소아청소년 대상 키 성장 목적의 성장호르몬 치료

Abstract

평가 배경 동 안건은 국민건강보험 요양급여의 기준에 관한 규칙 [별표 2] 비급여대상 중 ‘질병 치료가 아닌 단순히 키 성장을 목적으로 하는 진료’를 국민생활밀착형 의료기술로써 대국민 정보 제공을 위해 임상적 안전성 및 효과성의 확인이 필요한 의료기술재평가 안건으로 선정하였고, 이에 성장호르몬(growth hormone) 치료에 대한 재평가를 시행하는 것으로 심의되었다(2021년 제2차 의료기술재평가위원회, 2021.02.19.). 현재 국내에서는 소아성장호르몬결핍증, 터너증후군, 소아만성신부전, 프라더윌리증후군, 따라잡기 성장을 하지 못한 부당경량아, 누난증후군으로 인한 저신장증에만 성장호르몬 치료가 급여로 인정되고 있다. 이에 비급여로 사용되고 있는 성장호르몬 치료에 관한 국민적 관심이 합리적인 의료적 결정으로 이어질 수 있도록 본 평가에서는 체계적 문헌고찰을 통해 소아청소년 대상 키 성장 목적의 성장호르몬 치료의 안전성과 효과성에 관한 객관적 정보를 제공하고자 한다. 현재 건강보험에서 급여로 인정되지 않지만, 국내 식품의약품안전처로부터 허가를 승인받은 부분은 특발성 저신장증(idiopathic short stature) 이다. 특발성 저신장증이란, 소아의 키가 동일 연령 및 성을 가진 소아의 평균 키에 비하여 표준편차점수(standard deviation score, SDS) -2 미만이거나, 3백분위수 미만인 경우로, 저신장을 유발할 만한 원인이 없고, 출생 시 정상체중을 가지고 태어났으며, 사지와 체간(척추) 체형이 정상이고, 충분한 영양을 섭취하며, 정신사회적 문제가 없으면서, 성장호르몬 분비가 정상인 아동을 지칭한다. 본 평가에서는 기존 급여 외에 키 성장 목적으로 성장호르몬을 치료하는 모든 대상자를 포함하여 평가하고자 하였으며, 이에 특발성 저신장증뿐만 아니라, 특발성 저신장증 기준을 초과하는 정상 대상자(허가범위 초과)까지 포함하여 성장호르몬의 안전성 및 효과성에 대해 검토하였다.

평가 방법 동 안건의 안전성 및 효과성을 재평가하기 위하여 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “소아청소년 대상 키 성장 목적의 성장호르몬 치료 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌 선택 및 배제기준에 따라 두 명의 검토자가 독립적으로 수행하고, 의견의 불일치가 있는 경우, 소위원회 논의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림 위험 평가는 효과성 검토에 포함된 무작위 배정 임상시험 연구(randomized controlled trial, RCT)에 대해 수행하였으며, 비뚤림 위험 평가 도구로는 Cochrane의 Risk of Bias (RoB)를 사용하였다. 자료분석은 정량적 분석과 정성적 분석을 적용하였다. 최종적으로 본 평가에서 수행한 체계적 문헌고찰 결과를 바탕으로 소아청소년 대상 키 성장 목적의 성장호르몬 치료에 대해 대국민 정보를 제공하고자 하였다.

평가 결과 소아청소년 대상 키 성장 목적의 성장호르몬 치료의 임상적 안전성 및 효과성을 평가하기 위한 체계적 문헌고찰에는 총 40편의 연구가 선택되었다. 선택된 40편의 연구는 모두 특발성 저신장증을 대상으로 하였으며, 본 평가에서 검토하고자 했던 정상 대상자(허가범위 초과) 관련 연구는 확인되지 않았다. 이에 체계적 문헌고찰에서는 특발성 저신장증을 대상으로 한 연구 40편을 포함하였으며, 안전성 검토에는 35편, 효과성 검토에는 14편(RCT 12개)의 문헌이 포함되었다.

