Abstract

평가 배경 체외자기장 치료(Extracorporeal Magnetic Innervation)는 요실금 및 만성골반통증증후군 환자의 골반근을 지배하는 신경을 자기장에 의한 주기적인 전기자극으로 치료하는 기술이다. 동 기술은 보건복지부 급여화 추진 계획 분야 중 비뇨기계 관련 의료기술로서 내부모니터링을 통해 발굴된 주제로, 우선순위 심의를 통해 재평가대상으로 선정되었다. 2021년 제3차 의료기술재평가위원회(2021.3.12.)에서는 동 기술의 평가계획서 및 소위원회 구성(안)을 심의하였으며, 제12차 의료기술재평가위원회(2021.12.10.)에서 최종심의하였다.

평가 방법 요실금 및 만성골반통증증후군 환자에서 체외자기장 치료에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “체외자기장 치료 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회 구성은 비뇨의학과 2인, 산부인과 2인, 가정의학과 1인, 근거기반의학 1인의 전문가 6인으로 구성하였다. 평가의 핵심질문은 “요실금 환자 및 만성골반통증증후군 환자에서 체외자기장 치료는 임상적으로 안전하고 효과적인가?”이었다. 안전성은 체외자기장 치료와 관련하여 보고된 이상반응을 결과지표로 하였고, 효과성은 요실금 환자에서는 대상으로는 요실금 증상개선, 삶의 질을 결과지표로 평가하였고, 만성골반통증증후군 환자에서는 하부요로 증상개선, 삶의 질을 결과지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌선택 및 배제기준에 따라 2명의 검토자가 독립적으로 수행하고, 의견의 불일치가 있는 경우에는 검토자간 합의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림위험 평가는 연구유형에 따라 무작위배정임상시험연구는 Cochrane의 Risk of Bias, 그 외에 비무작위 연구는 Risk of Bias Assessment for Non-randomized studies (RoBANS) 2.0 국문판을 사용하여 평가하였으며, 최종 선택된 문헌을 대상으로 2명의 평가자가 독립적으로 평가를 실시하였으며, 의견이 불일치한 경우 평가자간 합의를 통해 일치된 결과를 도출하였다. 모든 자료는 연구단위로 추출하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 방법을 이용하여 근거 수준을 평가하고, 평가 결과를 토대로 권고등급을 결정하였다. 임상적 안전성 및 효과성에 대한 결론은 합의를 통해서 결정하였다.

평가 결과 평가에 선택된 문헌은 총 17편(국내 2편, 국외 15편)으로 무작위배정임상시험은15편, 비무작위 연구 2편이 포함되었다. 비교시술별로는 무치료군과 비교 3편, 모의대조군과 비교 6편, 골반저근운동군과 비교 6편, 바이오피드백과 비교된 연구 2편, 약물치료와 비교된 연구 2편이었다. 선택문헌 총 17편 중 4편의 문헌에서 체외자기장 치료법과 기존 치료법을 병행하여 수행한 군(이하 ‘병용 중재군’)과 기존 치료법을 단독으로 수행한 군(이하 ‘비교군’)을 비교한 연구가 확인되어, 체외자기장 치료법을 단독으로 수행한 경우와 기존 치료법과 병행해서 수행한 경우를 구분하여 분석하였다.

안전성 선택문헌 17편 중 요실금 환자를 대상으로 한 1편에서 체외자기장 치료군(중재군)에서 설사, 변비, 근육통 등이 1~5%, 모의대조군(비교군)에서 설사, 고창(flatulence), 변비가 2%로 보고되었다. 그 외 문헌에서는 체외자기장 치료 관련 이상반응이 없었다고 보고하거나 안전성에 대한 언급이 없었다.

