반복 경두개자기자극술

Abstract

평가 배경 반복 경두개자기자극술(Repetitive Transcranial Magnetic Stimulation, 이하 rTMS)은 두부 가까이에 전도전자기 코일을 놓고 강력한 전류파를 흘려서 생긴 자기장이 두개골을 통과하여 두뇌피질 신경세포의 활성화 및 억제를 반복적으로 유도하는 의료기술로 현재 두뇌자극 방법 중 가장 비침습적인 시술이다. 경두개자기자극술은 신의료기술평가제도가 도입되기 이전인 2002년 10월에 행위비급여로 건강보험에 등재(보건복지부고시 제2002-68호, 2002.9.27.)되어 임상에서는 우울증을 비롯한 정신신경계 질환자에게 치료 목적으로 사용되고 있다. 본 기술은 내부모니터링을 통해 발굴된 주제로 우선순의 심의를 통해 재평가 대상으로 선정되었으며, 2022년도 제3차 의료기술재평가위원회(2022.3.11.)에서 체계적 문헌고찰을 통해 임상적 안전성 및 효과성에 대한 최신 근거를 확인하여 의료기술의 적정 사용을 지원하고자 재평가를 수행하였다. 또한, 건강보험요양급여비용 목록에 ‘경두개자기자극술’이라는 의료기술명으로 등재되어 있으나 임상에서 치료중재로써 재평가 목적에 맞게 평가기술명을 ‘반복 경두개자기자극술(Repititive transcranial magnetic stimulation)’로 수정하였다

평가 방법 반복 경두개자기자극술에 대한 안전성 및 효과성을 평가하기 위해 체계적 문헌고찰을 수행하였고 국민 참여단으로부터 사회적 가치를 평가하였다. 모든 평가방법은 평가목적을 고려하여 “반복 경두개자기자극술 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였고 소위원회는 총 6인(정신건강의학과 3인, 신경과 2인, 근거기반의학 1인)으로 구성하였다. 평가의 핵심질문은 ‘반복 경두개자기자극술은 약물치료 중인 주요 우울장애 성인환자에서 임상적으로 안전하고 효과적인가?’이었고, 안전성은 경련, 기억력 장애, 체내 삽입된 이식기기 문제 초래 등의 지표로, 효과성은 우울증상 평가지표(Hamilton Depression Rating Scale, HDRS) 변화, 우울증상 반응률(Response rates), 관해율(Remission rates), 재발률(Relapse rates)로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하여 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림위험 평가는 Cochrane의 Risk of Bias (RoB)를 사용하여 안건 담당자가 독립적으로 수행하여 의견합의 및 소위원회의 검토를 진행하였다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하였으며, 자료분석은 정량적 분석(quantitative analysis)을 진행하고 불가능한 경우에는 정성적(qualitative review) 분석으로 기술하였다. 체계적 문헌고찰 결과의 근거 수준은 Grading of Recommendations Assessment, Development and Evaluation (GRADE)를 적용하여 평가하였다. 또한, 국민참여단을 대상으로 사회적 가치평가는 온라인 설문조사를 통해 4개의 평가영역을 평가하였다.

평가 결과 약물치료 중인 주요 우울장애환자에서 반복 경두개자기자극술의 안전성과 효과성은 무작위배정 임상시험 연구를 대상으로 31편의 문헌(30개 연구; 국내 2개, 국외 28개)이 확인되었다. 약물치료의 병용치료로서 반복 경두개자기자극술 중재군과 Sham(모의대조군)을 비교한 연구는 27개이었으며, 병용치료로서 반복 경두개자기자극술 중재군과 전기경련치료군을 비교한 연구는 4편으로 대조군을 구분하여 평가하였다. 비뚤림 위험 평가결과 ‘무작위 배정순서 생성’은 다수의 연구에서 비뚤림 위험이 낮게 평가되었으나 ‘배정순서 은폐’는 명확히 제시되지 않아 비뚤림 위험이 약 70%의 연구에서 불확실로 확인되었다. 실행 비뚤림과 관련한 ‘연구참여자, 연구자에 대한 눈가림’은 약 80% (21편)의 연구에서 눈가림을 시행하지 않아 비뚤림 위험이 높거나 명확히 언급되지 않아 불확실로 나타났다. 또한, ‘결과 평가에 대한 눈가림’, ‘불완전한 결과 자료’는 비뚤림 위험이 대부분 낮거나 불확실로 평가되었으며, 민간연구비 지원’은 비뚤림 위험이 높다고 평가된 2편을 제외하고는 낮거나 불확실로 확인되었다. 또한, GRADE를 통한 근거수준은 우울증상 평가지표의 변화와 우울증상 반응률은 비뚤림 위험에서 불확실이 다수 확인되고 연구결과 간의 이질성이 높아 비일관성의 사유로 근거 수준은 낮음으로 평가되었으며, 관해율과 재발률은 중등도로 평가되었다.

