시아노아크릴레이트를 이용한 복재정맥 폐색술

Abstract

평가 배경 시아노아크릴레이트를 이용한 복재정맥 폐색술(saphenous vein occlusion with cyanoacrylate 또는 cyanoacrylate adhesive closure, CAC)은 복재정맥의 역류를 동반한 하지정맥류 환자의 치료를 위해 복재정맥 내 접착 폐색을 시행하는 기술이다. 동 기술은 2016년 3월 의료기기 허가-신의료기술평가 통합심사안건으로 신청된 후, 신의료기술로 인정되었으며, 이후 건강보험심사평가원에 결정신청되어 2018년 제5차 의료행위전문평가원회에서 비급여 행위로 심의되었다(보건복지부 고시 제2018-123호, ‘18.07.01. 시행). 동 기술은 신의료기술평가 후 5년이 경과된 기술로, 내부 모니터링을 통해 재평가 주제로 발굴되었으며, 보건의료 자원의 효율적 사용 도모와 의료기술의 최적 사용을 유도하기 위해 최신 근거를 확인하고자 재평가를 수행하였다.

평가 방법 시아노아크릴레이트를 이용한 복재정맥 폐색술의 안전성 및 효과성의 평가를 위해 체계적 문헌고찰을 수행하였으며, 모든 평가방법은 평가목적을 고려하여 “시아노아크릴레이트를 이용한 복재정맥 폐색술 소위원회(이하 ‘소위원회’)” 심의를 거쳐 확정하였다. 평가의 핵심질문은 하지정맥류 환자에서 시아노아크릴레이트를 이용한 복재정맥 폐색술은 임상적으로 안전하고, 효과적인가이며, 안전성 측면으로는 시술 관련 합병증 및 부작용을, 효과성 측면으로 표적 정맥의 폐쇄율, Venous Clinical Severity Score (VCSS), 삶의 질(Averdeen Varicose Vein Questionnaire [AVVQ], Chronic Venous Insufficiency Quality of life questionnaire [CIVIQ] 등)을 확인하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌선택 및 배제기준에 따라 2명의 평가자가 독립적으로 수행하고, 의견의 불일치가 있는 경우에는 평가자간 합의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림 위험 평가는 코크란의 Risk of Bias (RoB) 및 비무작위 연구 문헌의 비뚤림 위험 평가 도구인 Risk of Bias Assessment tool for Non-randomized Studies (RoBANS ver.2)를 사용하여 평가하였다. 최종 선택된 문헌을 대상으로 2명의 평가자가 독립적으로 평가하였으며, 의견이 불일치한 경우 평가자간 합의를 통해 일치된 결과를 도출하였다. 자료 분석은 기본적으로 비교중재의 유형에 따라 구분하여 수행하였으며, 정량적 분석이 가능할 경우 메타분석을 수행하고 불가능한 경우 질적 검토를 수행하였다. 체계적 문헌고찰 결과는 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 방법론을 이용하여 근거수준을 평가하였으며, 국민참여단 온라인 설문조사를 통한 사회적 가치평가를 수행하였다. 본 평가는 소위원회의 검토의견을 고려하여 의료기술재평가위원회에서 최종심의 후 권고등급을 제시하였다.

