관절액 제거 시술(관절천자)

Abstract

평가 배경 관절천자는 주사 바늘을 이용하여 관절강 속의 활액을 뽑아내어 진단 및 치료 목적으로 사용되는 시술로, 건강보험요양급여목록 필수급여 등재된 기술이다. 의료기술재평가에 대한 사회적 요구도를 수용하는 목적으로 수행된 ‘2020년 연구주제 대국민 수요조사’에서 HTA 국민 참여단은 관절액을 제거하는 시술을 반복적으로 시행하는 것이 안전하고 유효한지에 대한 의문을 제안하였다. 이에 보건의료서비스의 적정사용에 대한 대국민 정보 제공을 위한 목적으로 재평가를 수행하고자 하였으며, 2020년 제9차 의료기술재평가위원회(2020.09.11.)에서 의료기술재평가 안건으로 선정되어 체계적 문헌고찰을 통해 치료목적 관절천자의 임상적 안전성 및 유효성을 평가하였다.

평가 방법 체계적 문헌고찰을 통해 관절천자가 임상적으로 안전하고 유효한지를 평가하였으며 건강보험심사평가원 청구자료를 이용하여 관절천자의 이용현황을 파악하였다. 본 평가를 위해 총 5인(정형외과 2인, 류마티스내과 1인, 재활의학과 1인, 근거기반의학 1인)으로 소위원회를 구성하였으며 2020년 11월 19일부터 2021년 7월 30일까지 4회의 소위원회를 통해 동 기술의 안전성 및 유효성을 평가하였다. 체계적 문헌고찰의 평가대상은 치료적 유효성을 문헌을 통해 판단하기 어려운 감염성 질환은 배제하고 류마티스 관절염 및 골관절염, 외상(골절 등)으로 인한 관절혈종, 혈우병 및 항응고요법 적용, 원인미상의 삼출물 또는 활액막염으로 구분하였다. 안전성은 관절천자 시술로 인한 부작용 및 합병증(통증, 출혈, 감염, 사망 등)으로 평가하였고, 유효성은 통증 개선 정도, 관절가동범위 개선 정도, 임상증상 소실 기간, 재발, 일상 회복 기간 등의 지표로 평가하였다. 문헌의 비뚤림위험 평가는 연구설계에 따른 평가도구(RoB, RoBANS)를 사용하여 두 명의 검토자가 독립적으로 수행 후 의견 합의를 이루었다. 자료추출은 미리 정해놓은 양식을 활용하여 수행하였으며, 의견 불일치가 있을 경우 제 3자와 함께 논의하여 합의하였다. 자료분석은 연구별 보고 및 지표 방식이 다양하여 개별 연구에서 제시한 안전성 및 유효성 관련 결과를 질적으로 기술하여 검토하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 Grading of Recommendations Assessment, Development and Evaluation (GRADE)을 이용하여 근거수준을 평가하였다. 의료이용 현황분석과 관련하여 국내 관절천자의 시행규모 및 이용자 현황, 비용, 요양기관 종별현황, 상위 10순위 상병 연도별 추세 등을 분석하였다. 또한 다빈도 상병을 연간 10순위 기준으로 분석하였으며, 평균값은 환자수 대비, 시술건수 기준으로 환산하여 제시하였다. 본 평가는 관절천자에 대한 안전성과 유효성에 대한 정보를 국민에게 제공하는 것을 목적으로 하였으며 소위원회 논의를 통해 관절천자와 관련한 주의사항에 대한 대국민 정보를 별도 제시하였다.

평가 결과 관절천자 평가에 선택된 문헌은 총 16편으로, 질환별로 구분하여 골관절염 1편, 외상(골절 등)으로 인한 관절혈종 7편, 혈우병 및 항응고요법 적용 5편, 원인미상의 삼출물 또는 활액막염 3편이 해당하였다.

안전성 전체 16편 중 관절천자 안전성을 보고한 문헌은 총 10편(비무작위 비교연구 3편, 단일군 연구 7편) 이었다. 임상적 안전성 지표로 통증, 출혈, 감염, 사망, 그 외 기타 이상반응 발생을 검토하였다. 단일군 연구 3편에서 시술대상 중 0~5.2%에서 통증 발생을 보고하였으며, 5편에서 0~0.9%의 출혈 발생을 보고하였다. 관절천자로 인한 감염 발생은 비무작위 비교연구 및 단일군 연구 4편에서 0~0.2%로 보고하였다. 또한 단일군 연구 1편에서 시술과 관련한 사망은 발생하지 않은 것으로 보고하였다. 문헌상 분류가 모호한 기타 이상반응에 대해 보고한 단일군 연구 1편에서 천자 부위와 직접적으로 관련한 합병증(홍반, 멍, 통증)이 123명 중 2명(1.6%)에서 나타난 것으로 보고하였고, 그 외 3편(비무작위 비교연구 1편, 단일군 연구 2편)에서 관절천자와 관련한 합병증이 발생하지 않은 것으로 보고하였다.

