수술적 근관치료와 임플란트치료의 임상적 효과성 평가

Abstract

평가배경 수술적 근관치료는 비수술적 근관치료가 성공하지 못한 경우에 시행하는데, 근관치료 환자의 약 5%가 수술적 근관치료의 대상이다. 수술적 근관치료 중 치근단 절제술은 치근단 주위를 외과적 방법으로 처치하는 것이며, 의도적 재식술은 치아를 의도적으로 발치하여 구강 밖에서 치근단 수술을 한 후에 제자리에 다시 심는 것이다. 해당 주제는 자연치아의 보존 목적으로 시행되는 수술적 근관치료의 현황 및 효과를 임플란트치료와 같이 평가하여 해당 의료기술의 적정 사용을 유도하기 위해 대국민 수요조사에서 제안되었다. 이후 2020년 제5차 의료기술재평가위원회(2020.05.11.~05.15. 서면심의)에서 의료기술재평가 안건으로 선정되어 재평가를 수행하였다.

평가방법 치근단 절제술, 의도적 재식술 및 임플란트치료의 안전성 및 유효성에 대해 체계적 문헌고찰 및 후향적 코호트연구를 수행하고, 관련 세부 수행 방법은 소위원회에서 결정되었다. 모든 평가방법은 소위원회의 논의를 거쳐 확정하였다. 평가의 핵심질문은 “수술적 근관치료와 임플란트치료의 임상적 유효성은 어떠한가”이었으며, 유효성은 치조골, 치주조직 및 보철물의 상태를 고려한 성공률 및 생존율의 지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개 및 국내 5개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌선택 및 배제기준에 따라 2명의 검토자가 독립적으로 수행하였고, 의견의 불일치가 있는 경우에는 전체 연구진 논의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림위험 평가는 비교군이 있는 연구를 대상으로 연구설계에 따라 Cochrane의 Risk of Bias 및 ROBANS ver 2.0을 사용하여 평가하였다. 자료 분석은 정량적 분석과 정성적 분석을 적용하였다. 후향적 코호트연구의 대상자는 2014~2019년 동안 건강보험 청구자료에서 치근단 절제술 혹은 임플란트치료를 받은 18세 이상의 환자로, 사랑니, 치근단 재수술, 단일임플란트 아닌 경우, 자료 오류 및 추적기간 1년 미만인 경우는 제외하였다. 임상적 효과성의 1차 결과지표는 성공률로 치조골, 치주조직 및 보철물의 상태를 고려하여 치근단 절제술에서는 술식 이후 치근단 재수술, 재식술 및 발치 모두 청구되지 않은 경우로 정의하였고, 이를 만족하지 못하는 경우 실패로 간주하여 실패의 발생률 및 위험도를 파악하였다. 임플란트치료의 성공률은 치주소파술, 치은박리소파술, 임플란트 제거술, 치조골결손부 골이식술, 조직유도재생술, 임플란트 재수술, 악골 내 고정용 금속제거술 및 보철물 재부착 중 어느 한 경우도 청구되지 않은 경우로 정의하였다. 임상적 효과성의 2차 결과지표는 생존율로 치아발치술 또는 임플란트 제거술이 시행되지 않은 경우로 정의하였고, 발치의 발생률 및 위험도를 파악하였다. 임상적 효과성은 전체 및 65세 이상군을 대상으로 성, 연령, 소득수준, 치아위치, 질병력(당뇨, 고혈압, 류마티스관절염, 골다공증, 찰슨동반상병지수(Charlson comorbidity index, CCI)), 흡연력, 요양기관종류, 임플란트치료 관련 변수(임플란트 종류, 임플란트 식립시기, 보철 식립시기)를 고려한 다변량 콕스비례위험모형을 적용하여 실패 및 발치의 위험도(Hazard ratio, HR)를 제시하였다. 하위군 분석으로 치근단 절제술 및 임플란트치료를 대상으로 성향점수 매칭 후 콕스비례위험모형을 이용하여 임상적 효과성을 파악하였다.

