소아 환자에서 하지재활로봇을 이용한 보행치료

Abstract

평가 배경 하지재활로봇을 이용한 보행치료(Robot-Assisted gait training)는 건강보험에 등재 기술인 ‘보행치료(사130, 수가코드 MM302)’로 분류되어 의료현장에서 사용되고 있다. 하지재활로봇을 이용한 보행치료가 고식적 재활치료에 비해 치료효과 개선 등을 근거로 행위 재분류 및 하지재활로봇과 관련한 건강보험 수가인상에 대한 제조업체 및 의료계의 요구가 있다. 따라서 건강보험심사평가원은 기존 고식적 재활치료 대비 하지재활로봇을 이용한 보행치료의 임상적 유용성의 개선여부 및 경제적 가치(비용효과성)가 있는지 여부에 대한 재평가를 한국보건의료연구원에 의뢰하여(의료기술등재부-1378, 2020.06.17.), 2020년 제7차 의료기술재평가위원회(2020.07.10.)에서 재평가 계획서 및 소위원회 구성안에 대해 심의를 받아 재평가를 수행하였다. 본 평가의 목적은 소아청소년 환자에서 하지재활로봇 치료의 임상적 안전성 및 유효성 등에 대한 과학적 근거를 제공하고 이를 통해 의료기술의 적정사용 등 정책적 의사결정을 지원하는 것이다.

평가 방법 소아청소년 환자에서 하지재활로봇을 이용한 보행치료에 대한 안전성 및 유효성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 연구목적을 고려하여 “하지재활로봇을 이용한 보행치료 평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회 구성은 재활의학과 3인, 신경과 2인, 정형외과 1인, 신경외과 1인, 소아청소년과 1인, 근거기반의학 2인의 전문가 10인으로 구성하였다. 평가의 핵심질문은 “소아청소년 환자에서 하지재활로봇을 이용한 보행치료는 고식적 재활치료에 비해 안전하고 효과적인가?”이었고, 안전성은 보고된 이상반응을 지표로 하였고, 유효성은 대동작기능평가(Gross Motor Function Measures, 이하 GMFM) 총점, 서기(GMFM-D), 걷기, 뛰기, 도약(GMFM-E), 거리 기준 보행속도와 걸음수 기준 보행속도, 보행거리, 보행점수, 보행 지지시간, 균형기능(Berg Balance Scale, 이하 BBS), 경직(Modified Ashworth Scale, 이하 MAS) 등을 지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌선택 및 배제기준에 따라 2명의 평가자가 독립적으로 수행하고, 의견의 불일치가 있는 경우에는 평가자 간 합의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림위험 평가는 Cochrane의 Risk of Bias를 사용하여 평가하였으며, 최종 선택된 문헌을 대상으로 2명의 평가자가 독립적으로 평가를 실시하였으며, 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 모든 자료는 연구단위로 추출하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 GRADE 방법을 이용하여 근거 수준을 평가하고, 평가 결과를 토대로 권고등급을 결정하였다.

평가 결과 본 평가에 최종 선택된 연구는 총 12개(13편)이었고, 무작위배정임상시험연구(Randomized Controlled Trial, 이하 RCT)가 8개(9편)였고, 비무작위배정임상시험연구(Non-Randomized Controlled Trial, 이하 NRCT)가 4개였다. RCT 연구는 총 210명이었고, 하지재활로봇 단독 중재군과 고식적 재활치료를 보고한 연구는 5개였고, 하지재활로봇 및 고식적 재활치료를 병용한 중재군과 고식적 재활치료를 비교한 연구는 3개였다. NRCT 연구는 총 121명이었고, 하지재활로봇 단독 중재군과 고식적 재활치료를 보고한 연구는 1개였고, 하지재활로봇 및 고식적 재활치료를 병용한 중재군과 고식적 재활치료를 비교한 연구는 3개였다. 비뚤림위험 수준은 불충분한 결과자료에서 ‘높음’이 25%로 비뚤림위험이 높았다.

안전성 이상반응을 보고한 문헌은 1편(Straudi 둥, 2020)뿐이었고, 이상반응이 없었다고 보고하였다.