효과성 체외자기장 치료의 효과성은 대상 환자군 및 비교시술별로 구분하여 제시하였다. 대상환자군 별로, 여성 요실금 환자 문헌 11편, 남성 요실금 환자 문헌 3편, 남성과 여성이 혼재되어 있는 문헌은 1편, 만성골반통증증후군 환자 문헌 2편이 선택되었다. 여성 요실금 환자 문헌에서의 연구결과는 다음과 같다. 체외자기장 치료와 무치료군을 비교한 2편의 연구에서 치료기간 4주 후 요실금 증상개선은 체외자기장 치료군은 시술 전·후 유의한 개선을 보였으나, 무치료군에서는 시술 전·후 유의한 차이를 보이지 않았다. 삶의 질을 보고한 2편에서 체외자기장 치료군에서만 일부 삶의 질 지표에서 개선 효과를 확인할 수 있었다. 모의대조군과 비교한 연구는 5편이었다. 5편 중 1편에서 모의대조군보다 체외자기장 치료군에서 더 유의하게 요실금 횟수를 감소한 것으로 보고되었으나 이외 4편에서는 두 군간 유의한 차이를 보이지 않았다. 이외 요실금 양 및 과민성 방광개선 점수도 보고가 되었으나, 군간 유의한 차이가 없거나 통계적 유의성이 보고되지 않았다. 삶의 질 영향정도는 5편에서 다양한 도구 기준 결과가 보고되었으며, 이 중 1편의 문헌에서 체외자기장 치료군의 International Prostate Symptom Score (IPSS) 점수가 모의대조군에 비해 유의하게 감소한 것으로 나타났다. 골반저근운동군과 비교한 2편의 연구에서 요실금 증상개선 정도가 두 군에서 모두 시술 전·후 유의한 감소를 보였으나 군간 변화량에 대한 비교결과는 제시하지 않았다. 삶의 질 영향 정도를 보고한 2편중 1편은 중재군에서 일부 지표기준 유의한 개선을 보였으나 군간 변화량은 유의한 차이를 보이지 않거나 비교결과를 제시하지 않았다. 나머지 1편에서는 두 군 모두 시술 전·후 유의하게 Incontinence Quality of life (IQoL) 점수가 감소하였으나 군간 변화량 차이에 대한 통계적 유의성은 제시하지 않았다. 바이오피드백군과 비교한 1편의 연구에서 삶의 질 영향 정도는 치료(8주)가 끝난 후 IQoL 점수가 시술 전에 비해 유의하게 감소하였으나 군간 변화량 차이에 대한 통계적 유의성은 제시하지 않았다. 약물치료군과 비교한 1편의 연구에서 체외자기장 치료군 18명 중 9명에서 요실금 증상이 호전되었으며, 약물치료군 19명 중 10명이 호전되었다고 보고하였다. 하지만 군간 차이에 대한 통계적 유의성은 보고하지 않았다. 삶의 질은 두 군 모두 시술 전·후 유의하게 호전되었으나 군간 변화량 차이에 대한 통계적 유의성은 제시하지 않았다. 남성 요실금 환자 문헌에서의 연구결과는 다음과 같다. 무치료군과 비교한 1편에서 시술 1~3개월 후 체외자기장 치료군에서 요실금 증상점수가 가장 많이 감소되고 무치료군에서는 요실금 증상개선에 있어 가장 안 좋은 점수를 얻었다고 보고하였다. 하지만 두 군간 변화량 차이에 대한 통계적 유의성은 제시하고 있지 않았다. 골반저근운동군과 비교한 총 2편에서 비교군에 비해 체외자기장 치료군에서 요실금 증상개선을 보였다. 1편에서는 시술 1~3개월 후 시점에서 골반저근운동군에 비해 체외자기장 치료군에서의 요실금 증상이 더 많이 개선된 것으로 보고되었으나, 두 군간 변화량 차이에 대한 통계적 유의성은 제시하지 않았다. 다른 1편에서는 시술 2개월 시점에서 중재군에서의 요실금 증상이 비교군에 비해 더 개선되었으나 그 이후의 추적관찰시점에서는 유의한 차이를 보이지 않았다. 치료(8주) 종료 후 1일 시점에서 중재군과 비교군 모두에서 삶의 질 개선이 확인되었으나 중재군에서의 삶의 질이 더 좋았다. 만성골반통증증후군 환자 문헌에서의 연구결과는 다음과 같다. 무치료군과 비교한 1편에서 하부요로 증상은 중재군, 비교군 모두 1년 시점에서 모두 유의한 개선을 나타냈으나 군간 변화량 차이에 대한 통계적 유의성은 제시하지 않았다. 체외자기장 치료법 단독수행 이외 기존 치료법과 병행하여 수행한 연구는 4편(여성 요실금 환자 문헌 2편, 남성 요실금 환자 문헌 1편, 만성골반통증증후군 문헌 1편)이었다. 여성 요실금 환자 문헌에서의 연구결과는 다음과 같다. 체외자기장 치료법과 바이오피드백을 병행한 군과 바이오피드백만 수행한 군을 비교한 1편에서 병용중재군에서 요실금 증상은 시술 전·후 유의한 개선을 보였으나, 두 군간 유의한 차이가 없었다. 치료(8주)가 끝난 후 병용중재군에서의 삶의 질이 비교군에 비해 좋은 점수를 나타냈으나 두 군간 유의한 차이가 없었다. 체외자기장 치료법과 약물치료를 병행한 군과 약물치료만 수행한 군을 비교한 1편에서 병용중재군에서 요실금 증상은 시술 전·후 유의한 개선을 보였으나, 두 군간 차이에 대한 통계적 유의성은 보고하지 않았다. 또한 병용중재군 23명 중 9명에서 요실금 증상이 호전되었으며, 비교군 19명 중 10명이 호전되었다고 보고하였다. 삶의 질은 두 군 모두 시술 전·후 유의하게 개선되었으나 군간 변화량 차이에 대한 통계적 유의성은 제시하지 않았다. 남성 요실금 환자 문헌에서의 연구결과는 다음과 같다. 체외자기장 치료법과 골반저근 운동과 병행한 군과 골반저근운동만 수행한 군을 비교한 1편에서 시술 2~3개월 후 시점에서 비교군에 비해 병용중재군에서의 요실금 증상 및 삶의 질이 유의하게 개선된 것으로 보고되었으나 6개월 시점에서는 그 차이가 유의하지 않았다. 만성골반통증증후군 환자 문헌에서의 연구결과는 다음과 같다. 체외자기장 치료법과 약물치료를 병행한 군과 약물치료만 수행한 군을 비교한 1편에서 하부요로 증상은 치료기간 6주 후 병용중재군에서는 시술 전에 비해 유의하게 개선되었으나 약물치료를 단독군은 유의한 변화를 보이지 않았으나, 군간 변화량은 유의한 차이를 보이지 않았다. 삶의 질은 두 군 모두 시술 전후 유의한 개선을 보였으나 군간 변화량은 유의한 차이를 보이지 않았다. 근거수준은 비뚤림위험 평가 및 이질성 등을 고려하여 평가하였다. 종합해서, 요실금 환자를 대상으로는 매우 낮음(Very Low)-보통(Moderate)으로 확인되었고, 만성골반통증증후군 환자를 대상으로는 매우 낮음(Very Low)으로 확인되었다. 이에 소위원회에서는 요실금 환자 및 만성골반통증증후군 환자를 대상으로 동 시술을 수행시 일부 연구에서 시술 전후 증상개선이 확인되어 임상적 유용성이 기대되나 대부분 연구에서 비교시술 대비 체외자기장 치료군에서 유의한 효과가 확인되지 않았으며, 요실금 환자의 경우 세부질환별로 확인하였을 때도 결과는 다르지 않다는 의견이었다.

