원형자동문합기를 이용한 경항문 직장절제술 [원형자동문합기 포함]

Abstract

평가 배경 원형자동문합기를 이용한 경항문 직장절제술(Stapled Trans-anal Rectal Resection, STARR)은 직장중첩증 및 직장류 등의 이상소견을 보이는 직장의 전층을 원형자동문합기를 이용하여 절제하는 의료기술로, 신의료기술평가제도 확립 이전인 2008년에 행위비급여로 등재되어 현재까지 사용되고 있다. 해당 의료기술은 내부 모니터링을 통해 발굴된 주제로, 대상선별 임상자문회의 및 우선순위심의에서 재평가 적합성과 필요성을 인정받아 2021년 제3차 의료기술재평가위원회(2021.3.12.)에서 의료기술재평가 대상으로 선정되었다.

평가 방법 해당 의료기술에 대한 안전성 및 유효성을 평가하기 위해 체계적 문헌고찰을 수행하였다. 평가의 핵심질문은 “폐쇄성 배변장애 증후군 환자에서 원형자동문합기를 이용한 경항문 직장절제술은 임상적으로 안전하고 유효한가?”였으며, 모든 평가범위는 소위원회의 심의를 거쳐 확정하였다. 안전성은 해당 수술을 통한 부작용 및 합병증으로 확인하고, 유효성은 다른 수술적 치료와 비교한 비교연구로 제한하여 배변기능, 삶의 질, 수술정보를 통해 확인하고자 하였다. 문헌검색은 국외 3개, 국내 5개 데이터베이스에서 수행하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 비뚤림 위험 평가는 연구유형에 따라 Cochrane의 RoB와 ROBANS ver 2.0을 사용하여 두 명의 검토자가 독립적으로 수행하고 의견 합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 본 평가는 소위원회의 검토의견을 고려하여 의료기술재평가위원회에서 최종심의 후 권고등급을 제시하였다.

평가 결과 체계적 문헌고찰을 수행한 결과, 핵심질문에 적합한 총 34편(비교연구 9편, 단일군 연구 25편)의 문헌이 확인되었다. 안전성 및 유효성 결과는 다음과 같다.

안전성 9편의 비교연구(무작위배정 비교임상시험 5편, 비무작위 비교연구 4편) 및 25편의 단일군 연구에서 해당 의료기술의 임상적 안전성 결과를 확인할 수 있었다. 원형자동문합기를 이용한 경항문 직장절제술과 다른 수술적 치료를 비교한 4편의 비교연구에서는 개별 연구 및 메타분석 결과, 합병증 발생률(overall)에 있어 두 군간 유의한 차이가 없는 것으로 나타났다. 5편의 비교연구에서는 두 군 모두에서 사망, 패혈증, 누공 또는 괴사와 같은 중대한 이상반응은 발생하지 않은 것으로 나타났다. 8편의 비교연구에서는 개별 연구 및 메타분석 결과, 시술 후 배변절박, 변실금 및 장내가스, 이급후중 등과 같은 소화기계 및 비뇨기계 증상 발생에 있어 두 군간 유의한 차이가 없는 것으로 나타났다. 7편의 비교연구에서는 개별 연구 및 메타분석 결과, 시술 후 출혈, 통증 및 그 외 합병증 발생에 있어 두 군간 유의한 차이가 없는 것으로 나타났다. 비교연구에서 보고되는 안전성 결과가 제한적일 것으로 판단하여, 단일군 연구에서 확인한 주요 합병증 및 중대한 이상반응은 25편의 문헌에서 0~4.4% 범위내에서 보고되고 있었다. 주요 합병증을 보고하고 있는 11편의 문헌 중, 대부분(9편)에서는 주요 합병증이 전혀 발생하지 않은 것으로 보고되었으나 나머지에서 0~3.8% 보고되었다. 사망은 13편의 문헌 중 한 명도 발생하지 않은 것으로 확인되었으며, 패혈증은 12편의 문헌에서 0~4.4%, 누공은 8편의 문헌에서 0~1.3%, 괴사는 1편의 문헌에서 1명 발생한 것으로 보고되었다.

유효성 해당 의료기술의 임상적 효과성을 보고하고 있는 9편의 비교연구를 확인할 수 있었다. 원형자동문합기를 이용한 경항문 직장절제술과 다른 수술적 치료를 비교한 9편의 비교연구 중 일부 개별 연구에서는 해당 의료기술이 수술시간 및 재원기간에 있어 유의하게 좋은 경향성을 보였으나, 메타분석 결과 두 군간 유의한 차이가 없는 것으로 나타났다. 8편의 비교연구에서는 메타분석 결과, 시술 후 배변 기능 점수, 재발, 환자 만족도에 있어 두 군간 유의한 차이가 없는 것으로 나타났다. 1편의 비무작위 비교연구에서는 삶의 질 결과를 보고하고 있었으며, PAC-QoL의 신체적(physical) 영역에서 해당 의료기술이 유의하게 더 좋았으나, 나머지 영역(worries, psychological, dissatisfaction)에서는 두 군간 유의한 차이가 없는 것으로 나타났다.

