사시수술

Abstract

평가 배경 사시수술은 내사시 또는 외사시 등 환자의 사시교정을 목적으로 수행하는 수술로 2009년 보건복지부 고시에 의해 등재급여 또는 선택비급여로 사용되고 왔다. 10세 이하 소아의 사시교정 및 10세 이후 의학적 기능개선을 위해 수행되는 사시수술 경우는 국민건강보험상 급여로, 그 외에는 미용적 개선 목적을 가지는 경우로 보고 선택비급여로 분류된다. 동 기술은 우리 원의 내부 모니터링 활동에 의하여 발굴되었으며, 2022년 제3차 의료기술재평가위원회 (2022.03.11.)에서 안전성 및 효과성에 대한 과학적 근거검토 및 사회적 가치평가를 통해 의료기술의 적정사용 등의 정책적 의사결정을 지원하고자 하는 목적의 의료기술재평가 안건으로 선정되었다. 동 기술의 평가대상 범위는 외모개선 목적의 미용사시수술 영역보다는 등재급여 영역 중 의학적 기능개선에 해당하는지에 대한 혼선이 있을 수 있는 영역에서 사시수술의 안전성, 효과성의 근거평가가 필요하다는 소위원회 의견에 따라 ‘10세 이후에 발현된 복시(간헐적 복시 포함) 또는 이상두위를 동반한 일차사시 환자’로 설정하였다.

평가 방법 동 기술에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “사시수술 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 평가의 핵심질문은 “10세 이후에 발현된 복시(간헐적 복시 포함) 또는 이상두위를 동반한 일차사시 환자 대상 사시수술은 임상적으로 안전하고 효과적인가?”이며, 안전성은 과교정, 수술 관련 합병증 및 이상반응 지표로, 효과성은 시기능 개선(사시각 편위 변화, 복시 또는 이상두위 개선, 입체시 향상, 수술성공률), 재발률(또는 재수술률), 삶의 질(또는 환자 만족도) 지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하여 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 선택된 문헌이 모두 전후연구였으므로, 비뚤림위험 평가는 Risk of Bias Assessment tool for Non-randomized Studies (RoBANS Ver.2)를 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료분석은 양적 분석이 불가하여 질적 검토(qualitative review) 방법을 적용하였다. 체계적 문헌고찰 결과의 근거 수준은 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 접근 방법으로 평가하였다.

