격파치료 [근골격계질환] 하지 및 족부”에 대해 다음과 같이 심의하였다. 족저근막염 환자에서 체외충격파치료는 안전하며, 비교중재(스테로이드 주사, 보존적 요법, sham ESWT)에 비해 통증 감소, 기능 개선 및 삶의 질 향상에서 부분적으로 치료 효과가 확인되었다. 또한, 평가에 포함된 대상자가 대부분 만성 족저근막염 환자를 대상으로 한 점을 고려하여, 만성 족저근막염 환자에서 체외충격파치료는 안전하고, 효과적인 기술로 판단하여 ‘조건부 권고함’으로 심의하였다. 아킬레스건병증 환자에서 체외충격파치료는 안전한 기술이며, 보존적 요법에 비해 통증 감소, 기능 개선 및 삶의 질 개선 효과를 나타낸 현재까지의 문헌적 근거를 고려하여 ‘조건부 권고함’으로 심의하였다. 무릎건병증 환자에서 체외충격파치료는 안전한 기술이며, 보존적 요법에 비해 통증 감소, 기능 개선 및 삶의 질 개선 효과를 나타낸 현재까지의 문헌적 근거를 고려하여 ‘조건부 권고함’으로 심의하였다.

Background Extracorporeal shock wave therapy (ESWT) [musculoskeletal disorders] is a treatment method that involves applying extracorporeal shock wave to the lesion to promote vascular regeneration and reactivate or stimulate healing in tendons and surrounding tissues and bones to achieve pain reduction and functional improvement in cases with humeral medial epicondylitis, humeral lateral epicondylitis, plantar fasciitis, calcific tendinitis of the shoulder, or delayed union of fracture. ESWT [musculoskeletal disorders] is currently used as a non-coverage service (SZ083) and it was identified as an item for reassessment through discussion with the Health Insurance Review and Assessment Service (HIRA) in relation to implementation of coverage for uninsured medical benefits. Accordingly, during the fifth session of the Committee of Health Technology Reassessment in 2020 (May 11–13, 2020), the decision was made to assess the safety and effectiveness of this technology by a systematic review with the applied areas divided largely into “shoulders and upper extremities,” “pelvic and lumbar regions,” and “lower extremities and feet.” Subsequently, based on additional discussions by the Committee of Health Technology Reassessment and subcommittee, shoulder tendinopathy and medial and lateral epicondylitis in the upper extremities and greater trochanteric pain syndrome, plantar fasciitis, Achilles tendinopathy, knee tendinopathy, nonunion/delayed union, and myofascial pain syndrome in the lower extremities were selected as typical indications for ESWT for inclusion in the assessment. While the reassessment of ESWT [musculoskeletal disorders] was being conducted, additional request for reassessment of other disorders from HIRA was received in March 2021 (Preliminary Coverage Division-265; March 23, 2021). At the fifth session of the Committee of Health Technology Reassessment in 2021 (May 14, 2021), the committee deliberated on the assessment plan and subcommittee composition for the 16 disorders additional requested by HIRA (osteoarthritis, fatigue fracture, avascular necrosis, osteochondritis dissecans, adductor tendinopathy, pes anserine tendinopathy, peroneal tendinopathy, foot/ankle tendinopathy, bone marrow edema, Osgood-Schlatter disease, medial tibial stress syndrome, muscle sprain and strain, Dupuytren's contracture, plantar fibromatosis, de Quervain's disease, and trigger finger). Accordingly, reassessment of ESWT was conducted to provide scientific evidence for the safety and effectiveness of ESWT [musculoskeletal disorders] and support policy decision-making, including appropriate use of health technology.

Methods A systematic review was performed to assess the safety and effectiveness of ESWT [musculoskeletal disorders] for lower extremities and feet. All assessment methods were finalized after the review by the “ESWT [musculoskeletal disorders] for lower extremities and feet subcommittee” (hereinafter referred to as “subcommittee”) with consideration for the purpose of the assessment. Articles for the systematic review were searched in three international and five domestic databases based on the key question. Two reviewers independently screened and selected the articles in accordance with the inclusion and exclusion criteria. In the event of disagreement, the final articles were selected through a discussion with the subcommittee. Two reviewers independently performed risk of bias (RoB) assessment and reached a consensus on the findings – using Cochrane’s Risk of Bias Tool and the Risk of Bias Assessment for Non-randomized Studies (RoBANS). Data extraction was performed independently by two reviewers, using a predetermined format. Any disagreements were resolved by discussing the matter with a third person to reach a consensus. Data were analyzed by quantitative analysis and qualitative review. The systematic review results were assessed for level of evidence using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) method, while the grade of recommendation was determined based on the assessment results.

