대뇌운동피질자극술 [체내삽입형 신경자극기 이용]

Abstract

평가 배경 대뇌운동피질자극술[체내삽입형신경자극기 이용]은 다른 방법으로는 치료하기 어려운 중추 신경병인성 통증, 안면통증, 불인성 말초신경 통증 등에 대하여 통증을 완화시키기 위해 실시되는 시술이다. 동 기술은 2009년 5월 신의료기술평가위원회에서 안전성 및 유효성이 있는 시술로 평가하였으며, 이에 한국보건의료연구원은 내부 모니터링을 통해 동 기술을 의료기술의 적정 사용 유도 및 보건의료 자원의 효율적 사용을 위한 근거 검토가 필요한 안건으로 판단하여 2020년 제6차 의료기술재평가위원회(2020.06.12.~19.)에서 재평가 대상으로 선정하고, 2021년 제8차 의료기술재평가위원회(2021.08.13.)에서 동 기술의 임상적 안전성 및 유효성 평가에 대한 최종결과를 심의하였다.

평가 방법 대뇌운동피질자극술[체내삽입형신경자극기 이용]에 대한 안전성 및 유효성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “대뇌운동피질자극술[체내삽입형신경자극기 이용] 평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 평가의 핵심질문은 다음과 같다. · 불인성 신경병증성 통증환자에 적용한 대뇌운동피질자극술[체내삽입형 신경자극기 이용]은 안전하고 유효한가? · 이상운동질환 환자에 적용한 대뇌운동피질자극술[체내삽입형 신경자극기 이용]은 안전하고 유효한가? · 중증의 우울증, 중증의 강박장애 환자에 적용한 대뇌운동피질자극술[체내삽입형 신경자극기 이용]은 안전하고 유효한가? 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하여 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림위험 평가는 RoBANS를 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 자료분석은 정량적 분석(quantitative analysis)이 불가능하여 정성적(qualitative review) 분석을 적용하였다. 체계적 문헌고찰 결과의 근거 수준은 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 접근 방법으로 평가하였다.

평가 결과 대뇌운동피질자극술[체내삽입형신경자극기 이용]의 안전성과 유효성은 총 44편에 근거하여 평가하였다. 대상 질환별로 선택문헌을 살펴보면, 36편의 문헌이 불인성 신경병증성 통증 환자, 5편의 문헌은 이상운동질환 환자, 3편의 문헌은 불인성 신경병증성 통증과 이상운동질환환자 모두 포함하여 연구를 수행하였다. 중증의 우울증, 중증의 강박장애를 대상으로 한 문헌은 없었다. RoBANS를 사용하여 평가한 비뚤림위험 수준은 낮은 것으로 평가되었다.

안전성 총 42편에서 사망, 뇌전증을 포함한 신경학적 증상, 감염, 기기관련 부작용, 두개강내 출혈 혹은 혈종 및 국소출혈 등이 확인되었다. 이에 소위원회에서는 사망과 두 개강내 혈종 혹은 출혈을 중증 이상반응 혹은 부작용으로 판단하였다. 이 중 사망은 시술과 직접적인 관련성을 언급한 문헌은 없었고, 사망원인을 고려할 때 동 기술과 직적접인 관련성이 없는 사망으로 판단하였다. 두개강내 혈종 혹은 출혈은 시술과 직접적인 관련은 있으나 발생률이 낮아 수용가능한 수준이라고 판단하였다. 이외 보고된 이상반응 혹은 부작용 결과는 임상적으로 수용가능한 수준으로 판단하였다.

유효성 동 기술에 대한 총 26편 중 불인성 신경병증성 통증환자를 대상으로 한 문헌 24편에서 통증, 삶의 질, 약물사용 변화정도를 기준으로 유효성을 평가하였고, 이상운동질환을 대상으로 한 문헌은 2편으로 운동기능개선정도, 삶의 질, 약물사용 변화정도를 기준으로 평가하였다. 중증 우울증 및 중증 강박장애 환자를 대상으로 대뇌피질운동자극술을 수행한 문헌은 확인되지 않았다. 불인성 신경병증성 통증 환자 대상 문헌 대부분에서 시술 전후 통증감소를 보고하였으며 40% 이상 통증감소를 보고한 환자비율은 38.9-75%이었다. 또한 삶의 질을 보고한 3편의 문헌 중 2편에서는 시술 전에 비해 시술 후 삶의 질이 개선되었음을 보고하였다. 약물사용 변화정도를 보고한 7편의 문헌 중 1편의 문헌에서 시술 전에 비해 시술 후 약물사용감소를 보고하였고, 3편의 문헌에서 보고된 약물사용감량 환자의 비율이 42.3~61.2%이었다. 근거수준은 대부분 문헌에서 비뚤림위험, 비일관성, 비정밀성에서 심각으로 판단하여 ‘Very Low’로 평가하였다. 이에 소위원회에서는 근거수준은 매우 낮으나 동 기술이 기존의 약물치료나 기존 시술로 통증조절이 제한적인 불인성 신경병증성 통증 환자들의 통증감소에 도움을 줄 수 있어 해당 환자들에 한하여 임상적으로 유효하다는 의견이었다. 이상운동질환을 대상으로 동 기술의 유효성 평가에 포함된 연구는 총 2편으로, 시술 전·후 운동장애의 유의한 개선을 보고하고 있어 임상적 유용성이 기대되나 문헌적 근거가 충분하지 않아 연구가 더 필요한 기술로 판단하였다. 중증 우울증 및 중증 강박장애를 대상으로 대뇌운동피질자극술을 수행한 문헌은 확인되지 않아 소위원회에서는 동 기술에 대한 유효성을 판단할 근거가 부족하여 동 기술에 대해 평가할 수 없다는 의견이었다.