결론 및 제언 체외자기장 치료 소위원회는 현재 평가 결과에 근거하여 다음과 같이 제언하였다. 체외자기장 치료는 i) 요실금 환자를 대상으로 요실금 개선을 목적을 수행시 안전성은 문제가 없으나, 효과성은 입증할 만한 근거가 충분하지 않다고 판단하였으며, ii) 만성골반통증증후군 환자를 대상으로 하부요로 증상개선을 목적으로 수행 시 안전성은 문제가 없으나, 효과성을 입증할 만한 근거가 충분하지 않다고 판단하였다. 2021년 제12차 의료기술재평가위원회(2021.12.10.)에서는 소위원회 검토 결과를 바탕으로 논의하여 다음과 같이 심의하였다. 요실금 환자에서 요실금 증상개선을 목적으로 수행하거나 만성골반통증증후군 환자에서 하부요로 증상개선을 목적으로 체외자기장 치료를 수행하는 것은 안전성에는 문제가 없으나 효과성을 입증할 만한 근거가 충분하지 않아 요실금 환자와 만성골반통증증후군 환자에서 체외자기장 치료를 ‘권고하지 않음’으로 심의하였다(권고등급: 권고하지 않음).

Extracorporeal Magnetic Innervation

Background Extracorporeal magnetic innervation (ExMI) is a technology used to treat patients with urinary incontinence (UI) or chronic pelvic pain syndrome (CPPS) by applying periodic electrical stimulation generated by a magnetic field on the nerves that innervate the pelvic muscles. This medical technology related to the urinary system within the comprehensive plan of national health insurance by the Ministry of Health and Welfare is a topic identified through internal monitoring and has been selected for reassessment through the priority review. In 2021, the third session of the Committee of Health Technology Reassessment (March 12, 2021) reviewed the assessment plan for this technology and the proposed composition of a subcommittee. The final review was conducted by the twelfth session of the Committee of Health Technology Reassessment (December 10, 2021).