평가 결과 체계적 문헌고찰 수행 결과 최종 선택된 문헌은 치근단 절제술 17편(성공률 17편, 생존율 6편), 의도적 재식술 4편 및 임플란트치료 76편(성공률 5편, 생존율 76편)이었다. 최종 선택된 문헌은 모두 단일군연구로 비교군이 없었기에 비뚤림위험 평가는 수행하지 않았다. 치근단 절제술의 성공률에 선택된 17편 문헌 중 추적기간 5년 미만 문헌 14편의 성공률은 91%(95% CI: 87-93%, I2=65%)로 추적기간 5년 이상 문헌 3편의 성공률 80%(95% CI: 65-90%, I2=0%)보다 통계적으로 유의하게 높았다(P <0.01). 문헌 간 이질성이 중등도 이상으로 높았던 추적기간 5년 미만의 문헌을 대상으로 한 하위군 분석에서는 역충전재료 Mineral Trioxide Aggregate (MTA)의 성공률이 92%(95% CI: 89-94%, I2=0%)로 그 외 재료(Others)의 성공률인 87%(95% CI: 82-91%, I2=64%)보다 유의하게 높았다(P=0.05). 또한, 연구수행대륙별로는 유럽(92%, 95% CI: 88-95%, I2=1%), 북미(94%, 95% CI: 84-98%, I2=0%) 및 아시아의 성공률(87%, 95% CI: 81-92%, I2=59%) 간 차이가 유의하였다(P=0.03). 메타회귀분석에서 이질성에 영향을 주는 유의한 변수는 없었고, 출판비뚤림은 유의하지 않았으며, 민감도 분석 결과 치근단 절제술의 전체 성공률에 영향을 주는 문헌은 없었다. 치근단 절제술의 생존율에 포함된 문헌은 6편이었고, 전체 생존율은 94%(95% CI: 85-97%, I2=85%) 이었다. 의도적 재식술 생존율에 포함된 추적기간 5년 이상인 4편의 문헌에서 파악한 전체 생존율은 86%(95% CI:75-93%, I2=82%)이었다. 임플란트치료 성공률에 선택된 문헌은 추적기간 5년 이상인 5편이었으며 치조골, 치주조직 및 보철물 상태를 고려한 전체 성공률은 77%(95% CI: 65-85%, I2=68%)이었다. 임플란트치료 생존율 문헌은 76편으로, 전체 생존율은 추적기간 5년 미만 문헌들에서 99%(95% CI: 98-99%, I2=49%), 5년 이상 문헌에서는 98%(95% CI: 97-99%, I2=56%)로 두 군간 차이는 유의하지 않았다(P=0.31). 이질성이 중등도 이상으로 높았던 추적기간 5년 미만 군을 대상으로 수행한 하위군 분석에서 임플란트 유형 및 보철 형태별 임플란트치료의 생존율 차이는 유의하지 않았다. 임플란트치료 생존율의 메타회귀분석결과 유의한 변수는 없었으며, 출판비뚤림은 유의하였고, 민감도분석결과 전체 생존율에 영향을 주는 문헌은 없었다. 단일군연구만을 대상으로 한 체계적 문헌고찰 결과 치근단 절제술, 의도적 재식술 및 임플란트치료에서 모두 임상적 효과성을 확인할 수 있었다. 다만 치근단 절제술은 추적기간이 5년 이상이면 성공률이 유의하게 감소하였다. 임플란트치료의 성공률은 대상 문헌 수가 적었기에 향후 임플란트치료는 치조골, 치주조직 및 보철물의 상태를 엄격하게 평가한 성공률 기준을 적용하여 임상적 효과성을 평가하는 연구가 필요하다 2012년부터 2019년까지 건강보험 청구자료를 이용한 국내 현황분석 결과 치근단 절제술, 재식술 재식술에는 의도적 재식술 및 외상으로 인해 완전 탈구된 재식술이 포함됨. 및 임플란트치료 임플란트치료는 2014년 7월 1일에 보험급여가 시작되었기에 2014~2019년을 대상으로 함. 는 총 1,914,523명 대상 2,970,341건 수행되었다. 술식별로는 치근단 절제술 209,272건(7.0%), 재식술 41,502건(1.4%) 및 임플란트치료 2,719,567건(91.6%)이 수행되었으며, 연평균 증가율은 치근단 절제술은 0.8%, 재식술은 1.7%이며, 임플란트치료는 193.5%이었다. 치근단 절제술 및 임플란트치료의 임상적 효과성을 평가하기 위하여 건강보험 청구자료를 기반으로 후향적 코호트를 구축한 결과, 치근단 절제술의 대상 치아는 124,470개(평균연령: 49.3세)이었다. 