평가 결과 동 평가에 포함된 문헌은 총 17편(대상자 수 총 578명)의 전후연구였으며, 안전성 및 효과성의 결과를 정리하면 다음과 같다. 안전성 사시수술 관련 안전성은 과교정 여부, 수술 관련 합병증 및 이상반응으로 살펴보았다. 과교정을 보고한 총 5편 연구 중 1편(Wright et al., 2017)에서 대상자 중 7.1% (2/28)에서 평균 7.5 프리즘디옵터(Prism Dioptor, PD) 수준의 과교정이 보고되었고, 그 외 4편에서는 과교정이 발생하지 않은 것으로 보고하였다. 수술 관련 합병증 및 이상반응을 보고한 3편의 연구 모두 합병증 및 이상반응이 발생하지 않았다고 제시하였다. 효과성 사시수술의 효과성은 시기능 개선 여부(사시각 편위 변화, 복시 또는 이상두위 개선 여부, 입체시 향상 여부, 수술성공률), 재발률 또는 재수술률, 그리고 삶의 질 또는 환자만족도를 지표로 평가하였다. 이 중 시기능 개선은 사시각 편위 변화, 복시 또는 이상두위 개선 여부, 입체시 향상 여부, 그리고 이러한 사항들을 단독 또는 종합하여 정의내린 수술성공률로 다시 나누어 살펴보았다. 첫 번째 효과성 지표인 시기능 개선 중 사시각 편위 여부를 살펴본 연구는 총 15편으로 내사시를 다룬 14편 중 3편에서 수술 전후 편위가 통계적으로 유의하게 감소하였고, 나머지 연구들도 수술 후 정위 수준에 해당하는 약 4PD 이하로 편위가 감소한 것을 확인하였다. A-V형 사시를 다룬 연구 1편에서는 수술 전후 수직불일치의 유의한 감소를 보고하였다. 사시와 동반한 복시 또는 이상두위의 개선을 다룬 연구 총 16편 중 15편이 내사시에 동반된 복시 또는 간헐적 복시 환자를 대상으로 하였다. 1편의 연구에서 복시 설문(DQ)상 수술 전후 증상개선 효과를 확인하였고, 나머지 14편에서도 복시증상이 있던 대상자의 75~100%에서 증상개선을 보고하였다. 나머지 1편은 A-V형 사시에 동반된 이상두위 환자 중 86.2%에서 증상이 소실됨을 보고하였다. 눈의 고도의 융합기능인 입체시 향상 여부를 제시한 5편의 연구 중 2편에서 수술 전 입체시 측정이 불가하였던 대상자들이 수술 후 모두 입체시를 회복하였고, 나머지 3편에서도 대상자 전체 또는 다수가 일정 수준 이상의 우수한 입체시를 획득한 것으로 제시하였다. 이상의 내용들을 단독 또는 복합적으로 반영하여 각 연구별로 별도 정의내린 수술성공률을 6편의 연구에서 75~100%로 보고하였다. 두 번째 효과성 지표는 재발률 또는 재수술률로 총 8편의 연구에서 추적관찰 기간 중 증상 재발현 또는 증상이 잔존하여 추가 중재가 필요한 경우를 3.5~37.5%로 보고하였다. 상세하게는 3편의 연구에서 내사시 재발이 3.5~11.1%, 다른 5편의 연구에서는 복시 또는 내사시가 지속된 경우가 10.5~37.5%이었다. 수술 후 3개월 시점에서 복시여부를 보고한 연구에서 대상자 중 17.2%에서 복시가 확인된 것으로 보고하였다. 이상의 모든 재발 대상자에 대하여 프리즘 치료 또는 추가 수술을 적용한 것으로 보고하였고, 추가 조정을 위한 재수술률은 5편의 연구에서 3.5~14.3%이었다. 세 번째 효과성 지표인 삶의 질 또는 환자만족도는 1편의 연구에서 사시수술 후 삶의 질이 유의하게 향상된 결과를 보고하였다.

결론 및 제언 ‘10세 이후에 발현된 복시(간헐적 복시 포함) 또는 이상두위를 동반한 일차사시 환자’에서 사시수술은 과교정 등 수술 관련 합병증 및 이상반응 발생 사례가 아주 적거나 발생하지 않아 안전한 기술로 평가하였다. 효과성은 사시수술 후 사시각 편위가 유의하게 감소하였거나, 정위에 해당하는 수준이 됨을 확인하였고, 주요 동반 증상인 복시 또는 이상두위의 개선 효과는 물론 눈의 고도의 융합기능에 해당하는 입체시 개선, 이상의 내용을 반영한 수술성공률 모두에서 시기능 개선효과를 확인하였다. 그리고 사시 재발률(또는 재수술률)은 환자의 특성을 고려할 때 증상 지속 또는 재발 사례가 용인 가능한 수준이고, 사시수술로 인한 삶의 질(또는 환자만족도) 개선효과를 확인하였다. 그리고 평가에 포함된 연구들이 단일군 전후비교연구 수준이고, 대상자수가 적은 약한 근거에서 도출된 결과라는 점은 수술과 동일선상에서 비교할 만 한 대체기술이 없고, 특정 조건의 대상자로 한정하여 평가한 경우이므로 대상자 수가 적을 수밖에 없어 이상의 결과를 통하여 사시수술을 효과적인 기술로 평가하였다. 2022년 제12차 의료기술재평가위원회(2022.12.9.)에서는 소위원회 검토 결과에 대하여 논의한 결과, 의료기술재평가사업 관리지침 제4조제10항에 의거 “사시수술”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 ‘10세 이후에 발현된 복시(간헐적 복시 포함) 또는 이상두위를 동반한 일차사시 환자를 대상으로 한 사시수술’은 안전하고 효과적이라는 소위원회 검토 결과에 대하여 연령제한 없이 해당 증상을 동반한 일차사시에서 사시수술을 안전하고 효과적인 기술로 판단하였다. 이에 따라 의료기술재평가위원회는 ‘복시(간헐적 복시 포함) 또는 이상두위를 동반한 일차사시 환자를 대상으로 한 사시수술’을 ‘권고함’으로 심의하였다(권고등급: 권고함).