결론 및 제언 대뇌운동피질자극술[체내삽입형 신경자극기 이용] 소위원회에서는 현재 평가 결과에 근거하여 다음과 같이 제언하였다. 대뇌운동피질자극술[체내삽입형 신경자극기 이용]은 동 기술과 직접적인 관련 있다고 판단된 중증 이상반응 및 부작용으로 두 개강내 혈종 혹은 출혈이 확인되었고, 두 개강내 혈종 혹은 출혈과 이외 경미한 이상반응 및 부작용의 발생정도를 고려할 때 동 기술의 안전성은 임상적으로 수용 가능한 수준으로 판단하였다. 동 기술의 유효성은 i) 불인성 신경병증성 통증 환자 대상에서 통증감소에 도움을 줄 수 있는 유효한 기술로 판단하였으며, ii) 이상운동질환 환자 대상으로는 임상적 유용성은 기대되나 문헌적 근거가 충분하지 않아 연구가 더 필요한 기술로 판단하였고, iii) 중증 우울증 및 중증 강박장애 환자에서는 동 기술의 유효성을 판단할 문헌이 없어 유효성을 평가할 수 없었다. 2021년 제3차 의료기술재평가위원회는 ‘대뇌운동피질자극술[체내삽입형 신경자극기 이용]’에 대해 소위원회의 검토결과에 근거하여 다음과 같이 심의하였다(2021. 03. 09.). 대뇌운동피질자극술[체내삽입형 신경자극기 이용]은 이상반응 및 부작용이 수용 가능한 수준으로 안전하고 i) 불인성 신경병증성 통증 환자를 대상으로 수행 시 통증감소에 도움을 줄 수 있어 유효한 기술로 판단하여 권고함(권고등급 I-b)으로 심의하였고, ii) 이상운동질환 환자를 대상으로 수행 시 임상적 유용성은 기대되나 문헌적/ 근거가 충분하지 않아 연구가 더 필요한 기술로 판단하여 보류(insufficient)하고 추후 재심의하기로 하였다. iii) 중증 우울증 및 중증 강박장애 환자를 대상으로 수행 시 유효성을 판단할 문헌이 없으므로 유효성을 평가할 수 없어 권고하지 않음(권고등급 II)으로 심의하였다. 2021년 제8차 의료기술재평가위원회는 제3차 의료기술재평가위원회에서 보류한 ‘대뇌운동피질자극술[체내삽입형 신경자극기 이용]’의 이상운동질환 대상의 유효성에 대해 소위원회의 검토결과에 근거하여 다음과 같이 재심의하였다(2021. 08. 13.). 대뇌운동피질자극술[체내삽입형 신경자극기 이용] 이상운동질환 환자를 대상으로 수행시 임상적 유용성은 기대되나 문헌적 근거가 충분하지 않아 연구가 더 필요한 기술로 판단하여 불충분(권고등급: 불충분)으로 심의하였다.

Motor cortex stimulation with implantable neurostimulators

Background Motor cortex stimulation with implantable neurostimulators is performed to alleviate neuropathic pain that cannot be relieved with other methods. This technique is employed to treat spinal, refractory peripheral, and facial neuropathic pain. In May 2009, the New Health Technology Assessment Committee established that motor cortex stimulation was safe and effective, but the National Evidence-based healthcare Collaborating Agency has recently determined that this technique requires reassessment in light of the new scientific evidence on the topic. Therefore, a reassessment plan was approved by the 6th Health Technology Reassessment Committee of 2020 (August 14, 2020) and the relevant subcommittee was designated. The conclusions of the review were drawn by the 8th Health Technology Reassessment Committee in 2021 (August 13, 2021). The purpose of this reassessment is to support policy making concerning the appropriate uses of motor cortex stimulation, as well as the adequate allocation of public healthcare resources to fund this technology.

