교질반응 TTT [화학반응-장비측정] (Thymol Turbidity Test, TTT)

Abstract

평가 배경 교질반응 TTT [화학반응-장비측정]은 간 질환(의심) 환자를 대상으로 혈장 단백 성분의 양적, 질적 이상을 통해 간 기능을 평가하는 기술이다. 동 기술은 연도별 환자 수, 총 사용량 및 진료금액 총액이 모두 감소하는 쇠퇴기술에 해당하여 진단검사의학 전문가들이 재평가를 제안하였으며, 2020년 제6차 의료기술재평가위원회(2020.6.12. ~ 6.19.)에서 의료기술재평가 안건으로 선정되었다. 2020년 제10차 의료기술재평가위원회(2020.10.16.)에서는 교질반응 TTT [화학반응-장비측정]의 안전성 및 유효성 평가 결과를 최종 심의하였다.

평가 방법 교질반응 TTT [화학반응-장비측정]의 안전성 및 유효성 평가는 체계적 문헌고찰을 통해 수행하였다. 모든 평가방법은 평가목적을 고려하여 “교질반응 TTT [화학반응-장비측정] 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 평가의 핵심질문은 ‘간 질환(의심) 환자에서 교질반응을 통해 간 기능을 평가하는 것이 안전하고 유효한가?’이다. 문헌검색은 국외 3개, 국내 5개 데이터베이스를 활용하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림 위험 평가는 QUADAS-2를 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 제3자와 함께 논의할 예정이었으나 이견은 없었다. 자료분석은 정량적 분석(quantitative analysis)이 불가능하여 정성적(qualitative review) 분석을 적용하였다.

평가 결과 교질반응 TTT [화학반응-장비측정]의 안전성 및 유효성은 총 6편(국외 6편; 환자-대조군 연구 5편, 증례연구 1편)에 근거하여 평가하였다. 중재검사는 구체적으로 Maclagan, Shank-Hoagland, Reinhold 방법 등이 사용되었으며, 기준치 값은 4, 4.7, 5, 5.2, 15 units으로 문헌마다 상이하였다. 안전성 교질반응 TTT [화학반응-장비측정]의 안전성은 환자의 검체를 이용하여 체외에서 이루어지는 검사로 동 검사가 검체 채취과정 이외에는 환자에게 직접적인 위해를 가하지 않으므로 안전성에는 문제가 없는 것으로 평가하였다.

유효성 교질반응 TTT [화학반응-장비측정]의 유효성은 진단정확성, 비교검사와의 상관성, 질병과의 관련성 지표로 평가하고자 하였으나, 진단정확성, 비교검사와의 상관성을 보고한 문헌은 없었다. 질병과의 관련성을 보고한 총 6편 중 5편에서 건강 또는 환자 대조군과 티몰 혼탁 값을 비교하였다. 이 중 2편(1편은 건강 대조군, 다른 1편은 환자 대조군과 비교)에서만 통계적으로 유의한 차이를 보고하였고, 다른 3편에서는 통계적 유의성을 보고하지 않았다. 나머지 1편에서는 간 질환자를 대상으로 중재검사를 포함한 총 10가지 검사의 유용성을 비교하였고, 그 결과 연구에 포함된 검사 중 중재검사의 변별력이 가장 낮게 보고되었다.

결론 및 제언 따라서, 문헌적 근거를 토대로 소위원회에서는 다음과 같이 제언하였다. 교질반응 TTT [화학반응-장비측정]은 검체 채취과정 이외에는 환자에게 직접적인 위해를 가하지 않으므로 안전성에는 문제가 없는 기술이나, 주요 결과 지표인 진단정확성 및 비교검사와의 상관성을 보고한 문헌이 확인되지 않았고, 질병과의 관련성을 보고한 문헌에서도 일관된 결과를 보고하고 있지 않아 유효하지 않은 기술로 판단하였다. 추가적으로 동 검사는 임상적으로 사용하지 않고, 간 기능을 평가하는 검사로 총 단백 검사와 알부민 검사가 사용되고 있으며, 현재 건강보험 요양급여 목록에 간 기능을 평가하는 검사가 다수 존재하며, 동 기술의 사용을 권고하지 않을 경우 임상적으로 발생할 수 있는 문제는 없다는 의견이었다. 의료기술재평가위원회는 의료기술재평가사업 관리지침 제4조 제10항에 의거 “교질반응 TTT [화학반응-장비측정]”에 대해 검체 채취과정 이외에는 환자에게 직접적인 위해를 가하지 않으므로 안전성에는 문제가 없는 기술이나, 주요 결과 지표인 진단정확성 및 비교검사와의 상관성을 보고한 문헌이 확인되지 않았고, 질병과의 관련성을 보고한 문헌에서도 일관된 결과를 보고하고 있지 않아 유효성이 없어 권고하지 않음으로 심의하였다(2020. 11. 5.).