안전성 특발성 저신장증 대상 성장호르몬 치료의 안전성은 부작용 및 이상반응으로 평가하였다. 일차 연구들에서 제시하고 있는 성장호르몬 치료의 부작용 종류 및 보고형식이 다양하여, 1) 전체 부작용 발생률과 2) 특정 부작용 사례로 구분하여 검토하였다. 또한, 특정 부작용 사례의 경우, 소위원회 논의를 바탕으로 성장호르몬 치료와 관련이 있는 주요 부작용을 선정하여 검토하였다. 전체 부작용 발생률을 보고한 연구 결과를 종합하면, 이상사례 발생률 0~91.2%, 중대한 이상사례 0~13.0%로 다양하였으며, 연구 특성에 따라 보고된 전체 부작용 발생률에 차이가 있었으나, 대부분 경증으로, 성장호르몬 치료와의 관련성이 없는 것으로 확인되었다. 또한, 성장호르몬 치료와의 관련성이 확인된 부작용은 드물게 나타나는 것으로 확인되었다. 특정 부작용 사례를 보고한 연구 결과를 종합하면, 다음과 같다. 1) 종양: 4편(3개 연구)에서 보고된 종양 발생률은 0%에서 최대 0.72%이었다. 해당 연구들에서 종양 발생은 성장호르몬 치료와의 관련성이 없는 것으로 확인하였다고 보고하였다. 2) 고혈당: 고혈당 관련 부작용 결과를 제시한 문헌은 총 9편(7개 연구)으로, 성장호르몬 치료 시 고혈당은 0~21.5%, 고인슐린혈증은 17% 수준이었으며, 대부분 성장호르몬 치료 중단 후 정상으로 회복되었다고 보고하였다. 또한, 당뇨병 발생률은 0.1% 이하로 보고되었다. 3) 척추측만증: 8편(6개 연구)에서 보고된 척추측만증 발생률은 0~6.8%이었으며, 성장호르몬 치료 이전에 척추측만증이 있었던 아동에서의 재발률은 16.4% (11/75명)이었다. 또한, 1개의 RCT (Leschek et al., 2004)에서 척추측만증 발생률은 성장호르몬 치료군과 대조군(위약군) 간 유의한 차이가 없다고 보고하였다. 4) 양성 두개내압 상승: 7편(6개 연구)의 문헌에서 보고된 양성 두 개내압 상승 발생률은 모두 0%이었다. 5) 대퇴골두골단분리증: 5편(4개 연구)의 문헌에서 보고된 대퇴골두골단분리증 발생률은 0~0.4%이었다. 6) 갑상선저하증: 8편(7개 연구)의 문헌에서 보고된 갑상선저하증 발생률은 0~6%이었다. 이 중 가장 높은 발생률(6%)을 보고한 Ying 등(2018)의 연구에 따르면, 갑상선 기능이 저하된 경우 갑상선호르몬 제제로 치료하였으며 해당 수치는 성장호르몬 결핍증 환자에서의 발생률에 비해서는 낮은 결과라고 밝혔다. 또한, 다른 1편(김덕희 등, 1996)에서는 특발성 저신장증 98명을 대상으로 한 코호트 연구로, 성장호르몬 치료에 따른 Free T4 검사 결과, 치료 전 1.06±0.28에서 1.19±0.19로 치료 전-후 차이가 없었다고 보고하였다. 7) 여성형 유방 관련 부작용: 3편(2개 연구)의 문헌에서 보고된 여성형 유방 발생률은 0%, 0.32%이었다. 8) 관절통: 3편(2개 연구)의 문헌에서 보고한 관전통 발생률은 1.8~2.6%이었다. 9) 과민증: 과민증 관련 부작용을 보고한 4편의 결과를 살펴보면, 두드러기 8.3% (3명), 발진 2.2% (1명), 피부소양감 3.1% (3명), 주사 부위 피부 발적 및 부종이 7.41% (2명)에서 발생하였으며, 주사 부위 발적 및 부종의 경우, 온찜질 후 회복되었다고 밝히고 있다. 10) 기타: 성장호르몬 치료에 따른 기타(부종[얼굴 부음], 점 크기 증가) 부작용 결과를 제시한 문헌은 1편으로 해당 문헌에서는 성장호르몬 치료에 따른 부작용으로 손과 발이 커지는 경우가 3.1% (3명)에서 확인되었다고 보고하였다. 이상 언급된 안전성 결과를 종합하면, 발생된 부작용은 성장호르몬 치료와의 관련성이 적으며, 성장호르몬 중단으로 해소되었다고 보고되는 경향을 나타내었다. 또한, 특정 부작용 사례는 대부분 성장호르몬 치료로 인해 예상되는 부작용의 범위를 초과하지 않는 것으로 확인되었다. 이에 소위원회는 현재 문헌적 근거에 기반해서 성장호르몬 치료의 안전성은 임상 적용이 가능한 수준으로 수용 가능하다고 판단하였다. 다만, 소위원회에서는 안전성 검토에 포함된 문헌의 추적관찰기간이 대부분 단기간(최대 10년)이므로, 장기간 추척관찰한 연구가 필요하다는 의견이었다.

