혈청 간섬유화 검사

Abstract

평가 배경 혈청 간섬유화 검사(serum liver fibrosis test)는 만성 간질환의 징후나 증상을 보이는 환자에서 간섬유화 평가, 예후예측, 치료효과 평가 목적으로 시행되는 혈액검사로, 2013년 신의료기술평가 이후 비급여(보건복지부 고시 제2014-195호(2014.10.31.))로 고시된 기술이다. 혈청 간섬유화 검사는 2013년 신의료기술평가 당시 만성 간질환의 징후나 증상을 보이는 환자를 대상으로 간섬유화 평가, 예후예측, 치료효과 평가 목적으로 사용 시 침습적인 간 생검에 보조적으로 도움을 줄 수 있는 안전성 및 유효성이 있는 검사(권고등급 C)로 심의된 바 있다. 2013년 신의료기술평가 이후 동 기술관련 안전성 및 유효성에 대한 의료기술재평가의 필요성을 확인하여 내부모니터링을 통해 재평가 주제로 발굴되었다. 따라서 동 기술은 2020년 제1차 의료기술재평가위원회(2020.01.10.)에서 재평가 주제 우선순위 심의를 통해 수행과제로 선정되었으며, 보건의료자원의 효율적 사용을 위한 근거를 제공하고자 재평가를 수행하였다. 총6인(소화기내과(간 전공) 2인, 진단검사의학과 2인, 영상의학과 1인, 근거기반의학 1인)으로 구성된 재평가 소위원회(이하 ‘소위원회’라 한다)’는 2020년 8월부터 2021년 1월까지 약 5개월간 총 4회에 걸쳐 회의를 진행하였다. 이후 2021년 제3차 의료기술재평가위원회(2021.03.12.)에서 동 기술에 대한 평가결과 및 권고등급에 대한 최종 심의가 이루어졌다.

평가 방법 본 평가는 만성 간질환 의심 및 확진 환자에서 간섬유화 평가, 예후예측 평가를 위해 혈청 간섬유화 검사가 안전하고 유효한 기술인지 여부를 업데이트 체계적 문헌고찰을 통해 평가하였다. 문헌검색은 국외(3개) 및 국내(5개) 데이터베이스를 통해 검색하였다. 문헌선정은 문헌선택 및 배제기준에 따라 두 명의 검토자가 독립적으로 진행하였고, 최종 선택문헌은 소위원회 심의를 거쳐 확정하였다. 문헌의 비뚤림위험 평가는 QUADAS-2를 이용하여 두 명의 검토자가 독립적으로 수행하여 의견 합의를 이루었다. 자료분석은 양적분석(quantitative analysis)이 가능하여 메타분석을 수행하였다. 권고등급은 소위원회에서 논의된 평가결과를 토대로 의료기술재평가위원회에서 결정하였다.

평가 결과 업데이트 체계적 문헌고찰 수행결과 혈청 간섬유화 검사의 안전성과 유효성에 대해 보고한 문헌 총 44편 (2013년 기 신의료기술평가 선택문헌 16편 포함)이 최종 선정되었다.

안전성 혈청 간섬유화 검사의 안전성은 검사관련 합병증, 검사결과의 위음성으로 인한 치료지연, 위양성으로 인한 과잉진단으로 평가하였다. 혈청 간섬유화 검사는 혈액기반 검사로 검체 채취를 위한 채혈은 체외에서 이루어지는 행위로 검사관련 안전성에는 문제가 없고, 이와 관련하여 합병증을 보고한 문헌도 없어 안전한 기술로 평가하였다. 검사결과와 관련하여 치료지연 또는 과잉진단 결과를 보고한 문헌은 없었다.

