뇌졸중 환자에서 하지재활로봇을 이용한 보행치료

Abstract

평가 배경 하지재활로봇을 이용한 보행치료(Robot-Assisted gait training)는 건강보험에 등재 기술인 ‘보행치료(사130, 수가코드 MM302)’로 분류되어 의료현장에서 사용되고 있다. 하지재활로봇을 이용한 보행치료가 고식적 재활치료에 비해 치료효과 개선 등을 근거로 행위 재분류 및 하지재활로봇과 관련한 건강보험 수가인상에 대한 제조업체 및 의료계의 요구가 있다. 따라서 건강보험심사평가원은 기존 고식적 재활치료 대비 하지재활로봇을 이용한 보행치료의 임상적 유용성의 개선여부 및 경제적 가치(비용효과성)가 있는지 여부에 대한 재평가를 한국보건의료연구원에 의뢰하여(의료기술등재부-1378, 2020.06.17.), 2020년 제7차 의료기술재평가위원회(2020.07.10.)에서 재평가 계획서 및 소위원회 구성안에 대해 심의를 받아 재평가를 수행하였다. 본 평가의 목적은 뇌졸중 환자에서 하지재활로봇 치료의 임상적 안전성 및 유효성 등에 대한 과학적 근거를 제공하고 이를 통해 의료기술의 적정사용 등 정책적 의사결정을 지원하는 것이다.

평가 방법 뇌졸중 환자에서 하지재활로봇을 이용한 보행치료에 대한 안전성 및 유효성 평가를 위해 체계적 문헌고찰을 수행하고, 체계적 문헌고찰 결과를 바탕으로 경제성 평가를 고려하였다. 모든 평가방법은 평가목적을 고려하여 “하지재활로봇을 이용한 보행치료 재평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회 구성은 재활의학과 3인, 신경과 2인, 정형외과 1인, 신경외과 1인, 소아청소년과 1인, 근거기반의학 2인의 전문가 10인으로 구성하였다. 평가의 핵심질문은 “뇌졸중 환자에서 하지재활로봇을 이용한 보행치료는 고식적 재활치료에 비해 안전하고 효과적인가?”이었고, 안전성은 보고된 이상반응을 결과지표로 하였고, 유효성은 균형기능, 기능 수행능력, 근력, 보행기능(거리, 속도, 걸음 등), 뇌졸중 중증도, 삶의 질 등을 결과지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌선택 및 배제기준에 따라 2명의 평가자가 독립적으로 수행하고, 의견의 불일치가 있는 경우에는 평가자 간 합의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림위험 평가는 Cochrane의 Risk of Bias를 사용하여 평가하였으며, 최종 선택된 문헌을 대상으로 2명의 평가자가 독립적으로 평가를 실시하였으며, 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 모든 자료는 연구단위로 추출하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 GRADE 방법을 이용하여 근거 수준을 평가하고, 평가 결과를 토대로 권고등급을 결정하였다. 결론 도출은 하지재활로봇 중재군 전체에 대해 분석결과 및 결론을 우선적으로 제시하고, 임상적 안전성 및 유효성에 대한 결론은 합의를 통해서 결정하였으며, 만장일치로 합의되지 않아 투표를 진행하였다. 1차 투표에서 동점이 되어 2차 투표를 진행해 최종 결론을 도출하였다. 또한, 체계적 문헌고찰 결과를 바탕으로 비용-효과분석 또는 비용최소화 분석 수행 혹은 조건부가치측정법을 이용한 하지재활로봇을 통한 임상적 가치에 대한 지불의사금액 조사의 수행 가능성에 대해 소위원회에서 논의하였다.

평가 결과 본 평가에 최종 선택된 연구는 총 63개(66편)이었고, 무작위배정임상시험연구(Randomized Controlled Trial, 이하 RCT)가 52개, 비무작위배정임상시험연구(Non-Randomized Controlled Trial, 이하 NRCT)가 11개였다. RCT연구는 총 2,112명이었고, 하지재활로봇 단독중재군과 고식적 재활치료를 보고한 연구는 20개였고, 하지재활로봇 및 고식적 재활치료를 병용한 중재군과 고식적 재활치료를 비교한 연구는 32개였다. NRCT 연구는 총 381명이었고, 하지재활로봇 단독중재군과 고식적 재활치료를 보고한 연구는 2개였고, 하지재활로봇 및 고식적 재활치료를 병용한 중재군과 고식적 재활치료를 비교한 연구는 9개였다. 비뚤림위험 수준은 불충분한 결과자료에서 ‘높음’이 21.2~27.3%로 비뚤림위험이 높았다.