평가 결과 체계적 문헌고찰을 수행한 결과, 핵심질문을 충족하는 26개 연구(총 31편 문헌)가 선정되었다. 연구유형별로 살펴보면, 무작위배정 비교임상연구(randomized controlled trial, RCT) 4개, 비무작위 임상연구(non-randomized study, NRS) 22개가 포함되었다. 비교중재의 유형별로는 수술과 비교한 연구가 5개(RCT 1개, NRS 4개), 고주파 치료(radiofrequency ablation, RFA)와 비교한 연구가 18개(RCT 2개, NRS 16개), 레이저 치료(endovenous laser ablation, EVLA)와 비교한 연구가 12개(RCT 2개, NRS 10개)이었다. 안전성 시아노아크릴레이트를 이용한 복재정맥 폐색술의 안전성은 시술 관련 합병증 및 부작용을 검토하였으며, 안전성 결과는 비교중재의 유형에 따라 제시하였다. 수술과 비교한 연구 5개(RCT 1개, NRS 4개)에서 중증 합병증은 발생하지 않았다고 보고하였으며, 중재군에서는 알러지반응, 혈전정맥염이, 수술군에서는 반상출혈, 비복신경 신경병증, 저린감이 더 많이 발생하는 것으로 보고되었다. 고주파 치료와 비교한 17개 연구(RCT 2개, NRS 15개) 중 대부분의 연구에서 중증 합병증은 발생하지 않았다고 보고하였다. 다만, Korepta 등(2022) 연구에서 심부정맥혈전증(deep vein thrombosis, DVT)이 CAC군에서 1명(6.3%) 발생하였고, Yang 등(2019) 연구에서는 DVT가 CAC군에서 1명(0.9%), RFA군 1명(0.6%)에서 발생하였으나, 2건 모두 무증상으로 항응고요법으로 치료되었다고 보고하였다. 기타 합병증 관련 대부분은 두 군 간 유의한 차이는 확인되지 않았으며, 알레르기 반응의 경우 2편의 연구에서 RFA군에 비해 CAC군에서 유의하게 많이 발생한 반면, 반상출혈은 6편의 연구에서 RFA군에 비해 CAC군에서 유의하게 적게 발생한 것으로 보고되었다. 레이저 치료와 비교한 10개 연구(RCT 2개, NRS 8개)에서 중증 합병증은 대부분의 연구에서 발생하지 않은 것으로 보고하였다. 다만, Korepta 등(2022) 연구에서 DVT가 CAC군 1명(6.3%)에서 발생을 보고하였다. 기타 합병증 관련하여, 반상출혈, 감각이상, 색소침착 등의 합병증은 EVLA군에 비해 CAC군에서 유의하게 적게 발생한 것으로 보고되었다. 안전성 결과를 종합하면, 시아노아크릴레이트를 이용한 복재정맥 폐색술과 관련하여 중증 합병증은 대부분 발생하지 않았으며, 일부 연구에서 드물게 보고된 심부정맥혈전증은 무증상인 경우로 추가 처치 후 치료되었다. 그 외 합병증은 대부분 비교 군 간 유의한 차이가 없었으며, 반상출혈, 감각이상, 색소침착 등의 합병증은 기존 치료법에 비해 중재군에서 발생률이 낮았다. 다만, 알레르기 반응은 중재군에서 10~15% 수준으로 기존 치료법에 비해 높았으나, 대부분 보존적 치료(비스테로이드소염제 등)로 치료되었다. 이에 소위원회에서는 시아노아크릴레이트를 이용한 복재정맥 폐색술은 안전한 의료기술이라는 의견이었다. 효과성 시아노아크릴레이트를 이용한 복재정맥 폐색술의 효과성은 정맥 폐쇄율, 증상 완화 측면에서 VCSS, 통증, 삶의 질(AVVQ, CIVIQ 등)로 확인하였으며, 효과성 결과는 비교중재의 유형에 따라 제시하였다. 첫째, 수술과 비교한 연구는 총 5개(RCT 1개, NRS 4개)이었다. 폐쇄율은 4개 연구(RCT 1개, NRS 3개)에서 보고하였으며, 시점에 따라 단기(1년 이하), 중기(1~3년), 장기(3년 초과)를 구분하여 분석하였다. 단기 폐쇄율은 1개의 RCT 연구에서 1년 시점의 폐쇄율이 두 군 모두 100% (중재군 63명, 수술군 63명)로 보고되었으며, NRS 연구(2개)의 메타분석 결과에서는 중재군이 수술군에 비해 단기 폐쇄율이 유의하게 낮았다(odds ratio [OR] 0.03, 95% confidence interval [CI] 0.00~0.24, I2=0%). 중기 폐쇄율은 NRS 1개 연구에서 3년 시점 폐쇄율은 두 군 모두 100% (중재군 50명, 수술군 90명)으로 보고되었다. 장기 폐쇄율을 보고한 연구는 확인되지 않았다. VCSS를 보고한 4개 연구(RCT 1개, NRS 3개) 모두 시술 후 1년 시점에 증상이 개선된 것으로 나타났다. 두 군 간의 비교 시, 4개 연구 중 2개 연구에서 두 군 간 차이가 유의하지 않았으며, 나머지 2개 연구에서는 결과의 방향성이 일관되지 않았다. 삶의 질 지표 중 질환 특이적 삶의 질 측정도구인 AVVQ는 RCT 1개에서 두 간 차이가 유의하지 않았으며, CIVIQ는 NRS 1개에서 시술 후 1년 시점에 CAC군과 수술군에서 모두 삶의 질이 향상된 것으로 보고하였다(군 간 통계적 유의성은 구체적으로 보고되지 않음). 일반적 건강 관련 삶의 질(Short Form-36 [SF-36])은 NRS 연구 1개에서 시술 후 1년 시점에 모든 군의 삶의 질 점수가 시술 전에 비해 향상된 것으로 보고되었다. 