하지절단 환자에서 하지재활로봇을 이용한 보행치료

Abstract

평가 배경 하지재활로봇을 이용한 보행치료(Robot-Assisted gait training)는 건강보험에 등재 기술인 ‘보행치료(사130, 수가코드 MM302)’로 분류되어 의료현장에서 사용되고 있다. 하지재활로봇을 이용한 보행치료가 고식적 재활치료에 비해 치료효과 개선 등을 근거로 행위 재분류 및 하지재활로봇과 관련한 건강보험 수가인상에 대한 제조업체 및 의료계의 요구가 있다. 따라서 건강보험심사평가원은 기존 고식적 재활치료 대비 하지재활로봇을 이용한 보행치료의 임상적 유용성의 개선여부 및 경제적 가치(비용효과성)가 있는지 여부에 대한 재평가를 한국보건의료연구원에 의뢰하여(의료기술등재부-1378, 2020.06.17.), 2020년 제7차 의료기술재평가위원회(2020.07.10.)에서 재평가 계획서 및 소위원회 구성안에 대해 심의를 받아 재평가를 수행하였다. 본 평가의 목적은 하지 절단 및 기타 질환 환자에서 하지재활로봇 치료의 임상적 안전성 및 유효성 등에 대한 과학적 근거를 제공하고 이를 통해 의료기술의 적정사용 등 정책적 의사결정을 지원하는 것이다.

평가 방법 하지 절단 및 기타 질환 환자에서 하지재활로봇을 이용한 보행치료에 대한 안전성 및 유효성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 연구목적을 고려하여 “하지재활로봇을 이용한 보행치료 평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회 구성은 재활의학과 3인, 신경과 2인, 정형외과 1인, 신경외과 1인, 소아청소년과 1인, 근거기반의학 2인의 전문가 10인으로 구성하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌선택 및 배제기준에 따라 2명의 평가자가 독립적으로 수행하고, 의견의 불일치가 있는 경우에는 평가자 간 합의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림위험 평가는 Cochrane의 Risk of Bias를 사용하여 평가하였으며, 최종 선택된 문헌을 대상으로 2명의 평가자가 독립적으로 평가를 실시하였으며, 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 모든 자료는 연구단위로 추출하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 GRADE 방법을 이용하여 근거 수준을 평가하고, 평가 결과를 토대로 권고등급을 결정하였다. 소위원회는 문헌 선정과정 중 외상성 뇌손상 환자와 하지 화상 환자에서 하지재활로봇을 이용하여 보행치료를 시행한 문헌도 하지재활로봇을 이용한 보행치료의 효과를 확인할 수 있는 문헌이라 판단하여 대상 적응증을 추가기로 결정하였고, 문헌수가 각각 1편으로 “하지 절단 및 기타 질환”으로 하나의 보고서로 기술하고, 핵심질문 및 결론은 질환별로 각각 기술하기로 하였다. 재평가의 핵심질문은 1) 하지 절단 환자에서 하지재활로봇을 이용한 보행치료는 고식적 재활치료에 비해 안전하고 효과적인가?, 2) 외상성 뇌손상 환자에서 하지재활로봇을 이용한 보행치료는 고식적 재활치료에 비해 안전하고 효과적인가?, 3) 하지 화상환자에서 하지재활로봇을 이용한 보행치료는 고식적 재활치료에 비해 안전하고 효과적인가? 이다.

평가 결과

1) 하지 절단 본 평가에 최종 선택된 연구는 총 0편으로 하지 절단 환자에서 하지재활로봇을 이용한 보행치료의 근거가 없었다.

2) 외상성 뇌손상 외상성 뇌손상 환자 대상 문헌은 1편이었고, 미국에서 연구되었고, 대상자는 18세 이상 성인 외상성 뇌손상으로 인한 보행제한이 있는 환자였다.

안전성 외상성 뇌손상 환자 대상 문헌에서는 안전성 관련 결과지표를 보고하지 않아 근거가 없었다.

유효성 외상성 뇌손상 환자 대상 문헌에서는 보행관련 결과지표인 보행속도(자기 조절 속도, 최대 속도), 거리(6분 걷기), 보폭 비대칭비(Step-length asymmetry ratio), 편측지지 시간의 비대칭비(single support time asymmetry ratio) 등을 보고하였다. 이들 결과지표는 모두 하지재활로봇을 이용한 보행치료군과 고식적 재활치료군간 통계적으로 유의한 차이가 없었다.

3) 하지 화상 하지 화상 환자 대상 문헌은 1편이었고, 한국에서 연구되었으며, 대상자는 하지 부위 50% 초과 화상이 있고 피부이식술을 받은 환자였다.