유효성 관절천자의 유효성은 총 11편으로 무작위배정 비교임상시험 2편, 비무작위 비교연구 5편, 단일군 연구 4편이 해당되었다. 골관절염을 대상으로 시행한 단일군 연구 1편에서 관절천자 시술 후 통증 개선 정도를 보고하였으며, 통증 평가와 관련한 모든 변수에서 시술 전 대비 통계적으로 유의미한 개선이 확인되었다(p˂0.0001). 외상(골절 등)으로 인한 관절혈종 대상의 경우, 통증 개선 정도와 관련하여 무작위배정 비교임상시험 1편과 비무작위 비교연구 2편, 단일군 연구 1편이 포함되었다. 1편의 무작위배정 비교임상시험에서 관절천자 후 3주, 6주의 시점에서 통증 정도가 관절천자를 시행하지 않은 대조군 대비 통증 수준이 높은(moderate, severe) 환자의 비율이 낮았다. 비무작위 비교연구 2편 중 1편에서도 관절천자 후 2주, 8주 시점에 통증 수준이 높은(moderate, severe) 환자 수가 관절천자를 시행하지 않은 대조군 대비 적었으며, 그 외 1편에서는 관절천자 시행군에서의 7-14일 시점 통증 개선 정도가 대조군(미시행)에 비해 더 크게 나타났고 두 군간 유의한 차이를 보고하였다(p=0.005). 또한 단일군 연구 1편에서는 관절천자 후(평균 8.9일)에 통증 정도가 유의하게 감소한 것으로 나타났다(p=0.005). 그 외 관절천자를 시행한 환자에서 통증 개선 정도를 보고한 단일군 연구 1편에서 시술 후 1일차에 통계적으로 유의미한 통증 완화를 보고하였다(p˂0.001). 관절가동범위 개선 정도와 관련하여 비무작위 비교연구 2편이 포함되었다. 1편의 연구에서 관절천자 시행 후 7-14일 시점 관절가동범위가 관절천자를 시행하지 않은 대조군에 비해 통계적으로 유의한 개선효과를 보고하였다(p=0.001). 또 다른 연구 1편에서는 추적관찰 2주, 8주 시점에서 flexion 범위와 2주 시점에서 abduction 범위가 관절천자 미시행군에 비해 유의하게 큰 것으로 보고하였다(p˂0.001). 그 외 관절천자 중재 후 팔꿈치 운동 범위를 보고한 단일군 연구에서 1일차에 통계적으로 유의미한 운동범위의 증가를 보고하였으며(p˂0.001), 7명을 대상으로 관절천자 시행 결과를 질적 기술한 1편의 단일군 연구에서는 시술 후 모든 환자의 관절가동범위가 즉시 개선되었고, 시술 후 4일에서 3개월에 걸쳐 관절가동범위가 정상으로 회복되었다. 단, 1명의 환자에서 회내 상실 10도가 확인되었으나 6주 결과 정상 기능 회복으로 보고되었다. 혈우병 및 항응고요법 적용을 대상으로 시행한 연구는 통증 완화 기간, 이전 관절가동범위 회복 기간, 일상 회복 기간(학교/직장 복귀) 결과 지표에 대하여 1편의 비무작위 비교연구에서 보고하였다. 각각의 지표는 관절천자를 시행하지 않은 대조군에 비해 더 짧은 기간 내에 통증 완화, 이전 관절가동범위로의 회복, 학교/직장 등의 일상생활의 회복을 보고하였다(p˂0.001). 원인 미상의 삼출물 또는 활액막염 대상의 경우, 무작위배정 비교임상시험 1편에서 통증 및 관절가동범위 개선 정도를 보고하였으며, 관절천자 시행 2, 4일에 대조군(위약군)에 비해 중재군의 통증 점수가 유의하게 더 낮았고(p˂0.05), 관절가동범위가 유의하게 더 큰 것으로 보고하였다(p˂0.05). 비무작위 비교연구 1편에서는 대조군(미시행)에 비해 중재군에서 유의하게 더 짧은 증상소실기간을 보고하였다(p˂0.001). 무작위배정 비교임상시험 1편에서 관절천자 시행 후 1주 이내 외상환자에서 37%, 외상 과거력이 없는 환자에서 14% 무릎 삼출물이 재발한 것으로 보고하였다.