유효성 혈청 간섬유화 검사의 유효성은 ⅰ) 진단정확도(간섬유화 평가 및 예후예측), ⅱ) 의료결과에의 영향(간 조직검사 감소에 따른 관련 부작용 감소, 삶의 질 개선, 생존기간 연장)으로 평가하였다. 동 기술의 고시항목 상세 내용에 따르면 실시목적에 치료효과 평가가 명시되어 있지만 실제적으로 간 섬유화 평가 내에 항 바이러스제의 치료효과도 포함되기 때문에 별도의 구분 없이 간섬유화 평가에 대한 진단정확도에 포함하여 평가하기로 하였다. 혈청 간섬유화 검사의 간섬유화 평가에 대한 진단정확도는 총 25편에서 보고되었다. 혈청 간섬유화 검사의 AUC는 유의한 간 섬유화(F≥2)의 경우 0.61, 진행된 간 섬유화(F≥3)는 0.88, 간경화(F=4)는 0.85로 유의한 간섬유화 대비 진행된 간섬유화 및 간경화에서 높게 나타났다. 혈청 간섬유화 검사와 비교하여 간섬유화 스캔(FibroScan) 검사의 진단정확도를 제시한 문헌은 총 17편이었다. FibroScan 검사의 AUC는 유의한 간 섬유화 (F≥2)의 경우 0.86, 진행된 간 섬유화(F≥3)는 0.91, 간경화 (F=4)는 0.93으로 F≥2 대비 F≥3, F=4에서 조금 높게 나타났다. 혈청 간섬유화 검사의 예후예측에 대해 보고한 문헌은 총 9편 이었고, 예후예측에 대한 추적관찰 기간은 2.7~11.2년 이었다. 예후예측에 대한 결과를 보고한 문헌 가운데 6편에서 혈청 간섬유화 검사 결과의 수치가 높은 군과 낮은 군 간에 유의한 차이를 나타냈으며, 혈청 간섬유화 검사 수치가 높은 진행된 섬유화가 있는 환자군(F4)에서 간 관련 문제발생(간 세포암, 식도 정맥류, 복수 등) 및 사망 발생으로 불량한 예후가 관찰되었다고 보고하였다. 또한 추적관찰기간 10년과 20년 시점에서 각각 무사고 생존율(event-free survival rate)예측을 보고한 문헌이 1편 이었으며, 혈청 간섬유화 검사 점수가 높을수록 생존율은 낮았다. 혈청 간섬유화 검사의 의료결과에의 영향은 간 조직검사 감소에 따른 관련 부작용 감소, 삶의 질 개선, 생존기간 연장으로 평가하였다. 선택된 문헌 가운데 간 조직검사 감소에 따른 관련 부작용 감소를 보고한 문헌은 0편이었고, 5편에서 동 검사를 통해 간 조직검사를 48-86% 감소시킬 수 있을 것으로 계산하였다. 삶의 질 개선, 생존기간 연장 등을 보고한 문헌은 0편이었다. 재평가 소위원회에서는 만성 간질환 의심 및 확진환자를 대상으로 혈청 간섬유화 검사의 수행은 간 섬유화 평가에 대한 진단정확도 및 간 조직검사률 감소와 관련하여 의료결과에의 영향이 수용 가능한 수준으로 임상적으로 유용한 검사라는 의견이었다.

경제성 선택 문헌 중 경제적 영향력에 대한 관련결과를 보고한 문헌 1편(Soto et al. 2017)이였다. Soto et al.(2017)은 C형간염 환자에서 연간 순차적 혈청 간섬유화 검사 + 간 경직도 측정(liver stiffness measurement)을 위한 영상검사 vs 연간 순차적 혈청 간섬유화 단독검사로 인구 100명당 각각 12.9명, 13.3명의 간섬유화 관련 사망을 막을 수 있다고 보고하였다. 점진적 비용효과비(Incremental cost-effectiveness ratios, ICER)는 질보정수명(Quality-adjuster life year, QALY) 당 각각 €13,400 및 €11,500 였다고 보고하였다. 알코올성 간질환 환자에서는 간섬유화 관련 사망은 순차적 혈청 간섬유화 검사+간 경직도 측정 영상검사 대비 혈청 간섬유화 단독 검사에서 인구 100명당 각각 11.7명, 22.1명 감소하였고, 점진적 비용효과비(ICER)는 질보정수명(QALY) 당 각각 €280 및 €190 였다고 보고하였다.

