삼차신경자극술 [체내삽입형 신경자극기이용]

Abstract

평가 배경 삼차신경자극술[체내삽입형 신경자극기이용]은 만성 난치성 삼차신경병증성 통증 환자를 대상으로 삼차신경부위에 전극을 삽입하여 전기적 자극을 통해 통증을 개선하기 위한 기술이다. 동 기술은 2009년 신의료기술평가 시 안전성 및 유효성이 있는 기술로 고시된 후 2010년 비급여로 등재된 기술이다(보건복지부 고시 제2010-4호, 2010.3.29.). 동 기술은 2020년 의료기술재평가사업의 내부모니터링을 통해 재평가 주제로 발굴되었으며, 2020년 제6차 의료기술재평가위원회(2020.6.12.~2020.6.19.)에서 안전성 및 유효성에 대해 총 7인(신경과 2인, 신경외과 2인 , 마취통증의학과, 재활의학과, 근거기반의학)으로 구성된 소위원회에서 체계적 문헌고찰을 통해 평가하도록 심의되었고 2020년 제12차 의료기술재평가위원회(2020.12.11.)에서 최종 심의하였다.

평가 방법 본 평가는 만성 난치성 삼차신경병증성 통증 환자를 대상으로 삼차신경자극술[체내삽입형 신경자극기이용]이 임상적으로 안전하고 유효한지 평가하기 위해 체계적 문헌고찰을 수행하였다. 체계적 문헌고찰은 위의 핵심질문을 토대로 국내 데이터베이스 5개(KoreaMed, 의학논문데이터베이스, 학술데이터베이스, 한국교육학술정보원, 과학기술정보통합서비스) 및 국외 데이터베이스 3개(Ovid MEDLINE, Ovid EMBASE, Cochrane Central Register of Controlled Trials)를 이용하여 문헌 검색하였으며, 문헌 선정/배제기준 적용 및 자료추출, 비뚤림 위험평가는 모두 2명의 평가자가 독립적으로 수행하였다. 비뚤림위험 평가는 연구유형에 따라 무작위연구는 Cochrane의 Risk of Bias, 그 외에 비무작위연구는 Risk of bias for nonrandomized studies (RoBANS) 2.0 국문판을 사용하였고, 문헌의 근거수준을 평가하기 위해 Grading of Recommendations Assessment, Development and Evaluation (GRADE)를 사용하였다. 소위원회의 검토의견을 고려하여 의료기술재평가위원회에서 최종 심의 후 권고등급을 제시하였다. 본 평가에서는 안전성의 경우 동 기술의 부작용 및 이상반응을 모두 확인하기 위해 연구대상자 수에 제한 없이 증례연구 및 증례보고까지 포함하여 평가하였으며, 유효성의 경우 연구결과의 일반화 가능성을 높이기 위해 영구적으로 삼차신경자극술을 받은 대상자가 10명 이상인 연구만 포함하여 평가하였다. 다만 전체 연구대상자가 9명 이하의 연구 중에 안전성에 대해 언급이 없는 연구는 적절하지 않은 연구결과로 배제하였다.

평가 결과 평가에 선택된 문헌은 총 24편으로 전후연구 11편, 증례연구 및 증례보고 15편이 포함되었다. 선택문헌에 포함된 대상자는 외상, 수술, 감염, 등의 다양한 원인으로 기인된 삼차신경병증성 통증 환자가 포함되었으며, 모두 기존 치료에 반응하지 않는 만성, 난치성 통증 환자로 안와신경 및 하악신경 등의 삼차신경 부위에 전극을 삽입한 환자들이었다. 안전성 평가 결과, 총 24편의 문헌에서 감염(9편, 0-20.6%), 상처(8편, 4.3-25.0%), 장치 관련 부작용(13편, 2.9-50.0%), 신경학적 증상(11편, 4.8-33.3%) 등이 확인되었다. 이 중 감염은 대부분의 경우 장치 제거 후, 항생제 치료 후 재이식을 수행하였다. 상처에서도 대부분의 경우 장치 제거 후 재수술 또는 재이식을 수행하였다. 장치 관련 부작용은 대부분 전극의 기능 부전이나 기계적 결함, 전극 위치 이탈 등으로 장치 교체나 재수술을 수행한 것으로 보고되었으며, 신경학적 증상 관련해서 눈꺼풀처짐, 지각이상, 둔한 감각, 두통, 현훈 및 이명을 동반한 전정계증후군, 일시적 안면마비, 경미한 이질통 등이 확인되었다. 유효성 평가는 총 11편의 전후연구 문헌으로 평가하였으며, 대부분 문헌에서 통증이 개선되고 약물사용량이 감소한 것으로 보고되었다. 환자만족도는 2편에서 중등도 이상 만족한 환자 비율이 70% 이상으로 보고되었으며, 삶의 질은 1편에서 24개월 추적 관찰한 결과 일상생활 및 사회활동에서는 유의한 개선을 보였으나, 휴식필요도 및 수면의 질에 대해서는 유의한 개선을 나타내지 못했다. 고용변화는 1편에서 시술 전에 8명 중 6명이 고용된 상태에서 시술 후 고용된 상태가 7명으로 변화된 것으로 보고되었다. 근거수준(GRADE 평가)은 일부 문헌에서 ‘교란변수’, ‘결과 평가’, ‘불완전한 결과자료’ 에 대해 비뚤림 위험이 높게 평가된 것을 고려하였으며, ‘Very Low’로 평가하였다. 이에 소위원회에서는 안전성 검토 결과, 신경학적 문제는 경미한 수준으로 임상적으로 문제가 되지 않을 것으로 판단하나 감염 및 장치 관련 문제들은 비교적 높은 수준으로 확인된다는 의견이었다. 이에 따라 시술 수행에 따른 감염 및 장치 관련 문제 발생 가능성을 고려하여 상당한 주의와 감독 하에 수행하고 시술 후 적절한 감염관리가 필요하다고 판단하였다. 유효성 검토 결과, 동 기술이 기존의 약물치료나 기존 시술로 통증 조절이 안 되는 만성 난치성 삼차신경병증성 통증 환자를 대상으로 통증 개선 및 약물사용 감소에 도움을 줄 수 있어, 시술 수행에 따른 감염 및 장치 관련문제 발생 가능성을 고려하더라도 환자에게 가져다줄 수 있는 이득이 크기 때문에 임상에 필요한 기술로 판단하였다. 다만 그 근거수준이 제한적임을 고려하여 잘 설계된 연구를 통한 근거축적은 지속적으로 더 필요하다는 의견이 있었다.