안전성 RCT 연구에서 이상반응을 보고한 연구는 20개였으며, 이 중 18개 연구는 이상반응 또는 중재와 관련된 이상반응이 없었다고 보고하였고, 2편에서 피부이상, 낙상, 발목관절 변형, 심부정맥혈전증, 사망 등을 보고하였다. 특히, 사망은 고식적 재활치료군에서 1명에서 발생하였으며, 중재와의 관련성 등은 언급이 없었다. NRCT 연구에서 이상반응을 보고한 연구는 4개였으며, 3개 연구에서는 이상반응이 없다고만 보고하였고, 1개 연구에서는 심각한 부작용은 없고, 통증이 1명에서 발생하였다고 보고하였다.

유효성 임상적 유효성 결과지표는 점수 기준 균형기능[버그 균형척도(Berg Balance Scale, 이하 BBS), Rivermead 이동성 지수(Rivermead Mobility Index, 이하 RMI)], 시간 기준 균형기능[Timed Up & Go 검사(이하 TUG), 과제 지향적 보행 평가(modified Emory Functional Ambulation Profile, 이하 mEFAP)], 기능 수행 능력[기능적 보행 지수(Functional Ambulation Category, 이하 FAC), 바델지수(Modified Barthel Index, 이하 BI), 일상생활 동작 평가(Functional Independence Measure, 이하 FIM)], 근력[푸글-마이어 하지평가(Fugl Meyer Lower/Extremity Motor function Assessment, 이하 FMA), 운동기능 검사(Motricity index, 이하 MI)], 거리 기준 보행속도, 걸음수 기준 보행속도, 보행거리, 뇌졸중 중증도(National Institutes of Health Stroke Scale, 이하 NIHSS), 삶의 질 전체 점수, 삶의 질 신체영역 점수 등을 분석하였다. 하지재활로봇을 이용한 보행치료 중재군 전체와 고식적 재활치료를 비교한 메타분석 결과에서는 점수 기준 균형기능(BBS, RMI)과 기능 수행 능력(FAC, BI, FIM)지표에서 하지재활로봇을 이용한 보행치료가 고식적 재활치료에 비해 통계적으로 유의하게 개선효과가 있었다(균형기능(BBS, RMI): 29편, 표준평균차이(Standardized Mean Difference, 이하 SMD) 0.37, 95% CI 0.11~0.63, I2=77.4%; 기능 수행 능력(FAC, BI, FIM): 26편, SMD 0.25, 95% CI 0.01~0.48, I2=72.7%). 다만, 이질성이 높아 해석에 주의가 필요하다. 이 외 시간 기준 균형기능(TUG, mEFAP)(15편, SMD –0.18, 95% CI –0.49~0.12, I2=57.0%), 근력(FMA, MI)(19편, SMD 0.10, 95% CI –0.11~0.30, I2=48.2%), 거리 기준 보행속도(32편, 가중평균차이(Weighted Mean Difference, 이하 WMD) 0.01 m/sec, 95% CI –0.05~0.07, I2=50.4%), 걸음수 기준 보행속도(8편, WMD –8.97 steps/min, 95% CI -26.81~8.86, I2=90.5%), 보행거리(17편, WMD 1.83 m, 95% CI –7.36~11.01, I2=11.9%), 뇌졸중 중증도(NIHSS)(3편, WMD –1.39점, 95% CI –4.04~1.26, I2=90.8%), 삶의 질 전체 점수(6편, SMD 0.07, 95% CI –0.19~0.34, I2=13.5%), 삶의 질 신체영역 점수(3편, WMD –0.97점, 95% CI –5.27~3.34, I2=0.0%)는 하지재활로봇을 이용한 보행치료군과 고식적 재활치료군 간 차이가 없었다. 