Results Based on discussions with the subcommittee, plantar fasciitis, Achilles tendinopathy, and patellar tendinopathy were selected as target disorders for reassessment of “ESWT [musculoskeletal disorders] lower extremities and feet.” A total of 71 articles (70 overseas and one domestic articles) that assessed the application of ESWT to the three disorders were selected. By each disorder, there were 54 articles on plantar fasciitis (53 overseas and 1 domestic articles), 11 articles on Achilles tendinopathy (11 overseas and 0 domestic articles), and 7 articles on patellar tendinopathy (7 overseas and 0 domestic articles).

1)Plantar fasciitis A total of 54 articles (47 randomized controlled trials [RCTs] and 6 non-randomized studies [NRSs]) were included in the assessment of ESWT for patients with plantar fasciitis. The total sample size consisted of 2,448 in the intervention groups and 2,381 in the control groups. With respect to the type of comparator intervention, there were 14 studies on comparison with steroid injection, 19 studies on comparison with conservative therapy, and 21 studies on comparison with sham ESWT. The patients included in this assessment were mostly patients with chronic plantar fasciitis who had symptoms persisting for at least six months.

Safety Safety of ESWT for patients with plantar fasciitis was assessed based on procedure-related adverse events (AEs) and complications. Procedure-related AEs and complications were reported in 28 articles, of which, 16 articles (57.1%) reported no ESWT-related AEs. ESWT-related AEs reported in the remaining 12 articles were most pain that occurred during the procedure or 3-5 days after the procedure, while mild neurological symptoms (i.e., numbness) were also identified. The reported AEs were resolved within one week after treatment and most were identified to be mild AEs anticipated with ESWT.

Effectiveness Effectiveness of ESWT for patients with plantar fasciitis was assessed based on pain, function, and quality of life (QoL). The effectiveness outcomes were presented by type of comparator intervention. First, there 14 articles on comparison of effectiveness between ESWT and steroid injection. A total of 13 articles reported on pain outcomes and the meta-analysis (including 10 RCTs) results showed no statistically significant difference between ESWT and steroid injection groups (MD –0.60; 95% CI –1.29, 0.09; I2=91%). A total of eight articles reported on functional outcomes and the meta-analysis (including 7 RCTs) results showed statistically significant improvement in function indicators in ESWT group, as compared to steroid injection group (SMD 0.45; 95% CI 0.27, 0.63; I2=0%). Meanwhile, there were no articles on comparison between ESWT and steroid injection that reported QoL outcomes. Second, there were 19 articles on comparison between ESWT and conservative therapy. A total of 17 articles (20 comparators) reported on pain outcomes and the meta-analysis (including 15 RCTs and 15 comparators) results showed no statistically significant difference between ESWT and conservative therapy groups (SMD 0.07; 95% CI –0.59, 0.74; I2=95%). A total of 13 articles (15 comparators) reported on functional outcomes and the meta-analysis (including 11 RCTs and 12 comparators) results showed statistically significant improvement in function indicators in ESWT group, as compared to conservative therapy group (SMD –0.99; 95% CI – 1.91, -0.07; I2=96%). A total of four articles reported on QoL outcomes, of which, three studies showed statistically significant improvement in QoL in ESWT group, as compared to conservative therapy group. Third, there were 21 articles on comparison between ESWT and ESWT. A total of 21 articles reported on pain outcomes and the meta-analysis (including 11 RCTs) results showed statistically significantly lower pain scores in ESWT group, as compared to sham ESWT group (MD –1.71; 95% CI –2.44, -0.98; I2=70%). A total of 12 articles reported on functional outcomes and the meta-analysis (including 7 RCTs) results showed statistically significant improvement in function indicators in ESWT group, as compared to sham ESWT group (SMD 0.84; 95% CI 0.23, 1.45; I2=91%). Meanwhile, three articles on comparison between ESWT and sham ESWT groups reported QoL outcomes, with one study reporting statistically significant improvement in physical and mental health domains in ESWT group, as compared to sham ESWT groups, and the other two studies reporting significant differences in only some domains at specific time points.

