모발검사(미네랄·중금속)

Abstract

평가 배경 모발검사(미네랄·중금속)는 건강상태 확인 등의 목적으로 모발조직에 축적된 미네랄·중금속을 분석하여 체내 영양 미네랄의 불균형 상태와 중금속 오염 및 미네랄 간의 상호작용을 측정하는 검사로서, 선택 비급여 의료기술에 해당한다. 선택 비급여는 신체적 필수 기능개선을 직접 목적으로 하지 않고 의료소비자의 선택에 의해 사용되는 의료기술을 의미한다. 의료소비자인 국민들에게 모발검사(미네랄·중금속)에 대해 올바른 정보를 제공하고 이를 통해 해당 의료기술의 적정사용을 지원하고자 내부 모니터링으로 발굴된 주제이다. 2022년 제5차 의료기술재평가위원회(2022.05.13.)에서는 체계적 문헌고찰 검토를 통해 모발검사(미네랄·중금속)의 임상적 유용성에 대한 의과학적 근거를 평가하고, 동 안건에 대하여 대국민 정보문을 제공하는 것으로 심의하였다.

평가 방법 동 평가에서는 모발검사(미네랄·중금속)의 임상적 유용성을 확인하기 위하여 체계적 문헌고찰의 검토(Overview of Systematic Reviews)를 수행하였다. 모든 평가방법은 평가목적을 고려하여 가정의학과 2인, 진단검사의학과 2인, 근거기반의학 1인으로 구성된 “모발검사(미네랄·중금속) 재평가 소위원회(이하 ‘소위원회’)”의 심의를 거쳐 확정하였다. 동 평가의 핵심질문은 ‘건강상태 확인 등의 목적으로 사용하는 모발검사(미네랄·중금속)는 임상적으로 안전하고 효과적인가?’ 이었다. 본 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 선택문헌에 대한 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 수행하였으며, 해당 문헌들의 비뚤림위험은 AMSTAR 2(A MeaSurement Tool to Assess systematic Reviews 2) 도구를 사용하여 평가하였다. 자료추출 및 비뚤림위험평가는 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 논의하여 합의하였다. 자료합성은 선택문헌에서 보고한 결과지표가 상이하여 양적 합성이 불가능하여 질적 검토 방법을 적용하였다. 2022년 제12차 의료기술재평가위원회(2022.12.09.)에서는 소위원회의 검토 의견을 고려하여 대국민 정보문을 확정하였다.

평가 결과 최종 선택문헌은 체계적 문헌고찰 4편으로 출판연도는 2020년대 1편, 2010년대 1편, 2000년대 2편이었다. 모두 국외 문헌이었으며, 멕시코, 폴란드, 영국, 홍콩에서 각 1편씩 출판되었다. 4편의 체계적 문헌고찰 문헌을 대상으로 AMSTAR 2를 이용하여 비뚤림위험 평가를 수행한 결과, 전반적 신뢰도가 매우 낮음(75%, 3편) 또는 낮음(25%, 1편)으로 전반적인 비뚤림위험은 높은 것으로 나타났다.

안전성 모발검사(미네랄·중금속)의 안전성은 위음성 및 위양성으로 평가하고자 하였으나, 해당 지표를 보고한 문헌은 없었다. 소위원회는 동 검사는 신체에 위해를 가하지 않는 비침습적인 검사이므로 안전성에는 문제가 없다는 의견이었다.

효과성 모발검사(미네랄·중금속)의 효과성은 진단정확성, 의료결과와의 상관성으로 평가하고자 하였으나, 해당 지표를 보고한 문헌은 없었다. 이에 소위원회에서는 건강인을 대상으로 모발검사를 수행한 연구의 기타 결과지표를 검토하였다. Lavalle-Carrasco 등(2021)은 불소 노출 인구집단에서 불소 농도를 관찰하기 위해 모발검사를 수행한 5편의 선택문헌에 대하여 질적으로 기술하였으며, 이 중 4편에서는 모발이 불소 노출 정도를 측정하는 데 적절하다고 보았으며, 1편은 적절한 생체표지자가 아닐 수 있다고 명시하였다. Mikulewicz 등(2013)의 연구에서는 분석 방법론, 특히 시료 준비, 분해, 분석 방법이 상이하여 참조값으로 보고된 모발 미네랄 수치가 다양했으며, 표준적인 절차를 정교화하고, 나아가 모발 미네랄 검사를 검증하고 자세한 방법론을 제공하는 것이 필요하다고 명시하였다. Lowe 등(2009)의 연구에서는 모발 중 아연 농도가 아연 보충제 섭취 이후 유의하게 증가하였으며, 건강인에서 모발 중 아연은 신뢰할 만한 생체표지자라고 보았다. Ng 등(2007)은 아동 및 청소년에서 수은 노출을 측정하기 위한 모발검사와 소변과의 상관계수는 0.459, 제대혈의 경우 0.640, 혈액의 경우 0.6107로 보고하였으며, 모발 중 수은과 혈액 중 수은 간 상관성이 확인되었지만 임상에서 혈액 혹은 소변 중 수은 검사를 대체할 만큼 상관성이 높지 않다고 보았다.