유효성 임상적 유효성 결과지표는 대동작기능평가 총점(GMFM-총점), 서기(GMFM-D), 걷기, 뛰기, 도약(GMFM-E), 거리 기준 보행속도와 걸음수 기준 보행속도, 보행거리, 보행점수, 보행 지지시간, 경직(MAS) 등을 지표로 평가하였다. 하지재활로봇을 이용한 보행치료와 고식적 재활치료를 비교한 RCT 연구 메타분석 결과, 대동작기능평가 서기(GMFM-D) (3편, 가중평균차이(Weighted Mean Difference, 이하 WMD) 0.23점, 95% CI –5.56~6.02, I2=14.9%), 대동작기능평가 걷기, 뛰기, 도약(GMFM-E) (3편, WMD 3.91점, 95% CI –3.05~10.88, I2=0.0%), 거리 기준 보행속도(7편, WMD 0.03 m/sec, 95% CI –0.01~0.06, I2=0.0%), 걸음수 기준 보행속도(4편, WMD 0.58 steps/min, 95% CI -4.23~5.38, I2=0.0%), 보행거리(3편, WMD 0.02m, 95% CI –0.04~0.08, I2=0.0%), 걸음길이 등 걸음 특성(6편, SMD –0.26, 95% CI –1.04~0.51, I2=77.4%), 보행 지지시간(4편, 표준평균차이(Standardized Mean Difference, 이하 SMD) 0.04, 95% CI, -0.86~0.93, I2=80.5%)은 하지재활로봇 중재군과 고식적 재활치료군간 차이가 없었다. 또한, 대동작기능평가 총점(GMFM-총점)(1편, WMD 0.20점, 95% CI –8.57 ~8.97, I2=NA), 보행관련 점수(1편, SMD 0.03, 95% CI –0.90~0.95, I2=NA), 경직(MAS) (1편, WMD 0.19점, 95% CI –0.27~0.65, I2=NA)은 1편에서만 결과를 보고하여 경향성만 확인하였다.

결론 및 제언 체계적 문헌고찰 결과, 소아청소년 환자에서 하지재활로봇을 이용한 보행치료와 고식적 재활치료의 안전성은 대부분의 연구에서 이상반응이 없다고 기술하거나 심각한 부작용 등이 보고되지 않아 안전한 기술로 평가하였다. 하지재활로봇을 이용한 보행치료 중재의 유효성 지표인 대동작기능평가 서기(GMFM-D), 걷기, 뛰기, 도약(GMFM-E), 거리 기준 보행속도, 걸음수 기준 보행속도, 보행거리, 걸음길이 등 걸음 특성, 보행 지지시간 등은 하지재활로봇 중재군과 고식적 재활치료군간 차이가 없었다. 따라서 소위원회에서는 본 평가결과 소아청소년 환자에서 하지재활로봇을 이용한 보행치료는 안전한 기술이며, 유효성은 고식적 재활치료와 유사한 기술로 평가하였다(근거의 신뢰수준, Low). 2021년 제7차 의료기술재평가위원회(2021.07.09.)에서는 소위원회 검토 결과와 관련하여 논의를 하였고, 제8차 의료기술재평가위원회(2021.08.14.)에서 의료기술재평가사업 관리지침 제4조제10항에 의거 “소아청소년 환자에서 하지재활로봇을 이용한 보행치료”에 대해 다음과 같이 심의하였다. 소아청소년 환자에서 하지재활로봇을 이용한 보행치료는 안전한 기술이며, 대동작기능평가 서기(GMFM-D), 걷기, 뛰기, 도약(GMFM-E), 거리 기준 보행속도, 걸음수 기준 보행속도, 보행거리, 걸음길이 등 걸음 특성, 보행 지지시간 등 유효성 결과지표에서 기존의 고식적 재활치료와 효과차이가 없다는 것을 근거로 조건부 권고로 심의하였다(권고등급: 조건부 권고함).

Robot-assisted gait training in pediatric patients

Background Robot-assisted gait training (RAGT) rehabilitation falls within the category of “Gait Training (Sa130, charge code MM302)”, and as such, is covered by the South Korean national health insurance program. This technology is commonly utilized in the field of physiotherapy, and it has been shown to lead to better clinical outcomes than palliative rehabilitation. On this basis, the manufacturers of the RAGT technology, as well as the healthcare industry, have demanded a rise in the national health insurance charge for this type of rehabilitation. The Health Insurance Review & Assessment Service has thus requested the National Evidence-Based Healthcare Collaborating Agency to assess whether RAGT is more cost-efficient and clinically effective than conventional palliative rehabilitation (Health Technology Registrar No. 1378; June 17, 2020). Therefore, a reassessment plan was approved by the 7th Health Technology Reassessment Committee in 2020 (July 10, 2020) and the relevant subcommittee was designated. The purpose of the assessment was to examine the current scientific evidence for the clinical safety and effectiveness of RAGT-based rehabilitation for pediatric patients, with the intent to support policy making concerning the appropriate uses of this technology.

