무릎 골관절염 환자에서 관절경적 반월판 연골절제술

Abstract

평가 배경 관절경적 반월판 연골절제술은 관절 내시경을 통해 불안정한 연골판이나 퇴행성 변화가 있는 연골판을 절제해 파열 진행을 막기 위한 수술법으로 1993년에 건강보험에 등재되어 손상형태, 질병, 연령 등의 별도 기준 없이 급여되고 있는 행위이다. ‘무릎 골관절염 환자에서 관절경적 반월판 연골절제술’에 대한 안전성 및 유효성 재평가는 2020년 대국민 수요조사에서 제안되었고, 2020년 제4차 의료기술재평가위원회(2020.4.9.~17.)에서 평가계획서 및 소위원회 구성안에 대해 심의를 받아 재평가로 수행하였다. 본 평가의 목적은 무릎 골관절염 환자에서 관절경적 반월판 연골절제술의 임상적 안전성 및 유효성 등에 대한 과학적 근거를 제공하고 이를 통해 의료기술의 적정사용 등 정책적 의사결정을 지원하는 것이다.

평가 방법 무릎 골관절염 환자에서 관절경적 반월판 연골절제술에 대한 안전성 및 유효성 평가를 위해 체계적 문헌고찰 및 국내실증자료 분석을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “무릎 골관절염 환자에서 관절경적 반월판 연골절제술 평가 소위원회(이하 ‘소위원회’라 하였다)”의 심의를 거쳐 확정하였다. 소위원회 구성은 정형외과 3인, 류마티스내과 2인, 재활의학과 1인, 근거기반의학 1인의 전문가 7인으로 구성하였다. 체계적 문헌고찰의 핵심질문은 “반월판 파열이 동반된 무릎 골관절염 환자에서 관절경적 반월판 연골절제술은 안전하고 유효한 시술인가?”이었고, 안전성은 보고된 부작용 발생을 지표로 하였고, 유효성은 환자보고통증, 무릎기능, 삶의 질, 인공관절치환술 시행을 지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 기존의 결과를 활용한 체계적 문헌고찰 방법론(using Existing Systematic Review)을 이용하였고, 문헌 선정과정은 문헌선택 및 배제기준에 따라 2명의 평가자가 독립적으로 수행하고, 의견의 불일치가 있는 경우에는 평가자간 합의를 통해 최종 논문을 결정하였다. 문헌의 질평가는 AMSTAR, RoB, RoBANS 2.0을 사용하였으며, 최종 선택된 문헌을 대상으로 2명의 평가자가 독립적으로 평가를 실시하였으며, 의견이 불일치한 경우 평가자간 합의를 통해 일치된 결과를 도출하였다. 모든 자료는 연구단위로 추출하였다. 실증자료를 이용한 비교분석은 무릎 골관절염 환자에서 반월판 연골절제술 시행 여부에 따라 10년 추적하여 무릎 인공관절치환술 발생까지의 시간, 모든 원인 사망까지의 시간에 차이가 있는지 검증하는 것을 목적으로 하였다. 이를 위해 2006~2019년도 국민건강보험공단의 맞춤형연구자료를 이용하여 2007~2009년도 50~79세 반월판 연골손상을 동반한 무릎 골관절염 환자를 반월판 연골절제술 시행여부에 따라 구분하고 최대 10년 추적하는 후향적 매칭 코호트를 구축하여 비교분석 하였다.

평가 결과 체계적 문헌고찰에서 최종 선택된 논문은 총 5편으로, 무작위배정 비교임상시험 연구 4편(3개의 임상시험), 코호트 연구 1편이었다. 체계적 문헌고찰 결과는 무릎 골관절염 환자에서 반월판 연골절제술과 물리치료를 병행한 경우, 반월판 연골절제술만 시행한 경우로 구분하여 안전성과 유효성을 물리치료군과 비교하여 제시하였다. 반월판 연골절제술의 장기적인 효과를 파악하기 위해 건강보험자료로 구축한 36,974명의 후향적 매칭코호트에서 반월판 연골절제술군의 무릎 인공관절치환술 발생률과 사망률을 반월판 연골절제술을 받지 않은 군과 비교하였다.

