자기공명영상 유도하 고강도초음파집속술 [뇌]

Abstract

평가 배경 자기공명영상 유도 하 고강도초음파집속술[뇌]는 자기공명영상 유도 하에 체외초음파를 정위적으로 고강도 집속하여 비침습적으로 표적 뇌 조직에 비가역적 병소를 만듦으로써 운동장애 증상을 개선하고 치료하는 기술이다. 동 기술은 내부모니터링을 통해 개발된 주제로, 관련 학회에서 사용량 증가가 예상되어 재평가가 필요한 기술이라는 의견과 함께 보건의료자원의 효율적 사용을 위한 근거제공을 목적으로 2020년 제8차 의료기술재평가위원회(2020.8.14.)에서 의료기술재평가 안건으로 선정되었다. 2021년 제7차 의료기술재평가위원회(2021.7.9.)에서는 자기공명영상 유도 하 고강도초음파집속술[뇌]의 안전성 및 유효성 평가 결과를 최종 심의하였다.

평가 방법 자기공명영상 유도 하 고강도초음파집속술[뇌]에 대한 안전성 및 유효성 평가는 업데이트 체계적 문헌고찰을 통해 수행하였다. 모든 평가방법은 평가목적을 고려하여 “자기공명영상 유도 하 고강도초음파집속술[뇌] 재평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회 는 신경외과 2인, 신경과 2인, 영상의학과 1인, 근거기반의학 1인 총 6인의 위원으로 구성하였다. 평가의 핵심질문은 ‘운동장애 환자를 대상으로 자기공명영상 유도 하 고강도초음파집속술을 통해 뇌의 회로를 정상화하여 질병을 치료하는 것이 안전하고 유효한가?’이었고, 안전성은 시술 관련 부작용 및 이상반응 지표로, 유효성은 임상증상 개선, 삶의 질 지표로 평가하였다. 문헌검색은 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림위험 평가는 무작위 배정 임상시험 연구(randomized controlled trial)는 Cochrane의 Risk of Bias (RoB)를, 비무작위연구(Non-randomized studies)는 Risk of Bias for Nonrandomized Studies (RoBANS Ver.2)를 각각 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 자료분석은 정량적 분석(quantitative analysis)이 불가능하여 정성적(qualitative review) 분석을 적용하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 GRADE 방법을 이용하여 근거 수준을 평가하고, 평가 결과를 토대로 권고등급을 결정하였다.

평가 결과 자기공명영상 유도 하 고강도초음파집속술[뇌]의 안전성과 유효성은 총 36편(본태성 떨림 환자 25편, 파킨슨병 환자 6편, 본태성 떨림 환자 및 파킨슨병 환자 4편, 기타 운동장애 환자 1편)에 근거하여 평가하였다.

본태성 떨림 환자 일시적 이상반응은 비교연구의 경우 뇌심부자극술(deep brain stimulation , DBS)와 비교한 연구(1편)에서 신경학적 이상반응인 감각이상, 보행 불안정, 구음장애, 정신상태 변화 등이 중재군 및 비교군 모두 보고되었다. 신체적 이상반응은 중재군(두통, 현기증/어지러움, 오심/구토, 핀 부위 화상)과 비교군(납 침식, 출혈)에서 다르게 보고되었다. 가짜 치료와 비교한 연구(1편)에서 신경학적 이상반응인 감각이상, 보행장애, 운동측정장애, 불균형 감각 등이 중재군과 비교군 모두 보고되었다. 신체적 이상반응인 오심, 따끔거림, 허리통증, 불안, 핀 부위 통증은 중재군 및 비교군 모두 보고되었으며, 중재군에서만 불편감, 현기증, 구토가 보고되었다. 임상증상 개선은 DBS와 비교한 연구(1편)에서 시술 12개월 후 중재군에서 개선된 정도가 양측 DBS 시행군에서의 개선된 정도보다 통계적으로 유의하게 열등하였다. 거짓 치료와 비교한 연구(1편)에서는 시술 이후 중재군에서만 임상증상이 개선되었다. 전후연구(12편)에서 Clinical Rating of Score for Tremor (CRST)로 임상증상 개선을 평가한 문헌 8편 중 6편에서 시술 전과 비교하여 마지막 추적관찰 시점에서 유의하게 개선되었다. 삶의 질은 DBS와 비교한 연구(1편)에서 중재군 및 양측 DBS군 모두 시술 이후 유의하게 삶의 질이 개선되었다. 거짓 치료와 비교한 연구(1편)에서는 시술 이후 중재군에서만 유의하게 삶의 질이 개선되었다. 전후연구 총 5편의 문헌 중 4편의 문헌에서 시술 전과 비교하여 통계적으로 유의하게 삶의 질이 개선되었다고 보고하였다.