Methods A systematic literature review was conducted to assess the safety and effectiveness of ExMI in patients with UI or CPPS. All assessment methods were finalized after a review by the “ExMI subcommittee” (hereinafter “Subcommittee”). The Subcommittee was composed of six experts – two from urology, two from obstetrics and gynecology, one from family medicine, and one from evidence-based medicine. The key question in the assessment was “Is ExMI clinically safe and effective for patients with UI or CPPS?” For safety, reported ExMI-related adverse events (AEs) were used as the outcome indicator. For effectiveness, improvement in UI symptoms and quality of life (QoL) were assessed as outcome indicators for patients with UI, and improvement in lower urinary tract symptoms and QoL were assessed as outcome indicators for patients with CPPS. For the systematic literature review, three international and five domestic databases were searched based on the key question. Two reviewers independently selected the articles in accordance with the inclusion and exclusion criteria, and in the event of any disagreement, the reviewers reached a consensus to select the final articles. For the risk of bias (RoB) assessment of articles selected, Cochrane’s Risk of Bias Tool was used for randomized controlled trials (RCTs), and the Korean version of Risk of Bias Assessment for Non-randomized studies (RoBANS) 2.0 was used for non-randomized studies (NRSs). Two assessors independently assessed the selected articles, and in the event of any disagreement, the reviewers reached a consensus to derive matching results. All data were extracted by unit of study. The systematic literature review results were assessed for level of evidence using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) method, while the grade of recommendation was determined based on the assessment results. A conclusion on clinical safety and effectiveness was derived by reaching a consensus.

Assessment results A total of 17 articles (two Korean and 15 overseas articles) were selected for the assessment, including 15 RCTs and two NRSs. Comparators used included no treatment (n = 3), simulated control (n = 6), pelvic floor muscle exercise (n = 6), biofeedback (n = 2), and drug therapy (n = 2). Of the 17 articles selected, four articles contained studies that compared combined therapy with ExMI and conventional therapy (“co-intervention group”) to conventional therapy alone (“comparator group”). Accordingly, ExMI alone and ExMI combined with conventional therapy were analyzed separately.

Safety Of the 17 articles selected, one study on patients with UI reported diarrhea, constipation, and muscle pain in 1-5% of patients in the ExMI group (intervention group) and diarrhea, flatulence, and constipation in 2% of patients in the simulated control (comparator group). All other articles reported no ExMI-related AEs or did not mention safety.