결론 및 제언 모발검사(미네랄·중금속) 재평가 소위원회는 현재 평가 결과에 근거하여 다음과 같이 제언하였다. 모발검사(미네랄·중금속)는 신체에 위해를 가하지 않는 비침습적인 검사로 안전성에는 문제가 없으나, 체내 미네랄과 중금속의 농도를 측정하기 위한 검사로서의 정확성과 임상적 유용성을 입증하는 문헌적인 근거가 없다고 평가하였다. 2022년 제12차 의료기술재평가위원회(2022.12.09.)에서는 소위원회 검토 결과와 대국민 정보문에 대해 논의하였으며, 의료기술재평가사업 관리지침 제4조 제10항에 의거 “모발검사(미네랄·중금속)”에 대해 소위원회 결론은 원안대로 심의 의결하고 대국민 정보문을 일부 수정하여 심의 의결하였다. 의료기술재평가위원회는 모발검사(미네랄·중금속)는 신체에 위해를 가하지 않는 비침습적인 검사이므로 안전성에는 문제가 없으나, 체내 미네랄과 중금속의 농도를 측정하기 위한 검사로서의 정확성과 임상적 유용성을 입증하는 문헌적인 근거가 없다고 판단하였다.

Background Hair analysis (minerals/heavy metals) is an elective non-covered health technology used to analyze mineral and heavy metal levels accumulated in hair tissues to measure nutritional mineral imbalance, heavy metal contamination, and interactions between minerals in order to check one’s health status. An elective non-covered health technology refers to a health technology that a health consumer elects to use without the direct purpose of improving essential physical function. This item was identified via internal monitoring to provide correct information about hair analysis (minerals/heavy metals) to the general public, which represents health consumers, and to support the appropriate use of this health technology. During the fifth session of the Committee of Health Technology Reassessment in 2022 (May 13, 2022), the decision was made to assess the medical and scientific evidence of the clinical usefulness of hair analysis (minerals/heavy metals) through a systematic review and to provide public information on this agenda item.

Methods In this assessment, an overview of systematic reviews was performed to identify the clinical usefulness of hair analysis (minerals/heavy metals). All of the assessment methods were finalized after a review by the “hair analysis (minerals/heavy metals) reassessment subcommittee” (hereinafter referred to as “subcommittee”) with consideration for the purpose of the assessment. The subcommittee was composed of two experts each from family medicine and diagnostic laboratory medicine and one expert from evidence-based medicine. The key question in the assessment was “Is hair analysis (minerals/heavy metals) used for the purpose of checking health status clinically safe and effective?” Three international and five domestic databases were searched based on this key question, and two reviewers independently screened and selected the articles in accordance with the inclusion and exclusion criteria. Data extraction from selected articles was performed using a predetermined format. The risk of bias (RoB) in the articles was assessed using the AMSTAR-2 (A MeaSurement Tool to Assess systematic Reviews 2) tool. Data extraction and RoB assessment were performed independently by two reviewers, and for any disagreements, a consensus was reached through discussions. For data synthesis, a qualitative review was used since quantitative synthesis was not possible due to different outcome indicators reported in the selected articles. During the twelfth session of the Committee of Health Technology Reassessment in 2022 (December 9, 2022), public information was confirmed in consideration of the review opinion of the subcommittee.

Results Four systematic reviews were ultimately selected. One was published during the 2020s, one during the 2010s, and two during the 2000s. The systematic reviews were from outside of Korea – one each from Mexico, Poland, England, and Hong Kong. RoB assessment using AMSTAR-2 on articles included in the four systematic reviews showed that the overall reliability was very low (75%, n=3) or low (25%, n=1). Accordingly, the overall RoB was found to be high.

Safety The safety of hair analysis (minerals/heavy metals) was assessed based on false-positives and false-negatives, but there were no articles that reported on these indicators. The subcommittee opined that this analysis does not present any safety problems since it is a non-invasive method that causes no harm to the body.

Effectiveness The effectiveness of hair analysis (minerals/heavy metals) was assessed based on diagnostic accuracy and correlation with medical outcomes, but there were no articles that reported on these indicators. Accordingly, the subcommittee reviewed other outcome indicators in studies that performed hair analysis on healthy individuals. Lavalle-Carrasco et al. (2021) performed a qualitative review of five selected articles on hair analysis for the observation of fluoride concentration in populations exposed to fluoride. Among these articles, four found that hair is appropriate for measuring fluoride exposure level, whereas one found that hair may not be an appropriate biomarker. In the study by Mikulewicz et al. (2013), differences in analytical methodologies, especially sample preparation, decomposition, and analysis methods, caused variability in the hair mineral levels reported as reference values, indicating the need to refine the standard procedure, validate the hair mineral analysis method, and establish a detailed methodology. In the study by Lowe et al. (2009), hair zinc concentration increased significantly after taking zinc supplements, and hair zinc concentration was viewed as a reliable biomarker for healthy individuals. Ng et al. (2007) reported that the correlation coefficient of hair analysis for measuring mercury exposure in children and adolescents was 0.459 with urine, 0.640 with cord blood, and 0.6107 with blood. It was also found that a correlation between mercury in hair and blood was found, but this correlation was not high enough to replace blood or urine mercury tests in clinical practice.

Conclusions and recommendations Based on the current assessment results, the subcommittee recommended the following: Hair analysis (minerals/heavy metals) does not present any safety problems as a non-invasive method that causes no harm to the body, but there is no literary evidence demonstrating its accuracy and clinical usefulness as a test for measuring mineral and heavy metal concentrations in the body. During the twelfth session of the Committee of Health Technology Reassessment in 2022 (December 9, 2022), the subcommittee findings and public information and, in accordance with Article 4-10 of the Guidelines for Management of Health Technology Reassessment Project, the original subcommittee’s conclusions on “hair analysis (minerals/heavy metals)” were reviewed and approved. Partially revised public information was also reviewed and approved. The Committee of Health Technology Reassessment determined that hair analysis (minerals/heavy metals) does not present any safety problems since it is a non-invasive method that causes no harm to the body, but there is no literary evidence demonstrating its accuracy and clinical usefulness as a test for measuring mineral and heavy metal concentrations in the body.