효과성 특발성 저신장증 대상 성장호르몬 치료의 효과성은 성인 신장, 성인 신장 표준편차점수, 신장, 신장 표준편차점수, 키 성장 속도, 키 성장 속도 표준편차점수, 심리적 지표로 평가하였다. 효과성 검토에 RCT 12개(문헌 기준 14편)가 포함되었으며, 포함된 연구의 총 대상자수는 중재군 504명, 대조군 308명으로 총 812명이었다. 성인 신장(adult height)은 2개의 RCT에서 보고되었으며, 메타분석 결과 성장호르몬 치료를 하지 않은 군에 비해 성장호르몬 치료군에서 성인 신장이 유의하게 큰 것으로 확인되었다(MD 5.29 cm; 95% CI 2.77, 7.82). 성인 신장 표준편차점수(standard deviation score)는 3개의 RCT에서 보고되었으며, 메타분석 결과, 성장호르몬 치료군과 치료를 하지 않은 군 간에 성인 신장 표준편차점수는 차이가 없었다(MD 0.48; 95% CI –0.29, 1.25). 신장(height)은 1개의 RCT에서 보고되었으며, 성장호르몬 치료 후 4년 시점에 치료군과 무치료군 간 통계적으로 유의한 차이가 확인되었다고 보고하였다(p <0.0005). 신장 표준편차점수는 8개의 RCT에서 보고되었으며, 메타분석 결과, 무치료군에 비해 성장호르몬 치료군에서의 신장 표준편차점수가 통계적으로 유의하게 큰 것으로 확인되었다(MD 0.59; 95% CI 0.40, 0.79). 키 성장 속도는 6개의 RCT에서 보고되었으며, 무치료군에 비해 성장호르몬 치료는 키 성장 속도를 통계적으로 유의하게 증가시키는 것으로 확인되었다(MD 3.93 cm/year; 95% CI 2.89, 4.97). 키 성장 속도 표준편차점수는 1편의 연구에서 보고되었으며, 표준용량 성장호르몬 치료군, 고용량 성장호르몬 치료군, 대조군(무치료군), 세 군 간 차이가 통계적으로 유의하였다고 보고하였다(p <0.001). 성장호르몬 치료에 따른 심리적 만족감, 삶의 질 개선과 같은 심리적 지표 관련 결과는 확인되지 않았다. 이상 효과성 결과를 종합하면, 특발성 저신장증 아동에서 성장호르몬 치료는 단기간 신장 및 성장 속도 증가에 도움이 되며, 대조군(무치료군 또는 위약군)에 비해 성인 신장이 5.29 cm 유의하게 증가하는 것으로 확인되었으나, 성인 신장 표준편차점수에서는 군 간 차이가 확인되지 않았다. 이에 특발성 저신장증 아동에서 성장호르몬 치료는 단기간 신장 및 성장 속도 증가에 도움이 되나, 성인 신장 증가 효과는 불명확한 것으로 판단하였다. 소위원회에서는 성장호르몬 치료의 효과는 개인별 차이가 있을 수 있으며, 성인 신장 증가 효과를 명확하게 확인하기 위해서는 잘 설계된 장기간 연구가 필요하다는 의견이었다.