안전성 반복 경두개자기자극술의 안전성은 총 30개 연구 중 20개 연구를 대상으로 평가하였으며, 핵심적인 결과지표인 경련, 기억력 장애, 체내 삽입된 이식기기로 인한 문제 발생과 부작용은 관찰되지 않았다. 또한 본 기술의 안전성과 관련된 가이드라인에서는 체내 이식기기를 가지고 있는 환자를 본 기술의 금기증으로 제시하고 있어 대부분의 연구에서 연구대상자에서 배제하였다. 그 외 안전성과 관련하여 13편의 문헌에서 두통을 포함한 통증 발생을 보고하였으나 중재군과 모의대조군 간에 통계적으로 유의한 차이는 없었으며(rTMS 14.7% vs Sham 15.0%, Risk Ratio (RR) 1.14, 95% Confidence Interval (CI) 0.77~1.67), 일부 문헌에서는 치료 동안 경감되거나 회복이 되었다고 보고하였다.

효과성 체계적 문헌고찰을 수행한 결과, 약물치료 중인 주요 우울장애환자에서 반복 경두개자기자극술의 추가 치료효과(Add-on treatment effect)를 확인하고자 총 31편(30개 연구)의 무작위배정 비교임상시험을 선정하여 모의대조군과 전기경련치료군으로 대조군을 구분하여 효과성을 분석하였다. 16개의 무작위배정 비교임상시험의 메타분석 결과, 기존 약물치료를 받고 있는 주요 우울장애환자에서 반복 경두개자기자극술을 병용치료할 경우, 모의대조군 병용치료보다 우울 평가지표(16개 RCT)가 유의하게 개선이 되었으며(MD 3.62, 95% CI 1.69~5.54), 우울 치료반응률(15개 RCT)도 유의하게 개선되었다(RR 2.37, 95% CI 1.44~3.92). 그러나 우울 평가지표와 치료반응율의 메타분석 결과는 I2 통계량이 75%를 초과하여 높은 이질성을 보였다. 반면, 약물치료와의 병용치료로서 반복 경두개자기자극술(10개 RCT) 후의 우울관해율은 모의대조군과 유의한 차이는 없었으며(RR 1.54, 95% CI 0.97~2.42, I2 75%), 재발률(3개 RCT)도 두 군간 유의한 차이가 없었다(RR 0.55, 95% CI 0.26~1.16, I2 39%) 약물치료 중인 주요 우울장애환자를 대상으로 반복 경두개자기자극술과 전기경련치료법 간의 무작위배정 비교임상연구를 수행한 연구는 4개이었다. 메타분석한 결과(2개), 전기경련치료법은 반복 경두개자기자극술보다 우울 평가지표의 개선 효과가 유의하게 높았으며(MD -4.19, 95% CI –6.51~-1.86, I2 0%), 관해율도 전기경련치료법이 유의하게 높았다(RR 0.35, 95% CI 0.35~0.81, I2 0%).

