다발성경화증 환자에서 하지재활로봇을 이용한 보행치료

Abstract

평가 배경 하지재활로봇을 이용한 보행치료(Robot-Assisted gait training)는 건강보험에 등재 기술인 ‘보행치료(사130, 수가코드MM302)’로 분류되어 의료현장에서 사용되고 있다. 하지재활로봇을 이용한 보행치료가 고식적 재활치료에 비해 치료효과 개선 등을 근거로 행위 재분류 및 하지재활로봇과 관련한 건강보험 수가인상에 대한 제조업체 및 의료계의 요구가 있다. 따라서 건강보험심사평가원은 기존 고식적 재활치료 대비 하지재활로봇을 이용한 보행치료의 임상적 유용성의 개선여부 및 경제적 가치(비용효과성)가 있는지 여부에 대한 재평가를 한국보건의료연구원에 의뢰하여(의료기술등재부-1378, 2020.06.17), 2020년 제7차 의료기술재평가위원회(2020.7.10.)에서 재평가 계획서 및 소위원회 구성안에 대해 심의를 받아 재평가를 수행하였다. 본 평가의 목적은 다발성경화증 환자에서 하지재활로봇 치료의 임상적 안전성 및 유효성 등에 대한 과학적 근거를 제공하고 이를 통해 의료기술의 적정사용 등 정책적 의사결정을 지원하는 것이다.

평가 방법 다발성경화증 환자에서 하지재활로봇을 이용한 보행치료에 대한 안전성 및 유효성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “하지재활로봇을 이용한 보행치료 평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회 구성은 재활의학과 3인, 신경과 2인, 정형외과 1인, 신경외과 1인, 소아청소년과 1인, 근거기반의학 2인의 전문가 10인으로 구성하였다. 평가의 핵심질문은 “다발성경화증 환자에서 하지재활로봇을 이용한 보행치료는 고식적 재활치료에 비해 안전하고 효과적인가?”이었고, 안전성은 보고된 이상반응을 지표로 하였고, 유효성은 보행기능(거리, 속도, 걸음 등), 균형, 삶의 질, 피로, 통증, 경직, 독립기능, 질병 중증도를 결과지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌선택 및 배제기준에 따라 2명의 평가자가 독립적으로 수행하고, 의견의 불일치가 있는 경우에는 평가자 간 합의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림위험 평가는 Cochrane의 Risk of Bias를 사용하여 평가하였으며, 최종 선택된 문헌을 대상으로 2명의 평가자가 독립적으로 평가를 실시하였으며, 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 모든 자료는 연구단위로 추출하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 방법을 이용하여 근거 수준을 평가하고, 평가 결과를 토대로 권고등급을 결정하였다. 결론 도출은 하지재활로봇 중재군 전체에 대해 분석결과 및 결론을 우선적으로 제시하고, 임상적 안전성 및 유효성에 대한 결론은 합의를 통해서 결정하였다.

평가 결과 본 평가에 최종 선택된 연구는 총 9개(10편)의 무작위배정임상시험연구(Randomized Controlled Trial, RCT)이었다. RCT 연구는 총 371명이었고, 하지재활로봇 단독중재군과 고식적 재활치료를 보고한 연구는 4개였고, 하지재활로봇 및 고식적 재활치료를 병용한 중재군과 고식적 재활치료를 비교한 연구는 5개였다. 비뚤림위험 수준은 선택적 보고에서 ‘높음’이 33.3%로 비뚤림위험이 높았다.

안전성 임상적 안전성 결과지표는 보고한 이상반응을 보고한 연구의 모든 지표를 검토하였다. 최종 선택문헌에서 이상반응을 보고한 연구는 4개였으며, 이 중 3개 연구는 이상반응 또는 중재와 관련된 이상반응이 없었다고 보고하였다. Vaney (2012)의 연구는 하지재활로봇 및 고식적 재활치료 병용 중재군과 고식적 재활치료군을 비교한 연구로 재활치료 중 환자 1명에서 발목 염좌(ankle sprain)가 발생하였지만 해당 보행치료와 관련성이 없다고 보고하였다.

