파킨슨 환자에서 하지재활로봇을 이용한 보행치료

Abstract

평가 배경 하지재활로봇을 이용한 보행치료(Robot-Assisted gait training)는 건강보험에 등재 기술인 ‘보행치료(사130, 수가코드 MM302)’로 분류되어 의료현장에서 사용되고 있다. 하지재활로봇을 이용한 보행치료가 고식적 재활치료에 비해 치료효과 개선 등을 근거로 행위 재분류 및 하지재활로봇과 관련한 건강보험 수가인상에 대한 제조업체 및 의료계의 요구가 있다. 따라서 건강보험심사평가원은 기존 고식적 재활치료 대비 하지재활로봇을 이용한 보행치료의 임상적 유용성의 개선여부 및 경제적 가치(비용효과성)가 있는지 여부에 대한 재평가를 한국보건의료연구원에 의뢰하여(의료기술등재부-1378, 2020.06.17.), 2020년 제7차 의료기술재평가위원회(2020.07.10.)에서 재평가 계획서 및 소위원회 구성안에 대해 심의를 받아 재평가를 수행하였다. 본 평가의 목적은 파킨슨병 환자에서 하지재활로봇 치료의 임상적 안전성 및 유효성 등에 대한 과학적 근거를 제공하고 이를 통해 의료기술의 적정사용 등 정책적 의사결정을 지원하는 것이다.

평가 방법 파킨슨병 환자에서 하지재활로봇을 이용한 보행치료에 대한 안전성 및 유효성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 연구목적을 고려하여 “하지재활로봇을 이용한 보행치료 평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회 구성은 재활의학과 3인, 신경과 2인, 정형외과 1인, 신경외과 1인, 소아청소년과 1인, 근거기반의학 2인의 전문가 10인으로 구성하였다. 평가의 핵심질문은 “파킨슨병 환자에서 하지재활로봇을 이용한 보행치료는 고식적 재활치료에 비해 안전하고 효과적인가?”이었고, 안전성은 보고된 이상반응을 지표로 하였고, 유효성은 파킨슨병 중증도, 거리 기준 보행속도, 걸음수 기준 보행속도, 보행거리, 걸음길이 등 걸음 특성, 보행관련 점수, 균형기능, 보행동결, 삶의 질 등을 결과지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌선택 및 배제기준에 따라 2명의 평가자가 독립적으로 수행하고, 의견의 불일치가 있는 경우에는 평가자 간 합의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림위험 평가는 Cochrane의 Risk of Bias를 사용하여 평가하였으며, 최종 선택된 문헌을 대상으로 2명의 평가자가 독립적으로 평가를 실시하였으며, 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 모든 자료는 연구단위로 추출하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 GRADE 방법을 이용하여 근거 수준을 평가하고, 평가 결과를 토대로 권고등급을 결정하였다.

평가 결과 본 평가에 최종 선택된 연구는 총 9개였고, 모두 무작위배정임상시험연구(Randomized Controlled Trial, 이하 RCT)였다. 환자수는 총 439명이었고, 하지재활로봇 및 고식적 재활치료 병용중재와 고식적 재활치료를 비교한 연구는 2편(22.2%)이었고, 7편은 하지재활로봇 단독요법과 고식적 재활치료를 비교하였다. 비뚤림위험 수준은 불충분한 결과자료에서 ‘높음’이 33.3%로 비뚤림위험이 높았다.

안전성 이상반응을 보고한 문헌은 5편이었고, 5편 모두 이상반응이 없었다고 보고하였다.