결론 및 제언 관절액 제거 시술(관절천자) 재평가 소위원회는 본 평가를 통해 다음과 같이 제언하였다. 관절천자의 안전성은 수용 가능한 수준이며, 유효성은 관절천자 시술 후 최소 1일에서 최대 22주 기간의 추적 관찰 기간 동안 통증 및 출혈로 인한 혈관절증 완화, 관절가동범위 개선 등 단기적인 효과가 확인되었다. 그러나 관절천자의 장기적인 효과 또는 반복 시술에 대한 유효성을 판단하기에는 현재 문헌의 근거 수준은 불충분하다고 평가하였다. 본 평가에서 확인한 관절천자 이용현황은 2020년 시술건수가 총 956,282건으로 확인되었다. 소위원회에서는 약물의 주입 없이 삼출액을 제거하는 것만으로는 감염이 극히 드물게 발생하지만, 관절천자는 침습적 행위로 인한 감염, 출혈, 통증 악화 등 안전성과 관련하여 주의가 요구되고, 지속적인 관리가 필수적인 기술로 평가하였다. 또한 관절천자는 관절질환의 진단 및 치료 목적으로 사용되는 불가피한 기술이지만 대부분 보조적으로 시행되며, 근본적인 원인에 대한 치료가 적절히 이루어지는 것이 더 중요하고, 선별되지 않은 관절천자는 권장하지 않는다는 의견이 종합되었다. 2021년 제9차 의료기술재평가위원회는 ‘관절액 제거 시술(관절천자)’에 대해 소위원회의 검토 결과에 근거하여 다음과 같이 심의하였다(2021.09.10.). 관절액 제거 시술(관절천자)의 안전성은 수용 가능한 수준이며, 단기적으로 통증 및 출혈로 인한 혈관절증 완화, 관절가동범위 개선 등에 있어 효과가 있다는 연구결과가 있지만, 임상적 안전성 및 유효성을 판단하기에 현재의 문헌적 근거는 불충분하다. 또한 관절천자는 관절질환에서 진단 및 보조적 치료법으로 빈번하게 수행되는 의료기술로, 과정이 침습적이므로 안전한 사용을 위한 주의와 지속적인 관리가 필수적으로 이루어져야 한다.

주요어 골관절염, 류마티스 관절염, 혈관절증, 관절천자 ᑬSᑭ!ᢄSᢅ!ᨡSᨢ!᪭S᪮!ᵀSᵁ!ṫSṬ!ẢSả!ὄSὅ!넷partial meniscectomy, Comparative effectivenessYʶº鋖ė뼀␀␀А♉ɥַᇭ耂ÿ평가 배경 체외충격파치료 [근골격계질환]은 상완골 내상과염 및 외상과염, 족저 근막염, 견관절 석회화 건염, 골절 지연 유합 등에 체외에서 충격파를 병변에 가해 혈관 재형성을 돕고, 건 및 그 주위조직과 뼈의 치유 과정을 자극하거나 재활성화시켜, 통증의 감소와 기능의 개선을 위한 치료법이다. 체외충격파치료 [근골격계질환]은 현재 비급여 행위(조-84)로 사용되고 있으며, 의학적 비급여의 급여화 추진과 관련하여 건강보험심사평가원과의 협의를 통해 재평가 항목으로 발굴되었다. 이에 2020년 제5차 의료기술재평가위원회(2020. 5. 11.~13.)에서는 체계적 문헌고찰을 통해 해당 기술의 안전성 및 효과성을 평가하며 이 때 적용 부위를 크게 ‘어깨 및 상지’, ‘고관절 및 요추부’, ‘하지 및 족부’로 나누어 평가하도록 심의하였다. 이후 의료기술재평가 소위원회와 평가위원회에서의 추가적인 논의 의료기술재평가 소위원회 및 평가위원회의 안건 관련 논의사항과 세부적인 경과과정은 [부록 1], [부록 2] 참고 를 통해 체외충격파치료의 대표적 적응 질환으로, 상지 부위에서는 어깨 건병증, 내외측 상과염을, 하지 부위에서는 대전자동통증후군, 족저근막염, 아킬레스건병증, 무릎건병증, 불유합/지연유합, 근막동통증후군을 선정하고 이에 대해 평가하기로 하였다. 체외충격파치료 [근골격계질환]에 대한 재평가 수행 중, 2021년 3월 건강보험심사평가원으로부터 그 외 질환에 대해 재평가를 추가 의뢰받았다(예비급여부-265, 2021. 3. 23.). 2021년 제5차 의료기술재평가 위원회(2021. 5. 14.)에서는 심평원에서 추가 의뢰된 16개 질환(골관절염, 피로골절, 무혈성괴사, 박리성 골연골염, 내전근 건병증, 거위발 건병증, 비골근 건병증, 발․발목 건병증, 골수 부종, 오스굿-슐라터 병, 경골 스트레스 증후군, 근육 염좌, 뒤퓌트랑, 발바닥 섬유종증, 드퀘르벵 병, 방아쇠 수지)에 대해 평가계획서 및 소위원회 구성에 대해 심의하였다. 이에 체외충격파치료 [근골격계질환]에 대해 안전성 및 효과성에 대한 과학적 근거를 제공하고, 의료기술의 적정 사용 등 정책적 의사결정을 지원하고자 체외충격파치료(extracorporeal shock wave therapy, ESWT)의 재평가를 수행하였다. 본 보고서에서는 체외충격파치료 [근골격계질환]의 적용 질환 중 ‘골수 부종, 오스굿-슐라터 병, 경골 스트레스 증후군’에 대해 안전성 및 효과성에 대한 과학적 근거를 평가하고, 이를 제시하였다.