Background Strabismus surgery, which is performed to correct strabismus (including esotropia and exotropia), is a listed benefit or selective non-benefit item, according to a 2009 notice from the Ministry of Health and Welfare. Strabismus surgery performed to correct strabismus in children aged up to 10 years and improve medical function in children aged less than 10 years is recognized as a covered benefit under the national health insurance, whereas all other cases are classified as elective procedures for cosmetic improvement and are not covered by insurance. This technology was identified through our institution’s internal monitoring, and during the third session of the Committee of Health Technology Reassessment in 2022 (March 11, 2022), it was selected as a health technology reassessment agenda item to support policy decision-making, such as the appropriate use of this health technology, by reviewing the scientific evidence regarding its safety and effectiveness and assessing its social value. In accordance with the subcommittee’s opinion that there is a need to assess the evidence of the safety and effectiveness of strabismus surgery in areas where there may be some confusion regarding whether the purpose of the surgery qualifies as improvement of medical function as a listed benefit (as opposed to cosmetic strabismus surgery for the purpose of improving appearance), the scope of assessment for this technology was set as “patients who have primary strabismus and diplopia (including intermittent diplopia) or abnormal head posture that began after the age of 10 years.”

Methods A systematic review was conducted to assess the safety and effectiveness of this technology. All assessment methods were finalized after a review by the “strabismus surgery subcommittee” (hereinafter referred to as the “subcommittee”) with consideration of the assessment’s purpose. The key question in the assessment was, “Is strabismus surgery clinically safe and effective in patients who have primary strabismus and diplopia (including intermittent diplopia) or abnormal head posture that began after the age of 10 years?” Safety was assessed based on overcorrection, surgery-related complications, and adverse reactions (ARs) indicators, while effectiveness was assessed based on improvement in visual function (change in strabismus angle deviation, improvement in diplopia or abnormal head posture, improvement in stereoacuity, and rate of surgical success), the rate of recurrence rate or reoperation, and quality of life or patient satisfaction indicators. In the systematic review, three international and five domestic databases were searched based on the key question. Two reviewers independently screened and selected the articles (from pretest-posttest studies) in accordance with the inclusion and exclusion criteria, and then independently assessed the risk of bias in each article (RoB) using the Risk of Bias Assessment tool for Non-randomized Studies (RoBANS Ver.2) and reached a consensus. Data were qualitatively analyzed since a quantitative analysis was not possible. The level of evidence was assessed using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) method.

Results A total of 17 articles (total number of participants: 578), all from pretest-posttest studies, were included in the assessment. The safety and effectiveness results are summarized below.

Safety The safety of strabismus surgery was assessed based on overcorrection, surgery-related complications, and ARs. Among the five articles that reported overcorrection, one (Wright et al., 2017) reported a mean overcorrection of 7.5 prism diopters (PD) in 7.1% (2/28) of participants. The remaining four articles reported no overcorrections. All three articles that mentioned surgery-related complications and ARs reported no incidence of complications or ARs.