Methods To assess the safety and effectiveness of RAGT-based rehabilitation for pediatric patients, a systematic review of the current literature was performed. All assessment methods were designed, approved, and carried out by the RAGT Reassessment Subcommittee (hereinafter referred to as the subcommittee) through discussions about the purpose of the assessment. The subcommittee consisted of ten experts: three rehabilitation medicine specialists, two neurologists, one orthopedist, one neurosurgeon, one pediatrician, and two specialists in the field of evidence-based medicine. The key question of the assessment was the following: “is RAGT safer and more effective than palliative care in the rehabilitation of pediatric patients?” Safety was assessed by examining the reported adverse reactions. Effectiveness was assessed based on the following parameters: the Gross Motor Function Measures (GMFM) total score; the GMFM standing score (GMFM-D); the walking, running, jumping GMFM score (GMFM-E); distance-based gait speed; step count-based gait speed; gait distance; gait score; gait support time; the Berg Balance Scale (BBS) to assess balance ability; and lastly, the Modified Ashworth Scale (MAS) for muscle tone assessment. Relevant articles were sourced from three overseas and five domestic databases. To minimize the risk of selection bias, two reviewers independently screened the available literature, and selected relevant articles based on the designated inclusion and exclusion criteria. After screening, the two reviewers converged on a single conclusion, and in the case of disagreement, a discussion was initiated to reach an agreement. The risk of bias was assessed using the Cochrane Risk of Bias (RoB). Based on the results of the systematic review, the Health Technology Reassessment Committee determined the level of recommendation according to the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) method.

Results A total of 12 studies (13 articles) were selected and analyzed. Of these, eight (nine) and four were randomized controlled trials (RCTs) and non-randomized controlled studies (NRS), respectively. The total number of patients analyzed by RCTs was 210. A total of five studies compared a single RAGT treatment with palliative rehabilitation, while three compared the combination of RAGT and palliative rehabilitation with palliative rehabilitation only. The total number of patients analyzed by NRSs was 121. Only one study compared a single RAGT treatment and palliative rehabilitation, while three compared the combination of RAGT and palliative rehabilitation with palliative rehabilitation only. Due to the insufficient data, the RoB was calculated to be high (25%).

Safety In regard to clinical safety, adverse reactions were investigated only in one study, which did not report any side effects.

Effectiveness According to the meta-analysis, RAGT-treated patients and those who received palliative rehabilitation did not significantly vary for the following parameters: GMFM-D (weighted mean difference [WMD]: 0.23; 95% confidence interval [CI]: -5.56, 6.02; I2 = 14.9%; n = 3), GMFM-E (WMD, 3.91; 95% CI: -3.05, 10.88; I2 = 0.0%; n = 3), distance-based gait speed (WMD, 0.03 m/s; 95% CI: -0.01, 0.06; I2 = 0.0%; n = 7), step count-based gait speed (WMD, 0.58 steps/min; 95% CI: -4.23, 5.38; I2 = 0.0%; n =4), gait distance (WMD, 0.02 m; 95% CI: -0.04, 0.08; I2 = 0.0%; n = 3), gait characteristics (standardized mean difference [SMD]: -0.26; 95% CI: -1.04, 0.51; I2 = 77.4%; n = 6), and gait support time (SMD = 0.04; 95% CI: -0.86, 0.93; I2 = 80.5%; n = 4). Only a trend could be established for GMFM-total (WMD, 0.20; 95% CI: -8.57, 8.97; I2 = N/A; n = 1), gait score (SMD = 0.03; 95% CI: -0.90, 0.95; I2 = N/A; n = 1), and MAS (WMD, 0.19; 95% CI: -0.27, 0.65; I2 = N/A; n = 1) because only one article reported on these parameters.

Conclusions and recommendations Based on the level of evidence gathered from this systematic review, the subcommittee suggests the following: RAGT is a safe technique for the rehabilitation of pediatric patients, as no major adverse reactions or severe side effects have been reported. Concerning effectiveness, RAGT did not differ from palliative rehabilitation in regard to standing, walking, running, jumping, distance-based gait speed, step count-based gait speed, gait distance, gait score, gait characteristics, and gait support time. Therefore, the subcommittee determined that RAGT is a safe technique that affords a level of effectiveness similar to that of palliative rehabilitation (quality of evidence: low).

Based on the findings of the 7th Health Technology Reassessment Committee in 2021 (July 9, 2021), the 8th Health Technology Reassessment Committee in 2021 (August 14, 2021) drew the following conclusions regarding RAGT for the rehabilitation of pediatric patients based on the subcommittee review and Article 4 Paragraph 10 of the Health Technology Reassessment Guideline: RAGT is a safe technique in patients with PD. In addition, RAGT is as effective as palliative rehabilitation, Therefore, the Health Technology Reassessment Committee conditionally recommends RAGT for the rehabilitation of pediatric patients (GRADE recommendation: conditional).