하지재활로봇 단독중재군과 고식적 재활치료를 비교한 메타분석 결과에서는 시간 기준 균형기능(TUG, mEFAP)(9편, SMD -0.22, 95% CI –0.56~0.12, I2=42.4%), 기능 수행 능력(FAC, BI, FIM)(8편, SMD 0.11, 95% CI –0.13~0.35, I2=6.1%), 근력(FMA, MI)(9편, SMD 0.19, 95% CI –0.16~0.54, I2=50.8%), 거리 기준 보행속도(16편, WMD –0.03 m/sec, 95% CI -0.11~0.05, I2=57.1%), 걸음수 기준 보행속도(6편, WMD –13.62 steps/min, 95% CI -36.24~9.01, I2=93.0%), 보행거리(10편, WMD –2.96m, 95% CI -13.74~7.82, I2=3.9%), 삶의 질 전체 점수(3편, SMD 0.17, 95% CI –0.18~0.51, I2=0.0%), 삶의 질 신체영역 점수(2편, WMD –1.34점, 95% CI –5.74 ~3.06, I2=0.0%)는 하지재활로봇 중재군과 고식적 재활치료군 간 차이가 없었다. 또한, 점수 기준 균형기능(BBS, RMI)(11편, SMD 0.21, 95% CI –0.19~0.60, I2=72.0%), 뇌졸중 중증도(NIHSS)(1편, WMD 0.9점, 95% CI –0.08~1.88, I2=NA)는 보고한 문헌이 1편밖에 없어 경향성만을 확인하였다. 하지재활로봇 및 고식적 재활치료 병용 중재군과 고식적 재활치료를 비교한 메타분석 결과에서는 점수 기준 균형기능(BBS, RMI)과 기능 수행 능력(FAC, BI, FIM), 뇌졸중 증증도(NIHSS) 지표에서 하지재활로봇 및 고식적 재활치료가 고식적 재활치료에 비해 통계적으로 유의하게 개선효과가 있었다(균형기능(BBS, RMI): 18편, SMD 0.48, 95% CI 0.13~0.83, I2=80.1%; 기능 수행 능력(FAC, BI, FIM): 18편, SMD 0.32, 95% CI 0.001~0.63, I2=79.4%; 뇌졸중 중증도(NIHSS): 2편, WMD –2.6점, 95% CI –5.14~-0.06, I2=77.9%). 다만, 이질성이 높아 해석에 주의가 필요하다. 이 외 시간 기준 균형기능(TUG, mEFAP)(6편, SMD -0.15, 95% CI –0.75~0.45, I2=72.5%), 근력(FMA, MI)(10편, SMD 0.04, 95% CI –0.23~0.30, I2=50.7%), 거리 기준 보행속도(16편, WMD 0.05 m/sec, 95% CI –0.000~0.10, I2=28.7%), 걸음수 기준 보행속도(2편, WMD 6.67 steps/min, 95% CI –8.42~21.77, I2=0.0%), 보행거리(7편, WMD 14.18 m, 95% CI -3.04~32.01, I2=0.7%), 삶의 질 전체 점수(3편, SMD –0.07, 95% CI-0.50~0.35, I2=39.6%)는 하지재활로봇 및 고식적 재활치료 병용 중재군과 고식적 재활치료군 간 차이가 없었다. 또한, 삶의 질 신체영역 점수(1편, WMD 7.40점, 95% CI –13.40~28.20, I2=NA)는 보고한 문헌이 1편밖에 없어 경향성만을 확인하였다. 또한, 뇌졸중 발생시점 6개월 이내(아급성) 대상자 문헌만을 분석한 결과, 하지재활로봇을 이용한 보행치료는 고식적 재활치료에 비해 점수 기준 균형기능(BBS, RMI), 기능 수행 능력(FAC, BI, FIM), 보행거리 지표에서 통계적으로 유의하게 개선효과가 있었다.

경제성 평가 가능성 논의 결과 체계적 문헌고찰 결과에서 임상적으로 최소 유효값 등을 산출하기 위해 유사지표를 표준 평균 차이(SMD)로 산출하지 않고, 각 결과 지표별로 가중평균차이(WMD)로 합성한 결과, 하지재활로봇 단독 중재와 고식적 재활치료를 비교한 결과지표 중 통계적으로 유의한 지표는 근력(FMA) 지표뿐이었다(WMD 1.69점, 95% CI 0.59~2.79). 하지만, 1.69점이 개선되었다는 것에 대한 임상적 의미를 설명하기가 어렵고, 일반인들에게 설명할 시나리오를 구현하는데 한계가 있으며, 현재 결과로 지불의사금액(Willingness to Pay, 이하 WTP)조사를 수행한다고 해도 결과해석이 안 될 가능성이 높다는데 의견일치를 이뤘다. 또한, 하지재활로봇의 효과를 설명할 수 있는 지표가 불명확하고, 각 결과지표에 대한 임상적으로 중요한 최소한의 차이 기준(Minimal Clinical Important Change, 이하 MCID)도 없어 소위원회에서는 비용-효과분석 및 지불의사금액 조사를 수행하지 않기로 결정하였다.