결론 및 제언 해당 소위원회는 현재 평가결과에 근거하여 다음과 같이 제언하였다. 원형자동문합기를 이용한 경항문 직장절제술은 다른 수술적 치료와 비교하여 유사한 수준의 합병증 발생률을 가지고 있는 안전한 의료기술이며, 다른 수술적 치료보다 수술시간, 재원기간, 배변기능, 환자 만족도에 있어 좀 더 유리한 경향성을 보였으나 통계적으로 유의한 차이는 없어 다른 수술적 치료와 유사한 효과성을 가지고 있는 의료기술로 판단하였다. 2021년 제10차 의료기술재평가위원회(2021.10.15.)에서는 소위원회 검토 결과에 근거하여 “원형자동문합기를 이용한 경항문 직장절제술”에 대해 다음과 같이 심의하였다. 원형자동문합기를 이용한 경항문 직장절제술은 다른 수술적 치료와 비교하여 유사한 안전성 및 효과성을 가지고 있는 의료기술로 나타났으나, 다양한 치료대안이 존재하고 이들과 비교하여 우월한 효과를 확인하기는 어려워 적절한 환자를 대상으로 수행할 수 있는 치료대안 중 하나로 판단하였다. 이에 따라 의료기술재평가위원회는 폐쇄성 배변장애 증후군 환자를 치료하는 의료기술로 원형자동문합기를 이용한 경항문 직장절제술을 ‘조건부 권고함’으로 심의하였다.

Background Stapled trans-anal rectal resection (STARR) is a health technology in which a circular stapler is used for the resection of the whole-mount section of the rectum with an abnormality such as rectal intussusception or rectocele. The technique was listed as part of Non-payment for Performance in 2008, which was before the New Health Technology Assessment was established. The theme was detected via internal monitoring and as the need and suitability of reassessment were approved by the clinical assessment council and priority deliberation, the technique was adopted for HTR by the 3rd HTR committee of 2021 (Mar 12, 2021).

Methods To assess the safety and effectiveness of STARR, a systematic review was performed. The key question of the assessment was “Is STARR clinically safe and effective for use in patients with obstructed defecation syndrome (ODS)?” The assessment scope was confirmed through subcommittee discussions. The safety was assessed based on the surgery-related side effects and complications. The effectiveness was assessed based on defecation, quality of life, and surgery-related data, while limiting the review to comparative studies.

For the review, articles were obtained from a search of three overseas and five domestic databases. Two reviewers independently screened and selected the articles according to the article inclusion and exclusion criteria. The risk of bias was evaluated using the RoB and RoBANS version 2.0 of Cochrane depending on the study type by the two independent reviewers, followed by a discussion to reach an agreement. For data extraction, the preset format was used by the two independent reviewers, followed by a discussion with a third party to reach an agreement. The HTR committee conducted the final deliberation based on the subcommittee report and suggested the grade of recommendation.

Results Thirty-four articles addressing the key question (nine controlled studies and twenty-five uncontrolled studies) were found as a result of systematic review; the assessed safety and effectiveness are presented below.

Safety The clinical safety of STARR was reported in nine controlled studies (five randomized controlled and four non-randomized controlled clinical studies) and twenty-five uncontrolled studies.

Four controlled studies comparing STARR with another surgical treatment reported no significant between-group variation in the overall incidence of complications based on the results of individual studies and meta-analysis. Five controlled studies reported no serious adverse reactions such as death, septicemia, fistula, or necrosis for both groups. Eight controlled studies reported no significant between-group variation in post-operative symptoms of the digestive or urinary system such as urgency, fecal soiling, intestinal gas or tenesmus based on the results of individual studies and meta-analysis. Seven controlled studies reported no significant between-group variation in postoperative hemorrhage, pain or other complications based on the results of individual studies and meta-analysis.

Safety data reported in the controlled studies were presumed to be limited, and the incidence of main complications and serious adverse reactions reported in twenty-five uncontrolled studies ranged between 0% and 4.4%. Most (nine) of the eleven studies reporting on the main complications showed the complete lack of main complications, while the remaining two studies reported an incidence between 0% and 3.8%. The incidence of death was 0% in thirteen studies, septicemia was 0–4% in twelve studies, fistula was 0–1.3% in eight studies, and necrosis was a single case in one study.

Effectiveness The clinical effectiveness of STARR was reported in nine controlled studies, comparing STARR with another surgical treatment.

While a number of independent studies among these reported a significant trend of more favorable results in terms of operation time and length of stay, the meta-analysis found no significant between-group variation. The meta-analysis of eight controlled studies found no significant between-group variation for postoperative defecation, recurrence, and satisfaction by the patient. In one non-randomized uncontrolled study, the quality of life was reported. The STARR scored higher in the physical category of the PAC-QoL but no significant between-group variation was shown for the other categories (worries, psychological, and dissatisfaction).

Conclusions and Recommendations Thus, based on the current evidence, the subcommittee suggest the following:

STARR is a health technology with a level of safety similar to other surgical treatments with respect to the incidence of complications. Regarding the operation time, length of stay, defecation and patient satisfaction, a trend of more favorable results was shown by STARR compared to other surgical treatments although no significant variation was found, indicating a level of effectiveness similar to other surgical treatments.

Based on the subcommittee report on the review of previous studies on the STARR, the conclusion of the 10th HTR committee of 2021 (Oct 15, 2021) was as follows:

STARR is a health technology with similar levels of safety and effectiveness to other surgical treatments. Nevertheless, the effects of STARR could not be ascertained as superior to the various alternative treatments. Thus, STARR was determined as an alternative treatment for suitable patients, and the HTR committee concludes that STARR is “conditionally recommended” for the treatment of ODS patients.