Background of Assessment Colloidal reaction TTT [Chemical Reaction-Equipment Measurement] is a technology to evaluate liver function through quantitative and qualitative abnormalities of plasma protein components in patients with (suspected) liver disease. This technology corresponds to an obsolete technology in which the number of patients, total usage, and total amount of treatment have been reducing by years, so diagnostic laboratory medicine experts suggested a re-assessment. It was selected as an agenda item for Health Technology Reassessment at the 6th Health Technology Reassessment Committee (2020.6.12 - 6.19.) in 2020. , and the safety and effectiveness assessment results of the Colloidal reaction TTT [Chemical Reaction-Equipment Measurement] were finally deliberated at the 10th Health Technology Reassessment Committee (2020.10.16.) in 2020.

Assessment Method The safety and effectiveness assessment of the colloidal reaction TTT [Chemical Reaction-Equipment Measurement] was performed through a systematic literature review. All assessment methods were determined after deliberation by the “Colloidal reaction TTT [Chemical Reaction-Equipment Measurement] subcommittee (hereinafter referred to as the ‘sub-committee’) in consideration of the assessment purpose. The key question in the assessment is ‘Is it safe and effective to evaluate liver function through colloidal reaction in patients with (suspected) liver disease?’. For the literature search, three overseas and five domestic databases were used, and two reviewers independently selected them according to the literature inclusion and exclusion criteria. The risk assessment of the risk of bias in the literature was performed independently by two reviewers using QUADAS-2 and consensus was reached. Data extraction was performed independently by two reviewers using a pre-determined data extraction format. If there was any disagreement, it was planned to be discussed with a third party, but there was no disagreement. For data analysis, quantitative analysis was not possible, so a qualitative review was applied.

Assessment Results The safety and effectiveness of Colloidal reaction TTT [Chemical Reaction-Equipment Measurement] were evaluated based on a total of 6 articles (6 overseas studies; 5 case-control studies, 1 case study). For interventional testing, the Maclagan, Shank-Hoagland, and Reinhold methods were specifically used. The baseline values were 4, 4.7, 5, 5.2, and 15 units, which differed for each literature.

Safety The safety of colloidal reaction TTT [Chemical Reaction-Equipment Measurement] is an in vitro test that uses a patient's sample. Since the test does not directly harm the patient other than the sample collection process, it was evaluated that there is no problem in safety.

Effectiveness The effectiveness of colloidal reaction TTT [Chemical Reaction-Equipment Measurement] was evaluated as an indicator of diagnostic accuracy, correlation with comparative tests, and relevance to disease, but there was no literature reporting diagnostic accuracy and correlation with comparative tests. Thymol turbidity values were compared with healthy or patient controls in 5 out of 6 cases reporting relevance to disease. Of these, only two articles (one in comparison with the healthy control group and the other with the patient control group) reported statistically significant differences, and the other three articles did not report statistical significance. The remaining 1 article compared the usefulness of a total of 10 tests, including interventional tests, for liver disease patients. As a result, the discriminatory power of the interventional test was reported as the lowest among the tests included in the study.

Conclusion and Suggestions Therefore, based on the literature basis, the subcommittee made the following recommendations. Colloidal reaction TTT [Chemical Reaction-Equipment Measurement] does not directly harm patients other than the sample collection process, so there is no safety problem. However, it was judged as an invalid technology because there are no documents reporting the correlation with diagnostic accuracy and comparative tests, which are the main outcome indicators and it did not report consistent results even in the literature reporting the relationship with the disease. In addition, this test is not used clinically, but the total protein test and albumin test are used as tests to evaluate liver function. There are a number of tests evaluating liver function in the current health insurance care benefit list, and the opinion was that there would be no clinical problems if the use of this technology was not recommended. “Colloidal reaction TTT [Chemical Reaction-Equipment Measurement]” does not directly harm patients other than the sample collection process and there is no safety problem, but the literature reporting the correlation with diagnostic accuracy and comparative tests, which are the main outcome indicators, was not confirmed and the literature reporting the correlation with the disease did not report consistent results. Therefore, The Health Technology Reassessment Committee deliberated that it was not recommended due to lack of effectiveness in accordance with Article 4, Paragraph 10 of the Management Guidelines for the Health Technology Reassessment Project (2020. 11. 5.).