결론 및 제언 체계적 문헌고찰 결과, 뇌졸중에서 하지재활로봇을 이용한 보행치료 중재군 전체와 고식적 재활치료의 안전성은 대부분의 연구에서 이상반응이 없다고 기술하거나 심각한 부작용 등이 보고되지 않아 안전한 기술로 평가하였다. 하지재활로봇을 이용한 보행치료 중재의 유효성 지표 중 점수 기준 균형기능(BBS, RMI)과 기능 수행 능력(FAC, BI, FIM)지표에서 하지재활로봇을 이용한 보행치료가 고식적 재활치료에 비해 통계적으로 유의하게 개선효과가 있었으나 이질성이 높아 해석에 주의가 필요하다. 이 외 시간 기준 균형기능(TUG, mEFAP), 근력(FMA, MI), 보행속도, 보행거리, 뇌졸중 중증도(NIHSS), 삶의 질 등은 하지재활로봇 중재군과 고식적 재활치료군 간 차이가 없었다. 따라서 소위원회에서는 본 평가결과 뇌졸중 환자에서 하지재활로봇을 이용한 보행치료는 안전한 기술이며, 유효성은 고식적 재활치료와 유사한 기술로 평가하였다(근거의 신뢰수준, Low). 뇌졸중에서 하지재활로봇 단독중재군과 고식적 재활치료의 안전성은 대부분의 연구에서 이상반응이 없다고 기술하거나 심각한 부작용 등이 보고되지 않아 안전한 기술로 평가하였다. 유효성 지표인 시간 기준 균형기능(TUG, mEFAP), 기능 수행 능력(FAC, BI, FIM), 근력(FMA, MI), 보행속도, 보행거리, 삶의 질 등 모든 유효성 결과지표에서 하지재활로봇 단독 중재군과 고식적 재활치료군 간 차이가 없었다. 따라서 소위원회에서는 본 평가결과 뇌졸중 환자에서 하지재활로봇 단독 중재는 안전한 기술이며, 유효성은 고식적 재활치료와 유사한 기술로 평가하였다(근거의 신뢰수준, Low). 뇌졸중에서 하지재활로봇 및 고식적 재활치료 병용중재군의 안전성은 대부분의 연구에서 이상반응이 없다고 기술하거나 심각한 부작용 등이 보고되지 않아 안전한 기술로 평가하였다. 하지재활로봇 및 고식적 재활치료 병용중재의 유효성 지표 중 점수 기준 균형기능(BBS, RMI), 기능 수행 능력(FAC, BI, FIM), 뇌졸중 중증도(NIHSS)는 병용 중재군이 고식적 재활치료에 비해 통계적으로 유의하게 개선효과가 있었으나, 이질성이 높아 결과해석에 주의가 필요하다. 그 외 시간 기준 균형기능(TUG, mEFAP), 보행속도 및 거리, 삶의 질 등은 효과차이가 없었다. 따라서 소위원회에서는 본 평가결과 뇌졸중 환자에서 하지재활로봇 및 고식적 재활치료 병용 중재는 안전한 기술이며, 유효성은 고식적 재활치료와 유사한 기술로 평가하였다(근거의 신뢰수준, Low). 또한, 뇌졸중 발생시점 6개월 이내(아급성) 대상자 문헌만을 분석한 결과, 하지재활로봇을 이용한 보행치료는 고식적 재활치료에 비해 점수 기준 균형기능(BBS, RMI), 기능 수행 능력(FAC, BI, FIM), 보행거리 지표에서 통계적으로 유의하게 개선효과가 있었다. 2021년 제7차 의료기술재평가위원회(2021.07.09.)에서는 소위원회 검토 결과와 관련하여 논의를 하였고, 제8차 의료기술재평가위원회(2021.08.14.)에서 의료기술재평가사업 관리지침 제4조제10항에 의거 “뇌졸중 환자에서 하지재활로봇을 이용한 보행치료”에 대해 다음과 같이 심의하였다. 뇌졸중 환자에서 하지재활로봇을 이용한 보행치료는 안전한 기술이며, 균형기능(BBS, RMI), 기능 수행 능력(FAC, BI, FIM)지표는 기존 기술인 고식적 재활치료에 비해 통계적으로 유의한 개선효과가 있었고, 이 외 시간 기준 균형기능(TUG, mEFAP), 근력(FMA, MI), 보행속도, 보행거리, 뇌졸중 중증도(NIHSS), 삶의 질 등은 하지재활로봇 중재군과 고식적 재활치료군 간 차이가 없다는 것을 근거로 조건부 권고로 심의하였다(권고등급: 조건부 권고함).