2)Achilles tendinopathy A total of 11 articles (8 RCTs and 3 NRSs) were included in the assessment of ESWT for patients with Achilles tendinopathy. The total sample size consisted of 332 in the intervention groups and 281 in the control groups. With respect to the type of comparator intervention, there were seven studies on comparison with conservative therapy and four studies on comparison with sham ESWT. The patients included in this assessment were mostly patients with symptoms persisting for at least six months.

Safety Safety of ESWT for patients with Achilles tendinopathy was assessed based on procedure-related AEs and complications. Procedure-related AEs and complications were reported in eight articles. There were no serious complications reported and the incidence of mild complications reported in three articles ranged between 8.8% and 12.5%. Procedure-related AEs reported in the articles included mostly pain during and after the procedure, redness, and temporary numbness, which were resolved without any additional intervention.

Effectiveness Effectiveness of ESWT for patients with Achilles tendinopathy was assessed based on pain, function, and QoL. The effectiveness outcomes were presented by type of comparator intervention. First, there were seven articles on comparison between ESWT and conservative therapy. All seven articles reported pain outcomes and meta-analysis (including 4 RCTs) results showed statistically significant improvement in pain in ESWT group, as compared to conservative therapy group (SMD –0.56; 95% CI –1.04, -0.09; I2=68%). All seven articles also reported on functional outcomes and meta-analysis (including 4 RCTs) results showed no statistically significant difference between ESWT and conservative therapy groups (SMD –0.36; 95% CI –0.44, 1.17; I2=89%). In one article that reported on QoL, ESWT group showed statistically significant improvement in QoL, as compared to conservative therapy group. Second, there were four articles on comparison between ESWT and sham ESWT. A total of three articles reported on pain outcomes and meta-analysis (including 3 RCTs) results showed greater improvement in ESWT group, as compared to sham ESWT group, but the difference was not statistically significant (MD –0.84; 95% CI –2.13, 0.45; I2=42%). A total of four articles reported on functional outcomes and meta-analysis (including 4 RCTs) results showed greater functional improvement in ESWT group, as compared to sham ESWT group, but the difference was not statistically significant (SMD 0.72; 95% CI –0.09, 1.53; I2=87%). In one article that reported on QoL, there was no significant difference in QoL between ESWT and sham ESWT groups at 1-year follow-up.

3)Patellar tendinopathy A total of seven articles (6 RCTs and 1 NRSs) were included in the assessment of ESWT for patients with patellar tendinopathy. The total sample size consisted of 172 in the intervention groups and 173 in the control groups. With respect to the type of comparator intervention, there were three studies on comparison with conservative therapy and four studies on comparison with sham ESWT. The patients included in this assessment were mostly patients with symptoms persisting for at least three months.

Safety Safety of ESWT for patients with Achilles tendinopathy was assessed based on procedure-related AEs and complications. Procedure-related AEs and complications were reported in four articles. Two articles reported no procedure-related AEs and complications of any kind, while one article reported mild complications in 6.1% of cases and the other article reported temporary numbness in one patient (3.7%), which was resolved after application of cold pack.

Effectiveness Effectiveness of ESWT for patients with Achilles tendinopathy was assessed based on pain, function, and QoL. The effectiveness outcomes were presented by type of comparator intervention. First, there were three articles on comparison between ESWT and conservative therapy. All three articles reported pain outcomes and meta-analysis (including 2 RCTs) results showed statistically significantly lower pain in ESWT group, as compared to conservative therapy group (MD –3.89; 95% CI –4.32, -3.46, I2=0%). Two articles reported on functional outcomes, with both reporting statistically significant improvement in function indicators in ESWT group, as compared to conservative therapy group. Meanwhile, there were no articles on comparison between ESWT and conservative therapy that reported QoL outcomes. Second, there were four articles on comparison between ESWT and sham ESWT. All four articles reported pain outcomes and meta-analysis (including 4 RCTs) results showed no statistically significant difference between ESWT and sham ESWT groups (MD –0.05; 95% CI –0.71, 0.61; I2=0%). All four articles also reported functional outcomes and meta-analysis (including 3 RCTs) results showed no statistically significant difference between ESWT and sham ESWT groups (MD –4.62; 95% CI –10.03, 0.79; I2=0%). Meanwhile, there were no articles on comparison between ESWT and sham ESWT that reported QoL outcomes.