둘째, 고주파 치료와 비교한 연구 총 18개(RCT 2개, NRS 16개)이었다. 폐쇄율은 14개 연구(RCT 2개, NRS 12개)에서 보고하였다. 단기/중기/장기 폐쇄율 모두 메타분석 결과 두 군 간 차이가 유의하지 않았다. 시점별로 살펴보면, 단기 폐쇄율은 11개 연구(RCT 2개, NRS 9개)에서 보고하였으며, RCT와 NRS 각각에 대해 메타분석 결과 두 군 간 차이가 유의하지 않았다(OR 1.38, 95% CI 0.63~3.02, I2=0%; OR 0.66, 95% CI 0.40~1.10, I2=33%). 중기 폐쇄율은 6개 연구(RCT 2개, NRS 4개)에서 보고하였으며, RCT와 NRS 각각에 대해 메타분석 결과 두 군 간 차이가 유의하지 않았다(OR 1.39, 95% CI 0.70~2.76, I2=0%; OR 0.72, 95% CI 0.29~1.81, I2=8%). 장기 폐쇄율을 보고한 2개 연구(RCT 1개, NRS 1개)에서 군 간 차이가 없었다. 장기 폐쇄율은 5년 시점 결과로, Morrison 등(2015)의 RCT 연구에서는 중재군 93.6% (44/47명), 고주파 치료군 100% (33/33명)이었으며, El Kilic 등(2022)의 후향적 코호트 연구에서는 중재군 80.8% (59/73명), 고주파 치료군 86.6% (71/82명)이었다. VCSS를 보고한 11개 연구(RCT 2개, NRS 9개) 중 대부분의 연구(9개)에서 두 군 간 차이가 유의하지 않았다. 삶의 질 지표 중 AVVQ는 4개 연구(RCT 1개, NRS 3개)에서 모든 시점에서 두 군 간 차이가 유의하지 않은 것으로 보고되었다. CIVIQ는 NRS 1개에서 시술 후 1년 시점에 두 군에서 모두 삶의 질이 향상된 것으로 보고하였다(군 간 통계적 유의성은 구체적으로 보고되지 않음). 일반적 건강 관련 삶의 질은 2개 연구(RCT 1개, NRS 1개)에서 보고되었으며, RCT 연구에서는 EuroQol-5 Dimension (EQ-5D)로 삶의 질 측정 결과 두 군 간 차이가 유의하지 않았으며, 다른 1개 NRS 연구에서 SF-36을 사용하여 평가하였으며, 일부 영역에서 수술+RFA 대비 CAC군에서의 삶의 질 결과가 유의하게 낮은 것으로 보고하였다. 셋째, 레이저 치료와 비교한 연구 총 12개(RCT 2개, NRS 10개)이었다. 폐쇄율은 10개 연구(RCT 2개, NRS 8개)에서 보고하였다. 9개 연구(RCT 2개, NRS 7개)에서 보고한 단기 폐쇄율은 RCT와 NRS 각각에 대해 메타분석 결과 두 군 간 차이가 유의하지 않았다(OR 1.40, 95% CI 0.71~2.78, I2=0%; OR 0.61, 95% CI 0.25~1.52, I2=72%). 중기 폐쇄율은 4개 연구(RCT 1개, NRS 3개)에서 보고하였으며, RCT와 NRS 각각에서 두 군 간 차이가 유의하지 않았다(OR 1.23, 95% CI 0.53~2.83; OR 0.58, 95% CI 0.16~2.03, I2=76%). 장기 폐쇄율은 NRS 1개(El Kilic et al., 2022)에서 5년 시점 폐쇄율이 중재군 80.8% (59/73명), 레이저 치료군 70.1% (54/77명)이었으며, 군 간 차이가 없다고 보고하였다. VCSS는 9개 연구(RCT 2개, NRS 7개)에서 보고되었으며, 대부분의 연구(7개)에서 두 군 간 차이가 유의하지 않았다. 삶의 질 지표 중 AVVQ는 NRS 1개 연구에서 두 군 간 차이가 유의하지 않았으며, CIVIQ는 2개 연구(RCT 1개, NRS 1개)에서 모든 시점에서 두 군 간 차이가 유의하지 않은 것으로 보고되었다. 일반적 건강 관련 삶의 질 결과를 보고한 문헌은 확인되지 않았다. 효과성 결과를 종합하면, 중재군의 폐쇄율은 1편을 제외한 모든 연구에서 80.8~100%를 보고하였다. 기존 치료와 비교 결과에서는 수술과 비교된 단기 시점의 NRS 2개 연구결과에서만 중재군의 폐쇄율이 낮았고, 그 외 연구들에서는 고주파 치료 또는 레이저 치료와 비교 시 단기/중기/장기 폐쇄율은 유의한 차이가 없는 것으로 확인하였다. 또한, 증상 완화 측면에서 VCSS와 삶의 질 결과는 비교중재에 상관없이 대부분의 연구에서 군 간 유의한 차이가 없는 것으로 확인되었다. 이에 대해 소위원회에서는 수술과 비교한 문헌이 매우 제한적이며, 현행 하지정맥류 치료 가이드라인에서 혈관내 치료법(고주파 치료, 레이저 치료)을 일차 치료로 권하고 있으므로 수술과의 비교 결과 보다는 정맥 내 열 치료인 고주파 치료 및 레이저 치료와 비교한 결과가 임상적으로 의미 있다는 의견이었다. 또한, 본 재평가에서는 이전 신의료기술평가 당시 검토되지 않았던 5년 이상의 장기 추적관찰 결과를 확인한 결과, 시아노아크릴레이트를 이용한 복재정맥 폐색술의 장기 폐쇄율이 기존 치료법과 유사한 수준으로 지속된다는 것을 확인하였다. 다만, 아직까지 잘 설계된 RCT 연구에서의 장기 추적관찰 결과는 제한적이므로 향후 추가 근거에 대한 재검토가 필요하다는 의견이었다.