평가 방법 체외충격파치료 [근골격계질환] 기타 뼈 질환에 대한 안전성 및 효과성 평가는 체계적 문헌고찰을 통해 수행하였다. 모든 평가방법은 평가목적을 고려하여 “체외충격파치료 [근골격계질환] 기타 뼈 질환 재평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회는 정형외과 4인, 재활의학과 3인, 마취통증의학과 1인, 신경외과 1인, 류마티스내과 1인, 영상의학과 1인, 근거기반의학 1인 총 12인의 위원으로 구성하였다. 평가의 핵심질문은 ‘골수 부종 환자, 오스굿-슐라터 병 환자, 경골 스트레스 증후군 환자를 대상으로 체외충격파로 치료하는 것이 안전하고 효과적인가?’이었고, 안전성은 시술 관련 부작용 및 이상반응 지표로, 효과성은 통증완화, 기능 개선, 삶의 질 지표로 평가하였다. 문헌검색은 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림위험 평가는 무작위배정 비교임상시험 연구(Randomized Controlled Trial, RCT)는 Cochrane의 Risk of Bias (RoB)를, 비무작위 비교연구(Non-randomized Study, NRS)는 Risk of Bias Assessment tool for Non-randomized Study (RoBANS)(Ver.2)를 각각 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우에는 제3자와 함께 논의하여 합의하였다. 자료분석은 정량적 분석이 불가능하여 정성적 분석을 적용하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 방법을 이용하여 근거 수준을 평가하고, 평가 결과를 토대로 권고등급을 결정하였다.

평가 결과 체외충격파치료 [근골격계질환] 기타 뼈 질환의 안전성과 효과성은 총 6편(골수 부종 환자 4편, 경골 스트레스 증후군 환자 2편, 오스굿-슐라터 병 환자 0편)에 근거하여 평가하였다.

골수 부종 환자 골수 부종 환자에서의 안전성은 시술 관련 부작용 및 이상반응 지표로 총 4편의 문헌을 토대로 평가하였다. 3편의 체외충격파치료 시행군에서는 연조직 종창, 통증, 멍이 보고되었으며, 수술적 치료 시행군에서는 국소 혈종, 창상 치유 불량, 보존적 요법 시행군에서는 두통, 안면 발진이 각각 보고되었다. 나머지 1편에서는 시술관련 부작용 및 이상반응이 발생하지 않았다고 보고하였다. 골수 부종 환자에서의 효과성은 통증완화, 기능개선, 삶의 질 지표로 총 4편의 문헌을 토대로 평가하였다. 통증완화의 경우, 수술적 치료 시행군과 비교한 1편은 단측 및 양측 모두 중재군에서 대조군보다 더 유의하게 시각통증척도(Visual Analogue Scale, VAS) 점수가 개선되었다. 보존적 요법 시행군과 비교한 3편(RCT 1편, NRS 2편) 중 2편에서 대조군보다 중재군의 VAS 점수가 더 개선되었고 다른 1편(RCT)은 중재군보다 대조군의 VAS 점수가 더 개선되었으나 3편의 통계적 유의성은 일관되지 않았다. 기능개선의 경우, 수술적 치료 시행군과 비교한 1편은 단측의 경우 중재군이 대조군보다 더 유의하게 점수가 개선되었다. 양측의 경우 중재군과 대조군 모두 점수가 개선되었으나 한쪽만 유의하였다. 보존적 요법 시행군과 비교한 3편에서 Western Ontario and McMaster University Osteoarthritis Index (WOMAC), Knee Society Score (KSS) 지표를 이용하여 평가하였다. 3편 모두 대조군보다 중재군에서 점수가 개선되었으나 통계적 유