결론 및 제언 소위원회는 현재 문헌에 근거하여 소아청소년 대상 키 성장 목적의 성장호르몬 치료의 안전성 및 효과성 결과를 다음과 같이 제시하였다. 첫째, 특발성 저신장증 대상 성장호르몬 치료의 안전성 검토 결과, 발생된 부작용과 성장호르몬 치료와의 관련성이 적으며, 성장호르몬 중단 조치 등으로 인해 대부분 부작용이 해소되는 것으로 보고되었다. 또한, 확인된 부작용은 대부분 성장호르몬 치료로 인해 예상되는 부작용의 범위를 초과하지 않는 것으로 확인되었다. 이에 현재 문헌적 근거에 기반해서 안전성은 임상 적용이 가능한 수준으로, 수용 가능하다고 판단하였다. 특발성 저신장증 대상 성장호르몬 치료의 효과성은 단기간 신장 및 성장 속도 증가에 도움이 되나, 성인 신장 증가 효과는 불명확한 것으로 판단하였다. 이에 소위원회에서는 성장호르몬 치료의 효과는 개인별 차이가 있을 수 있으며, 성인 신장 증가 효과를 명확하게 하기 위해서는 잘 설계된 장기간 연구가 필요하다는 의견이었다. 둘째, 성장호르몬의 허가범위를 초과하는 대상자(off-label)에서의 성장호르몬 사용에 대한 근거는 확인되지 않았다. 이에 국외 가이드라인(European Society of Paediatric Endocrinology, Growth Hormone Research Society, Pediatric Endocrine Society, 2016) 및 소위원회 논의를 바탕으로 허가범위를 초과한 성장호르몬 사용은 과학적 근거가 부족하여 권고하지 않으며, 오직 임상연구 상황에서만 적용되어야 한다는 의견이었다. 2021년 제12차 의료기술재평가위원회(2021.12.10.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “소아청소년 대상 키 성장 목적의 성장호르몬 치료”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 현재 문헌적 근거에 기반해서 특발성 저신장증 대상 성장호르몬 치료의 안전성은 임상 적용이 가능한 수준으로, 수용 가능하다고 판단하였다. 다만, 안전성 검토에 포함된 문헌의 추적관찰기간이 대부분 단기간이므로 장기간 추적관찰한 연구가 필요하다는 의견이었다. 특발성 저신장증 대상 성장호르몬 치료의 효과성은 단기간 신장 및 성장 속도 증가에 도움이 되나, 성인 신장에 대한 성장호르몬의 치료효과가 일관되지는 않은 것으로 판단하였다. 이에 성장호르몬 치료의 효과는 개인별 차이가 있을 수 있으며, 성인 신장 증가 효과를 명확하게 확인하기 위해서는 잘 설계된 장기간 연구가 필요하다는 의견이었다. 성장호르몬의 허가범위를 초과하는 대상자(off-label)에서의 성장호르몬의 사용에 대한 근거는 확인되지 않았다. 이에 의료기술재평가위원회는 국외 가이드라인 및 소위원회 검토의견을 바탕으로 허가범위를 초과하여 성장호르몬을 사용하는 것은 과학적 근거 부족으로 인해 권고되지 않으며, 오직 임상연구 상황에서만 적용되어야 한다고 심의하였다. 본 평가 결과를 바탕으로 국민들의 합리적 선택을 지원하기 위해 대국민정보를 제공하였다.

Growth hormone therapy for children and adolescents with short stature

Background This agenda item was selected as a subject of health technology reassessment that requires the verification of clinical safety and effectiveness for providing public information. Accordingly, the determination was made to conduct a reassessment on growth hormone therapy (GHT) (2021 second session of the Committee of Health Technology Reassessment; February 19, 2021). Currently, in South Korea, GHT is covered by insurance only for short stature due to pediatric growth hormone deficiency, Turner syndrome, pediatric chronic renal failure, Prader–Willi syndrome, small for gestational age children without catch-up growth, and Noonan syndrome. Accordingly, this assessment aimed to provide objective information about the safety and effectiveness of GHT for height growth in children and adolescents through a systematic literature review to enable the public interest in GHT used as a non-benefit item to lead to reasonable medical decisions. Although it is currently not covered by health insurance, it has been approved for use by the Ministry of Food and Drug Safety for idiopathic short stature. Idiopathic short stature refers to a condition in which a child has a height with a standard deviation score (SDS) of <-2 relative to the average height for a given age and sex or height below the third percentile, no identifiable cause of short stature, normal birth weight, normal body type with respect to limbs and trunk (spine), sufficient intake of nutrition, no psychosocial problems, and normal secretion of growth hormones. The purpose of this systematic review was to investigate the safety and effectiveness of GHT for children and adolescents with short stature who are not covered by insurance.