Conclusions and recommendations Based on the evidence currently available through existing literature, the subcommittee presented the following findings:

1)Plantar fasciitis ESWT applied to patients with plantar fasciitis caused no serious AEs and most AEs reported were mild AEs that may be expected with ESWT. Accordingly, the subcommittee concluded that ESWT is a safe technology that can be applied clinically. ESWT showed significant improvement in pain and functional indicators when compared to sham ESWT, but significant improvement only in functional indicators when compared to steroid injection. As compared to conservative therapy, ESWT did not show significant differences in pain and functional outcomes, but ESWT group showed greater improvement in functional indicators. Accordingly, the subcommittee concluded that the effectiveness of ESWT is similar to that of conventional therapies such as steroid injection and conservative therapy. Moreover, considering that the patients included in the assessment were patients with chronic plantar fasciitis who had symptoms persisting for at least six months, the subcommittee determined that ESWT can be a meaningful treatment modality that can be expected to produce long-term therapeutic effect in patients with chronic plantar fasciitis.

2)Achilles tendinopathy ESWT applied to patients with Achilles tendinopathy caused no serious AEs and most AEs reported were mild AEs that may be expected with ESWT. Accordingly, the subcommittee concluded that ESWT is a safe technology that can be applied clinically. ESWT showed greater effectiveness in improving pain, function, and QoL as compared to conservative therapy, but no significant difference as compared to sham ESWT. Accordingly, the subcommittee concluded that it is difficult to assess the effectiveness of ESWT in patients with Achilles tendinopathy based on the fact that ESWT did not show significant improvement as compared to sham ESWT, the studies included in the assessment were small scale studies, and the level of evidence in articles published to date is low.

3)Patellar tendinopathy ESWT applied to patients with patellar tendinopathy caused no serious AEs and most AEs reported were mild AEs that may be expected with ESWT. Accordingly, the subcommittee concluded that ESWT is a safe technology that can be applied clinically. ESWT showed greater effectiveness in improving pain, function, and QoL as compared to conservative therapy, but no significant difference as compared to sham ESWT. Accordingly, the subcommittee concluded that it is difficult to assess the effectiveness of ESWT in patients with patellar tendinopathy based on the fact that ESWT did not show significant improvement as compared to sham ESWT, the studies included in the assessment were small scale studies, and the level of evidence in articles published to date is low. An integration subcommittee was convened to review the conclusions for ESWT [musculoskeletal disorders] for each disorder and derive an integrative conclusion. The integration subcommittee opined that it would be more meaningful to assess the effectiveness of ESWT against conservative therapy than other comparator interventions and concluded that the effectiveness of ESWT for plantar fasciitis, Achilles tendinopathy, and patellar tendinopathy can recognized since its significant effect relative to conservative therapy was confirmed. During the second session of the Committee of Health Technology Reassessment in 2022 (February 18, 2022), the following conclusions were made on “ESWT [musculoskeletal disorders] for lower extremities and feet” based on the subcommittee findings and integration subcommittee review in accordance with Article 4-10 of the Guidelines for Management of Health Technology Reassessment Project. ESWT for patients with plantar fasciitis was found to be safe and showed partial therapeutic effect in reducing pain and improving function and QoL, as compared to comparator interventions (steroid injection, conservative therapy, and sham ESWT). Accordingly, ESWT is a safe and effective technology that is “conditionally recommended” for patients with chronic plantar fasciitis in consideration of the fact that most of the patients included in the assessment were patients with chronic plantar fasciitis. ESWT for patients with Achilles tendinopathy is a safe technology that is “conditionally recommended” in consideration of evidence of its effective in reducing pain and improving function and QoL, as compared to conservative therapy, reported in literature published to date. ESWT for patients with patellar tendinopathy is a safe technology that is “conditionally recommended” in consideration of evidence of its effective in reducing pain and improving function and QoL, as compared to conservative therapy, reported in literature published to date.