안전성 무릎 골관절염 환자에서 반월판 연골절제술의 안전성을 보고한 문헌은 총 2편의 무작위배정 비교임상연구였다(METEOR trial, ESCAPE trial). METEOR trial에서는 반월판 연골절제술과 물리치료를 병행한 군과 물리치료군만 시행한 군에서 이상반응 발생 비율에 유의한 차이가 없다고 보고하였다. 12개월 추적기간 동안 사망 등 심각한 부작용은 병행군에서 3명, 물리치료군에서 2명이 발생하였고, 경증 또는 중등도 부작용은 병행군에서 15명, 물리치료군에서 13명이 발생하였다. ESCAPE trial에서는 반월판 연골절제술만 시행한 군과 물리치료만 시행한 군을 24개월 추적하였고, 심각한 부작용(심혈관계, 신경계 혹은 내과적 상태, 혈전색전증, 반복 수술)이 반월판 연골절제술군에서 9명, 물리치료군에서 8명에서 발생하였다고 보고하였다.

유효성 2개의 임상시험에서 보고된 3편의 무작위배정 비교임상연구와 1편의 코호트 연구에서 무릎 골관절염 환자에서 반월판 연골절제술의 유효성을 평가하였다. 4편의 문헌에서 보고한 유효성 지표는 무릎기능, 무릎통증, 신체활동기능 및 전반적인 건강상태였다. 반월판 연골절제술과 물리치료를 병행한 군과 물리치료군만 시행한 군의 비교 결과는 METEOR trial과 Kirkley(2008)에서 보고되었다. 무릎기능에 대해, METEOR trial에서는 6개월, 12개월, 5년 시점의 변화량이 두 군 간 유의한 차이가 없다고 보고하였고, Kirkley(2008)에서는 24개월 시점의 변화량이 두 군 간 차이가 유의하지 않다고 보고하였다. 무릎통증에 대해, METEOR trial에서 6개월, 12개월, 5년 시점의 변화량은 두 군 간 유의한 차이가 없었고, Kirkley(2008)에서 24개월 시점의 변화량도 두 군 간 유의한 차이가 없었다. 신체활동점수는 METEOR trial에서6개월, 12개월 시점의 변화량은 두 군 간 차이가 유의하지 않았고, Kirkley(2008)은 24개월 시점의 신체활동점수가 두 군에서 유의한 차이가 없다고 보고하였다. 인공관절치환술 발생에 대해 METEOR trial에서 배정된 대로 분석(intention to treat)한 결과, 두 군의 5년 발생률 차이는 유의하지 않았다. 자기공명영상으로 확인한 무릎 관절 손상 수준을 층화하여 분석한 MacFarane(2019)은 관절내 손상이 적은 경우 병행군의 6개월 시점의 무릎통증이 물리치료군에 비해 유의하게 개선되었다고 보고하였다. 반월판 연골절제술 단독과 물리치료 단독 시행의 비교는 ESCAPE trial에서 물리치료의 비열등성 시험의 결과로 확인되었다. 무릎기능에 대해, 24개월 추적 결과는 물리치료가 반월판 연골절제술에 비해 열등하지 않다는 결과를 보였다. 24개월 시점의 무릎통증 감소는 반월판 연골절제술군에서 유의하게 컸다. 24개월 시점의 신체활동 변화량은 두 군에 차이가 없었고, 전반적 건강상태, 관절염 중증도 변화에서도 두 군 간 차이는 유의하지 않았다. 무릎 골관절염 환자에서 반월판 연골절제술의 장기적인 효과를 파악하기 위해 국민건강보험자료를 이용한 코호트에서 반월판 연골절제술군에서 10년 추적기간 동안 인공관절치환술 발생은 9.62%에서 확인되었고, 반월판 연골절제술 시행부터 인공관절치환술 시행까지의 기간은 평균 5.88년이었다. 추적시작 시점의 대상자 특성을 보정하였을 때 반월판 연골절제술을 받은 경우 그렇지 않은 군에 비해 무릎 인공관절치환술 발생이 25% 높았다. 반월판 연골절제술군에서 추적기간 동안 사망한 경우는 5.20%이고, 반월판 연골절제술을 받지 않은 군과 비교하였을 때 통계적으로 유의한 차이가 없었다.