Methods A systematic review was conducted to reassess the clinical safety and effectiveness of GHT. All the assessment methods were finalized after a review by the “GHT for the purpose of height growth in children and adolescents subcommittee” (hereinafter “Subcommittee”). For the systematic literature review, three international and five domestic databases were searched based on the key question. Two reviewers independently selected the articles in accordance with the inclusion and exclusion criteria, and in the event of any disagreement, the final articles were selected through a discussion by the Subcommittee. A risk of bias (RoB) assessment of the articles was performed on the randomized controlled trials (RCTs) included in the review of effectiveness using the Cochrane RoB tool. Quantitative and qualitative data analyses were conducted. The ultimate goal was to provide public information about GHT for the purpose of height growth in children and adolescents based on the results from the systematic literature review performed in this assessment.

Results Forty articles were selected for the systematic literature review in the assessment of the clinical safety and effectiveness of GHT for the purpose of children and adolescents with short stature. All 40 articles were related to idiopathic short stature, including 35 articles for the review of safety and 14 (12 RCTs) for the review of effectiveness.

Safety The safety of GHT for idiopathic short stature was assessed based on adverse events (AEs) and adverse reactions (ARs). Due to the variety in the types of AEs reported and the reporting formats, major GHT-related AEs were reviewed by 1) total incidence of AEs and 2) cases involving specific AEs. With respect to the cases involving specific AEs, major GHT-related AEs were selected based on a discussion by the Subcommittee and reviewed accordingly. In studies that reported on the total incidence of AEs, the incidence of AEs varied widely between 0 and 91.2%, and the incidence of serious AEs also varied between 0 and 13.0%. Whereas the total incidence of AEs reported varied according to the characteristics of the studies, most AEs were found to be mild and unrelated to GHT. Moreover, GHT-related AEs appeared rarely. The study results on the cases of specific AEs reported were as follows: 1) Tumor: The incidence of tumors reported in four articles (three studies) was between 0% and 0.72%. The articles reported that the incidence of tumors found in these studies were unrelated to GHT. 2) Hyperglycemia: Nine articles (seven studies) reported hyperglycemia-related AEs. During GHT, the incidence of hyperglycemia was 0–21.5% and that of hyperinsulinemia was 17%, although most patients showed returns to normal levels upon discontinuation of GHT. Moreover, the incidence of diabetes was reported to be <0.1%. 3) Scoliosis: The incidence of scoliosis reported in eight articles (six studies) was 0–6.8%, whereas the recurrence rate among children with scoliosis before GHT was 16.4% (11/75 children). Moreover, one RCT (Leschek et al., 2004) reported no significant difference in the incidence of scoliosis between the GHT and control (placebo) groups. 4) Benign intracranial hypertension: All seven articles (six studies) that mentioned benign intracranial hypertension reported an incidence of 0%. 5) Slipped capital femoral epiphysis: The incidence of slipped capital femoral epiphysis reported in five articles (four studies) was 0–0.4%. 6) Hypothyroidism: The incidence of hypothyroidism reported in eight articles (seven studies) was 0–6%. According to a study by Ying et al. (2018), which reported the highest incidence (6%), patients with hypothyroidism were treated with thyroid hormone agents, and the values were lower than the incidence in patients with growth hormone deficiency. In another cohort study on 98 patients with idiopathic short stature (Kim et al., 1996), free T4 test results showed no difference between before (1.06±0.28) and after (1.19±0.19) GHT. 7) Gynecomastia: The incidence of gynecomastia reported in two studies was 0% and 0.32%. 8) Arthralgia: The incidence of arthralgia reported in three articles (two studies) was 1.8–2.6%. 9) Hypersensitivity: Hypersensitivity-related AEs reported in four articles included urticaria (n=3, 8.3%), rash (n=1, 2.2%), skin redness (n=3, 3.1%), and injection site redness and swelling (n=2, 7.41%). In cases involving injection site redness and swelling, recovery was achieved after heat therapy. 10) Others: One article reported on other GHT-related AEs (swelling [face] and increased spot size), in which swelling of the hands and feet was reported as a GHT-related AE in three patients (3.1%). Based on the aforementioned safety outcomes, AEs that had occurred had little association with GHT and tended to be resolved with the discontinuation of GHT. In addition, most of the cases involving specific AEs did not deviate from the range of AEs typically expected with GHT. Accordingly, the Subcommittee determined that the safety of GHT is clinically acceptable based on the current literature evidence. However, the Subcommittee also opined that additional long-term follow-up studies are needed since most of the articles included in the safety assessment had short follow-up periods.