유효성 임상적 유효성 결과지표는 파킨슨병 중증도(Unified Parkinson's Disease Rating Scale, 이하UPDRS) 총점(UPDRS-총점), 운동성 검사(UPDRS-III), 일상에서의 운동경험(UPDRS-II), 거리 기준 보행속도, 걸음수 기준 보행속도, 보행거리, 걸음길이 등 걸음 특성, 보행관련 점수, 점수 기준 균형기능(Berg Balance Scale, 이하 BBS), 시간 기준 균형기능(Timed Up & Go 검사, 이하 TUG), 보행동결, 삶의 질 전체, 신체영역, 정신영역 점수 등을 지표로 평가하였다. 하지재활로봇을 이용한 보행치료와 고식적 재활치료를 비교한 메타분석 결과, 보행관련 점수는 하지재활로봇 중재군이 고식적 재활치료군에 비해 개선효과가 있었다(3편, 표준평균차이(Standardized Mean Difference, 이하 SMD) 0.55, 95% CI 0.20~ 0.91, I2=23.4%). 그 외 결과지표인 파킨슨병 중증도-총점(UPDRS-총점)(4편, 가중평균차이(Weighted Mean Difference, 이하 WMD) –3.50점, 95% CI –10.66~3.66, I2=92.7%), 파킨슨병 중증도 운동성 검사(UPDRS-III)(4편, WMD –1.77점, 95% CI –4.37 ~0.83, I2=57.4%), 거리 기준 보행속도(5편, WMD 0.08 m/sec, 95% CI –0.01~ 0.18, I2=67.4%), 걸음수 기준 보행속도(2편, WMD 6.54 steps/min, 95% CI –8.57~21.64, I2=72.2%), 보행거리(5편, WMD 17.52 m, 95% CI –24.46~59.50, I2=0.0%), 걸음길이 등 걸음 특성(2편, SMD 0.57, 95% CI –0.59~1.72, I2=85.2%), 점수 기준 균형기능(BBS)(4편, WMD 2.34점, 95% CI –0.68~5.36, I2=86.0%), 시간 기준 균형기능(TUG)(5편, WMD 0.55 sec, 95% CI –0.47 ~1.56, I2=91.5%)은 두 군간 차이가 없었다. 또한, 파킨슨병 중증도-일상에서의 운동경험(UPDRS-II) (1편, WMD –2.87점, 95% CI –5.23~-0.51), 보행동결(1편, WMD –0.50점, 95% CI –2.80~1.80), 삶의 질 전체 점수(1편, WMD 10.50점, 95% CI –0.04~21.04), 삶의 질 신체영역 점수(1편, WMD 10.98점, 95% CI –5.18~7.14), 삶의 질 정신영역 점수(1편 WMD –0.70점, 95% CI –6.29~4.89)는 결과값을 보고한 문헌 각 1편으로 경향성만을 확인하였다.

결론 및 제언 체계적 문헌고찰 결과, 파킨슨병 환자에서 하지재활로봇을 이용한 보행치료와 고식적 재활치료의 안전성은 대부분의 연구에서 이상반응이 없다고 기술하거나 심각한 부작용 등이 보고되지 않아 안전한 기술로 평가하였다. 하지재활로봇을 이용한 보행치료 중재의 유효성 지표인 보행관련 점수는 하지재활로봇 중재군이 고식적 재활치료군에 비해 개선효과가 있었고, 그 외 결과지표인 파킨슨병 중증도-총점(UPDRS-총점), 운동성 검사(UPDRS-III), 거리 기준 보행속도, 걸음수 기준 보행속도, 보행거리, 걸음길이 등 걸음 특성, 점수 기준 균형기능(BBS), 시간 기준 균형기능(TUG)은 두 군간 차이가 없었다. 따라서 소위원회에서는 본 평가결과 파킨슨병 환자에서 하지재활로봇을 이용한 보행치료는 안전한 기술이며, 유효성은 고식적 재활치료와 유사한 기술로 평가하였다(근거의 신뢰수준, Very Low). 2021년 제7차 의료기술재평가위원회(2021.07.09.)에서는 소위원회 검토 결과와 관련하여 논의를 하였고, 제8차 의료기술재평가위원회(2021.08.14.)에서 의료기술재평가사업 관리지침 제4조제10항에 의거 “파킨슨병 환자에서 하지재활로봇을 이용한 보행치료”에 대해 다음과 같이 심의하였다. 파킨슨병 환자에서 하지재활로봇을 이용한 보행치료는 안전한 기술이며, 보행관련 점수에서 하지재활로봇이 고식적 치료에 비해 유의한 개선효과가 있었고, 파킨슨병 중증도-총점(UPDRS-총점), 운동성 검사(UPDRS-III), 거리 기준 보행속도, 걸음수 기준 보행속도, 보행거리, 걸음길이 등 걸음 특성, 점수 기준 균형기능(BBS), 시간 기준 균형기능(TUG) 등 결과지표에서 기존의 고식적 재활치료와 효과차이가 없다는 것을 근거로 조건부 권고로 심의하였다(권고등급: 조건부 권고함).