안전성 하지 화상 환자에서 하지재활로봇 중재군과 비교군 모두 피부 및 관절통 등의 이상반응은 없었다고 보고하였다.

유효성 하지 화상환자 대상 문헌에서 보행관련 결과지표는 보행거리(6분 걷기), 보행점수(Functional Ambulation Category, 이하 FAC) 등을 보고하였고, 그 외 통증정도(Visual Analogue Scale, 이하 VAS)를 보고하였다. 보행거리의 변화량은 하지재로봇을 이용한 보행치료군이 94 m, 고식적 재활군이 51 m가 개선되었으나 두 군간 차이는 통계적 유의성의 경계에 있었다(P=0.05). 보행점수와 통증지표는 두 군간 차이가 없었다.

결론 및 제언

1) 하지 절단 하지 절단 환자 대상 하지재활로봇 문헌 0건으로 근거가 없어 결론을 내릴 수 없었다.

2) 외상성 뇌손상 외상성 뇌손상 환자에서 하지재활로봇을 이용한 보행치료 문헌은 1편뿐이었고, 안전성에 대한 결과를 보고하지 않았다. 유효성은 보행속도, 거리, 지지정도 등 보행관련 결과지표 모두 고식적 재활치료와 비교해 통계적으로 유의한 차이가 없었다. 따라서 소위원회에서는 본 평가결과 외상성 뇌손상 환자에서 하지재활로봇을 이용한 보행치료의 안전성은 확인할 수 없었고, 유효성은 고식적 재활치료와 유사한 기술로 평가하였다(근거의 신뢰수준, Very Low).

3) 하지 화상 하지 화상 환자에서 하지재활로봇을 이용한 보행치료 문헌은 1편뿐이었고 안전성 결과에서는 이상반응이 없었다고 보고하였다. 유효성은 보행거리, 보행점수와 통증개선 정도 모두 고식적 재활치료와 비교해 통계적으로 유의한 차이가 없었다. 따라서 소위원회에서는 본 평가결과 하지 화상 환자에서 하지재활로봇을 이용한 보행치료는 안전한 기술이며, 유효성은 고식적 재활치료와 유사한 기술로 평가하였다(근거의 신뢰수준, Very Low).

2021년 제7차 의료기술재평가위원회(2021.07.09.)에서는 소위원회 검토 결과와 관련하여 논의를 하였고, 제8차 의료기술재평가위원회(2021.08.14.)에서 의료기술재평가사업 관리지침 제4조제10항에 의거 “하지 절단 및 기타 질환 환자에서 하지재활로봇을 이용한 보행치료”에 대해 다음과 같이 심의하였다. 하지절단 환자 대상 문헌이 0건으로, 하지절단 환자에서 하지재활로봇의 안전성 및 유효성을 판단하기에 근거가 불충분하다고 심의하였다(권고등급: 불충분). 성인 외상성 뇌손상 환자를 대상으로 한 문헌은 1편으로, 성인 외상성 뇌손상 환자에서 하지재활로봇의 안전성 및 유효성을 판단하기에 근거가 불충분하다고 심의하였다(권고등급: 불충분). 하지 화상 환자를 대상으로 한 문헌은 1편으로, 하지 화상 환자에서 하지재활로봇의 안전성 및 유효성을 판단하기에 근거가 불충분하다고 심의하였다(권고등급: 불충분).

Robot-assisted gait training in patients with lower limb amputation, traumatic brain injury, and lower limb burns

Background Robot-assisted gait training (RAGT) rehabilitation falls within the category of “Gait Training (Sa130, charge code MM302)”, and as such, is covered by the South Korean national health insurance program. This technology is commonly utilized in the field of physiotherapy, and it has been shown to lead to better clinical outcomes than palliative rehabilitation. On this basis, the manufacturers of the RAGT technology, as well as the healthcare industry, have demanded a rise in the national health insurance charge for this type of rehabilitation. The Health Insurance Review & Assessment Service has thus requested the National Evidence-Based Healthcare Collaborating Agency to assess whether RAGT is more cost-efficient and clinically effective than conventional palliative rehabilitation (Health Technology Registrar No. 1378; June 17, 2020). Therefore, a reassessment plan was approved by the 7th Health Technology Reassessment Committee in 2020 (July 10, 2020) and the relevant subcommittee was designated. The purpose of the assessment was to examine the current scientific evidence for the clinical safety and effectiveness of RAGT-based rehabilitation for patients who have undergone lower limb amputation, have suffered traumatic brain injury (TBI), or have lower limb burns, with the intent to support policy making concerning the suitable use of this technology.