결론 및 제언 소위원회에서는 현재 문헌적 근거를 바탕으로 다음과 같이 결과 및 의견을 제시하였다. 시아노아크릴레이트를 이용한 복재정맥 폐색술은 중증 합병증이 거의 발생하지 않았고, 기존 치료법과 유사한 수준의 안전성을 나타내었다. 이에 소위원회에서는 시아노아크릴레이트를 이용한 복재정맥 폐색술은 안전한 의료기술로 판단하였다. 시아노아크릴레이트를 이용한 복재정맥 폐색술은 주요 의료결과인 폐쇄율이 기존 치료법과 비교 시 일부 연구에서 수술에 비해 단기 시점 결과에서 중재군의 폐쇄율이 낮게 나타났으나, 수술과 비교한 문헌이 매우 제한적이며, 현행 하지정맥류 치료 가이드라인에서 정맥 내 열 치료를 일차 치료로 권하고 있으므로 수술과의 비교 결과보다는 정맥 내 열 치료인 고주파 치료 및 레이저 치료와 비교한 결과가 임상적으로 의미가 있다는 의견이었다. 고주파 치료 또는 레이저 치료와 비교 시 단기/중기/장기 폐쇄율은 유의한 차이가 없고, 장기 추적관찰 결과 폐쇄율이 80.8% 이상인 것으로 확인하였다. 이에 소위원회에서는 시아노아크릴레이트를 이용한 복재정맥 폐색술의 폐쇄율은 임상적으로 수용 가능하며, 시술 후 유의하게 증상이 개선되었고, 삶의 질이 향상되어 효과적인 치료법으로 판단하였다. 2022년 제12차 의료기술재평가위원회(2022.12.09.)에서는 소위원회 검토 결과에 근거하여 다음과 같이 심의하였다. 의료기술재평가위원회에서는 복재정맥의 역류를 동반한 하지정맥류 환자에서 시아노아크릴레이트를 이용한 복재정맥 폐색술을 ‘조건부 권고함’으로 심의하였다(권고등급: 조건부 권고함).