Robot-assisted gait training in patients with Parkinson’s disease

Background Robot-assisted gait training (RAGT) rehabilitation falls within the category of “Gait Training (Sa130, charge code MM302)”, and as such, is covered by the South Korean national health insurance program. This technology is commonly utilized in the field of physiotherapy, and it has been shown to lead to better clinical outcomes than palliative rehabilitation. On this basis, the manufacturers of the RAGT technology, as well as the healthcare industry, have demanded a rise in the national health insurance charge for this type of rehabilitation. The Health Insurance Review & Assessment Service has thus requested the National Evidence-Based Healthcare Collaborating Agency to assess whether RAGT is more cost-efficient and clinically effective than conventional palliative rehabilitation (Health Technology Registrar No. 1378; June 17, 2020). Therefore, a reassessment plan was approved by the 7th Health Technology Reassessment Committee in 2020 (July 10, 2020) and the relevant subcommittee was designated. The purpose of the assessment was to examine the current scientific evidence for the clinical safety and effectiveness of RAGT-based rehabilitation for patients with Parkinson’s disease (PD), with the intent to support policy making concerning the appropriate uses of this technology.

Methods To assess the safety and effectiveness of RAGT-based rehabilitation for patients with PD, a systematic review of the current literature was performed. All assessment methods were designed, approved, and carried out by the RAGT Reassessment Subcommittee (hereinafter referred to as the subcommittee) through discussions about the purpose of the assessment. The subcommittee consisted of ten experts: three rehabilitation medicine specialists, two neurologists, one orthopedist, one neurosurgeon, one pediatrician, and two specialists in the field of evidence-based medicine. The key question of the assessment was the following: “ is RAGT safer and more effective than palliative care in the rehabilitation of patients with PD?” Safety was assessed by examining the reported adverse reactions. Effectiveness was assessed based on the post-rehabilitative severity of PD, distance-based gait speed, step count-based gait speed, gait distance, gait characteristics, gait score, balance ability, freezing of gait (FoG), and quality of life (QoL). Specifically, the following indicators of effectiveness were examined: the Unified Parkinson's Disease Rating Scale (UPDRS) to assess PD severity, the UPDRS-III to determine the level of mobility; the UPDRS-II to examine the level of daily exercise, distance-based gait speed, step count-based gait speed, gait distance, gait characteristics, and gait score; the Berg Balance Scale (BBS) to analyze score-based balance ability; and lastly, the Timed Up & Go (TUG) test to determine time-based balance ability, FoG, and QoL (total, body, and mental score). Relevant articles were sourced from three overseas and five domestic databases. To minimize the risk of selection bias, two reviewers independently screened the available literature, and selected relevant articles based on the designated inclusion and exclusion criteria. After screening, the two reviewers converged on a single conclusion, and in the case of disagreement, a discussion was initiated to reach an agreement. The risk of bias was assessed using the Cochrane Risk of Bias (RoB). Based on the results of the systematic review, the Health Technology Reassessment Committee determined the level of recommendation according to the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) method.