Robot-assisted gait training for stroke patients

Background Robot-assisted gait training (RAGT) rehabilitation falls within the category of “Gait Training (Sa130, charge code MM302)”, and as such, is covered by the South Korean national health insurance program. This technology is commonly utilized in the field of physiotherapy, and it has been shown to lead to better clinical outcomes than palliative rehabilitation. Based on this, manufacturers of the RAGT technology, as well as the healthcare industry, have demanded a rise in the national health insurance charge for this type of rehabilitation. The Health Insurance Review & Assessment Service has thus requested the National Evidence-Based Healthcare Collaborating Agency to assess whether RAGT is more cost-efficient and clinically effective than conventional palliative rehabilitation (Health Technology Registrar No. 1378; June 17, 2020). Therefore, a reassessment plan was approved by the 7th Health Technology Reassessment Committee in 2020 (July 10, 2020) and the relevant subcommittee was designated. The purpose of the assessment was to examine the current scientific evidence for the clinical safety and effectiveness of RAGT-based rehabilitation for stroke patients, with the intent to support policy making concerning the appropriate uses of this technology.

Methods To assess the safety and effectiveness of RAGT-based rehabilitation for stroke patients, a systematic review of the current literature was performed. All assessment methods were designed, approved, and carried out by the RAGT Reassessment Subcommittee (hereinafter referred to as the subcommittee) through discussions about the purpose of the assessment. The subcommittee consisted of ten experts: three rehabilitation medicine specialists, two neurologists, one orthopedist, one neurosurgeon, one pediatrician, and two specialists in the field of evidence-based medicine. The key question of the assessment was the following: “is RAGT safer and more effective than palliative care in the rehabilitation of stroke patients?” Safety was assessed by examining the reported adverse reactions. Effectiveness was assessed based on post-rehabilitative balance ability, functional performance, muscular strength, gait function (e.g., distance, speed, stride), stroke severity, and quality of life (QoL). Relevant articles were sourced from three overseas and five domestic databases. To minimize the risk of selection bias, two reviewers independently screened the available literature, and selected relevant articles based on the designated inclusion and exclusion criteria. After screening, the two reviewers converged on a single conclusion, and in the case of disagreement, a discussion was initiated to reach an agreement. The risk of bias was assessed using the Cochrane Risk of Bias (RoB). Based on the results of the systematic review, the Health Technology Reassessment Committee determined the level of recommendation according to the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) method. Concerning the subcommittee conclusions on safety and effectiveness, a unanimous agreement was not reached, thus voting was carried out. The result of the first round of voting was a tie, and the final conclusion was reached only after a second round. In addition, the subcommittee planned to investigate the level of willingness to pay according to the clinical value of RAGT and to carry out a cost-effectiveness, cost-minimization, or contingent valuation analysis.

Results The following indicators of effectiveness were examined: the Berg balance scale (BBS) and Rivermead Mobility Index (RMI) for score-based analyses of balance ability; the Timed Up & Go (TUG) test for time-based analyses of balance ability; the modified Emory Functional Ambulation Profile (mEFAP) to evaluate task-oriented gait; the Functional Ambulation Category (FAC), Modified Barthel Index (BI), and Functional Independence Measure (FIM) to appraise functional performance; the Fugl-Meyer Assessment (FMA) to analyze muscular strength; the motricity index (MI) to examine motor function, distance-based and step count-based gait speed, and gait distance; and lastly, the National Institutes of Health Stroke Scale (NIHSS) to assess stroke severity, total QoL score, and QoL body score. A total of 63 studies (66 articles) were selected, of which, 52 and 11 were randomized controlled trials (RCTs) and non-randomized controlled studies (NRS), respectively. The total number of patients analyzed in RCTs was 2,112. A total of 20 studies compared the RAGT intervention with palliative rehabilitation, while 32 compared the combination of RAGT and palliative rehabilitation with palliative rehabilitation only. The total number of patients analyzed in NRSs was 381. Only two studies compared the RAGT intervention with palliative rehabilitation, while nine compared the combination of RAGT and palliative rehabilitation with palliative rehabilitation only. Due to the insufficient data, the RoB was calculated to be high (21.2–27.3%).