Effectiveness The effectiveness of strabismus surgery was assessed based on improvement in visual function (change in strabismus angle deviation, improvement in diplopia or abnormal head posture, improvement in stereoacuity, and rate of surgical success), rate of recurrence or reoperation, and quality of life or patient satisfaction indicators. Of these, improvement in visual function was examined separately. Regarding the first effectiveness indicator (i.e., improvement in visual function), strabismus angle deviation was reported in 15 articles. Among the 14 articles on esotropia, three reported a statistically significant postoperative decrease, while the remaining studies also reported a postoperative decrease in deviation to orthophoria of approximately ≤ 4 PD. In addition, one article on A-V pattern strabismus reported a significant postoperative decrease in vertical transposition. Among the 16 articles on improvement in strabismus with diplopia or abnormal head posture, 15 were on patients with esotropia and diplopia or intermittent diplopia. One article reported postoperative symptom improvement based on a diplopia questionnaire, while the remaining 14 reported postoperative improvement in 75–100% of patients with diplopia symptoms. The remaining article reported that symptoms resolved in 86.2% of patients with A-V pattern strabismus and abnormal head posture. Of the five articles that reported on improvement in stereoacuity, a highly integrated ocular function, two reported that patients in whom preoperative stereoacuity could not be measured showed postoperative recovery of stereoacuity, while the remaining three reported that all or most patients achieved excellent postoperative stereoacuity above a certain level. The surgical success rate, as defined in each study by either a single factor or combination of factors, was reported as 75–100% in six articles. The second effectiveness indicator was the recurrence or reoperation rate. Eight articles reported that additional intervention was needed in 3.5–37.5% of cases due to symptom recurrence or residual symptoms during the follow-up period. Specifically, three articles reported that esotropia recurrence was found in 3.5–11.1% of cases, while five other articles reported that diplopia or esotropia persisted in 10.5–37.5% of cases. In a study that reported on postoperative diplopia at three months, diplopia was confirmed in 17.2% of participants. The articles reported that prism therapy or additional surgery was applied to all patients who had recurrence, and the reoperation rate for additional correction was reported to be 3.5–14.3% in five articles. The third effectiveness indicator, quality of life or patient satisfaction, was reported in one article, which reported significantly enhanced postoperative outcomes.

Conclusions and recommendation Strabismus surgery for “patients who have primary strabismus and diplopia (including intermittent diplopia) or abnormal head posture that began after the age of 10 years” was assessed to be a safe health technology since very few or no cases of surgery-related complications (such as overcorrection) and ARs were found. Regarding its effectiveness, a postoperative improvement in visual function was observed as follows: a significant decrease in strabismus angle deviation or recovery to orthophoria; an improvement in major comorbidity diplopia or abnormal head posture; an improvement in stereoacuity, which is a highly integrated ocular function; and a surgical success rate accounting for the above. Regarding the rate of strabismus recurrence or reoperation, the rate of symptom persistence or recurrence cases was within acceptable levels considering the patients’ characteristics, and a postoperative improvement in quality of life or patient satisfaction was also observed. Moreover, the results were derived based on weak evidence from a small sample size; the studies assessed were single-group pretest-posttest studies due to the unavailability of comparative alternatives to the surgery; and the participants were limited to only those who satisfied specific conditions. Considering the above, strabismus surgery was considered an effective health technology. During the twelfth session of the Committee of Health Technology Reassessment in 2022 (December 9, 2022), the committee discussed the subcommittee’s findings and reached the following conclusions on strabismus surgery in accordance with Article 4-10 of the Guidelines for Management of Health Technology Reassessment Project. Based on the subcommittee’s findings that strabismus surgery for “patients who have primary strabismus and diplopia (including intermittent diplopia) or abnormal head posture that began after the age of 10 years” is safe and effective, the Committee of Health Technology Reassessment determined that strabismus surgery is a safe and effective health technology for patients with primary strabismus and applicable symptoms without any age restriction. Accordingly, the Committee of Health Technology Reassessment “recommends” strabismus surgery for patients who have primary strabismus and diplopia (including intermittent diplopia) or abnormal head posture (grade of recommendation: recommended).