Background Cyanoacrylate glue closure (CAC), also known as saphenous vein occlusion with cyanoacrylate or cyanoacrylate adhesive closure, is used to perform adhesive occlusion within the saphenous vein for the treatment of lower limb venous insufficiency accompanied by venous reflux. This procedure was recognized as a new medical technology after being submitted as a medical device application in March 2016 and was subsequently approved. Thereafter, an application for determination was filed with the Health Insurance Review and Assessment Service (HIRA), and it was reviewed as a non-reimbursable procedure during the fifth Medical Practice Professional Assessment Committee in 2018. This technology was identified as an item for reassessment by internal monitoring, as it has been five years since the initial assessment. Accordingly, reassessment was performed to determine the latest evidence to promote the efficient use of healthcare resources and induce optimal use of health technology.

Methods To evaluate the safety and efficacy of CAC, a systematic literature review was conducted. All assessment methods were finalized after review by the “cyanoacrylate glue closure subcommittee” (hereinafter referred to as the “subcommittee”), with consideration of the assessment. The key question in the assessment was “Is CAC clinically safe and effective in patients with varicose veins?” Safety was assessed based on procedure-related complications and adverse events (AEs), whereas effectiveness was assessed based on saphenous vein occlusion rate, venous clinical severity score (VCSS), and quality of life (QoL; Aberdeen Varicose Vein Questionnaire [AVVQ] and chronic venous insufficiency quality of life questionnaire [CIVIQ]). In the systematic review, three international and five domestic databases were searched based on the key question, and two reviewers independently screened and selected the articles in accordance with the inclusion and exclusion criteria. In the event of any disagreement, the final articles were selected through discussions among the reviewers to reach a consensus. Risk of bias (RoB) assessment was performed for the selected articles using the Cochrane RoB and Risk of Bias Assessment tool for Non-randomized Studies (RoBANS ver.2). Two reviewers independently assessed the final selected articles, and in the event of any disagreement, consensus was reached through discussion among the reviewers. Data analysis was performed according to different comparator interventions, using meta-analysis if a quantitative analysis was possible and qualitative review if not. The systematic review results were assessed for the level of evidence using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) method. In addition, an online questionnaire survey was conducted on a public participation group to assess social value. For this assessment, the Committee of Health Technology Reassessment considered the subcommittee review opinions and presented their recommendation after a final review.

Results From the systematic review, a total of 26 studies (31 articles) that addressed the key question were selected. Among them, four were randomized controlled trials (RCTs) and 22 were non-randomized studies (NRS). In terms of the types of comparator interventions, there were five studies comparing the procedure with surgery (one RCT and four NRS), 18 studies comparing it with radiofrequency ablation (RFA) (two RCTs and 16 NRS), and 12 studies comparing it with endovenous laser ablation (EVLA) (two RCTs and 10 NRS).

Safety The safety of CAC was assessed based on procedure-related complications and AEs, and the safety outcomes were presented according to the types of comparator interventions. There were no reported cases of severe complications in five studies (one RCT and four NRSs) comparing CAC to surgery. Allergic reactions, thrombophlebitis, and postoperative bleeding were reported to occur more frequently in the intervention group, whereas non-recurrent nerve palsy and increased paresthesia were more commonly reported in the surgery group. Among 17 studies (two RCTs and 15 NRSs) comparing CAC to RFA, the majority of studies reported no occurrence of severe complications. However, in a study by Korepta et al. (2022), one case (6.3%) of DVT was reported in the CAC group, and in a study by Yang et al. (2019), DVT occurred in one case (0.9%) in the CAC group and in one case (0.6%) in the RFA group, but both these patients were asymptomatic and treated with anticoagulant therapy. No significant differences were observed in most other complication-related outcomes between the two groups. Meanwhile, two studies reported a significantly higher incidence of allergic reactions in the CAC group than in the RFA group, and six studies reported a significantly lower incidence of ecchymosis in the CAC group than in the RFA group. Among the 10 studies comparing (two RCTs and eight NRSs) CAC to EVLA, the majority reported no occurrence of severe complications. However, in a study by Korepta et al. (2022), one case (6.3%) of DVT was reported in the CAC group. Other complication-related outcomes such as postoperative bleeding, sensory abnormalities, and pigmentation were significantly less frequent in the CAC group compared to the EVLA group. In summarizing the safety outcomes, there were no severe complications related to CAC in most studies, whereas the patients were asymptomatic in the few cases of DVT reported, and the condition was resolved without additional treatment. Meanwhile, no significant differences were noted in other complications between the two groups in most studies, whereas complications such as ecchymosis, paresthesia, and pigmentation occurred at a lower rate in the intervention group than in the conventional therapy. However, allergic reaction was found in 10-15% of patients in the intervention group, which was higher than that of patients in the conventional therapy group, and the condition was treated conservatively (e.g., nonsteroidal anti-inflammatory drugs). Accordingly, the subcommittee determined that CAC is a safe health technology.