추적기간 1~5년별 성공률은 각각 91.0%, 86.3%, 82.5%, 79.2% 및 76.5%로 추적기간 경과에 따라 성공률은 낮아졌으나, 상급종합 및 치과대학부속치과병원(이하 치대부속치과병원)에서는 5년 시점의 성공률이 85.0%로 전체 성공률보다 높았다. 연구대상을 평균 40.2개월 추적한 결과 치근단 절제술의 실패 발생률은 10,000 인년(person-year) 당 7.2이었다. 치근단 절제술의 실패위험도는 의원을 기준으로 상급종합병원 및 치대부속치과병원(HR 0.65, 95% CI: 0.61-0.68)에서 유의하게 낮았다. 치아 위치 및 유형별로는 상악 전치를 기준으로 하악 대구치(HR 3.01, 95% CI: 2.88-3.14), 상악 소구치(HR 2.35, 95% CI: 2.26-2.43), 상악 대구치(HR 1.99, 95% CI: 1.91-2.09) 및 하악 소구치(HR 1.51, 95% CI:1.42-1.61)의 순으로 실패위험도가 높았다. 그 외 남성, 연령이 높은 군, 골다공증 질병력, 찰슨동반상병지수(≧1), 과거 흡연력 군에서 치근단 절제술 실패위험도가 높았다. 치근단 절제술의 추적 기간별 생존율은 1~5년별로 각각 92.4%, 88.0%, 84.2%, 80.7% 및 77.6%로 추적기간 경과에 따라 낮아졌으나 상급종합 및 치대부속치과병원의 5년 시점 생존율은 85.5%로 전체보다 높았다. 그 외 발치에 유의하게 영향을 주는 위험요인은 실패위험도의 위험요인과 유사하였다. 임플란트치료의 연구대상은 65세 이상 2,232,654개(평균연령: 72.6세)이었으며, 추적기간 1~5년별 성공률은 각각 95.7%, 92.0%, 88.5%, 85.5% 및 82.9%이었다. 연구대상자를 평균 31개월 추적한 결과 실패 발생은 10,000 인년 당 4.0이었다. 임플란트치료의 실패위험도는 의원을 기준으로 상급종합병원 및 치과대학부속치과병원(HR 0.64, 95% CI: 0.61-0.67)에서 유의하게 낮았다. 치아 위치 및 유형별로는 상악 전치를 기준으로 하악 전치(HR 0.59, 95% CI: 0.57-0.60), 하악 소구치(HR 0.62, 95% CI: 0.61-0.63), 하악 대구치(HR 0.80, 95% CI: 0.79-0.81), 상악 소구치(HR 0.81, 95% CI: 0.80-0.83) 및 상악 대구치(HR 0.88, 95% CI: 0.87-0.90) 순으로 실패위험도가 낮았다. 그 외 70세 이상 군, 보철 식립시기(지연) 및 임플란트 식립시기(조기, 지연)에서 실패위험도는 낮았고, 남성, 고혈압, 류마티스 관절염, 골다공증, 찰슨동반상병지수(≧1), 과거흡연군에서 임플란트치료 실패위험도가 높았고 임플란트 종류별로는 유의하지 않았다. 임플란트치료의 생존율은 추적기간 1~5년별로 99.7%에서 98.6%로 거의 유사하였으며, 임플란트 제거에 유의하게 영향을 주는 위험요인은 실패위험도의 위험요인과 유사하였다. 한편, 65세 이상 치근단 절제술군 및 임플란트치료군 각각 24,826개를 성향점수 층화매칭하여 실패 및 발치 위험도를 분석한 결과 치근단 절제술의 실패위험은 임플란트치료군 보다 2.67배 높았으며(95% CI: 2.55-2.79), 발치 위험도는 32.3배 높았다(95% CI: 28.8-36.0). 후향적 코호트연구에서는 국내 공공자료원을 이용하여 치근단 절제술 및 임플란트치료의 현황을 파악하고 각 술식의 성공률과 생존율을 분석하였다. 치근단 절제술의 성공률 및 생존율은 추적기간 5년 이후에 다소 낮아졌지만, 상급종합병원 및 치과대학부속치과병원 등과 같이 엄격한 수술기준(시설, 장비, 인력 등)을 적용한 전문기관 등에서는 여전히 높았다. 한편 치근단 절제술의 성공률을 높이기 위해서는 치아위치 및 유형, 성, 연령, 흡연력 및 골다공증 등의 질병력을 함께 고려해야 할 것이다. 임플란트치료는 추적기간 5년 동안 임상적 효과성이 확인되었으며, 상급종합병원 및 치과대학 부속치과병원에서 성공률 및 생존율이 높았다. 임플란트치료의 임상적 효과성 제고를 위해 성, 연령, 임플란트 식립시기, 보철 식립시기, 흡연 및 질병력에 대한 고려가 필요하다.