결론 소위원회는 현재 평가 결과에 근거하여 다음과 같이 제언하였다.

삼차신경자극술[체내삽입형 신경자극기이용]은 기존의 약물치료나 기존 시술로 통증 조절이 안 되는 만성 난치성 삼차신경병증성 통증 환자를 대상으로 통증 개선을 목적으로 수행 시, 통증 개선 및 약물사용 감소에 도움을 줄 수 있어 시술 수행에 따른 감염 및 장치 관련 문제 발생 가능성을 고려하더라도 환자에게 가져다줄 수 있는 이득이 크기 때문에 안전성 및 유효성 있는 기술로 판단하였다. 다만 그 근거수준이 제한적임을 고려하여 잘 설계된 연구를 통한 근거축적은 지속적으로 더 필요하다는 의견이 있었다. 의료기술재평가위원회는 소위원회 검토 결과에 근거하여 다음과 같이 심의하였다(2020.12.11.). 삼차신경자극술[체내삽입형 신경자극기이용]은 기존의 약물치료나 기존 시술로 통증 조절이 안 되는 만성 난치성 삼차신경병증성 통증 환자를 대상으로 통증 개선을 목적으로 수행 시, 통증 개선 및 약물사용 감소에 도움을 줄 수 있어 시술 수행에 따른 감염 및 장치 관련문제 발생 가능성을 고려하더라도 환자에게 가져다줄 수 있는 이득이 크기 때문에 안전성 및 유효성 있는 기술로 심의하였다. 의료기술재평가위원회는 기존 치료(약물치료 등)로 통증 조절이 안 되는 만성 난치성 삼차신경병증성 통증 환자를 대상으로 통증 개선을 목적으로 삼차신경자극술[체내삽입형 신경자극기이용]을 사용하는 것에 대해 권고함(권고등급: Ib)로 심의하였다.

Background of Assessment Trigeminal nerve stimulation [with implantable pulse generators] is a technology to improve pain through electrical stimulation by inserting electrodes into the trigeminal nerve area in patients with chronic intractable trigeminal neuropathy pain. This technology was announced as a technology with safety and effectiveness in the new health technology assessment in 2009, and then was registered as a non-benefit service in 2010 (Ministry of Health and Welfare Notice No. 2010-4, 2010.3.29.). This technology was selected as a reassessment agenda through internal monitoring of the Health Technology Reassessment Project in 2020. At the 6th Health Technology Reassessment Committee in 2020 (June 12, 2020 - June 19, 2020), a subcommittee consisting of a total of 7 members (2 from the department of neurology, 2 from the department of neurosurgery, the department of anesthesiology and pain medicine, the department of rehabilitation medicine, the department of evidence-based medicine) on safety and effectiveness was deliberated to evaluate through systematic literature review. The final deliberation was made at the 12th Health Technology Reassessment Committee (2020.12.11.) in 2020.