Effectiveness The effectiveness of CAC was assessed based on the saphenous vein occlusion rate, VCSS, and QoL (AVVQ and CIVIQ), and the effectiveness outcomes were presented according to the types of comparator interventions.

First, five studies (one RCT and four NRSs) compared CAC to surgery. The occlusion rate was reported in four studies (one RCT and three NRSs) and was analyzed by time point as short-term (< 1 year), mid-term (1–3 years), and long-term (> 3 years). With respect to short-term occlusion rates, one RCT reported a 100% occlusion rate in both the groups (intervention group: n=63 and surgery group: n=63) at one year. A meta-analysis of two NRSs reported a significantly lower short-term occlusion rate in the intervention group than in the surgery group (odds ratio [OR] 0.03, 95% confidence interval [CI] 0.00~0.24, I2=0%). Regarding the mid-term occlusion rate, one NRS reported a 100% occlusion rate in both the groups (intervention group: n=50 and surgery group: n=90) at three years. However, there were no studies that reported on the long-term occlusion rate. In all the four studies (one RCT and three NRSs) that reported on VCSS, symptom improvement was found one year after the procedure. Among the four studies that compared the two groups, two studies reported no significant difference between the two groups, whereas the remaining two studies reported inconsistent results. Among the QoL indicators, one RCT reported no significant difference in AVVQ, which is a measure of disease-specific QoL, between the two groups, while one NRS reported significant improvement in QoL based on CIVIQ in both the CAC and surgery groups at one year after the procedure (did not report specific details about statistical significance between the groups). Concerning general health-related QoL (Short Form-36 [SF-36]), one NRS reported that QoL scores improved in all groups one year after the procedure compared to before the intervention.

Second, 18 studies (two RCTs and 16 NRSs) compared CAC to RFA. The occlusion rate was reported in 14 studies (two RCTs and 12 NRSs). All meta-analysis on the short/mid/long-term occlusion rates indicated no significant differences between the two groups. With respect to different time points, the short-term occlusion rate was reported in 11 studies (two RCTs and nine NRSs), and a meta-analysis of RCTs and NRSs showed no significant differences between the two groups (OR 1.38, 95% CI 0.63~3.02, I2=0%; OR 0.66, 95% CI 0.40~1.10, I2=33%). The mid-term occlusion rate was reported in six studies (two RCTs and four NRSs), and a meta-analysis of RCTs and NRSs showed no significant differences between the two groups (OR 1.39, 95% CI 0.70~2.76, I2=0%; OR 0.72, 95% CI 0.29~1.81, I2=8%). The long-term occlusion rate was reported in two studies (one RCT and one NRS), with no significant difference between the groups. The long-term occlusion rate was defined as results at five years. In an RCT by Morrison et al. (2015), the long-term occlusion rate was 93.6% (n=44/27) in the intervention group and 100% (n=33/33) in the RFA group. In a retrospective cohort study by El Kilic et al. (2022), the long-term occlusion rate was 80.8% (n=59/73) in the intervention group and 86.6% (n=71/82) in the RFA group. Among 11 studies (two RCTs and nine NRSs) that reported on VCSS, most studies (n=9) reported no significant difference between the two groups. Among QoL indicators, AVVQ was used in four studies (one RCT and three NRSs), which reported no significant difference between the two groups at all time points. CIVIQ was used in one NRS, which reported improved QoL in both groups one year after the procedure (they did not report specific details about statistical significance between the groups). General health-related QoL was reported in two studies (one RCT and one NRS). In the RCT that used EuroQol-5 Dimension (EQ-5D) to measure QoL, no significant difference was reported between the two groups, whereas in the other NRS that used SF-36, the QoL was significantly lower in the CAC group than in the surgery and RFA group in some domains of QoL.