파킨슨병 환자 시술 관련 부작용 및 이상반응을 보고한 비교연구는 확인되지 않았다. 4편의 전후연구에서 일시적 이상반응은 보행 실조 60% (1편), 행동 억제 20% (1편), 객관적 언어장애 14.9% (1편), 미각 감퇴증 14.3% (1편), 주관적 보행 불안정 14.3% (1편), 안면 비대칭 10% (1편) 등이 보고되었다. 시술 중 이상반응으로 운동장애 및 편마비 10% (1편), 입술 감각이상 14.3% (1편), 통증 13% (1편), 두통 42.9 ~ 100% (2편), 프레임 핀 고정 부위 통증 100% (1편), 요통 40% (1편), 현기증 28.6% (1편), 어지럼증 57.1% (1편)가 보고되었다. 영구적 이상반응은 위장 폐쇄로 인한 사망 1건(1편)을 포함하여 객관적 언어장애 19.4% (1편), 주관적 언어장애 3.2 ~ 10% (2편), 이상 운동증 14.3 ~ 33.3% (1편), 발성부전 9.7% (1편)가 보고되었다. 임상증상 개선과 삶의 질을 보고한 비교연구는 확인되지 않았다. 단일군 연구 8편 (모두 전후연구) 중 Unified Parkinson Disease Rating Scale (UPDRS)로 임상 증상을 평가한 결과 시술 전과 추적 관찰 이후 5편에서 통계적으로 유의하게 개선되었다. 단일군 연구 4편 (모두 전후연구) 중 Parkinson’s Disease Questionnaire (PDQ-39)로 삶의 질을 평가한 문헌 3편 중 2편에서, World Health Organization Quality of Life (WHOQOL) brief로 평가한 1편의 문헌(일부 지표만)에서 시술 전과 비교하여 추적관찰 이후 통계적으로 유의하게 개선되었다고 보고하였다.

기타 운동장애 환자 기타 운동장애(서경) 환자를 대상으로 자기공명영상 유도 하 고강도초음파집속술을 시행한 1편의 증례보고에서 중재시술 이후 혀의 따끔거림, 알루미늄 미각, 오른손과 보행의 어색함을 보고하였다.

결론 및 제언 따라서 소위원회에서는 현재 문헌적 근거를 바탕으로 다음과 같이 결과를 제시하였다. 소위원회에서는 본태성 떨림 환자를 대상으로 자기공명영상 유도 하에 고강도초음파[뇌]는 기존 기술인 뇌심부자극술과 비교하여 유효성이 비교우위에 있지 않다고 보고된 바도 있으나, 뇌심부자극술에서도 출혈, 감염 등 이상반응이 발생할 수 있고 초음파를 이용한 기술이 비침습적이라는 장점이 있다는 것을 고려할 때, 약물치료에 반응하지 않은 환자를 대상으로 시행을 고려할 수 있는 기술이라고 판단하였다(근거의 신뢰수준, Low). 소위원회에서는 파킨슨병 환자를 대상으로 자기공명영상 유도 하에 고강도초음파[뇌]는 기존 기술인 뇌심부자극술과 직접적으로 비교된 문헌이 없어 상대적인 유효성을 평가하기 어렵다는 점과 파킨슨병 환자에서 고강도초음파[뇌] 시술 후 사망을 포함하여 언어장애 등의 심각한 이상반응이 보고되고 있다는 점을 고려할 때, 효과 대비 안전성에 대한 근거가 부족한 기술로 평가하였다. 따라서, 고강도초음파[뇌]는 뇌심부자극술을 포함한 기존 기술의 적용이 불가한 환자에서 제한적으로만 고려될 수 있는 기술이라고 평가되었다(근거의 신뢰수준, Low). 소위원회에서는 기타 운동장애 환자를 대상으로 자기공명영상 유도 하에 고강도초음파를 정위적으로 집속하여 비침습적으로 표적 뇌 조직에 병소를 만듦으로써 운동장애 증상을 개선하고 치료하는 기술로, 현재 보고된 문헌으로는 근거가 충분하지 않아 안전성 및 유효성을 평가하기 어렵다는 의견이었다. 2021년 제7차 의료기술재평가위원회(2021.7.9.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “자기공명영상 유도 하 고강도초음파집속술[뇌]”에 대해 다음과 같이 심의하였다. 자기공명영상 유도 하 고강도초음파집속술[뇌]은 본태성 떨림 환자를 대상으로 뇌의 회로를 정상화하여 질병을 치료하는 자기공명영상 유도 하 고강도초음파집속술 [뇌]는 약물치료에 반응하지 않은 환자를 대상으로 시행을 고려할 수 있는 기술로 심의하였다(권고등급: 조건부 권고). 자기공명영상 유도 하 고강도초음파집속술[뇌]은 파킨슨병 환자를 대상으로 기 신의료기술평가 이후 보고된 문헌을 토대로 안전성 및 유효성을 평가하기에 충분하지 않다고 평가하였다(권고등급: 불충분). 자기공명영상 유도 하 고강도초음파집속술[뇌]은 기타 운동장애 환자를 대상으로 기 신의료기술평가 이후 보고된 문헌을 토대로 안전성 및 유효성을 평가하기에 충분하지 않다고 평가하였다(권고등급: 불충분).