결론 및 제언 소위원회는 치조골, 치주조직 및 보철물의 상태를 엄격하게 평가한 성공률 및 생존율을 기준으로 검토된 문헌적 근거 및 후향적 코호트분석을 토대로 수술적 근관치료 및 임플란트치료의 임상적 효과성을 확인하였으며, 특히 상급종합병원 및 치과대학부속치과병원 등과 같이 엄격한 수술기준(시설, 장비, 인력 등)을 적용한 전문기관 등에서 임상적 효과성이 더 높다고 평가하였다. 2021년 제11차 의료기술재평가위원회(2021.11.12.)에서는 수술적 근관치료와 임플란트치료의 임상적 효과성 평가에 대한 소위원회 결론을 최종 심의하였다.

Assessment of the clinical effectiveness of surgical endodontic treatment and dental implant treatment

Background Surgical endodontic treatment is performed when non-surgical endodontic treatment is unsuccessful. Approximately 5% of all patients who receive endodontic treatment may require surgical endodontic treatment. Among the techniques of surgical endodontic treatment, apical surgery is used to treat the periapical region, and intentional replantation involves intentional extraction of a tooth and its replantation after extraoral root canal therapy. This topic was proposed in the public needs survey to promote the appropriate use of this technique by assessing the status of surgical endodontic treatment performed for the preservation of natural teeth together with implant treatment. Subsequently, a reassessment was conducted by the fifth session of the Committee of Health Technology Reassessment in 2020 (May 11–15, 2021; written review).

Methods A systematic literature review and a retrospective cohort study were conducted on the safety and effectiveness of apical surgery, intentional replantation, and dental implant treatment. The specific methods were determined by a subcommittee. The key question of the assessment was “Are surgical endodontic treatment and dental implant treatment clinically effective?” The effectiveness was assessed based on the success rate, survival rate, and the status of the periodontal tissue and prosthesis. Three international and five domestic databases were searched based on the key question. Two reviewers independently selected the articles in accordance with the inclusion and exclusion criteria, and in the event of any disagreement, the final articles were selected through a discussion with the entire research team. Risk of bias (RoB) assessment of the selected articles was performed using the Cochrane RoB tool or RoBANS ver 2.0 depending on the control group. Data analysis was performed by qualitative review and quantitative analysis. The study population of the retrospective cohort study comprised patients aged ≥18 years who underwent apical surgery or dental implant treatment according to the 2014–2019 health insurance claims data. The following exclusion criteria were applied: extraction of wisdom teeth or apical re-surgery, placement of more than one dental implant, data error, and follow-up of <1 year. The primary indicator of clinical effectiveness was the success rate, which was defined as no claims for apical re-surgery, replantation, and extraction after the initial apical surgery considering the status of the periodontal tissues and prosthesis. Cases that did not meet these criteria were considered to have failed, and their incidence rate and hazard ratio (HR) were identified. Successful dental implant treatment was defined as no subsequent claims for any of the following: periodontal curettage, periodontal flap surgery, dental implant removal, bone grafting at alveolar bone defects, guided tissue regeneration, implant replantation, removal of hardware for internal fixation, and re-cementation. The secondary indicator of clinical effectiveness was survival rate, which was defined as no subsequent claims for tooth extraction or dental implant removal, and the incidence rate and HR of tooth extraction were identified. Clinical effectiveness was first assessed for the entire study population and also for participants aged ≥ 65 years. The results were presented as failure rate and HR of tooth extraction by applying a multivariable Cox proportional hazard model based on patient sex, age, income level, tooth position, disease history (diabetes, hypertension, rheumatoid arthritis, osteoporosis, and Charlson comorbidity index [CCI]), smoking status, type of care institution, and dental implant-related variables (type of implant, implant placement period, and prosthesis placement period). In addition, clinical effectiveness was identified by sub-group analysis on apical surgery and dental implant treatment using a Cox proportional hazard model after propensity score matching.