Background Arthrocentesis is a technique for diagnostic and treatment purposes, whereby blood in the glenoid cavity is collected using a syringe needle. The technique is listed as part of the Mandatory Benefit of the National Health Insurance Coverage.

In the 2020 Research Theme Public Demand Survey conducted for the purpose of incorporating the social demand regarding HTR, the public HTA panel questioned whether repeated removal of joint fluid through arthrocentesis is safe and effective. Thus, reassessment was planned for the purpose of providing public information regarding the appropriate use of public health care services. The theme was selected as the HTR target by the 9th HTR committee of 2020 (Sep 11, 2020), and the safety and effectiveness of arthrocentesis were assessed with respect to the treatment purpose through a systematic review.

Methods Through a systematic review, the safety and effectiveness of arthrocentesis were assessed, and the current use of arthrocentesis was determined using the claims data from the Health Insurance Review and Assessment Service. For the assessment, a subcommittee was organized with five members (two orthopedic surgeons, one rheumatologist, one specialist of rehabilitation medicine and one specialist of evidence-based medicine. The subcommittee performed the assessment of safety and effectiveness for arthrocentesis through four discussions during the period of Nov 19, 2020, to Jul 30, 2021.

For the systematic review, infectious diseases that could not be readily investigated in previous studies for therapeutic effectiveness were excluded; the included conditions were rheumatic arthritis and osteoarthritis, hemarthrosis caused by trauma (fracture, etc.), hemophilia and anticoagulation therapy, and exudation or synovitis of an unknown cause. The safety was assessed based on procedure-related side effects and complications (pain, hemorrhage, infection, death, etc.). The effectiveness was assessed based on pain improvement, joint range of motion (RoM) improvement, time until loss of clinical symptoms, rate of recurrence, and time until return to daily life. The risk of bias was evaluated using tools based on the study design (RoB and RoBANS) by the two independent reviewers, followed by a discussion to reach an agreement. For data extraction, a preset format was used by the two independent reviewers, followed by a discussion with a third party to reach an agreement. For data analysis, the safety and effectiveness reported in individual studies were qualitatively analyzed, as each study used a variety of reporting and indexing methods. The level of evidence was determined using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) for the systematic review in this assessment.

Regarding the analysis of the current use of arthrocentesis as part of public health care, the performance scale of arthrocentesis and user status, cost, current use at different recuperation centers, and annual trend of the top ten diseases, were analyzed. In addition, the annual top ten high-frequency diseases were analyzed with the mean value estimated in terms of the number of patients and the number of operations.

The purpose of this assessment was to provide the public information regarding the safety and effectiveness of arthrocentesis, and through subcommittee discussions, the public information regarding the precautions for arthrocentesis was additionally presented.

Results For the assessment of arthrocentesis, sixteen articles were selected and divided according to disease: one study on osteoarthritis, seven studies on hemarthrosis caused by trauma (fracture, etc.), five studies on hemophilia and anticoagulation therapy, and three studies on exudation or synovitis of an unknown cause.

Safety Ten out of the sixteen articles reported on the safety of arthrocentesis (three non-randomized controlled studies and seven uncontrolled studies).

The clinical safety indicators were pain, hemorrhage, infection, death, and other adverse reactions. In three uncontrolled studies, pain was reported by 0–5.2% of patients, while 0–0.9% of patients in five studies experienced hemorrhage. The infection due to arthrocentesis was 0–0.2% in four studies including uncontrolled and non-randomized controlled studies. In one uncontrolled study, no death occurred in relation to arthrocentesis. In another uncontrolled study reporting on other adverse reactions that could not be unambiguously categorized, complications (erythema, bruising, and pain) in direct association with the area of arthrocentesis were observed in 1.6% of patients (2/123). However, three studies (one non-randomized controlled and two uncontrolled studies) reported the lack of complications in association with arthrocentesis.