Third, 12 studies (two RCTs and 10 NRSs) compared CAC to EVLA. The occlusion rate was reported in 10 studies (two RCTs and eight NRSs). The short-term occlusion rate was reported in nine studies (two RCTs and seven NRSs); a meta-analysis on RCTs and NRSs showed no significant difference between the two groups (OR 1.40, 95% CI 0.71~2.78, I2=0%; OR 0.61, 95% CI 0.25~1.52, I2=72%). There were four studies (one RCT and three NRSs) reporting on the mid-term occlusion rates, with the RCT and NRSs showing no significant difference between the two groups (OR 1.23, 95% CI 0.53~2.83; OR 0.58, 95% CI 0.16~2.03, I2=76%). The long-term occlusion rate was reported in one NRS (El Kilic et al., 2022), with the intervention and EVLA groups showing no significant difference, with an occlusion rate of 80.8% (n=59/73) and 70.1% (n=54/77), respectively, at five years. Among nine studies (two RCTs and seven NRSs) that reported on VCSS, most (n=7) reported no significant difference between the two groups. Among QoL indicators, AVVQ was used in one study, which reported no significant difference between the two groups. CIVIQ was used in two studies (one RCT and one NRS), which reported no significant difference between the two groups at all time points. Meanwhile, articles that reported on general health-related QoL were not found.

In summarizing the effectiveness outcomes, the occlusion rate in the intervention group was 80.8–100% in all studies except one. Among studies comparing CAC to conventional therapy, two NRSs comparing CAC to surgery reported lower occlusion rates in the intervention group, whereas other studies reported no significant differences in the short/mid/long-term occlusion rates when comparing CAC to RFA or EVLA. Moreover, with respect to symptom relief based on VCSS and QoL outcomes, most studies reported no significant differences between the groups regardless of the comparator intervention. Accordingly, because of the limited number of articles comparing CAC to surgery and the current treatment guidelines for varicose veins recommending endovenous treatment (e.g., RFA and EVLA) as the first-line treatment, the subcommittee determined that the results from comparisons to endovenous therapy, such as RFA and EVLA, were more clinically significant than those from comparisons to surgery. Moreover, this reassessment examined the results from long-term follow-up (≥ five years), which were not checked during the initial new health technology assessment. The results showed that long-term occlusion by CAC was sustained at a level similar to that by conventional therapy. However, long-term follow-up results from well-designed RCTs are still limited, indicating a need for additional review in the future for any new evidence.

Conclusions and recommendations Based on available evidence in the literature, the subcommittee presented the following findings and opinions. CAC showed minimal occurrence of severe complications and demonstrated a similar level of safety compared to conventional therapy. Therefore, the subcommittee determined that CAC is a safe health technology. In comparing the occlusion rates, a key health-related outcome of CAC, between CAC and conventional therapy, some studies reported lower short-term occlusion rates in the intervention group than in the surgery group. However, only few articles on comparisons with surgery and current treatment guidelines for varicose veins recommend endovenous treatment as the first-line treatment method. Therefore, the subcommittee determined that the results from comparison to endovenous treatments such as RFA and EVLA were more clinically significant than those from comparison to surgery. Comparisons of CAC with RFA or EVLA showed no significant differences in the short/mid/long-term occlusion rates, with long-term follow-up results showing an occlusion rate of ≥80.8%. Accordingly, the subcommittee determined that CAC is an effective treatment method based on results showing clinically acceptable levels of occlusion, significant improvement in symptoms, and improvement of QoL after the procedure. The following conclusions were made based on the subcommittee findings during the twelfth session of the Committee of Health Technology Reassessment in 2022 (December 9, 2022). The Committee of Health Technology Reassessment “conditionally recommends” using CAC in patients with varicose veins and saphenous venous reflux (Grade of recommendation: conditionally recommended).