결론 및 제언 혈청 간섬유화 검사 소위원회는 현재 문헌에 근거하여 다음과 같이 제언하였다. 혈청 간섬유화 검사는 침습적인 간 조직검사 대비 접근이 용이하고 간단한 검사로, 만성 간질환 의심 및 확진환자를 대상으로 간섬유화의 전체 진행단계(유의한 간섬유화(F≥2) 진행된 간섬유화(F≥3), 간경화(F=4))에 있어 섬유화의 진행정도 및 예후예측 평가에 있어 도움을 줄 수 있는 안전하고 유효한 검사라는 의견이었다. 2021년 제3차 의료기술재평가위원회(2021.03.12.)에서는 소위원회 검토결과에 근거하여 혈청 간섬유화 검사는 혈액기반 검사로 혈청표본 추출을 위한 채혈은 체외에서 이루어지는 행위로 검사관련 안전성에는 문제가 없고, 위음성으로 인한 치료지연과 위양성으로 인한 과잉 진단 등을 보고한 문헌도 없어 안전한 기술로 평가하였다. 또한 동 검사는 침습적인 간 조직검사 대비 접근이 용이하고 간단한 검사로, 만성 간질환 의심 및 확진환자를 대상으로 모든 간섬유화 진행 단계(유의한 간섬유화, 진행된 간섬유화, 간경화)에 있어 섬유화 진행정도 및 예후예측 평가 등의 목적으로 사용 시 간섬유화 평가에 있어 도움을 줄 수 있는 안전하고 유효한 의료기술로 판단하여 권고함(권고등급 I-b)으로 심의하였다.

Background Serum liver fibrosis test is a blood test to assess liver fibrosis, expect prognosis and assess treatment effect in patient with signs or symptoms of chronic liver diseases. This test is a medical technology publicly announced (a Ministry of Health and Administration Announcement #2014-195 (Oct. 31, 2014) as a non-payment item after the New Medical Technology Assessment in 2013.

Serum liver fibrosis test was assessed as safe and effective by assisting invasive liver biopsy when used to assess liver fibrosis, expect prognosis and assess treatment effect through New Medical Technology Assessment in 2013

As a result of internal monitoring, it was decided that the technology - serum liver fibrosis test - be re-assessed for safety and effectiveness. Therefore, the technology was brought to the 1st Health Technology Reassessment Committee in 2020 for re-assessment, in an attempt to find the ground of efficient use of health and medical resources.

Comprised of 6 different sub-committee members (2 gastroenterologists (specialized in liver diseases), 2 laboratory doctors, 1 radiologist, 1 evidence-based medicine specialist), the sub-committee for re-assessment of the technology (the “sub-committee”) held four different review meetings starting August 2020 and ended January 2021. Results of such re-assessment and the final recommendation grade was determined in the 3rd Health Technology Reassessment Committee in 2021 (Mar. 12, 2021).

Methodology Systematic review was performed to re-assess the clinical safety and effectiveness of the technology - serum liver fibrosis test - in assessment of liver fibrosis and expectation of prognosis in patient with suspected chronic liver disease or confirmed patient with chronic liver disease.

Three different international databases and five different domestic databases were used to select studies. Selection of studies based on the inclusion and exclusion criteria, and performed by two reviewers independently. The selected studies were screened by the sub-committee for confirmation.

These two separate reviewers reached a consensus in that they perform risk of bias assessment on a separate basis, using QUADAS-2. The collected data were meta-analyzed since they were adequate for quantitative analysis. The Health Technology Reassessment Committee determined recommendation grade for the technology based on the discussions underwent by the sub-committee.

Result A total of 44 studies (including 16 studies from the previous New Medical Technology Assessment in 2013) were systematically reviewed for assessment of safety and effectiveness of serum liver fibrosis test.

Safety Safety of serum liver fibrosis test was assessed in terms of test-related complication, delay in treatment due to false negative result, and over-diagnosis due to false positive result.

The technology is a blood test where sampling of specimen out of the sampled blood is conducted in in vitro settings. Neither test-related safety nor safety-related complication was reported. Therefore, the technology was assessed safe. None of the selected studies reported delayed treatment or over-diagnosis in relation to the test results.

Efficacy Efficacy of this technology was assessed in terms of i) diagnostic accuracy (assessment of liver fibrosis and expectation of prognosis) and ii) impact on medical results (decrease in side effects related to decreased liver biopsy needs, improvement in quality of life, extended survival). The purpose of the technology includes, under the Public Announcement, assessment of treatment effect. With liver fibrosis assessment covers assessment of therapeutic effect of anti-virus agent, this study replaces assessment of treatment effect with diagnostic accuracy.

