자기유래배양피부이식술

Abstract

평가 배경 자기유래배양피부이식술은 충분한 공여부의 확보가 어려운 광범위한 중화상 환자의 피부조직을 생검하여 배양한 피부를 이식하는 시술로서, 신의료기술평가제도가 확립되기 전인 2005년 비급여로 등재되었다. 2021년 제3차 의료기술재평가위원회(2021.03.12.)에서는 보건의료 자원의 효율적 사용을 지원하기 위한 근거를 제공하기 위하여, 체계적 문헌고찰을 통해 자기유래배양피부이식술의 안전성 및 유효성에 대한 의과학적 근거를 재평가하고, 동 안건에 대하여 권고등급 결정을 수행하는 것으로 심의하였다.

평가 방법 자기유래배양피부이식술의 안전성 및 유효성을 재평가하기 위하여 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 성형외과 2인, 외과 2인, 피부과 1인, 근거기반의학 1인으로 구성된 자기유래배양피부이식술 재평가 소위원회(이하 소위원회)의 심의를 거쳐 확정하였다. 체계적 문헌고찰의 핵심질문은 ‘광범위한 중화상 환자에서 자기유래배양피부이식술은 임상적으로 안전하고 유효한가?’이다. 본 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림위험 평가는 Cochrane의 Risk of Bias와 RoBANS ver 2.0을 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 자료분석은 정성적 분석을 적용하였으며, GRADE 방법을 이용하여 근거 수준을 평가하였다. 2021년 제10차 의료기술재평가위원회(2021.10.15.)에서는 소위원회의 검토 의견을 고려하여 최종 권고등급을 결정하였다.

평가 결과 자기유래배양피부이식술의 재평가에 선택된 문헌은 총 22편(무작위배정 임상시험연구 1편, 코호트연구 9편, 안전성 결과를 보고한 단일군연구 12편)이었으며, 문헌의 비뚤림위험 수준은 대체로 낮은 것으로 평가되었다.

안전성 자기유래배양피부이식술의 안전성은 소위원회의 논의를 바탕으로 시술 관련 부작용 및 이상반응을 기준으로 평가하였다. 군 간 부작용 및 이상반응의 결과를 비교한 2편의 연구 중 폐렴과 패혈증의 발생률을 보고한 1편에서는 자기유래배양피부이식술을 시행한 군에서 높은 경향을 보였고, 주요 합병증을 보고한 다른 1편에서는 자기유래배양피부이식술을 시행하지 않은 군에서 발생률이 더 높았으나 모두 통계적으로 유의하지는 않았다. 자기유래배양피부이식술군의 부작용 및 이상반응을 보고한 16편의 문헌에서 국소 감염의 발생률은 0% ~ 80%로 다양하게 보고되었으며, 이식편 완전 소실의 발생률은 1.8% ~ 16.7%로 보고되었다. 소위원회에서는 이상의 보고된 부작용 및 이상반응의 발생이 기저상태 등 환자 요인의 영향이 크며 동 시술과 직접적인 관련성을 판단하기 어렵다는 의견이었다.

유효성 자기유래배양피부이식술의 유효성은 소위원회의 논의를 바탕으로 이식편 생착률, 상처치유, 생존율 및 사망률, 흉터의 질, 입원기간을 기준으로 평가하였다. 소위원회에서는 대부분의 문헌과 실제 임상 현장에서 자기유래배양피부이식술이 부분층피부이식술과 같은 기존 치료들과 병용되고 있으므로, 병용 시술 종류와 관계없이 자기유래배양피부이식술을 시행한 군을 중재군으로, 시행하지 않은 군을 대조군으로 정의하고 통합적으로 분석하는 것이 적절하다고 판단하였다. 이식편 생착률을 보고한 1편의 문헌에서는 자기유래배양피부이식술을 시행한 군과 시행하지 않은 군 간 유의한 차이가 없는 것으로 보고되었다. 상처치유를 보고한 2편의 문헌 모두에서 자기유래배양피부이식술 시행군에서의 상처치유 지표가 미시행군에 비해 유의하게 향상되었다. 생존율 및 사망률을 보고한 4편의 문헌 중 생존분석을 통한 사망 위험비를 보고한 2편의 연구 모두에서 자기유래배양피부이식술 시행군에서의 사망 위험비가 미시행군보다 유의하게 낮았다. 생존 및 사망의 비율을 보고한 3편의 연구에서도 모두 자기유래배양피부이식술을 시행한 환자에서 낮은 사망률을 보고하였으나 1편에서만 통계적으로 유의한 차이를 보였다. 흉터의 질을 보고한 3편의 연구에서 모두 측정 도구가 달랐지만, 전체적으로 자기유래배양피부이식술 시행군에서 미시행군에 비해 유의하게 흉터의 질이 좋은 것으로 나타났다. 입원기간을 보고한 6편의 연구에서 모두 자기유래배양피부이식술 시행군의 입원기간이 미시행군보다 길었으며 2편에서 그 차이가 유의하였다. 소위원회에서는 입원기간이 길다는 것은 높은 생존 확률을 의미하며, 자기유래배양피부이식술 시행군에서 추가 배양 시간의 영향이 있겠지만 입원기간은 시술 자체의 영향보다 환자 요인이 많이 작용한다는 의견이었다.