Results A total of nine randomized controlled trials (RCTs) were selected and analyzed. The total number of patients analyzed by RCTs was 2,439. Two studies (22.2%) compared RAGT in combination with palliative rehabilitation and palliative rehabilitation alone, while seven studies compared a single RAGT treatment and palliative rehabilitation. Due to the insufficient data, the RoB was calculated to be high (33.3%).

Safety In regard to clinical safety, adverse reactions were investigated in five studies, which did not report any side effects.

Effectiveness The meta-analysis of all RCTs showed that RAGT results in significant improvements in gait score (standardized mean difference [SMD]: 0.55; 95% confidence interval [CI]: 0.20, 0.90; I2 = 23.4%; n = 3). However, no significant difference was detected for the following parameters: PD severity total score (UPDRS: weighted mean difference [WMD]: -3.50; 95% CI: [-10.66, 3.66; I2 = 92.7%; n = 4), PD severity mobility score (UPDRS-III: WMD, -1.77; 95% CI: -4.37, 0.83; I2 = 57.4%; n = 4), distance-based gait speed (WMD, 0.08 m/s; 95% CI: -0.01, 0.18; I2 = 67.4%; n = 5), step count-based gait speed (WMD, 6.54 steps/min; 95% CI: -8.57, 21.64; I2 = 72.2%; n = 2), gait distance (WMD, 17.52 m; 95% CI: -24.46, 59.50; I2 = 0.0%; n = 5), gait characteristics such as gait length (SMD = 0.57; 95% CI: -0.59, 1.72; I2 = 85.2%; n = 2), score-based balance ability (BBS: WMD, 2.34; 95% CI: -0.68, 5.36; I2 = 86.0%; n = 4), and time-based balance ability (TUG: WMD, 0.55 s, 95% CI: -0.47, 1.56; I2 = 91.5%; n = 5). Only a trend could be established for PD severity daily exercise (UPDRS-II: WMD, -2.87; 95% CI: -5.23, -0.51; I2 = N/A ; n = 1), FoG (WMD, -0.50; 95% CI: -2.80, 1.80; I2 = N/A ; n = 1), total QoL score (WMD, 10.50; 95% CI: -0.04, 21.04; I2 = N/A ; n = 1), QoL body score (WMD, 10.98; 95% CI: -5.18, 7.14; I2 = N/A; n = 1), and QoL mental score (WMD, -0.70; 95% CI: -6.29, 4.89; I2 = N/A) because only one article reported on these parameters.

Conclusions and recommendations Based on the level of evidence gathered from this systematic review, the subcommittee suggests the following: RAGT is a safe technique for the rehabilitation of patients with PD, as no major adverse reactions or severe side effects have been reported. Concerning effectiveness, RAGT resulted in significantly greater improvements in gait score than palliative rehabilitation. On the other hand, no significant difference was detected in PD severity total score and mobility score, distance-based gait speed, step count-based gait speed, gait distance, gait characteristics such as gait length, score-based balance ability, and time-based balance ability between the two types of rehabilitation. Therefore, the subcommittee determined that RAGT is a safe technique that affords a level of effectiveness similar to that of palliative rehabilitation (quality of evidence: low).

Based on the findings of the 7th Health Technology Reassessment Committee in 2021 (July 9, 2021), the 8th Health Technology Reassessment Committee in 2021 (August 14, 2021) drew the following conclusions regarding RAGT for the rehabilitation of patients with PD based on the subcommittee review and Article 4 Paragraph 10 of the Health Technology Reassessment Guideline: RAGT is a safe technique in patients with PD. In addition, RAGT is mildly effective, compared to the conventional palliative rehabilitation treatment, as it results in greater improvements in gait score. Therefore, the Health Technology Reassessment Committee conditionally recommends RAGT for the rehabilitation of patients with PD (GRADE recommendation: conditional).