Safety Twenty RCTs reported on the adverse reactions of RAGT. No intervention-related side effects were reported in 18 studies, while two reported skin irritation, falling, ankle joint deformity, deep vein thrombosis, and death. Death was reported for one subject in the palliative rehabilitation group, but whether the cause of death was related to the intervention was not mentioned. Four NRSs examined adverse reactions: four studies reported no side effects, and one of these outlined the case of a patient with pain .

Effectiveness The meta-analysis comparing all RAGT intervention groups with all palliative rehabilitation groups showed that RAGT resulted in significant improvements in the score-based analyses of balance ability and functional performance compared to palliative rehabilitation (BBS, RMI: standardized mean difference [SMD], 0.37; 95% confidence interval [CI]: 0.11, 0.63; I2 = 77.4%; n = 29; FAC, BI, FIM: SMD = 0.25; 95% CI: 0.01, 0.48; I2 = 72.7%; n= 26). However, these results should be interpreted with care considering the high heterogeneity of the data. The RAGT and palliative rehabilitation groups did not vary significantly for the following parameters: time-based balance (TUG, mEFAP: SMD = -0.18; 95% CI: -0.49, 0.12; I2 = 57.0%; n = 15), muscular strength (FMA, MI: SMD = 0.10; 95% CI: -0.11, 0.30; I2 = 48.2%; n = 19), distance-based gait speed (weighted mean difference [WMD, 0.01 m/s; 95% CI: -0.05, 0.07; I2 = 50.4%; n = 32), step count-based gait speed (WMD, -8.97 steps/min; 95% CI: -26.81, 8.86, I2 = 90.5%; n = 8), gait distance (WMD, 1.83 m; 95% CI: -7.36, 11.01; I2 = 11.9%; n = 17), stroke severity (NIHSS: WMD, -1.39; 95% CI: -4.04, 1.26; I2 = 90.8%; n = 3), total QoL score (SMD = 0.07; 95% CI: -0.19, 0.34; I2 = 13.5%; n = 6), and QoL body score (WMD, -0.97; 95% CI: -5.27, 3.34; I2 = 0.0%; n = 3). In the meta-analysis comparing a single RAGT treatment with palliative rehabilitation, the following parameters showed no significant difference: time-based balance (TUG, mEFAP: SMD = -0.22; 95% CI: -0.56, 0.12; I2 = 42.4%; n = 9), functional performance (FAC, BI, FIM: SMD = 0.11; 95% CI: -0.14, 0.36; I2 = 6.1%; n = 8), muscular strength (FMA, MI: SMD = 0.19; 95% CI: 0.16, 0.54, I2 = 50.8%; n= 9), distance-based gait speed (WMD, -0.03 m/s, 95% CI: -0.11, 0.05; I2 = 57.1%; n = 16), step count-based gait speed (WMD, -13.62 steps/min; 95% CI: -36.24, 9.01; I2 = 93.0%; n = 6), gait distance (WMD, -2.96 m; 95% CI: -13.74, 7.82; I2 = 3.9%; n = 10), total QoL score (SMD = 0.17; 95% CI: -0.18, 0.51; I2=0.0%; n = 3), and QoL body score (WMD, -1.34; 95% CI: -5.74, 3.06; I2 = 0.0%; n = 2). Only a trend could be established for score-based balance (BBS, RMI: SMD = 0.21; 95% CI: -0.19, 0.60; I2 = 72.0%; n = 11) and stroke severity (NIHSS: WMD, 0.9; 95% CI: -0.08, 1.88; I2 = N/A; n = 1) because only one article reported on these parameters. The meta-analysis showed that the combination of RAGT and palliative rehabilitation resulted in significant improvements in score-based balance, functional performance, and stroke severity compared to palliative rehabilitation alone (BBS, RMI: SMD = 0.48; 95% CI: 0.13, 0.83; I2 = 80.1%; n = 18; FAC, BI, FIM: SMD = 0.32; 95% CI: 0.001, 0.63; I2 = 79.4%; n = 18; NIHSS: WMD, -2.6; 95% CI: -5.14, -0.06; I2 = 77.9%; n = 2). However, these results should be interpreted with care considering the high heterogeneity of the data. However, the groups receiving RAGT in combination with palliative rehabilitation or palliative rehabilitation alone did not significantly vary for the following parameters: time-based balance (TUG, mEFAP: SMD = -0.15; 95% CI: -0.75, 0.45; I2 = 72.5%; n = 6), muscular strength (FMA, MI: SMD = 0.04; 95% CI: -0.23, 0.30; I2 = 50.7%; n = 10), distance-based gait speed (WMD, 0.05 m/s; 95% CI: -0.000, 0.10; I2 = 28.7%; n = 16), step count-based gait speed (WMD, 6.67 steps/min; 95% CI: -8.42, 21.77; I2 = 0.0%; n = 2), gait distance (WMD, 14.18 m; 95% CI: -3.04, 32.01; I2 = 0.7%; n = 7); and total QoL score (SMD = -0.07; 95% CI: -0.50, 0.35; I2 = 39.6%; n = 3). Only a trend could be established for QoL body score (WMD, 7.40; 95% CI: -13.40, 28.20; I2 = N/A; n = 1) because only one article reported on this parameter. Lastly, when studies concerning patients within 6 months of subacute management after stroke onset were analyzed alone, significant improvements in score-based balance, functional performance, and gait distance indicators were detected after RAGT compared to palliative rehabilitation.