결론 및 제언 자기유래배양피부이식술 재평가 소위원회는 현재 평가 결과에 근거하여 다음과 같이 제언하였다. 소위원회는 자기유래배양피부이식술은 통상적인 화상치료에서 발생하는 감염의 위험이 있으나 그 외에 중대한 시술 관련 부작용 및 이상반응은 보고되지 않고, 기존 시술 방법과 비교할 때 안전성에는 문제가 없는 것으로 판단하였으며, 자기유래배양피부이식술과 부분층피부이식술을 병용할 때 부분층피부이식술 단독군보다 상처치유와 흉터의 질에 있어 통계적으로 유의하게 향상된 결과를 보고하고 있어 효과적이라고 평가하였다. 결론적으로, 자기유래배양피부이식술은 충분한 공여부의 확보가 어려운 광범위한 중화상 환자에서 기존 부분층피부이식술과 병용 시에 안전하고 효과적인 기술로 평가하였다. 2021년 제10차 의료기술재평가위원회(2021.10.15.)에서는 소위원회 검토 결과에 근거하여 “자기유래배양피부이식술”에 대해 다음과 같이 심의하였다. 자기유래배양피부이식술은 충분한 공여부의 확보가 어려운 광범위한 중화상 환자에서 기존 부분층피부이식술과 병용 시에 안전하고 효과적인 기술로 평가하였다. 이에 의료기술재평가위원회는 충분한 공여부의 확보가 어려운 광범위한 중화상 환자를 대상으로 기존 부분층피부이식술과 병용 시에 자기유래배양피부이식술을 ‘권고함’으로 심의하였다.

Background Cultured epidermal autograft (CEA) is a transplantation technique for major burn patients through skin harvest from biopsy and culture, when an adequate level of donor site cannot be obtained. The technique was listed as part of Non-payment in 2005, which was before the New Health Technology Assessment was established. The 3rd HTR committee of 2021 (Mar 12, 2021) concluded that the medical science evidence for the safety and effectiveness of CEA should be reassessed through systematic review to lead to the decision on the grade of recommendation of the technique to provide the supporting evidence for more efficient use of public health care resources.

Methods To assess the safety and effectiveness of CEA, a systematic review was performed. All methods were discussed and confirmed in consideration of the purpose of the assessment by the CEA Reassessment Sub-committee (hereinafter referred to as the subcommittee) consisting of two plastic surgeons, two surgeons, one dermatologist, and one specialist of evidence-based medicine. The key question of the systematic review was “Is CEA clinically safe and effective for major burn patients?” For the systematic review based on the key question, articles were searched in three overseas and five domestic databases, and two reviewers independently screened and selected the articles according to the article inclusion and exclusion criteria. The risk of bias was evaluated using the RoB and RoBANS version 2.0 of Cochrane by the two independent reviewers, followed by a discussion to reach an agreement. For data extraction, a preset format was used by the two independent reviewers, followed by a discussion with a third party to reach an agreement. For data analysis, qualitative analysis was performed, and the GRADE method was used to determine the level of evidence. The 10th HTR committee of 2021 (Oct 15, 2021) finalized the grade of recommendation based on the subcommittee report.

Results For the reassessment of CEA, twenty-two articles were selected (one randomized clinical study, nine cohort studies, and twelve single-arm studies reporting on safety) and the risk of bias was generally low.

Safety The safety of CEA was assessed through subcommittee discussions based on procedure-related side effects and adverse reactions. Among the two studies reporting on the comparison of side effects and adverse reactions between the CEA and the non-CEA groups, one study reported on the incidence of pneumonia and septicemia, where the trend was high for the CEA group. The other study reported on main complications, where the incidence was higher for the CEA group, although without significant variation. In sixteen studies reporting on side effects and adverse reactions in the CEA group only, the incidence of local infection varied in the range of 0–80%, while the incidence of complete loss of graft was 1.8–16.7%. The subcommittee stated that it is difficult to define a direct association between CEA and the reported side effects and adverse reactions that are highly likely to have arisen from a patient factor such as the basal state.

Effectiveness The effectiveness of CEA was assessed through subcommittee discussions based on engraftment, wound healing, survival rate and mortality, scar quality, and length of stay. The subcommittee deemed it suitable to define the CEA group as the intervention group irrespective of the type of other procedures in the combined treatment and the non-CEA group as the control group, as CEA was being performed with the conventional methods such as split-thickness skin graft in most studies and clinical practice. In one study reporting on the rate of engraftment, no significant difference was found between the CEA and non-CEA groups. In two studies reporting on wound healing, the indicators consistently showed significant improvements in the CEA group compared to the non-CEA group. Four studies reported on survival and mortality, among which two studies reported on the risk of mortality based on survival analysis to show a significantly lower risk in the CEA group, while three studies reported on the ratio between survival and death to show lower mortality in the CEA group with one study showing significant difference. Three studies reported on scar quality using different measures, but overall, the scar quality was significantly higher in the CEA group than in the non-CEA group. All six studies reporting on the length of stay showed that the length was longer in the CEA group than in the non-CEA group with two studies showing significant difference. The subcommittee stated that the rate of survival is higher for a longer length of stay, and despite the potential effect of the additional culture time in the CEA group, the length of stay was influenced more by patient factors than the procedure itself.

Conclusions and Recommendations Thus, based on the current evidence, the subcommittee suggested as follows: For CEA, despite the risk of infection following conventional burn treatments, no serious procedure-related side effects or adverse reactions have been reported, and the level of safety was determined acceptable when compared to conventional procedures. The combined treatment of CEA and a split-thickness skin graft was reported with significantly improved results of wound healing and scar quality, indicating the effectiveness of CEA. In sum, CEA is a safe and effective technique as part of the combined treatment with split-thickness skin graft for major burn patients when an adequate level of donor site cannot be obtained. Based on the subcommittee report, the conclusion of the 10th HTR committee of 2021 (Oct 15, 2021) was as follows: CEA is a safe and effective technique as part of the combined treatment with split-thickness skin graft for major burn patients when an adequate level of donor site cannot be obtained. Thus, the HTR committee concludes that CEA is “recommended” for major burn patients as part of the combined treatment with split-thickness skin graft when an adequate level of donor site cannot be obtained.