Assessment results The articles included in the systematic literature review included 17 articles on apical surgery (success rate in 17 articles and survival rate in 6 articles), 4 articles on intentional replantation, and 76 articles on dental implant treatment (success rate in 5 articles and survival rate in 76 articles). Since all the articles selected were on single-group studies with no control group, RoB assessment was not performed. Among the 17 studies that reported on the success rate of apical surgery, 14 studies with a follow-up of <5 years reported a success rate of 91% (95% CI: 87–93%, I2=65%), which was significantly higher than the rate of 80% reported in three articles with follow-up of ≥5 years (95% CI: 65–90%, I2=0%; P <0.01). In the sub-group analysis on articles with follow-up period of <5 years that showed moderate or higher heterogeneity, the success rate of retro-filling material mineral trioxide aggregate (MTA) was 92% (95% CI: 89–94%, I2=0%), which was significantly higher than the 87% success rate for other materials (95% CI: 82–91%, I2=64%; P=0.05). In addition, there were significant differences in the success rate based on the continent where the study was conducted (P=0.03): Europe (92%, 95% CI: 88–95%, I2=1%), North America (94%, 95% CI: 84–98%, I2=0%), and Asia (87%, 95% CI: 81–92%, I2=59%). Meta-regression analysis results showed no variables with significant influence on heterogeneity, while publication bias was not significant. Moreover, sensitivity analysis too did not identify any study that influenced the overall success rate of apical surgery. In six studies that reported on the survival rate of apical surgery, the overall survival rate was 94% (95% CI: 85–97%, I2=85%). In four studies that reported on the survival rate of intentional replantation with follow-up of ≥5 years, the overall survival rate was 86% (95% CI: 75–93%, I2=82%). In five studies with a follow-up of ≥5 years that reported on the success rate of dental implant treatment, the overall success rate considering the status of the periodontal tissues and prosthesis was 77% (95% CI: 65–85%, I2=68%). In 76 studies that reported on the survival rate of dental implant treatment, the overall survival rate was 99% in those with follow-up <5 years (95% CI: 98–99%, I2=49%) and 98% in those with follow-up ≥5 years (95% CI: 97–99%, I2=56%), showing no significant difference between the two groups (P=0.31). In the sub-group analysis on studies with a follow-up of <5 years that showed moderate or higher heterogeneity, there was no significant difference in the survival rate of dental implant treatment based on implant type and prothesis shape. Meta-regression analysis of the survival rate of dental implants showed no significant variables, while publication bias was significant. Moreover, sensitivity analysis too did not identify any study that influenced the overall survival rate. In the systematic literature review on only single-group studies, clinical effectiveness of apical surgery, intentional replantation, and dental implant treatment was verified. However, the success rate of apical surgery decreased significantly when the follow-up period was of ≥5 years. Since there was only a small number of studies on the success rate of dental implant treatment, future studies need to assess the clinical effectiveness of dental implant treatment by evaluating the success rate based on rigorous assessment of the status of the periodontal tissues and prosthesis. The analysis was performed using the 2012–2019 health insurance claims data from Korea, which included information regarding apical surgery, replantation, and dental implant treatment. Replantation included intentional replantation and replantation for complete dislocation due to trauma. For dental implant treatment, data from 2014–2019 were used since this procedure became an insurance benefit item on July 1, 2014. A total of 1,914,523 patients and 2,970,341 cases were included. Apical surgery, replantation, and dental implant treatment were performed in 209,272 (7.0%), 41,502 (1.4%), and 2,719,567 (91.6%) cases, respectively, while the rate of annual average increase in the procedures was 0.8%, 1.7%, and 193.5%, respectively. A retrospective cohort was constructed based on the health insurance claims data to assess the clinical effectiveness of apical surgery and dental implant treatment. The results showed that apical surgery was