결론 및 제언 체계적 문헌고찰 결과, 약물치료 중인 주요 우울장애 환자에서 반복 경두개자기자극술의 추가치료 시 임상적으로 중대한 이상반응이나 부작용은 보고되지 않았고 일부 문헌에서 시술 관련 이상반응을 보고하였다. 해당 통증은 치료기간에 경감되거나 치료중단 및 임상적 추가 치료없이 치료동안 회복된 것으로 확인되어 소위원회는 주요 우울장애환자에서 해당 의료기술은 안전한 의료기술로 평가하였다. 우울증상 평가지표 개선과 우울 치료반응률에서는 반복 경두개자기자극술 병용중재군이 모의대조군 병용치료보다 유의하게 효과적이었으나, 그 외 결과지표인 관해율과 재발률에서는 두 군간 유의한 차이가 없었다. 소위원회에서는 선택문헌에서의 추적관찰시점이 6개월 이내로 우울증의 심상적 관해 및 재발을 확인하기에는 너무 짧고 이를 보고한 문헌도 3편으로 임상적 근거로는 부족하다고 평가하였다. 또한, 반복 경두개자기자극술의 중재효과를 확인하기 위해서는 표준 프로토콜의 이행과 충분한 경과 관찰기간이 필요하다는 의견을 제시하였다. 추가적으로 소위원회는 연구대상자의 특성과 중재법의 유형, 추적 관찰기간의 차이, 결과지표의 다양성 등에 따라 결과의 이질성이 높고, 포함된 문헌의 근거수준이 높지 않아 평가결과에 대한 해석에 주의가 필요할 것으로 판단하였다. 또한, 반복 경두개자기자극술과 전기경련치료법 간 효과성의 경우, 10년 이전에 수행된 무작위배정 비교임상연구의 결과로서 최근에는 반복 경두개자기자극술과 전기경련치료법 간의 직접 비교연구를 수행하지 않아 임상적 효과성의 최신 근거로는 적절하지 않으며, 근거의 양이 부족하다고 판단하였다. 따라서, 소위원회에서는 본 평가결과를 바탕으로 반복 경두개자기자극술은 안전하고 비침습적인 의료기술로서 약물치료 중인 주요 우울장애환자에게 추가치료 시 그렇지 않은 군에 비해 우울관해율 및 재발율과 같은 장기 효과를 확인하기에는 근거가 불충분하다고 평가하였다. 다만 국내 임상상황과 치료가이드라인을 고려하여 기존 약물치료에 반응하지 않는 치료저항성 환자에서 반복 경두개자기자극술이 임상적 치료대안으로 적용 가능한 보조요법의 하나로 판단하였다. 2022년 제12차 의료기술재평가위원회(2022.12.9.)에서는 소위원회 결론과 안건명 변경 의견에 동의하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “반복 경두개자기자극술”에 대해 다음과 같이 심의하였다. 반복 경두개자기자극술은 약물치료 중인 주요 우울장애 환자에게 국소 자기장을 이용하여 두뇌피질 자극을 통해 신경세포를 활성화 및 억제를 유도하는 비침습적인 의료기술로서 일시적 부작용은 있으나 영구적인 중대한 이상반응은 확인되지 않아 안전한 의료기술로 평가하였다. 또한, 약물치료 중인 주요 우울장애 환자에서 반복 경두개자기자극술의 추가치료는 병용치료를 하지 않은 군에 비해 우울증상 평가지표 및 치료반응률이 유의하게 개선되었으나, 장기 치료효과를 확인하기 위한 관해율 및 재발률에 대한 근거는 불충분하다고 판단하였다. 다만, 약물치료에 불응하는 주요 우울장애 환자의 치료대안인 전기경련치료를 최종적으로 적용하기 전 단계에서는 항우울제를 중단할 수는 없고 추가적인 치료대안이 없는 임상적 상황을 고려하여 반복 경두개자기자극술은 약물치료와 병용하여 사용 가능한 의료기술로 평가하였다. 이에 따라 의료기술재평가위원회는 기존 약물치료를 받는 주요 우울장애 환자에서 우울증상을 개선하기 위해 반복 경두개자기자극술을 ‘조건부 권고함’으로 심의하였다.