Effectiveness The effectiveness of ExMI was presented separately by study population and comparator intervention. The study population included 11 articles on female patients with UI, three articles on male patients with UI, one article on both males and females, and two articles on patients with CPPS. The study results in articles on female patients with UI were as follows: In two articles that reported on a comparison between the ExMI and no-treatment groups, the ExMI group showed a significant improvement in UI symptoms after 4 weeks of treatment, whereas the no-treatment group showed no significant difference between before and after the intervention. In two articles that reported on QoL, improvement in some QoL indicators was found only in the ExMI group. Five studies compared ExMI to a simulated control. Of these, only one article reported a significant reduction in the frequency of UI in the ExMI group, as compared to the simulated control. In the remaining four articles, there was no significant difference between the two groups. In addition, the amount of UI and scores for improvement of overactive bladders were reported, but the results showed no significant differences between the groups, or statistical significance was not reported. The degree of impact on QoL measured using various tools was reported in five articles. Of these, one article reported that the ExMI group showed a significant decrease in the International Prostate Symptom Score (IPSS), as compared to the simulated control. In two studies that compared ExMI to pelvic floor muscle exercise, both groups showed a significant decrease in UI symptoms before and after the intervention, but the results of comparison between the two groups concerning the amount of change were not presented. Of the two articles that reported on the degree of impact on QoL, one article reported that significant improvement in some indicators was found in the intervention group but did not report on any significant difference in the amount of change between the groups or comparison results. The other article reported that both groups showed a significant decrease in the incontinence QoL (IQoL) score before and after the intervention but did not report any statistical significance regarding the difference in the amount of change between the groups. In one study that compared ExMI to biofeedback, the results for the degree of impact on QoL showed a significant decrease in the IQoL score at the end of the intervention (8 weeks), as compared to before the intervention. However, the statistical significance regarding the difference in the amount of change between the groups was not presented. In another study that compared ExMI to drug therapy, nine out of 18 patients in the ExMI group showed improvement in UI symptoms, while 10 out of 19 patients in the drug therapy group showed improvement. However, statistical significance regarding the difference in the amount of change between the groups was not presented. The study results in articles on male patients with UI were as follows: In one study that compared ExMI to the no-treatment group, the ExMI group showed the largest decrease in the UI symptom scores at 1-3 months after the intervention, while the no-treatment group showed the worst score for improvement in UI symptoms. However, statistical significance regarding the difference in the amount of change between the groups was not presented. In two studies that compared ExMI to pelvic floor muscle exercise, the ExMI group showed greater improvement in UI symptoms, as compared to the pelvic floor muscle exercise group. One article reported greater improvement in UI symptoms at 1-3 months after the intervention, as compared to the pelvic floor muscle exercise group. However, statistical significance regarding the difference in the amount of change between the groups was not presented. The other article reported that the intervention group showed greater improvement in UI symptoms than the control group at 2 months after intervention, but no significant differences were found in subsequent follow-up visits. Improvement in QoL was found in both the intervention and control groups 1 day after the completion of the intervention (8 weeks), but the improvement in QoL was greater in the intervention group. The study results in articles on patients with CPPS were as follows: In one study that compared ExMI to the no-treatment group, both the intervention and control groups showed significant improvement in lower urinary tract symptoms after one year. However, statistical significance regarding the difference in the amount of change between the groups was not presented. Four studies used combined therapy with ExMI and conventional therapy (two on female patients with UI, one on male patients with UI, and one on patients with CPPS). The study results in articles on female patients with UI were as follows: In one study that compared combined therapy (ExMI + biofeedback) and biofeedback alone, the combined therapy group showed a significant improvement in UI symptoms between before and after the intervention. However, a significant difference between the two groups was not found. Upon completion of the treatment (8 weeks), the combined therapy group showed a better QoL score than the control group, but there was no significant difference between the two groups. In a study that compared combined therapy (ExMI + drug therapy) and drug therapy alone, the combined therapy group showed a significant improvement in UI symptoms before and after the intervention. However, the statistical significance of the difference between the groups was not reported. Moreover, 9 out of 23 patients in the combined therapy group showed improvement in UI symptoms, while 10 out of 19 patients in the control group showed improvement. While both groups showed significant improvement in QoL before and after the intervention, statistical significance regarding the difference in the amount of change between the groups was not presented. The study results in articles on male patients with UI were as follows: In one study that compared combined therapy (ExMI + pelvic floor muscle exercise) and pelvic floor muscle exercise alone, the combined therapy group showed a significant improvement in UI symptoms and QoL at 2-3 months after the intervention, as compared to the control group. However, the difference was not significant 6 months after the intervention. The study results in articles on patients with CPPS were as follows: In one study that compared combined therapy (ExMI + drug therapy) and drug therapy alone, the intervention group showed significant improvement in lower urinary tract symptoms at 6 weeks after the treatment period, whereas the drug therapy group showed no significant change. However, there was no significant difference in the amount of change between the two groups. While both groups showed significant improvement in QoL before and after the intervention, there was no significant difference in the amount of change between the two groups. The level of evidence was assessed by considering RoB assessment and heterogeneity. In summary, the level of evidence was found to be “very low” to “moderate” for patients with UI and “very low” for patients with CPPS. Accordingly, the Subcommittee has determined that the technology being assessed was expected to show clinical effectiveness based on some studies reporting improvement of symptoms between before and after the intervention in patients with UI or CPPS. However, most studies did not demonstrate significant effects of ExMI relative to the comparators used and it is determined that the results were no different when checked by specific diseases in patients with UI.

Conclusions and recommendations The Subcommittee recommends the following based on the current assessment results: ExMI i) has no safety concerns when performed on patients with UI for improvement of UI symptoms, but evidence to demonstrate its effectiveness is still insufficient, and ii) has no safety concerns when performed on patients with CPPS for improvement of lower urinary tract symptoms, but evidence to demonstrate its effectiveness is still insufficient. During the 2021 twelfth session of the Committee of Health Technology Reassessment (December 10, 2021), the Subcommittee findings were discussed, and the following determination was made. Currently, ExMI has no safety concerns when performed for the improvement of UI symptoms in patients with UI and improvement of lower urinary tract symptoms in patients with CPPS. However, due to insufficient evidence to demonstrate its effectiveness, ExMI is “not recommended” for patients with UI or CPPS (Grade of recommendation: Not recommended).