performed on 124,470 teeth (mean age: 49.3 years). The success rate during a follow-up of 1–5 years was 91.0%, 86.3%, 82.5%, 79.2%, and 76.5%, respectively, showing a decreasing trend in success rate with an increasing follow-up duration. Meanwhile, the success rate after five years in tertiary and university-affiliated dental hospitals was 85.0%, which was higher than the overall success rate. The failure rate of apical surgery after a mean follow-up of 40.2 months was 7.2 per 10,000 person-years. The HR of apical surgery failure was significantly lower in tertiary and university-affiliated dental hospitals relative to dental clinics (HR 0.65, 95% CI: 0.61–0.68). With respect to tooth position and type, the HR of failure relative to maxillary anterior teeth was highest in mandibular molars (HR 3.01, 95% CI: 2.88–3.14), followed in order by maxillary premolars (HR 2.35, 95% CI: 2.26–2.43), maxillary molars (HR 1.99, 95% CI: 1.91–2.09), and mandibular premolars (HR 1.51, 95% CI: 1.42–1.61). In addition, the HR of apical surgery failure was also higher among men, older patients, patients with a history of osteoporosis, patients with CCI score of ≥1, and former smokers. The survival rate of apical surgery at a follow-up of 1–5 years was 92.4%, 88.0%, 84.2%, 80.7%, and 77.6%, respectively, showing a decreasing trend in survival rate with an increasing follow-up duration. Meanwhile, the survival rate after five years in tertiary and university-affiliated dental hospitals was 85.5%, which was higher than the overall survival rate. In addition, the risk factors with significant influence on tooth extraction were similar to those for procedure failure. Dental implant treatment was performed on 2,232,654 teeth in patients aged ≥65 years (mean age: 72.6 years). The success rate at a follow-up of 1–5 years was 95.7%, 92.0%, 88.5%, 85.5%, and 82.9%, respectively. The failure rate after a mean follow-up of 31 months was 4.0 per 10,000 person-years. The HR of dental implant failure was significantly lower in tertiary and university-affiliated dental hospitals relative to dental clinics (HR 0.64, 95% CI: 0.61–0.67). With respect to tooth position and type, the HR of failure relative to maxillary anterior teeth was highest in mandibular anterior teeth (HR 0.59, 95% CI: 0.57–0.60), followed in order by mandibular premolars (HR 0.62, 95% CI: 0.610–.63), mandibular molars (HR 0.80, 95% CI: 0.79–0.81), maxillary premolars (HR 0.81, 95% CI: 0.80–0.83), and maxillary molars (HR 0.88, 95% CI: 0.87–0.90). In addition, the HR of dental implant failure was also lower among patients aged ≥70 years, and based on prosthesis placement period (delayed) and implant placement period (early and delayed), it was higher among men, patients with hypertension, rheumatoid arthritis, or osteoporosis, patients with CCI score of ≥1, and former smokers; there was no significant difference based on implant type. The survival rate of dental implant treatment at a follow-up of 1–5 years was similar, ranging between 99.7% and 98.6%. In addition, the risk factors with significant influence on implant removal were similar to those for procedure failure. Meanwhile, analysis of 24,826 teeth each in apical surgery and dental implant treatment groups of patients aged ≥65 years for the HR of failure and tooth extraction after stratified propensity score matching showed that the HR of failure was 2.67 times higher (95% CI: 2.55–2.79) and that of tooth extraction was 32.3 times higher (95% CI: 28.8–36.0) in the apical surgery group than in the dental implant treatment group. In the retrospective cohort study, the current status of apical surgery and dental implant was identified using public data sources in Korea, along with the success and survival rates of both procedures. The success and survival rates of apical surgery decreased slightly after five years, but the rates remained high in specialized institutions, such as tertiary and university-affiliated dental hospitals, which maintain strict operating standards (facility, equipment, personnel, etc.). Meanwhile, factors such as the position and type of teeth, sex, age, smoking status, and history of osteoporosis should also be considered to increase the success rate of apical surgery. The clinical effectiveness of dental implant treatment during five years of follow-up was verified, with tertiary and university-affiliated dental hospitals showing higher success and survival rates.