Background Repetitive transcranial magnetic stimulation (rTMS) is a type of health technology in which an electromagnetic coil is placed close to the cranium to create a flow of a powerful current wave, and the magnetic field generated penetrates through the skull to repeatedly induce the activation and inhibition of cortical nerve cells. It is currently the most non-invasive brain stimulation method. TMS was registered as a non-benefit item in the health insurance system in October 2002 prior to the implementation of the New Health Technology Assessment System (Ministry of Health and Welfare Notice No. 2002-68; November 27, 2002) and has been used for therapeutic purposes in patients with psychoneurological disorders, including depression. This technology was an item identified through internal monitoring and subsequently selected for reassessment by priority review. During the third session of the Committee of Health Technology Reassessment in March 11, 2022, the decision was made to perform a reassessment to support the appropriate use of this health technology by identifying the latest evidence on its clinical safety and effectiveness through a systematic review. In addition, although this technology is registered under the name “transcranial magnetic stimulation” in the national health insurance medical fee schedule, the agenda title was revised to “repetitive transcranial magnetic stimulation” in accordance with the purpose of health technology reassessment as a therapeutic intervention used in clinical practice.

Methods A systematic review was performed to assess the safety and effectiveness of rTMS, and its social value was assessed via a public participation group. All the assessment methods were finalized after a review by the “rTMS subcommittee” (subcommittee) with consideration for the purpose of the assessment. The subcommittee was composed of six experts – three from psychiatry, two from neurology, and one from evidence-based medicine. The key question in the assessment was “Is rTMS clinically safe and effective for adult patients on medication for major depressive disorder?” Safety was assessed based on indicators such as convulsion, memory impairment, and problems with implant devices, while effectiveness was assessed based on changes in the depression assessment scale (Hamilton Depression Rating Scale; HDRS), response rates, remission rates, and relapse rates. For the systematic review, three international and five domestic databases were searched based on the key question, and two reviewers independently screened and selected the articles in accordance with the inclusion and exclusion criteria. Risk of bias (RoB) assessment was performed independently by the reviewer using the Cochrane RoB tool for consensus building and subcommittee review. Data extraction was performed using a predetermined format. The data were analyzed using quantitative analysis, but when this was not possible, qualitative review was used. The systematic review results were assessed for level of evidence using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) method. In addition, an online questionnaire survey was conducted on a public participation group to assess four domains of social value.

Results The safety and effectiveness of rTMS for patients on medication for major depressive disorder were assessed based on 31 articles from 30 randomized clinical trials (RCTs; 2 Korean and 28 international RCTs). Twenty-seven RCTs compared rTMS with sham, and four RCTs compared rTMS with electroconvulsive therapy (ECT). In all studies, patients received combination medication therapy with antidepressant. In the RoB assessment, the RoB for “randomization” was assessed to be low in many studies. Meanwhile, the RoB for “allocation concealment” was assessed to be uncertain in approximately 70% of the studies since allocation concealment was not clearly presented in those studies. With respect to performance bias, the RoB for “blinding of participants and personnel” was assessed to be high since such blinding was not applied in approximately 80% of the studies (n=21) or the RoB was assessed to be uncertain in studies that did not clearly mention such blinding. Moreover, the RoB for “blinding to outcome assessment” and “incomplete outcome data” was assessed to be mostly low or uncertain, while the RoB for “private research funding” was assessed to be low or uncertain except in two studies in which the RoB was assessed to be high. Furthermore, with respect to the level of evidence based on the GRADE method, changes in HDRS and response rates were assessed to have low levels of evidence due to many studies with uncertain RoB and high heterogeneity between study results. Meanwhile, remission and relapse rates were assessed to have moderate levels of evidence.

Safety In 20 out of 30 studies that assessed the safety of rTMS, there were no adverse events (AEs) and no occurrence of key outcome indicators (convulsion, memory impairment, and problems with implant devices). Moreover, guidelines related to the safety of this technology lists implant devices as a contraindication; thus, most studies excluded patients with an implant device. With respect to other safety-related issues, 13 studies reported the occurrence of pain, including headache, but there was no statistically significant difference between the intervention and sham control groups (rTMS, 14.7% vs. Sham, 15.0%; risk ratio [RR], 1.14; 95%, confidence interval [CI], 0.77~0.67). Some studies reported pain reduction or recovery during the treatment period.