유효성 임상적 유효성 결과지표는 보행기능(거리, 속도, 걸음 등), 균형, 삶의 질, 피로, 통증, 경직, 독립기능, 질병 중증도의 지표를 기준으로 하지재활로봇을 이용한 보행치료 중재군 전체와 고식적 재활치료군을 비교 평가하였다. 특정시간 동안 보행한 거리(6MWT, 2MWT)를 기준으로 하지재활로봇 중재군과 고식적 재활치료군을 비교한 5편의 문헌을 검토한 결과 두 군 간의 유의한 차이를 확인할 수 없었다(SMD: -0.09; 95% CI: -0.46, 0.29). 보행속도(10WMS, Gait Speed, T25FW)에서도 두 군 간의 차이(6편; SMD: -0.05; 95% CI: -0.33, 0.23)가 없었으며 보폭(Step length)을 기준으로 한 보행기능(2편; SMD: 0.33; 95% CI: -0.53, 1.19)과 양측지지 시간(Double Support Time) (%)에서도 두 군 간의 유의한 차이가 없었다(3편; WMD: -2.49; 95% CI: -6.80, 1.82). 버그 균형 척도(BBS)를 기준으로 보행치료 이후 균형개선효과를 비교한 4편의 문헌에서 하지재활로봇 중재군과 고식적 재활치료군 두 군 간의 유의한 차이를 확인할 수 없었으며(WMD: 0.19; 95% CI: -2.51, 2.88), TUG에서도 두 군 간의 유의한 차이가 없었다(3편; WMD: -2.26; 95% CI: -9.55, 5.03). 보행치료 이후 육체적 영역의 삶의 질 개선을 보고한 3편의 연구를 합성한 결과 두 군 간의 차이가 없었으며(SMD: -0.14; 95% CI: -0.50, 0.21). 전반적인 삶의 질을 비교한 연구에서 유의한 차이가 없었다(2편; SMD: 0.28; 95% CI: -0.17, 0.74). 보행치료로 인해 다발성경화증 환자들에서의 피로 개선을 측정한 4편의 연구를 검토한 결과에서도 하지재활로봇 중재군과 고식적 재활치료 군 간의 차이를 확인할 수 없었다(WMD: -0.05; 95% CI: -1.14, 1.04).

결론 및 제언 체계적 문헌고찰 결과 다발성경화증에서 하지재활로봇을 이용한 보행치료 중재군 전체와 고식적 재활치료의 안전성은 4편의 연구에서 이상반응이 없다고 보고허가나 해당 중재와 관련이 없는 경미한 부작용을 보고하여 안전한 기술로 평가하였다. 유효성은 하지재활로봇 중재군과 고식적 재활치료군와 비교하여 보행, 균형, 삶의 질, 피로 등 주요 결과지표 모두에서 통계적으로 유의한 차이가 없었다. 따라서 소위원회에서는 본 평가결과 다발성경화증 환자에서 하지재활로봇을 이용한 보행치료는 안전한 기술이며 유효성은 고식적 재활치료와 유사한 기술로 평가하였다(근거의 신뢰수준, Low). 2021년 제7차 의료기술재평가위원회(2021.07.09.)에서는 소위원회 검토 결과와 관련하여 논의를 하였고, 제8차 의료기술재평가위원회(2021.08.13.)에서는 의료기술재평가사업 관리지침 제4조제10항에 의거 “다발성경화증 환자에서 하지재활로봇을 이용한 보행치료”에 대해 다음과 같이 심의하였다. 다발성경화증 환자에서 하지재활로봇을 이용한 보행치료는 안전한 기술이며, 보행, 균형, 삶의 질, 피로 등 유효성 결과지표에서 기존의 고식적 재활치료와 효과차이가 없다는 것을 근거로 조건부 권고로 심의하였다(권고등급: 조건부 권고함).

Robot-assisted gait training in patients with multiple sclerosis

Background Robot-assisted gait training (RAGT) based rehabilitation falls within the category of “Gait Training (Sa130, charge code MM302)”, and as such, is covered by the South Korean National Health Insurance (NHI) program. This technology is commonly utilized in the field of physiotherapy, and it has been shown to lead to better clinical outcomes than conventional rehabilitation. On this basis, the manufacturers of the RAGT technology, as well as the healthcare industry, have demanded a rise in the NHI charge for this type of rehabilitation. The Health Insurance Review & Assessment Service has thus requested the National Evidence-Based Healthcare Collaborating Agency to assess whether RAGT is more cost-efficient and clinically effective than conventional rehabilitation (Health Technology Registrar No. 1378; June 17, 2020). Therefore, a reassessment plan was approved by the 7th Health Technology Reassessment (HTR) Committee of 2020 (July 10, 2020) and the relevant subcommittee was designated. The purpose of the assessment was to examine the current scientific evidence for the clinical safety and effectiveness of RAGT-based rehabilitation for patients with multiple sclerosis, with the intent to support policy making concerning the appropriate uses of this technology.