Assessment Method In this evaluation, a systematic literature review was performed to assess whether trigeminal nerve stimulation [with implantable pulse generators] is clinically safe and effective for patients with chronic intractable trigeminal neuropathic pain. For the systematic literature review, the literature search was conducted in 5 domestic databases (KoreaMed, Korean Medical database, Academic Database, Korea Education and Research Information Service (KERIS), National Digital Science Links (NDSL)) and 3 foreign databases (Ovid MEDLINE, Ovid EMBASE, Cochrane Central Register of Controlled Trials) based on the above key questions. The application of the literature inclusion and exclusion criteria, data extraction, and risk of bias evaluation were all independently performed by two evaluators. For the risk of bias assessment, Cochrane's Risk of Bias was used for randomized studies, and the Risk of bias for nonrandomized studies (RoBANS) 2.0 Korean version was used for other non-randomized studies, depending on the study type. To evaluate the level of evidence in the literature, Grading of Recommendations Assessment, Development and Evaluation (GRADE) was used. In consideration of the subcommittee's review opinion, the Health Technology Reassessment Committee presented the recommendation grade after final deliberation. In this assessment, for safety, case studies and case reports were evaluated without limiting the number of study subjects in order to check all side effects and adverse events of the technology. For effectiveness, only studies with 10 or more subjects who had undergone permanent trigeminal nerve stimulation were included to increase the generalizability of the study results. However, studies in which safety was not mentioned among studies with 9 or fewer study subjects were excluded from the evaluation as the results of the study were inappropriate.

Assessment Results A total of 24 studies were selected for evaluation, including 11 before-and-after studies and 15 case studies and case reports. Subjects included in the selected literature were patients with trigeminal neuropathic pain caused by various causes such as trauma, surgery, infection, etc. All of them were patients with chronic, intractable pain who did not respond to existing treatment and had electrodes inserted into the trigeminal nerve, such as the orbital nerve and mandibular nerve. As a result of safety evaluation, infections (9 studies, 0-20.6%), wounds (8 studies, 4.3-25.0%), device-related side effects (13 studies, 2.9-50.0%), and neurological symptoms (11 studies, 4.8-33.3%) were confirmed in a total of 24 studies. Of these, in most cases of infection, re-implantation was performed after device removal and antibiotic treatment. Also, in most cases of wounds, reoperation or re-implantation was performed after removal of the device. It was reported that most of the device-related side effects are malfunctioning of the electrode, mechanical defects, and deviation of the electrode position, and the device replacement or reoperation has been conducted. In relation to neurological symptoms, ptosis, paresthesia, dull sensation, headache, temporary facial palsy, mild allodynia, and vertigo and tinnitus were confirmed. Effectiveness assessment was evaluated using a total of 11 before-and-after studies, and most of the studies reported improved pain and reduced drug use. As for patient satisfaction, the proportion of patients who were moderately or more satisfied was reported to be 70% or more in 2 studies, and the quality of life was significantly improved in daily life and social activities as a result of 24 months of follow-up in 1 study. However, there was no significant improvement in the need for rest and sleep quality. As for employment change, it was reported in 1 study that 6 out of 8 patients were employed before the procedure, and the employment status changed to 7 people. As for the level of evidence (GRADE evaluation), the high risk of bias was considered for 'confounding ', 'measurement of the outcome, and ‘missing data' in some literature, and it was evaluated as 'Very Low'. As a result of the safety review, the subcommittee was of the opinion that neurological problems were not considered to be a clinical problem at a minor level, but infection and device-related problems were confirmed at a relatively high level. Accordingly, it was determined that the procedure was performed under considerable caution and supervision in consideration of the possibility of infection and device-related problems resulting from the procedure, and that appropriate infection control was necessary after the procedure. As a result of the effectiveness review, this technology can help improve pain and reduce drug use for patients with chronic intractable trigeminal neuropathic pain whose pain cannot be controlled with conventional drug treatment or existing procedures. Therefore, even considering the possibility of device-related problems, it was judged as a technology necessary for clinical practice because the benefit it can bring to the patient is large. However, considering the limited level of evidence, it is necessary to continuously accumulate more evidence through well-designed studies.

Conclusion The subcommittee made the following recommendations based on the current evaluation results. Trigeminal nerve stimulation [with implantable pulse generators] helps to improve pain and reduce drug use when performed for pain relief in patients with chronic intractable trigeminal neuropathic pain whose pain cannot be controlled by conventional medications or existing procedures. It was judged to be a safe and effective technology because the benefit it can bring to the patient is large even if the possibility of infection and device-related problems arising from the procedure is considered. However, considering the limited level of evidence, there was an opinion that it is necessary to continuously accumulate more evidence through well-designed studies. The Health Technology Reassessment Committee deliberated as follows based on the results of the subcommittee's review (December 11, 2020). Trigeminal nerve stimulation [with implantable pulse generators] was deliberated as a safe and effective technology because it helps to improve pain and reduce drug use when it is performed for pain relief in patients with chronic intractable trigeminal neuropathic pain whose pain cannot be controlled by conventional medications or existing procedures and has a large benefit to the patient even when considering the possibility of infection and device-related problems arising from the procedure. The Health Technology Reassessment Committee considered the use of trigeminal nerve stimulation [with implantable pulse generators] as a 'recommended' (recommendation grade: 1b) for pain relief in patients with chronic intractable trigeminal neuropathic pain whose pain could not be controlled with conventional treatment.