결론 및 제언 무릎 골관절염에서 반월판 연골절제술 재평가 소위원회는 본 평가결과가 건강보험급여와 관련된 의사결정의 근거가 되기에는 제한점이 많다고 판단하였다. 체계적 문헌고찰 및 국내 실증자료분석 결과에 무릎 골관절염 환자에서 반월판 연골절제술을 시행하는데 고려하는 ‘심한 골관절염’, ‘무릎잠김증상 동반’에 대한 평가 내용이 포함되어 있지 않기 때문이다. 본 평가결과는 골관절염 중증도가 낮고 무릎잠김증상이 없는 무릎 골관절염 환자에게 반월판 연골절제술 시행한 경우, 보존적 치료에 비해 단기적 통증감소 효과가 더 좋았다. 그러나 반월판 연골절제술로 인한 무릎기능, 신체활동 효과의 차이는 없었고, 중ㆍ장기적 이익 및 안전성에도 차이가 없었다. 국내 실증자료를 통해 반월판 연골절제술 받은 경우 10년 내 무릎 인공관절치환술 비율이 25%정도 높았다. 그러나 이러한 차이에는 동일한 무릎의 인공관절 치환 여부, 반월판 연골 손상 정도, 수술 선호도, 실비보험, 경제수준 등 측정되지 못한 요인에 의한 영향이 포함되어 있어 평가근거로 활용하는데 한계가 있다고 소위원회는 판단하였다. 소위원회는 무릎 골관절염 환자에서 널리 시행되고 있는 반월판 연골절제술의 안전성 및 유효성을 재평가하기 위해서는 중증도가 높은 무릎 골관절염 대상자, 반월판 연골 파열 정도, 무릎잠김증상 여부, 반월판 연골절제술이 시행된 무릎에서의 인공관절치환술 발생 여부, 수술 선호도 등을 알 수 있는 전향적 임상연구가 필요하다고 제언하였다. 2021년 9차 의료기술재평가위원회는 ‘무릎 골관절염 환자에서 반월판 연골절제술’에 대해 소위원회의 평가결과에 근거하여 다음과 같이 심의하였다(2021.9.10.). 무릎 골관절염 환자에서 반월판 연골절제술은 보존적 치료법과 비교할 때 안전성의 차이가 없고, 단기 통증 완화에 더 효과적이나, 무릎기능, 신체활동, 전반적인 건강상태, 관절염 중증도 변화에 대한 단기 및 장기효과, 장기적 통증 완화 효과의 차이가 없다는 평가결과가 존재하였다. 또한 실증자료로 수술군에서 장기적으로 인공관절이식을 하는 비율이 높음을 제시하였다. 그러나 반월판 연골절제술은 현재 널리 시행되고 있는 기술로 문헌적 근거 및 실사용 근거가 불충분하여 향후 추가적인 재평가를 위해 전향적 임상연구가 필요하다고 의결하였다.

Arthroscopic partial meniscectomy in patients with knee OA

Background Arthroscopic partial meniscectomy (APM) is performed to prevent the development of rupture by removing the meniscus when it shows instability of degenerative changes. This procedure has been on the list of National Health Insurance (NHI) reimbursement since 1993 and has been performed without any specific guidelines concerning injury type, disease, or age. A public survey conducted in 2020 has suggested the need for reassessing the safety and effectiveness of APM in patients with knee osteoarthritis (OA). Accordingly, a reassessment plan was approved by the 4th Health Technology Reassessment Committee of 2020 (Apr 9–17, 2020) and the relevant subcommittee was designated. The purpose of this reassessment is to examine the current scientific evidence for the clinical safety and effectiveness of APM in patients with knee OA, with the intent to support policy making concerning the appropriate uses of this procedure.

Methods To assess the safety and effectiveness of APM in patients with knee OA, a systematic review (SR) and an analysis using real-world data (RWD) were performed. All assessment methods were designed, approved, and carried out by the subcommittee that consisted of seven experts: three orthopedists, two rheumatologists, one specialist of rehabilitation medicine, and one specialist of evidence-based medicine. The key question of SRs was the following: “is APM safe and effective for patients with knee OA complicated by meniscal damage?” Safety was assessed by examining the reported adverse events (AEs). Effectiveness was assessed based on the following parameters: patient-reported pain, knee function, quality of life (QoL), and the conversion to total knee replacement (TKR). We searched existing SRs through Ovid-Medline, Cochrane Library, Epistemonikos, and additionally used Ovid-EMBASE and Korean literature databases such as KoreaMed and KMBASE for primary studies to update for the existing SR. To minimize the risk of selection bias, two reviewers independently screened the available literature and selected relevant studies based on the designated inclusion and exclusion criteria. After screening, the two reviewers converged on a single conclusion, and in the case of disagreement, a discussion was initiated to reach an agreement. The risk of bias was assessed using the RoBANS 2.0, Cochrane Risk of Bias (RoB), and AMSTAR tools. The protocol for this SR was registered in PROSPERO (CRD42020215965) on December 11, 2020. In addition, a retrospective longitudinal analysis was conducted to examine the rate of TKR and overall survival rate among OA patients who underwent APM due to meniscal tear and compare with those who did not in the real world, using the national health information database of the National Health Insurance Service (NHIS). Among knee OA patients aged 50-79, those who were treated with APM due to meniscal damage from 1st, January 2007 to 31st, December 2009 were selected as the APM group while those not treated with APM despite the presence of meniscal damage were selected as control group. The APM group and control group were matched based on a propensity score and were followed-up from the index date until the earliest occurrence of: TKR, death, or the time when the follow-up is 10 years.