Methods To assess the safety and effectiveness of RAGT-based rehabilitation for patients who have undergone lower limb amputation, have suffered TBI, or have lower limb burns, a systematic review of the current literature was performed. All assessment methods were designed, approved, and carried out by the RAGT Reassessment Subcommittee (hereinafter referred to as the subcommittee) through discussions about the purpose of the assessment. The subcommittee consisted of ten experts: three rehabilitation medicine specialists, two neurologists, one orthopedist, one neurosurgeon, one pediatrician, and two specialists in the field of evidence-based medicine. For the systematic review based on the key question, the relevant articles were sourced from three overseas and five domestic databases. To minimize the risk of selection bias, two reviewers independently screened the available literature, and selected relevant articles based on the designated inclusion and exclusion criteria. After screening, the two reviewers converged on a single conclusion, and in the case of disagreement, a discussion was initiated to reach an agreement. The risk of bias was assessed using the Cochrane Risk of Bias (RoB) tool. Based on the results of the systematic review, the Health Technology Reassessment Committee determined the level of recommendation according to the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) method. During the article selection process, the subcommittee determined that the studies reporting on the RAGT in patients with TBI or lower limb burn also allowed the evaluation of the effectiveness of the RAGT and should thus be added to the indication. The relevant studies were found as a single article each so that a single report of patients with lower limb amputation and other diseases was produced, while the key questions and conclusions were stated for each indication. The key questions were as follows: 1) Is RAGT safer and more effective than palliative rehabilitation in patients with lower limb amputation? 2) Is RAGT safer and more effective than palliative rehabilitation in patients with TBI? 3) Is RAGT safer and more effective than palliative rehabilitation in patients with lower limb burn?

Results 1) Lower limb amputation No study has explored the use of RAGT in patients with lower limb amputation.

2) Traumatic brain injury Only one case report outlined the use of RAGT-based rehabilitation in an 18-year-old American patient with limited ambulation following TBI. Similarly, only one study reported on the use of RAGT in a cohort of South Korean patients who had received skin grafts due to severe lower limb burns covering more than 50% of the skin surface.

Safety The article on the patient with TBI did not report on the safety of RAGT. The study of patients with lower limb burns reported no adverse reactions, including no skin or joint pain in neither the intervention nor control groups.

Effectiveness When comparing the patient with TBI against control individuals , no significant difference was detected in gait speed (self-selected and maximal velocity), gait distance (6-min walk), step-length symmetry, step-length asymmetry ratio, and single support time asymmetry ratio.

3) Lower limb burn One article of a study conducted in South Korea on patients with lower limb burn was found; the subjects had 50% excess lower limb burn and received skin graft.

Effectiveness In the article on the patients with lower limb burn, the following indicators were used; gait distance (6 min walk), Functional Ambulation Category for independent gait, and Visual Analogue Scale for pain. The variation in gait distance burns who underwent RAGT showed an improvement of 94 m in gait distance (assessed through a 6-min walk) compared to the 51 m achieved by the general rehabilitation control group (P = 0.05). However, the levels of independent gait and pain did not significantly differ between the two groups.

Conclusions and recommendations Based on the level of evidence gathered from this systematic review, the subcommittee suggests the following:

1) Lower limb amputation Concerning safety and effectiveness, no conclusions could be draw in regard to the use of RAGT in patients who have undergone lower limb amputation due to the complete lack of previous studies.

2) Traumatic brain injury The subcommittee has determined that, for patients who have experienced TBI and lower limb burns, RAGT is as effective as palliative rehabilitation, but its safety for this patient population remains unverified (quality of evidence: very low).

3) Lower limb burn Only one article was found on RAGT in patients with lower limb burn, which reported no adverse reaction to verify the safety of the technique. The reported indicators of effectiveness were gait distance, independent gait, and pain, all of which showed no significant difference from general rehabilitation. Thus, the subcommittee determined that RAGT is a safe technique in patients with lower limb burn with a similar level of effectiveness to palliative rehabilitation (quality of evidence: very low). Based on the findings of the 7th Health Technology Reassessment Committee in 2021 (July 9, 2021), the 8th Health Technology Reassessment Committee in 2021 (August 14, 2021) drew the following conclusions regarding RAGT for the rehabilitation of patients with lower limb amputation, TBI, or lower limb burns based on the subcommittee review and Article 4 Paragraph 10 of the Health Technology Reassessment Guideline: Due to the lack of relevant studies, the safety and effectiveness of RAGT for patients with lower limb amputation, TBI, and lower limb burns cannot be verified (GRADE recommendation: insufficient).