Cost-effectiveness and willingness to pay For each parameter, the WMD, rather than the SMD, was calculated. The results indicated, when individually comparing RAGT-treated groups with patients undergoing palliative rehabilitation, only the FMA score reached statistical significance (WMD, 1.69; 95% CI: 0.59, 2.79). The obtained WMD of 1.69 could not be confidently interpreted as a clinically significant improvement. Therefore, the subcommittee reached the conclusion that this value could not be utilized to securely investigate the level of willingness to pay, as the results of such investigation would be likely invalid. In addition, no clearly positive clinical outcomes were ascribed to RAGT, and criteria for confidently establishing clinical improvements are lacking. Therefore, the subcommittee decided not to carry out any analyses of cost-effectiveness and willingness to pay.

Conclusions and recommendations Based on the level of evidence gathered from this systematic review, the subcommittee suggests the following: RAGT is a safe technique for the rehabilitation of stroke patients as no major adverse reactions or severe side effects have been reported. Concerning effectiveness, RAGT results across all studies showed significantly greater improvements in score-based balance ability and functional performance than palliative rehabilitation. On the other hand, no difference was detected in time-based balance, muscular strength, gait speed and distance, stroke severity, and QoL between the two types of rehabilitation. In addition, no significant difference was found when comparing patients treated with a single RAGT session with patients undergoing palliative rehabilitation across all analyzed parameters. However, RAGT in combination with palliative rehabilitation resulted in greater improvements in score-based balance ability, functional performance, and stroke severity than palliative rehabilitation alone than palliative rehabilitation alone. Once again, no differences were detected in time-based balance ability, gait speed and distance, and QoL. Lastly, RAGT was found to result in great improvement in score-based balance ability, functional performance, and gait distance within 6 months of stroke onset. Therefore, the subcommittee determined that RAGT is a safe technique that affords a level of effectiveness similar to that of palliative rehabilitation (quality of evidence: low).

Based on the findings of the 7th Health Technology Reassessment Committee in 2021 (July 9, 2021), the 8th Health Technology Reassessment Committee in 2021 (August 14, 2021) drew the following conclusions regarding RAGT for the rehabilitation of stroke patients based on the subcommittee review and Article 4 Paragraph 10 of the Health Technology Reassessment Guideline: RAGT is a safe technique for stroke patients. In addition, RAGT is mildly effective, compared to the conventional palliative rehabilitation treatment, as it results in greater improvements in balance ability and functional performance. Therefore, the Health Technology Reassessment Committee conditionally recommends RAGT for the rehabilitation of stroke patients (GRADE recommendation: conditional).