Effectiveness The effectiveness of GHT for idiopathic short stature was assessed based on adult height, adult height SDS, height, height SDS, height growth rate, height growth rate SDS, and psychological indicators. A total of 12 RCTs (14 articles) were included in the effectiveness assessment. The total number of participants included in these RCTs was 812, i.e., 504 in the intervention group and 308 in the control group. Adult height was reported in two RCTs, and the meta-analysis results showed that the GHT group had significantly higher adult height than the non-GHT group (MD 5.29 cm; 95% confidence interval [CI] 2.77, 7.82). Adult height SDS was reported in three RCTs, and the meta-analysis results showed no difference in adult height SDS between the GHT and non-GHT groups (MD 0.48; 95% CI –0.29, 1.25). Height was reported in one RCT, which reported no significant difference between the treatment and non-treatment groups at four years after GHT (p<0.0005). Height SDS was reported in eight RCTs, and the meta-analysis results showed that the GHT group had a statistically significantly higher height SDS than the non-GHT group (MD 0.59; 95% CI 0.40, 0.79). Height growth rate was reported in six RCTs. The results showed a statistically significant increase in the GHT group compared with the non-GHT group (MD 3.93 cm/year; 95% CI 2.89, 4.97). Height growth rate SDS was reported in one study, which reported significant differences among the three groups—standard dose GHT, high-dose GHT, and control (non-GHT) groups (p<0.001). Results related to psychological indicators, such as psychological satisfaction and improvement in quality of life, were not verified. The aforementioned effectiveness outcomes showed that GHT can be helpful for the short-term increase in height and growth rate among children with idiopathic short stature. Moreover, although a significant increase of 5.29 cm in adult height was found in the GHT group as compared with the control group (no treatment or placebo), there was no significant difference in adult height SDS between the groups. Accordingly, it was determined that GHT can be helpful for short-term increase in height and growth rate among children with idiopathic short stature, although its effect on increasing adult height is unclear. The Subcommittee also opined that the effectiveness of GHT may vary among individuals, and well-designed long-term studies are needed to clearly identify its effect on increasing adult height.

Conclusions and Recommendations The Committee of Health Technology Reassessment determined that the safety of GHT for idiopathic short stature is clinically acceptable based on the current literature evidence. However, additional long-term follow-up studies are needed since most of the articles included in the safety assessment had short follow-up periods. With respect to the effectiveness of GHT for idiopathic short stature, it can be helpful for short-term increase in height and growth, although the therapeutic effect of GHT for adult growth is determined to be inconsistent. Accordingly, it is opined that the effectiveness of GHT may vary among individuals, and well-designed long-term studies are needed to clearly identify its effect on increasing adult height. Evidence for using GHT in subjects who exceed the allowable range of growth hormone (off-label use) was not verified. Accordingly, the Committee of Health Technology Reassessment does not recommend the use of GHT that exceeds the allowable range due to the lack of scientific evidence, based on international guidelines and discussion by the Subcommittee, and that such use should be permitted only in clinical trials. Public information for supporting reasonable choices by the public was provided based on the findings in this assessment.