Magnetic resonance imaging-guided high intensity focused ultrasound of the brain

Background MRI-guided High Intensity Focused Ultrasound (HIFU) [Brain] is a technique to improve and treat the symptoms of dyskinesia. An irreversible lesion is formed on the target brain tissue in a non-invasive way through the MRI-guided stereotactic focusing of in vitro high-intensity ultrasound. The technique was detected during internal monitoring; its use was predicted to increase in a relevant conference, and as it was determined to require reassessment and for the purpose of providing the evidence for an efficient use of public health care resources, the technique was selected as a target of Health Technology Reassessment by the 8th Health Technology Reassessment Committee in 2020 (August 14, 2020). The results for the safety and effectiveness of the technique MRI-guided HIFU [Brain] were reviewed and conclusions were drawn by the 7th Health Technology Reassessment Committee in 2021 (July 9, 2021).

Methods To assess the safety and effectiveness of MRI-guided HIFU, a systematic review of the current literature was performed. All assessment methods were designed, approved, and carried out by the MRI-guided HIFU Reassessment Subcommittee (hereinafter referred to as the subcommittee) through discussions about the purpose of the assessment. The subcommittee consisted of six members: two neurosurgeons, two neurologists, one radiologist, and one specialist in the field of evidence-based medicine. The key question of the assessment was the following: ‘is it safe and effective to treat patients with dyskinesia using MRI-guided HIFU to restore the functioning of brain circuits?’ Safety was assessed by examining procedure-related side effects and adverse reactions. Effectiveness was assessed by analyzing the level of improvement in clinical symptoms and quality of life (QoL). The articles were sourced from three overseas and five domestic databases. To minimize the risk of selection bias, two reviewers independently screened the available literature utilizing a predefined format, and selected relevant articles based on the designated inclusion and exclusion criteria. After screening, the two reviewers converged on a single conclusion, and in case of disagreement, a third party was appointed to help reach an agreement. The risk of bias was assessed using the Cochrane Risk of Bias for randomized controlled trials and the Risk of Bias for Nonrandomized Studies (Ver. 2) for non-randomized studies. As quantitative analysis was not possible, data were analyzed through qualitative review. The results of the systematic review were evaluated to estimate the level of evidence according to the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) method.

Results The safety and effectiveness of MRI-guided HIFU [Brain] were assessed based on 36 articles (25 articles on patients with essential tremor, 6 articles on patients with Parkinson’s disease, 4 articles on patients with essential tremor and patients with Parkinson’s disease, and 1 article on patients with a different type of dyskinesia).

Patients with essential tremor One study comparing MRI-guided HIFU and deep brain stimulation (DBS), which was utilized as the control condition, reported the occurrence of transient adverse neurological reactions (paresthesia, gait instability, dysarthria, and changes in mental state) for both the control and intervention groups. However, physical side effects were found to vary between the intervention group (headache, vertigo/dizziness, nausea/vomiting, and burn around the target point) and control group (skin erosion and hemorrhage). Clinical symptoms at the 12-month follow-up only slightly improved in HIFU-treated patients compared to DBS-treated patients. In addition, QoL was significantly improved in both intervention groups In another study comparing MRI-guided HIFU with placebo treatment, neurological adverse reactions (paresthesia, gait instability, dysmetria, and sense of imbalance) were reported for both the control and intervention group. Some physical adverse reactions (nausea, prickling, back pain, anxiety, and pain around the target point) were reported in patients in both the control and intervention groups, but only the latter group showed discomfort, vertigo, and vomiting. Only the HIFU-treated group showed improvements in clinical symptoms and in QoL. In 12 pretest-posttest studies, six out of eight reports that utilized the Clinical Rating Scale for Tremor (CRST) reported significant improvements in clinical symptoms on the last day of follow-up compared to the pre-treatment status.