Effectiveness Eleven studies reported on the effectiveness of arthrocentesis: two randomized controlled clinical studies, five non-randomized controlled studies, and four uncontrolled studies.

In one uncontrolled study on osteoarthritis, the pain improvement after arthrocentesis was shown; all variables in pain evaluation significantly improved compared to the preoperative state (p˂0.0001).

The pain improvement in patients with hemarthrosis caused by trauma (fracture, etc.) was reported in one randomized controlled clinical study, two non-randomized controlled studies, and one uncontrolled study. In the randomized controlled clinical study, the percentage of patients with moderate or severe pain at W3 and W6 after arthrocentesis compared to the control was low. One of the two non-randomized controlled studies also reported a lower percentage of patients with moderate or severe pain at W2 and W8 after arthrocentesis compared to the control, while the other study reported a larger level of pain improvement at D7 to D14 in the arthrocentesis group compared to the control with significant between-group variation (p=0.005). In one uncontrolled study, the pain improvement after arthrocentesis (mean 8.9 days) was significant (p=0.005). In addition, one uncontrolled study reporting on the pain improvement in patients who received arthrocentesis showed significant pain improvement on the first day after surgery (p˂0.001).

The joint RoM improvement was reported in two non-randomized controlled studies. In one study, the joint RoM at D7 to D14 after arthrocentesis showed significant improvements compared to the control (p=0.001). In the other study, the flexion at W2 and W8 and the abduction at W2 in the follow-up period were shown to have significantly increased in the arthrocentesis group compared to the control (p˂0.001). Furthermore, in an uncontrolled study reporting on elbow RoM after arthrocentesis, a significant increase in RoM was found at D1 (p˂0.001), while in an uncontrolled study qualitatively analyzing the result of arthrocentesis in seven patients showed immediate improvements in joint RoM in all patients after surgery, reaching normal levels between 4 days and 3 months after surgery. A case of loss of pronation at 10° was reported, although normal function was restored at W6.

Hemophilia and anticoagulation therapy was reported in one non-randomized controlled study regarding the period until pain relief, recovery of previous joint RoM, and return to daily life (school/work). Each indicator showed a significantly reduced period in the arthrocentesis group compared to the control (p˂0.001).

Exudation or synovitis of an unknown cause was reported in one randomized controlled clinical study regarding the improvements of pain and joint RoM. Compared to the control (placebo), the pain score of the intervention group at D2 and D4 after arthrocentesis was significantly lower (p˂0.05), while the joint RoM was significantly higher (p˂0.05). In one non-randomized controlled study, the period until loss of symptoms was significantly shorter in the intervention group than in the control group (p˂0.001). In one randomized controlled clinical study, the percentage of knee exudation was 37% in trauma patients after arthrocentesis and 14% in patients with no history of trauma.

Conclusions and Recommendations Thus, based on the results of this assessment, the subcommittee suggested the following:

Arthrocentesis is a technique with an acceptable level of safety, and short-term effects such as the improvements in pain, hemorrhage-induced hemarthrosis, and joint RoM were found in the follow-up period of a minimum of one day to a maximum of 22 weeks after arthrocentesis. However, the current level of evidence is insufficient to verify the long-term effects or effects of repeated procedures.

The current use of arthrocentesis was 956,282 cases as of 2020. The subcommittee determined that, despite a very low incidence of infection with the removal of exudation without drug injection, arthrocentesis is an invasive procedure that warrants safety precautions regarding infection, hemorrhage, and increased pain, for which continuous management is essential. Furthermore, although arthrocentesis is indispensable in diagnosing and treating joint diseases, it is mostly performed as an adjunct; it is more important that suitable treatments are provided for the fundamental cause. Thus, arthrocentesis is not recommended without screening.

Based on the subcommittee report, the conclusion of the 9th HTR committee of 2021 (Sep 10, 2021) was as follows:

Arthrocentesis is a technique with an acceptable level of safety. Short-term effects such as improvements in pain, hemorrhage-induced hemarthrosis and joint RoM were found, but the current level of evidence is insufficient to verify the safety and effectiveness of the technique. In addition, as arthrocentesis is an invasive health technology commonly performed as a diagnostic and adjunct for joint diseases, precautions should be taken at all times and continuous management is essential to ensure safe use.