Methods To assess the safety and effectiveness of motor cortex stimulation with implantable neurostimulators, a systematic review of the current literature was performed. All assessment methods were designed, approved, and carried out by the Motor Cortex Stimulation with Implantable Neurostimulators Reassessment Subcommittee (hereinafter referred to as the subcommittee) through discussions about the purpose of the assessment. The key questions of the assessment were as follows: · Is motor cortex stimulation with implantable neurostimulators safe and effective for patients with refractory neuropathic pain? · Is motor cortex stimulation with implantable neurostimulators safe and effective for patients with motor disorder? · Is motor cortex stimulation with implantable neurostimulators safe and effective for patients with severe depression, and severe obsessive-compulsive disorder?” Relevant articles were sourced from three overseas and five domestic databases. To minimize the risk of selection bias, two reviewers independently screened the available literature and selected relevant articles based on the designated inclusion and exclusion criteria. After screening, the two reviewers converged on a single conclusion, and in the case of disagreement, a third party was appointed to help reach an agreement. The risk of article section bias was assessed using the RoBANS tool. As quantitative analysis was not possible, data were qualitatively reviewed. Based on the results of the systematic review, the Health Technology Reassessment Committee determined the level of recommendation according to the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) method.

Results A total of 44 articles were selected and analyzed. Of these, 36 reported on patients with refractory neuropathic pain, five examined patients with motor disorders, and three analyzed both these patient populations. No article reported on the use of motor cortex stimulation for severe depression and obsessive-compulsive disorder. The risk of bias was determined to be low.

Safety Side effects following motor cortex stimulation were reported in 42 articles, which reported death, neurological symptoms such as epilepsy, infection, device-related adverse reactions, intracranial hemorrhage or hematoma, and local bleeding. Therefore, the most severe side effects were death and intracranial hemorrhage or hematoma. Death was not directly correlated with the use of motor cortex stimulation, while intracranial hemorrhage or hematoma was. Regardless, the incidence rate of these two adverse reactions was low, and the remainder of the observed side effects or adverse reactions did not pose any serious health risks. Therefore, this technique was determined to afford an acceptable level of safety.

Effectiveness In 24 out of 26 articles on patients with refractory neuropathic pain, the indicators of effectiveness were pain, quality of life (QoL), and variation in drug use. In two articles on patients with motor disorders, the indicators were the level of motor function improvement, QoL, and variation in drug use. Concerning refractory neuropathic pain, the majority of studies reported that 38.9–75% of patients showed a reduction in pain of ≥ 40%, and two out of three studies observed a significant improvement in QoL. In addition, one out of seven studies reported a decline in drug addiction, with three reports indicating that 2.3–61.2% of patients decreased their drug use. The level of evidence in most studies was determined to be very low based on the substantially high risk of bias, inconsistency, and imprecision. Despite this, the subcommittee has determined that the technique seems clinically effective for pain reduction in patients with refractory neuropathic pain. Concerning motor disorders, only two studies reported on the effectiveness of motor cortex stimulation to reduce symptoms. While significant improvements were reported after intervention, the subcommittee determined that the currently available evidence is insufficient. No study reported on motor cortex stimulation in patients with severe depression or severe obsessive-compressive disorder, and thus, the use of this technique in patients with motor disorders still requires further research.

Conclusions and recommendations Based on the level of evidence gathered from this systematic review, the subcommittee suggests the following: Motor cortex stimulation with implantable neurostimulators was determined to be adequately safe as no severe adverse reactions were directly associated with the use of this technique, other than intracranial hemorrhage or hematoma. In addition, the incidence of mild side effects was found to be low. Concerning effectiveness, the technique can successfully reduce pain in patients with refractory neuropathic pain and appears to be clinically useful for patients with motor disorders, but further research on this topic is necessary. However, the effectiveness of motor cortex stimulation remains unclear for patients with severe depression or severe obsessive-compulsive disorder as no relevant study has explored the use of this technique in these patient populations.

The 3rd Health Technology Reassessment Committee of 2021 (Mar 9, 2021) drew the following conclusions based on the subcommittee review: Motor cortex stimulation with implantable neurostimulators is a safe technique that leads to only mild side effects and is recommended for pain reduction in patients with refractory neuropathic pain (GRADE recommendation: I-B, recommended). However, further research needs to be conducted to verify its effectiveness for patients with motor disorders, even though clinical effectiveness is predicted. The 8th Health Technology Reassessment Committee (August 13, 2021) reached the same conclusion (GRADE recommendation: insufficient). Lastly, motor cortex stimulation cannot be presently recommended for patients with depression or obsessive-compulsive disorder because of insufficient evidence despite potential clinical utility (GRADE recommendation: II )