Results In SR, five studies were finally selected. Four were randomized controlled trials (RCTs) reporting the results of three clinical trials, and one was a cohort study. In all studies, patients with knee OA underwent either APM, physical therapy (PT), or a combination of both, and their clinical outcomes were compared against those of patients who did not undergo APM but only received PT. In an analysis of RWD, a total of 36,974 patients were included in APM groups and no-APM group after 1:1 matching, and TKR incidence and mortality were compared between both groups.

Safety According to the SR results, two studies reporting on the results of the METEOR and ESCAPE trials examined safety. In the METEOR trial, no significant difference in AEs was detected between patients who underwent APM and received PT and those who only received PT. Concerning severe AEs, three and two cases of death were recorded during the 12-month follow-up period in the APM + PT and PT only group, while 15 and 13 patients presented with mild-to-moderate AEs in either group, respectively. In the ESCAPE trial, during the 24-month follow-up, severe AEs (e.g., cardiovascular, or repeat knee surgery) were reported for nine APM-treated patients and eight patients who underwent only PT.

Effectiveness According to the SR results, three studies of two RCTs and a cohort study were used to assess the effectiveness of APM in patients with knee OA. The indicators of effectiveness in the four studies were knee function, knee pain, physical activity, and overall health. In the METEOR trial, the APM + PT and the PT only group did not significantly differ in knee function and knee pain at the 6-month, 12-month, and 5-year follow-ups. Similarly, Kirkley et al. (2008) did not detect any difference in knee function and knee pain at 24 months after treatment. No significant difference at follow-up was detected in physical activity score in either study. In addition, the METEOR trial did not find any significant difference in the 5-year TKR incidence between the two groups. On the contrary, MacFarane et al. (2019) analyzed data from the METEOR trial post hoc and reported that those with the least and moderate damage (bone marrow lesions, cartilage, and meniscal damage) measured with MRI showed greater improvement in pain with APM + PT than with PT only, while those with the greatest damage had a similar improvement. In the ESCAPE trial, the non-inferiority of PT in respect to APM was assessed 24 months after treatment. Concerning knee function, physical activity, overall health, and arthritis level, PT was not inferior to APM. On the other hand, APM led to a greater reduction in knee pain than PT alone. According to the analysis of RWD, 9.62% of patients who had undergone APM needed a TKR in the 10 years. The mean time elapsed between APM, and TKR was 5.88 years. After adjustment for baseline confounders, the TKR incidence in the APM group was calculated to be 25% higher than that in the no-APM group. The mortality rate was 5.20%, which did not significantly differ between APM and non-APM groups.

Conclusions and recommendations Based on the level of evidence gathered from these SR and analysis of RWD, the subcommittee suggests the following: Due to the lack of sufficient evidence for APM for knee OA, decisions related to NHI benefits could not be made. Especially no results could be found in regard to two important parameters, severe OA or knee locking, which are when deciding to perform APM. The present assessment highlighted that, in patients with mild or moderate OA and no locked knee symptoms, short-term pain reduction was greater after APM than after PT. However, knee function, physical activity, and safety, as well as mid-to-long-term health parameters showed no significant improvement after APM. In analysis of RWD, the incidence of TKR within 10 years was about 25% higher for APM group although the difference could be attributed to factors such as TKR of the same APM-treated knee, level of meniscal damage, surgery-related preference, private health insurance, and economic status. The subcommittee thus determined that the effectiveness of APM cannot be verified. In addition, the subcommittee suggested the need for a prospective clinical study to know the severity of OA, the degree of meniscal damage, and knee locking symptoms, the occurrence of TKR in the APM-treated knee, and the preference for surgery, with the intent to determine the safety and effectiveness of APM in patients with knee OA.

The 9th Health Technology Reassessment Committee in 2021 (Sep 10, 2021) drew the following conclusions regarding APM in knee OA patients based on the subcommittee review: APM in patients with knee OA does not grant increased safety compared to PT. Despite short-term positive effects on pain reduction, the short- and long-term outcomes of APM on knee function, physical activity, overall health, and OA severity, as well as on the long-term effects on pain levels, did not significantly vary from those obtained with PT. In addition, the long-term incidence of TKR was higher after APM. Although APM is widely performed at present, the current scientific evidence supporting its safety and effectiveness is insufficient, and prospective clinical studies should be conducted to further elucidate this.