Effectiveness Based on the systematic review results, 31 articles (30 RCTs) were selected to analyze the effectiveness compared to that of the controls (shame and electroconvulsive therapy) to identify the add-on treatment effect of rTMS in patients on medication for major depressive disorder. A meta-analysis of 16 RCTs showed that combination therapy with rTMS in these patients was significantly more effective in improving depression assessment scale scores (16 RCTs; mean difference [MD], 3.62; 95% CI 1.69~5.54) and response rates (15 RCTs; RR, 2.37; 95% CI, 1.44~3.92) as compared to combination therapy with sham control. However, the studies included in this meta-analysis showed high heterogeneity with I2 statistics >75%. On the other hand, rTMS combined with drug therapy showed no significant difference in remission rates (10 RCTs; RR, 1.54; 95% CI, 0.97~2.42; I2, 75%) and relapse rates (3 RCTs; RR, 0.55; 95% CI, 0.26~1.16; I2, 39%) as compared to combination therapy with sham control. There were four RCTs that compared rTMS and electroconvulsive therapy in patients on medication for major depressive disorder. A meta-analysis of two RCTs showed that electroconvulsive therapy was significantly more effective in improving depression assessment scale scores (MD, -4.19; 95% CI, –6.51~-1.86; I2, 0%) and remission rates (RR, 0.35; 95% CI, 0.35~0.81; I2, 0%) as compared to rTMS.

Conclusions and recommendations The systematic review results showed that no clinically serious adverse reactions (ARs) or AEs were observed when rTMS was used as an add-on treatment in patients on medication for major depressive disorder, while some articles reported procedure-related ARs. Pain was reduced during the treatment period or recovery from pain was achieved upon the discontinuation of treatment and without any additional clinical treatment. Accordingly, the subcommittee assessed this health technology to be safe for patients with major depressive disorder. With respect to an improvement in depressive symptom indicators and response rates, combined intervention with rTMS was significantly more effective than was sham control, but there was no significant difference in other outcome indicators (remission and relapse rates) between the two groups. The subcommittee assessed that the clinical evidence was insufficient owing to a follow-up period of less than six months, which was too short to clearly identify the clinical remission and relapse rates of depression, and since there were only three articles that analyzed this. The subcommittee also opined that the implementation of a standard protocol and sufficient follow-up observation period are needed to verify the intervention effect of rTMS. In addition, the subcommittee determined that caution is needed in interpreting the assessment results because of high heterogeneity due to differences in participant characteristics, types of interventions, length of follow-up period, and diversity of outcome indicators, as well as the fact that the articles included in the analysis had low levels of evidence. Moreover, a comparison of effectiveness between rTMS and electroconvulsive therapy was based on results from an RCT conducted more than 10 years earlier, and there were no recent studies on a direct comparison between rTMS and electroconvulsive therapy. Accordingly, it was determined that the amount of evidence is insufficient and the findings are not appropriate as the most recent evidence of clinical effectiveness. As such, based on the findings in this assessment, the subcommittee concluded that there is insufficient evidence of the long-term effects (remission and relapse rates) of rTMS as a safe and non-invasive health technology used as an add-on treatment for patients on medication for major depressive disorder. However, in consideration of clinical settings and treatment guidelines in South Korea, it was determined that rTMS may be an adjuvant therapy for treatment-resistant patients who do not respond to conventional antidepressants treatment. During the twelfth session of the Committee of Health Technology Reassessment on December 9, 2022, the following conclusions were made on “rTMS” based on an agreement with the subcommittee findings and the opinion to change the agenda title with the Guidelines for the Management of Health Technology Reassessment Project. rTMS for patients on medication for major depressive disorder is a non-invasive health technology that induces the activation and inhibition of nerve cells through cortical stimulation using a local magnetic field, and although transient AEs were reported, no permanent serious ARs were identified. Accordingly, rTMS is assessed to be a safe health technology for patients on medication for major depressive disorder. In addition, using rTMS as an add-on treatment in patients on medication for major depressive disorder showed significant improvement in depressive symptom indicators and response rates as compared to groups that did not receive combination therapy. Despite this, the evidence on remission and relapse rates for identifying its long-term therapeutic effect was insufficient. However, considering the clinical situation wherein the use of antidepressants cannot be discontinues and additional treatment options are not available in the step prior to finally applying electroconvulsive therapy, which is a treatment option for patients with major depressive disorder who do not respond to medication therapy, it was assessed that rTMS can be used in combination with antidepressants treatment. Accordingly, the Committee of Health Technology Reassessment “conditionally recommends” the use of rTMS for improving depressive symptoms in patients on medication for major depressive disorder.