Patients with Parkinson’s disease No comparative study has reported on HIFU-related side effects. However, in four pretest-posttest studies, the following transient adverse reactions were reported: ataxia (60%, n = 1), behavioral inhibition (20%, n = 1), objective aphasia (14.9%, n = 1), hypogeusia (14.3%, n = 1), subjective gait instability (14.3%, n = 1), and facial asymmetry (10%, n = 1). The reported procedure-related adverse reactions were dyskinesia and hemiplegia (10%, n = 1), paresthesia of the lips (14.3%, n = 1), pain (13%, n = 1), headache (42.9–100%, n = 2), pain around the target point (100%, n = 1), back pain (40%, n = 1), vertigo (28.6%, n = 1), and dizziness (57.1%, n = 1). These studies also reported lasting adverse effects including death due to gastrointestinal obstruction (n = 1), objective aphasia (19.4%, n = 1), subjective aphasia (3.2–10%, n = 2), dyskinesia (14.3–33.3%, n = 1), and hypophonia (9.7%, n = 1). No comparative study reported on clinical outcomes or QoL. In five out of eight uncontrolled studies, all of which had pretest-posttest design, clinical symptoms were examined using the Unified Parkinson Disease Rating Scale, and significant improvements were observed at follow-up. In four uncontrolled pretest-posttest studies examining QoL, significant improvements were reported in two out of three studies using the Parkinson’s Disease Questionnaire, and in one study that employed the World Health Organization Quality of Life-BREF assessment.

Patients with a different type of dyskinesia One case report of MRI-guided HIFU outlined the case of a patient with chirospasm with post-operative prickling of the tongue, taste of aluminum, and awkwardness in the right hand and gait.

Conclusions and recommendations Based on the level of evidence obtained from the systematic review, the subcommittee suggests the following: For patients with essential tremor, MRI-guided HIFU of the brain has been reported to be more effective than the more conventional DBS. In addition, considering the possibility of adverse reactions following DBS, such as hemorrhage and infection, as well as the non-invasiveness of HIFU, this technique is applicable for patients who are unresponsive to drug therapy (quality of evidence: low). For patients with Parkinson’s disease, there is not sufficient evidence to assess the safety of MRI-guided HIFU of the brain. No article directly compared the technique with the more conventional DBS, which prevents the evaluation of relative effectiveness; in addition, severe adverse reactions including death and aphasia were reported for patients with Parkinson’s disease after HIFU. Thus, MRI-guided HIFU [Brain] is a technique whose application should be limited to patients for whom conventional techniques cannot be applied (quality of evidence: low). For patients with a different type of dyskinesia, it is difficult to assess the safety and effectiveness of MRIguided HIFU [Brain] as a non-invasive method to improve and treat the symptoms of dyskinesia through MRI-guided stereotactic focusing of in vitro high-intensity ultrasound on the preformed target brain lesion, due to the lack of sufficient evidence across the published article.

The 7th Health Technology Reassessment Committee in 2021 (July 9, 2021) draw the following conclusions regarding MRI-guided HIFU [Brain] based on the subcommittee review and the Article 4, Paragraph 10 of the Health Technology Reassessment Guideline: MRI-guided HIFU [Brain] is a technique applicable to patients with essential tremor who are unresponsive to drug therapy. The technique can be used to restore the normal brain circuit in these patients (Grade of Recommendation: Conditional). MRI-guided HIFU [Brain] is a technique whose safety and effectiveness cannot be assessed for patients with Parkinson’s disease based on the articles published since the previous New Health Technology Assessment (Grade of Recommendation: Insufficient). MRI-guided HIFU [Brain] is a technique whose safety and effectiveness cannot be assessed for patients with dyskinesia based on the articles published since the previous New Health Technology Assessment (Grade of Recommendation: Insufficient).