Methods To assess the safety and effectiveness of RAGT-based rehabilitation for patients with multiple sclerosis, a systematic review of the current literature was performed. All assessment methods were designed, approved, and carried out by the RAGT Reassessment Subcommittee (hereinafter referred to as the subcommittee) through discussions about the purpose of the assessment. The subcommittee consisted of ten experts: three rehabilitation medicine specialists, two neurologists, one orthopedist, one neurosurgeon, one pediatrician, and two specialists in the field of evidence-based medicine. The key question of the assessment was the following: “is RAGT-based rehabilitation safer and more effective than conventional therapy in the rehabilitation of patients with multiple sclerosis?” Safety was assessed by examining the reported adverse event. Effectiveness was assessed based on gait function (e.g., distance, speed, and stride), balance, quality of life (QoL), fatigue, pain, spasticity, independent function, and disease severity. Relevant articles were sourced from three overseas and five domestic databases. To minimize the risk of selection error, two reviewers independently screened the available literature, and selected relevant articles based on the designated inclusion and exclusion criteria. After screening, the two reviewers converged on a single conclusion, and in the case of disagreement, a discussion was initiated to reach an agreement. The risk of bias in included studies was assessed using the Cochrane Risk of Bias (RoB) tool. Based on the results of the systematic review, the HTR Committee determined the level of recommendation according to the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) method. The conclusions were drawn primarily for the result interpretation for the entire RAGT based intervention groups, while pursuing agreements for the clinical safety and effectiveness.

Results A total of nine studies (10 articles) were selected and analyzed, and all were randomized controlled trials (RCTs). The total number of patients analyzed by the studies was 371. Four studies compared a single RAGT based rehabilitation with conventional therapy, while five compared the combination of RAGT and conventional rehabilitation with conventional rehabilitation alone. Due to the incomplete outcome data, the risk of attrition bias was calculated to be high (33%).

Safety Three out of the four studies that examined adverse event reported that no side effects arose. The remaining study reported a case of ankle sprain during rehabilitation although no correlation with gait training was found.

Effectiveness When comparing the RAGT-treated group and patients who received conventional rehabilitation, no statistical significant difference was detected in the following parameters: gait distance (6 minute walk test [MWT], 2MWT: standardized mean difference [SMD]: -0.09; 95% confidence interval [CI]: -0.46, 0.29; studies = 5), gait speed (10WMS, gait speed, timed 25-foot walk [T25FW]: SMD = -0.05; 95% CI: -0.33, 0.23; studies = 6), step length-based gait function (SMD = 0.33; 95% CI: -0.53, 1.19; studies = 2), and double support time (weighted mean difference [WMD]: -2.49; 95% CI: -6.80, 0.82]; studies = 3). Similarly, the two groups did not significantly differ in the Berg Balance Scale (BBS) (WMD, 0.19; 95% CI: -2.51, 0.88; studies = 4), the Timed Up and Go (TUG) test (WMD, -2.26; 95% CI: -9.55, 5.03; studies = 3), physical QoL (SMD = -0.14; 95% CI: -0.50, 0.21; studies = 3), overall QoL score (SMD = 0.28; 95% CI: -0.17, 0.74; studies = 2), and improvement of fatigue (WMD, -0.05; 95% CI: -1.14, 1.04; studies = 4).

Conclusions and recommendations Based on the level of evidence gathered from this systematic review, the subcommittee suggests the following: RAGT-based therapy is a safe technique for the rehabilitation of patients with multiple sclerosis, as no major adverse event, apart from a mild unrelated side effect, have been reported. In addition, because RAGT-based rehabilitation was not significantly more effective than conventional therapy, the subcommittee determined that RAGT-based rehabilitation is a safe technique for patients with multiple sclerosis, with a level of effectiveness similar to that of conventional rehabilitation (Level of evidence: low).

Based on the discussion of the 7th HTR Committee in 2021 (July 9, 2021), the 8th HTR committee in 2021 (August 13, 2021) drew the following conclusions regarding RAGT-based therapy for the rehabilitation of patients with multiple sclerosis based on the subcommittee review and Article 4, Paragraph 10 of the Health Technology Reassessment Guideline: RAGT-based rehabilitation is a safe technique in patients with multiple sclerosis and no significant difference with conventional therapy was detected regarding effectiveness such as gait, balance, QoL, fatigue. Therefore, the HTR committee conditionally recommends RAGT for the rehabilitation of patients with multiple sclerosis (GRADE recommendation: conditional).