Out of all selected studies, 25 reported diagnostic accuracy on liver fibrosis assessment in the technology. AUC for the technology, varying by stage of liver fibrosis, was 0.61 for statistically significant liver fibrosis (F≥2), 0.88 for progressed liver fibrosis (F≥3) and 0.85 for liver cirrhosis (F=4). Therefore, it can be said that AUC was higher in cases with progressed liver fibrosis or liver cirrhosis.

Out of all selected studies, 17 reported diagnostic accuracy of liver fibrosis scan (FibroScan) compared to the technology. AUC for FibroScan, also varying by stage of liver fibrosis, was 0.86 for statistically significant liver fibrosis (F≥2), 0.91 for progressed liver fibrosis (F≥3) and 0.93 for liver cirrhosis (F=4). Therefore, it can also be said that AUC was slightly higher in cases with progressed liver fibrosis or liver cirrhosis.

A total of 9 studies reported the prognosis result of serum liver fibrosis, and the average follow-up period was 2.7-11.2 years. Out of those 9 studies, 6 implied statistically significant inter-Group difference between High Serum Liver Fibrosis Result Group and Low Serum Liver Fibrosis Result Group. It was also reported in those 6 studies that bad prognosis such as liver-related problems (hepatocellular carcinoma, esophageal varix, ascites, etc.) in patients with liver cirrhosis (F=4), who had high serum liver fibrosis result.

There was only 1 study reporting event-free survival rate at Year 10 and Year 20 on the follow-up, where survival rate was lower when serum liver fibrosis result was high.

Impact of the technology on medical result was assessed by decrease in side effects related to decreased liver biopsy needs, improvement in quality of life and extended survival. None of the selected studies reported decrease in side effects related to decrease of liver biopsy needs. Out of all selected studies, 5 reported decrease in liver biopsy needs by 48% to 86%. None of the selected studies reported quality of life and extended survival.

With the above being said, the sub-committee decided that use of the technology - serum liver fibrosis test - in patient with suspected chronic liver disease or confirmed patient with chronic liver disease - has clinical benefit since its impact on medical results in relation to diagnostic accuracy and decrease in liver biopsy needs is tolerable.

Economic Feasibility Only 1 study (Soto et al. 2017) reported the technology's economic impact, where use of annual serum liver fibrosis test in combination with radiographic liver stiffness measurement and annual serum liver fibrosis test without any combination was analyzed to prevent the average of 12.9 deaths and 13.3 deaths per 100 population from taking place in patient with hepatitis C and incremental cost-effectiveness ratios(ICER) per quality-adjuster life year(QALY) were €13,400 and €11,500, respectively. In patient with alcoholic liver diseases, use of annual serum liver fibrosis test in combination with radiographic liver stiffness measurement and annual serum liver fibrosis test without any combination was analyzed to prevent the average of 11.7 deaths and 22.1 and ICER per QALY were €280 and €190, respectively.

Conclusion and Suggestion The sub-committee suggests, based on the selected studies, as follows:

Serum liver fibrosis test is a safe and effective invasive means of assisting liver biopsy when used to assess liver fibrosis, expect prognosis and assess treatment effect in patient with suspected chronic liver disease or confirmed patient with chronic liver disease, regardless of stage of liver fibrosis (statistically significant liver fibrosis (F≥2), progressed liver fibrosis (F≥3), liver cirrhosis (F=4)).

In the 3rd Health Technology Reassessment Committee in 2021 (Mar. 12, 2021), the technology as a blood test where sampling of specimen out of the sampled blood is conducted in in vitro settings was determined safe with no test-related safety affected and none of studies reporting delay in treatment due to false negative results or over-diagnosis due to false positive results. Appreciating the technology’s greater accessibility compared to any invasive means of liver biopsy, the Health Technology Reassessment Committee recommended (Recommendation Grade I-b) use of the technology for assessment of progression of fibrosis and expectation of prognosis in patient with suspected chronic liver disease or confirmed patient with chronic liver disease, regardless of stage of liver fibrosis (statistically significant liver fibrosis, progressed liver fibrosis, liver cirrhosis).
